Upplevelser av att leva med en traumatisk ryggmärgsskada : En litteraturöversikt / Experiences of living with a traumatic spinal cord injury : A literature review

Bakgrund: Ryggmärgens huvudsakliga funktion är att skicka och ta emot nervimpulser för rörelse och känsel. När en ryggmärg skadas innebär det förluster eller nedsättningar av kroppsliga funktioner och förmågor. Omfattningen av funktionsnedsättningen är beroende på vilken nivå i ryggmärgen som skadan sker. Vid en traumatisk ryggmärgsskada har omvårdnaden en stor betydelse för den fortsatta hälsan och sjuksköterskor har beskrivit att de har en mångsidig roll i arbetet. Att ha en funktionsnedsättning kan innebära ett behov av hjälp från andra personer, samt upplevelser av utanförskap och en exkludering från samhälleliga arenor. Syfte: Att belysa personers upplevelser av att leva med en traumatisk ryggmärgsskada. Metod: En litteraturöversikt baserad på tolv vetenskapliga artiklar; elva kvalitativa och en med mixad metod från databaserna CINAHL complete och Psycinfo. Vid sökning i databaserna användes ämnesord som Spinal Cord Injuries, Patients attitudes. Artiklarna har analyserats i enlighet med Fribergs metod. Resultat: Två huvudteman identifierades utifrån Roys adaptionsmodell, Adaptiv respons och ineffektiv respons. Varje huvudtema har tillhörande underteman. Under adaptiv respons återfanns, anhörigas roll i vardagslivet, hoppets betydelse, nya insikter och förändrade perspektiv och betydelsen av meningsfulla aktiviteter. Under ineffektiv respons återfanns upplevelser av att vara beroende, upplevelser av en förändrad identitet, viljan att avsluta sitt liv, upplevelser av ett förlorat hopp och livets ständiga kamp. Diskussion: I metoddiskussionen diskuteras metodens styrkor respektive svagheter. Under rubriken resultatdiskussion diskuteras litteraturöversiktens viktigaste resultat som var anhöriga, hopp, psykisk ohälsa och aktiviteter mot litteraturöversiktens bakgrund, ny litteratur samt mot Roys adaptionsmodell. / Background: The Spinal cord's main function is to send and receive nerve impulses for movement and sensation. Damage of the spinal cord means loss or impairment of bodily functions and abilities. The extent of the disability depends on the level of the spinal cord injury. At a traumatic spinal cord injury, care has a great importance for the continued health. Nurses have described that they have a versatile role in that work. Having a disability can mean a need for help from other people and the experiences of alienation and exclusion from social arenas. Aim: To illustrate people's experiences of living with a traumatic spinal cord injury. Method: A literature review based on twelve scientific articles; eleven qualitative and one with mixed method from the databases CINAHL Complete and PsycINFO. When searching in the databases, some of the terms that was used were Spinal Cord Injuries, Patients, Attitudes. The articles were analyzed according to Fribergs method. Results: Two main themes were identified from the Roy Adaptation Model, adaptive response and ineffective response. Each main theme has related sub-themes. Sub themes in adaptive response are, relatives role in everyday life, importance of hope, new insights and altered perspectives and the importance of meaningful activities. In ineffective response there are experiences of being dependent, experiences of an altered identity, the desire to end one’s life, experiences of a lost hope and life's constant struggle. Discussion: In the method discussion, strengths and weaknesses of the method are discussed. Under the heading discussion of results, the most important results of the literature overview that were relatives, hope, mental health and activities are discussed in comparison with the results overview background, new literature and Roy's adaptation model.

Identificação das práticas de auto-cuidado referentes ao funcionamento intestinal em pacientes com trauma raquimedular / Identification of self care practices related to bowel functioning in patients with spinal cord injury

O trauma raquimedular (TRM) é uma agressão na medula espinhal e pode acarretar severas alterações físicas e psíquicas ao paciente, refletindo na família e sociedade. Com a inexistência de terapêutica que reverta o quadro de TRM, é necessário investimento na reabilitação para melhorar a qualidade de vida destes pacientes. Através de um estudo exploratório-descritivo, com metodologia quantitativa foi investigada as práticas de autocuidado intestinais e complicações em 27 indivíduos com TRM. Todos os participantes eram do sexo masculino e predominância da faixa etária de 18 a 30 anos, solteiros, com até 1º grau incompleto, com renda familiar entre 2 e 5 salários mínimos, tendo o ferimento por arma de fogo como principal etiologia da lesão. As práticas de autocuidado intestinal mais utilizadas foram a massagem abdominal (32%), seguida da dieta rica em fibras (24%), uso de laxante (15%), toque dígito anal (15%), extração manual das fezes (7%) e outros (7%). Quanto às complicações intestinais, houve predomínio de constipação/impactação (50%), seguido de incontinência fecal (23%), sangramento anal (21%) e outras (6%). Em relação às internações hospitalares, consideradas oportunidades para o enfermeiro orientar sobre as alterações decorrentes do TRM, 59,3% dos pacientes apresentaram mais de uma internação, porém a transmissão de informação sobre alterações intestinais ocorreu em somente 22% dos casos. Diante do número reduzido de centros especializados em reabilitação, julgamos de grande importância a presença de enfermeiros com formação específica em reabilitação para atuarem na assistência, na educação dos pacientes e no ensino nas instituições formadoras de profissionais. / Spinal cord injury (SCI) is an aggression to the spinal cord and can bring physical and psychological alterations to patients lives, reflecting upon family and society. So far, there is no therapeutic to revert the injury of the spinal cord; thus, it is necessary to invest on rehabilitation to improve the patient´s quality of life. Practices of self care for bowel functioning and complications were studied in 27 individuals with SCI, through a descriptive-exploratory study with a quantitative methodology. All participants were men, mostly between 18 and 30 years of age, single, with low education level, family income between 1 to 5 minimal wages; and wounded by gun shots. Practices of self care mostly used were abdominal massage (32%), high fiber diet (24%), use of laxatives (15%), anal digital stimulation (15%), stool manual removal (7%) and others (7%). Related to complications, constipation/impactation was the most frequent (50%), followed by fecal incontinence (23%), bleeding (21%) and others (6%). Hospitalization is considered an opportunity for nurses to teach patients about SCI alterations and 59,3% of patients had more then one episode of hospitalization. However, only in 22% of the cases they received any information about alterations in bowel functioning. With so few rehabilitation centers in our region it is important to have nurses at terciary and general hospitals, educated in rehabilitation to deliver adequate nursing care and patient teaching. It is also necessary, in nursing educational institutions, to prepare profissionals with focus in rehabilitation.

"Ocorrência de úlcera de pressão em pacientes hospitalizados com lesão traumática da medula espinhal" / Occurrence of pressure ulcers in patients hospitalized with traumatic spinal cord injury.

A lesão traumática da medula espinhal (LTME) é um grande problema de saúde ocasionado principalmente por acidentes automobilísticos, quedas, mergulho e a violência, como ferimentos por arma de fogo (FAF). Pacientes com LTME tem um elevado risco para desenvolver úlcera de pressão (UP) devido à perda da mobilidade e motricidade abaixo do nível medular acometido. É importante a realização de estudos de ocorrência da UP para que medidas de intervenção possam ser implementadas. Assim, este estudo foi realizado de forma retrospectiva em um hospital universitário do interior do estado de São Paulo. Após a aprovação do comitê de regulamentação e normas éticas da instituição, os prontuários foram revisados e 47 desses atenderam aos critérios de inclusão: ter diagnóstico de LTME, ter idade igual ou superior a 18 anos na época do trauma; ter informações referentes a primeira hospitalização após o trauma. A maior parte dos pacientes eram do sexo masculino (97,5%), brancos (68,1%), com idade entre 21 e 30 anos (36,2%). A causa mais freqüente da LTME foi FAF (44,7%) seguido por colisão/capotamento de veículo (23,4%). Quanto ao nível neurológico da lesão, houve predomínio no nível torácico (44,7%) seguido pelo nível cervical. O tempo médio de hospitalização foi de 21,47 dias (DP= 28,50). Em relação à procedência do paciente, 46,8% eram de Ribeirão Preto. Houve ocorrência de UP em 20 (42,5%) dos pacientes, com a média de 2,3 UP por paciente. Os locais de maior freqüência foram na região sacral (36,95%) e calcâneos (17,4%). Apenas 25% dos prontuários tinham o registro da dimensão da UP, 80% apresentavam o registro do aspecto e em 52,1% não havia o registro do estágio de classificação. Houve uma maior freqüência de uso de medidas preventivas nos pacientes com UP. Durante o período de internação, 44,7% dos pacientes tiveram outras complicações clínicas, como pneumonia (25,5%). Dos pacientes com alta hospitalar, 34% tiveram outras internações nos primeiros 6 meses após a LTME, sendo o principal motivo a infecção do sítio cirúrgico, seguido da UP e infecção do trato urinário. Com este estudo, verificamos que há a necessidade de melhorar o registro referente aos dados do paciente para que intervenções possam ser propostas. Programas de educação permanente podem contribuir para modificar esta situação. Outros estudos de investigação sobre o tema são necessários. / Traumatic spinal cord injuries (TSCI) are a significant health issue which is mainly caused by automobil accidents, falls, diving, and violence, such as wounds by gun shot (GS). Patients with TSCI have an increased risk of developing pressure ulcers (PU) due to the loss of movement and motricity under the affected spinal level. Studies concerning PU occurrence are important so intervention measures can be implemented. Thus, this study was performed, in a retrospective way, at a university hospital of Upstate São Paulo. After the approval by the institution’s committee of regulamentation and ethical norms, patient records were reviewed and, from the total, 47 met the inclusion criteria: have a TSCI diagnosis, patient aged over 18 years at the time of injury; have information concerning the first hospitalization after the injury. Most patients were male (97.5%), white (68.1%), aged within 21 and 30 years (36.2%). The most common cause for TSCI was GS (44.7%) followed by vehicle crash/overturn (23.4%). Concerning the neurologic level of the lesion, there was a predominance of the toracic level (44.7%) followed by cervical level. The average time of hospitalization was,in days, 21.47 (SD= 28.50). In terms of patient’s residence, 46.8% were from Ribeirão Preto. Twenty patients (42.5%) presented PU, with an average of 2.3 PU per patient. The most frequent regions were the sacral (36.95%) and heels (17.4%). Only 25% of the patient record stated the PU’s dimensions, 80% stated the aspect, and 52.1% did not state the classification stage. There was a higher frequency of prevention measures with patients with PU. During the period of hospitalization, 44.7% of the patients presented other clinical issues, such as pneumonia (25.5%). From the patients discharged, 34% were hospitalized again within the first 6 months after the TSCI, and the main reason was surgical site infection, followed by PU, and infection of the urinary tract. With this study, it was verified that there is a need for improving the records of patient data so interventions could be proposed. Permanent education programs could contribute to modifying this situation. Other investigations about the subject are necessary.

Condições de saúde e cuidado domiciliar de indivíduos com lesão de medula espinhal / Health conditions and home care of individuals with spinal cord injury.

O estudo observacional e transversal teve por objetivos caracterizar indivíduos adultos que sofreram lesão de medula espinhal (LME) entre janeiro de 2003 a julho 2006, em hospitais credenciados pelo SUS no município de Ribeirão Preto (RP), avaliar a sua independência funcional utilizando a escala medida de independência funcional (MIF), considerando o nível de lesão, identificar a prevalência de úlcera de pressão (UP) e problemas de funcionamento intestinal, assim como as condições de cuidado domiciliar e o acesso e utilização de serviços de saúde. Após a aprovação do Comitê de Ética em Pesquisa, o estudo foi desenvolvido em duas fases. Na primeira foi feito levantamento de prontuários em 2 hospitais e, na segunda, entrevistas com sujeitos e cuidadores nos domicílios, utilizando instrumentos préviamente testados. Os resultados evidenciaram que 28 indivíduos residentes em RP sofreram LME no período e que 19 (67,9%) tiveram UP. Desses, 6 foram a óbito antes da segunda fase do estudo. Dentre os 22 sobreviventes, 91% eram do sexo masculino e 14 (63,6%) tinham entre 20 a 39 anos. Acidente de trânsito foi a etiologia principal (50%) da LME, seguida de queda (27,3%). Onze (50%) tiveram lesão cervical, dez (45,5%) lesão torácica e um lesão lombar. Quinze (68,2%) não tinham ocupação remunerada após a LME e 12 (54,5%) tinham a esposa como cuidador principal. Indivíduos com lesão cervical apresentaram escores menores na MIF total e motora, entretanto, a MIF cognitiva atingiu o valor máximo independente do nível da lesão. Dos 22 sujeitos, nenhum apresentou grau de dependência completa, 11 (50%) apresentavam dependência mínima, 6 (27,3%) dependência máxima e 5 (22,7%) independência moderada ou completa. Os 7 sujeitos com UP tinham maior dependência funcional. Quinze (68,2%) tinham constipação intestinal. O tempo total de cuidado variou de 4 a 15 horas diárias, com média de 9,63, (d.p. 3,4) para sujeitos com lesão cervical; 7,8 (d.p. 2,8) para lesão torácica e 4 para lombar. Houve aumento dos escores da MIF com o aumento do tempo pós-lesão, independente da participação em programa de reabilitação. O trauma causou maior impacto no domínio motor com diminuição da independência funcional nas diferentes atividades para os sujeitos com lesão cervical, entretanto, esses apresentaram escores mais elevados na função controle intestinal do que os indivíduos com lesão torácica. Os resultados apontam aspectos essenciais para a proposição de programa de reabilitação para essa população no contexto estudado. / The purposes of this cross-sectional observational study are: to characterize adult individuals that suffered Spinal Cord Injury (SCI) between January 2003 and July 2006 in hospitals of the Single Health System (SHS) in the city of Ribeirão Preto, and evaluate their functional independence using the Functional Independence Measurement (FIM) scale taking injury level into consideration; and to identify the prevalence of individuals with pressure ulcers (PU) and bowel function problems, as well as home care conditions and the accessibility and use of health care services. After being approved by the Research Ethics Committee, the study was performed in two phases: in the first, patient records from two hospitals were surveyed; in the second, patients and home-care providers were interviewed using previously tested instruments. Results showed that 28 individuals suffered SCI in the referred period, of which 19 (67.9%) developed PU. Six patients died before the second phase of the study. Among the 22 survivors, 91% were men and 14 (63.6%) were between 20 and 39 years old. The main SCI etiology was traffic accidents (50%), followed by falls (27.3%). Eleven (50%) individuals had cervical injury, ten (45.5%) thoracic injury, and one lombar injury. Fifteen (68.2%) were unemployed after SCI and 12 (54.5%) had their wife as their main care provider. Individuals with cervical injury presented lower total and motor FIM scores. However, cognitive FIM scores were the highest, regardless of the injury level. None of the 22 patients presented a degree of complete dependency; 11 (50.0%) presented minimum dependency, six (27.3%) maximum dependency, and five (22.7%) moderate or complete independency. The seven participants with PU were more functionally dependent. Fifteen (68.2%) had constipation. Total care time ranged from 4 to 15 daily hours, with an average of 9.63 (SD 3.4) for individuals with cervical injury; 7.8 (SD 2.8) for thoracic injury; and 4 for lombar injury. FIM scores increased with post-injury time, regardless of participating in rehabilitation. The trauma caused more impact in the motor domain, with reduced functional independence in various activities for individuals with cervical injuries. However, the latter presented higher scores in bowel control function compared to individuals with thoracic injury. Results highlight the essential aspects of their experiences to propose a rehabilitation program for this population in the studied context.

Padronização do modelo experimental de lesão da medula espinal e avaliação da lesão neurológica em camundongos / Standardization of a spinal cord lesion model and neurologic evaluation using mice

INTRODUÇÃO: A lesão da medula espinal é um dos grandes desafios da medicina. Apesar de décadas de pesquisa sobre o assunto, seu tratamento ainda não é satisfatório. A padronização de modelos de lesão da medula espinal permite a reprodutibilidade e a análise dos resultados sendo importante para a pesquisa sobre o tema. OBJETIVO: Validar a padronização de um modelo de lesão da medula espinal e avaliação da lesão neurológica em camundongos. MÉTODOS: Submetemos 30 camundongos BalbC divididos em 4 grupos experimentais e um grupo controle à lesão da medula espinal torácica por queda de peso de diferentes alturas (gerando lesões de graus variados). O grupo controle (SHAM) foi submetido apenas à laminectomia. Os camundongos foram avaliados por seis semanas durante as quais foram aplicadas escalas de avaliação funcional motora. Após seis semanas os animais foram sacrificados para avaliação histológica das medulas espinais lesadas. Os achados foram correlacionados entre si para validar se a lesão foi efetiva e se os grupos diferenciaram-se entre os diferentes graus de lesão. Adicionalmente avaliamos se as escalas utilizadas são aplicáveis e se são fiéis aos achados histológicos. RESULTADOS: Seis dos trinta camundongos do experimentos evoluíram para óbito sendo um do Grupo 3, um do Grupo 4 e quatro do Grupo 5. Um camundongo do Grupo 4 apresentou autofagia. O Grupo 5 foi excluído do experimento por alta mortalidade e perda de dados. Todas as escalas funcionais estudadas foram estatisticamente diferentes entre si e demonstraram evolução durante o experimento. Os achados foram confirmados por histologia e apresentaram uma correlação forte com as escalas BBB e BMS e moderada a forte com a escala MFS. A Escada Horizontal apresentou forte correlação com a degeneração neurológica porém não apresentou correlação com os demais parâmetros histológicos estudados. CONCLUSÃO: O modelo de lesão da medula espinal em camundongos apresentado neste estudo é efetivo, confiável e reprodutível, com exceção da lesão causada por queda de peso (10g) de 50mm de altura, que traz mortalidade inaceitável. Das escalas estudadas, BBB e BMS são as mais confiáveis, enquanto que a Escada Horizontal tem seu uso discutível / INTRODUCTION: Spinal cord lesion is a great medical challenge. Even with many decades of research, no satisfactory treatment is available yet. The standardization of animal experimentation models makes the spinal cord lesion reproducible allowing a reliable analysis of the results. Hence, standardization is a major concern in spinal cord lesion research. OBJECTIVE: To validate the standardization of a spinal cord lesion model with neurologic evaluation using mice. METHODS: Thirty BalbC mice were divided in four experimental groups and one control group and submitted to spinal cord lesion produced by weight drop from different heights (producing different severity lesions). The control group (SHAM) was submitted to laminectomy only. Every mice was followed up for six weeks during which functional motor scales were applied. After six weeks the animals were sacrificed for histological examination. Findings were correlat-ed to confirm if the spinal cord lesion was effective and if the groups were dif-ferent between themselves. Additionally all functional motor scales were corre-lated with the histological findings to confirm if the scales are reliable and truly represented the spinal cord lesion. RESULTS: Six mice died during the experi-mentation period (one mouse from the Group 3, one mouse from the Group 4 and four mice from Group 5). One mouse from Group 4 presented autophagia and was excluded from the experiment. Group 5 was excluded from the exper-iment for high mortality rates and data loss. All functional motor scales applied demonstrated significant results with moderate or strong correlation with the histological findings. The Horizontal Ladder scale had strong correlation with neurologic degeneration but had weak or worse correlation with the rest of the histological parameters studied. CONCLUSION: The spinal cord lesion model using mice presented in this study is reliable and reproducible, excluding the lesion produced by a weight drop (10g) from 50mm, which brings unacceptable mortality rate. Of all fuctional motor scales studied, BBB and BMS scales are the most reliable. The use of the Horizontal Ladder scale, however, must be carefully evaluated

A mortalidade em pessoas com lesão medular é três vezes maior que na população em geral. As doenças cardiovasculares são umas das mais relevantes na lista de doenças que as acometem, ocorrendo cada vez mais precocemente. Dentre os fatores de risco para doenças cardiovasculares a inatividade física é um importante fator modificável para esta população. O grupo de tetraplégicos é o que demonstra maior comprometimento físico. Devido ao menor recrutamento muscular voluntário, as possibilidades terapêuticas para o condicionamento cardiorrespiratório são limitadas. Entre as alternativas terapêuticas está o cicloergometro com estimulação elétrica funcional computadorizada - CFES. Este estudo teve como objetivo comparar os efeitos do condicionamento dos músculos estimulados por corrente elétrica em pacientes tetraplégicos na frequência de treinamento de uma (1) vez por semana e de três (3) vezes por semana, durante seis meses de treinamento. Quatorze indivíduos adultos, homens, tetraplégicos, divididos aleatoriamente em dois grupos, realizaram CFES com as diferentes frequências de treinamento. Foi avaliada a variável trabalho desenvolvida durante cada sessão. A análise estatística foi realizada através de inferência bayesiana. As observações realizadas no presente estudo indicam que o desempenho musculoesquelético do grupo de treinamento de três vezes por semana pode ser melhor que o de uma vez por semana. A variabilidade de desempenho encontrada em estudos longitudinais sugere protocolos de intervenção mais abrangentes. / Mortality in people with spinal cord injury is three times higher than in the overall population. Cardiovascular diseases are the most prevalent one affecting these individuals, occurring increasingly at an earlier age. With the risk factors for cardiovascular diseases, physical inactivity is an important and modifiable aspect for this population. The quadriplegic group is the one showing the larger physical impairment. Due to the lower volunteer muscle recruitment, therapeutic possibilities for cardiorespiratory conditioning are limited. Among the therapeutic alternatives is the computadorized functional electric stimulation - CFES. This study aimed to compare the effects of ergometric training over the conditioning of the muscles of quadriplegic patients, stimulated by electric current, once and three times a week, for six months of training. Fourteen quadriplegic male adults were divided randomly into two groups, performing the training in the different treatment frequencies. Mechanical work varialble was assessed throughout each treatment session. The statistical analysis was performed by Bayesian Inference. The study\'s observations indicate that the musculoskeletal performance of those in the three times a week treatment may be better than of those in the lower frequency group. The variability of the performance found with longitudinal studies suggests broader intervention protocols.

Efeito neuroprotetor da hipotermia epidural após a lesão medular contusa em ratos / Neuroprotective effect of epidural hypothermia after spinal cord lesion in rats

Introdução: A lesão da medula espinhal é uma entidade clínica grave e extremamente incapacitante. Muitos esforços estão sendo realizados para melhorar a resposta neurológica ao trauma da medula espinhal. Dentre eles, destacamos o uso de agentes farmacológicos, a descompressão e estabilização cirúrgica precoces e a hipotermia. A hipotermia pode ser induzida de forma sistêmica ou local. Várias complicações, como arritmias cardíacas, coagulopatias e infecções, foram associadas ao uso sistêmico da hipotermia. Porém, sua aplicação local demanda a necessidade de intervenção cirúrgica de emergência e manejo pós-operatório complicado. Objetivo: Avaliar o efeito neuroprotetor da hipotermia epidural em ratos. Material e método: Foram arrolados 30 ratos Wistar pesando entre 320-360 g e divididos aleatoriamente em dois grupos: o grupo da hipotermia epidural e o grupo controle, com 15 ratos cada. Uma contusão medular produzida por queda padronizada de peso de 10 g, a 25 mm de altura, usando o New York University (NYU) Impactor, foi realizada após a laminectomia em T9-10 em todos os ratos. Os ratos do grupo da hipotermia foram submetidos ao resfriamento a 9-10 °C por um período de 20 minutos, logo após a contusão medular. Os grupos foram analisados durante seis semanas quanto à função motora utilizando-se a escala BBB e o teste do plano inclinado. Ao final da sexta semana, foi realizado ainda o exame de potencial evocado motor dos ratos, cujos resultados foram comparados entre os dois grupos. Resultados: A avaliação da função motora através da aplicação da pontuação da escala BBB ao longo das seis semanas não evidenciou diferenças estatisticamente significantes entre os dois grupos. Não encontramos diferenças estatísticas na avaliação motora através da pontuação do teste do plano inclinado ao longo das seis semanas do estudo. Os valores de latência e amplitude do potencial evocado motor não mostraram diferenças estatísticas significantes entre os grupos ao final da última semana do estudo. Conclusão: A hipotermia não apresentou efeito neuroprotetor quando aplicada no sítio da lesão, logo após a contusão medular, no espaço epidural de ratos Wistar / Introduction: Spinal cord injury (SCI) is a critical and extremely disabling clinical condition. Considerable effort has been made to improve the neurological response to the spinal cord lesion. We must highlight pharmacological agents, early surgical decompression and stabilization and hypothermia. Therapeutic hypothermia can be achieved systemically or locally. Many complications have been associated to the systemic hypothermia, such as cardiac arrhythmias, coagulopathies and infection. However, local application demands surgical intervention and difficult post operative care. Objetive: To evaluate the neuroprotective effect of epidural hypothermia in rats. Methods: Wistar rats (n = 30; weighting 320-360 g) were randomized in two groups: the hypothermia and the control group, with 15 rats in each. A spinal cord lesion was produced by the standardized drop of a 10 g-weight from a height of 2,5 cm, using the New York University Impactor, after the laminectomy at the T9-10 level. Rats of the hypothermia group underwent epidural hypothermia for 20 minutes immediately after spinal cord injury. Motor function was assessed during six weeks using the BBB motor scores and inclined plane test. At the end of the last week, neurologic status was monitored by the motor evoked potential exam and the results were compared between the two groups. Results: Analysis of the BBB scores during the six-weeks period did not show any statistically significant difference between the two groups. We did not find any significant difference between the groups in the scores of the inclined plane test during the six-weeks period. Latency and amplitude values of the motor evoked potential exam did not show any statistically significant difference between the two groups at the end of the study. Conclusion: Hypothermia did not produce any neuroprotective effect when applied immediately after spinal cord contusion, at the injury level and in epidural space of Wistar rats

Identificação das práticas de auto-cuidado referentes ao funcionamento intestinal em pacientes com trauma raquimedular / Identification of self care practices related to bowel functioning in patients with spinal cord injury

O trauma raquimedular (TRM) é uma agressão na medula espinhal e pode acarretar severas alterações físicas e psíquicas ao paciente, refletindo na família e sociedade. Com a inexistência de terapêutica que reverta o quadro de TRM, é necessário investimento na reabilitação para melhorar a qualidade de vida destes pacientes. Através de um estudo exploratório-descritivo, com metodologia quantitativa foi investigada as práticas de autocuidado intestinais e complicações em 27 indivíduos com TRM. Todos os participantes eram do sexo masculino e predominância da faixa etária de 18 a 30 anos, solteiros, com até 1º grau incompleto, com renda familiar entre 2 e 5 salários mínimos, tendo o ferimento por arma de fogo como principal etiologia da lesão. As práticas de autocuidado intestinal mais utilizadas foram a massagem abdominal (32%), seguida da dieta rica em fibras (24%), uso de laxante (15%), toque dígito anal (15%), extração manual das fezes (7%) e outros (7%). Quanto às complicações intestinais, houve predomínio de constipação/impactação (50%), seguido de incontinência fecal (23%), sangramento anal (21%) e outras (6%). Em relação às internações hospitalares, consideradas oportunidades para o enfermeiro orientar sobre as alterações decorrentes do TRM, 59,3% dos pacientes apresentaram mais de uma internação, porém a transmissão de informação sobre alterações intestinais ocorreu em somente 22% dos casos. Diante do número reduzido de centros especializados em reabilitação, julgamos de grande importância a presença de enfermeiros com formação específica em reabilitação para atuarem na assistência, na educação dos pacientes e no ensino nas instituições formadoras de profissionais. / Spinal cord injury (SCI) is an aggression to the spinal cord and can bring physical and psychological alterations to patients lives, reflecting upon family and society. So far, there is no therapeutic to revert the injury of the spinal cord; thus, it is necessary to invest on rehabilitation to improve the patient´s quality of life. Practices of self care for bowel functioning and complications were studied in 27 individuals with SCI, through a descriptive-exploratory study with a quantitative methodology. All participants were men, mostly between 18 and 30 years of age, single, with low education level, family income between 1 to 5 minimal wages; and wounded by gun shots. Practices of self care mostly used were abdominal massage (32%), high fiber diet (24%), use of laxatives (15%), anal digital stimulation (15%), stool manual removal (7%) and others (7%). Related to complications, constipation/impactation was the most frequent (50%), followed by fecal incontinence (23%), bleeding (21%) and others (6%). Hospitalization is considered an opportunity for nurses to teach patients about SCI alterations and 59,3% of patients had more then one episode of hospitalization. However, only in 22% of the cases they received any information about alterations in bowel functioning. With so few rehabilitation centers in our region it is important to have nurses at terciary and general hospitals, educated in rehabilitation to deliver adequate nursing care and patient teaching. It is also necessary, in nursing educational institutions, to prepare profissionals with focus in rehabilitation.

CondiÃÃes de funcionalidade de pessoas com lesÃo medular fundamentadas no Ãndice de barthel: proposta de intervenÃÃo de enfermagem / Conditions of functionality of persons with spinal cord injury based on the barthel index: motion for intervention of nursing

Este estudo busca avaliar as condiÃÃes de funcionalidade para o desenvolvimento das Atividades da Vida DiÃria das pessoas com lesÃo medular, com base no Ãndice de Barthel. Para a consecuÃÃo dos objetivos, optei por realizar um estudo transversal-quantitativo realizado no Ãmbito hospitalar e domiciliar. A amostra foi constituÃda por 74 pessoas com lesÃo medular traumÃtica, sendo 47 hospitalizadas e 27 domiciliares, as quais foram atendidas ou acompanhadas em um hospital pÃblico municipal, referÃncia em emergÃncia e trauma em Fortaleza-CearÃ-Brasil. A coleta de dados ocorreu durante os meses de marÃo a junho de 2010, mediante a utilizaÃÃo de um formulÃrio estruturado composto por variÃveis independentes referentes aos dados sociodemogrÃficos e clÃnicos sobre a lesÃo medular e por variÃveis dependentes que corresponde à escala do Ãndice de Barthel, a qual avalia dez atividades da vida diÃria. Os aspectos Ãticos relativos à pesquisa em seres humanos foram respeitados durante todo o processo. Para anÃlise estatÃstica dos dados foi utilizado o programa Predictive Analytics SoftWare (PASW) versÃo 18.0. atravÃs do Mann â Whitney Test e Kruskal â Wallis Test para comparar as diferentes pontuaÃÃes do Ãndice de Barthel entre as caracterÃsticas envolvidas. Os achados foram discutidos com base na literatura pertinente, sendo evidenciado que as pessoas mais acometidas pela lesÃo medular foram, na maioria, homens jovens, com idade entre 18 e 40 anos, com baixa escolaridade, sem ocupaÃÃo e residentes em Fortaleza. A lesÃo mais evidente foi a paraplegia completa, com tempo de lesÃo atà seis meses, decorrentes de perfuraÃÃo por arma de fogo, aspectos estes comums aos grupo hospitalar e domiciliar. Quanto a funcionalidade para o desenvolvimento das AVDs, apÃs a aplicaÃÃo do Ãndice de Barthel, de uma forma geral, manifestou-se de forma distinta em relaÃÃo aos grupos. Os hospitalizados demonstraram maior dependÃncia, enquanto, que os domiciliares demosnstraram maior independÃncia. Ao analizar cada categoria ficou evidente que as atividades: higiene pessoal, intestinos, deslocaÃÃo, mobilidade, uso de toilet, vestir e banho, apresentaram resultados divergentes, os hospitalizados mostraram maior dependÃncia, em quanto que os domiciliares, uma maior independÃncia. Apenas nas atividades alimentaÃÃo, bexiga e escada, à que foi obitido resultados convergentes, tanto para os hospitalizados quanto para os domiciliares, os quais demonstraram independÃncia para alimentar-se, incontinÃncia urinÃria e incapacidade para subir/descer escadas sem ajuda. ApÃs levantados destes achados foi proposto um modelo de intervenÃÃo fundamentado no processo de enfermagem, o qual compreendeu 23 diagnÃsticos de enfermagem da North American Nursing Diagnosis Association (NANDA), 32 intervenÃÃes de enfermagem da Nursing Interventions Classification (NIC) e 23 resultados de enfermagem da Nursing Outcomes Classification (NOC). Para concluir, cabe ressaltar que o Ãndice de Barthel mostrou ser uma escala de fÃcil aplicaÃÃo, eficaz na obtenÃÃo dos resultados e com sensibilidade em detectar Ãs alteraÃÃes clÃnicas dos pacientes. Por fim, este estudo, alÃm de atingir os objetivos propostos, contribuiu para o crescimento e autonomia do conhecimento e da prÃtica de enfermagem neurolÃgica brasileira, em especial à cearense, ao propor um modelo de intervenÃÃo de enfermagem com vista à promoÃÃo da saÃde e melhoria da qualidade de vida das pessoas com lesÃo medular. / This research seeks to evaluate functionality conditions for the development of Activities of Daily Living with people suffer of spinal cord injuries based on Barthel Index. For this purpose I opted to do a qualitative research in home and hospital environments. The sample was made up of 74 people with traumatic spinal cord injuries â 47 were in hospital and 27 at home â who were nursed in public municipal hospitals which were a reference in trauma and emergency in the city of Fortaleza-CearÃ-Brazil. The collecting of the data occurred from March through June of 2010 using a form made up of independent variables referring to clinical and sociodemographic and dependent variables data which corresponds to the Barthel index scale that evaluates ten activities of daily living. The ethical aspects related to the research carried out in human beings were respected during the whole process. For statistical data analysis, we used the program âPredictive Analytics Softwareâ (PASW), version 18.0. The findings were discussed based on the pertinent literature, becoming evidenced that the people who were most affected by spinal cord injury were mostly young men, between 18 and 40 years old, with low school attendance and without occupation and Fortaleza residents. The most evident was complete paraplegia with a six-month span due to gunfire perforation which are common aspects to the hospital and home groups. As for the functionality for AVDs development, after the Barthel index application, in a general way, manifested distinctively in relation to the groups. Those who were hospitalized demonstrated greater dependence whilst those at home demonstrated greater independence. By analyzing each category it got clear that the activities: personal hygiene, intestines, transferring, mobility, toilet using, dressing and bathing, presented diverging outcomes. Those who were hospitalized showed greater dependence whilst those who were at home displayed greater independence. Only in eating activities the results were converging both to the hospitalized and home patients. The latter ones demonstrated independence to feed, urinary incontinence and were incapable of going up / going down the stairs without help. After these findings, we proposed an intervention model grounded on the nursing process which was made up of 23 nursing diagnoses from the âNorth American Nursing Diagnosis Associationâ (NANDA), 32 nursing interventions from the âNursing Interventions Classificationâ (NIC) and 23 nursing outcomes from the Nursing Outcomes Classification (NOC). To sum up, it is appropriate to state that the Barthel Index came to be an easy application scale, efficient in obtaining the outcomes and sensitive in detecting the patientâs clinical alterations. And last, but not least, this research, besides of achieving the proposed goals, contributed to the enhancing of knowledge and its autonomy and the Brazilian neurological nursing practice, especially that from CearÃ, by proposing a nursing intervention model aiming to promote health and life quality betterment of the people who suffer of spinal cord injury.
