Kvalita života pacientů s revmatoidní artritidou / Quality of Life in Rheumatoid Arthritis Patients

The objective of the presented thesis is to explore the impact of rheumatoid arthritis on the patient's life and to compare the subjectively perceived quality of life of such patients with that in healthy population. The set objectives were achieved through a combination of quantitative and qualitative research. The quantitative survey took place as part of the team research project GAJU No. 120/2012/S through the use of the standardized WHOQOL-100 questionnaire handed out to 200 patients suffering from rheumatoid arthritis. In addition to further tests, the statistical data analysis included comparison of the quality of life in the aforesaid patient population with the quality of life in the control healthy population, which also participated in the above stated project. The qualitative survey used the technique of semi-structured interviews with 11 patients suffering from rheumatoid arthritis. The research has shown that the patients with rheumatoid arthritis have a worse quality of life in the fields of physical condition and independence than healthy population. On the other hand, a better quality of life was found in the fields of milieu and spirituality. The results also referred to pain and fatigue as the major factors influencing the quality of life of such patients and drew attention to their interconnection with other dimensions. The qualitative survey supplemented the quantitative data with a practical detailed description of individual problems. Among other things, it detected factors that help the patients to overcome obstacles associated with the disease. For example, the research revealed the family influence or the compensation of restraints by adaptations of the environment or changes of the daily routine. Interesting conclusions have been derived from the information about the effect of adaptation to the disease; it was found out that in the course of their life with the disease, patients get used to certain difficulties, so they perceive them as less severe than in the beginnings of the disease. The thesis provides a complex view of the matters of the quality of life of patients suffering from rheumatoid arthritis and refers to this disease not only as to a disease of the body but also to a disease of the human soul. Among other things, it provides evidence that the concept of the quality of life is applicable in the field of nursing. With regard to the above stated facts, the thesis is useful for the extension of the theoretical knowledge in the field of nursing as well as for the clinical practice.

Kvalita života osob s revmatickými chorobami / Quality of life in rheumatoid arthritis patients

The diploma thesis deals with the quality of life with rheumatic diseases. The thesis focuses on patients suffering from rheumatoid arthritis (RA) in particular, since the rheumatic diseases are the most common chronic inflammatory rheumatoid disease spread worldwide. The aim of the diploma thesis is to explore the physical quality of life with RA of selected patients aged from 19 to 70 years. The theoretical part offers a literature review on the stated topic with focus on the quality of life with RA. The second part offers the results of a quantitative research which was conducted using a questionnaire in a rheumatologist´s office in Saint Anna Faculty Hospital in Brno and in the Hodonin Spa. The HAQ-DI index; GARS questionnaire; a visual-analogical scale rating the intensity of pain and DAS28 score assessing the activity of hospitals were used to achieve the aim. Two research hypotheses were proposed in order to conduct the research: 1. The patients who have been suffering from RA for up to five years will achieve significantly more positive score; 2. The differences between genders in the physical quality of life will be statistically more important. The hypotheses were statistically verified using the IBM SPSSS Statistics 24 software. The results of the thesis suggest, that the quality of life with RA is influenced by pain, which is more intense for women. We also claim that the patients suffering from RA might be less self-reliant, therefore, most of them are dependent on help of others or they need to use various devices. Furthermore, the results show that gender provides statistically crucial differences in the physical quality of life. The outcomes found by this thesis can be used for patients suffering from RA; to raise awareness of the disease and its impact on life.

The JAMAR components are: functional scales and quality of life scales. The scores were analyzed by intraclass correlation coefficient (ICC) for reliability between test-retest. The Median test was used to compare functional and quality of life scales, physician global assessment, pain and overall well-being for children and parents global assessment scales, Juvenile Arthritis Disease Activity Score (JADAS), clinical Juvenile Arthritis Disease Activity Score (cJADAS), Juvenile Arthritis Parent / Child Assessment Index (JAPAI / JACAI) scales. The Kappa test was used to measure the degree of agreement between parents, children and physician about disease activity perception. The Spearman correlation test was used to verify the association between JAPAI / JACAI and cJADAS scores. Results: The Brazilian-Portuguese version of JAMAR was obtained by 3 independent direct translations and 2 reverse translations and a final version by consensus expert committee. The pre-test was applied in ten JIA children of 9.8 to 18.6 years and their parents. Questionnaires from 72 parents and 69 children with JIA, and 72 parents and 62 children healthy were filled out. Partindo-se da hipótese de que cães infectados por Leishmania chagasi apresentam distúrbios locomotores, investigou-se a freqüência, distribuição e caracterização dos achados radiográficos e do líquido sinovial das articulações do cotovelo, cárpica, joelho e társica, de 100 cães com o diagnóstico parasitológico e sorológico positivo para leishmaniose visceral. Radiograficamente observou-se a presença de lesões osteoarticulares em 83% dos cães, tais como, irregularidade do trabeculado ósseo (28,62%), esclerose subcondral (20,77%), osteólise (18,12%), edema (12,93%), colapso articular (12,71%), osteólise e proliferação óssea associada (3,98%), proliferação óssea (2,65%), aumento de opacidade medular e fratura patológica (0,11%) respectivamente, sendo as articulações társicas (75%) e cárpicas (54%) as mais acometidas. A avaliação citológica foi realizada em 603 de 800 articulações estudadas, onde 199 apresentaram formas amastigotas de Leishmania sp., enquanto 405 tinham ausência do parasito e 196 estavam contaminadas com sangue ou com material insuficiente para análise. A maioria das amostras de líquido sinovial apresentou alteração na contagem diferencial das células, sendo, o aumento do número de células mononucleares ativadas e neutrófilos o achado mais comumente encontrado. Os resultados obtidos permitiram concluir que cães com leishmaniose visceral apresentam lesões articulares com distribuição difusa, com maior acometimento das articulações distais e ausência de sinais radiográficos patognomônicos, e que se faz necessária a avaliação do líquido sinovial para auxiliar no diagnóstico diferencial... / Visceral leishmaniasis can be included as a cause of dog inflammatory polyarthritis, however there is few and contraditory informations about osteoarticular lesions associated with this disease. Based from the hypothesis that dogs infected by Leishmania chagasi present locomotor disturbances, the frequency, distribution and characterization of radiographic findings and synovial fluid of elbow, carpus, knee and tarsus articulations from 100 dogs with positive parasitologic and serologic diagnosis for visceral leishmaniasis were studied. The presence of osteoarticular lesions were radiographically observed in 83 per cent of the dogs, and characterized by trabecular bone irregularity (28.62%), subchondral sclerosis (20.77%), osteolysis (18.12%), edema (12.93%), articular collapse (12.71%), association of osteolysis and bone proliferation (3.98%), periosteal proliferative reaction (2.65%), increased medullar opacity and pathologic fracture (0.11%). The most affected articulations were the tarsus (75%) and carpus (54%). The citologic assessment was realized on 603 of 800 studied articulations, whith 199 presenting amastigote forms of Leishmania sp.,while 405 had no parasites and 196 were contaminated with blood or had insufficient sample. The most of synovial fluid samples showed a change differential cell counting alterations, with high number of activated mononuclear cells and neutrophils being the common findings. The results allowed to conclude that dogs affected by visceral leishmaniasis presented lesions joint with diffuse distribution, with more distal articulations involvement and absence of pathognomonic radiographic signs, and that is needed to asses the synovial fluid to assist in differential diagnosis of arthropathy in areas endemic for canine visceral leishmaniasis.

Formas pro-mastigotas de L. chagasi foram inoculadas nas articulações escápuloumeral e fêmoro-tibial de 14 cães. Os animais foram submetidos a avaliações clínicas diárias por 30 dias, bem como análises radiográfica, citológica e físico-química do líqüido sinovial. As alterações clínicas mais encontradas foram: claudicação, dor, restrição de movimentos e aumento de temperatura local. No liquido sinovial, houve aumento gradativo de leucócitos, predominando mononucleares e, posteriormente, polimorfonucleares. Aumento de proteínas, diminuição do pH (de alcalino tendendo a acido), bem como da viscosidade, e qualidade do coagulo ruim foram observados ao longo do estudo. Estatisticamente, a articulação femoro-tibial apresentou maiores alterações quando comparada à escápuloumeral. Em 64% dos animais, observouse a presença do parasita na articulação e não foram encontradas alterações radiográficas ósseas, no período estudado. O confronto dos valores físicos, químicos e achados citológicos do liquido sinovial, permitiu subsidio importante para afirmar que há ocorrência de artrite infecciosa, porém sem caráter proliferativo ou sequer degenerativo, no período estudado. / The aim of this study was to describe the evolution of the locomotor and cytological articular alterations, trying to unveil the mechanism through which the arthritis develops in the experimental leishmaniasis in dogs. Pro-amastigotes forms of L. chagasi were inoculated in the humeral-scapular and the femoral-tibial articulations of 14 dogs. The animals were submitted to daily clinical evaluations for a thirty-day period, as well as cytological and physiochemical analysis of the synovial fluid The most frequent clinical alterations were: limping articular volume increase, pain and local temperature increase. In the synovial fluid, there was a gradual increase of leukocytes, predominating mononuclear cells and, later, polymorphonuclear ones. Protein increase, decrease of the pH (from alkaline towards neutral), as well as viscosity, and coagulation test of bad quality were observed along the study. Statistically, the femoro-tibial articulation presented greater alterations when compared to the humeral-scapular one. In 64% of the animals, the parasite presence was observed in the articulation and no osseous radiographic alterations were found in the period of the study. The comparison of the physical and chemical values and the cytological findings in the synovial fluid supplied important information to state that there are occurrences of septic arthritis, in the initial phase, however without prolipherative or degenerative characteristics, in the period of the study.

Efeito analgésico da morfina e tramadol na artrite e sinovite induzidas experimentalmente em bovinos

O uso de analgésicos opióides, em ruminantes é limitado ou indefinido, como no caso do tramadol. Objetivou-se avaliar os efeitos analgésicos do sulfato de morfina e cloridrato de tramadol em bovinos, submetidos à artrite e sinovite interfalângica distal transitórias. Foram utilizados seis bovinos no protocolo de indução experimental de artrite e sinovite, através da administração intra-articular de anfotericina B, na dose total de 20mg. Cinco horas após a aplicação deste fármaco, os animais foram submetidos a dois tratamentos distintos, sendo o primeiro com morfina, na dose de 0,5 mg/kg, via intramuscular, em duas aplicações com intervalo de seis horas e posteriormente o tramadol, na dose de 1,8 mg/kg, via intramuscular, em dose única, respeitando-se um intervalo de 20 dias entre os tratamentos. Os animais foram avaliados em intervalos de 3 horas, num total de 24 horas, observando-se os parâmetros fisiológicos de frequências cardíaca e respiratória, motilidade ruminal, deambulação e atividade pedométrica. Pela observação clínica e comportamental, baseada na pedometria, se verificou alterações de deambulação e atividade pedométrica para ambos os fármacos. Os parâmetros fisiológicos aparentemente sofreram influência do meio e da manipulação animal, o que inviabilizou sua utilização para análise dos resultados. Por meio das variáveis: número de passos, número de toques ao solo, número de decúbitos e tempo de decúbito observou-se um padrão de inquietação compatível com o de nocicepção podal. Os fármacos não interferiram na claudicação durante o período de máxima estimulação dolorosa, entre 6 e 12 horas após a administração de anfotericina B. Concluiu-se que a morfina e o tramadol, nas doses aqui testadas por via intramuscular, não produziram analgesia satisfatória, frente ao modelo / Diseases that develop podal nociceptive stimulus are frequent in clinical cattle. The use of opioid analgesics in ruminants is limited or unlimited as in the case of tramadol. This study aimed to evaluate the analgesic effects of morphine sulfate and tramadol hydrochloride in cattle undergoing distal interphalangeal arthritis and transient synovitis. Six steers were used in the experimental protocol for induction of arthritis and synovitis, by intra-articular administration of amphotericin B, a total dose of 20 mg. Five hours after application of the drug, the animals were submitted to two different treatments, the first being with morphine at a dose of 0.5 mg/kg intramuscularly in two doses at an interval of six and subsequently tramadol in dose of 1.8 mg/kg, intramuscularly in a single dose, respecting an interval of 20 days between treatments. The animals were evaluated at 3-hour intervals for a total of 24 hours, observing the physiological parameters of heart and respiratory rate, rumen motility, ambulation activity and pedometer. By clinical and behavioral observation, based on pedometrics has revealed changes in walking activity and pedometer for both drugs. Physiological parameters apparently were influenced by the environment and animal handling, which prevented its use for analysis of the results. By means of the variables: number of steps, number of touches the ground, and decubitus number of time decubitus observed a pattern compatible with caring podal nociception. The drugs do not interfere in lameness during the period of maximum pain stimulation between 6 and 12 hours after administration of amphotericin B. A maioria das terapias utilizadas para o seu tratamento é baseada na inibição global da resposta imune, podendo aumentar a susceptibilidade a agentes infecciosos. O objetivo geral deste projeto foi definir estratégias tolerogênicas específicas para profilaxia ou terapia da AR. Para isso, em uma primeira etapa, estabelecemos um modelo de artrite induzida por proteoglicano (PG) bovino. Camundongos BALB/c fêmeas retired breeders imunizados com PG bovino associado ao adjuvante brometo de dimetil dioctadecil amônio apresentaram os sinais clínicos característicos da artrite, como eritema e edema decorrentes da inflamação articular de uma ou mais patas. A análise histopatológica mostrou a presença de hiperplasia sinovial, infiltrado inflamatório (formação de pannus), destruição da cartilagem e erosão óssea. A incidência da doença foi de 100% e os animais artríticos produziram níveis significativos de citocinas pró e anti-inflamatórias e anticorpos IgG1 e IgG2a anti-PG. Em uma segunda etapa, testamos o potencial profilático do PG. A inoculação de três doses de 50 μg de PG determinou um efeito profilático caracterizado pela diminuição significativa da incidência da artrite e do escore clínico dos animais. A diminuição da produção de IFN-g e IL-17, bem como o aumento da produção de IL-4 e IL-10 por células esplênicas estimuladas com PG, podem estar contribuindo para o efeito profilático observado neste grupo. Em uma terceira etapa, avaliamos o potencial terapêutico da associação da vitamina D ativa (VitD3) com o PG. A associação VitD3+PG determinou um efeito terapêutico na artrite experimental caracterizado por diminuição significativa do escore clínico. Most of the therapies used for RA treatment are based on general suppression of immune response, which may increase the susceptibility to infectious agents. The main objective of this work was to establish specific tolerogenic strategies for prophylaxis or therapy of RA. For this purpose, we first established a model of arthritis induced by bovine proteoglycan (PG). Female BALB/c retired breeder mice immunized with bovine PG associated with dimethyl-dioctadecyl ammonium bromide adjuvant developed a typical arthritis characterized by erythema and edema resulting from joint inflammation of one or more paws. Histopathological analysis showed the presence of synovial membrane hyperplasia, inflammatory infiltrates (pannus formation), cartilage destruction and bone erosion. Disease incidence was 100% and the arthritic mice produced significant levels of pro and anti-inflammatory cytokines and IgG1 and IgG2a anti-PG antibodies. Further, we tested the prophylactic potential of PG. Three doses of 50 μg of PG determined a prophylactic effect characterized by a significant decrease in both, arthritis incidence and clinical scores. The decrease in IFN-g and IL-17, as well as the increase in IL-5 and IL-10 production by spleen cells stimulated with PG may be contributing to the prophylactic effect observed in this group. Lastly, we evaluated the therapeutic potential of the combination of active vitamin D (VitD3) with PG. The VitD3+PG association determined a therapeutic effect in experimental arthritis. There was a significant decrease in the clinical scores after VitD3+PG treatment that was confirmed by the histopathological analysis. Most mice paws from the treated group presented well preserved joint structures that were similar to the ones present in healthy animals. Background: Rheumatoid arthritis (RA) is a common chronic autoimmune disorder characterised by persistent synovitis, typically manifested as symmetric polyarthritis of small hand joints with various extra-articular manifestations. Accurate disease activity measurement is a key component of RA management that facilitates therapeutic optimalization in order to slow down the disease progression and to prevent an irreversible joint damage. The aim of this work was to study the role of candidate serum inflammatory markers and their associations with the disease activity in patients with RA presented by traditional variables of disease activity as well as by musculoskeletal ultrasonography. Results: The first part of our work pointed out relationship between serum calprotectin and clinical as well as ultrasound activity in RA. We have revealed that serum calprotectin is an independent predictor of ultrasound synovitis. Moreover, we have demonstrated the potential of calprotectin to identify patients with residual activity in spite of achieving clinical remission. In the second part, we have provided a detailed analysis of 20 candidate serum markers and found out a tight associations between IL-6, IL-7, IL-22, IL-34, YKL-40, CXCL-13, MMP-3, resistin and visfatin with clinical and ultrasound activity....

Velocidade de crescimento e níveis de interleucina-6 na artrite idiopática juvenil / Growth velocity and interleukin-6 levels in juvenile idiopathic arthritis

Objetivos: Avaliar associações da velocidade de crescimento com marcadores inflamatórios e dose cumulativa de glicorticóide em uma coorte de pacientes com Artrite Idiopática Juvenil acompanhados por 1 ano. Material e Métodos: Foram avaliados 79 pacientes com AIJ segundo critérios da ILAR. A atividade clínica da doença foi classificada por médicos reumatologistas pediátricos. Os dados antropométricos foram mensurados e classificados de acordo com as normas da Organização Mundial da Saúde. Foram utilizadas curvas de velocidade de crescimento segundo Tanner; considerou-se baixa velocidade de crescimento valores de escore Z ≤ -2. Concentrações séricas de IL-6 foram mensuradas por ELISA no período basal, e valores acima de 1 pg/ml foram considerados elevados. Resultados: Baixa velocidade de crescimento teve uma prevalência de 25,3% e esteve associada com atividade da doença no período do seguimento (p=0,085), valores elevados de IL-6 (interleucina-6) (p=0,003), velocidade de sedimentação globular (VSG) (p=0,022) e proteína C reativa (PCR) (p=0,001) e maior dosagem cumulativa de glicocorticóide (0=0,044). Na regressão linear múltipla tendo como variável dependente a velocidade de crescimento, observou-se que somente os níveis elevados de IL-6 foram independente e negativamente associados com a velocidade de crescimento (p=0,025). Conclusão: Baixa velocidade de crescimento é altamente prevalente em crianças com AIJ. Níveis elevados de IL-6 têm um importante impacto negativo no crescimento desses pacientes, enquanto a exposição ao glicocorticóide total parece ser um fator secundário. / Objective: To evaluate associations of growth velocity with inflammatory markers and cumulative dose of glucocorticoid in a cohort of patients with Juvenile Idiopathic Arthritis (JIA) followed during 1 year. Methods: Seventy-nine patients were evaluated by criteria according to the ILAR. The disease activity was evaluated by a pediatric rheumatologist. The anthropometic data were measured and classified according to the World Health Organization standards. Growth velocity curves were used according to Tanner, values below the Z-score ≤ -2 were considered low growth velocity. Serum concentrations of IL-6 were measured by ELISA in the baseline period, and values over 1pg/ml were considered as elevated. Results: The prevalence of low growth velocity was 25.3%, and it was associated with: active disease on follow-up visit (p=0,085), elevated interleukin-6 (IL-6) (p=0,003), erythrocyte sedimentation rate (ESR) (p=0,022) and C-reactive protein (CRP) (p=0,001) and higher cumulative glucocorticoid doses (0=0,044). In the multiple linear regression with growth velocity as the dependent variable, only elevated IL-6 levels were independently and negatively associated with growth velocity (p=0,025). Conclusion: Low growth velocity is highly prevalent in children with JIA. Elevated IL-6 levels seem to have an important negative impact on growth in these children, while total glucocorticoid exposure appears to be a secondary factor.

Avaliação da prevalência da obesidade e síndrome metabólica em pacientes com artrite idiopática juvenil

A Artrite idiopática juvenil (AIJ) é a artropatia crônica mais prevalente na infância e adolescência. A prevalência da síndrome metabólica, assim como da obesidade, vem apresentando um rápido aumento, atingindo todas as faixas etárias, incluindo a infância. A síndrome metabólica é caracterizada por um conjunto de riscos para doença cardiovascular e diabetes melito tipo 2, incluindo adiposidade abdominal, resistência à insulina, dislipidemias e hipertensão arterial sistêmica. Além destes componentes, a inflamação tem sido reconhecida cada vez mais como um fator importante na síndrome metabólica e obesidade, e pacientes com doenças caracterizadas por processos inflamatórios crônicos, como a AIJ, poderiam representar grupos de risco especiais. Os glicocorticoides são utilizados rotineiramente no controle da inflamação da artrite idiopática juvenil, em doses elevadas e com uso prolongado. O uso crônico do glicocorticoide pode induzir resistência à insulina, hipertensão arterial sistêmica e obesidade, aumentando o risco de desenvolver síndrome metabólica. O presente trabalho tem como objetivo avaliar a prevalência de obesidade e síndrome metabólica em pacientes com AIJ. Em pacientes acompanhados no Serviço de Reumatologia do Hospital de Clínicas de Porto Alegre (HCPA) e Hospital da Criança Santo Antônio (complexo Santa Casa) foram observados uma prevalência de 19,7% de síndrome metabólica e 22,7% de obesidade, sem diferença entre os subtipos da doença. A obesidade foi associada com tempo de duração da doença, obesidade abdominal, pressão arterial elevada, resistência à insulina e presença de síndrome metabólica. O IMC, circunferência da cintura, triglicerídeos, baixos níveis de HDL-c, pressão arterial sistólica e diastólica, níveis séricos de insulina e resistência a insulina (HOMA-ir) mostraram associação com a SM (p<0,05). Não houve associação entre a presença de SM e dose cumulativa de glicocorticoide, atividade da doença e tempo de duração da doença. Os resultados mostram uma alta frequência de obesidade e síndrome metabólica em pacientes com AIJ, sugerindo um aumento do risco de futuras complicações cardiovasculares. e parecem ser independentes do uso de glicocorticoide. Ações de intervenção são necessárias entre os pacientes com AIJ para reduzir o excesso de peso, evitar as complicações metabólicas e fatores de risco cardiovasculares na vida adulta. / Juvenile idiopathic arthritis is the most prevalent chronic arthropathy in childhood and adolescence. The prevalence of metabolic syndrome, as well as obesity, is increasing fast, in all age groups, including the childhood. Metabolic syndrome is defined as a cluster of risk factors for cardiovascular and type 2 diabetes, including abdominal obesity, insulin resistance, dyslipidaemia and hypertension. Besides these components, inflammation has been increasingly considered as a significant component in metabolic syndrome and obesity, and patients with diseases characterized by the presence of chronic inflammation, such as JIA, could represent special groups of risk. Glucocorticoids are used routinely in the management of the inflammation of JIA, in high doses and long-term. Long-term use of the glucocorticoids can cause insulin resistance, hypertension and obesity, increasing the risk for the metabolic syndrome. The aim of the present study was to evaluate prevalence of the obesity and metabolic syndrome in patients with juvenile idiopathic arthritis (JIA). In patients followed in the Hospital de Clínicas de Porto Alegre (HCPA) and Hospital da Criança Santo Antônio (Santa Casa Complex) service of reumatology were observed a prevalence of 19.7% of metabolic syndrome and 22.7% of obese, without difference between the subtypes of the disease. Obesity was associated with disease duration, abdominal obesity, arterial hypertension, insulin resistance and presence of metabolic syndrome. BMI, waist circunference, triglycerides, low HDL-c level, systolic and diastolic BP, fasting insulin serum levels and insulin resistance (HOMA-ir) showed significant association with MetS (p<0,05). There was no correlation between the presence of metabolic syndrome and cumulative glucocorticoid dose, disease activity and duration of disease. The results showed that there were high frequencies of obesity and metabolic syndrome in JIA patients and use appears to be indeoendent of the use glucocorticoid. Intervation actions are needed among JIA patients, to decrease excess weight, metabolic complications and cardiovascular risk factors in adulthood.
