Estudo experimental comparativo de remendos arteriais de polidimetilsiloxano com reforço de tecido de poliéster (PDMSr) versus politetrafluoretileno expandido (PTFEe) em aorta de coelhos / Patch of polydimethylsiloxane reinforced with polyester fabric for aortic angioplasty in rabbits

Introdução: Apesar de bons resultados descritos na literatura, o substituto ideal para a utilização como remendo no fechamento arterial ainda não existe. Por este motivo, ainda há espaço para a busca por remendo que seja biocompatível, e que apresente facilidade de manuseio e resultados satisfatórios. Avaliamos remendos arteriais de silicone (polidimetilsiloxano com reforço em poliéster - PDMSr) em comparação com remendos arteriais de PTFEe. Objetivo: O objetivo deste trabalho é comparar, em modelo experimental, em coelhos, os resultados de remendos arteriais feitos em PDMSr com remendos de PTFEe. Materiais e Método: A amostra foi definida em 10 animais que completassem todas as etapas da pesquisa em cada grupo. Os animais foram submetidos à laparotomia mediana e abertura longitudinal da aorta de aproximadamente 8mm, realizando-se o seu fechamento com remendo do grupo selecionado, seguido do fechamento por planos. Os animais foram mantidos em biotério até o 60º PO, quando, então, realizou-se arteriografia de controle, e análise macro e microscópica de peça. Resultados: Para se atingir a amostra desejada, foram necessários 12 procedimentos no grupo PDMSr e 16 no grupo PTFEe. Ocorreram 2 óbitos no grupo PDMSr e 6 no grupo PTFEe. Apesar do número maior de óbitos no grupo PTFEe não houve diferença estatística na sobrevida entre os grupos. Um animal do grupo PDMSr apresentou monoparesia em pata posterior direita e um animal do grupo PTFEe apresentou hérnia incisional. Não houve diferença estatística nas complicações entre os grupos. O tempo operatório foi estatisticamente maior no grupo PTFEe quando comparamos todos os animais, fato que não se repetiu quando excluímos os animais que faleceram. O peso e o tempo operatório foram estatisticamente maiores nos animais que morreram. Todos os animais que chegaram ao final do tempo de estudo apresentavam aorta patente. Na análise macroscópica da peça, houve, estatisticamente, maior reação tecidual periprótese no grupo PTFEe. A microscopia eletrônica de varredura evidenciou cobertura de todo o remendo de PDMSr por tecido similar ao endotélio, enquanto, nos remendos de PTFEe, o crescimento limitou-se às bordas da linha de sutura e ilhas isoladas no seu centro. Conclusão: O material estudado apresentou resultados comparáveis ao do PTFEe, porém com menor reação tecidual local e maior proliferação celular para a luz do vaso. Apesar de novos estudos serem necessários, inclusive para avaliação de uso em humanos, o presente estudo apresenta resultados promissores que encorajam a continuidade de sua pesquisa / Introduction: Although good results are reported for various materials for use as patches for arterial closure, as yet none of these is ideal. Therefore, research is continuing into development of a patch that is biocompatible and provides ease of handling, while having satisfactory outcomes. A new silicone arterial patch (polydimethylsiloxane reinforced with polyester fabric, PDMSr) was compared with patches made of expanded polytetrafluoroethylene (ePTFE). Objective: To compare the outcomes between arterial patches made of PDMSr with those made of ePTFE, in an experimental rabbit model. Materials and Method: Rabbits were placed in two groups, and received either PDMSr or ePTFE arterial patches (PDMSr group and ePTFE group, respectively). The animals underwent laparotomy and longitudinal opening of the aorta, which was then closed with the selected patch, followed by suture of all layers. The animals were kept in their cages until the 60th postoperative day, when arteriography, removal of the aorta, and macroscopic and optic and scanning electron microscopic analyzes of the aorta were performed. Ten rabbits from each group that had completed all stages of the research were included in analyses. Results: Twelve procedures were performed in the PDMSr group and 16 in the ePTFE group. There were 2 deaths in the PDMSr group and 6 in the ePTFE group. Despite the higher number of deaths in the ePTFE group, there was no statistical difference in survival rate between the groups. One animal in the PDMSr group developed monoparesis in its right hind paw and 1 animal in the ePTFE group had an incisional hernia. There was no statistical difference in complications between the groups. The operative time was significantly longer in the ePTFE group when all animals were included in analysis, but not when animals that died were excluded. Body weight was significantly greater and operative time was significantly longer in animals that died. Postsurgical aortic patency in the survival animals was 100% in both groups by arteriography. Macroscopically, tissue reaction around the prosthesis was greater in the ePTFE group (statistically significant). Microscopically, the PDMSr patches were entirely covered with a cellular endothelium-like tissue, while tissue growth on the ePTFE patches was limited to the edges of the suture line and to isolated central islands. Conclusion: The two materials showed comparable outcomes; however, PDMSr showed cellular proliferation to the entire graft, and less local inflammatory reaction compared with ePTFE. Although further studies are required, including assessment in humans, the results of the present study indicate that PDMSr shows promise as an arterial patch material

Prevalência do transtorno dismórfico corporal em pacientes dermatológicos e avaliação da crítica sobre os sintomas nessa população / Prevalence of body dysmorphic disorder among dermatological patients and level of insight about their symptoms

São cada vez mais freqüentes as queixas cosméticas na sociedade contemporânea objetivando a perfeição das formas do corpo e da pele. Os dermatologistas e cirurgiões plásticos são frequentemente consultados para avaliar e tratar essas queixas. Sendo assim é importante conhecer o Transtorno Dismórfico Corporal, inicialmente chamado de dismorfofobia que foi pouco estudado até recentemente. Esse transtorno é relativamente comum, por vezes incapacitante, e envolve uma percepção distorcida da imagem corporal caracterizada pela preocupação exagerada com um defeito imaginário na aparência ou com um mínimo defeito corporal presente. A maioria dos pacientes apresenta algum grau de prejuízo no funcionamento social e ocupacional e como resultado de suas queixas obsessivas com a aparência podem desenvolver comportamentos compulsivos, em casos mais graves há risco de suicídio. O nível de juízo crítico é prejudicado, não reconhecem que seu defeito é mínimo ou inexistente e freqüentemente procuram tratamentos cosméticos para um transtorno psíquico. A prevalência do transtorno na população geral é de 1 a 2% e em pacientes dermatológicos e de cirurgia cosmética de 2,9 a 16%. Neste estudo investigou-se a prevalência do Transtorno Dismórfico Corporal em pacientes dermatológicos. Entrevistadores treinados avaliaram com questionários e entrevistas semi-estruturadas (SCID) pacientes que procuravam tratamentos cosméticos clínicos e cirúrgicos (grupo Cosmiatria, n=150), que procuravam a dermatologia em geral (grupo Geral, n=150) e grupo controle de 50 pacientes. Três psiquiatras independentes fizeram a melhor estimativa diagnóstica (best estimate diagnosis). Foram diagnosticados 32 pacientes (Cosmiatria 14%; Geral 6,7%; Controle 2%). As diferenças entre as prevalências nos três grupos foram significativas, bem como entre o grupo da Cosmiatria e o Controle. A regressão logística mostrou maior prevalência na Cosmiatria do que no grupo Geral e Controle, em indivíduos solteiros e com menor índice de massa corpórea. A gravidade foi moderada (em escala validada) e as obsessões foram mais significativas no grupo da Cosmiatria do que no Geral. A aplicação de escala de avaliação de crenças mostrou que o nível de juízo crítico estava mais prejudicado nos pacientes do grupo da Cosmiatria. Nenhum paciente havia sido diagnosticado previamente. As comorbidades psiquiátricas foram freqüentes, principalmente o Transtorno Depressivo Maior e o Transtorno Obsessivo Compulsivo. A comparação de subgrupos de pacientes que tinham o Transtorno Dismórfico Corporal ou este associado ao Transtorno Obsessivo-Compulsivo não mostrou diferenças significativas quanto às variáveis demográficas, comorbidades psiquiátricas, gravidade do transtorno ou nível de juízo crítico. As queixas dermatológicas mais freqüentes, em média duas, foram: discromias, acne, quanto à forma do corpo e ao envelhecimento. No grupo Cosmiatria a maioria dos pacientes já havia se submetido a tratamentos ou cirurgias cosméticas com resultados insatisfatórios. Os achados desse estudo apontam para uma maior prevalência em pacientes dermatológicos, principalmente nos que procuram tratamentos cosméticos, sugerindo que possam ser mais obsessivos e ter pior nível de juízo crítico em relação aos seus sintomas. Considerando a alta prevalência do Transtorno Dismórfico Corporal em pacientes dermatológicos e que os tratamentos cosméticos raramente melhoram seus sintomas, o treinamento dos profissionais para a investigação sistemática, diagnóstico e encaminhamento para tratamento psiquiátrico parece fundamental / Cosmetic concerns are increasingly pervading the contemporary societies, focusing on body shape and the skin perfection. Dermatologists and plastic surgeons are frequently consulted to evaluate and treat these concerns. Therefore it is important to be aware of the Body Dysmorphic Disorder, also known as dysmorphophobia, little studied until recently. This disorder is relatively common sometimes causing impairment involving a distorted perception of the body image characterized by an excessive preoccupation with an imagined or minimal appearance defect. Most of the patients experience some degree of impairment in social or occupational functioning and as a result, their obsessive thoughts, may lead to repetitive behaviors and in severe cases, to attempted suicide. Most individuals have poor insight and they do not acknowledge the defect is minimal or inexistent and seek out cosmetic treatments for a psychiatric disorder. The prevalence of this disorder among general population ranges from 1 to 2 % and in dermatological and cosmetic surgery patients ranges from 2, 9 to 16%. This study assessed the prevalence of Body Dysmorphic Disorder in dermatological patients. Trained interviewers used questionnaires (BDDQ) and semi-structured clinical interviews (SCID) to access patients seeking clinical or surgical cosmetic treatments (Cosmetic group, n=150), seeking dermatology in general (General group, n=150) and a control group of 50 subjects. Three independent psychiatrists assigned the best estimate diagnosis and 32 patients were diagnosed with the disorder (Cosmetic 14%, General 6, 7% and Control 2%). Prevalence differences in the three groups were significant, also for Cosmetic and Control groups. In the logistic regression a higher prevalence has been noticed in the Cosmetic group (when compared with General and Control groups), in patients that are not currently married and with less body mass index. The severity of the symptoms assessed by a validated scale (BDD-YBOCS) was moderate and the obsessions were significantly higher in the Cosmetic group as compared to the General one. Beliefs were assessed by a scale (BABS) and insight were significantly poorer in the Cosmetic group. None of the patients had been previously diagnosed. Psychiatric co-morbidities were frequent, mostly Major Depressive Disorder and Obsessive-Compulsive Disorder. Comparing two patients subgroups with just Body Dysmorphic Disorder and patients with Obsessive-Compulsive Disorder as comorbidity show no differences according to demographic variables, Axis I psychiatric comorbidity, symptoms severity and level of insight. The most frequent dermatological concerns were dyschromias, acne, the shape of the body and ageing. In the Cosmetical group most of the patients have performed clinical or surgical cosmetic treatments with poor results. Our findings provide further support for higher prevalence of Body Dysmorphic Disorder in dermatological patients, mainly those seeking cosmetic treatments and suggests that those patients might be more obsessive and having poorer insight into their symptoms. Taking into account the high prevalence of Body Dysmorphic Disorder in dermatological patients and cosmetic treatments that rarely improve symptoms there are compelling reasons for training professionals to systematically investigate, diagnose and refer these patients to adequate psychiatric treatment

Em um estudo prospectivo sobre o tratamento das artroplastias de quadril infectadas, com perdas ósseas e fístulas ativas, 25 pacientes foram tratados em dois tempos e 36 pacientes foram tratados em dois tempos com espaçador de cimento impregnado com vancomicina. O acompanhamento médio foi de dois anos e onze meses. A taxa de recidiva infecciosa foi de 29,2% nos tratados em dois tempos e de 8,8% nos tratados com espaçador. O Escore de Harris para Quadril médio passou de 19,3 para 69,0 pontos nos casos tratados em dois tempos e de 19,7 para 72,2 pontos nos pacientes tratados com espaçador. Ao final do estudo, 86,1% dos tratados com espaçador e em 33,3% dos tratados em dois tempos tinham próteses em bom funcionamento e sem infecção. O espaçador de cimento com antibiótico é o tratamento de escolha nas próteses infectadas de quadril / We report a prospective study of 61 patients with chronically deep infected hip replacements with actively discharging sinuses, treated with a two-stage revision protocol, with and without a cement spacer impregnated with vancomycin. The average follow-up was two years and eleven months. Twenty-five patients were treated without a spacer and seven had recurrence of infection. Thirty-three patients were treated with a spacer and three had recurrence of infection. The average Harris Hip score increased from 19,3 to 69,0 on the non-spacer patients and from 19,7 to 75,2 on the spacer group. At the end of the study, the success rate was 86,1% for the spacer group and 33,3% for the non-spacer group. The use of the spacer increased the results of the two-stage chronic infected hip replacements

Dans un contexte qualifié d’ « économie du savoir », où la croissance de l’économie mondiale repose de plus en plus sur les activités intensives en connaissance, la thèse s’interroge sur la capacité de deux petits États insulaires en développement – Maurice et Trinidad – à s’adapter pour s'intégrer à cette dynamique. Ces anciennes colonies britanniques, membres du Commonwealth depuis leur accession à l'indépendance dans les années soixante, font état d’indicateurs de développement en constante progression et sont souvent considérées comme des modèles de réussite économique. La thèse étudie la mise en place des institutions dans ces pays et cherche à évaluer dans quelle mesure l’héritage de la période coloniale, à travers la trace qu'ils en conservent dans la période contemporaine, peut faciliter la transition de leurs économies vers les secteurs intensifs en savoir. Pour ce faire, elle retrace leur trajectoire de développement institutionnel et économique des origines jusqu’à nos jours et elle s’intéresse notamment aux mesures prises aujourd’hui pour doter le pays du cadre nécessaire à l’économie de la connaissance, en particulier à travers le rôle de l’enseignement supérieur en tant qu’outil fondamental pour former le capital humain. Tout en reconnaissant que certains des atouts dont disposent les deux pays pour s’engager dans ce processus avec succès peuvent être mis en relation avec leur passé colonial, la thèse approfondit l’hypothèse que leur transformation en économies de la connaissance risque aussi d'être entravée par des caractéristiques héritées de cette période et qui, des décennies après l'accession à l'indépendance, freinent une large diffusion de la connaissance au sein de leur population. / At a time when economic growth is thought to be driven primarily by knowledge, the purpose of this research is to analyse to what extent two Small Island Developing States – Mauritius and Trinidad – are adequately equipped for this new economic context. As former British colonies, they both joined the Commonwealth upon gaining independence in the nineteen-sixties and they have since largely succeeded in overcoming their initially unfavourable conditions. Today, their good political, economic and social development is widely acknowledged. The main focus of this research is to evaluate how far colonial legacy, which is still visible in the contemporary institutions of both these states, can favour the transition they wish to undertake towards knowledge-intensive activities as drivers of economic growth. For this purpose, the research analyses the development trajectory followed by their institutions and their economy spanning the whole period of their colonial and post-independence history; additionally, this research considers the steps which are undertaken nowadays to build the framework required to foster a knowledge economy by studying more specifically the role of higher education as an essential tool for human capital formation. While upholding that some of the assets which could help them achieve their objectives can be linked to their colonial past, the research contends that the transformation of these two countries into knowledge-based economies could be hindered by the legacy of colonisation on some of their current features and which, decades after gaining independence, impedes the widespread diffusion of knowledge amongst their population.

Los consejos de las artes y el principio de "arm's length" en las políticas culturales subnacionales: Un estudio comparativo entre Cataluña (España), Escocia (Reino Unido) y Jalisco (México)

Se realiza una investigación comparada de los consejos de las artes de tres regiones subnacionales de países distintos: Cataluña (España), Escocia (Reino Unido) y Jalisco (México) para investigar la aplicación del principio de “arm’s length”, su ejecución, su relación con el contexto cultural y político, así como su inclusión en sistemas institucionales distintos al anglosajón. Con una metodología cualitativa y el uso de estudios de caso, se llevan a cabo trabajos de campo en cada lugar mediante entrevistas, observaciones y trabajo documental, aplicando el canon conjunto de concordancias y diferencias para su análisis. Se concluye que un consejo de las artes no es necesariamente un organismo bajo el principio de “arm’s length” y se reflexiona sobre la influencia que los actores involucrados buscan ejercer a través de sus diversos recursos. La relación entre los consejos y sus contextos muestra las dialécticas entre legitimidad/eficiencia, transparencia/clientelismo y diferenciación/estandarización y tensiones entre centro/periferia. Desde la política se utiliza este principio como argumento retórico, mientras se pretende tener mayor control sobre las decisiones, aunque esto aumente su nivel de responsabilidad. Desde el sector cultural se ignora que el principio busca también mantener su distancia con él para evitar clientelismos. Se evidencia un mimetismo que copia modelos ajenos sin adaptarlos suficientemente al contexto y sin profundizar en experiencias externas ni con reflexiones teóricas, lo cual deja al descubierto algunos errores del proceso. / Titol: Els consells de les arts i el principi d’“arm’s length” en les politiques culturals subnacionals: un estudi comparatiu entre Catalunya (Espanya), Escòcia (Regne Unit) i Jalisco (Mèxic). S'evidencia un mimetisme que copia models aliens sense adaptar-los suficientment al context i sense aprofundir en experiències externes ni amb reflexions teòriques, la qual cosa deixa al descobert alguns dels errors del procés. / Title: The arts councils and the arm’s length principle in cultural policy at a subnational level: A comparative study between Catalonia (Spain), Jalisco (Mexico) and Scotland (UK). Abstract: Comparative research on the arts councils of three sub-national regions from different countries, Catalonia (Spain), Jalisco (Mexico) and Scotland (United Kingdom), is carried out to examine the application of the arm's length principle, its performance, its relation to the cultural and politic context and its inclusion in institutional systems other than the Anglo-Saxon system. With a qualitative methodology and case study strategy, fieldwork is performed in each place through interviews, observations and documentary work, applying the joint canon of agreement and difference for analysis. The conclusion reached is that an arts council is not necessarily a body under the arm's length principle and the influence that stakeholders seek to exert through their various resources is reflected upon. The relationship between arts councils and their context shows the dialectics between legitimacy/efficiency, transparency/clientelism and differentiation/standardisation, and the tensions between the centre/periphery. This principle is used from politics as a rhetorical argument, while the aim is to have more control over decisions, even though this increases the level of accountability. The cultural sector is unaware that the principle also seeks to keep its distance from it to avoid clientelism. There is an evident mimicry, copying foreign models without adapting them sufficiently to the context and without delving more deeply into external experiences and theoretical reflections, which reveals some mistakes of the process.

Mathematics teacher–students attitude towards information and communication technology across three countries / D.J. Laubscher

Low achievement in Mathematics is a problem that faces schools, colleges and universities, not only in South Africa, but worldwide. Many efforts have been made to explore different ways in which teaching and learning can be more effective. ICT provides endless possibili–ties to enhance the teaching and learning of Mathematics. The purpose of this study is to determine teacher–students’ attitude towards ICTs in order to plan for effective, efficient and appropriate methods of Mathematics education. It further aims to determine to what extent these students will be willing to use ICTs in their own studies as well as in the teaching pro–cess in their classrooms, and aims to compare the students’ attitudes across three countries i.e. South Africa, Tanzania and Finland. The population consisted of Mathematics education teacher–students enrolled at the following institutions: School for Continuing Teacher Educa–tion (SCTE) at the North–West University in South Africa (317 students), the Iringa University College, University of Tumaini in Tanzania (111 students), and the University of Joensuu in Finland (56 students). This study followed a quantitative cross–sectional survey design com–prising a single mode research questionnaire to three groups of students in three countries. The instrument that was used was based on the Loyd and Gressard Computer Attitude Sur–vey (1984) (CAS), with a few additional questions to add to the relevance of this study. Questionnaires were completed by each group of students. The questionnaires were posted to each Mathematics teacher–student at the SCTE with a return envelope included. The re–searcher visited Tanzania and administered the questionnaire to the students at their univer–sity and some students in Finland completed the survey online while others completed a pa–per–based copy, which was returned to the researcher. Descriptive statistical techniques, reliability and validity of the instrument scale, inferential statistics (ANOVA), and cross–tabulations were used, and where appropriate, effect sizes were calculated. Findings indi–cated that the teacher–students have a positive attitude to computers and that they are willing to use ICTs in their own studies and in their teaching at school. The comparison of their atti–tudes revealed that the South African and Tanzanian teacher–students have a more positive attitude towards ICTs than the Finnish students. The teacher–students in the two African countries were also more willing to use ICTs than the students in Finland, despite their in–creased exposure and access to technology. To create an environment of effective Mathe–matics education, a number of factors play an important role. Mathematics education sup–ported by good pedagogical content knowledge can be transformed into effective Mathemat–ics education with the aid of a positive computer attitude and willingness to use technology. It further aims to determine to what extent these students will be willing to use ICTs in their own studies as well as in the teaching pro–cess in their classrooms, and aims to compare the students’ attitudes across three countries i.e. South Africa, Tanzania and Finland. The population consisted of Mathematics education teacher–students enrolled at the following institutions: School for Continuing Teacher Educa–tion (SCTE) at the North–West University in South Africa (317 students), the Iringa University College, University of Tumaini in Tanzania (111 students), and the University of Joensuu in Finland (56 students). This study followed a quantitative cross–sectional survey design com–prising a single mode research questionnaire to three groups of students in three countries. The instrument that was used was based on the Loyd and Gressard Computer Attitude Sur–vey (1984) (CAS), with a few additional questions to add to the relevance of this study. Questionnaires were completed by each group of students. The questionnaires were posted to each Mathematics teacher–student at the SCTE with a return envelope included. The re–searcher visited Tanzania and administered the questionnaire to the students at their univer–sity and some students in Finland completed the survey online while others completed a pa–per–based copy, which was returned to the researcher. Descriptive statistical techniques, reliability and validity of the instrument scale, inferential statistics (ANOVA), and cross–tabulations were used, and where appropriate, effect sizes were calculated. Findings indi–cated that the teacher–students have a positive attitude to computers and that they are willing to use ICTs in their own studies and in their teaching at school. The comparison of their atti–tudes revealed that the South African and Tanzanian teacher–students have a more positive attitude towards ICTs than the Finnish students. The teacher–students in the two African countries were also more willing to use ICTs than the students in Finland, despite their in–creased exposure and access to technology. To create an environment of effective Mathe–matics education, a number of factors play an important role. Mathematics education sup–ported by good pedagogical content knowledge can be transformed into effective Mathemat–ics education with the aid of a positive computer attitude and willingness to use technology. What Role of God and National Curriculum in School life? : A Comparative Study of Schools with a Muslim Profile in England and Sweden

The purpose of this study is to gain a better understanding of principles and ethics that dominate four schools with a Muslim profile, two in Sweden and two in England. The specific objectives of the study are:  to examine educational policies with regard to primary schools with a confessional orientation in Sweden and England; to compare two primary schools with a Muslim profile in Sweden with two such schools in England; and in these four schools to describe and examine the manner in which school heads, teachers and other staff deal with the encounters between the values found in the national curriculum of Sweden and England respectively and the principles and ethics embodied in their private philosophy of life; to describe and examine the views of school heads, teachers and other staff on school leadership and any educational, ideological or personal role model they emulate; to describe and examine the expectations and views of parents with regard to the school with a Muslim profile; and describe the views of the pupils regarding their schools and the norms and values in school and; finally, to examine the attitudes of some local authority politicians in Sweden to MP schools. The findings indicate great difference between the two schools with a Muslim profile in Sweden, on the one hand, and the two schools in England, on the other. The fundamental reason for that lies in the parameters which had been established in these countries as the conditions for being permitted to establish and run a school with a confessional orientation. Since the schools in both countries had conformed to the relevant legislation and framework in their respective countries with regard to such schools, they had therefore consequently developed in different directions. / Partly financed by Vetenskapsrådet (Swedish Research Council)
