Intensified consumption makes abstinence symptoms more probable to appear. The social factors that contributed to the experimentation become less motivating and the dependence happens to be controlled by the avoidance of the symptoms of the abstinence. The continued use of the cigarette causes changes in brain physiology that makes its cessation process something continuous and complex. Some triggers identified as abstinence-hindering refer to deficits in social skills. The term HS is used to designate learned behavioral capacities that involves social interactions. The present study was conducted from February to March 2017 and evaluated and correlated degree of nicotine dependence and social skills in sixty smokers patients hospitalized by vascular surgery team at a general hospital in the interior of the State of São Paulo, with the purpose of suggesting a therapeutic group where the deficits in social skills found in this study will be able to be work on. A descriptive type of search was performed. Participants were approached and invited to participate in the research whose data were analyzed quantitatively. Of the 60 participants, 23 were women and 37 were men, mean age was 62.85 years, with 52.51 years using cigarette. Only 18.33% tried to stop smoking and the beginning of use for them were at 10.33 years. Only 21.66% of the dependents did not have a smoker's family member, and 5% reported that their support group were non-smoker. According to the Fagerström test for nicotine, the majority of the evaluated patients have low and medium degree of dependence (adding up the two values). For social skills, the total scores for both genders correlate with the Fagerström test, which shows that the more dependent the more socially skilled, which may mean that the tobacco user feels socially skillful without being, if it is so, largely from a social skills training as part of the smoking cessation treatment. / O tabaco é uma das principais causas de morte prematura no mundo e sua dependência é mantida pela nicotina e determinada por processos biológicos, comportamentais, psicológicos e socioculturais. O consumo intensificado torna possível o surgimento dos sintomas de abstinência. Os fatores sociais que contribuíram para a experimentação passam a ser menos motivadores e a dependência passa a ser controlada pela evitação dos sintomas da abstinência. O uso contínuo do cigarro causa alterações na fisiologia cerebral que farão o processo de cessação do mesmo ser contínuo e complexo. Alguns gatilhos identificados como dificultadores da abstinência referem-se a déficits em habilidades sociais. O termo HS é utilizado para designar capacidades comportamentais aprendidas que envolvem interações sociais. O presente trabalho foi realizado de fevereiro à março de 2017 e avaliou e correlacionou grau de dependência de nicotina e habilidades sociais em sessenta pacientes tabagistas internados pela equipe de cirurgia vascular em um hospital geral do interior do Estado de São Paulo com o intuito de sugerir um grupo terapêutico para que os déficits em habilidades sociais encontrados sejam trabalhados posteriormente. Foi realizada uma pesquisa do tipo descritiva. Os participantes foram abordados e convidados a participar da pesquisa cujos dados obtidos foram analisados quantitativamente. Dos 60 participantes, 23 eram mulheres e 37 eram homens, de idade média de 62,85 anos, com 52,51 anos de uso de cigarro. Apenas 18,33% tentaram parar de fumar e o início do uso foi aos 10,33 anos. Apenas 21,66% dos dependentes não possuíam nenhum familiar também tabagista e 5% referiu que o grupo de apoio não era fumante. Segundo o teste Fagerström para Nicotina a maioria dos avaliados encontra-se com grau de dependência baixa e média (somando-se os dois valores). Quanto as habilidades sociais os escores totais para ambos os gêneros correlacionam -se com o teste de Fagerström, o que mostra que quanto mais dependente mais habilidoso socialmente, o que pode significar que o usuário do tabaco se sinta habilidoso socialmente sem o ser, beneficiando-se assim, largamente de um treino de habilidades sociais como parte do tratamento de cessação do tabagismo.

Apesar de realizadas na prática clínica, há falta de estudos demonstrando quais os fatores são responsáveis pelo seu sucesso. Objetivo: avaliar, em coelhos, qual é a influência do desenho da alça de veia femoral com anastomose na artéria femoral, na presença de fluxo (patência) após sete dias. Método: Trinta e nove coelhos foram submetidas à microanastomose arteriovenosa com técnica microcirúrgica. As alças foram acomodadas em dois desenhos, um, chamado \"circular\" e outro, o mais alongado possível sem dobras na alça. Os parâmetros avaliados foram: presença ou não de fluxo, sinais de hemólise, alterações hemodinâmicas sofridas pela veia submetida à pressão arterial. Resultados: Após sete dias, o fluxo estava presente em 68% das alças \"anguladas\" e em 75% das alças \"circulares\" (p > 0,05). Houve, intragrupo, diminuição estatisticamente significante da pCO2 e aumento estatisticamente significante do pH. Não houve diferença estatisticamente significante no restante dos parâmetros avaliados entre os dois modelos de alça. Method: Thirty-nine rabbits were subjected to arteriovenous microanastomosis with microsurgical technique. The loops were accommodated in two designs, one named \"circular\" and the other, the more elongated possible without folds. The parameters evaluated were: presence or absence of flow, hemolysis, hemodynamic changes suffered by vein submitted to blood pressure. Desenvolvimento e aplicação prática de shunt vascular temporário por punção: estudo experimental em porcos / Development and practical application of a puncture temporary vascular shunt: an experimental study in pigs

Os shunts vasculares temporários (SVT) são utilizados como uma técnica eficaz para reestabelecer rapidamente o fluxo sanguíneo em casos de lesão vascular com isquemia do membro ou órgão acometido, no qual a revascularização deverá ser postergada. Habitualmente, o SVT é inserido dentro dos cotos proximal e distal do vaso lesado através de uma abertura na pele, visando restaurar a perfusão e interromper a isquemia. O objetivo deste estudo é comparar a pressão arterial média (PAM em mmHg) e o fluxo sanguíneo (em ml/min) em dois modelos de SVT, um habitual e outro implantado por punção, bem como o tempo para a inserção destes dispositivos e suas patências primária e secundária. Realizamos experimentos em 30 suínos, somando 60 intervenções de revascularização arterial temporária dos membros posteriores: trinta SVT habituais e trinta por punção. Analisamos a PAM durante os procedimentos nos membros posteriores e o fluxo através dos dois tipos de SVT. A análise de fluxo mostrou uma diferença significativa entre os SVT testados (p=0,001), sendo menor no grupo SVT por punção. No entanto, o tempo decorrido (min) para inserção do SVT habitual foi maior do que o tempo para inserção do SVT por punção (15,32 ± 3,08 vs. 10,37 ± 1,7, p=0,001). Além disso, observamos uma recuperação da PAM nos membros submetidos aos dois tipos de SVT próxima à PAM sistêmica em 100% dos experimentos. Os resultados revelaram patência primária, secundária, e taxa de complicações similares entre os dois tipos de SVT. Concluímos que o fluxo foi menor no SVT por punção, mas a recuperação da PAM foi semelhante e com menor tempo de inserção do SVT por punção / Temporary vascular shunts (TVS) are used as an effective technique to rapidly restore blood flow in cases of vascular injury with ischemia of the affected limb or organ, in which revascularization shall be postponed. Usually, TVS is positioned within the proximal and distal stumps of the injured vessel, through an opening of the skin, in order to restore perfusion and stop the ischemia. We sought to compare mean blood pressure (MBP in mmHg) and blood flow (ml/min) between two types of TVS, a standard one and a puncture one, as well as the time spent to insert these devices. We performed an experimental study on 30 pigs, including 60 vascular interventions in posterior limbs: thirty standard TVS and thirty puncture TVS. MBP was analyzed during the interventions in both posterior limbs and the flow through both types of TVS. Flow analysis revealed a significant difference between the two types of TVS (p=0,001), being lower in the puncture TVS. However, the time spent during standard TVS insertion was greater than that of the puncture shunt (15,32 ± 3,08 min vs.10,37 ± 1,7 min, p=0,001). In addition, we observed a limb MBP recovery close to systemic MBP in 100% of the experiments. The results show similar primary and secondary patency and complication rate in both TVS types. Therefore, we conclude that the flow was lower in the puncture TVS, but the MBP recovery was similar and it took less time to be inserted

Rezultati lečenja aortoilijačne bolesti endovaskularnim procedurama i klasičnim hirurškim pristupom / Results of the treatment of diseases of the aortoiliac endovascular procedures and conventional surgical access

<p>UVOD: Uspe&scaron;nost revaskularizacije aortoilijačne bolesti procenjuje se prohodno&scaron;ću (patentno&scaron;ću) nakon revaskularizacije i brojem komplikacija. Osnovni cilj ove studije je bio utvrditi da li postoji razlika u učestalosti komplikacija, prohodnosti (patentnosti) i uticaj prohodnosti arterija donjeg vaskularnog korita na rekonstrukcije aortoilijačne bolesti nakon lečenja endovaskularnim i klasičnim hirur&scaron;kim pristupom. MATERIJAL I METODOLOGIJA: uzorak je činilo 229 bolesnika lečenih od aortoilijačne bolesti koji su pripadali TASC-u B i C a nakon procene prohodnosti femoropoplitealnog segmenta kao i broja prohodnih potkolenih arterija, bolesnici su stratifikovani u dve glavne grupe a svaka grupa od po tri podgrupe prema kvalitetu utočnog korita. Prvu grupu čine bolesnici sa jednoetažnom ili multiplim kraćim lezijama aortoilijačne signifikantne stenoze, sa ne signifikantnom aortoilijačnom stenozom i sa aortoilijačnom okluzijom. Drugu grupu čine bolesnici sa vi&scaron;eetažnom aortoilijačnom signifikantnom stenozom uz signifikantnu femoropoplitealnu stenozu i/ili popliteokruralnu stenozu. Studija je kreirana kao retrospektivna i prospektivna gde smo pratili rezultate revaskularizacije endovaskularnim procedurama i klasičnim hirur&scaron;kim pristupom. REZULTATI: Računanjem Mahalanobisove distance između homogenost jednoetažne, multiplih kraćih lezija i vi&scaron;eetažne aortoilijačne bolesti kod ispitivanih bolesnika dobija se najmanje rastojanje između homogenost jednoetažne, multiplih kraćih lezija i vi&scaron;eetažne aortoilijačne bolesti zato smo se i rukovodili da jednoetažne lezije, multiple kraće lezije tretiramo endovaskularnim procedurama a vi&scaron;eetažne lezije klasičnim hirur&scaron;kim pristupom. Nije bilo statističke značajne razlike među komplikacijama endovaskularnih procedura i klasičnog hirur&scaron;kog pristupa p&gt;0,1. U lečenju aortoilijačne bolesti endovakularne procedure imaju manji broj bolničkih dana, podjednaku primarnu asistiranu patentnost ali ne&scaron;to slabiju primarnu patentnost u odnosu na klasični hirur&scaron;ki pristup dok je sekundarna patentnost neznatno lo&scaron;ija od klasičnog hirur&scaron;kog pristupa. Da bi smo odreditli uticaj prohodnosti arterija donjeg vaskularnog korita na rekonstrukcije aortoilijačne bolesti kreirali smo matematički model kome smo dali obeležja prema kome bi mogli prognozirati smanjenje primarne patenosti nakon aortoilijačne revaskularizacije endovaskularnim procedurama i klasičnim hirur&scaron;kim pristupom. Postojanjem značajnih razlika između stepena patentnosti u odnosu na obeležja faktora rizika, definisali smo funkciju razgraničenja svih mogućih kombinacija od dva stepena patentnosti. Na osnovu te funkicije (x) moguće je izvr&scaron;iti prognozu kom stepenu patentnosti pripada (dobra,solidna, lo&scaron;a) za bolesnike koji nisu obuhvaćeni ovom studijom, kao i pouzdanost tom stepenu. ZAKLJUČAK: Aortoilijačna bolest je u većini slučajeva udružena sa bole&scaron;ću donjeg vaskularnog korita. Bez obzira na ekstenzivnost aortoilijačne bolesti endovaskularne procedure su efektivna i bezbedna, adekvatna alternativa klasičnom hirur&scaron;kom pristupu. Ova studija pokazuje da su endovaskularne procedure i klasični hirur&scaron;ki pristup u lečenju aortoilijačne bolesti komplementarne a ne komparativne metode.</p> / <p>BACKGROUND. The effectiveness of revascularization aortoiliac occlusive disease is the estimated patency after revascularization and is connected to the number of complications. The main objective of this study was to determine whether there is a difference in the incidence of complications and patency of lower artery vascular tree on the reconstruction aortoiliac occlusive disease after treatment of endovascular and conventional surgical approach. MATERIAL AND METHODOLOGY: The sample consisted of 229 patients treated for the aortoiliac occlusive disease who belonged in TASC B and C after a mobility assessment of the femoropopliteal segment and the number of walk-on below knee arteries, patients were stratified into two groups and each group of three subgroups according to the quality of the inflow. The first group consists of patients with one storey or multiple shorter lesions, and aortoiliac significant stenosis with aortoiliac not as significant stenosis and occlusion aortoiliac. The second group consists of patients with multi-floor aortoiliac significant stenosis with a significant femoropopliteal stenosis and/or popliteocrural stenosis. The study was designed as a retrospective and prospective study, where we track the results of endovascular revascularization procedures and compare to the conventional surgical approach. RESULTS: By calculating the Mahalanobis distance between the homogeneity of one story, multiple lesions and multi-storey short aortoiliac occlusive disease the studied patients received a minimum distance between the homogeneity of one story, multiple lesions and multi-storey short aortoiliac disease, multiple lesions treated fewer endovascular procedures and multi-storey lesions than a classical surgical approach. There were no statistically significant differences among the complications of endovascular procedures and classic surgical approach p&gt; 0.1. The patients treated with aortoiliac (endovascular) procedure have fewer hospital days, equal-assisted primary patent protection or slightly less primary patent protection compared to a traditional surgical approach while secondary patent protection is slightly worse than the classic surgical approach. In order to determine the impact of vascular patency of the arteries of the lower stories to the reconstruction treatment of aortoiliac occlusive diseases, we have created a mathematical model to which we gave the characteristics by with which it could predict the reduction of primary patency of aortoiliac revascularization after endovascular procedures and the conventional surgical approach. The existence of significant differences between the degree of patent protection in relation to the characteristics of the risk factors, we have defined the function of the demarcation of all possible combinations of two degrees of patent protection. Based on this function of (x) it is possible to forecast to what extent patent protection belongs (good, solid&sbquo; bad) for patients who are not included in this study, as well as the reliability of this level. CONCLUSION: the aortoiliac occlusive disease is in most cases associated with vascular disease of the lower artery vascular tree. Regardless of the extensiveness of the disease, aortoiliac occlusive disease endovascular procedures are safe and effective, an adequate alternative to the conventional surgical approach. This study shows that endovascular procedures and the conventional surgical approach in the treatment of aortoiliac occlusive diseases with a complementary rather than competitive method.</p>

Determinação de incidência, preditores e escores de risco de complicações cardiovasculares e óbito total, em 30 dias e após 1ano da cirurgia, em pacientes submetidos a cirurgias vasculares arteriais eletivas / Incidence, predictors, risk scores of cardiovascular complications, and total death rate within 30 days and 1 year after elective arterial surgery

Introdução: Estima-se que ocorram 2,5 milhões de mortes por ano relacionadas a cirurgias não cardíacas e cinco vezes este valor para morbidade, com limitações funcionais e redução na sobrevida em longo prazo. Pacientes que deverão ser submetidos à cirurgia vascular são considerados de risco aumentado para eventos adversos cardiovasculares no pós-operatório. Há, ainda, muitas dúvidas em como fazer uma avaliação pré-operatória mais acurada desses pacientes. Objetivo: Em pacientes submetidos à cirurgia vascular arterial eletiva, avaliar a incidência e preditores de complicações cardiovasculares e/ou óbito total, e calcular a performance dos modelos de estratificação de risco mais utilizados. Métodos: Em pacientes adultos, consecutivos, operados em hospital terciário, determinou-se a incidência de complicações cardiovasculares e óbitos, em 30 dias e em um ano. Comparações univariadas e regressão logística avaliaram os fatores de risco associados com os desfechos e a curva ROC (receiver operating characteristic) examinou a capacidade discriminatória do Índice de Risco Cardíaco Revisado (RCRI) e do Índice de Risco Cardíaco do Grupo de Cirurgia Vascular da New England (VSG-CRI). Resultados: Um total de 141 pacientes (idade média 66 anos, 65% homens) realizou cirurgia de: carótida 15 (10,6%), membros inferiores 65 (46,1%), aorta abdominal 56 (39,7%) e outras (3,5%). Complicações cardiovasculares e óbito ocorreram, respectivamente, em 28 (19,9%) e em 20 (14,2%), em até 30 dias, e em 20 (16,8%) e 10 (8,4%), de 30 dias a um ano. Complicações combinadas ocorreram em 39 (27,7%) pacientes em até 30 dias e em 21 (17,6%) de 30 dias a um ano da cirurgia. Para eventos em até 30 dias, os preditores de risco encontrados foram: idade, obesidade, acidente vascular cerebral, capacidade funcional ruim, cintilografia com hipocaptação transitória, cirurgia aberta, cirurgia de aorta e troponina alterada. Os escores Índice de Risco Cardíaco Revisado (RCRI) e Índice de Risco Cardíaco do Grupo de Estudo Vascular da New England (VSG-CRI) obtiveram AUC (area under curve) de 0,635 e 0,639 para complicações cardiovasculares precoces e 0,562 e 0,610 para óbito em 30 dias, respectivamente. Com base nas variáveis preditoras aqui encontradas, testou-se um novo escore pré-operatório que obteve AUC de 0,747, para complicações cardiovasculares precoces, e um escore intraoperatório que apresentou AUC de 0,840, para óbito em até 30 dias. Para eventos tardios (de 30 dias a 1 ano), os preditores encontrados foram: capacidade funcional ruim, pressão arterial sistólica, cintilografia com hipocaptação transitória, ASA (American Society of Anesthesiologists Physical Status) classe > II, RCRI (AUC 0,726) e troponina alterada. Conclusões: Nesse grupo pequeno e selecionado de pacientes de elevada complexidade clínica, submetidos à cirurgia vascular arterial, a incidência de eventos adversos foi elevada. Para complicações em até 30 dias, mostramos que os índices de avaliação de risco mais utilizados até o momento (RCRI e VSG-CRI) não apresentaram boa performance em nossa amostra. A capacidade preditiva de um escore mais amplo pré-operatório, e uma análise de risco em dois tempos: no pré-operatório e no pós-operatório imediato, como o que simulamos, poderá ser mais efetiva em estimar o risco de complicações / Introduction: Approximately 2.5 million deaths are caused by non-cardiac surgeries per year, while morbidity, represented by functional impairment and a decline in long-term survival, accounts for five times this value. Patients who require a vascular surgery are considered at an increased risk for adverse cardiovascular events in the postoperative period. However, the method for obtaining a more accurate preoperative evaluation in these patients has not yet been determined. Objective: In patients undergoing elective arterial vascular surgery, the incidence and predictors of cardiovascular complications and/or total death were determined and the performance of risk stratification models was assessed. Methods: The incidence of cardiovascular complications and death within 30 days and 1 year after vascular surgery was determined in consecutive adult patients operated in a tertiary hospital. Univariate comparison and logistic regression analysis were used to evaluate risk factors associated with the outcome, and the receiver operating characteristic (ROC) curve determined the discriminatory capacity of the Revised Cardiac Risk Index (RCRI) and the Cardiac Risk Index of the New England Vascular Surgery Group (VSG-CRI). Results: In all, 141 patients (mean age, 66 years; 65% men) underwent vascular surgery, namely for the carotid arteries (15 [10.6%]), inferior limbs (65 [46.1%]), abdominal aorta (56 [39.7%]), and others (5 [3.5%]). Cardiovascular complications and death occurred in 28 (19.9%) and 20 (14.2%) patients, respectively, within 30 days after surgery, and in 20 (16.8%) and 10 (8.4%) patients, respectively, between 30 days and 1 year after the surgical procedure. Combined complications occurred in 39 patients (27.7%) within 30 days and in 21 patients (17.6%) between 30 days and 1 year after surgery. The risk predictors for cardiovascular events that occurred within 30 days were age, obesity, stroke, poor functional capacity, transitory myocardial hypocaptation on scintigraphy, open surgery, aortic surgery, and abnormal troponin levels. The RCRI and VSG-CRI showed an under the curve area of 0.635 and 0.639 for early cardiovascular complications as well as of 0.562 and 0.610 for death within 30 days, respectively. Based on the predictors found in this study, a new preoperative score was proposed, based on an AUC of 0.747 obtained for early cardiovascular complications and an intraoperative score that presented an AUC of 0.840 for death within 30 days. For late events (between 30 days and 1 year), the predictors were poor functional capacity, systolic blood pressure, presence of transitory myocardial hypocaptation on scintigraphy, class > II American Society of Anesthesiologists Physical Status score, RCRI (AUC= 0.726), and abnormal troponin levels. Conclusions: In this small group of patients with increased clinical complexity who underwent arterial surgery, the incidence of adverse events was high. In our series, we found that RCRI and VSG-CRI do not reasonably predict the risk of cardiovascular complications. The predictive capacity of a modified preoperative score and evaluating the risk preoperatively and early postoperatively, such as that simulated in this study, may be more effective in determining the risk of complications

Introdução: Estima-se que ocorram 2,5 milhões de mortes por ano relacionadas a cirurgias não cardíacas e cinco vezes este valor para morbidade, com limitações funcionais e redução na sobrevida em longo prazo. Pacientes que deverão ser submetidos à cirurgia vascular são considerados de risco aumentado para eventos adversos cardiovasculares no pós-operatório. Há, ainda, muitas dúvidas em como fazer uma avaliação pré-operatória mais acurada desses pacientes. Objetivo: Em pacientes submetidos à cirurgia vascular arterial eletiva, avaliar a incidência e preditores de complicações cardiovasculares e/ou óbito total, e calcular a performance dos modelos de estratificação de risco mais utilizados. Métodos: Em pacientes adultos, consecutivos, operados em hospital terciário, determinou-se a incidência de complicações cardiovasculares e óbitos, em 30 dias e em um ano. Comparações univariadas e regressão logística avaliaram os fatores de risco associados com os desfechos e a curva ROC (receiver operating characteristic) examinou a capacidade discriminatória do Índice de Risco Cardíaco Revisado (RCRI) e do Índice de Risco Cardíaco do Grupo de Cirurgia Vascular da New England (VSG-CRI). Resultados: Um total de 141 pacientes (idade média 66 anos, 65% homens) realizou cirurgia de: carótida 15 (10,6%), membros inferiores 65 (46,1%), aorta abdominal 56 (39,7%) e outras (3,5%). Complicações cardiovasculares e óbito ocorreram, respectivamente, em 28 (19,9%) e em 20 (14,2%), em até 30 dias, e em 20 (16,8%) e 10 (8,4%), de 30 dias a um ano. Complicações combinadas ocorreram em 39 (27,7%) pacientes em até 30 dias e em 21 (17,6%) de 30 dias a um ano da cirurgia. Para eventos em até 30 dias, os preditores de risco encontrados foram: idade, obesidade, acidente vascular cerebral, capacidade funcional ruim, cintilografia com hipocaptação transitória, cirurgia aberta, cirurgia de aorta e troponina alterada. Os escores Índice de Risco Cardíaco Revisado (RCRI) e Índice de Risco Cardíaco do Grupo de Estudo Vascular da New England (VSG-CRI) obtiveram AUC (area under curve) de 0,635 e 0,639 para complicações cardiovasculares precoces e 0,562 e 0,610 para óbito em 30 dias, respectivamente. Com base nas variáveis preditoras aqui encontradas, testou-se um novo escore pré-operatório que obteve AUC de 0,747, para complicações cardiovasculares precoces, e um escore intraoperatório que apresentou AUC de 0,840, para óbito em até 30 dias. Para eventos tardios (de 30 dias a 1 ano), os preditores encontrados foram: capacidade funcional ruim, pressão arterial sistólica, cintilografia com hipocaptação transitória, ASA (American Society of Anesthesiologists Physical Status) classe > II, RCRI (AUC 0,726) e troponina alterada. Conclusões: Nesse grupo pequeno e selecionado de pacientes de elevada complexidade clínica, submetidos à cirurgia vascular arterial, a incidência de eventos adversos foi elevada. Para complicações em até 30 dias, mostramos que os índices de avaliação de risco mais utilizados até o momento (RCRI e VSG-CRI) não apresentaram boa performance em nossa amostra. A capacidade preditiva de um escore mais amplo pré-operatório, e uma análise de risco em dois tempos: no pré-operatório e no pós-operatório imediato, como o que simulamos, poderá ser mais efetiva em estimar o risco de complicações / Introduction: Approximately 2.5 million deaths are caused by non-cardiac surgeries per year, while morbidity, represented by functional impairment and a decline in long-term survival, accounts for five times this value.
