Indicadores do risco de broncoaspiração em pacientes com acidente vascular cerebral isquêmico em fase aguda / Indicators of the risk of bronchoaspiration in patients with acute ischemic stroke

Introdução: O acidente vascular cerebral (AVC) é a principal causa de mortalidade no Brasil, sendo considerado um dos maiores problemas de saúde pública do país. A alteração da deglutição, conhecida como disfagia, tem alta incidência e prevalência e contribui para o aumento dos índices de mortalidade, morbidade, custos hospitalares e principalmente, para redução da qualidade de vida dos pacientes e cuidadores. Desta forma, o estabelecimento de diagnósticos, prognósticos e tratamentos adequados são fundamentais. Objetivo: Investigar os indicadores de risco de broncoaspiração em pacientes com AVC isquêmico (AVCi) em fase aguda admitidos em pronto-socorro (PS). Métodos: Participaram do estudo 134 adultos admitidos no PS de um hospital terciário, com diagnóstico de AVCi em fase aguda, submetidos à avaliação fonoaudiológica em beira de leito. As etapas de coleta de dados envolveram: avaliação fonoaudiológica clínica do risco de broncoaspiração, determinação da gravidade do AVCi de acordo com a Escala da National Institute of Health Stroke Scale (NIHSS) no momento da avaliação fonoaudiológica, avaliação objetiva da deglutição por meio do exame de videofluoroscopia da deglutição (VDF), levantamento das variáveis demográficas e indicadores clínicos. Resultados: Os 134 pacientes foram divididos em dois grupos de acordo com os resultados da avaliação fonoaudiológica: G1 (grupo de risco para disfagia) - 42 participantes que falharam na avaliação por apresentar pelo menos um destes sinais clínidos (ausculta cervical alterada, voz molhada, tosse ou engasgo) e G2 (grupo sem risco para disfagia) - 92 participantes que não apresentaram falha na avaliação. As análises indicaram os seguintes resultados significantes: os participantes de G1 apresentaram média de idade superior a 69 anos, maior pontuação na NIHSS, demoraram mais tempo para receber alta fonoaudiológica, receberam maior número de indicação de via alternativa de alimentação após avaliação fonoaudiológica e apresentaram pior desfecho (receberam menos alta fonoaudiológica). O sinal clínico preditor de broncoaspiração que mais diferenciou os grupos foi a presença de tosse, com o volume de oferta de 50 ml. Conclusão: Pacientes com AVCi agudo que apresentarem idade superior a 69 anos, com escore >= 9 na NIHS, e que apresentarem tosse após a deglutição no teste com água (50 ml) em beira de leito, devem ser priorizados para avaliação fonoaudiológica completa e se necessário, encaminhados para a confirmação dos resultados por exame de imagem / Introduction: Stroke is the leading cause of mortality in Brazil and is considered one of the country\'s major public health problems. The swallowing disorder, known as dysphagia, has a high incidence and prevalence and contributes to an increase rates of mortality, morbidity, hospital costs, and mainly to reduce the quality of life of patients and caregivers. In this way, the establishment of adequate diagnoses, prognostics and treatments are fundamental. Objective: To investigate the risk indicators of bronchoaspiration in acute stroke patients admitted to Emergency Room (ER). Methods: 134 adults admitted to the ER of a tertiary hospital with a diagnosis of acute ischemic stroke (AIS), submitted to a bedside speech-therapist assessment, participated in the study. Data gathering involved: clinical speech-language assessment of the risk of bronchoaspiration, determination of the severity of stroke according to the National Institute of Health Stroke Scale (NIHSS) at the time of the speech-language evaluation, objective assessment of swallowing by examination of Videofluoroscopy of Swallowing (VFS), and the verification of demographic variables and clinical indicators. Results: The 134 patients were divided into two groups according to the results of the speech-language evaluation. Participants who failed (i.e. presence of indicatives of oropharyngeal dysphagia) when there was positive response on at least one of the following protocol items: cervical auscultation, wet or gurgly sounding voice after drinking, coughing, or choking were grouped as \"Risk for Dysphagia\" (n=42), and those who did not fail were grouped as \"No Risk\" (n=92). The analysis indicated the following significant results: patients with a higher risk of bronchoaspiration presented mean age over 69 years, NIHSS score <= 9 points, had more indication of alternate feeding after speech-language evaluation and presented poorer outcome (fewer individuals discharged from swallowing rehabilitation sessions). The predictor of bronchoaspiration that most differentiated the groups was the presence of cough, with a supply volume of 50 ml. Conclusion: Patients with AIS, admitted to the ER, who present age >= 69 years, score on the NIHSS >= 9, cough after swallowing in water test (50 ml), should be prioritized for the complete speech-language evaluation, and sent to support the results by image examination

The feeding and swallowing impact survey (FS-IS): cross-cultural adaptation for the South African context

Background: Feeding and swallowing difficulties (FSD) are found in typically developing children and children with complex medical and developmental conditions. These difficulties may have negative health consequences which can be stressful for caregivers as they are required to provide care for their children in the home environment. The Feeding and Swallowing Impact Survey (FS-IS) is a subjective rating scale used to determine the health-related quality of life (HRQoL) of caregivers of children diagnosed with FSD. However, this scale is not yet available in any of the official South African languages (except for English), and has not yet been culturally or linguistically adapted for the South African context. Research aims: The study aimed to cross culturally adapt and validate the FS-IS for the South African context by 1) describing the content validity of the FS-IS in a South African context; 2) describing the cultural and linguistic appropriateness of the English, isiXhosa and Afrikaans versions of the FS-IS; and 3) describing the experiences caregivers of children have in caring for their child with FSD using the FS-IS, in a pilot study. Methodology: A descriptive exploratory design was used to cross-culturally adapt and pilot the FS-IS, which consists of 3 subsections related to daily activities, worrying and problems with feeding. Five expert speech-language therapist (SLT) participants were identified to review the FS-IS for content validity. The FS-IS was then translated into Afrikaans and isiXhosa using the forward and back translation process. Caregivers (n=15) were identified at feeding clinics to determine the cultural and linguistic appropriateness of the FS-IS in English (n=5), Afrikaans (n=5) and isiXhosa (n=5). Their recommendations were taken into consideration and changes made. The pilot study included caregivers of children with FSD attending feeding clinics at two institutions (n=32) who completed the FS-IS. The participants in the pilot study included parents as primary caregivers (n=28; 88%), grandparents (n=2; 6%), as well as foster parents (n=2; 6%). Thirty-one participants were female with 14 English speaking, 9 isiXhosa and 9 Afrikaans speaking. The caregivers were the primary caregivers of children with a variety of FSD including non-oral feeds, oral feeds with specific modifications and picky or selective eaters. Results: The FS-IS was found to have content validity as experts and caregiver participants judged it to be contextually relevant for the South African context. Caregivers considered the items on the FSIS important, clear and appropriate for speakers of their native language as well as for fellow South African families, with minor changes suggested for the isiXhosa translated version. The tool has high internal consistency (Cronbach's alpha = 0.827) as well as excellent intra and inter-rater reliability (100% agreement). Daily activities that caregiver participants found most difficult included getting help from others (50%, n=16) and leaving their child in the care of others as they are scared to have others feed or take care of their child (62.5%, n=19). The majority of caregiver participants reported concerns related to their child's general health (84%, n=27) and whether they were doing enough to help with their child's FSD (50%, n=16). Few caregivers reported difficulties with feeding, with 87.5% (n=28) reporting no difficulties as a result of the time taken to prepare meals and 72% (n=23) reported no difficulty due to professionals or family having differing opinions about how to feed their child with FSD. Conclusion: The results confirm that the FS-IS is a reliable and valid tool for the identification of caregivers with reduced HRQoL related to caring for their child with FSD in a South African context. The adapted and translated FS-IS can therefore be used to identify caregivers who may need additional support or referral for further management from the multidisciplinary team. The results highlighted the complexity of caring for a child with FSD and the effects of the burden of care on caregivers. Early identification of the HRQoL of caregivers will not only benefit the caregivers but also the child they are caring for as the HRQoL of caregivers impacts on the QoL of the child with FSD.

Effets d’une hyperbilirubinémie modérée sur la physiologie de la déglutition nutritive et de la coordination déglutition-respiration chez l’agneau prématuré / Effect of moderate hyperbilirubinemia on nutritive swallowing and swallowing-breathing coordination in the preterm lamb

Résumé : L’ictère néonatal, lié à l’hyperbilirubinémie, touche 90% des nouveau-nés prématurés. L’hyperbilirubinémie provoque des troubles neurologiques aigus (somnolence, dystonie, difficultés alimentaires, atteinte de l’audition) et a été reliée à des anomalies du contrôle respiratoire. Ces résultats suggèrent une neurotoxicité de la bilirubine sur le tronc cérébral, qui pourrait aussi affecter les centres déglutiteurs. Le but de cette étude est de tester notre hypothèse selon laquelle l’hyperbilirubinémie altère la déglutition nutritive et la coordination déglutition-respiration chez l’agneau prématuré. 11 agneaux prématurés (nés 14 jours avant terme) ont été randomisés dans un groupe contrôle (n = 6) et un groupe bilirubine (n = 5). À 5 jours de vie, une hyperbilirubinémie modérée (150-250 μmol/L) était induite pendant 17h chez les agneaux du groupe bilirubine. La déglutition était étudiée par l’enregistrement de la pression pharyngée, et la respiration était évaluée par pléthysmographie d’inductance respiratoire et oxymétrie de pouls. L’effet de l’hyperbilirubinémie sur la déglutition nutritive (DN) était évalué pendant une alimentation au biberon standardisée, avec du lait de brebis. Un second enregistrement était réalisé 48h plus tard, après normalisation de la bilirubine. L’alimentation était moins performante (mL/min) dans le groupe bilirubine (p = 0,002) avec des DN moins fréquentes (p = 0,003) et de plus petit volume (p = 0,01). Ces différences n’étaient pas retrouvées à distance de l’hyperbilirubinémie. La coordination déglutition-respiration était également altérée chez les agneaux du groupe bilirubine, indiquée par une tendance à la diminution de la durée de l’inhibition respiratoire pendant les bouffées de DN (p < 0,1), ceci pendant et après l’hyperbilirubinémie. Simultanément, pendant l’alimentation, les agneaux du groupe bilirubine passaient plus de temps en désaturations sévère (< 80%) que ceux du groupe contrôle. Enfin, une diminution de la fréquence respiratoire associée à une augmentation des apnées étaient observées dans les minutes suivant l’alimentation dans le groupe bilirubine (p < 0,05). La déglutition et la coordination déglutition-respiration sont altérées par l’hyperbilirubinémie modérée, chez l’agneau prématuré. La diminution globale d’efficacité de l’alimentation au biberon est associée à une poursuite de la respiration durant les bouffées de DN, ce qui pourrait favoriser les aspirations pulmonaires de lait. / Abstract : Rationale: Jaundice, secondary to hyperbilirubinemia (HB), develops in 90% of preterm newborns. HB induces acute neurological disorders (somnolence, abnormal tone, feeding difficulties, auditory dysfunction) and has been linked to alterations in respiratory control. These findings suggest neurotoxicity in the brainstem that could also affect swallowing centers. This study aims to test our hypothesis that HB impairs nutritive swallowing and swallowing‐breathing coordination in the preterm lamb. Methods: Two groups of preterm lambs (born 14 days prior to term), control (C, n = 6) and hyperbilirubinemia (HB, n = 5) were studied. At day 5 of life, moderate HB (150‐250 μmol/L) was induced during 17 h in HB lambs. Swallowing was assessed via recording of pharyngeal pressure, and respiration was studied by respiratory inductance plethysmography and pulsed oximetry. HB effect on nutritive swallowing was assessed during standardized bottle‐feeding with ewe milk. A second recording was performed 48 hours later, after recovery from hyperbilirubinemia. Results: Swallows were less frequent (p = 0.003) and of smaller volume (p = 0.01) in HB lambs, with a consequent decrease in minute swallowing in HB lambs (p = 0,004). These differences were not found after bilirubinemia was returned to normal. Swallowing‐breathing coordination was also impaired in HB lambs, as indicated by a tendency towards a decrease in % time with respiratory inhibition during bursts of swallows, during and after hyperbilirubinemia. Simultaneously, severe desaturations (< 80%) were longer in HB lambs than in C lambs. Finally, a decreased respiratory rate was observed immediately after feeding (p < 0,05), along with increased apneas duration. Conclusions: Both swallowing and swallowing‐breathing coordination are altered by acute, moderate HB in preterm lambs. The overall decreased efficiency at bottle‐feeding is accompanied by continuation of breathing during bursts of swallows, which may promote lung aspiration.

Sväljningssvårigheter förekommer hos personer i alla åldrar, men är vanligast hos de äldre. Konsekvenserna av sväljningssvårigheter i form av undernäring och aspirationspneumonier bidrar till ökad ohälsa och stora samhällskostnader. Sväljningssvårigheter är inte en sjukdom i sig, utan är ett symptom av andra sjukdomar. Eftersom undernäring är vanligt hos personer med sväljningssvårigheter, bör sjuksköterskan ha kännedom om patienters erfarenheter av sväljningssvårigheter. Syftet var att beskriva patienters erfarenheter av att ha sväljningssvårigheter. Genom utförande av en allmän litteraturstudie framkom fyra teman: Erfarenheter av att sväljningssvårigheter ställer upp hinder för måltiden, Erfarenheter av begränsningar i det dagliga livet, Erfarenheter av att hantera vardagen och Den glädjelösa måltiden. Måltiden präglades av hostattacker, kvävningskänslor och att maten fastnade i halsen. Både det sociala livet och familjelivet begränsades. Patienterna hanterade situationen genom att undvika att dela måltider med andra. Patienterna kände ingen glädje av måltiden längre. Vidare forskning gällande allmänhetens attityd gentemot sväljningssvårigheter är av intresse. Ett framtagande av en konsistensanpassad meny för restauranger vore hjälpsamt för patientgruppen. / Swallowing difficulties occur in people of all ages, but is increasingly common in the elderly. Malnourishment and aspiration pneumonia are consequences of swallowing difficulties that lead to increase in ill-health and high costs for society. Swallowing difficulties are not an illness, but a symptom of other diseases. Since malnourishment is common in people with swallowing difficulties nurses should have knowledge of patients with experiences of having swallowing difficulties. The purpose of this study was to describe patients’ experiences of swallowing difficulties. Through the performance of a literature review four themes emerged: Experiences of swallowing difficulties hindering the meal, Experiences of limitations in daily life, Experiences of dealing with everyday life and The joyless meal. The meal was characterized by coughing, feelings of choking and food getting stuck in the throat. Social life and family life alike were limited. Patients handled the situation by avoiding eating with others. Patients no longer experienced joy in eating. Further research on public’s attitude towards swallowing difficulties are of interest. The development of a texture modified menu for restaurants would be helpful for this patient group.

Relação entre a lesão cerebral e alterações da deglutição em crianças com paralisia cerebral / Relationship between brain injury and changes in swallowing in children with cerebral palsy

Estudamos a relação entre local e tipo da lesão cerebral com a gravidade da disfagia, e a relação entre o grau de comprometimento motor com a gravidade da disfagia em crianças com paralisia cerebral. Foi um estudo retrospectivo de exames de videofluoroscopia da deglutição e ressonância magnética cerebral de crianças com diagnóstico de paralisia cerebral e disfagia, atendidas do Hospital das Clinicas da Faculdade de Medicina de Ribeirão Preto (HCFMRP), Universidade de São PauloBrasil, de 2008 a 2016. Foi realizada analise descritiva dos dados e utilizado o teste exato de Fisher para verificar a relação entre as variáveis estudadas. Foram incluídos na pesquisa exames de 20 crianças com idades ao exame de ressonância magnética entre 2 anos e 1 mês de vida a 10 anos e 2 meses, e ao exame de videofluoroscopia da deglutição entre 9 meses a 16 anos e 2 meses. Quanto ao gênero, seis crianças eram do sexo feminino, e 14 do sexo masculino. Com relação à classificação da função motora grossa, duas crianças eram do nível III, cinco do nível IV e 13 do nível V. Quanto a gravidade da disfagia, sete crianças apresentaram disfagia leve/moderada, três com disfagia moderada e 10 crianças com disfagia moderada/grave. As principais alterações evidenciadas na ressonância magnética, quanto à localização da lesão, foram em corpo caloso (60%), ventrículos cerebrais (60%), substancia branca (55%), tronco encefálico (35%) e cerebelo (15%). Os tipos de lesões cerebrais mais freqüentes foram encefalopatia hipóxico-isquêmica (35%), leucomalacia periventricular (25%) e encefalomalacia multicistica (15%). Não houve relação entre o local da lesão cerebral e gravidade da disfagia, entretanto houve relação entre o tipo de lesão cerebral e a gravidade do distúrbio de deglutição (p=0,02). Além disso, houve relação entre a gravidade da disfagia e o grau de comprometimento da função motora grossa. Deste modo, a gravidade da disfagia foi relacionada com encefalomalacia multicística e com o comprometimento da função motora grossa, e nossos resultados mostram a importância da avaliação de deglutição tão logo, nestes pacientes, seja realizado o diagnóstico médico. Além disso, ressaltamos a importância da avaliação da deglutição em todos graus de lesão, devido à presença de disfagia leve a intensa. / We studied the relationship between site and type of brain injury with the severity of dysphagia, and the relationship between the degree of motor impairment and the severity of dysphagia in children with cerebral palsy. It was a retrospective study of videofluoroscopy examinations of swallowing and magnetic resonance imaging of children diagnosed with cerebral palsy and dysphagia, attended at Hospital das Clinicas of the Ribeirão Preto Medical School (HCFMRP), University of São Paulo-Brazil, from 2008 to 2016. A descriptive analysis of the data was performed and Fisher\'s exact test was used to verify the relationship between the variables studied. Included in the study were examinations of 20 children aged between 2 years and 1 month of life to 10 years and 2 months, and videofluoroscopy of swallowing between 9 months and 16 years and 2 months. Six children were female, and 14 were male. Regarding the classification of gross motor function, two children were of level III, five of level IV and 13 of level V. Regarding the severity of dysphagia, seven children presented mild/moderate dysphagia, three with moderate dysphagia and 10 children with moderate/severe dysphagia. The most freqeunt lesions seen in magnetic ressonance imaging (MRI) were in the corpus callosum (60%), cerebral ventricles (60%), white matter (55%), brainstem (35%) and cerebellum (15%). The most frequent types of brain lesions were hypoxicischemic encephalopathy (35%), periventricular leukomalacia (25%) and multicystic encephalomalacia (15%). There was no relationship between the site of the brain lesion and the severity of dysphagia, however, there was a relationship between the type of brain injury and the severity of the swallowing disorder (p = 0.02). In addition, there was a relationship between the severity of dysphagia and the degree of impairment of gross motor function. Thus, the severity of dysphagia was related to multicystic encephalomalacia and the involvement of gross motor function, and our results show the importance of swallowing assessment as soon as the medical diagnosis is made in these patients. In addition, we emphasize the importance of evaluating swallowing in all degrees of injury, due to the presence of mild to severe dysphagia

Neonatal Intensive Care Unit Speech-Language Pathologists’ Perceptions of Infants with Neonatal Abstinence Syndrome

Objective: The purpose of this study was to determine the characteristics, assessment, and treatment of infants with Neonatal Abstinence Syndrome (NAS) as perceived by Speech-Language Pathologists (SLP) and whether it differed from those of other Neonatal Intensive Care Unit populations. Methods: A secure web-based questionnaire with 62 questions collected information on NAS, caseloads, treatment environment, and demographics. Twenty-six respondents initiated the survey; 42% completed most or all questions. Response analyses included descriptive and nonparametric inferential statistics. Results: Infants with NAS were on the caseloads of 73% of respondents. The majority (79%) only saw infants with NAS and feeding problems. Primary problems included incomplete or increased time to complete feeds, increased/excessive/irregular sucking rates, and reflux. Working on teams, respondents provided assessment, treatment, and education of infant feeding and state. Conclusion: Growing demand for SLP intervention with infants with NAS is likely to persist if opioid use continues to increase as projected.

Características da deglutição em idosos submetidos a diferentes estratégias de reabilitação oral protética / Characteristics of swallowing in elderly individuals submitted to different strategies for prosthetic oral rehabilitation

O objetivo deste estudo foi verificar se diferentes estratégias de reabilitação oral protética acarretam modificações nos achados clínicos e videoendoscópicos da deglutição em idosos. Métodos: participaram da pesquisa 42 idosos saudáveis com desempenho cognitivo adequado (26 mulheres e 16 homens, 60-82 anos), desdentados totais ou parciais, separados em três grupos segundo avaliação odontológica prévia: próteses parciais fixas ou removíveis (PP) (n = 15); próteses totais removíveis superior e inferior (PTR) (n = 15); e próteses totais removíveis superior e próteses totais fixas implanto-suportadas inferior (PTFIS) (n = 12). O grupo controle foi constituído de 15 idosos dentados totais (DT) (6 homens e 9 mulheres, 60-80 anos) que atenderam aos critérios de inclusão na amostra. Avaliaram-se as consistências sólida (pão), pastosa (10 ml) e líquida (10 ml). Para a avaliação clínica propôs-se uma classificação da gravidade da disfunção da deglutição orofaríngea, distribuída em cinco níveis. Na avaliação instrumental, da qual participaram 37 idosos, realizou-se teste de sensibilidade laringofaríngea, e classificou-se a gravidade do distúrbio de deglutição, de acordo com a escala de comprometimento funcional (gravidade) da deglutição após a VED, ordenada em graus de 0 à III. Para a análise comparativa entre os grupos em ambas as avaliações, e para comparação entre os níveis de comprometimento da deglutição, utilizou-se o Teste de Kruskal-Wallis, adotando-se o nível de significância inferior a 5%. Resultados: Os achados clínicos e videoendoscópicos da deglutição demonstraram não haver diferenças estatisticamente significantes (p>0,05) entre os grupos estudados, nas três consistências ofertadas. Especificamente na avaliação clínica, para a consistência pastosa o grupo DT apresentou maior prevalência de deglutição funcional, enquanto a disfagia orofaríngea moderada foi o padrão de maior ocorrência no grupo PTR (p<0,01). Para o sólido, os grupos de reabilitação oral demonstraram predomínio das disfagias (p<0,01) em relação à deglutição normal e funcional, sendo predominante a disfagia orofaríngea moderada para o grupo PTR (p<0,01). A VED demonstrou, para todos os grupos, que na deglutição de líquido houve distribuição semelhante entre os níveis, enquanto nas consistências sólida e pastosa encontrou-se predomínio de disfagia (p<0,01), havendo maior ocorrência da disfagia orofaríngea moderada (p<0,01) para o grupo PTR na consistência pastosa. O teste de sensibilidade laringofaríngea demonstrou que 19 (51,35%) indivíduos apresentaram alteração da sensibilidade, sendo 6 (40%) do grupo PP, 7 (63,63%) do grupo PTR e 6 (54,54%) do grupo PTFIS. Conclusão: Idosos saudáveis podem apresentar diferentes níveis de deglutição, havendo maior ocorrência de disfagia orofaríngea para as consistências pastosa e sólida, sendo que diferentes estratégias de reabilitação oral não resultam em modificações no padrão de deglutição desses sujeitos. / This study investigated if different strategies of prosthetic oral rehabilitation cause changes in the clinical and videoendoscopic findings of swallowing in elderly individuals. Methods: The study was conducted on 42 healthy elderly subjects with adequate cognitive performance (26 women and 16 men, 60-82 years), completely or partially edentulous, divided in three groups according to the previous dental evaluation: wearing fixed or removable partial dentures (RPD) (n = 15); wearing maxillary and mandibular removable complete dentures (RCD) (n =15); and wearing maxillary removable complete dentures and mandibular implant-supported fixed complete dentures (ISFCD) (n = 12). The control group was composed of 15 completely dentate elderly individuals (DT) (6 men and 9 women, 60-80 years) who met the inclusion criteria. Solid (bread), paste (10 ml) and liquid (10 ml) food textures were analyzed. Clinical evaluation was performed by classification of the severity of dysfunction of oropharyngeal swallowing, divided in five scores. The instrumental evaluation was applied to 37 elderly individuals, comprising the laryngopharyngeal sensitivity test for classification of the severity of the swallowing disorder according to the scale of functional impairment (severity) of swallowing after the VED, divided in scores 0 to III. Comparison between groups in both evaluations and between the scores of impairment of swallowing was performed using the Kruskal-Wallis test, at a significance level below 5%. Results: The clinical and videoendoscopic findings of swallowing revealed no statistically significant differences (p>0.05) between the study groups, for the three textures. Specifically with regard to the clinical evaluation for the paste texture, the DT group exhibited higher prevalence of functional swallowing, while the RCD group presented higher occurrence of moderate oropharyngeal dysphagia (p<0.01). With regard to the solid texture, the groups of oral rehabilitation presented predominance of dysphagia (p<0.01) in relation to normal and functional swallowing, with predominance of moderate oropharyngeal dysphagia for the RCD group (p<0.01). The VED demonstrated, for all groups, that the swallowing of liquid had a similar distribution between scores, while for the solid and paste textures there was predominance of dysphagia (p<0.01), with higher occurrence of moderate oropharyngeal dysphagia (p<0.01) for the RCD group for the paste texture. The laryngopharyngeal sensitivity test demonstrated that 19 (51.35%) individuals presented alterations in sensitivity, being 6 (40%) in the RPD group, 7 (63.63%) in the RCD group and 6 (54.54%) in the ISFCD group. Conclusion: Healthy elderly individuals may present different scores of swallowing, with higher occurrence of oropharyngeal dysphagia for the paste and solid textures; different strategies of oral rehabilitation did not cause changes in the pattern of swallowing of these individuals.

A demência é uma síndrome caracterizada pelo declínio de memória e déficit cognitivo que influenciam no desempenho social do indivíduo. É uma junção de sinais e sintomas que culminam em uma deterioração crônica, e em geral progressiva do funcionamento do intelecto, da personalidade e da comunicação. Entre os principais tipos de demência encontra-se a demência do tipo Alzheimer, a demência vascular, a demência com corpos de Lewy e a demência frontotemporal. Independentemente do tipo de demência, um dos seus primeiros indícios é o comprometimento da memória, maior dificuldade de atenção e concentração, perda de habilidades intelectivas, desorientação espaço-temporal, mudanças no comportamento, no humor e na personalidade. Além destas alterações, conforme a doença evolui, o paciente pode apresentar dificuldades para se alimentar e deglutir, e é função do fonoaudiólogo intervir nos distúrbios de memória, fala e deglutição. O presente trabalho teve por objetivo confeccionar uma cartilha de orientações fonoaudiológicas, relacionadas à linguagem e à deglutição, aos cuidadores e/ou familiares de pacientes com demência. Teve como base as dúvidas mais frequentemente encontradas sobre as manifestações fonoaudiológicas apresentadas no questionário que foi aplicado aos cuidadores e/ou familiares dos pacientes com problemas neurológicos, assim como a contribuição de achados teóricos. / Dementia is a syndrome that is characterized by the decline in memory and cognitive deficits that influence the social performance of an individual. It is a combination of signs and symptoms that culminate in a chronic deterioration, and generally progressive operation of intellect, personality and the communication of an individual. Among the main types of dementia is the dementia of Alzheimer, vascular dementia, dementia with Lewy bodies and frontotemporal dementia. Whatever the type of dementia, the first signs are memory impairment, increased difficulty of attention and concentration, loss of skills intellect, spatial-temporal disorientation, changes in behavior, mood and in personality. In addition to these changes, as the disease progresses, patients may find it difficult to get food and swallow. This is a function of the speech pathologist involved in the disturbances of memory, speech and swallowing. The study aimed was to manufacture a book of guidelines that speech therapists, related to language and swallowing, to caregivers and / or relatives of patients with dementia. It was based on the questions most frequently found on the demonstrations phonoaudiologicals presented in the questionnaire that was applied to caregivers and / or relatives of patients with neurological problems, as well as the contribution of theoretical findings.

O uso do biofeedback eletromiográfico no treinamento para correta execução da manobra de Mendelsohn em portadores de doença de Machado-Joseph: estudo prospectivo randomizado / The use of electromyographic biofeedback for training of the Mendelsohn maneuver in carriers of Machado-Joseph disease: a random prospective study

INTRODUÇÃO: na reabilitação da disfagia faríngea, a manobra de Mendelsohn (MM) é a única descrita como efetiva para o trabalho de elevação da laringe e seqüente abertura do segmento faringo-esofágico, desde que executada corretamente. OBJETIVO: avaliar a eficácia do uso do biofeedback eletromiográfico no processo de treinamento para correta execução da manobra de Mendelsohn em portadores de Doença de Machado-Joseph (DMJ), com disfagia alta. CASUÍSTICA: participaram deste estudo 20 indivíduos sem queixas quanto à deglutição e 20 disfágicos, portadores de DMJ, que se equiparavam em termos de distribuição de sexos e média de idade, tinham cognitivo preservado, e foram separados em grupos: A e C, de disfágicos, e B e D, de controles. MÉTODOS: todos foram submetidos a anamnese dirigida, avaliação clínica da deglutição e a eletromiografia para verificação do tempo de sustentação da elevação da laringe, repetida depois de treinamento para correta execução da manobra de Mendelsohn, feito com (grupos A e B) ou sem (grupos C e D) a utilização do biofeedback eletromiográfico.Dificuldades relacionadas à realização da MM foram observadas diretamente e relatadas em resposta a questionário. RESULTADOS: a comparação dos disfágicos com os controles não evidenciou diferenças marcantes quanto à avaliação clínica da fase oral da deglutição, pois os doentes demonstravam apenas déficits quanto a protrusão, retração e tônus da língua. Entretanto, quanto à fase faríngea, mostraramse diferenças mais significativas, com os disfágicos apresentando, à deglutição de líquidos e sólidos, estase faríngea, acompanhada de tosse e/ou engasgo e penetração e/ou aspiração, alterações ausentes nos controles. Houve significante aumento do tempo de sustentação da elevação da laringe, de controles e pacientes, à deglutição de saliva, durante a realização da manobra de Mendelsohn, após o treinamento, principalmente se conduzido com suporte de biofeedback eletromiográfico. O mesmo ocorreu com a deglutição de iogurte, pastoso, consistência preferida pelos pacientes. Os pacientes do grupo A chegaram a atingir o mesmo desempenho que seus controles (grupo B), à realização da MM. À observação direta de dificuldades encontradas durante a realização da manobra, notou-se dificuldade para sustentar a elevação da laringe. Quando os indivíduos foram inquiridos a respeito, destacou-se dificuldade de entendimento da MM. De significante, extraiu-se, à observação direta e ao inquérito, que os pacientes têm mais expressivas dificuldades de controle respiratório durante a realização da MM do que os controles. CONCLUSÕES: 1. o treinamento orientado, verbal e gestual, para a correta execução da MM, é capaz, por si só, de aumentar o tempo de sustentação da elevação da laringe em portadores de DMJ e controles; 2. o biofeedback eletromiográfico agrega valor ao treinamento, promovendo ainda maior tempo de sustentação da elevação da laringe, à execução da MM, em portadores de DMJ, que atingem índices semelhantes aos dos controles, na deglutição de saliva e de pastoso; 3. os portadores de DMJ apresentam freqüentemente dificuldade de controle respiratório, sem repercussão clínica, durante a aplicação da MM. / Introduction: In the rehabilitation of pharyngeal dysphagia, the Mendelsohn maneuver (MM) is the only technique described as effective for elevating the larynx with subsequent opening of the esophageal-pharyngeal segment, provided it is correctly performed. Objective: To evaluate the efficacy of electromyographic biofeedback for training of the Mendelsohn maneuver in Machado-Joseph patients (MJD) with high dysphagia. Case Studies: Forty individuals participated in this study, including twenty with no complaints of deglutition and twenty MJD dysphagic patients , who were all similar in terms of gender distribution, average age, and cognitive function. Method: The medical history of each patient was reviewed. Each subject underwent a clinical evaluation of deglutition, as well as electromyography to determine the amount of time for which larynx elevation was sustained. Electromyography was repeated following achievement of the correct performance of MM. Training for MM was completed with (groups A and B) or without (groups C and D) electromyographic biofeedback. Difficulties related to performance of MM were observed directly and reported through the completion of a questionnaire. Results: Comparison between dysphagic patients and controls did not reveal significant differences with respect to the clinical evaluation of the oral phase of deglutition, since afflicted patients only demonstrated deficits related to the protrusion, retraction and tonus of the tongue. However, significant differences were observed with respect to the pharyngeal phase. Dysphagic patients presented with pharyngeal stasis during deglutition of liquids and solids, accompanied by coughing and/or choking as well as penetration and/or aspiration; these symptoms were absent in the controls. After training, there was a significant increase during MM in the time of larynx elevation as well as in the deglutition of saliva in both controls and patients. The time of larynx elevation was further increased when training was conducted with electromyographic biofeedback support. Similar results were found for the deglutition of yogurt, which has a pasty consistency preferred by the patients. Group A patients performed MM at the same level of competency as controls (group B). During direct observation, difficulties in sustaining the elevation of the larynx were noted. Questioning of the individuals experiencing difficulty revealed a lack of understanding with respect to MM. Specifically, it was determined that patients had more pronounced difficulty in respiratory control during MM as compared to normal controls. Conclusions: 1. Guided training, both verbal and gestural, for correct performance of the MM can increase the time of sustained larynx elevation in MJD patients and controls; 2. The use of electromyographic biofeedback during training further increases the time of sustained larynx elevation in MJD patients, allowing them to reach indexes similar to those of controls with respect to deglutition of saliva and semi-fluids; 3. Carriers of MJD frequently have difficulty in respiratory control, but without clinical repercussions, during the application of MM.
