Acute coronary syndrome: bridging the gap. / CUHK electronic theses & dissertations collection

Acute coronary syndrome (ACS), a term used to cover a group of clinical symptoms compatible with acute myocardial ischemia, represents a high-risk group of patients with coronary heart disease (CHD). To improve quality of care, international guidelines for the management of ACS have been established and are updated regularly. In the era of evidence based medicine, adherence to therapeutic guidelines is essential for optimal care of ACS patients. However, most data on ACS epidemiology, treatment and outcomes are derived from western population. There are limited data in Chinese population in terms of prevalence, presentation, response to treatment and clinical outcome. / Among 624 patients finished Short Form (SF)-36 questionnaires, health related quality of life (HRQoL) were compared between patients underwent PCI versus those treated conservatively across 3 age groups (<60, 60-79 and ≥80 years). PCI was performed in 73.6%,55.7% and 21.3% in patients aged <60,60-79 and older than 80 years, respectively (p<0.01). Elderly patients were more likely to be female (16.9 vs. 35.4 vs. 54.6%, p<0.01) and had more co-morbidities (p<0.01). Older patients were less likely to undergo angiography (84.8 vs. 65.2 vs. 24.8%, p<0.01). Baseline HRQoL decreased with advancing age (p<0.01). However, elderly patients who underwent PCI-experienced the most improvement in physical health than younger age groups. PCI was an independent predictor (OR, 1.79,95% CI: 1.10-2.92) of better physical health status at 6 months. In conclusion, elderly ACS patients who underwent PCI experienced the most improvement in physical health compared to younger patients. Our findings suggest that age per se should not deter against revascularization because of potential benefits in HRQOL. / In summary, this is the first registry which described patients' characteristics, treatment and management practices, and hospital outcomes over the whole spectrum of ACS in Hong Kong. The study identified gaps between guideline and clinical practice as well as the reasons of these gaps, and measured the impact of such gaps on the outcomes of patients with ACS. Compared with internationally reported data, Hong Kong patients are different in terms of age and risk factors distribution. Treatment gaps exist between international therapeutic guideline recommendations and clinical practice, especially among the high risk population, the elderly and female patients. Better understanding and narrowing these gaps between guideline and practice will lead to improvement in quality of care and clinical outcomes. Increase use ofrisk stratification models and health status assessments may improve decision making in the management of ACS. / Patients with ACS were divided into low- and high-predicted risk of mortality at 6 months using the GRACE risk score (≥142.5 was defined as high-risk). We evaluated the use of in-hospital angiography, revascularization, anti-platelet, angiotensin converting enzyme inhibitors (ACEI) or angiotensin receptor blockers (ARB), beta-blockers and statins therapy between high and low-risk patients. There were 259 patients in the high- and 742 in the low-risk groups. Paradoxically, high-risk compared to low-risk patients were less likely to underwent coronary angiography and/or revascularization during the index hospitalization (33% vs. 64% and 25% vs. 50%, both p<0.01). Hospital initiated pharmacotherapies are also lower in high-risk patients (24% vs. 55% for c1opidogrel, 49% vs. 58% for ACEI/ARBs, 54% vs. 69% for beta-blockers and 56% vs. 77% for statins; all p<0.01). After adjustment, high-risk patients remained less likely to undergo revascularization (adjusted odds ratio [OR], 0.47; 95% CI, 0.33-0.73, p<0.001) than low-risk patients. Advanced age, increased creatinine level and higher GRACE score were independent predictors for failure to administer evidence-based therapies. Thus, patients with ACS at high risk of mortality were paradoxically less likely to undergo revascularization or receive medications according to guidelines. Better adherence to evidence-based therapies in high-risk patients may improve clinical outcome and quality of health care. / The Hong Kong ACS registry was designed to investigate epidemiology, treatment and outcome of ACS patients under current medical care system, it was conducted in a university affiliated teaching hospital from February 2006 to December 2009. Clinical characteristics and treatment data were collected at baseline, 30 days and 6 months after onset in a standard defined case report form. SF-36 questionnaire was completed after admission and at 6 months. Outcomes were evaluated mortality and morbidity in clinical aspect and quality of life in aspect of health status. / The Main findings were as followed: Totally 1001 patients admitted with ACS were recruited. Among all patients enrolled, 31.7% were diagnosed with ST-segment elevation myocardial infarction, 42.7% with non-S'T-segrnent myocardial infarction and 21.6% with unstable angina. The median age was 72 (interquartile range 61-79) years; 77.2% were >60 years old, and 31.5% were women. / Women presented more often with NSTE-ACS than men (77.3% of women vs. 63.2% of men, p<0.001). Despite having greater cornorbidities including hypertension, diabetes, hypercholesterolemia, renal impairment and history of heart failure etc., women were observed to have higher GRACE (global registry of acute coronaryevents) score than men (128+/-32 vs. 118+/-37, p score than men (128+/-32 vs. 118+/-37, p<0.01). Women were less likely to be assigned invasive procedures (43.3% vs. 62.9%, p<0.001) as well as pharmacotherapies such as clopidogrel (41.1% vs. 58.8%, p<0.001), glycoprotein (GP) IIb/IIIa antagonists (5.3% vs. 11.6%, p=0.001) and statins (64.1% vs. 77.2%, p<0.01) et al. than men. For in-hospital mortality, the adjusted odds ratio for men compared to women was similar (odds ratio [OR]: 1.32, 95% CI: 0.62-2.83, p=0.47). The higher 6 month mortality and major cardiac events rate in women were not significant after adjusting for differences in clinical characteristics and percutaneous coronary intervention (PCI) (OR=1.02; 95% CI 0.62 to 1.68; p=0.95). In summary, there were differences in baseline characteristics and in the management of women and men admitted for ACS. Advanced age and high comorbidities prevalence could explain most of the difference between genders suggesting that decision making bias in clinical practice is anti-age but not anti-female. Impacto da raça e ancestralidade na apresentação e evolução da doença de Crohn no Brasil / The impact of race and ancestry in the presentation and progression of Crohn\'s disease in Brazil

INTRODUÇÃO: A doença de Crohn (DC) é uma doença inflamatória intestinal (DII) crônica de etiologia desconhecida, apesar de fatores genéticos e ambientais estarem envolvidos. Os fatores étnicos relacionados com a DC são muito controversos. Parece haver uma maior incidência em brancos, porém alguns estudos demonstram aumento da prevalência entre negros nos últimos 30 anos e sugerem diferenças na apresentação clínica. O Brasil apresenta uma população miscigenada, com importante imigração européia e africana. Temos dois objetivos principais: 1) caracterizar os pacientes com DC em um centro de referência brasileiro e 2) correlacionar as características fenotípicas dos nossos pacientes com a raça/cor e ancestralidade. MÉTODOS: Foram incluídos 273 pacientes acompanhados no Ambulatório de Intestino do HC-FMUSP com o diagnóstico de DC previamente estabelecido. Dados demográficos, o curso e as características clínicas da doença, manifestações extra-intestinais, uso de medicamentos e escores de gravidade foram registrados a partir dos prontuários e de entrevistas com os pacientes. RESULTADOS: A maioria dos pacientes era do sexo feminino (164 pacientes, 60%). A média da idade dos pacientes foi 40,3 anos + 13,9, sendo que a média da idade de início dos sintomas foi 28,4 anos + 12,7. A raça/cor predominante foi branca (68,8%), seguida por parda (21,5%), preta (7,8%), amarela (1,5%) e indígena (0,4%). História familiar de DII estava presente em 36 pacientes (13,2%). Cento e trinta e três pacientes referiram história de tabagismo (48,7%). Doença fistulizante foi registrada em 45,2% dos pacientes e 67% dos pacientes apresentaram algum tipo de manifestação extra-intestinal. A localização predominante da doença conforme a classificação de Viena foi íleo-colônica em 39,5% dos pacientes. A distribuição de sexo por raça/cor foi semelhante em todas as raças. Não foi encontrada diferença estatisticamente significativa entre as diferentes raças. Analisando apenas brancos e negros (pretos + pardos), verificamos que foi semelhante a distribuição de sexo (60% de sexo feminino entre os brancos e 59% entre os negros; p=0,98), a presença de história familiar de doença inflamatória intestinal (16,2% entre os brancos e 7,6% entre os negros, p=0,08), a presença de doença fistulizante (43,5% dos brancos versus 48,7% dos negros; p=0,49), a presença de manifestações extra-intestinais (67% dos negros versus 66,9% dos brancos; p=1,0) e uso de corticosteróides (65,7% dos negros versus 73,9% dos brancos; p=0,21). Os brancos apresentaram doença restrita ao íleo terminal em 42,4% versus 23,4% nos negros (p=0,009). Não houve diferença na proporção de indivíduos necessitando cirurgia: 46,2% dos negros foram submetidos a cirurgia comparado com 53,2% dos brancos (p=0,34). Ao analisarmos a ancestralidade e não raça/cor declarada, também não encontramos nenhuma associação com idade do início dos sintomas, doença fistulizante, manifestações extra-intestinais, uso de imunossupressores, uso de corticosteróides ou cirurgia prévia. CONCLUSÕES: No Brasil, não encontramos diferenças na apresentação clínica ou gravidade da DC de acordo com raça/cor ou com a ancestralidade. / INTRODUCTION: Crohns disease (CD) is increasingly recognized in diverse ethnic populations. Ethnic factors related do CD are highly controversial. Apparently, there seems to be a higher incidence among white subjects. However, studies conducted in USA have shown an increase in prevalence among black subjects in the last 30 years. Overall, there is lack of information concerning the clinical phenotype (disease presentation and progression) so as to race, and literature remains controversial. Brazil has a multiracial society, with important European immigration and also Africans. AIM: to analyze the influence of race and/or ancestry on clinical presentation and progression in a multiracial society living in the same environment. METHODS: Two hundred and seventy three patients with CD seen in gastroenterology clinic of Hospital das Clínicas between June 2005 and July 2006 were analyzed to determine whether there were significant differences among racial/ancestry groups. Race was self-declared according to the classification used by IBGE (Brazilian Demographical Census classifies people according to race or skin color, which is declared by the person himself/herself according to the fallowing options: white, black, brown/mixed, yellow or indian). Ancestry (also declared by the patient) was described as European, African, mixture of African and European, Indian, Asian or another/unknown. The definition of disease location was according to the Vienna classification. The severity of disease was measured by the use of steroids, the use of immunomodulators (azathioprine or methotrexate) and the need of surgeries related to the disease. The evaluation of extra-intestinal manifestations consisted of 3 main categories: ophthalmological, dermatological and articular. Statistical analyses were performed by SPSS statistic software. RESULTS: Whites comprised 68.8%, blacks 7.8%, brown/mixed 21.5% and yellow 1.5%. One patient reported itself as American Indian. Comparing the whites and the African Americans (blacks and brown/mixed), we found no significant difference with respect to family history of inflammatory bowel disease (16.2% vs 7.6%; p=0.08), mean age at the onset of symptoms (27+12 vs 30+12; p=0.07), steroids use (73.9% vs 65.7%; p=0.21), immunomodulators use (74.3% vs 74%; p=0.96), surgery need (53.2% vs 46.2; p=0.34), perforating disease (43.5% vs 48.7%; p=0.49), ocular manifestations (8.8% vs 9.0%; p=0.96), skin manifestations (15.5 vs 15.8%; p=0.94), articular (64.5% vs 63.3% p=0.89). Smoking habits, a possible confounding factor, was similar in the two groups. The whites have more frequent disease location at the ileum (42.4% vs 23.4%; p= 0.009). Analyzing the ancestry, there were 36.8% European, 3.8% mixture of African and European, 9.2% African, 7.9% Indian, 1.3 Asian and 41% Brazilian/other/unknown. If we analyze the European ancestry compared to African, there were no significant difference. CONCLUSIONS: There were no significant differences among the races (as defined by skin color) in most of the studied variables. There were difference only in respect to disease location, as described previously by others. It seems that in a multiracial society as Brazil, race, in respect of skin color or ancestry, may have some influence, but it is not an important factor defining presentation or progression of CD.

The effects of a childbirth psychoeducation programme on learned resourcefulness, maternal role competence and satisfaction, and depressive symptoms in Chinese childbearing women. / CUHK electronic theses & dissertations collection

A convenience sample of 184 first-time childbearing women was recruited from two public hospitals with one hospital randomly selected as the experimental group. The experimental group (n = 92) received the childbirth psychoeducation programme and routine childbirth education. The comparison group (n = 92) received the routine childbirth education only. / Outcomes on learned resourcefulness, maternal role competence and perinatal depression were measured by C-SCS, C-PSOC (Efficacy Subscale) and Edinburgh Postnatal Depression Scale (EPDS), respectively, at baseline, immediately post-intervention, six weeks and six months postpartum. Maternal role satisfaction was assessed at six weeks and six months postpartum using C-PSOC (Satisfaction Subscale). Doubly multivariate analysis of covariance was performed to compare the effects of childbirth psychoeducation programme between the experimental and comparison groups. In addition, 16 participants in the experimental group were interviewed at six weeks postpartum to explore their perceived impacts of childbirth psychoeducation programme in helping them cope with the experience of new motherhood. Content analysis was used to analyze the interview data. / Results of the phase I study indicated good psychometric properties of C-SCS and C-PSOC in Chinese childbearing women. Results in the phase II study revealed significant improvement in learned resourcefulness at six weeks postpartum (p = 0.004), and overall reduction in depressive symptoms (p = 0.01) for women receiving the childbirth psychoeducation programme compared with the routine childbirth education group after adjusting for baseline group differences on age and social support. No significant change was detected on maternal role competence. However, women receiving the childbirth psychoeducation programl1'l;e had significantly higher level of satisfaction in the maternal role at six weeks postpartum (p = 0.01). / The crisis nature of early motherhood, the frequent feeling of incompetence in the maternal role, the increasing evidence of postpartum depression in the Chinese population, coupled with the changing nature of socio-cultural environment challenge midwives to make continued refinement of childbirth education to enhance women's adjustment during the transition to motherhood. Learned resourcefulness has been identified as an important coping repertoire that promotes healthy adjustment in the perinatal period. The aim of this study was to determine the effectiveness of a childbirth psychoeducation programme based on the concept of learned resourcefulness. / The qualitative interviews revealed that the experimental group perceived the childbirth psychoeducation programme to be helpful in increasing their confidence in the maternal role, improving their emotional well-being and fostering the development of learned resourcefulness skills. The findings of this study support the effectiveness of childbirth psychoeducation programme based on the concept of learned resourcefulness for reducing depressive symptoms in first-time Chinese childbearing women, and highlight the contributions midwives can make to continue improving the quality of childbirth education in Chinese society. / The study had two phases. The first phase aimed at establishing the psychometric properties of the Chinese versions of Self-Control Schedule (C-SCS) and Parenting Sense of Competence Scale (C-PSOC), which were used as outcome measures in the second phase. Cardiovascular and chronic kidney disease in Chinese type 2 diabetic patients: from prognosis to management. / CUHK electronic theses & dissertations collection

Conclusions. The growing epidemic of type 2 diabetes and its cardiorenal complications place a major burden on our health care system. Diabetic kidney disease is of particular importance in Asian populations including Chinese. In this series of studies, using a large prospective cohort established since 1995, I confirmed the powerful predictive value of albuminuria on cardio-renal complications. Inhibition of the RAAS interacted with both modifiable and genetic factors, notably the ACE I/D polymorphism, on the development of cardio-renal complications. In addition, it was found that CKD predicts CVD independent of albuminuria. Based on two prospective studies, I confirmed the effectiveness of global risk-factor control using structured care protocol to prevent these devastating complications. (Abstract shortened by UMI.) / I then examined the possible independent and interactive effects of CKD and albuminuria on cardio-renal outcomes in the original cohort of 5,004 patients. Glomerular filtration rate was estimated (eGFR) by the Modification of Diet in Renal Disease equation. The frequency of CKD as defined by eGFR <60ml/min/1.73m 2 was 15.8% in the cohort at baseline, when 6% of patients had serum creatinine ≥150mumol/L. / In collaboration with colleagues, I have conducted a series of studies to examine the prognostic factors for cardio-renal complications in Chinese type 2 diabetic patients. The modulating effects of RAAS inhibition and the effectiveness of rnuitidisciplinary care to prevent ESRD are also examined. / Research Hypotheses. (1) Albuminuria is a prognostic factor on cardio-renal outcomes in type 2 diabetes patients; (2) Chronic Kidney Disease is associated with other metabolic risk factors and phenotypes and is a prognostic factor on cardio-renal outcomes in type 2 diabetes patients; (3) Angiotensin-converting-enzyme insertion/deletion polymorphism is a prognostic factor on cardio-renal outcomes in type 2 diabetes patients, and has an effect on treatment responses with RAAS blockage with ACE inhibitors; (4) Structured care models by risk stratification using various prognostic factors and adherence to care protocol can improve cardio-renal outcome in type 2 diabetes patients. / Results. In a prospective cohort of 5,004 patients, I examined the effect of albuminuria and ACE inhibition on survival and cardio-renal outcomes in 3,773 patients who had been observed for at least 6 months with a mean follow up period of 35.8 months. / Taken together, measurement of serum creatinine alone without GFR estimation may underestimate the frequency of CKD in Chinese type 2 diabetic patients. O camarão-rosa Farfantepenaeus subtilis (Perez-Farfante, 1967) e uma espécie de importância econômica, capturado pela pesca industrial na Plataforma Continental do Amazonas. Este estudo teve como objetivo avaliar padrões espaços-temporais da abundancia relativa desse recurso a partir de uma serie de capturas realizadas por barcos da frota industrial, especializadas com o uso de ferramentas de sistema de informações geográficas. A abundancia relativa de camarão-rosa (CPUE) foi relacionada a batimetria, as características do substrato, a vazão do rio Amazonas e as variáveis oceanográficas obtidas por sensoriamento: temperatura da superfície do mar e concentração de clorofila-a. Entre as categorias de tipo de substrato, observou-se maior intensidade de arrastos na região de lama mosqueada. Nessa região, características como tipo de substrato (lama), relevo submarino, taxa de sedimentação (<1 cm.ano-1), e salinidade (>30) constituem o habitat ideal para o camarão-rosa. Maiores valores de CPUE estiveram associados a menores temperaturas e a maiores valores de concentração de clorofila-a, características que ocorrem no período de maior vazão do rio Amazonas, no primeiro semestre. Foi observada a ocorrência de três períodos com diferentes níveis de produção: fevereiro a abril (maior vazão), com maior abundancia relativa de camarão-rosa; maio a julho; e agosto a setembro (menor vazão), com menor abundancia. Os resultados mostraram que a abundancia relativa de F. subtilis não se distribui de modo uniforme no espaço nem na variação sazonal. / Knowledge of spatial distribution of fishing resources is essential to fisheries management. The southern brown shrimp Farfantepenaeus subtilis (Perez-Farfante, 1967) is an industrial fishing fleet target on the Amazon Continental Shelf. This study aimed to evaluate F. subtilis relative abundance spatial-temporal patterns from commercial fishery data, using Geographic Information System tools. Catch per unit effort (CPUE), used as index of relative abundance, was related to bathymetry, substrate characteristics, Amazon river flow and the oceanographic variables obtained by remote sensing: sea surface temperature and chlorophyll-a concentration. Analyzing substrate characteristics, the commercial shrimp trawls were concentrated at the mottled mud region, where the overlap of environmental factors was crucial for a higher incidence of F. subtilis. In this region, features such as substrate (mud), sedimentation rate (<1 cm.yr-1) and salinity (> 30) are the ideal habitat for F. subtilis. Higher CPUE values were associated with lower temperatures and higher values of chlorophyll-a concentration, which occur at the greatest flow of the Amazon river, during the first six months of the year. It was observed the occurrence of three periods with different levels of production along the year: from February to April, with higher CPUE of southern brown shrimp, from May to July, and August to September, with lower CPUE. The results showed that relative abundance of F. subtilis is not distributed uniformly in space nor in seasonal variation.

São reconhecidos, tradicionalmente, para o grupo seis táxons: Icterus chtysocephalus, I. cayanensis cayanensís. I. cayanensis tibialis, I. cayanensis tibialis, I. cayanensis valenciobuenoi, I. cayanensis periporphyrus e I. cayanensis pyrrhopterus, que se substituem geograficamente ao longo de grande parte da América do Sul. Neste estudo foi feita a descrição dos padrões de variação geográfica. Foram diagnosticadas quatro espécies, à luz do conceito filogenético de espécie: Icterus cayanensis (Amazônia Meridional), Icterus chrysocephalus (Amazônia Setentrional), Icterus tibialis (Caatinga) e Icterus pyrrhopterus (Chaco); os táxons I. cayanensis valenciobuenoi e I. cayanensis periporphyrus foram sinonimizados. Entre as formas amazonicas (chrysocephalus e cayanensis) foi detectada a presença de uma zona híbrida mais extensa do que aquela reportada na literatura. No Brasil Central foi diagnosticada a maior zona de intergradação conhecida para aves, com aproximadamente 2.300 km de extensão, produto do intercruzamento entre Icterus tibialis e Icterus pyrrhopterus, formas distribuídas pela Caatinga e Chaco, respectivamente. Postula-se que as zonas de intergradação diagnosticadas neste estudo são produto do intercruzamento de populações previamente diferenciadas em isolamento geográfico. / The Icterus cayanensis-chrysocephalus species complex shows an intricate pattern of geographic variation in body measurements and plumage. Traditionally, six taxa have been recognized in this group: Icterus chrysocephalus, I. cayanensis cayanensis, I. cayanensis tibialis, I. cayanensis valenciobuenoi, I. cayanensis periporphyrus and I. cayanensis pynhopterus, which replace each other geographically in South America. In this work, the geographic pattern of plumage and size variation was described for this species complex, and inferences were made about the evolutionary processes behind those pattern. Under the phylogenetic species concept (PSC), four species are diagnosed; Icterus cayanensis (Southern Amazon), I. chrysocephalus (Northern Amazon), I. tibialis (Caatinga) and I. pyrrhopterus (Chaco). The taxa lcterus cayenansis vaienciobuenoi lhering (1902) and Icterus cayanensis periporphyrus (Bonaparte 1950) were synonymizied. A hybrid zone more extensive than hitherto reported in the literature was detected between two Amazonian taxa: I. chrysocephalus and I. cayanensis. In Central Brazil, it was recognized one of the widest zones of intergradations known for birds to date, approximately 2.300 km wide, between I. tibialis and I. pyrrhopterus. R is postulated here that those hybrid zones were formed by secondary contact, after a period of differentiation in geographic isolation.

Contudo, há poucos estudos feitos em ambiente natural na Amazônia brasileira envolvendo aspectos ecológicos e/ou comportamentais da espécie e praticamente nenhum sobre suas associações com outras espécies. Neste trabalho foram estudados os padrões gerais da ecologia e do comportamento de dois grupos de S. sciureus e suas associações com outras espécies de primatas no Mosaico de Unidades de Conservação da Usina Hidrelétrica de Tucuruí. Os sítios de estudo foram a Ilha de Germoplasma (IG) e a Zona de Preservação da Vida Silvestre Base 4 (B4). Os dados foram coletados pelos métodos de varredura instantânea e ad libitum por seis meses entre março e outubro de 2009. A área de uso dos grupos correspondeu a aproximadamente 75 ha na B4 e 77,5 ha na IG. No uso do espaço vertical, houve preferência pelos estratos inferiores e médios. Além disso, houve um marcado padrão no uso dos estratos ao longo o dia, com maior frequência de uso dos estratos mais altos nas duas primeiras horas de atividades, dos estratos mais baixos das 10 às 14 horas e dos estratos intermediários no final do dia. Os comportamentos de forrageio (50% IG; 49% B4) e locomoção (29% ambos) foram mais frequentes que alimentação (12% IG; 15% B4), interação social (6% IG; 4% B4) e descanso (3% para ambos), concordando com outros estudos na Amazônia. A dieta foi predominantemente frugívora (75% B4, 71% IG), diferindo de uma série de estudos que caracterizaram todo o gênero como altamente insetívoro. As espécies vegetais mais importantes foram Attalea maripa no período chuvoso e Inga spp. no período seco, para ambos os grupos. A frequência de associação foi 100% do tempo (B4) e 49% (IG) com Cebus apella, 20% (B4) com Chiropotes satanas e 3% (IG) com Chiropotes utahicki. Houve encontro com Alouatta belzebul e Saguinus niger nos dois sítios, com Aotus azarae na B4 e Callicebus moloch na IG. O grupo da IG passou mais tempo em associação durante a estação chuvosa. O tempo em associação com C. satanas foi maior no período seco, sem diferença sazonal para C. utahicki e C. apella. Houve diferença entre S. sciureus, C. apella e C. satanas no uso do espaço vertical, no tipo de suporte e nos itens alimentares explorados. Os macacos-de-cheiro apresentaram nicho maior que os macacos-prego para uso de espaço vertical e itens alimentares, e os macacos-prego apresentaram nicho maior para tipo de suporte. A maior sobreposição de nichos nas três dimensões medidas foi entre C. apella e S. sciureus.

Os objetivos deste trabalho foram: (1) revisar a validade e a diagnosabilidade dos táxons atualmente agrupados em L. albolineatus com base em caracteres morfológicos, vocais e moleculares e (2) reavaliar os limites interespecíficos entre estes táxons. Foram mensurados 150 espécimes depositados em 8 museus do Brasil e EUA. Para a análise molecular, foram seqüenciados um total de 940 pb do gene mitocondrial ND2 para 35 indivíduos de todos os táxons de L. albolineatus. As análises filogenéticas foram realizadas nos programa PAUP 4.0 b 10 e MrBayes 3.1 utilizando-se os métodos de parcimônia (MP), máxima verossimilhança (MV) e inferência Bayesiana. A combinação de dados morfológicos e moleculares revelou a existência de 5 clados fortemente apoiados estatisticamente: clado 1 (agrupando indivíduos da AE Rondônia), clado 2 (agrupando espécimes das AE Belém, Xingu e Tapajós), clado 3 (incluindo espécimes da AE Inambari), clado 4 (incluindo indivíduos da AE Imeri) e clado 5 (agrupando indivíduos da AE Guiana). Todos os clados corresponderam a táxons já nomeados, exceto o clado 3 para o qual nenhum nome válido se encontra disponível, já que o nome fuscicapillus na verdade se aplica ao clado 1 e, portanto, deve ser considerado sinônimo sênior de madeirae. A principal separação genética e morfológica em L. albolineatus acontece entre o táxon nominal e os demais, embora cada um dos 5 clados possa ser considerado uma espécie distinta (com base no Conceito Filético Geral de Espécie) através de uma combinação única de caracteres morfológicos, vocais e moleculares diagnósticos. / Lepidocolaptes albolineatus (Aves: Dendrocolaptidae) is a polytypic biological species including the following taxa: L. a. albolineatus occurring in the Guiana Area of Endemism (hereafter AE), L. a. duidae (Imeri AE), L. a. fuscicapillus and L. a. madeirae (both in the Rondônia AE), and L. a. layardi (Tapajós, Xingu, and Belém AEs). The main goals of the present study were: (1) review the validity and diagnoses of the taxa grouped under L. albolineatus based on a combination of morphological, vocal, and molecular characters, and (2) re-evaluate inter-specific limits between among those taxa. A total of 150 specimens deposited in Brazilian and North American collections were measured. In the molecular analysis a total of 940 bp of the mitochondrial gene ND2 belonging to 35 individuals representing all taxa of L. albolineatus was sequenced. PAUP 4.0 b 10 and MrBayes 3.1 were used to generate phylogenetic trees under parsimony, maximum likelihood, and Bayesian approaches. Morphological and molecular data strongly supported the existence of five natural populations (clades) within L. albolineatus: clade 1 (grouping populations from the Rondônia AE), clade 2 (including populations of the Belém, Xingu, and Tapajós AEs), clade 3 (including specimens from the Inambari AE), clade 4 (grouping specimens from the Imeri AE), and clade 5 (including specimens of the Guiana AE). All clades corresponded to already named taxa, except clade 3, which has no valid name yet since the name fuscicapillus is in fact applicable to clade 1, and therefore must be considered the senior synonymous of madeirae. The main genetic and morphological separation in L. albolineatus occurs between the nominate taxon (clade 5) and all remaining taxa and clades, although each clade recognized in the analysis can be regarded as a separate species under the General Lineage Species Concept, since they are all mutually diagnosed based on a combination of morphological, vocal, and genetic characters.

Orbital Remote Sensing appears as an alternative tool to monitoring important variables such as sea surface temperature (SST) and suspended sediment concentration (SSC). This study aims at assessing spatial distribution of SST and SSC in the continental shelf area of the Brazilian State of Espírito Santo using, respectively, Band 2 (0.569&#956;m) and Band 6 (11.435&#956;m) data from Thematic Mapper (TM) sensor on board of satellite Landsat 5. Thus, a data bank was built out of 127 images pre-processed and registered by the TM sensor. Of these 127 images, only 4 were selected for SST and SSC analyses. During image pre-processing, geometric and reflectance corrections are made. Geometric correction is carried out based on a second image orthorectified so that there is correct georeferencing. Reflectance correction is performed using an atmospheric compensation model. Through this model, sea surface reflectance is obtained. SST is estimated based on the adjustment of Brightness Temperature (BT) derived from Band 6 with data from the Multi-scale Ultra-high Resolution Sea Surface Temperature (MURSST). In case of not enough sediment field data to carry out curve adjustment, SSC calculation is performed using the empirical logarithm algorithm proposed by Tassan (1987), which uses Band 2 data. The corrected reflectance spectrum is compared to TOA reflectance spectrum. The corrected spectrum complied with the literature on natural targets analyzed: cloud, vegetation, water and water with sediments. Landsat BT adjustment with SST of MURSST showed to be significant, according to statistical F-test and to r2 = 0.87. Results of the 4 scenes for SST and SSC fields. For winter SST, a more homogeneous temperature of ~23ºC is observed, whereas the summer scene is characterized by thermal contrasts between the area closer to the coast (~21ºC) and the area further from the coast (~26ºC). Further, hotspot plumes associated to the mouths of Jucu and Doce Rivers and Espírito Santo Bay are also verified. For the SSC fields, higher-concentration plumes are observed, which are advected southeast. The highest concentrations are associated to the Doce River (over 60 mg/L). SSC values comply with the values reported in the relevant literature, i.e. close to the coastal line, in the summer scene, values of ~15mg/L were verified. On the other hand, for autumn and winter scenes, these values are about ~35mg/L. Both atmospheric correction and SSC distribution values found in this study are according to the data and studies reported in the relevant literature. Therefore, we can conclude that the method employed showed to be functional to process the data gathered in this study / O monitoramento da qualidade da água nas regiões internas à Plataforma Continental (PC) apresenta-se dispendioso quando são usados métodos convencionais. O Sensoriamento Remoto Orbital apresenta-se como uma ferramenta alternativa ao monitoramento de variáveis importantes como a Temperatura da Superfície do Mar (TSM) e a Concentração de Sedimentos em Suspensão (CSS). Este trabalho tem como objetivo avaliar a distribuição espacial da TSM e da CSS na região da PC capixaba usando, respectivamente, os dados da Banda 2 (0,569&#956;m) e da Banda 6 (11,435&#956;m) do sensor Thematic Mapper (TM) abordo do satélite Landsat 5. Sendo assim, é construído um banco de dados a partir de 127 imagens pré-processadas e registradas pelo sensor TM, e destas imagens, 4 foram selecionadas para a análise dos campos de TSM e CSS. No pré-processamento é realizada a correção geométrica e a correção da reflectância. A correção geométrica é realizada tomando como referência uma segunda imagem ortoretificada para que haja um correto georreferenciamento. A correção da reflectância é feita utilizando um modelo de compensação atmosférica, e através desse modelo é obtida a reflectância superficial da água do mar. A TSM é estimada a partir do ajuste da Temperatura de Brilho (TB) derivada da Banda 6 com os dados do produto Multi-scale Ultra-high Resolution Sea Surface Temperature (MURSST). Na falta de dados de campo de sedimento para realizar o ajuste de curva, o cálculo da CSS é feito utilizando o algoritmo logarítmico empírico proposto por Tassan (1987) que usa os dados da Banda 2. O espectro da reflectância corrigida é comparado com o espectro da reflectância TOA. O espectro corrigido mostrou-se condizente com o da literatura para os alvos naturais analisados: nuvem, vegetação, água e água com sedimento. O ajuste da TB do Landsat com a TSM do MURSST apresentou-se significante, segundo a Estatística teste F e com um r2 = 0,87. Os resultados das 4 cenas para os campos de TSM e CSS. Para a TSM de inverno observa-se uma distribuição de temperatura mais homogênea de ~23ºC, enquanto a cena de verão é caracterizada por contrates térmicos entre a região junto da costa (~21ºC) e a região mais afastada (~26ºC). Além disso, verificam-se plumas térmicas associadas às desembocaduras de rios, Jucu e Doce, e à Baía do Espírito Santo. Para os campos de CSS são observadas plumas de maior concentração sendo advectadas para sudoeste, sendo as maiores concentrações associadas ao Rio Doce, acima de 60 mg/L. Os valores de CSS apresentam-se em concordância com os valores reportados na literatura, de forma que, próximo à linha de costa, na cena de verão, são observados valores de~15mg/L. Já para cenas de outono e inverno os valores são entorno de ~35mg/L. Ambos os resultados, de correção atmosférica e de distribuição da CSS, apresentam-se em conformidade com os dados e trabalhos da literatura e portanto, concluiu-se que o método utilizado se mostrou funcional para o processamento dos dados deste trabalho
