As redes móveis heterogêneas já são realidade e sua densificação tem sido apontada como uma das soluções propostas para atendimento das demandas esperadas para as redes celulares de quinta geração (5G). Nas redes atuais, no entanto, ainda é comum o uso de parâmetros fixos na configuração da rede, mas esta estratégia nem sempre se mostra eficiente. É dentro deste contexto que se consolida o conceito de redes auto organizáveis nas quais os vários parâmetros da rede são ajustados automaticamente com base em medições em tempo real e sistemas inteligentes. Esta tese apresenta uma estratégia para otimização de handover em redes LTE com densa implantação de small cells. Com base em medições e lógica fuzzy são propostos algoritmos para o auto ajuste de parâmetros da rede. A partir de simulação discreta utilizando o MATLAB, os resultados são obtidos e apresentados através das principais métricas de avalição de desempenho de handover. / The increase in mobile devices and applications in recent years has led to an overload of the network infrastructure responsible for disposing this traffic, thus affecting the performance of the network as the user experience.Heterogeneous mobile networks are already a reality and their densification has been put forward as one of the suggested solutions to meet the expected demands for 5th generation (5G) mobile networks. However, in the current networks, it is still common for fixed parameters to be used for the configuration of the network although this strategy does not always prove to be efficient. It is within this context that the concept of selforganizing networks (SONs) has been established, in which several network parameters are automatically adjusted on the basis of measurements and intelligent systems in real time. This thesis presents a strategy to optimize the handover in LTE networks with dense small cells. Based on measurements and fuzzy logic new algorithms are proposed for self-tuning network parameters. From discrete simulation using MATLAB, the results are evaluated and presented through the main performance metrics of handover.

Strategier och faktorer av betydelse för patientsäkerheten vid överrapportering: En litteraturöversikt / Strategies and factors of importance to patient safety in case of handover: A Literature Review

Bakgrund: Överrapportering av patienter inom slutenvården är ett vanligt förekommande moment för sjuksköterskor. Det finns strategier och faktorer som har betydelse för patientsäkerheten vid överrapportering av patienter mellan sjuksköterskor inom sluten hälso- och sjukvård. Genom att uppmärksamma strategier och faktorer som har betydelse för överrapportering av patienter inom slutenvården kan en säkrare patientvård bedrivas. Syfte: Att belysa strategier och faktorer som har betydelse för patientsäkerheten vid överrapportering av patient mellan sjuksköterskor inom sluten hälso- och sjukvård. Metod: Designen som använts var en litteraturöversikt. Litteraturöversikten grundade sig på 15 vetenskapliga artiklar som var av kvalitativ, kvantitativ eller mixad metod. Datainsamling har gjorts via PubMed och Cinahl. Resultat: Resultatet av inkluderade artiklar visade att strategier och faktorer hade betydelse för patientsäkerheten vid överrapportering. Strategier som var viktiga att belysa var "förberedelse inför rapport, frågor, bedside handover och rapporteringsmallar som Situation, Bakgrund, Aktuellt och Rekommendation (SBAR) och head to toe". Faktorer som beskrevs i resultatet var "rapportens innehåll, tidsbrist, teamsamarbete och attityder, miljö och teknik". Slutsats: Litteraturöversikten slutsats kom fram till att flera strategier hade en inverkan på överrapportering, dessa var "förberedelse inför rapport, frågor, bedside handover och rapporteringsmallar som SBAR och head to toe". Faktorer hade också en inverkan på överrapportering och dessa var "rapportens innehåll, tidsbrist, teamsamarbete och attityder, miljö och teknik." Dessa strategier och faktorer har positiv och negativ inverkan på sjuksköterskans arbete mot en säker vård och patientsäkerhet. / Background: Reporting of a patients within inpatient healthcare was a common occuring moment for nurses. By paying attention to strategies and factors of relevance for reporting of should it be possible with a more safe patient care. Aim: To illustrate strategies and factors of relevance patient safety when reporting of patients between nurses within inpatient healthcare. Method: The design used was literature review. The review was based on 15 scientific articles of qualitative, quantitative or mixed method. The data have been collected from PubMed and Cinhal. Result: The result showed that strategies and and factors was of importance for the patient safety when reporting of. Strategies important to focus on was "preparation before reporting of, questions, bedside handover and reporting forms as Situation, Background, Assessment and Recommendations (SBAR) and head to toe. Factors described in the result were " content of the report, lack of time, team cooperation and attitudes, environment and technique. Conclusion: The conclusion of the literature review was that a number of strategies have impact on reporting of. These strategies are "preparation before reporting of, questions, bedside handover and reporting forms as SBAR and head to toe". Even factors have an impact on reporting of. Theses are "reporting content, lack of time, team cooperation and attitudes, environment and technique. These strategies and factors have impact on the work nurses conduct to secure healthcare and patient safety.

Esta tese apresenta uma arquitetura para redes da próxima geração capaz de fornecer acesso heterogêneo sem fio e handover vertical transparente para as aplicações multimídia. A tese considera diferentes tecnologias sem fio e também adota o padrão IEEE 802.21 (Media Independent Handover – MIH) para auxiliar na integração e gerenciamento das redes heterogêneas sem fio. As tecnologias que a arquitetura possui são: IEEE 802.11 (popularmente denominada de WiFi), IEEE 802.16 (popularmente denominada de WiMAX) e LTE (popularmente denominada de redes 4G). O objetivo é que arquitetura tenha a capacidade de escolher entre as alternativas disponíveis a melhor conexão para o momento. A arquitetura proposta apresenta mecanismos de predição de Qualidade de Experiência (Quality of Experience - QoE) que será o parâmetro decisivo para a realização ou não do handover para uma nova rede. A predição para determinar se haverá ou não mudança de conectividade será feita com o uso da inteligência computacional de Redes Neurais Artificiais. Além disso a arquitetura também apresenta um mecanismo de descarte seletivo de pacotes especifico para aplicações multimídia. A proposta é avaliada via simulação utilizando-se o ns-2 (Network Simulator) e os resultados de desempenho são apresentados através das métricas de QoS, de QoE e também visualmente através da exibição de frames dos vídeos transmitidos na arquitetura. / The Next Generation Networks are architectures that allow vertical handovers and the user-centric vision by the appropriate Quality of Experience (QoE) provisioning for multimedia applications. The discussion is much less protocol-oriented perspective and is more service-oriented perspective. This thesis presents an architecture for next-generation networks to provide wireless heterogeneous access and seamless vertical handover for multimedia applications. The proposal considers different wireless technologies and also adopts the standard IEEE 802.21 (Media Independent Handover – MIH) to assist in the integration and the mobility management of heterogeneous wireless networks. The technologies in architecture are: IEEE 802.11 (popular known as Wi-Fi), IEEE 802.16 (popular known as WiMAX) and LTE (popular known as Fourth Generation – 4G). The objective is to choose the best connection for user. The proposal architecture presents mechanisms to predict quality of experience that will be decisive parameter to do or do not the handover, the prediction will be using artificial intelligence neural networks, in addition to architecture also provides a mechanism for QoE discard packets to specific multimedia applications. The proposal is evaluated by simulation using the ns-2 (Network Simulator) and the performance results are presented through the QoS/QoE metrics and also visually by displaying the video frames transmitted in architecture.

Frontline and Middle-Level Nursing Leader Transition Within the Military Health System

Healthcare delivery within the military requires a multifaceted approach to achieve the desired outcomes of safe, effective, patient-centered, timely, efficient, and equitable health care. The prospect of maintaining a cycle of continuous process improvements within military clinical settings hinge on frontline leaders and middle-level managers who must be prepared to execute the mission and motivate, supervise, coach and mentor the staff. This project showcases a review of current literature translated into the development of an evidence-based Transformational Leadership Induction Program (TLIP) module that consisted of 4 subsections: the environment of care, clinical decision support systems, human resources management, and change management as well as resources for successful leadership within the organization. The training is designed to bridge the transition gap, facilitate role orientation and induction, and socialize frontline and middle-level managers during their role transition. The results of a need assessment survey, approved by the organization, were completed by 30 incumbents and resulted in 57% (17) providing feedback and role-specific contents that were integrated into the development of the TLIP module. The overall response to the survey was positive with 82% (14) of the respondents either agreeing or strongly agreeing with the items that they reviewed. The TLIP module provides a medium that translates current evidence into a succinct training platform capable of enhancing leadership transition and handoff. The TLIP module enables a culture of trust, enhances staff satisfaction, and fosters change management and succession planning within the military healthcare system.

Överlämning och rapportering av patienten mellan olika funktioner och personal skall ske så att patientens omvårdnadsbehov tillgodoses på ett tillfredsställande sätt. Studiens syfte var att beskriva överlämnande och rapportering i patientens hem från distriktssköterska till ambulanspersonal. Postenkät, med slutna frågor av Likert typ och med möjlighet att motivera svaren, har använts i studien. Databearbetningen har gjorts med statistical package for social sciencies (SPSS). Enkäten avslutades med fem öppna frågor, vars svar analyserats enligt kvalitativ innehållsanalys. Studien beskriver hur uppfattningarna mellan distriktssköterskor och ambulanspersonal inte alltid är samstämmiga. Om ambulanspersonal frågar efter eller om distriktssköterskor lämnar viss information vid överlämning av patient. Professionernas insikt i varandras yrken är begränsad och en vilja uttrycktes om att öka denna kunskap. Detta kunde vara ett steg i riktning mot att förbättra kommunikationen i överlämnandet och därmed säkra att viktig information följer patienten genom hela vårdkedjan. Exploitation de la diversité des réseaux

Aujourd'hui, les utilisateurs mobiles intègrent plusieurs interfaces sans fil dans leurs dispositifs mobiles, tels que IEEE 802.11, des technologies cellulaires 2G/3G/4G, WiMAX ou Bluetooth, car ces technologies hétérogènes peuvent fournir un accès Internet dans les zones urbaines. Dans ce contexte, il existe un potentiel pour les utilisateurs mobiles d'exploiter la diversité des interfaces sans fil, afin d'être connectés aux réseaux de la meilleure manière possible, à tout moment et partout. Cependant, afin de profiter de cette diversité des réseaux il est nécessaire d'avoir une gestion efficace de la mobilité et de la multi-domiciliation. En ce qui concerne la mobilité, les utilisateurs mobiles ont besoin de découvrir les réseaux sans fil et basculer entre des points d'accès d'une façon transparente et sans coupures. Afin de supporter la multi-domiciliation et de permettre l'utilisation de plusieurs réseaux sans fil simultanément, il est nécessaire de définir des mécanismes de sélection des réseaux visant à attribuer les flux d'applications aux différentes inter- faces sans fil d'une manière optimale. Dans cette thèse, nous avons d'abord caractérisé la diversité des réseaux en explorant et en analysant les performances des déploiements sans fil actuelles dans les zones urbaines, en particulier les réseaux cellulaires et les réseaux communautaires basés sur IEEE 802.11. Ensuite, nous avons étudié la mobilité dans les réseaux IEEE 802.11, particulièrement le processus de découverte des points d'accès, en fournissant deux algorithmes adaptatifs qui visent à utiliser les paramètres de découverte les plus appropriés dans chaque scénario. Nous évaluons ces algorithmes par l'expérimentation et nous comparons leurs performances par rapport aux stratégies utilisant des paramètres par défaut. Enfin, nous étudions la sélection des réseaux dans un scénario multi-domicilié et nous proposons un algorithme de prise de décision pour trouver l'attribution optimale des flux aux différentes interfaces, en prenant en compte des critères de qualité de service et de consommation d'énergie. Cet algorithme de décision est modélisé par un problème d'optimisation multi-objectif et est résolu avec des algorithmes génétiques. Nous évaluons, par le biais de simulations, les performances de notre approche contre des algorithmes de décision basés sur des préférences.

Advanced Routing Protocols for Satellite and Space Networks

Satellite systems have the advantage of global coverage and offer a solution for providing broadband access to end users. Local terrestrial networks and terminals can be connected to the rest of the world over Low Earth Orbit (LEO) satellite networks simply by installing small satellite interfaces. With these properties, satellite systems play a crucial role in the global Internet to support real-time and non-real-time applications. Routing in satellite networks, and the integration of satellite networks and the terrestrial Internet are the key issues to support these services. Furthermore, the developments in space technologies enable the realization of deep-space missions such as Mars exploration. The Interplanetary Internet is envisioned to provide communication services for scientific data delivery and navigation services for the explorer spacecrafts and orbiters of future deep-space missions. The unique characteristics posed by deep-space communications call for different research approaches from those in terrestrial networks. The objective of this research is to develop advanced architectures and efficient routing protocols for satellite and space networks to support applications with different traffic types and heterogeneous quality-of-service (QoS) requirements. Specifically, a new QoS-based routing algorithm (QRA) is proposed as a connection-oriented routing scheme to support real-time multimedia applications in satellite networks. Next, the satellite grouping and routing protocol (SGRP) is presented as a unicast routing protocol in a two-layer satellite IP network architecture. The border gateway protocol - satellite version (BGP-S) is then proposed as a unified routing protocol to accomplish the integration of the terrestrial and satellite IP networks at the network layer. Finally, a new routing framework, called the space backbone routing (SBR), is introduced for routing through different autonomous regions in the Interplanetary Internet. SBR provides a self-contained and scalable solution to support different traffic types through the Interplanetary Internet.
