<p>This thesis gives an overview about innovations in organisations as well as the importance of a favourable organisational microclimate for the enhancement of creativity and innovation. Generating ideas is always a very fragile process for any organisation engaged in innovative activities. In order to support good and successful ideas the companies have to have a favourable organisational microclimate. This microclimate is discussed against the background of certain conditions which organisations or rather companies should obtain in order to prevail innovations and cope with them in a successful way. The main focus of this thesis is on the conditions, which can favour and support the process of emerging innovations. The conditions are teamwork, management participation, effective communication and information flow, as well as innovative culture. During the different chapters however, some interrelations can therefore be seen.</p><p>Furthermore nine interviews with three different innovative Latvian companies support our theoretical framework of this thesis, how the different conditions are interrelated with each other in practice and their occurrence in the different companies.</p><p>In the end a discussion about the results is started, where it can be seen whether more conditions emerged.</p>

Este estudo é fruto da pesquisa de mestrado do Programa de Pós-Graduação em Enfermagem da Universidade do Estado do Rio de Janeiro (ENF/UERJ). Teve por objetivo identificar e analisar as práticas realizadas pelos enfermeiros na equipe da ESF do município de Itaboraí, correlacionando-as com a Política Nacional de Atenção Básica, (PNAB, 2012). A escolha pelo estudo das práticas de enfermagem na equipe da ESF iniciou-se a partir de reflexões em torno do projeto de pesquisa denominado Saberes e Práticas na Atenção Primária a saúde: diálogos entre usuários e profissionais de saúde no Estado do Rio de Janeiro. Estas reflexões levaram ao entendimento de que as práticas de saúde dos enfermeiros influenciam e são influenciadas por constructos socias tais como a cultura, política, economia e pelas relações de poder existentes no interior das equipes de saúde; o que pode potencializar ou minimizar a realização de práticas integrais que atendam as necessidades de saúde dos usuários, a consolidação no novo modelo de assistência à saúde e o fortalecimento da identidade profissional do enfermeiro. Trata-se de um estudo descritivo, de natureza qualitativa. A pesquisa foi desenvolvida no período de 2012 a 2013, sendo utilizada a entrevista semiestruturada e a observação semiestruturada como instrumentos de coleta de dados. Foi utilizado como critério de inclusão as USFs com equipe mínima de saúde completa e cujos enfermeiros possuíssem, pelo menos, dois anos de atuação na estratégia. Os critérios de exclusão foram: USFs que não possuíssem equipe mínima completa e cujos enfermeiros tivessem menos de dois anos de atuação na ESF. Foram entrevistados 10 enfermeiros. A técnica de análise utilizada foi a análise temática proposta por Bardin associada a análise do diário de campo chegando as seguintes categorias: A práticas dos enfermeiros na ESF de Itaboraí frente a PNAB, (2012); elementos orientadores das práticas dos enfermeiros: o normativo e o social e as práticas do enfermeiro na ESF e o trabalho em equipe. Concluiu-se que os enfermeiros realizam práticas assistências, de educação em saúde e de gerência, sendo as mesmas embasadas pelos manuais do MS e pelas necessidades de saúde da população adscrita. No entanto, estas práticas encontram alguns entraves para serem realizadas dentro da equipe causando desânimo para o enfermeiro e reduzindo suas potencialidades. Acredita-se que este estudo aponta para a necessidade de uma reavaliação no âmbito organizacional da assistência da ESF neste município, assim como parece imprescindível o diálogo entre as experiências práticas e os conhecimentos teórico de forma a orientar a construção de saberes e a formulação de uma visão crítica sobre as práticas. / This Study comes from a research about Pos-Graduation Program in Nursing from the University State in Rio de Janeiro (ENF/UERJ). The objectives are identified and analyze health practices carried out by nurses in the ESF team of the municipality of Itaboraí, correlating them with the National Policy of Primary Health Care, (PNAB, 2012). The choice by the study of nursing practices in ESF team began from reflections around the research project called knowledge and practices in Primary Health Care: dialogues between users and health professionals in the State of Rio de Janeiro. Such reflections led to the understanding that the health practices of nurses influence and are influenced by social constructs such as culture, politics, economics and the power relations within the health teams; What can maximize or minimize the possibilities for realization of integral practices that contemplate the real needs of users' health, the consolidation in the new model of health care and strengthening the professional identity of the nurse. This is a descriptive study of qualitative nature. The survey was developed in the period of 2012 until 2013, being used the semi structured interview and semi-structured observations as data collection instruments. It was used as a criterion for inclusion the USFs with minimum full health team and whose own nurses, at least two years of experience in the strategy. Exclusion criteria were: USFs own minimal team not complete and whose nurses had less than two years of experience on ESF. 10 nurses were interviewed. The analysis technique used was the thematic analysis proposed by Bardin associated with analysis of the field journal arriving the following categories: nurses' practices in the ESF of Itaboraí front of PNAB, (2012); I guiding elements: the nurses' practices and the social and legal practices of nurses on ESF and teamwork. It was concluded that nurses perform practice assists, health education and management, being informed by the MS manuals and the registered population health needs. However, these practices are some obstacles to be performed within the team causing dismay to the nurse and reducing their potential. It is believed that this study points to the need for a reassessment within the organizational assistance from the ESF in this municipality, as well as seems essential dialogue between the practical experiences and theoretical knowledge in order to guide the construction of knowledge and the formulation of a critical view on the practices.

Vård- och omsorgspersonal utför ett klientnära arbete och samarbetar och får handledning av arbetsterapeuter. Studier visar att brister i kommunikation och samarbete, leder till minskad säkerhet för klienten. Den arbetsterapeutiska handledningen uppskattades av vård- och omsorgspersonalen då det underlättade i deras arbete. Idag saknas det forskning som närmare studerar vård- och omsorgspersonalens erfarenheter av arbetsterapeutiskt samarbete och handledning. Syfte: Syftet med studien var att beskriva vård- och omsorgspersonalens erfarenheter av samarbete och handledning av arbetsterapeuter. Metod: Författarna valde en kvalitativ ansats. Studiens data material insamlades via semistrukturerade intervjuer. Tio yrkesverksamma personer som arbetade inom kommunal vård- och omsorgs valde att delta i studien. Informanterna intervjuades via mobiltelefon samt personligt möte Intervjuerna analyserades med hjälp av kvalitativ innehållsanalys. Analysen av intervjumaterialet resulterade i tre huvudkategorier samt två underkategorier Resultat: Resultatet visar att tillgänglighet till arbetsterapeut, kommunikation inom teamet och förhållningssättet hos arbetsterapeuten var av stor betydelse för vård- och omsorgspersonalen. Handledningen av arbetsterapeuter ansågs vara fördelaktig i det klientnära arbetet. Slutsats: Tillgänglighet av arbetsterapeut främjar samarbete och handledning och arbetsterapeuter bör vara pedagogiska, lyhörda och tydliga i sin kommunikation. Den arbetsterapeutiska handledningen ansågs som betydelsefull och informanterna efterfrågade mer handledning och ett närmare samarbete. / Background: Healthcare workers whom perform close to the client and collaborates and get supervision from occupational therapists. Several studies shows that a lack of communication and teamwork leads to insufficient patients safety. The study also show that occupational therapist supervision was appreciated by healthcare workers as it facilitated their work. However recent research is lacking in the study of healthcare professionals experience of occupational therapy supervision. Purpose: The purpose of the study was to describe the healthcare personnel experiences of collaboration and supervision of occupational therapist.Method: The authors chose a qualitative approach and the study method is collected via semi-structured interviews. Ten professionals who worked in healthcare service in municipality participated. The interviews performed by mobile and personal meeting. Interviews were analyzed by qualitative content analysis. Analysis of the interview material resulted in three main categories and two subcategories. Results: Results show that the availability of the occupational therapist, communication within the team, and the occupational therapists approach was of great importance to healthcare workers. Supervision and management of occupational therapist was considered beneficial in the client centered work. Conclusion: Accessibility of occupational therapist promotes cooperation and supervision. Occupational therapists should be educational, responsive and clear in their communication. The occupational therapy supervision was considered important and the informants demanded more guidance and closer cooperation.

Följsamhet vid ocklusionsbehandling : - hur kan ögonsjuksköterskan främja den? / Adherence to occlusion therapy : in what way can the ophthalmic nurse improve it?

Följsamheten av ocklusionsbehandling hos barn med amblyopi är bristfällig. Det finns begränsat med forskning om vilka erfarenheter vårdpersonal har av denna bristande följsamhet, och det vore därför av intresse att undersöka vilka åtgärder ögonsjuksköterskor upplever kan vidtas för att främja följsamheten. Syftet med pilotstudien var därför att undersöka hur ögonsjuksköterskan kan främja följsamhetenav ocklusionsbehandling. Studien genomfördes som en beskrivande, kvalitativ pilotstudie med induktiv ansats där data analyserades med manifest kvalitativ dataanalys. I resultatet framkom fem kategorier, vilka påvisade vikten av att ögonsjuksköterskor tänker på att bemöta vårdnadshavare och barn med ett icke-dömande förhållningssätt, att ge tydlig och individanpassad information både muntligt och skriftligt, samt olika praktiska råd kring själva genomförandet av behandlingen. I resultatet framkom även vikten av att skapa en personlig relation vårdpersonal, vårdnadshavare och barn emellan och betydelsen av täta återbesök, samt hur ett gott samarbete och stöd kollegor emellan kan främja följsamheten. Rekommendationer för vårdverksamhet är bland annat att ge vårdnadshavare och barn kontinuitet vid besöken och rekommendationer för utbildning inom omvårdnad att lära ut vikten av ett positivt förhållningssätt och att ställa öppna frågor. Större studier med samma syfte är av intresse, samt forskning kring hur information på internet påverkar följsamheten. / Adherence to occlusion therapy with amblyopic children is inadequate. There is limited with research about medical staff’s experiences of inadequate occlusion therapy. Therefore it is of importance investigating which arrangements ophthalmic nurses’ perceive can be taken to improve adherence. The purpose of this pilot study was to investigate how ophthalmic nurses can improve adherence to occlusion therapy. The study was conducted as a descriptive, qualitative pilot study with an inductive approach where data was analyzed with manifest qualitative content analysis. Five different categories were revealed and showed the importance of a non-judgmental attitude among ophthalmic nurses towards the child and the caregivers, the importance of giving both verbal and written comprehensible, individualized information. Practical advice about carrying out the occlusion therapy emerged in the data along with the importance of creating a personal relationship between the medical staff and the child/caregiver. Frequent revisits were seen as important, and good support and cooperation between colleagues was believed to be able to improve adherence. Recommendations for care activities is to give the child and the caregivers continuity during visits and a recommendation for nursingeducation is to teach the importance of a positive attitude and open-ended questions. Larger studies with the same purpose are of interest, also further research of how information on the internet affects adherence to occlusion therapy.
