Kvinnors upplevelser av sexuell hälsa i samband med bröstcancer : en litteraturöversikt / Women's experiences of sexual health in connection with breast cancer : a literature review

Bakgrund Idag överlever ungefär 90 procent av de kvinnor som drabbats av bröstcancer 10 år efter diagnos. Kvinnor som drabbas av bröstcancer påverkas negativt både psykiskt och socialt av att den fysiska kroppen förändrats i samband med behandling. Studier har visat på att kvinnor undviker att diskutera sin sexuella hälsa med vårdpersonal på grund av osäkerhet om hur det ska tas emot och önskar att vårdpersonal tar upp ämnet. Då sexualitet och sexuell hälsa är ett tabubelagt ämne och anses vara en privat angelägenhet, gör att problem som hade kunnat förebyggas förblir olösta till följd av bristande kommunikation. Syfte Syftet var att belysa kvinnors upplevelser av sexuell hälsa i samband med bröstcancer. Metod En icke-systematisk litteraturöversikt genomfördes med metoden för en integrerad dataanalys. Resultatet baserades på 15 vetenskapliga originalartiklar som togs fram genom databassökningar i PubMed och CINAHL. Resultat Många kvinnor upplever sexuell dysfunktion i samband med bröstcancer. De sexuella problem kvinnorna rapporterat om har handlat om bland annat minskad vaginal lubrikation, smärta i samband med samlag och penetration samt minskad sexuell lust och upphetsning. För många kvinnor bidrog de fysiska förändringarna i samband med bröstcancerbehandlingen till att deras kroppsuppfattning, identitet och självkänsla påverkades negativt. Slutsats En stor andel av kvinnor med bröstcancer upplever att den sexuella hälsan försämras. Det har framkommit att en del kvinnor haft enklare att hantera de kroppsliga förändringarna i samband med behandling för bröstcancer genom att de utvecklat särskilda copingstrategier. I tidigare studier har det framkommit att vårdpersonal inte berör patienters sexuell hälsa på grund av bristande kunskaper inom ämnet. Genom att vårdpersonal undviker att lyfta kvinnors sexuella hälsa i samband med bröstcancerbehandling orsakar det ett onödigt vårdlidande för patienten. / Background Today, close to 90 percent of women survive 10 years after a breast diagnosis. Women who suffer from breast cancer are negatively affected both mentally and socially by the physical changes in connection with breast cancer. Studies have shown that women avoid discussing their sexual health and want healthcare professionals to address the topic. The fact that sexuality and sexual health are a taboo subject and are considered a private matter means that problems that could have been prevented remain unresolved as a result of poor communication. Aim The purpose was to shed light on women's experiences of sexual health during breast cancer. Method A non-systematic literature review was performed using the method for an integrated data analysis. The results were based on 15 original scientific articles produced through database searches in PubMed and CINAHL. Results Many women experience sexual dysfunction in connection with breast cancer. The sexual problems the women have reported on have included reduced vaginal lubrication, pain in connection with intercourse and penetration, and reduced sexual desire and arousal. For many women, the physical changes associated with breast cancer treatment contributed to a negative impact on their body image, identity, and self-esteem. Conclusions A large proportion of women with breast cancer experience that their sexual health deteriorates.It has emerged that some women have found it easier to cope with the physical changesassociated with treatment for breast cancer by developing special coping strategies. Previousstudies have shown that healthcare professionals do not address patients' sexual health due tolack of knowledge in the subject. Because healthcare professionals avoid promoting women'ssexual health in connection with breast cancer, it causes unnecessary suffering for the patient.

Skolsköterskors arbete med att upptäcka och identifiera traumatiska barndomsupplevelser : en enkätstudie / School nurses’ work in detecting and identifying traumatic childhood experiences : a survey study

Bakgrund: Elever upplever traumatiska barndomsupplevelser (TBU) som påverkar förmågan att hantera livet som barn och vuxen. Det är angeläget att upptäcka och identifiera elever med TBU eftersom de har rätt till skydd, rehabilitering och social återanpassning. Uppgiften är central i skolsköterskans hälsofrämjande roll. Syfte: Undersöka hur skolsköterskor arbetar med att upptäcka och identifiera traumatiska barndomsupplevelser hos elever. Metod: En retrospektiv enkätstudie användes och 100 skolsköterskor lämnade sina svar mellan 20 februari och 21 mars. Nästan alla hade specialistsjuksköterskeutbildning. Resultat: Frågor om TBU ställdes av 66 % och 34 % gjorde det inte. Majoriteten frågade muntligt. Frågan om TBU fanns i skolans hälsoenkät hos 38 % och en använde validerade formulär för att upptäcka och identifiera TBU. Av samtliga respondenter ville 78 % lära sig använda Barnafrids validerade formulär för att upptäcka och identifiera TBU. Konklusion: Många frågar om TBU men en tredjedel gör det inte. Mindre än hälften hade frågor i skolans hälsoenkät. Få kände till att validerade formulär kunde hjälpa till i arbetet med TBU och användes av en. Skolsköterskor, vårdgivare, lärosäten och forskare kan tillsammans utveckla systematisk screening av TBU. Det främjar elevers hälsa, välbefinnande och möjlighet att nå utbildningens mål. / Background: Students experience adverse childhood experiences (ACE) that affect the ability to cope with life as a child and as an adult. It's important to detect and identify ACE because students have the right to protection, rehabilitation and social reintegration. This task is central for school nurse’s health promoting role.  Aim: To investigate how school nurses’ work to detect and identify ACE in students.  Method: A retrospective survey study was used and 100 school nurses provided their responses between 20 February and 21 March. Almost all had specialist nursing training.  Findings: Questions about ACE were asked by 66% and 34% did not. A majority asked verbally. ACE questions was present in 38 % of the school health questionnaire. One used validated form and of all 78% wanted to learn how to use validated form to detect and identify ACE.  Conclusion: School nurses ask about ACE but a third do not. Less than half had questions in the school health questionnaire and few knew that validated forms could help in the work with ACE. School nurses, healthcare providers, universities and researchers can together develop systematic screening for ACE. It promotes students' health, wellbeing and the opportunity to reach the goals of the education.

Respond bias has always been a risk when it comes to interpreting personality data. For this reason two social desirability measures were created to combat this problem during research and workplace application. The first of these measures is the Marlowe-Crowne Social Desirability Scale created to measure a need for approval. The second of these measures is Balanced Inventory of Desirable Responding, which stems from a theory that describes social desirability as both a deception towards others and towards the self. For either of these measures to be usable, however, they need to be reliable and valid. This study then is intended to validate these two instruments in a diverse South African population sample and to look at the reliability of the items in these instruments and their factor structure. The objective of this study was to investigate both of these measures and to determine their psychometric properties and how they compare to the theory in literature. A convenient and purposive sample of N = 359 individuals from across South Africa was contacted via electronic means and asked to partake in this study. A questionnaire survey was forwarded to them with the intention of measuring social desirability. This included both the Marlowe-Crowne Social Desirability Scale (MCSDS) and Balanced Inventory of Desirable Responding (BIDR) measure. In addition a demographical questionnaire was included (gender, race, language group and age). The statistical analysis was done via the SPSS program during data examination: descriptive statistics, exploratory factor analysis (with Maximum Likelihood as extraction method), Cronbach’s alpha coefficients, and product-moment correlations were conducted. The results of this analysis indicated that although these measures are widely accepted and used internationally, the full version of both the measures is not valid and reliable within this South African sample. Although not all items from the scales could be validated, there were items that indicated very acceptable psychometric properties. Various recommendations were made for the context of using these measures to ascertain an individual’s response bias and for future research. A person attempting to use these measures should only focus on using the reliable items from this study. These items could be applied in developing a shortened version of these measures. Demi-vérités et vrais mensonges : une analyse des processus liés à la dissimulation dans les questionnaires informatisés

De nombreux chercheurs et cliniciens sont sceptiques quant à la validité des questionnaires autoadministrés, lorsqu’utilisés auprès d’une population carcérale (Gendreau, Irvine et Knight, 1973), surtout si celle-ci est composée de délinquants sexuels (Marshall et Hall, 1995). La sensibilité des sujets investigués jumelés à la transparence des questions expose l’évaluateur à la possibilité que le participant dissimule et modifie ses réponses (Tierney et McCabe, 2001). L’objectif de ce projet est de comprendre les processus impliqués dans la performance des participants à une évaluation autoadministrée. Les données de 282 délinquants sexuels ayant complétés le Multidimensional Inventory of Development, Sex, and Agression (MIDSA) ont été analysées afin de mieux comprendre l’interaction entre les échelles de désirabilité sociale, les temps de latence et les coefficients d’ajustement du modèle de Rasch. La convergence des analyses de temps de latence et des échelles de désirabilité sociale semble indiquer que certains participants dissimuleraient consciemment leurs réponses. Notamment, les participants détectés par les échelles de désirabilité sociales sembleraient répondre de manière de plus lente aux échelles d’évaluations, et certains d’entre eux offriraient des patrons de réponses incohérents à la prescription du modèle de Rasch. Les hypothèses permettant d’expliquer les potentiels mécanismes liés à la dissimulation seront discutées. / Many researchers and clinicians are skeptical towards the validity of self-reported assessment used within a forensic population (Gendreau, Irvine, & Knight, 1973), especially when used within a sexual offender population (Marshall & Hall, 1995). The sensitivity of the topics addressed as well as the transparency of the questions expose the evaluator to possible dissimulations or distortions in the subject responses (Tierney & McCabe, 2001). Responses of 282 sexual offenders who completed the Multidimensional Inventory of Development, Sex, and Aggression (MIDSA) were analyzed in order to understand the interaction between response time, social desirability scales and Rasch person-fit scores in order to account for response distortion. The convergence of latency times and social desirability scales seems to indicate that certain participants consciously manipulate their responses. Notably, participants detected by social desirability scales seem to respond more slowly to particular items. Other participants may answer in ways that are incoherent with the predicted responses according to the Rasch model. Hypotheses as to potential mechanisms underlying this process will be discussed.

Pratiques éducatives des parents d’enfants ayant un TDA/H : le point de vue des parents et d’un observateur

Cette thèse s’intéresse à la mesure des pratiques éducatives et à leur capacité à rendre compte de ces pratiques. Notamment, la sensibilité des instruments à rendre compte des changements dans les pratiques éducatives suite à un programme d’entraînement aux habiletés parentales est abordée tout en tenant compte des facteurs déterminants de ces pratiques. Le corps de cette thèse est composé de deux articles. L’objectif de la première étude consiste à comparer deux instruments de mesure fréquemment utilisés pour mesurer les pratiques éducatives. Les deux instruments, comparés sur la base de données empiriques, sont le Parenting Practices Interview (PPI), un instrument développé afin de mesurer les changements suite au programme d’entraînement aux habiletés parentales Ces Années Incroyables, et l’Alabama Parenting Questionnaire (APQ). Le premier s’adresse aux parents d’enfants de 3 à 8 ans alors que le second cible les parents d’enfants d’âge scolaire. Cent vingt parents ont complété le PPI et l’APQ. Des analyses corrélationnelles, une analyse en composante principale et des corrélations canoniques ont été utilisées afin de comparer les différentes pratiques mesurées par ces deux instruments. Les résultats indiquent que ces deux instruments mesurent sensiblement les mêmes pratiques parentales. Quelques sous-échelles du PPI ne sont pas mesurées par l’APQ et représentent des pratiques éducatives additionnelles. Puisque ces deux instruments mesurent sensiblement les mêmes pratiques, le choix du questionnaire devrait être fait en fonction de l’objectif de l’étude (p.ex. évaluer l’efficacité du programme Ces Années Incroyables ou non), de la facilité d’utilisation de l’instrument et de l’âge des enfants. Le deuxième article a pour objectif d’évaluer les changements dans les pratiques éducatives suite à la participation des parents à un programme d’entraînement aux habiletés parentales (PEHP) à l’aide d’une grille d’observation spécifiquement développée dans le cadre de ce projet. De plus, cette étude vise à identifier les facteurs modérateurs du changement dans les pratiques éducatives telles certaines caractéristiques de l’enfant et des parents. Soixante-dix-sept familles ayant un enfant de 6 à 9 ans avec un trouble du déficit d’attention/hyperactivité (TDA/H) ont été retenues pour cette étude. Les familles ont participé soit au PEHP, soit elles ont reçu du soutien téléphonique (ST), ou elles ont continué à recevoir les services dans la communauté. Les pratiques éducatives ont été évaluées avant et après l’intervention à l’aide d’une mesure observationnelle. Les résultats indiquent que seuls les parents ayant participé au PEHP utilisent davantage de pratiques positives et ont diminué les pratiques sévères et négatives suite à l’intervention. De plus, le sous-type de TDA/H, la présence ou non de comorbidité chez l’enfant et le cumul de risque à l’intérieur d’une famille, calculé à partir de caractéristiques des parents, ne modèrent pas le changement suite à l’intervention. Cette étude démontre donc la capacité de la mesure observationnelle à détecter des changements suite à l’intervention de même que l’efficacité du PEHP auprès des familles d’enfants d’âge scolaire ayant un TDA/H. / The purpose of this thesis was to examine various measures of parenting practices and their ability to account for these practices. The instrument’s sensitivity to changes following a parent training program is also examined while taking into account potential moderators of treatment response. Two studies constitute the body of this thesis. The first study compared two parenting practices questionnaire on the basis of empirical data. The Parenting Practices Interview (PPI) is a self-report questionnaire for 3 to 8-year old children used to evaluate changes in parenting practices following a parent training program (PTP). The Alabama Parenting Questionnaire (APQ) is a self-report questionnaire for school-age children that has been used for multiple research purposes. One hundred and twenty parents completed both the PPI and APQ. Correlation analyses, principal component analysis and canonical correlations were performed, with results indicating that the PPI and APQ measure similar parenting practices. Only a few scales from the PPI were not measured by the APQ and represented additional content. Because both measures cover the same parenting practices aspects equally well, choice of questionnaire should be based on purpose of study, practicality and age of children. The second study examined changes in parenting practices following a parent training program for families of school-age children diagnosed with attention-deficit/hyperactivity disorder (ADHD). Changes were examined with an observational measure specifically created for this project. Seventy-seven families were assigned to either the Incredible Years parent training program (PTP), a support phone call group (SPC), or to a community services (C) control group. Parenting practices were assessed before and after the intervention through direct observations. Results showed that parents in the PTP group reduced the percentage of harsh/negative parenting practices and increased the percentage of positive parenting practices used following the intervention while parents from the other two groups did not change. In addition, ADHD subtypes, comorbidity and cumulative risks based on parent’s characteristics did not moderate treatment response. Findings from this study show that the new observational measure is sensitive enough to detect changes following a PTP and also indicate that a parent training program is a valuable intervention for families of school-age children diagnosed with ADHD.
