Validation of a recently proposed equation for the estimation of small, dense LDL particles from routine lipid measures in a population of mixed ancestry South Africans

Thesis (MSc (Biomedical Technology))--Cape Peninsula University of Technology, 2016. / Cardiovascular diseases (CVD) are the leading cause of global mortality, of which over 75% occurred in low- and middle-income countries such as South Africa. The lipid profile, specifically decreased levels of high density lipoprotein cholesterol (HDL-C), elevated triglyceride levels and the presence of small-dense low density lipoprotein (sdLDL) has been reported associated with CVD. An increased number of sdLDL is also common in metabolic syndrome (MetS), visceral obesity and diabetes mellitus, the last a known risk factor for CVD. The modification of low density lipoprotein (LDL) size, or number of sdLDL particles, has been reported to significantly reduce CVD risk, but not conclusively so and needs further investigation. In this regard, sdLDL particles are seldom estimated routinely for clinical use because of financial and other limitations. Currently, an alternative approach for estimating sdLDL is to use equations derived from routine lipid measures, as has been proposed by several groups. However, there is a need for extensive evaluation of this equation across different ethnic and disease groups, especially since reports showed an inadequate performance of the equation in a Korean population. The aim of this study was to assess the performance of a recently proposed equation for the estimation of sdLDL in healthy and diabetic mixed ancestry South Africans. Furthermore, we also investigated the role of sdLDL as a cardiometabolic risk factor, as measured against known risk factors such as the glycemic and lipid profiles.

This research aims to determine a protocol for the mechanical control of the dental biofilm performed by the professional. A protocol that was able to maintain the periodontal health of the patients under orthodontic treatment with fixed appliances, and in order to do so, it used a non-controlled, randomized and blind clinical essay. The sample involved 40 adolescents who were under the installation of fixed orthodontics appliances and it was divided in three groups, as follows: monthly controlled group (group 1) composed of 11 patients, the quarterly controlled group (group 2) with 16 patients and the semestrial controlled group (group 3) with 13 patients. For data collection, an interview and clinical exams with probing depth measurement, quantity of keratinized mucosa, Gingival Index and the Plaque Index were used. On the initial exam all patients received brushing guidelines as well as the professional control of dental biofilm, with periodontal scaler, Robinson s brush and prophylactic paste. However, Group 1 returned every month for control procedures; Group 2 every three months and Group 3 after six months. The intervention had a six-month duration (for all the three groups), when all the exams were repeated by another examiner who did not know which group each patient was inserted in. Finally, the research concluded that despite the fact there is no statistically significant difference among the three groups, clinically the patients from the monthly group presented a better response to professional control, with less accumulation of dental biofilm and less rate of gingival inflammation. Thus, the mechanical control of the dental biofilm performed by the professional could not avoid gingival increase, characterized by the raise of probing depth measurement, neither the quantity of keratinized mucosa / A presen?a do aparelho ortod?ntico fixo dificulta a higieniza??o, tornando comum o aparecimento de problemas periodontais nestes pacientes, apesar de manterem contato todo m?s com o dentista. O objetivo desta pesquisa foi determinar um protocolo, de controle mec?nico do biofilme dent?rio executado pelo profissional, que fosse capaz de manter a sa?de periodontal de pacientes em tratamento ortod?ntico com aparelho fixo, para tanto se empregou um ensaio cl?nico, n?o-controlado, randomizado e cego. A amostra envolveu 40 adolescentes na fase de instala??o do aparelho ortod?ntico fixo, e foi dividida em tr?s grupos sendo eles: o grupo do controle mensal (grupo 1) composto de 11 pacientes, o grupo do controle trimestral (grupo 2) com 16 pacientes e o grupo do controle semestral (grupo 3) com 13 pacientes. Para a coleta de dados adotou-se uma entrevista e exame cl?nico com medi??es da profundidade de sondagem, quantidade de mucosa ceratinizada, ?ndice Gengival e o ?ndice de Placa. No exame inicial todos os pacientes receberam orienta??o de escova??o e controle profissional de biofilme dent?rio, com curetas periodontais, escova de Robinson e pasta profil?tica. Sendo que o grupo 1 retornou a cada m?s para o controle; o 2 a cada tr?s meses e o 3 ap?s seis meses. A interven??o teve uma dura??o de seis meses (para os tr?s grupos), quando todos os exames foram repetidos por outro examinador que desconhecia a qual grupo pertencia cada paciente. Ao final da pesquisa foi poss?vel concluir que apesar de n?o existir diferen?a estatisticamente significativa entre os tr?s grupos, clinicamente os pacientes do grupo mensal apresentaram uma melhor resposta ao controle profissional, com menor ac?mulo de biofilme dent?rio e menor grau de inflama??o gengival. O objetivo deste trabalho é analisar as ações de prevenção da transmissão vertical do vírus da AIDS durante o pré-natal, parto e puerpério, bem como as ações de vigilância epidemiológica, entre as mulheres grávidas soropositivas ao vírus HIV, residentes em um município do interior do estado de São Paulo. Trata-se de estudo realizado em duas fases, com abordagem quantitativa na primeira e qualitativa na segunda. Na primeira, descreve-se a situação epidemiológica dos 11 municípios pertencentes àquele Departamento, no período de janeiro de 2000 a dezembro de 2005, partindo-se das notificações e das fichas epidemiológicas de 112 gestantes portadoras do vírus HIV e das crianças nascidas destas gestações. As informações referem-se à idade das mulheres, local de sua moradia, momento de identificação da infecção, local e data do parto, além daquelas relativas aos procedimentos assistenciais terapêuticos e profiláticos. Na abordagem qualitativa, ou seja, na segunda fase foram realizadas entrevistas nas quais foi utilizado um roteiro padronizado e pré-testado com enfermeiros e médicos que prestam atendimento às gestantes e parturientes nas Unidades Básicas de Saúde e Hospital do município sede do Departamento Regional. Os resultados mostram, entre outras coisas, que todas as gestantes residiam em zona urbana, a maioria era branca (67,9%), de baixa escolaridade, sendo que mais de dois terços não haviam completado o primeiro grau e 42,9% tinham idade abaixo de 25 anos. Mais da metade conhecia seu status de soro-positividade antes da gravidez (53,6%) e para 10,7%, essa era a 2ª gravidez após o diagnóstico. Foram observadas falhas na profilaxia da transmissão vertical na gestação, parto e pós-parto. Nessa última intervenção, 17 mulheres não receberam AZT intravenoso e três recém-nascidos não fizeram profilaxia com AZT oral e outros três iniciaram a profilaxia 24 horas depois do parto. A estimativa de transmissão vertical do HIV foi de 4,5%. Na etapa qualitativa, foi observado que as solicitações de exames anti-HIV eram rotina estabelecida no pré-natal, não havendo, no entanto, aconselhamento pré e pósteste. No momento que antecede o parto, é sempre solicitado teste rápido. A necessidade da profilaxia, a indicação do parto cesariano e a inibição da lactação são de conhecimento dos profissionais, porém, esses profissionais não referem à forma como manejam essas intervenções. Conforme dados epidemiológicos resultantes desta pesquisa, algumas atitudes relacionadas à assistência às gestantes HIV positivas e a seus filhos não atendem as recomendações do Ministério da Saúde para a redução da transmissão vertical e as falas dos profissionais reforçam a necessidade de programas de capacitação e treinamentos na forma de educação continuada sobre o assunto para alcançar o objetivo proposto nas diretrizes nacionais de saúde / Abstract: A priority for public health research and intervention in Brazil is the vertical transmission of the AIDS virus, which has increased among women of reproductive age, resulting in a decreasing male/female ratio. The objective of this study is to analyze actions to prevent the vertical transmission of the AIDS virus during pregnancy, childbirth, and postpartum period, as well as the actions of epidemiological monitoring, of HIV-positive pregnant women residing in a city in the interior of the state of São Paulo. The study has two phases, one quantitative and the other qualitative. The first phase consisted of a descriptive study of the epidemiological situation in 11 municipalities pertaining to the Department of Health in the period from January, 2000, to December, 2005, based on epidemiological notifications and case records of HIV-positive pregnant women and the children resulting from those pregnancies. Information collected included the women's age, residence; time the HIV infection was identified, locale and date of delivery, as well as data on therapeutic and prophylactic care procedures. In the qualitative phase of the study, semi-structured interviews were conducted with nurses and physicians working in Primary Health Care Units and hospitals in the city where the Regional Health Department is located and, who provide care to the pregnant women and newborns. All the pregnant women reside in the urban area, most are White, with low educational levels, and 42.9% were less than 25 years old. More than half were aware of their HIV-positive status before becoming pregnant, and 10.7% of these were in their second pregnancy after having been diagnosed. Errors were observed in prophylactic procedures to prevent vertical transmission during pregnancy, during childbirth, and post-partum. With reference to the latter, 17 women were not given intravenous AZT, three newborns were not given AZT orally, and an additional three initiated prophylaxis 24 hours postpartum. The estimated vertical transmission of HIV was 4.46%. In the qualitative phase, it was observed that requests for anti-HIV tests were routine in pre-natal care, but with no counseling provided before or after the test. A rapid test is routinely requested at the moment just before childbirth, but there are no institutional standardized procedures that obligate staff to follow the recommendations that come with the test. The need for prophylaxis, cesarean birth, and inhibition of lactation are known by the professionals, however they do not refer to the way they normally deal with those procedures with the women. It was also observed that pediatricians deal better than other professionals with patients diagnosed as HIV-positive. The observed failures in health care and epidemiological surveillance for HIV-positive pregnant women and for their children confirm that the Ministry of Health HIV/AIDS protocols were not always followed by the health professionals, which shows the needs for continuing education, as they themselves pointed out / Doutorado / Saude Coletiva / Doutor em Saude Coletiva

Hábitos de vida saudáveis e uma dieta balanceada aliados ao alto consumo de frutas e vegetais, estão associados à prevenção de doenças e à manutenção da saúde. A maçã possui em sua composição compostos bioativos que podem agir na prevenção e no controle das dislipidemias. Tendo em vista a preocupação da Saúde Pública em encontrar uma fonte alternativa na redução das doenças cardiovasculares, esse trabalho teve como objetivo analisar a composição química da maçã, variedade Gala, e estudar os efeitos do seu consumo no ganho de peso, consumo alimentar, níveis sanguíneos de colesterol total, HDL-C, LDL-C, triglicerídeos, colesterol hepático e colesterol excretado em ratos Wistar hipercolesterolêmicos. Foram utilizados 6 animais em cada grupo de tratamento (controle, 5%, 15% e 25% de maçã na dieta) durante 30 e 60 dias. Foi observado neste estudo, quanto a composição centesimal da maçã, que uma maçã (200g), é capaz de fornecer 14,5% das recomendações de fibras totais e 55% de vitamina C, além de quantidades consideráveis de compostos fenólicos (0,38g/100g em base fresca) e tanino (0,16g/100g em base fresca). No ensaio biológico, os animais de ambos os tempos, apresentaram uma redução não significativa no ganho de peso e no consumo de dieta com o aumento das concentrações de maçã. Ao final de 30 dias, todas as dietas proporcionaram uma redução significativa (p ≤ 0,05) nos níveis de triglicerídeos comparativamente ao grupo controle e não significativa com relação aos níveis de HDL-C. As dietas com 15% e 25% de maçã mostraram reduções significativas nos níveis sanguíneos de colesterol total e LDL-C e um aumento no teor de colesterol excretado em relação ao grupo controle. A dieta com 25% de maçã promoveu uma redução significativa nos níveis de colesterol hepático em comparação ao grupo controle. Aos 60 dias, os níveis sanguíneos de colesterol total, LDL-C, HDL-C e triglicerídeos de ratos alimentados com as dietas 5%, 15% e 25% de maçã foram semelhantes ao grupo controle.A partir desses resultados, pode-se concluir que a maçã Gala é um potente alimento no controle dirigido das dislipidemias em ratos. Uma dieta rica em verduras, legumes e frutas, inclusive a maçã, associado a um estilo de vida saudável, ao longo do tempo, podem ser considerados fundamentais na prevenção e redução do risco de doenças, principalmente as cardiovasculares. / Healthy life habits and an equilibrate diet, associated with a high fruit and vegetable intake, are joined with the prevention of diseases and health maintenance. The apple has in its composition bioactives compounds that can help in the prevention and control of hyperlipidemia. With the worry of Public Health to find an alternative source in the reduction of the cardiovasculars diseases, the objective of this work was to analyse the chemical composition of the Gala apple and to study the effects of its consumption in the gain of weigh, food intake, seric levels of total cholesterol, HDL-C, LDL-C, triglycerides, hepatic cholesterol and feces cholesterol in hypercholesterolemics Wistar rats. Six animals were distributed in each treatments (control, 5%, 15% and 25% apple diet) during 30 and 60 days. This study showed that one apple (200g) can provide 14,5% of the total fiber and 55% of the vitamin C within the recommendation, besides considerable quantities of phenolic compounds (0,38 g/100g wet weigh) and tannins (0,16 g/100g wet weigh). In the biological assay all the animals showed a non significative reduction in the gain of weigh and food intake with the increase of apple concentrations in the diets. At the end of 30 days, all the diets provided a significative reduction in the levels of tryglicerides comparable to the control group and non significative in relation to HDL-C levels. The 15% and 25% apple diets showed significatives reductions in the seric levels of total cholesterol and LDL-C and an increase in the level of feces cholesterol in relation to the control group. The 25% apple diet provided a significative reduction in the hepatic cholesterol levels comparable to the control group. After 60 days, the seric levels of total cholesterol, LDL-C, HDL-C and tryglicerides in rats fed with 5%, 15% and 25% apple diets were alike to the control group. These results confirm the importance of the Gala apple in the rats hyperlipidemia control. A diet rich in vegetables and fruits, including apple, associated with a healthy life habits, along the time, can be considered in the prevention and reduction against the risk of disease, mainly, the cardiovascular ones.

The AIDS of aid?: long-term organisation challenges of a CBO dealing with HIV/AIDS, poverty and donor aid

The following treatise first frames the role of CBOs in responding to the HIV/Aids crisis in relation to their position in the global health governance system through a literature survey that moves from an analysis of the global structures down to the local. The survey covers the role of international organisations, international NGOs (INGOs), national governments, local NGOs and CBOs and outlines the context in which Masizakhe is working within the global health governance structure. Secondly the research design and methodology are outlined focusing on the longitudinal, case study and participant--‐observation approaches. Hypotheses, conceptualisation, definitions, key variables are described and data collection methods and fieldwork practice extrapolated upon. Following that data capturing, editing and analysis are discussed in conjunction with shortcomings and sources of error. In the fourth chapter the research discusses the history, structure and outlines the research findings by comparing what has changed within the organisation over time, presenting and discussing the results. The outcomes of this research have shown that existing problems in this particular CBO are very difficult to overcome without committed, sustained support from donors, government, community and the organisation’s members. CBOs are often hamstrung by a series of intersecting factors which hamper their ability to problem--‐solve, even when the route to overcoming the problem is clear, particularly when the capacity and will to do so is not always present from both within the organisation and from outside support systems. These challenges then impact on the overall quality of and ability to deliver the services the organisation is structured to deliver. The major challenge for the organisation remains the inconsistent donor cycle and resultant instability thus created within an organisation already working in a highly volatile, unstable situation marked by poverty and disease. Thus the title, The Aids of Aid?, captures the essence of Masizakhe’s struggle with its own syndrome of problems. It summarises a comment made by the project secretary said that: “Sometimes it feels like we are not only fighting for the health of our people – We are fighting for the health of our organisation. We are a sick organisation trying to help sick people. All we need is donors and funding –we can’t live without them, and when they don't give, we get sick” (Stamper, Pers Comm, 2011). The other emergent challenges were a battle internally with ‘founder syndrome’, lack of management transparency and a dysfunctional board.

Criminal liability for wilful HIV/AIDS infection: a comparative study

South Africa‘s high prevalence of HIV/AIDS coupled with a high crime rate and incidence of sexual violence necessitated the enquiry and study into the role of criminal law to address the wilful transmission of HIV.1 This study shows that criminal law can be used to punish offenders for wrongdoing and therefore finds application in the wilful transmission of HIV.2 The study distinguishes the dividing line between the justifiable use of criminal law and where use of the criminal law becomes discriminatory in nature and counterproductive to public health measures. The United Nations (hereinafter referred to as the UN) laid down guiding principles for countries to adopt when using the criminal law and stated that countries should use existing criminal law offences to prosecute intentional HIV infections.3 The South African Law Commission (hereinafter referred to as the SALC) endorses this approach. South Africa‘s use of the criminal law, in response to harmful HIV behaviour is in line with the UN recommendations as it uses the existing common law offences to prosecute the wilful transmission of HIV, namely murder, attempted murder and assault. Drawing from the writer‘s comparative study in Chapter Six below, South Africa, members of the Zimbabwean parliament, Canada, as well as the American Bar Association have all concluded that the use of specific HIV-related legislation creates some a form of stigmatization towards people living with HIV and is therefore not warranted. This study shows that criminal law has a role to play in the wilful transmission of HIV; however the creation of HIV specific legislation is not recommended and existing criminal law offences should be used to address harmful HIV related behaviour. Such an approach is in line with the guiding principles laid down by the UN and SALC.

Inicialmente a testagem dava-se apenas nos serviços especializados, os chamados COAS ou CTA mas, devido à magnitude alcançada pela epidemia de HIV/aids, bem como a necessidade de diagnóstico precoce dos casos, fez-se necessário a disponibilização do teste anti-HIV na rede básica de saúde. O aconselhamento tem sido apontado como uma técnica a ser aplicada na oferta e na entrega do resultado do teste. Este se baseia na mudança de comportamento de pessoas e grupos sociais, proporcionando-lhes a oportunidade de adotarem práticas sexuais mais seguras ou reduzindo o dano de um determinado comportamento de risco. Através de pesquisa qualitativa, analisamos a oferta do teste para detecção do HIV nas Unidades de Saúde da Família da rede municipal de saúde de Recife-PE, sob a ótica dos profissionais de saúde. Verificamos que a indicação do referido teste se dá, quase sempre no consultório, durante a consulta clínica. O que motiva a oferta do teste é, geralmente um quadro sindrômico compatível com a aids ou o programa de assistência ao pré-natal. Os profissionais revelaram dificuldades em absorver a demanda espontânea pelo teste. A maioria dos entrevistados desconhece o aconselhamento, quanto técnica para ofertar o exame, reverter comportamentos de risco e dar o resultado do teste anti-HIV. Isso aponta para a necessidade urgente de qualificar a oferta do teste na rede básica, fazendo com que ela seja, de fato, instrumento de combate ao avanço da epidemia do HIV/aids / Abstract: The offering of serologic tests has been placed as one of the strategies in the HIV/AIDS epidemic disease combat. At first, the testate were applied in the specialized services only, the called COAS or CTA, however, with the magnitude reached by the HIV/AIDS epidemy, as well the necessity of a precocious diagnostic to the cases, the availability at the health public service of the anti-HIV test was necessary. The counselling has been pointed as a technique to be applied in the offering and delivery of the test result. That is based in the behaviour changed of social groups and people, providing them the opportunity to adopt safe sexual practices or reducing the damage of determinate risk behaviour. Through the qualitative search, we analyse the test offering to detect the HIV in the Family Health Units in the municipal public health of Recife-PE, under the health professional¿s optics. We realised that the indication of the referred test happens, almost often, in the doctor¿s office during the procedure. What motivate the test offering is, in general, a syndromic diagnostic compatible with AIDS or the pre-natal assistance program. The professions revelled some difficult in absolving the spontaneous demand for the test. The majority of the interviewed does not know the counselling as a technique to offer the exam, revert behaviour risks and give the anti-HIV result. Esta pesquisa compara a prevalência, consciência, tratamento e controle de HAS entre população urbana e ribeirinha em Porto Velho, região Amazônica, assim como avalia outros fatores de risco cardiovascular. Foi conduzido um estudo transversal, fundamentado em inquérito domiciliar em indivíduos de 35 a 80 anos, recrutados entre julho e dezembro de 2013. Realizado entrevista com questionário padronizado, medidas de pressão arterial (PA), peso, altura e circunferência abdominal (CA). HAS foi definido através de indivíduos que relataram ter a doença, ou prescritos para uso de medicações anti-hipertensivas ou aqueles que tinham PA sistólica ? 140 mmHg ou PA diastólica ? 90 mmHg, na média de duas medidas usando dispositivo digital automático. Consciência foi baseada em autorrelatos, tratamento no uso de medicamento anti-hipertensivo, e controle foi definido quando indivíduos apresentavam PA menor do que 140/90 mmHg. Foi calculado índice de massa corpórea (IMC) e CA para avaliação de obesidade e obesidade abdominal. Também foi avaliado, através de autorrelatos, a taxa de diabetes, dislipidemia, tabagismo. Entre 1410 participantes, 750 (53,19%) tinham HAS e 473 (63,06%) eram cientes do diagnóstico. Daqueles que tinham consciência do diagnóstico, a maioria 404 (85.41%) recebia tratamento farmacológico, mas a taxa de controle foi baixa. As percentagens de prevalência e tratamento foram maiores na área urbana, respectivamente, (55,48% vs. 48,87%)(p=0,02) e (61,25% vs. 52,30%)(p<0,01). A consciência de HAS foi maior na área ribeirinha (61,05% vs. - 67,36%)(p<0,01), mas as taxas de controle, tanto entre todos os hipertensos quanto naqueles que faziam tratamento farmacológico, foram similares, respectivamente, (22,11% vs. 23,43%)(p=0,69) e (33,88% vs. 34,32%) (p=0,77). Não houve diferença significativa no sobrepeso (40,93% vs. 40,28%)(p=0,73); obesidade (19,10% vs 19,63%)(p=0,68) e tabagismo (18,56% vs. 16,76%)(p=0,09). Cerca de metade dos participantes apresentavam HAS. A prevalência foi mais alta nos urbanos, mas a diferença para os ribeirinhos foi pequena. Dos indivíduos hipertensos, tanto na área urbana quanto ribeirinha, menos de um quarto tinham HAS controlada / Abstract: High blood pressure (hypertension) is a major preventable cause of cardiovascular morbidity and mortality. Several studies indicate to the increase its prevalence in the world and low control rate, but there are few data on the riverside communities. This research compares the prevalence, awareness, treatment and control of hypertension between urban and riverside population in Porto Velho, the Amazon region, as well as evaluating other cardiovascular risk factors. A cross-sectional study was conducted, based on a household survey in individuals 35-80 years recruited between July and December 2013. Directed interview with standardized questionnaire, blood pressure measurements (PA), weight, height and waist circumference (WC). Hypertension was defined by individuals who reported having the disease, or prescribed for use of antihypertensive medications or those who had systolic blood pressure ? 140 mmHg or diastolic BP ? 90 mmHg, the mean of two measurements using automatic digital device. Awareness was based on self-reports, treatment in the use of antihypertensive medication, and control was defined as a BP ? 140/90 mm Hg. We calculated body mass index (BMI) and WC for assessing obesity and abdominal obesity. We also assessed through self-report, the rate of diabetes, dyslipidemia, smoking. Among 1410 participants, 750 (53.19%) had hypertension and 473 (63.06%) were aware of their diagnosis. Of those who were aware of the diagnosis, 404 (85.41%) received pharmacological treatment, but the control rate was low. The percentages of prevalence and treatment were higher in urban areas, respectively (55.48% vs. 48.87%) (p = 0:02) and (61.25% vs. 52.30%) (p <0.01). Awareness was higher in the riverside area (61.05% vs. 67.36%) (p <0.01), but control rates, both among all hypertensive patients and in those who were pharmacological treatment were similar, respectively, (22.11% vs . 23.43%) (p = 0.69) and (33.88% vs. 34.32%) (p = 0.77). - There was no significant difference in the overweight (40.93% vs. 40.28%) (p = 0.73); obesity (19.10% vs. 19.63%) (p = 0.68) and smoking (18.56% vs. 16.76%) (p = 0.09). INTRODUÇÃO: Estudos epidemiológicos demonstram que as doenças cardiovasculares e suas complicações estão associadas ao estilo de vida das pessoas. Há evidências de que o processo aterosclerótico se inicia na infância, e que sua prevenção pode ser mais efetiva se iniciada precocemente, com ações de educação em saúde que visem a promover a prática regular de atividade física e a mudança de hábitos alimentares. OBJETIVOS: Observação das representações dos adolescentes sobre hábitos alimentares e práticas de atividade física no contexto da promoção da saúde cardiovascular. Verificação d a exeqüibilidade das técnicas de ensino-aprendizagem baseadas em Paulo Freire, Pichon-Rivière, Prochaska e Di Clemente na reflexão com os adolescentes sobre a prevenção primária dos fatores de risco relacionados com essas representações. Testar a hipótese de que a presença da doença cardiovascular nos familiares têm influência nas representações observadas. MÉTODOS: Alunos da sétima série de uma escola pública de São Paulo foram levantados por meio de questionário epidemiológico para avaliar os riscos associados ao estilo de vida. Dois grupos diferentes de alunos, selecionados de acordo com a presença de doença cardiovascular nos pais, receberam a intervenção educativa em dinâmicas aplicadas no decorrer de 10 reuniões de grupo. RESULTADOS: A matriz de Prochaska e Di Clemente permitiu avaliar que a representação dos temas de alimentação e atividade física foi modificada nos dois grupos, que passaram do estágio de pré-contemplação para contemplação; no entanto, dez reuniões não foram suficientes para os grupos se manterem no estágio de preparação para mudança, oscilando com o estágio de contemplação. A análise feita pela matriz de Pichon- Rivière demonstra que o aprendizado do grupo sem história familiar aparece de forma clara como conhecimento construído sobre os temas propostos, enquanto o grupo com história familiar possui conhecimento pré-existente, e adquire novos conceitos de maneira mais lenta. Justificando as diferentes formas de abordagem aplicadas às atividades dos grupos no presente trabalho. CONCLUSÃO: A intervenção por meio de grupos educativos baseados em Paulo Freire, Pichon-Rivière e Prochaska e Di Clemente se mostrou útil para observar as representações dos adolescentes sobre hábitos alimentares e práticas de atividade física no contexto da promoção da saúde cardiovascular. Durante as atividades dos grupos educativos foi possível avaliar a informação pré-existente, como também, estabelecer com eles um diálogo construtivo para a prevenção primária dos fatores de risco relacionados com essas representações. / INTRODUCTION: Epidemiologic studies demonstrated that the cardiovascular disease and its complications are associated with people\'s life style. There are evidences that the atherosclerotic process begins in infancy and that its prevention can be more effective if it is precociously started by taking educative actions concerning health, which aim at promoting regular physical activity practice and the change of eating habits. OBJECTIVE: Observe the adolescents\' representations on eating habits and physical activities practices to promote cardiovascular health. Verify the techniques applied in the teaching - learning process, based on Paulo Freire, Pichon- Rivière, Prochaska and Di Clemente and together with the adolescents reflect on the primary prevention of the risks related to these representations. Test the hypothesis that the presence of the cardiovascular disease in the family influences in the representations observed. METHODS: Students of the 7th. grade of a public elementary school of São Paulo were surveyed by means of an epidemiologic questionnaire to evaluate the risks associated with their life style. Two different groups of students, chosen according to the presence of cardiovascular disease in their parents were followed during ten sections, when educative dynamics were applied. RESULTS: Prochaska and Di Clemente\'s matrix enabled the evaluation of the representations related to eating and physical activity. The representations were modified in both groups, which changed from pre- contemplation stage to contemplation stage. However, the ten group meetings were not enough for the groups to keep in the stage of preparation for the change, oscillating to the stage of contemplation. The analyses made by Pichon-Rivière?s matrix demonstrates that the learning process in the group which belongs to a family with no risk of cardiovascular disease presents a clear constructed knowledge on the proposed themes, whereas the group belonging to families under such risks has a pre- existing knowledge and acquires new concepts more slowly, justifying the different approaches applied to the activities of the groups in this study. CONCLUSION: The intervention by means of educative groups based on Paulo Freire, Pichon-Rivière, Prochaska and Di Clemente was useful to observe the representations of the adolescents on eating habits and physical activities practices to promote cardiovascular health. During the activities of the educative groups it was possible to evaluate the adolescents pre-existing information as well as establish a constructive dialogue for a primary prevention of the risk factors related to these representations.
