This study investigated caregivers' perceptions with regard to vaccine-preventable diseases in terms of six constructs of the Health Belief Model. A qualitative research design that is explorative, descriptive and contextual in nature was employed in order to understand and describe the perceptions influencing access and utilisation of services that lead to missed immunisation opportunities and consequently outbreaks of vaccine-preventable diseases. The data-collection techniques were individual unstructured in-depth interviews, field notes and clinical records. The sample consisted of twenty two (N=22) caregivers who volunteered to be interviewed. The study has highlighted that caregivers' perceptions or cognitive factors play an important role for having children in completing immunisation schedule to protect the public from vaccine-preventable diseases.
It is recommended that mass media programmes are needed to address the role of vaccines in reducing high morbidity and mortality rates caused by vaccine preventable diseases and improvement in access to immunisation services. / Health Studies / MPH (Health Studies)

O infarto do miocárdio (IM) é a doença cardiovascular que mais causa morte e invalidez em todo o mundo. O uso de animais experimentais tem auxiliado a compreender melhor a fisiopatologia e as formas de tratamento do IM. Sabendo que as dislipidemias estão associadas com o IM e que o treinamento físico pode ser prescrito para prevenção e tratamento de doenças cardiovasculares, no presente trabalho, investigamos os efeitos de dois tipos de treinamentos físicos em um modelo experimental de dislipidemia e isquemia miocárdica. Camundongos selvagens (WT) e knockout para o receptor LDL (LDL-/-) foram divididos em oito grupos: a) LDLr-/- sedentário (LDL-S); b) LDLr-/- infartado sedentário (LDL-IM-S); c) LDLr-/- infartado submetido a treinamento contínuo (LDL-IM-C); d) LDLr-/- infartado submetido a treinamento intervalado (LDL-IM-I); e) WT sedentário (WT-S); f) WT infartado sedentário (WT-IM-S); g) WT infartado submetido a treinamento contínuo (WT-IM-C); h) WT infartado submetido a treinamento intervalado (WT-IM-I). Após 60 dias da ligadura da artéria coronária descendente, o treino contínuo constou de corrida a 60% do máximo e o intervalado de 8 tiros de 4min a 80% do máximo e recuperação de 4min a 40% do máximo. Nos animais WT infartados, ambos os treinamentos aumentaram a tolerância ao esforço e provocaram diminuição do balanço simpatovagal e aumento do índice alfa em magnitudes semelhantes. O treinamento intervalado reduziu o número de fibras do tipo II em relação aos grupos WT-S e WT-IM-C, bem como reduziu a quantidade de fibras do tipo II-X em relação aos WT-S. A área de secção transversa das fibras do tipo I foi maior no grupo WT-IM-I do que no WT-IM-S e WT-S. A razão capilar/fibra foi maior nos animais do grupo WT-I do que no WT-S. A fração de ejeção e a fração de encurtamento foi menor no grupo LDL-IM-I em relação aos demais, mas sem diferenças entre os grupos WT-S, WT-IM-C e WT-IM-I. Nos animais LDL-/-, o LDL foi maior e o VLDL menor no grupo LDL-IM-C em relação aos demais. O HDLtg(%) foi superior no LDL-C em relação ao LDL-S. O HDLc (mg e %) do LDL-IM-I foi maior que o do grupo LDL-IM-C, sendo que o HDLc (mg) do LDL-IM-I foi, ainda maior do que o grupo LDL-S. O triglicérides total foi menor no grupo LDL-IM-C do que no LDL-S. Somente o grupo LDL-IM-I diminuiu a FC de repouso em relação ao grupo LDL-IM-S. A PA diastólica foi menor no grupo LDL-IM-S em relação ao LDL-S, enquanto que o grupo LDL-IM-I apresentou PA diastólica maior do que o grupo LDL-IM-C. A variância do intervalo de pulso foi maior no grupo LDL-S somente em relação ao grupo LDL-IM-I. Em conjunto nossos resultados demonstraram que os animais LDL possuem diferenças funcionais e fisiológicas importantes em relação ao WT, especialmente na morfologia muscular, na hemodinâmica e no controle autonômico. Knowing that the dyslipidemia associated with IM and that physical training can be prescribed for prevention and treatment of cardiovascular diseases, the present study investigated the effects of two types of physical training on an experimental model of dyslipidemia and myocardial ischemia. Wild mice (WT) and LDL receptor knockout (LDL-/- ) were divided into eight groups: a) LDLr-/- sedentary (LDL-S), b) LDLr-/- myocardium infarction sedentary (LDL-MI-S), c) LDLr-/- myocardium infarction submitted to continuous training (LDL-MI-C), d) LDLr-/- myocardium infarction submitted to interval training (LDLMI- I), e) sedentary WT (WT-S); f) WT myocardium infarction sedentary (WT-MI-S); g) WT myocardium infarction submitted to continuous training (WT-MI-C), h) WT myocardium infarction submitted to interval training (WT-IM-I). After 60 days of descending coronary artery ligation, the continuous training consisted of running at 60% of maximum, while the interval training consisted of eight sprints of 4 min at 80% of maximum and a 4 min recovery at 40% of maximum. In infarcted WT animals, both training programs increased exercise tolerance and promoted decrease of sympathetic-vagal balance and increase of alpha index in similar magnitudes. Nevertheless, the interval training reduced the number of type II fibers in infarcted WT animals compared to WT-S and WT-MI-C groups, as well as reduced the amount of fiber type II-X compared to WT-S. The cross-sectional area of the fiber type I was higher in the WTMI- I animals than in WT-MI-S and S-WT groups. The reason capillary/fiber was higher in group WT-I than in the WT-S. Ejection fraction and shortening fraction were lower in LDL-MII compared to the others, but with no differences among the WT-S, WT-IMI-C and WT-MI-I groups. About the LDL-/- animals, the LDL was higher and VLDL was lower in the group LDL-MI-C in relation to the others. The HDLtg (%) was higher in LDL-C compared to LDL-S. The HDLc (mg and %) of LDL-MI-I was higher than the LDL-MI-C group, and the HDLc (mg) of LDL-MI-I was even higher than LDL-S group. The total triglycerides was lower in LDL- MIC than in LDL-S animals. Only in LDL-MI-I group the resting HR was decreased in comparison to LDL-MI-S. The diastolic blood pressure was lower in LDL-MI-S in relation to LDL-S, while the LDL-MI-I group presented a higher diastolic BP than the LDL-MI-C group. The pulse interval variance was greater in LDL-S than in LDL-MI-I only. In conclusion, our results demonstrate that LDL animals have important functional and physiological differences compared to WT, especially in relation to muscle morphology, hemodynamic and autonomic cardiovascular control. Furthermore, MI leads to damage in both investigated strains and the two types of physical training attenuate similarly the impairment of most of the analyzed variables

Evaluation of the resource allocation process towards an HIV/AIDS workplace policy of a public service department in Limpopo, South AfricaRamaloko, Mokgadi Rose 02 1900 (has links)
The AIDS epidemic affects the capacity of the South African public service to deliver essential services. In response, the Department of Economic Development, Environment and Tourism in Limpopo implemented its HIV/AIDS workplace policy in 2003. This research evaluated the extent to which resources were allocated towards the implementation of the workplace policy, to effectively respond to the impacts of HIV/AIDS on the workplace.
A combined quantitative and qualitative approach was used. Data was collected through face-to-face interviews of 43 officers involved in the implementation of the workplace policy, using a semi-structured questionnaire.

Integrating indigenous and scientific knowledge in community-based early warning system development for climate-related malaria risk reduction in Mopani District of South AfricaRamutsa, Brenda Nyeverwai January 2020 (has links)
PhD (Geography) / Department of Geography and Geo-Information Sciences / Malaria is a climate-change concatenated biological hazard that may, like any other natural hazard, can lead to a disaster if there is a failure in handling emergencies or risks. A holistic solution for malaria mitigation can be provided when indigenous knowledge is complemented with scientific knowledge. Malaria remains a challenge in South Africa and Limpopo province is the highest burdened malaria-endemic region. Specifically, Vhembe District is the highest burdened followed by Mopani District (Raman et al., 2016). This research sought to mitigate malaria transmissions in Mopani District through the integration of indigenous and scientific knowledge. The study was carried out in Mopani District of South Africa and 4 municipalities were involved. These are Ba-Phalaborwa, Greater Tzaneen, Greater Letaba, and Maruleng. A pragmatism philosophy was adopted hence the study took a mixed approach (sequential multiphase design). Data was collected from 381 selected participants through in-depth interviews, a survey and a focus group discussion. Participants for the in-depth interviews were obtained through snowballing and selected randomly for the survey, while for the focus group discussion purposive sampling was used. The study applied constructivist grounded theory to analyze qualitative data and to generate theory. Statistical Package for Social Sciences version 23.0 was used for quantitative data. Based on empirical findings, it was concluded that temperature and rainfall among other various factors exacerbate malaria transmission in the study area. Results of the study also show that people in Mopani District predict the malaria season onset by forecasting rainfall using various indigenous knowledge based indicators. The rainfall indicators mentioned by participants in the study were used in the developed early warning system. An Early warning system is an essential tool that builds the capacities of communities so that they can reduce their vulnerability to hazards or disasters. In the design of the system, Apache Cordova, JDK 1.8, Node JS, and XAMPP software were used. The study recommends malaria management and control key stakeholders to adopt the developed early warning system as a further mitigation strategy to the problem of malaria transmission in Mopani District. / NRF

Avaliação do impacto à saúde causado pela queima prévia de palha de cana-de-açúcar no Estado de São Paulo / Evaluation of the health impact caused by the pre-harvest burning of sugarcane straw in the State of São PauloParaiso, Maria Leticia de Souza 12 December 2013 (has links)
O etanol de cana-de-açúcar se consolida como combustível renovável, o que promove nova expansão da cultura da cana-de-açúcar no Brasil e, principalmente, no Estado de São Paulo. Como a queima prévia controlada da palha de cana-de-açúcar ainda é considerada uma prática agrícola necessária para a viabilização econômica da colheita, em mais de 70% dos municípios do Estado de São Paulo a população é obrigada a conviver com essa poluição. Para estudar a distribuição desse fator de risco e sua relação com a saúde, realizei um estudo epidemiológico ecológico nos 645 municípios de São Paulo. Usei um modelo Bayesiano de regressão multivariada relacionando os efeitos na saúde com a exposição à queima prévia da palha de cana-de-açúcar, sendo controlados os efeitos das variações socioeconômicas (saneamento, educação e renda) e climáticas (temperatura máxima, umidade mínima e precipitação), através da inserção das mesmas no modelo. O efeito sobre a saúde foi medido por meio da Razão de Mortalidade e Morbidade Padronizada (RMP) dos desfechos: óbitos por doenças respiratórias nas faixas etárias acima de 65 anos e internações por doença respiratória, nas faixas etárias menores de 5 anos e acima de 65 anos de cada um dos municípios. Usei como medida de exposição à queima prévia dados obtidos no INPE: percentual da área de cana colhida com queima (PMQ), níveis médios de Aerossol e Focos de queima, testadas separadamente. Para resolver a autocorrelação entre os dados, estes foram considerados conforme sua disposição espacial, através da construção de uma matriz de vizinhança dos 645 municípios do Estado. Utilizei o método de simulação de Monte Carlo via Cadeias de Markov (MCMC) para \'suavizar\' as estimativas da RMP. A análise demonstrou que existe associação entre a queima prévia da palha de cana-de-açúcar e a ocorrência de doenças respiratórias, porque o aumento nos focos de queima (Focos) esteve associado significativamente com o aumento das internações por doenças respiratórias, na faixa etária de menores de cinco anos. Os resultados mostraram que a queima prévia da palha da cana-de-açúcar oferece efetivamente risco à saúde da população e, adicionados aos mapas coropléticos gerados, oferecem subsídios para a vigilância epidemiológica e contribuem para o estabelecimento de políticas públicas para controle da poluição do ar, que contemplem além dos grandes centros urbanos, os pequenos municípios. A eliminação desse fator de risco deve fazer parte das medidas primordiais de prevenção à saúde a serem adotadas no Estado / Ethanol from sugarcane is consolidated as a renewable fuel which promotes further expansion of the culture of sugarcane in Brazil and especially in the State of São Paulo. As the controlled pre-harvest burning of sugarcane straw is still considered an agricultural practice necessary for the economic viability of this crop in more than 70% of municipalities in the State of São Paulo the population is forced to live with this pollution. To study the distribution of this risk factor and its relationship with the health of the population, I conducted an ecological study in the 645 municipalities of São Paulo. I used a Bayesian multivariate regression model relating the health effects and the exposure to previous straw burning of sugarcane, controlling the effects of socioeconomic factors (sanitation, education and income) and climate (maximum temperature, minimum humidity and precipitation) by the insertion of these variables in the model. The effect on health was measured by Standardized Mortality and Morbidity Ratio (SMR) of the outcomes: deaths from respiratory diseases in the age group above 65 years old and admissions for respiratory disease in children less than 5 years old and above 65 years old of each of the municipalities. I used as a measure of exposure to the pre-harvest burning data obtained at INPE: percent of sugarcane area harvest with burning (PMQ), levels of Aerosol and Spotlights of burning, tested separately. To solve the autocorrelation in the data these were considered as their spatial arrangement, by building a neighborhood matrix of the 645 municipalities in the state. I used the Markov Chain-Monte Carlo simulation method (MCMC) to \'soften\' the estimates of the SMR. The analysis showed that there is an association between previous straw burning of sugarcane and respiratory diseases, because the increase in outbreaks of burning (Spotlights) was significantly associated with increased hospital admissions for respiratory diseases in children aged under five years old. The results show that the previous straw burning of sugarcane effectively offers health risk to the population and added to the choropleth maps generated provide valuable information for epidemiological surveillance and contribute to the establishment of public policies for the control of air pollution, which should contemplate beyond the major urban centers, the small towns. The elimination of this risk factor should be part of a primordial prevention measure to be taken in the state

CONCLUSION: A multi-faceted hand hygiene intervention with pocket-sized containers of alcohol-based hand rub was effective in increasing hand rubbing compliance and reducing incidence of total infections requiring hospitalization in elderly LTCFs. Its effect on outbreaks still needs further investigations. The questionnaire developed in this study may be a simple and effective method to assess the attitude and compliance change of the HCWs after implementing a hand hygiene programme. / DESIGN: Clustered randomized controlled trial / INTERVENTIONS: After a 3-month run-in period, we randomized three LTCFs to the treatment and three to the control group. The treatment group received pocket-sized containers of alcohol-based hand rub, education and reminding materials. The control group received basic life support education and workshops. They were followed up for another seven months. We measured the hand hygiene compliance of the HCWs by direct observation and recorded the incidence of infections of the residents from their hospital discharge summaries. / OBJECTIVE: To investigate the effectiveness of a multi-faceted hand hygiene programme with pocket-sized containers of alcohol-based hand rub for infection control in elderly long-term care facilities (LTCFs) / OUTCOMES & DATA ANALYSIS: Primary outcomes were direct observed compliances of hand washing and antiseptic hand rubbing of the HCWs, incidence of infections requiring hospitalization and death rate due to infection of the residents, and outbreaks of the LTCFs. Secondary outcomes were change in hand hygiene attitude and self-reported compliance. / PARTICIPANTS: All health care workers (HCWs) of the LTCFs recruited by snowball sampling. Their job categories were nurses, nursing assistants and physiotherapists. / RESULTS: In the treatment group, the compliance of alcohol-based hand rubbing increased significantly from 1.5% (5/333) to 15.9% (233/1465) (p=0.001) and total compliance increased from 25.8% (86/333) to 33.3% (488/1465) (p=0.01) after intervention. Total incidence of infections decreased from 31 cases in 21,862 resident days (1.42 per 1,000 resident-day (rd)) to 33 cases in 50,441 resident days (0.65 per 1,000 resident-day) (p=0.002) and death rate due to infection decreased from 8 cases in 21,862 resident days (0.37 per 1,000 rd) to 5 cases in 50,441 resident days (0.1 per 1,000 rd) in the treatment group (p=0.01). Pneumonia significantly decreased by 0.63 per 1000 rd (p=0.001). In the control group, there were no changes in both antiseptic hand rubbing and hand washing. The total incidence of infections increased from 0.49 to 1.04 per 1000 rd (p=0.004) and no change in death rate due to infection (p=0.45). No outbreaks of influenza and norovirus occurred during the whole study in both groups. / SETTING: Six LTCFs for the elderly / The attitude of antiseptic hand rubbing on infection control increased significantly in the treatment group but there was no change under different situations in the control group. The self-reported compliance of antiseptic hand rubbing increased significantly in the treatment group. There were no changes on the self-reported compliances in the control group. Com o objetivo de avaliar o desempenho do teste rápido DPP® LVC comparando com os testes de ELISA e RIFI (Bio-Manguinhos, Br), assim como ELISA e RIFI in-house, empregando como antígeno formas promastigotas de L. (L.) infantum chagasi, com ênfase a reatividade cruzada com outros agentes infecciosos, soros de cães infectados por L. (L.) infantum chagasi, clinicamente sintomáticos (n=48) e assintomáticos (n=39), assim como soros de cães sadios e não infectados (n=18), e soros de cães infectados por Babesia canis (n=9), Dirofilaria immitis (n=4), Trypanosoma cruzi (n=6), Ehrlichia canis (n=17), Neospora caninum (n=6), Toxoplasma gondii (n=9), Neospora/Toxoplasma coinfecção (n=4) e Toxocara canis (n=9) foram avaliados pelas diferentes técnicas de diagnóstico. DPP e ELISA in-house mostraram alta sensitividade (90.81% e 94.25%) e especificidade (95.06% e 97.53%), respectivamente para o diagnóstico de LVC sintomática e assintomática, mas apresentaram reação cruzada com Babesia canis, 44% para DPP e 22% para ELISA in-house. Os dois testes mostraram uma excelente concordância de resultados (kappa=0.9405, p < 0.0001). ELISA Bio-Manguinhos assim como o RIFI Bio-Manguinhos e RIFI in-house mostraram boa sensitividade (90.81%, 96.47% e 89.41%), mas baixa especificidade (77.78%, 69.14% e 65.82%), respectivamente; e mostraram reação cruzada com soros de animais infectados com Babesia canis, Dirofilaria immitis, Trypanosoma cruzi, Ehrlichia canis, Neospora caninum, Toxoplasma gondii. Os resultados mostraram que o DPP® CVL apresentou um bom desempenho para o diagnóstico da leishmaniose visceral canina sintomática e assintomática / In order to investigate the performance of the DPP® CVL rapid test comparing with ELISA and IFA (Bio-Manguinhos, Br), as well as ELISA and IFAT in house using L.
