Le rôle du stress oxydatif/nitrosatif dans la pathogénèse de l’encéphalopathie hépatique chronique

L'encéphalopathie hépatique (EH) est un syndrome neuropsychiatrique dû à une dysfonction hépatique où l'ammoniaque est un facteur central. Il a déjà été rapporté que l’intoxication aiguë d'ammoniaque induise le stress oxydatif/nitrosatif. La présente étude cible à évaluer le rôle du stress oxydatif/nitrosatif dans 2 modèles de l’EH chronique : (1) l’anastomose portocave (PCA) et (2) la ligation de la voie biliaire (BDL). Ces 2 modèles sont caractérisés par une hyperammoniémie et une augmentation d’ammoniaque centrale, cependant l’œdème cérébral est trouvé seulement chez les rats BDL. Des marqueurs du stress oxydatif/nitrosatif ont été évaluées dans le plasma et cortex frontal. Un stress nitrosatif central a été observé chez les rats PCA; tandis qu’un stress oxydatif/nitrosatif systémique a été démontré seulement chez les rats BDL. Ces résultats suggèrent (1) que l’hyperammoniémie chronique n’induise pas le stress oxydatif/nitrosatif systémique et (2) qu’un synergisme existe entre l’ammoniaque et le stress oxydatif/nitrosatif, en association avec l’œdème cérébral. / Hepatic encephalopathy (HE) is a neuropsychiatric complication due to liver failure where ammonia is believed to be central in the pathogenesis. Acute ammonia intoxication has demonstrated to induce oxidative/nitrosative stress in both in vivo and in vitro models. The present study was aimed to assess the role of oxidative/nitrosative stress in 2 models of chronic liver failure/HE; 1. portacaval anastomosis (PCA) and 2. bile duct ligation (BDL). Both models are characterised with hyperammonemia and increased brain ammonia however cerebral edema is only found in BDL rats. Oxidative/nitrosative stress markers were evaluated in plasma and frontal cortex of both animal models. Central nitrosative stress was observed in PCA rats, but systemic oxidative/ntrosative stress was demonstrated only in BDL rats. The results of our study suggest i) chronic hyperammonemia does not induce oxidative stress and ii) a synergistic effect between ammonia and systemic oxidative/nitrosative stress is associated with cerebral edema.

Cellular mechanisms involved in the recapitulation of endocrine development in the duct ligated pancreas

Thesis (PhD)--University of Stellenbosch, 2011. / ENGLISH ABSTRACT: Diabetes mellitus is amongst the leading causes of morbidity and mortality in the world, affecting young, adult and old people. Beta cell replacement therapy for insulin delivery remains the ultimate remedy for diabetes. However, insufficient donor pancreas and the use of immunosuppressive drugs prevent the wide-spread of this therapy. Other avenues of self generated beta cells within the organ itself need to be explored. Therefore, understanding the chronobiology of cellular mechanisms in the lineage of beta cell induced neogenesis is a valuable tool in improving beta cell replacement in patients with diabetes. The aim of this study was to induce recapitulation of the morpho-genetic sequence of endocrine cells development in the pancreas of rats after the pancreatic duct ligation (PDL) procedure. Serial sections of PDL tissues of the pancreas were obtained from 78 Sprague- Dawley rats and were assessed morphologically. The immunofluorescent tissues were statistically analysed using a computerized morphometry technique. The protein expression indices of Caspase3, Insulin, Pdx1, Ngn3, NeuroD and Pax6 were quantified. The efficiency levels of coexpression of these homeodomain proteins separately with insulin were defined by the ratio of the mean value of insulin expression to the mean value of their respective protein expression. The morphological changes were characterized by the appearance of granulated acinar cells at 6 hours post-PDL and the proliferation of endocrine tissues from 84 hours through to 120 hours. The morpho-immunofluorescent evaluation showed the highest immunoreactivity of Caspase3 and Pdx1 at 6 hours, Ngn3 at 36 hours, Pax6 and insulin at 84 hours while NeuroD expression was at 120 hours. The immunohistofluorescent analysis showed that caspase3 and Pdx1 were the first to be expressed at 6 hours while the insulin and NeuroD expression appeared later at 84 hours and 120 hours, respectively. However, Pax6 expression was continuous across time periods post-PDL, while Ngn3 expression showed a peak at 36 hours. The efficiency (highest and earliest expression) of co-expression of all these homeodomain proteins with insulin was restricted between 12 hours and 24 hours. The optimal efficiency was at 12 hours by Ngn3 with insulin. A good efficiency was shown for Pdx1 with insulin, NeuroD with insulin and Pax6 with insulin at 12 hours and 24 hours, respectively. A low efficiency was observed for insulin and caspase3 co-expression at 24 hours. This study suggests that for transplantation, PDL tissues harvested at an early time post-PDL (between 12 and 24 hours) could yield a higher success rate; the study also provides evidence for a connection between morphological changes in the PDL pancreas and the protein synthesis necessary for the lineage of endocrine cell development. / AFRIKAANSE OPSOMMING: Diabetes Mellitus resorteer onder die vernaamste oorsake van morbiditeit en mortaliteit wêreldwyd, en tuister jongmense, volwassenes en bejaardes. Daar bestaan egter ‘n wêreldwye tekort aan skenkerorgane met immuun-onderdrukingsterapie as ondersteuningsbehandeling. Beta-sel vervangingsterapie, vir die voorsiening van insulien, bly daarom die voorkeur behandeling vir die siekte wat noodsaak dat die wetenskap kyk na alternatiewe behandelingsregimens wat meganismes rondom orgaanregenerasie insluit. Begrip van die chronobiologie van die sellulêre meganismes betrokke rondom beta-sel ontwikkeling mag waardevolle lig werp op die neogenese van beta-selle wat gevolglik daartoe mag lei dat beta-sel vervanging as ‘n moontlike behandelingsterapie oorweeg mag word vir pasiënte met suikersiekte. Die oogmerk van hierdie studie is om die rekapitulasie van die morfo-genetiese volgorde van die endokriene pankreas na afbinding van die pankreasbuis te bepaal. Pankreasbuis afbinding is op 78 Sprague-Dawley laboratorium rotte onder algemene narkose uitgevoer, die pankreas is na voorafbepaalde tydsvakke verwyder en in histologiese seriesnitte gesny. Snitte is immunositochemiese gekleur en morfometries assesseer. Die afskeidingsindeks vir selboodskappers vir Caspase3, Insulien, Pdx1, Ngn3, NeuroD en Pax6 is kwantifiseer. Die gelyktydige afskeiding van elk van bogenoemde boodskappers tesame met insulien is omskryf as ‘n verhouding tot mekaar en in terme van dié van insulien. Die morfologiese verandering in die weefsel bespeur is gekenmerk deur die verskyn van gegranuleerde asinêre selle ses (6) ure na buisafbinding en die proliferasie van endokriene weefsel vanaf vier-en-tagtig (84) ure deurlopend tot een-honderd-en-twintig (120) ure. Die morfo-immunofluoresserende evaluering toon dat Caspase3 en Pdx1 by 6 uur die hoogste is, die van Ngn3 by 36 ure, Pax6 en insulien by 84 ure en NeuroD by 120 ure. Verder toon die analise dat Caspase3 en Pdx1 rondom 6 ure hul verskyning gemaak het terwyl dié van insulien en NeuroD eers rondom 84 tot 120 uur verskyn het. Die verskyning van Pax6 het deurlopend regoor al die tydsduurtes verskyn en Ngn3 het rondom 36 uur sy hoogste vlak bereik. Die gelyktydige uitdrukking van homeodomein proteïene tesame met insulien het slegs tussen die tydperke van 12 en 24 ure plaasgevind. Die uitdrukking van Pdx1 met insulien, NeuroD met insulien en Pax6 met insulien het almal tussen 12 en 24 ure plaasgevind. Caspase3 tesame met insulien is slegs by die 24 uur tydsperiode bespeur. Vir die oorplant van pankreas weefsel wat aan buisafbinding onderwerp is suggereer hierdie studie dat die geskikste tyd vir die oes van endokriene weefsel liewer vroeër (12 to 24 ure) as later uitgevoer behoort te word. Verder wil dit voorkom of hierdie tydsperiode ook die hoogste seltelling lewer. Die studie lewer waardevolle inligting oor die verwantskap tussen die morfologiese veranderings wat na buisafbinding plaasvind en die proteïen sintese wat sel-opvolgontwikkeling bevorder.

Sound propagation in a possibly lined annular duct with swirling and sheared mean flow : application to fan broadband noise prediction

L’évolution des turboréacteurs vers des taux de dilution toujours plus importants est associée à de nouvelles problématiques. Parmi elles, le raccourcissement de l’entrée d’air et de la tuyère est associé à une diminution du gain apporté par les traitements acoustiques de nacelle. La contribution des traitements situés dans l’espace entre la soufflante et le stator redresseur (OGV) va donc prendre de l’importance par rapport à l’ensemble des traitements. Cette zone, également appelée “interstage”, est caractérisée par une forte giration de l’écoulement moyen due à l’entraînement du fluide par le rotor. L’objectif de ce travail est de développer un modèle analytique afin d’évaluer l’effet de la giration sur le comportement des traitements acoustiques dans l’interstage, ainsi que sur le bruit à large-bande rayonnant en amont dû à l’interaction de la turbulence en aval de la soufflante avec les aubes des stators (OGV). Dans un premier temps, l’évolution de petites perturbations dans écoulement moyen tournant et cisaillé dans un conduit rigide est étudiée. Après avoir introduit les équations ainsi que les hypothèses du problème, l’analogie acoustique de Posson & Peake [122] est présentée. L’effet de la giration sur le contenu modal dans un conduit rigide est mis en évidence pour plusieurs types d’écoulements tournants. En particulier, le décalage des fréquences de coupures est étudié. L’étude est ensuite étendue au cas d’un conduit annulaire traité acoustiquement. Une attention particulière est portée sur la condition aux limites à appliquer aux parois du conduit. Dans ce cadre, une correction due aux effets centrifuges est apportée à la condition aux limites de Myers [101]. Une extension du modèle de Brambley [24] est aussi proposée afin de prendre en compte l’effet de l’épaisseur de la couche limite aux parois du conduit dans le cas tournant. Les effets combinés de la rotation et de la condition aux limites sur le contenu modal sont ensuite étudiés. En outre, une relation de dispersion pour les modes de surfaces en présence d’écoulement tournant est développée. À partir des développements précédents, un modèle de transmission acoustique est proposé afin d’évaluer l’effet de la giration sur le comportement des traitements acoustiques. La méthode repose sur le principe de raccordement modal appliqué à la conservation du débit massique et de l’enthalpie totale aux interfaces séparant les sections rigides et traitées. Une nouvelle méthode de projection basée sur les propriétés des polynômes de Chebyshev est proposée. À partir de ce modèle, l’efficacité des traitements acoustiques est étudiée pour différents écoulements tournants. Enfin, un modèle de prédiction du bruit à large-bande d’interaction rotor-stator est établi à partir de l’analogie de Posson & Peake [122], dans le but de prendre en compte l’effet de la giration sur la puissance acoustique rayonnée en amont. Le terme source est calculé selon le formalisme de Posson et al. [120]. Le modèle ainsi développé permet de prendre en compte une évolution radiale des paramètres géométriques et des propriétés statistiques de la turbulence incidente. Le modèle est ensuite évalué sur le cas test NASA SDT pour différents régimes et géométries. / The advent of modern turbofan engines such as UHBR goes along with new issues. Amongst others, the shortening of the inlet and exhaust yield a relatively higher importance of the liners located inside the interstage, where the flow is highly swirling. The present work aims at developing analytical models to assess the effect of the swirl both on the behavior of the interstage liners and on the upstream radiation of the fan-OGV interaction broadband boise. The evolution of small fluctuations in a rigid annular duct containing a swirling and sheared mean flow are studied first. After having introduced the governing equations and the main assumptions, the acoustic analogy of Posson & Peake [122] tailored to an annular duct with swirl and shear is presented. The effect of the swirl on the modal content in a rigid annular duct is highlighted for different types of swirl. In particular the shift of the cut-on thresholds is studied. Then, the modal analysis is extended to a duct with lined walls. A particular attention is paid on the boundary condition. Notably, a correction of the classical Myers boundary condition [101] is proposed to account for the centrifugal effects. An extension of Brambley’s boundary condition [24] is also derived to account for the boundary layer thickness to first order. The effect of both the swirl and the boundary condition on the modal content are studied. Besides, a dispersion relation for the surface waves is derived for the corrected Myers boundary condition. Based on the previous modal analyses, a transmission tool is developed to assess the effect of the swirl on the efficiency of a liner. The method, which relies on the mode-matching approach, is based on the conservation of the total enthalpy and the mass flow at the interfaces between the rigid and the lined sections. Due to the nature of the eigenfunctions, a new projection method based on the Chebyshev polynomial properties is proposed. Thanks to this model, the absorption is assessed for different types of swirl. Finally, a rotor-stator interaction broadband noise prediction model is derived from Posson & Peake’s acoustic analogy [122], to account for the effect of the swirl on the upstream radiated acoustic power. The source term is computed according to Posson et al.’s model [120]. It allows considering a radial variation of the geometry and the statistical properties of the incident turbulence. The model is assessed on the NASA SDT test case and the effect of the swirl is evaluated for several stator geometries and regimes.

Numerical simulation of acoustic propagation in a turbulent channel flow with an acoustic liner / Simulation numérique de la propagation acoustique en canal turbulent avec traitement acoustique

Les matériaux absorbants acoustiques, qui sont d’un intérêt stratégique en aéronautique pour la diminution passive du bruit des réacteurs d’avion, conduisent à une physique complexe où l’écoulement turbulent, des ondes acoustiques, et l’absorbant interagissent. Cette thèse porte sur la simulation de cette interaction dans le problème modèle d’un écoulement de canal turbulent avec des parois impédantes, par le biais de simulations numériques aux grandes échelles implicites, dans un contexte de calcul haute performance.Une étude est d’abord faite des grandes échelles dans un canal turbulent avec des parois rigides, en s’intéressant plus particulièrement à l’effet d’une faible compressibilité (Mach <3) sur les caractéristiques de ces échelles.Un canal turbulent avec une paroi de type impédance est ensuite simulé, avec une condition habituelle de périodicité dans le sens de l’écoulement. On observe que pour des faibles valeurs de la résistance et des fréquences de résonance basses, l’écoulement est instable, ce qui engendre une onde le long de l’absorbant, qui modifie la turbulence et augmente la trainée.Enfin, on se tourne vers une simulation de canal spatial en levant la condition de périodicité dans la direction de l’écoulement, ce qui permet d’introduire une onde acoustique en entrée de domaine. L’atténuation de l’onde dans l’écoulement turbulent est étudiée avec des parois rigides, puis un absorbant acoustique est introduit. Dans cette configuration plus réaliste, il est confirmé que l’écoulement peut devenir instable au bord amont de l’absorbant, ce qui empêche l’atténuation de l’onde acoustique incidente. / Acoustic liners are a key technology in aeronautics for the passive reduction of the noise generated by aircraft engines. They are employed in a complex flow scenario in which the acoustic waves, the turbulent flow, and the acoustic liner are interacting.During this thesis, in a context of high performance computing, a compressible Navier-Stokes solver has been developed to perform implicit large eddy simulations of a model problem of this interaction: a turbulent plane channel flow with one wall modeled as an impedance condition.As a preliminary step the wall-turbulence in rigid channel flows and associated large-scale motions are investigated. A straightforward algorithm to detect these flow features is developed and the effect of compressibility on the flow structures and their contribution to the drag are studied. Then, the interaction between the acoustic liner and turbulent flow is investigated assuming periodicity in the streamwise direction. It is shown that low resistance and low resonance frequency tend to trigger flow instability, which modifies the conventional wall-turbulence and also results in drag increase.Finally, the simulation of a spatial channel flow was addressed. In this case no periodicity is assumed and an acoustic wave can be injected at the inlet of the domain. The effect of turbulence on sound attenuation is studied without liner, before a liner is introduced on a part of the channel bottom wall. In this more realistic case, it is confirmed that low resistance acoustic liners trigger an instability at the leading edge of the liner, resulting in drag increase and excess noise generation.

Energy efficiency in a renovated modern office with activity-based work style

During renovation Ljusåret 2 was converted to a modern office with an activity based work style (ABW) with a Demand Controlled Volume (DCV) ventilation system connected to a closed-loop duct. Cooling is provided through air handling units and active water based beams, the underfloor heating system was kept. Written instruction and specification have been studied for the two different control systems Schneider EcoStructure and Lindinspect. Both control systems have been analyzed according to time schedule, set-point and process value by using different functions in software. To be able to perform a energy audit and look at indoor climate for Ljusåret 2 there have been studies according to underfloor heating, constructions of ventilation system, diversity factor for DCV, closed-loop-ducts, heat losses from ducts, cooling demand and energy certification. According to this audit, energy performance is calculated to 89.1 kWh/m2 according to building energy, activity energy is not audited or calculated. During design phase, an energy calculation was made by an energy consultant with the result of 81.3 kWh/m2. The estimated performance is a 9.6 % increase. This building is designed for Miljöbyggnad certification of level silver and should be ≤ 109 kWh/m2,year. According to audit and calculation for energy performance this level is possible to keep. The estimated energy performance have been calculated with only 4 month of statistics from January until April 2019 because Ljusåret 2 have just been renovated. District heating has been estimated through the energy signature by data from energy meter. Electrical components for the building have been measured and energy usage calculated. Energy produced by compression chiller have been estimated with calculated performance from design phase and adding heat transfer between rooms and supply ducts. Energy between rooms and supply ducts were not included in energy calculation during the design phase. According to the control system for the DCV system there have been some issues with high temperature in supply ducts even when they are supplied with 15 ºC from air- handling unit. There have been measurement to the ventilation system 5701-5704 that is connected to a close-loop duct with a result of temperatures between 15.2 ºC up to 21.4 ºC and the velocity has varied between 0.05-2.1 m/s in different measurement spots. This is an increase of 6.4 ºC. A heat transfer calculation have been made in Paroc Calculus to estimate heat transfer between room and supply ducts. The results of this calculation indicates the same level of temperature increases as when the system was measured. With no thermal insulation cooling capacity is lost to half after less than 5 m with a velocity of 0.2 m/s, after 15 m with a velocity of 1 m/s and 30 m with a velocity of 2 m/s . This should be compared with supply duct with 20 mm of thermal insulation that has lost its cooling capacity after less than 13 m with a velocity of 0.2 m/s, after 63 m with a velocity of 1 m/s and is increase with 4 ºC after 100 m with a velocity of 2 m/s. Using closed-loop ducts with velocity below 2.0 m/s and without thermal insulation combined with under tempered supply air is not a good combination. Even short length with low velocity and lack of thermal insulation is devastating because of heat transfer according to logarithmical temperature difference between room and supply ducts. A closed-loop duct is often designed as a pressure chamber and recommended when using DCV and/or VAV ventilation to avoid problems with noise and to be able to reduce the need of dampers. Problems with temperature increasing according to velocity in ducts must be taken in consideration. For Ljusåret 2 this will affect district heating usage where ducts are placed because underfloor heating must compensate heat transfer. Chilled water must be provided an extra time for rooms with both DCV and chilled beams and rooms with only DCV is less comfortable which they could been with a correct installation.

Estudo experimental, simulação numérica e modelagem fenomenológica da separação gravitacional de gás no fundo de poços direcionais / Experimental study, numerical simulation and phenomenological modeling of gravitational separation of gas in down-hole directional wells

Propõe-se o estudo do separador gravitacional de fundo de poço do tipo shroud invertido para poços direcionais e horizontais. A geometria inovadora é observada pela inclinação imposta, solução que transforma um escoamento vertical descendente, veloz e caótico, num escoamento inclinado e segregado em canal livre. Com a inclinação, minimizasse a incorporação de gás, no impacto do escoamento em superfície livre contra a interface de líquido formada pelo nível do anular interno (NAI), e a segregação é incrementada devido ao gradiente de velocidades e ao aparecimento da componente da gravidade na direção radial do poço de petróleo. O trabalho experimental tem como uma de suas metas o levantamento da eficiência de separação em função das vazões de líquido e gás, do ângulo de inclinação e o grau de interferência do escoamento no duto anular formado entre o separador e a parede do poço de petróleo, chamado neste trabalho de anular externo. Foi feito um trabalho de semelhança dimensional e foram detectados os números adimensionais pertinentes ao problema. Constatou-se que a vazão de gás e o escoamento no anular externo não interferem na eficiência de separação. Além disso, foi realizado um trabalho de caracterização dos padrões do escoamento gás-líquido em duto anular, através de visualização e análise no domínio do tempo e da frequência do sinal dinâmico da queda de pressão. Também são propostos dois modelos matemáticos, um modelo fenomenológico baseado em princípios físicos fundamentais, que foi capaz de prever com eficácia a região de máxima eficiência do separador shroud invertido, e um modelo numérico, que reproduziu a fenomenologia do processo de separação do gás. / This research project proposes the study of the inverted-shroud gravitational gas separator for directional and horizontal wells. The innovative geometry is observed by the inclination that transforms a vertical, fast and chaotic downward flow into an inclined and segregated free channel flow. Due to inclination, the incorporation of gas at the internal gasliquid interface (NAI) is minimized; the segregation is increased due to the velocity gradient and the gravitational term that arises in the radial direction of the oil well. One of the aims of the experimental work is the obtaining of new data of gas separation efficiency as a function of the flow of liquid and gas, inclination angle and the evaluation of the degree of interference of the external annular duct flow. A dimensional analysis was undertaken to identify the relevant dimensionless numbers. It was found that the gas flow and the flow pattern in the external annular duct do not interfere in the separation efficiency. A flow pattern characterization was carried out through visualization and time and frequency domain analysis of differential pressure signature signal. Two mathematical models are proposed, a phenomenological model based on fundamental physical principles, which was able to predict with good accuracy the region of maximum separation efficiency, and a numerical model, which reproduced the phenomenology of the gas separation process.

Investigação da distribuição de tamanho de bolhas em um separador gás-líquido do tipo shroud invertido / Investigation of bubble-diameter distribution in a gas-liquid inverted-shroud separator

Operações de produção de petróleo, que utilizam sistemas de bombeamento centrífugo submerso, constantemente encontram a presença de gás livre nos poços, o que pode gerar ou agravar problemas como cavitação e falhas dinâmicas, quando o gás é succionado pela bomba. O separador gravitacional do tipo shroud invertido é uma solução possível para este problema nos casos de operação em poços direcionais de petróleo. O trabalho tem como objetivo apresentar um estudo do diâmetro das bolhas que ocorrem no interior de um separador gravitacional gás-líquido do tipo shroud invertido. A finalidade é o aprimoramento de um modelo fenomenológico do funcionamento deste tipo de separador, aplicado à indústria petrolífera, sendo que o modelo fenomenológico garante total separação de gás, fornecidas determinadas condições. O modelo prevê, através do cálculo da energia cinética turbulenta, o tamanho médio das bolhas carregadas para o seio do líquido por aeração no duto anular. Partindo de estudos anteriores, uma verificação do modelo fenomenológico foi feita utilizando um aparato experimental com misturas bifásicas ar-água e ar-óleo em três diferentes inclinações. O aparato possui dimensões radiais reais de um poço de petróleo offshore. Foi utilizado um sensor 3D ORM para a medição do tamanho médio (sauter) das bolhas arrastadas pelo líquido até a entrada do tubo de produção, em diversas combinações de vazões da mistura água-ar. As descobertas provenientes deste estudo foram implementadas em um código computacional que será utilizado pela PETROBRAS, financiadora do projeto, para suas operações de bombeamento. / Oil mining operations powered by centrifugal submersible pumping systems suffer constant setbacks due to the presence of free gas in wells. Decompression in the reservoir liberates this gas in the form of bubbles that, upon reaching the suction end of the pump, cause cavitation and dynamic failures resulting in production and equipment losses. The Inverted-shroud gravitational separator is a possible solution to this problem. This work presents a study on diameters of bubbles that occur inside this separator. The goal is the improvement of the understanding of this kind of separator as well as the enhancement of a previously reported phenomenological model, which ensures total gas separation when the separator is installed in directional wells and under specific operational conditions. Empirically adjusted correlations are used to ensure that all entrained bubbles do not reach the pump. The model was tested for two-phase flows of water-air and oil-air mixtures using three different inclinations. Tests were performed with an experimental apparatus that simulates a pilot-scale well casing with an inverted-shroud separator installed. A 3D ORM particle-size sensor was employed in order to measure the average (sauter) diameter of entrained bubbles that are dragged by the liquid flow towards the end of the production tube. This investigation will be used to improve the reliability of the phenomenological model and reduce its dependency on a theoretical prediction of the bubble size. The findings were incorporated to the final version of an in-house gas separator design software developed at the request of PETROBRAS, the project funder and Inverted-shroud patent holder, for usage on its oil mining operations.

Estudo termoanalítico e caracterização química de cálculos biliares e bile humana / Thermoanalytical study and chemical characterization of gallstones and human bile

Este trabalho consiste na caracterização química e termoanalítica da composição química de cálculos biliares e bile humana, coletados da vesícula biliar de pacientes que foram submetidos à colecistectomia no Hospital Universitário da Universidade de São Paulo. As metodologias utilizadas para amostragem, caracterização química e termoanalítica foram previamente otimizadas por um estudo preliminar, trabalhando-se com um total de 38 amostras de cálculos biliares e bile da vesícula biliar humana. A técnica de análise elementar para a determinação dos teores de carbono, hidrogênio e nitrogênio foi usada para a caracterização química junto com a espectroscopia de absorção na região do infravermelho e difração de raios X. Esses resultados foram complementados por dados obtidos pelas técnicas termoanalíticas TG/DTG e DSC. O estudo do comportamento térmico foi realizado empregando as técnicas TG/DTG em atmosfera dinâmica de ar e razão de aquecimento de 10 ºC min-1. No caso da amostra de cálculo biliar SF15, considerada atípica por apresentar um teor de 81% de CaCO3 na sua composição química, utilizou-se atmosfera mista de ar+CO2. Em todos os ensaios foram empregados. Os eventos térmicos (físicos e químicos) foram evidenciados pelas curvas DSC, por picos no sentido endotérmicos e exotérmicos. A comparação dos perfis das curvas TG/DTG e os resultados de perdas de massa evidenciaram junto com os teores de CHN obtidos por análise elementar, possibilitaram classificar os cálculos biliares em oito grupos distintos (A, B, C, D, E, F, G e H). As curvas DSC, os espectros de infravermelho e os difratogramas de raios X, confirmaram a distinção da composição química desses grupos. Deste modo, foi possível a elaboração de uma nova classificação para os cálculos biliares da vesícula biliar humana baseado na sua composição química. / This work consists of the chemical characterization and thermoanalytical of the chemical composition of gallstones and human bile, collected of the gallbladder of patients that were submitted to cholecystectomy at University Hospital of São Paulo University. The methodologies used to sampling, chemical characterization and thermoanalytical were previously optimized for a preliminary study, working with a total of 38 gallstones and human bile samples. For chemical characterization elementary analysis techniques, to carbon, hydrogen and nitrogen determination, infrared spectroscopy and X-rays diffraction were used, these results complemented the obtained thermoanalytical data for TG/DTG and DSC techniques. The gallstones and bile samples thermal behavior study was accomplished using the techniques TG/DTG in dynamic air atmosphere. In the case gallstone SF15 sample, considered like abnormal due to its content of 81% of CaCO3 in its chemical composition. For this one, a mixture air + CO2 was used. In all experiments were used heating rate of 10 °C min-1. The thermal events (physical and chemical) were evidenced by the DSC curves, with endothermic and exothermic peaks. By means of the comparison TG/DTG curves profiles, the results of mass losses for them evidenced, with CHN\'s contents obtained for elementary analysis, they can classify the gallstones in eight distinct groups (A, B, C, D, E, F, G and H). The DSC curves, infrared spectra and X-rays diffraction, they confirmed the chemical composition distinction of these groups. Thus, it was possible the elaboration of a new classification for the gallstones formatted in the human gallbladder based in your chemical composition.

Avaliação da percepção auditiva da fala em pacientes submetidos ao reimplante coclear / Evaluation of auditory speech perception in patients submitted to cochlear reimplantation

Estudo retrospectivo, descritivo e longitudinal realizado na Seção de Implante Coclear, Centro de Pesquisas Audiológicas do Hospital de Reabilitação de Anomalias Craniofaciais da Universidade de São Paulo (IC-CPA/HRAC/USP). O objetivo deste estudo foi caracterizar o desempenho da percepção auditiva da fala de indivíduos submetidos à cirurgia de reimplante coclear. No total de 1.323 indivíduos submetidos à cirurgia de implante coclear no período de 1990 a janeiro de 2016, consta o registro de 84 indivíduos submetidos à cirurgia de reimplante coclear, correspondendo a 6,3% do total. Deste total de 84, 68 indivíduos atenderam aos critérios de inclusão/exclusão da pesquisa e foram avaliados utilizando-se testes de percepção auditiva da fala, em três momentos: antes da segunda intervenção cirúrgica, após o reimplante e última avaliação de acompanhamento. As variáveis estudadas foram: a) motivo do reimplante coclear; b) etiologia da perda auditiva; c) idade na primeira cirurgia; d) tempo de uso do implante coclear antes do reimplante; e) tempo de recuperação do escore anterior à intervenção; f) tempo de uso atual; g) escore nos procedimentos de avaliação da percepção auditiva da fala, pré e pós-reimplante coclear. Na análise dos dados foi possível observar que a causa mais comum do reimplante foi a falha do dispositivo interno, seguido de razões médicas. Na avaliação auditiva da fala, um total de 61 (89,7%) recuperaram o desempenho observado antes do reimplante coclear e sete (10,3%), não alcançaram o mesmo desempenho. Constatou-se que as variáveis idade do primeiro Implante e tempo de uso do primeiro Implante Coclear, assim como tempo de reimplante, influenciaram nos resultados encontrados. A cirurgia de reimplante coclear é um procedimento viável, contudo a família e o paciente devem ser orientados quanto aos possíveis comprometimentos na percepção auditiva da fala / A retrospective, descriptive and longitudinal study performed in the Cochlear Implant Section, Audiological Research Center of the Hospital of Rehabilitation of Craniofacial Anomalies of the University of São Paulo (IC-CPA / HRAC / USP). This study aimed at characterizing the auditory speech perception of individuals undergoing cochlear reimplantation surgery. Out of 1,323 individuals undergoing cochlear implant surgery from 1990 to January 2016, 84 (6.3%) subjects underwent cochlear reimplantation surgery. From this total of 84, 68 individuals met the inclusion / exclusion criteria of the study and were evaluated using auditory speech perception tests in three moments: before the second surgical intervention, after reimplantation and the last follow-up evaluation. The variables studied were: a) reason for cochlear reimplantation; b) etiology of hearing loss; c) age at first surgery; d) time cochlear implant use before reimplantation; e) recovery time from the previous intervention score; f) time of current use; g) score in the procedures of assessment of auditory speech perception before and after cochlear reimplantation. The data analysis showed that the most common cause of reimplantation was the failure of the internal device, followed by medical reasons. In the auditory speech evaluation, a total of 61 (89.7%) recovered performance before cochlear reimplantation, and seven (10.3%), did not reach the same performance. It was verified that the variables age at the first Implant and time using the first Cochlear Implant, as well as time of reimplantation, influenced the results. Cochlear replantation surgery is a viable procedure, however, the family and the patient should be advised on the possible compromising risks to the auditory speech perception
