O acesso às órteses, próteses e meios auxiliares de locomoção no Sistema Único de Saúde de uma microrregião de saúde do município de São Paulo / The access to orthosis, prosthesis and other mobility aids in the Unified Health System of a micro region of health of São Paulo city

O presente estudo objetivou caracterizar e analisar o acesso às Órteses, Próteses e Meios Auxiliares de Locomoção (OPM) no Sistema Único de Saúde de uma microrregião de saúde do município de São Paulo - SP. Trata-se de um estudo de abordagem quantitativa e qualitativa, acontecendo em duas etapas. Uma primeira de desenho metodológico transversal, retrospectivo e exploratório com dados de prontuários de usuários que tiveram OPM prescrita para um Centro Especializado em Reabilitação (CER) no período de janeiro de 2015 a dezembro de 2017. Uma segunda de abordagem qualitativa por meio de grupos de discussão com profissionais e gestores de serviços da atenção básica e especializada do território para a constituição de um Fluxograma Analisador para a concessão de OPM e por meio de entrevistas não diretivas com usuários de OPM indicados por profissionais da atenção básica para a constituição de Itinerários Terapêuticos. Com distribuição não normal dos quantitativos, foi utilizado o teste Kruskal-Wallis para análise da diferença entre as medianas, sendo considerado estatisticamente significativo um p <= 0,05. Técnicas de Fluxograma Analisador e de Análise de Conteúdo foram utilizadas para análises dos dados qualitativos. Na análise de 729 prontuários, houve uma predominância da filantropia no processo para concessão de OPM, sendo que 51,9% dos dispositivos foram dispensados pela instituição filantrópica conveniada. Porém, isto vem diminuindo, sendo que em 2017, 74,9% dos dispositivos solicitados foram dispensados pelo próprio CER. Não se observou um incremento no número de dispositivos dispensados no período, mas redução do tempo de dispensação com significância estatística e o CER levou em média 4,3 para dispensar dispositivos. Observa-se um fluxo pactuado entre os profissionais e serviços para concessão de OPM com algumas lacunas apontadas por profissionais e usuários na comunicação entre CER e atenção básica sobre recebimento do dispositivo pelos usuários e a respeito dos fluxos de reparos e manutenção. Conclui-se que há ainda predominância da filantropia no processo para fornecimento de OPM. Observou-se redução do tempo de dispensação de dispositivos, mas não acompanhado de aumento no acesso a eles no período e região estudados. Nota-se também um fluxo pactuado entre profissionais e serviços, porém com ausência de um fluxo de reparo e manutenção de OPM no território / The present study aimed to characterize and to analyze the access to Orthosis, Prosthesis and other Mobility Aids (OPM) in the Unified Health System of a micro region of health of São Paulo city. It is a study with quantitative and qualitative approach, which happened in two steps. The first cross-sectional, retrospective and exploratory with data of medical records of OPM users who had OPM prescribed to a Specialized Center in Rehabilitation (CER) in the period from January 2015 until December 2017. A second of qualitative approach by means of discussion groups with professionals and managers of primary care and secondary specialized care services of territory to the constitution of Analyzer Flowchart to the OPM grant and by means of non-directive interviews with OPM users who were indicated by primary care professionals to the constitution of Therapeutic Itineraries. With a non-normal distribution of quantitative data, the Kruskal-Wallis test to analyze of difference between the medians was used, being considered statistically significant p <= 0.05. Techniques of Analyzer Flowchart and Content Analyze were used to qualitative data analysis. In the analyze of 729 medical records, there was predominance of philanthropy in the process to OPM grant, being that 51.9% of devices were dispensed by agreed philanthropic institution. However, this has decreasing, being that in 2017, 74.9% of requested were dispensed by CER itself. It was not observed an increase in the number of dispensed devices in the period, but decrease of dispensation time with statistical significance, and the CER took in average 4.3 months to dispense devices. It is observed a flow agreed between the professionals and users to OPM grant with some gaps pointed by professional and users in the communication between CER and primary care about receipt of device by users and with respect the flows of repair and maintenance. It is conclude that there was decrease in the dispensation time of devices, but not accompanied of increase in the access to them in the period and region studied. It is also noted a flow agreed between professional and services, but with no a flow of repair and maintenance of OPM in the territory

How support workers enable, regulate or constrain the sexual expression of intellectually disabled people who live in service agency group homes is the subject of this thesis. A general literature search of what intellectually disabled people currently experience in their lives, including their experiences in the area of sexuality and intimacy, begins this investigation. Secondly, an extensive literature review of the support role, incorporating an appraisal of past and current issues related to the support position in general and to the area of sexuality support in particular, was completed. What intellectually disabled people themselves would like in relation to sexuality and intimacy support was included in this section. Thirdly, a review of research studies focussing on the operation of the support position within service agency systems was undertaken. These explorations revealed a high degree of reluctance on the part of workers to provide assistance in the sexuality area, despite a proven necessity for support to be made available to the intellectually disabled people they worked with. Review research studies suggested a variety of causal factors in explanation of this reluctance. These suggestions link to two meta-reason positions. Failure to prove support either stemmed from individual worker’s inactions due to ignorance and/or incompetence, or from wider systemic failures on the part of agency services to positively value and support this key service role in this area. However, little if any analysis of the possible influence of the broader social, emotional and cultural contexts, in which the concepts ‘sexuality’ and ‘(intellectual) disability’ are located, could be found in the studies reviewed. Eleven in-depth interviews were conducted with front-line support workers about their sexuality support practice. Preliminary readings of the interview texts revealed a similar reluctance on the part of the workers concerned to assist those they worked with in this area. Interview texts were then subjected to a post-modernist inspired, interpretive discursive analysis. This analysis uncovered and tracked how key power/knowledge effects inherent in the terms ‘(intellectual) disability’, ‘sexuality’, ‘gender’ and ‘desire’ inhering in the concept of an ‘ideal (sexual) couple’ interweave to shape the ‘no support necessary’ practice responses held in worker’s interview talk. From this exploration it is suggested that research studies of workers’ practices as an aspect of the promotion of change in support outcomes in the sexuality support area need to go beyond the parameters of recommendations that stem from considerations of either individual or systemic limitation alone. It remains a convincing point to suggest that poorly performing workers need retraining in this area and the overall value of the support role within service organizations needs reshaping. However, future research recommendations also need to engage more directly and effectively with the effects of the wider social and emotional “ideal (sexual) couple” ambiguities that also influence worker’s lack of assistance in this complex and sensitive support area. The use of a post-modern perspective as a helpful conceptual tool in unpacking the power these ambiguities hold within the support position is offered as a productive way forward for future research and practice development.

Darbo tyrimų objektas – neįgalių moksleivių, konfliktų sprendimo strategijos, sportinės veiklos motyvacija ir sav��s vertinimas. Tyrimo mokslinė problema - atskleisti skirtingą negalę turinčių asmenų konfliktų sprendimo strategijų, sportinės veiklos motyvų pasirinkimą ir savęs vertinimo lygį. Tyrimo tikslas – šiame darbe tiriame skirtingą negalią turinčių jaunuolių (silpnaregių, aklųjų, fiziškai neįgaliųjų ir su cerebriniu paralyžiumi) gebėjimo spręsti konfliktus, motyvaciją dalyvauti sportinėje veikloje ir jų savęs vertinimo ypatumus. Mūsų darbo uždaviniai: 1. Nustatyti skirtingą negalę turinčių jaunuolių, konflikto sprendimo strategijų, sportinės veiklos motyvų ir savęs vertinimo lygio raiškos ypatumus. 2. O material empírico foi coletado através de entrevistas semi-estruturadas realizadas no NEFEA (Núcleo de Atividade Física e Esporte Adaptado) da UEFS (Universidade Estadual de Feira de Santana), o qual desenvolve atividades esportivas junto às pessoas deficientes. O grupo participante do estudo foi atletas do núcleo, todos dependentes de alguma prótese para realizar as atividades cotidianas e no esporte, são homens, com idades variando entre 23 e 37 anos, sendo que têm em comum o fato de terem se tornado deficientes após um acidente ou doença. O estudo caracterizado como uma análise qualitativa teve a pesquisa de campo desenvolvida no período de julho de 2005 no NEFEA, onde foram feitas todas as entrevistas. Consistiu em problematizar os efeitos que as tecnologias provocam na vida cotidiana através do esporte destes indivíduos pelas transformações ocorridas em seus corpos, o estranhamento e a naturalização do “novo” corpo fabricado pelas tecnologias, um ser protético. Contou como base teórica os Estudos Culturais assim como autores que discutem o corpo sob concepções pós-modernas. A dissertação representa, por sua vez, as reflexões construídas destes indivíduos em constantes questionamentos com a hegemonia cultural, na qual este corpo protético e atleta, lembrado como híbrido, ao final foi deslumbrado como algo produzido e ressignificado constantemente, assim como inovável, moldável e provisório. / Salvador

Na etnografia que apresento, o objetivo é discutir o papel que os cavalos manifestam no interior desta terapêutica que, conforme definido pela Ande Brasil (2010), se propõe ao “desenvolvimento biopsicossocial de pessoas com deficiência e/ou necessidades especiais”. Foco minha análise nas sessões de atendimento; nestas, os praticantes (termo nativo, usado em referência às pessoas que fazem equoterapia e que, em sua maioria, são também chamadas de especiais), seus familiares, terapeutas e auxiliares-guia conectam-se aos cavalos e entre si de modos distintos. Seguindo os atores em seus modos relacionais de comunicação e ação, o corpo e suas disposições corporais emergem como o eixo comum para negociarem certos tipos de contato, comando, disciplina e controle. Neste cenário, pretendo examinar de que maneira as relações entre humanos e cavalos, de um lado, e as relações entre pessoas consideradas com e sem deficiência, de outro lado, aparecem juntas, e como este encontro permite repensar as noções de “humano” e “animal” a partir de seus impactos mútuos. Espera-se que os tópicos etnográficos aqui delineados possam, quiçá, contribuir para a temática das socialidades transespecíficas. / From an anthropological perspective, this research examines the relationships between people and horses in the “equine therapy”, a health treatment held, among other venues, at an Equine Center in São Carlos city, São Paulo (Brazil). In this ethnographic account, the goal is to discuss the role that horses play within this therapeutic which, according to Ande Brasil (2010), is intended to help with the “biopsychosocial development of people with disability issues and/or special needs”. I focus on the therapeutic sessions; in these, the praticantes (the native term, used in reference to the people addressed by this therapy, and who are, in majority, called specials), their parents, therapists, and auxiliary-guides connect to themselves and to the horses in distinct ways. By following these actors in their relational ways of communicating and acting, the body and its embodied dispositions emerge as the common axis at negotiating certain types of contact, command, discipline, and control. In this set, I examine the way in which the relationships between humans and animals, from one side, and the relationships between people with and without disability, from another side, when joined, might shake the notions of “human” and “animal” in their mutual impacts. It is expected that the ethnographic themes presented here may contribute to the transpecies socialities debate.

Pessoas com deficiência no mercado de trabalho: implicações da Lei de cotas

In order to ensure this right it was created compensatory benefits, the affirmative action, which under the right to work, provided a reservation of vacancies for people with disabilities in the formal labor market. This research assessed the implications of this legislation to create job opportunities for disabled people and whether those opportunities are guided by the paradigm of inclusion, encouraging the recruitment and retention of people with different levels and categories of disability in the workplace. Data were collected through semi-structured interviews with ten officials of the sector of Human Resources and twelve employees with disabilities. The interviews were record, transcribed in full and qualitatively analyzed by the analysis of content procedure. The results demonstrated that companies hire people with disabilities according to the requirement imposed by law and, especially, as a result of surveillance conducted by the Public Ministry of Labor and justify the difficulty in hiring these people by the lack of education, professional qualification and the need to adequacy of the company to receive them. In practice, these needs come before the requirements of qualification and the pre-selection of the categories of disabilities acceptable by the company, make the people using a wheelchair the most excluded. Among the participating companies, only three reported having conducted some sort of adjustment, given the need for the person hired by restricting the subsequent hiring people with the same type of disability or with disabilities that do not involve in new adaptations. For people with disabilities the law resulted in increased opportunities, but not in recognition of their potential for work, which favors its hiring, but does not create effective conditions for the development of their work. It was conclude that the Law of quotas fulfills its role in generating employment opportunities, but needs to be evaluated, since it does not allow the employment of disabled people in their various categories and levels of commitment and does not encourage the adequacy of these companies to receive people. In this sense, it points up the need for articulate efforts of different actors and social resources such as special education institutions, municipal programs aimed at drawing up actions that address the needs resulting from a process of inclusion in the workplace. / O direito da pessoa com deficiência ao trabalho, garantido pela Constituição brasileira, não resultou na efetiva inclusão destas pessoas no mercado de trabalho. No intuito de garantir este direito criaram-se benefícios compensatórios, as ações afirmativas, que no âmbito do direito ao trabalho, estipularam uma reserva de vagas para as pessoas com deficiência no mercado de trabalho formal. Esta pesquisa avaliou as implicações desta legislação na criação de oportunidades de trabalho às pessoas com deficiência e se estas oportunidades estão pautadas pelo paradigma da inclusão, favorecendo a contratação e permanência de pessoas com diferentes níveis e categorias de deficiência no trabalho. Os dados foram coletados por meio de entrevista semi-estruturada, junto a dez funcionários do setor de Recursos Humanos e a doze funcionários com deficiência. As entrevistas foram gravadas, transcritas na íntegra e analisadas qualitativamente por meio da análise de conteúdo. Os resultados demonstraram que as empresas contratam pessoas com deficiência em função da obrigatoriedade imposta pela lei e, principalmente, em decorrência da fiscalização realizada pelo Ministério Público do Trabalho e justificam a dificuldade em contratar estas pessoas pela falta de escolaridade, qualificação profissional e pela necessidade de adequação da empresa para recebê-las. Na prática, esta necessidade antecede os requisitos da qualificação e pré-seleciona as categorias de deficiência aceitáveis pela empresa, sendo as pessoas que utilizam cadeira de rodas as principais excluídas. Dentre as empresas participantes, apenas três relataram ter realizado algum tipo de adaptação, atendendo a necessidade da pessoa contratada, limitando as contratações subsequentes a pessoas com o mesmo tipo de deficiência ou com deficiência que não implicasse em novas adaptações. Para as pessoas com deficiência, a lei resultou no aumento de oportunidades, mas não no reconhecimento de suas potencialidades para o trabalho, o que favorece sua contratação, mas não cria condições efetivas para o desenvolvimento de seu trabalho. Conclui-se que a Lei de cotas cumpre o seu papel ao gerar oportunidades de emprego, porém precisa ser avaliada, uma vez que não contempla a contratação de pessoas com deficiência em suas diferentes categorias e níveis de comprometimento e não incentiva a adequação das empresas para receber estas pessoas. Neste sentido, aponta-se para a necessidade de articular os esforços de diferentes atores e recursos sociais como instituições de educação especial e programas municipais, visando à elaboração de ações que contemplem as necessidades decorrentes de um processo de inclusão no trabalho.

O movimento inclusivo traz à tona as necessidades de um público específico: as pessoas com deficiência. Este estudo teve como objetivo descrever e analisar a participação da pessoa com deficiência nas práticas do esporte escolar: quais os princípios teóricos que regem a Educação Física Escolar e a Educação Física para pessoas com deficiência? Qual o contexto sócio-cultural e os personagens mediadores da efetivação do esporte educacional para a pessoa com deficiência? Como se dá a estruturação da atual proposta de esporte escolar para a pessoa com deficiência? Utilizou-se a entrevista semi-estruturada para coletar dados junto a nove professores de Educação Física envolvidos com o esporte escolar na rede formal e pública de ensino. Após a transcrição das entrevistas, elas foram analisadas pela Análise de Enunciação, uma das técnicas da Análise de Conteúdo. Foi utilizada, também, a técnica de Pesquisa Bibliográfica para buscar informações sobre as bases teóricas da Educação Física escolar e para pessoas com deficiência. As obras utilizadas nesta etapa foram analisadas através da Leitura Informativa. Os dados sobre o modelo de esporte proposto atualmente para pessoas com deficiência no contexto escolar foram levantados através de Análise Documental, sendo utilizada para análise dos documentos a Análise de Categoria, que é também uma das técnicas da Análise de Conteúdo. Os dados evidenciaram a fragilidade da escola atual em relação à participação efetiva de pessoas com deficiência em suas práticas. Os professores da rede apresentaram dúvidas nos aspectos de capacitação profissional, estrutura e as possibilidades da pessoa com deficiência, mantendo-se fora das discussões pedagógicas voltadas à Educação Física e a este público. Apesar das teorias de Educação Física Escolar e Adaptada se aproximarem em seus princípios ideológicos, principalmente ao focar o trabalho pedagógico nas necessidades do indivíduo e não em suas habilidades, há pouco diálogo entre elas mesmo dentro do contexto escolar, no qual hoje se trabalha com ambas as perspectivas. Com relação ao modelo proposto, tem suas bases no desenvolvimento e consolidação do esporte Paraolímpico, o que o distancia das necessidades intrínsecas ao contexto escolar. Para a realização de um esporte educacional que atenda todos os alunos com qualidade é necessário primeiro que os profissionais de Educação Física, que lidam nos diferentes contextos e com os diferentes personagens, entendam e utilizem os conhecimentos da área como parte de uma rede. Tudo que foi e será produzido é interdependente e essencial à formação do profissional completo. / Abstract: The practice of sports is more and more broadcast in contemporary society, being practised in its several contexts and by their different characters anywhere in the world. The inclusion movement brings to light the need for a specific public: disabled people. This study had as objective, describe and analyze the participation of disabled people in the practices of school sports: What are the theoretical principles governing the Physical Education at school and Physical Education for the disabled people? What are the socio-cultural context and the characters which mediate the development of the sport education for the disabled people? How is the structure of the current proposal for school sports for disabled people? We used the semistructured interview to collect data from nine teachers of Physical Education who are involved with the sport school in the formal and public schools. After the interviews transcriptions, they were analyzed by Enunciation Analysis, a technique from Content Analysis. We used, too, the technique of Bibliographic Search to search for information on the theoretical foundations of Physical Education at school and for disabled people. The documents used in this step was analyzed by the Information Reading. The data collected from the currently model of sports for disabled people in the school context, were raised through Document Analysis. This data were analyzed by Categorial Analysis, another technique from Content Analysis. The data showed the fragility of the current school regarding effective participation of persons with disabilities in their practices. The teachers had doubts on aspects of professional training, structure and possibilities of the disabled people, keeping out of the pedagogical discussions about Physical Education and this public. Despite the theories of Physical Education at school and Adapted Physical Education move closer in its ideological principles, mainly when focus the pedagogical work on the needs of the individual and not in their skills, there is little dialogue between them even within the school context, in which currently works with both perspectives. Regarding the proposed model, has its foundations in the development and consolidation of the sport Paraolímpico, what makes the model remain distant from the needs of the school. For the realization of a sport education that meets all students with quality, is necessary that the Physical Education professionals who works in different contexts with different characters understand and use the knowledge of the area as part of a network. Abstract Context: Advancement in mobile technology is truth of this Century. Mobile phone is used mainly for calling and Short Message Service, but Smartphone are advanced form of mobile phone which have multiple features. Smartphone can have Global Positioning System, Wi-Fi, Bluetooth and Near Field Communication like features. These Smartphone devices are designed considering general public and can be difficult to use by people with limited cognition. But proper use of Smartphone features while developing applications to support people with cognitive disabilities is vital. The interfaces and structure of information can have impact on the success of application. Although, some special systems exist to support these people but they create a distinction among functional and non-functional population. Smartphone with Near Field Communication (NFC) technology feature was considered in this research work as a cognitive support for independence in their daily life activities. Objectives: The objectives were the Identification of issues to be considered while providing cognitive support through technology for cognitive disabled persons. And if technology used to support them is Smartphone then to investigate the appropriate structure of information to be displayed on screen to make them able to perform their activities by following instruction provided by application. To know how NFC Smartphone feature can be helpful to start an application designed to provide cognitive support i.e. providing them information of weather and suggesting appropriate clothes to wear. Methods: The systematic literature review was conducted to find the issues which should be considered while supporting cognitive disabled. The survey was used to identify the needs, abilities and disabilities of cognitive disabled persons. Interviews with higher administration involved in support facilities to the cognitive disabled were conducted to get in-depth knowledge from their experience which can be helpful for developing the application. The Participatory design workshop was conducted by involving stakeholders. Appropriate structure of information for weather and clothes scenario was decided. The NFC enabled Smartphone application was implemented and tested in real environment. For validation focus group interviews were conducted. Conclusions: The NFC enabled Smartphone application to assist in deciding clothes to be worn according to outside weather condition was evaluated in real environment. The results of user testing and focus group interviews indicate that Smartphone application through NFC feature can support people with cognitive disabilities. Furthermore, for validation purpose ethnographic methods can be used for validation of proposed NFC enabled Smartphone application.

This dissertation examines engagement of people with disabilities in the Czech labor market over the course of last fifteen years, period between 2000 and 2015, focusing on the Karlovy Vary region. In the first section, author briefly introduces current state of Czech social policy together with related issues, which encompass unemployment, legal stance and characteristics of disabled people within national and international settings. The broader conceptualisation is subsequently illustrated by concrete examples of disabled people support, which is currently offered in the Czech Republic according to statistical data. Furthermore, author also considers case of specific situation in the Karlovy Vary region. Second part of the research presents an analysis of respondents involved in social policy and their opinions and personal experiences in the region. In particular, local labor market, employment policy and related topics relating to advantages and disadvantages with recent changes. In conclusion, author summarises main facts and research results.

Diploma thesis "Supported employment as a option of solution of handicapped people participation at the labour market " attend to supported employment. It is the service, which provide assistance for handicapped people in participation at the open labour market among people without disability. Such employment, in contrast to sheltered employment, offer a possibility to do a meaningful work, space for self-fulfillment, salary and prevent handicapped people from social exclusion. Supported employment provides benefits for disabled people, for their families and also for employers. In the Czech republic is supported employment quite a new service, which is related to some problems. For example financing and absence of skilled workers at agencies for supported employment. Solving problems of the Czech concept of supported employment can be found in systems of supported employment in some developed European country, where is supported employment quite wide-spread, such as Norway. Future trend is oriented to extension of this service and then it will be more available for all of expectants. The main goal is also in increase of meaning about supported employment in society and establishes supported employment as a part of Czech legislation.
