Sjuksköterskors erfarenheter av att vårda patienter som önskar dödshjälp

Bakgrund: Dödshjälp har under många år varit en omdiskuterad debatt. Huruvida dödshjälp är lagligt eller inte skiljer sig åt mellan olika länder. I Sverige är dödshjälp ännu inte legaliserat. När en människa har en obotlig sjukdom eller vid vård i livets slutskede kan vissa människor uttrycka en önskan om att få hjälp att avsluta sitt eget liv, något som kan innebära att sjuksköterskan då behöver ta itu med dödshjälpsförfrågningar oavsett om dödshjälp är legaliserat eller inte samt finnas där för patienten. Syfte: Syftet med litteraturstudien var att beskriva sjuksköterskors erfarenheter av att vårda patienter som önskar dödshjälp. Metod: En beskrivande litteraturstudie som grundades på 12 vetenskapliga kvalitativa artiklar. Sökning av artiklar gjordes i databasen CINAHL. Därefter bearbetades och analyserades artiklarna enligt Aveyard tematiska analysmetod. Huvudresultat: Resultatet visade att sjuksköterskors erfarenheter var att dödshjälp utgjorde en komplex, känslomässig och intensiv process. Lidande och att bibehålla sin autonomi var de främsta orsakerna till patienters önskan om dödshjälp. Olika faktorer var avgörande för om sjuksköterskan valde att delta eller inte delta i dödshjälp.  Resultatet presenterades i tre huvudteman: dödshjälp är en komplex process, huvudorsaker till varför patienter önskar dödshjälp och sjuksköterskors erfarenheter av faktorer som påverkar ställningstagandet till dödshjälp.  Slutsats: Studier visar att sjuksköterskor kommer möta patienter som önskar dödshjälp oavsett var sjuksköterskorna är verksamma. Dödshjälp är förknippat med starka känslor och är en komplex process som utmanar sjuksköterskorna professionellt och personligen. Olika faktorer påverkar hur sjuksköterskorna förhåller sig till dödshjälpsprocessen och patienternas förfrågningar om dödshjälp. Litteraturstudien syftar till att sprida kunskap och förståelse om dödshjälp utifrån sjuksköterskors tidigare erfarenheter. / Background: Euthanasia has been a controversial debate for many years. Whether or not euthanasia is legal differs from country to country. In Sweden euthanasia is not legalized yet. When a person has an incurable disease or in end-of-life care, some people may express a desire to be helped to end their own life, which may mean that the nurse then needs to deal with euthanasia requests regardless of whether euthanasia is legalized or not and to be there for the patient. Aim: The aim of the literature study was to describe nurses' experiences of caring for patients who wish to be euthanized.  Method: A descriptive literature study based on 12 scientific qualitative articles. Articles were searched in the CINAHL database. The articles were then processed and analyzed according to Aveyard's thematic analysis method. Main Results: The results showed that nurses' experiences were that euthanasia was a complex, emotional and intense process. Suffering and maintaining one's autonomy were the main reasons for patients' desire for euthanasia. Different factors were decisive for whether the nurse chose to participate or not participate in euthanasia. The results were presented in three main themes: euthanasia is a complex process, main reasons why patients wish euthanasia and nurses' experiences of factors that influence attitudes towards euthanasia. Conclusion: Several studies show that nurses will encounter patients who wish to be euthanized regardless of where the nurses work. Euthanasia is associated with strong emotions and is a complex process that challenges nurses professionally and personally. Different factors influence how nurses relate to the euthanasia process and patient’s request for euthanasia. The literature study aims to spread knowledge and understanding about euthanasia based on nurses' previous experiences.

A Bilevel Approach to Resource Allocation for Utility-Based Request-Response Systems

We present a novel bilevel programming formulation that aims to solve a resource allocation problem for request-response systems. Our formulation is motivated by potential inefficiencies in the allocation of computational resources to incoming user requests in such systems. In our experience, systems often operate with a surplus of resources despite potentially incurring unjustifiable cost. Our work attempts to optimize the tradeoff between the financial cost of resources and the opportunity cost of unfulfilled user demand. Our bilevel formulation consists of an \textit{upper} problem which has a constraint value appearing in the \textit{lower} problem. We derive efficient methods for finding global solutions to the upper problem in two settings; first with logarithmic utility functions, and then with a particular type of sigmoidal utility function. A solution to the model we describe (1) determines the optimal number of total resources to allocate and (2) determines the optimal distribution of such resources across the set of user requests.

Dynamic Cooperative Communications in Wireless Ad-Hoc Networks

This dissertation focuses on an efficient cooperative communication method for wireless ad hoc networks. Typically, performance enhancement via cooperative communications can be achieved at the cost of other system resources such as additional bandwidth, transmit power, or more complex synchronization methods between cooperating signals. However, the proposed ooperative transmission scheme in this research utilizes system resources more efficiently by reducing the redundant and wasteful cooperating signals typically required, while maintaining the desired performance improvement. There are four main results in this dissertation. First, an efficient cooperative retransmission scheme is introduced to increase bandwidth efficiency by reducing wasteful cooperating signals. The proposed cooperative transmission method does not require any additional information for cooperation. Furthermore, we ensure good quality for the cooperating signals through a simple yet effective selection procedure. Multiple cooperating nodes can be involved in the cooperation without prior planning via distributed beamforming. The proposed cooperative retransmission scheme outperforms traditional retransmission by the source as well as other cooperative methods in terms of delay and packet error rate (PER). Secondly, the outage probabilities of the cooperative retransmission scheme are analyzed for both the perfect synchronization case and when offset estimation is performed for distributed beamforming. The performance with offset estimation is close to the perfect synchronization case, especially for short data packets. A low-rate feedback channel is introduced to adjust the phase shift due to channel variation and the residual frequency offset. It is shown that substantial gain can be achieved with a low-rate feedback channel, even for long data packets. Third, the throughput efficiency and average packet delay of the proposed cooperative retransmission scheme are analyzed using a two-state Markov model for both a simple automatic repeat request (ARQ) and a hybrid ARQ technique. The benefits of the cooperative ARQ approach are also verified in a multihop network with random configurations when there are concurrent packet transmissions. The average transmit power for the cooperating signals is also investigated in the proposed cooperative transmission scheme with various power control approaches. Finally, cooperative multiple input multiple output (MIMO) systems are examined, mainly focusing on power allocation methods to increase overall channel capacity. An efficient and simple power allocation method at the cooperating node is proposed which can be used for an arbitrary number of antennas without any additional information. / Ph. D.

Adaptive Protocols to Improve TCP/IP Performance in an LMDS Network using a Broadband Channel Sounder

Virginia Tech researchers have developed a broadband channel sounder that can measure channel quality while a wireless network is in operation. Channel measurements from the broadband sounder hold the promise of improving TCP/IP performance by trigging configuration changes in an adaptive data link layer protocol. We present an adaptive data link layer protocol that can use different levels of forward error correction (FEC) codes and link layer automatic retransmission request (ARQ) to improve network and transport layer performance. Using a simulation model developed in OPNET, we determine the effects of different data link layer protocol configurations on TCP/IP throughput and end-to-end delay using a Rayleigh fading channel model. Switching to higher levels of FEC encoding improves TCP/IP throughput for high bit error rates, but increases end-to-end delay of TCP/IP segments. Overall TCP/IP connections with link layer ARQ showed approximately 150 Kbps greater throughput than without ARQ, but lead to the highest end-to-end delay for high bit error rate channels. Based on the simulation results, we propose algorithms to maximize TCP/IP throughput and minimize end-to-end delay using the current bit error rate of the channel. We propose a metric, carrier-to-interference ratio (CIR) that is calculated from data retrieved from the broadband channel sounder. We propose algorithms using the carrier-to-interference ratio to control TCP/IP throughput and end-to-end delay. The thesis also describes a monitor program to use in the broadband wireless system. The monitor program displays data collected from the broadband sounder and controls the settings for the data link layer protocol and broadband sounder while the network is in operation. / Master of Science

A Study on Fingerprinting of Locally Assigned MAC-Addresses

The number of WiFi Devices is increasing every day, most people traveling hasa device with a WiFi network card enabled. This is something EffectSoft AB in Halmstad utilizes in their service Flow, to track and count devices. The accuracy of counted devices was however not accurate enough for a commercial use and this is where this candidate Thesis will continue research on how to improve the accuracy. It introduces the fundamental problem on how one cannot directly count transmitted MAC-Addresses to count present devices, since the manufacturers implement features against this such as MAC-Address randomization. It covers how manufacturers are not consistent in their implementation of the IEEE 802.11 standard, how this can be utilized to estimate how many devices are present in the networkwith three different approaches. It also concludes that Control Frame Attacks is not a viable approach any longer to count devices and the best method for counting devices are a combination of Passive Probe Request Analysis techniques. / Mängden enheter som kommunicerar över WiFi ökar dagligen och idag bär de flesta människor en enhet med ett aktiverat WiFi-nätverkskort. Detta använder EffectSoft AB, ett företag i Halmstad till sin teknik Flow för att räkna mobila enheter. Noggrannheten för beräkningen är dock inte tillräckligt bra för att produkten ska kunna vara applicerbar på marknaden och därav handlar denna kandidatuppsatsen om beräkning av mobila enheter. Denna rapport presenterar de problem som man stöter på vid beräkning av mobila enheter som till exempel randomisering av MAC-Adresser. Den täcker även hur tillverkare inte är konsekventa i sin implementation av IEEE 802.11 standarden och hur detta kan utnyttjas genom tre metoderför beräkning av antal mobila enheter. Det fastställs att Control Frame Attack inte längre är en möjlig metod för syftet samt att den bästa metoden för beräkning avantalet mobila enheter är en kombination av olika passiva Probe Request analyser.

TENA in a Telemetry Network System

This paper discusses a demonstration prototype that is being used to investigate the use of TENA across a constrained test environment simulating iNET capabilities. Some of the key elements being evaluated are throughput, latency, memory utilization, memory footprint, and bandwidth. The results of these evaluations will be presented. Additionally, the paper briefly discusses modeling and metadata requirements for TENA and iNET.

Elle est victime d'une erreur judiciaire. Plusieurs causes peuvent être à l'origine d'une telle injustice mais malheureusement il est difficile de le prouver. Le système de justice pénale au pays n'est pas à l'abri des condamnations injustifiées et plusieurs innocentes victimes en ont payé le prix. Or, comment une victime d'erreur judiciaire peut-elle prouver son innocence et ainsi recouvrer la liberté à laquelle elle a droit? Certes, l'aveu du coupable ou la découverte d'une nouvelle preuve peuvent servir de moyens de reconnaissance factuelle d'une condamnation injustifiée. Toutefois, certains obstacles procéduraux comme les délais d'appel, les règles d'admissibilité de la preuve nouvelle ou encore le mécanisme de correction des erreurs judiciaires prévu aux articles 696.1 et suivants du Code criminel peuvent compliquer le scénario. Avant le 4 juin 2002, cette dernière procédure était visée par l'article 690 du Code. Toutefois, suite à de nombreuses critiques ainsi qu'à une consultation publique orchestrée par le gouvernement canadien, des modifications législatives ont été apportées à cette procédure. Mais celles-ci sont-elles suffisantes? Nous pensons que certaines des modifications sont grandement appréciables. Cependant, la structure d'évaluation des dossiers et de recommandation des dossiers au ministre de la Justice mériterait la mise en place d'un comité formé d'avocats indépendants pour conseiller ce dernier. Cela est nécessaire afin de réduire à néant tout conflit d'intérêts potentiel de la part du Ministre et d'assurer la transparence du processus en cause. / A person is unjustly condemned to imprisonment. He or she is victim of a wrongful conviction. Many causes can lead to such an injustice but unfortunately, it is hard to prove. The country's criminal justice system makes no exception to wrongful verdicts of guilt and many innocent people have paid the priee. Thus, how can a person victim of a wrongful conviction prove his innocence and recover his rightful freedom? Of course, the confession of the true guilty person, the discovery of new evidence or a special inquiry commission are all ways of recognizing factually an unjust guilty verdict. However, certain procedural obstacles such as appeal delays, rules of admissibility of new evidence or even the procedure for correcting wrongful convictions provided under article 696.1 and following of the Criminal code, can complicate the scenario. Before June 4, 2002, the latter procedure was provided under article 690 of the Code. However, following great criticism as well as a public consultation orchestrated by the Canadian government, legislative modifications were brought to that procedure. Are the modifications made to the procedure sufficient? We believe that some of them are greatly appreciable. However, the evaluation and case recommendation structure to the Minister deserves the creation of a committee constituted by attorneys fully independent of the Minister able to advise him or her. This is necessary in order to fully reduce any potential conflicts of interest by the Minister and would ensure transparence of the process in question.
