Péče o seniory z pohledu sociální práce a etiky / Care about Seniors in Light of Social Work and Ethics

The first chapter examines the relationship of the society to the seniors, perception surrounding seniors, negative and positive views of the seniors in the history, then ageism. The second chapter refers to the dependence and vulnerability as the dimension of human existence. For that reason, here is paid attention to the care of seniors, with a more detailed focus on the seniors with dementia and the senior's dying. There aren't also omitted the care of social workers and therefore is a notice of the burn-out syndrome. In the last chapter is given positive influence of autonomy and dignified approach on the success life of the seniors. Furthermore, the importance of ethical standards contained in codes of ethics and ethical theories for the correct decision and behaviour of a social worker.

The thesis deals with palliative and hospice care. The thesis is divided into two parts - theoretical and practical. In the theoretical part there are provided with basic information about palliative care, it describes aspects of palliative care in terms of spiritual, social, psychological and physical. There are also recorded information on facilities that provide palliative care, their characteristic, mission and goals. The thesis is also reported on problems of dying, suffering and death. The work also records a pastoral and social care about dying seniors, a description of a multidisciplinary team of hospice care. The practikal part of the thesis is reported on a detection rate of awareness of palliative and hospice care at the clients of caritas Týn nad Vltavou. Next the research found out the respondents opinion of dignified dying in a domestic care

Věková struktura obyvatelstva Jihočeského kraje a koncepce péče o staré občany / The population age structure of South Bohemia and the concept of social care for older people

The present work deals with the population age structure of South Bohemia in the years 1991 to 2007, with main focus on people over 65 years. In the introduction deals with the objectives and assumptions of work, literature searching and processing methodology. Other chapters deal with the analysis of population age structure and analysis of current capacity in social care for older people, specifically in homes for the elderly. It will identify areas with higher rates of the elderly population. The central part of the work is its own projection of population development of the South Bohemian Region and its districts until 2057, focusing on older people. The last chapter is devoted to forecast capacity needs of care facilities for older people in the medium term. Work is completed on the cartographic output, tables and graphs.

Tendo essa relação como pressuposto e as características peculiares do processo de desenvolvimento capitalista brasileiro como referência, analisamos, neste trabalho, as políticas de educação e de saúde como determinadas, em última instância, pelas condições econômico-sociais e político-ideológicas vigentes. Nesse movimento, buscamos compreender as categorias do desenvolvimentismo e da globalização e as relações estabelecidas com a educação e a saúde, ultrapassando a análise interna das instituições públicas, visto que o estudo crítico das políticas de educação e saúde, através da apreensão da sua relação com a base econômica e a superestrutura ideológica, só é possível pela análise do modo de produção capitalista e seus modos de desenvolvimento. Nesta perspectiva, partimos da compreensão de que as políticas de educação e saúde básicas integram o conjunto das reformas econômicas e, portanto, estão circunscritas ao processo de reforma e modernização do Estado e seus objetivos políticos e econômicos. Sustentamos que, no contexto da ideologia da globalização, a educação e a saúde básicas são consideradas variáveis fundamentais para administrar a pobreza, contribuindo para a estabilidade política e social e, ao mesmo tempo, criando as condições favoráveis mínimas para o implemento das políticas de ajuste econômico, pois no processo de implementação dessas políticas existe um consenso entre o BID e o BIRD de que o Estado deve oferecer os serviços essenciais básicos, de educação e de saúde, para dar resposta às demandas imediatas, visando à proteção dos grupos vulneráveis. Verificamos que as reformas de educação e saúde estão inscritas no contexto mais amplo do processo de reestruturação econômica do país e, desse modo, os objetivos definidos pelo BIRD e pelo BID, nos empréstimos de ajuste estrutural e setorial e as estratégias definidas por essas instituições, sustentam o processo de reforma e modernização do Estado, a partir da definição e de implementação de políticas nacionais submetidas às condicionalidades expressas nos seus contratos de empréstimos. A articulação e a convergência das orientações e das propostas nos projetos financiados pelo BIRD e pelo BID para as reformas da Educação Básica (Ensino Fundamental) e da saúde modificaram as formas de financiamento e administração dessas áreas, mediante as estratégias de descentralização (através da privatização, da participação das ONGs na prestação desses serviços, da realização de parcerias com a comunidade e da municipalização do ensino e da saúde), baseadas na construção de critérios gerenciais para promover a concorrência, a qualidade, a produtividade (eficiência interna e externa, eficácia) e a racionalidade econômica (relação custo-benefício) no processo de distribuição e utilização dos recursos públicos. Procuramos demonstrar, dessa forma, o como as políticas de educação e de saúde básicas incorporaram e consolidaram esses conceitos, viabilizando os objetivos econômicos das políticas de ajuste estrutural e setorial e, ao mesmo tempo, contribuindo para o processo de reforma e modernização do Estado brasileiro e de suas instituições públicas de educação e saúde / Abstract: Education and health care policies are products from complex relationships between the economic-social and political-ideological dimensions from a certain historical context of capitalism development. Having this relationship as presupposed and the particular characteristics of the Brazilian capitalist development process as a reference, we analyzed, in this paper, the educational and health care policies as determined, in the last instance, by the current economic-social and political-ideological conditions. In this movement, we tried to understand the globalization and development systems categories and the relationships established with education and health care, going beyond the inner analysis of public institutions, because the critical study of education and health care policies, through the understanding of its relationship with the economical base and the ideological superstructure, it's only possible by the analysis of the way of capitalist production and its development ways. In this perspective, we started from the comprehension that the basic educational and health care policies integrate the economical reform conjoint and, therefore, are circumscribed to the reform and modernization process of the State and its political and economical objectives. We affirm that, in the globalization ideology context, basic education and heath care are considered essential variables to administrate the poverty, contributing to the social and political stability and , at the same time, creating minimal favorable conditions for the implement of economical adjustment policies, because the implementation process of these policies there is an agreement between BID and BIRD in which the State must offer the basic supply, in education and health care, to provide answers to immediate demands, seeking the protection of vulnerable groups. We checked that the educational and health care reforms are enrolled in the more ample context of the economical reorganization of the country and, this way, the objectives determined by BIRD and BID, in the loans of sectoral and structural adjustment and the strategies determined by these institutions, support the modernization and reform process of the State, from the definition and implementation of national policies submitted to conditions expressed in their loan contracts. The articulation and convergence of orientations and the proposals in the projects financed by BIRD and BID for the Basic Educational reforms (Elementary school) and health care modified the administration and financing forms of these areas, by means of decentralization strategies (through privatization, participation of NGOs in the performing of theses services, the realization of partnerships with the community and the education and health care municipalization), based on the construction of managing criteria to promote the competition, quality, productivity (internal and external efficiency, efficacy) and the economical rationality (cost-benefit relationship) in the distribution and usage process of public resource. We tried to demonstrate, this way, how the basic education and health care policies incorporated and consolidated these concepts, enabling the economical objectives of sectoral and structural adjustment policies and, at the same time, contributing to the reform and modernization process of the Brazilian State and the education and health care public institutions / Doutorado / Historia, Filosofia e Educação / Doutor em Educação

The form of psycho-social care involves changes in the concept of care, with a focus on the person who suffers and his or her subjectivity. Services are spaces where care is provided and for the restoration of people and their reinsertion in their relational and social world. In this context, this study seeks to analyze the potential for production of subjectivity in the dynamic of work at the Psycho-social Care Center in the municipality of Alegrete-RS. This is a case study, with a qualitative research approach. As theoretical references, it uses concepts from Guattari and those from Foucault concerning power relations and the micropolitics of the dynamic of work. The data used come from the study entitled Evaluation of Psycho-Social Care Centers in Southern Brazil II, which is a quantitative and qualitative study for the evaluation of services. This masters dissertation is a qualitative study based on field data from Alegrete, Rio Grande do Sul State. Data collection tools included semi-structured interviews with a pre-defined questionnaire conducted with 11 users and 21 workers and records in field diaries from 253 hours of field observations. The data were collected in July and August 2011, at a service of the Psycho-social care center of Alegrete. A thematic analysis was used for reading the data. Important movements were observed in the construction of practices and the working dynamics in the perspective of the psycho-social paradigm, as well as micropolitical factors that allow processes of subjective implication for users and workers based on the dynamic established in the operationalization of the work. These relations also present factors that restrict protagonism, autonomy and independence because of rigid, hierarchized and protective procedures. The presence of specialties and of power/knowledge prevent the establishment of horizontal relations in the context of service. The resistance produced by movements taken to confront prejudice toward mental illness and to create other options to promote a subjective repositioning for all the actors both users and workers - involved, indicates processes of production of singularized subjectivity. It is hoped that this study contributes to the development of processes of mobilization, innovation and differences in the psycho-social form of care, stimulating diversity, multiplicity and heterogeneity among the collective involved in this field. / A reforma psiquiátrica brasileira propõe mudanças paradigmáticas significativas no seu modo de relacionar-se com a loucura e operacionalizar o trabalho nos Centros de Atenção Psicossocial. O modo de atenção psicossocial envolve mudanças de concepção sobre o cuidado, com o foco na pessoa que sofre e sua subjetividade. Os serviços constituem-se em espaços de cuidado, resgate e reinserção das pessoas em seu mundo relacional e social. Neste contexto, o presente estudo procura analisar a potencialidade de produção de subjetividade, na dinâmica de trabalho no Centro de Atenção Psicossocial da cidade de Alegrete-RS. Caracteriza-se como um estudo de caso, com abordagem de pesquisa qualitativa, que utiliza como referencial teórico o paradigma do modo psicossocial, as concepções de subjetividade de Deleuze e Guattari, e os conceitos de Foucault sobre as relações de poder e a micropolítica da dinâmica de trabalho. Os dados utilizados fazem parte de um recorte da pesquisa intitulada Avaliação dos Centros de Atenção psicossocial da região sul do Brasil II, o qual se constitui de um estudo quantitativo e qualitativo de avaliação de serviços. Esta dissertação de mestrado é um estudo qualitativo a partir dos dados do campo de Alegrete Rio Grande do Sul. Como instrumentos de coleta de dados utilizou-se a entrevista semiestruturada realizada com 11 usuários e 21 trabalhadores, a observação de campo, com um roteiro pré-definido e registro em diário de campo num total de 253 horas. Os dados foram coletados em julho e agosto de 2011, em um serviço de Centro de Atenção Psicossocial do município de Alegrete. Utilizou-se a análise temática, para a leitura dos dados. Observou-se movimentos importantes de construção de práticas e dinâmicas de trabalho na perspectiva do paradigma psicossocial, bem como aspectos micropolíticos que possibilitam processos de implicação subjetiva para usuários e trabalhadores partindo da dinâmica estabelecida na operacionalização do trabalho. Estas relações apresentam ainda aspectos limitadores para o protagonismo, autonomia e independização, com a manutenção de procedimentos rígidos, hierarquizados e tutelares. A presença de especialismos e do saber-poder, colocam-se de forma a interromper os processos de horizontalização das relações no contexto do serviço. As resistências produzidas pelos diversos movimentos criados para o enfrentamento do preconceito em relação à loucura, na criação de outras possibilidades existenciais, de forma a promover um reposicionamento subjetivo para todos os atores envolvidos, usuários e trabalhadores, indicam processos de produção de subjetividade singularizada. Espera-se com este estudo contribuir para o desenvolvimento de processos de mobilização, inovação e diferenças no modo psicossocial de cuidar, estimulando a diversidade, multiplicidade e heterogeneidade nos coletivos implicados neste campo.

The master thesis investigates the care of orphans and abandoned children paying special attention to how the care was executed in Prague in the 1st half of the 20th century. First, the thesis describes the general development of care of these children and of legislation. It then focuses on Prague social care of children. The orphans and abandoned children could be provided either in foster families (this form of care was more frequent) or in orphanages. Prague children were often placed outside of Prague to the so called family colonies, where the foster families were concentrated and where it was easier to control the quality of care those children attained in foster families. The other option was care in orphanages. Those could be established by the city, by the church or by an independent charity association. For each option I chose one Prague orphanage as an example. I also tried to describe the everyday life in these institutions. The last chapter focuses on the period view on advantages and disadvantages of both forms of care. Att göra abstrakta begrepp och komplexa situationer konkreta : en avhandling om deltagarbaserad aktionsforskning i svensk vård och omsorg

This dissertation covers the subject of how abstract concepts and complex situations can be concretized through research together with practitioners. The dissertation is based on four empirical studies. The researcher role, the practitioner participation and the methods for data collection and analysis have varied. In study I the concept ‘Närsjukvård’ was explored to understand how practitioners, managers and politicians in hospitals, primary health care and municipalities interpreted the concept. The researcher acted as consultant who collected data by interviews and questionnaires. Practitioners’ participation was limited. ‘Närsjukvård’ was interpreted as accessibility to hospital beds, accessibility to primary health care, collaboration between care providers and continuity and developed home care. Study II aimed to explore how people experienced leg ulcer care. The researcher acted as a consultant who performed the interviews and  analysed the data. Although the informants experienced their encounters with the nurses as satisfying, the study illuminated low participation in the care and low practitioner involvement in issues about daily living with the leg ulcer. The findings were brought back to the informants and the practitioners. The project did not proceed towards development and change. In study III the aim was to explore the Swedish concept ‘trygghet’ by using stories from daily life. Four older women were interviewed and the Story Dialogue method was used together with assistant nurses and registered nurses who participated in data collection and analysis. Two themes emerged: Sense of Security and factors strengthening the Sense of Security. Together with the assistant nurses, areas for improvements were identified. Study IV aimed to explore the discharge planning situation in order to     generate ideas for development. Members from a discharge planning network participated in the whole research process. Conditions for a successful coordinated discharge planning situation were a system including: the participation of the patient, the competence of the staff and the support from the organisation. The group arranged a workshop about communication and interdisciplinary   collaboration. The findings resulted in a form with self-evaluation questions. In conclusion, this thesis illustrates that it is possible to clarify abstract concepts and complex situations together with practitioners. To do this successfully, sense making activities and to start from practitioners’ experiences and their own context are key factors. The studies illuminate that building trust, relationship and sense of participation are essential in health and social care in general and specifically in the participatory action research process.

Důsledky stárnutí populace na potřebu sociálních služeb pro seniory / Consequences of an aging population for the need for social care services for seniors

The thesis deals with the problem of aging of the population in the lower territorial unit in the municipality with extended competence in Příbram. The current situation of the population is dealt with, but the future population status is projected from 2015 to 2030. For their analysis are used demographic and economic indicators, results of own projection as well as a description of social care services provided in this area. The aging of the population is related to several key themes, mainly with the provision of adequate care for the elderly, and in the work it is also addressed in terms of financing services and the increasing burden of the economically active population. The demographic data on the local population is compared with the situation in the Czech Republic. The benefit of the work is primarily to determine the need for social care services for the elderly in the future, to identify possible shortcomings in their provision, capacity, quality and financing. The aim of the thesis is also to find possible solutions that are most effective and help to ensure safe and peaceful aging for all seniors in SO ORP Příbram. The result of the work is to map the current situation and to create a future estimation that will provide the necessary documents for better planning of social services for the elderly.

Důsledky stárnutí populace na potřeby služeb sociální péče / Consequences of ageing population for the needs of social care services

This thesis is focused on problems of consequences of ageing population and the needs of social care services in terms of bed capacity in retirements homes and nursing homes. The aim of the thesis is to set a projection of bed capacity in these facilities at the level of individual regions and the whole Czech Republic by 2051. The dissertation presents three methods of calculating the future bed capacity which shows that the bed capacity will need to be significantly increased in future. The results found show that there are regional differences in the demographic structure within the Czech Republic at regional level which leads to a different intensity of demand for bed capacity.
