Aftercare to chemically addicted adolescents : practice guidelines from a social work perspective

The susceptibility of adolescents to chemical addiction has become a major international concern. Approximately 25% of people in Central Asia and Eastern Europe who inject chemical substances are under the age of 20 years (Youth at the United Nations, 2006), while up to 75% of unintentional injuries among adolescents in America are related to substance abuse (Page & Page, 2003:196). On the national level, approximately 25% of adolescents under the age of 20 are involved in substance abuse (Western Cape Department of Social Services and Poverty Alleviation Transformation Plan, 2006:13). Focusing on the Western Cape, a report from the South African Epidemiology Network (2007:3) highlights that the youngest patient in in-patient treatment was nine years of age, and among 2 798 persons who received in-patient treatment, 27% were under the age of 20, more than any other age group in treatment. Treatment of adolescent chemical addiction should include preparation for treatment, treatment, and also aftercare services to ensure that the addicted adolescent develops skills to maintain sobriety (Meyer, 2005:292-293). Section Six of the South African Prevention and Treatment of Drug Dependency Act (1992) prescribes that chemically addicted persons should have access to professional aftercare services to ensure that treatment is not terminated prematurely. The motivation for this study was based on the fact that, despite this statutory requirement, the Western Cape Drug Forum (2005:3) identified the need for the development of aftercare services in 2005, indicating the lack of focus on aftercare as part of treatment. This concern was confirmed by practitioners in the field of adolescent chemical addiction and findings resulted from previous research regarding relapse experiences of chemically addicted adolescents (Van der Westhuizen, 2007:129-130). / Social Work / D. Afrikaans text / Die studie ondersoek die leemte aan teoreties gefundeerde riglyne om pleegouers in die bekendmakingsproses van hulle kind se MIV-positiewe status te ondersteun Die studie geskied binne die teoretiese raamwerk van ʼn intervensienavorsingsmodel. In die empiriese navorsing is die vrese en probleme van pleegouers by die bekendmakingsproses ondersoek. Daar is bevind dat hierdie vrese en probleme in ag geneem moet word in die beoogde riglyne. Die kognitiewe ontwikkeling van die kind is deur middel van ʼn literatuurstudie ondersoek om vas te stel watter kennis, denke en inligting die kind op verskillende ouderdomme begryp. Daar is bevind dat kinders wat MIV-positief is, moontlike agterstande ondervind en dat die riglyne gevolglik gebaseer moet word op die toenemende ontwikkeling van die kind, met begrip vir individuele ontwikkelingsverskille. Voorts is bevind dat pleegouers se eie gereedheid ʼn belangrike rol speel. Alhoewel die pleegouer die bekendmakingsproses self moet aanvoer en hanteer is dit belangrik dat die pleegouers deur ʼn interdissiplinêre span ondersteun en begelei sal word. / This study investigates the absence of theoretical grounded guidelines to support foster parents when disclosing paediatric HIV. The study was done within the theoretical framework of an intervention research model. The empirical research investigated the fears and problems experienced by foster parents during the disclosure process. It was found that these fears and problems should be taken into account when compiling guidelines. The child‟s cognitive development was researched by way of a literary study in order to determine what knowledge, thoughts and information the child is able to grasp at different ages. It was found that children who are HIV positive could possibly lag behind and that the guidelines should therefore be based on the accumulative development of the child, with consideration given to individual developmental differences. Furthermore, it was established that foster parents‟ own readiness plays an important role. For this reason the foster parents should be supported and guided by an interdisciplinary team, but that they themselves should take care and execute the process of disclosure. / Social Work / M. De « facture classique », cette thèse se veut particulièrement originale tant dans le choix de l’objet investigué que dans les résultats obtenus. En s’attachant à mieux comprendre la dynamique qui a prévalu au cours de l’intégration stratégique d’un projet culturel et managérial en faveur de la diversité dans une organisation du Supérieur, ce travail s’inscrit à la lisière de la stratégie et de la GRH. Il prend son origine dans le cadre général des études traitant de la non-discrimination et du management de la diversité en France, en s’intéressant toutefois à une organisation particulière puisqu’il est question de la Business School, dont le rôle socioéconomique est singulier. En effet, elle produit la future génération de managers et dans une moindre mesure les futurs leaders et décideurs économiques.Ce travail s’intéresse donc à l’application de l’approche conceptuelle et managériale de la diversité dans ce type d’organisation et aux changements induits par une telle démarche dans le cadre d’une étude processuelle longitudinale de cinq ans d’un cas unique (monographie). Reposant sur une ethnométhodologie à visée transformative, cette thèse restitue un changement en train de se faire en mobilisant l’approche contextualiste de Pettigrew. / Despite its classical style, this thesis is highly original in both the subject of the investigation – the strategic integration of diversity into the organizational culture of a French business school – and its results. Focused on the social responsibility of French higher education establishments, this thesis addresses issues that lie at the boundary of strategy and HRM. It targets the institutions that could be called the ‘antechamber’ of the business world, and that produce the next generation of managers: leading business schools. Diversity and non-discrimination at work are recent streams of research in the French managerial literature and this work takes a detailed look at the dynamics of change of a phenomenon that has received little attention: the construction of an organizational culture that fosters diversity. As the emphasis is on ‘organized’ and ‘organizing’ action rather than the institution itself, an intervention research strategy is adopted to investigate ongoing organizational change. This single-domain (monograph), longitudinal and process-based case study was carried out in situ, while Pettigrew’s contextualist approach provided the framework for a detailed description and in-depth analysis.

Alors que nous faisons face à une crise biologique majeure, acteurs institutionnel et ONGs exhortent les entreprises à s’engager pour enrayer cette érosion écologique. L’objectif de la thèse est d’analyser, évaluer et faire évoluer la contribution potentielle des entreprises à la conservation de la biodiversité dans le cadre de leur responsabilité sociétale (RSE).Un cadre d’analyse est développé pour évaluer la prise en charge par une entreprise de ses responsabilités écologiques en termes de moyens – les initiatives RSE a priori favorables à la biodiversité mises en œuvre – et de résultats – la compatibilité des activités de l’entreprise avec la conservation de la biodiversité. Après avoir caractérisé les initiatives RSE à partir de l’étude d’engagements d’entreprises endossés comme contributions à la Stratégie nationale pour la biodiversité, une méthode est proposée pour détecter si les activités d’une entreprise sont écologiquement non-durables. Le postulat est que le respect des capacités de charge des écosystèmes est un prérequis à la conservation effective de la biodiversité. Cette méthode, adaptée de l’évaluation environnementale absolue de la durabilité, est appliquée, dans le cadre de deux projets de recherche-intervention, au portfolio alimentaire du Groupe Casino et au cycle de vie de deux installations de stockage de déchets dangereux de SARP Industries. Des orientations stratégiques visant à éviter-réduire-compenser les pressions non durables sont proposées. Soulignant l’importance de renforcer la prise en charge des pressions étendues, cette thèse ouvre plus largement des perspectives pour les secteurs d’activité et les politiques publiques. / As we are experiencing a major biological crisis, institutional actors and NGOs are calling on businesses to engage efforts aiming at halting biodiversity loss. The objective of the thesis is to analyze and evaluate the potential contribution of corporate social responsibility (CSR) to biodiversity conservation.A framework is developed to analyze to which extent a company takes in charge its ecological responsibilities. After characterizing CSR initiatives based on the case study of business commitments to contribute to the French National Biodiversity Strategy, a method is proposed to test whether business activities exert unsustainable pressures on ecosystems. It is assumed that compatibility with ecosystems’ carrying capacities is a prerequisite for effective biodiversity conservation. This method, adapted from the “absolute environmental sustainability assessment” (AESA) approach, is applied in the context of research-intervention projects to the food portfolio of the mass-market retailer Groupe Casino and to the life cycle of two facilities of SARP Industrie specialized in the disposal and storage of hazardous waste. The comparison of the company’s environmental footprints with the ecological constraints is used to draw a comprehensive strategy based on the mitigation hierarchy. As our results highlight the opportunity to strengthen the management of extended pressures through CSR, possible implications for sectors and public policies are discussed.

Les entreprises de spectacle vivant évoluent dans un contexte de diminution de ressources externes et de modification des modalités d’attribution des subventions par l’application de la LOLF (Loi Organique relative aux Lois de Finances). Leur problématique repose sur les moyens à mettre en œuvre pour faire face à ces contraintes. Nous nous attachons à démontrer que leur développement dépend de l’amélioration de la logique de coopération entre les acteurs à partir du projet artistique et de l’appropriation des contraintes d’évaluation de la LOLF, pour révéler les performances cachées et développer des ressources internes. Cette recherche se construit à partir de l’hypothèse que l’amélioration de la performance globale des organisations dépend d’une action transformative qui opère selon trois axes interdépendants : l’intervention, les outils de management, le langage–acteur. A partir d’une recherche-Intervention dans un théâtre et d’un diagnostic qualitatif dans un autre, nous accompagnons les acteurs dans une conduite de changement et étudions les conditions de développement managérial dans ce type d’organisation. Nous analysons comment l’utilisation du langage dans le cadre de l’Intervention Socio-Economique modifie les représentations et contribue à l’élaboration d’un nouveau dispositif managérial. Nous proposons d’associer les outils de management socio-Économiques, utilisés comme des matrices de lecture de l’organisation, aux critères d’évaluation et de subvention des pouvoirs publics pour en faire des outils de réflexion sur le processus de réalisation et de diffusion de spectacle et construire une représentation partagée entre les différents acteurs. / Performing arts companies operate in a context of declining resources and changes in the rules governing the allocation of grants, in accordance with the LOLF (Organic Law relative to the Laws of Finance). Their challenge lies in finding ways to address these constraints. We aim to demonstrate that their development depends on improving the manner in which all parties cooperate, based on the artistic project and taking into account the constraints resulting from the evaluation used by the LOLF, to reveal hidden costs and to develop internal resources. This research is based on the hypothesis that improving the overall performance of organizations depends on a transformative action that operates in three interdependent areas: intervention, management tools, and the speech-Actor. Based on intervention-Research in one theatre and on a qualitative study in another, we have assisted actors in the process of change management and examined the conditions in which managerial development takes place in this type of organization. We analyse how language use in the context of Socio-Economic Intervention modifies the representations and contributes to the elaboration of a new managerial instrument. We propose the use of socio-Economic management tools, serving as a framework for analyzing the organisation, combined with the criteria of evaluation and allocation of public funding as a basis of reflexion on the process of creation and dissemination of shows, as well as a means of building a shared representation between the different actors.

This research concentrates on the development of an intervention model for the sexually abused latency aged child. The focus is on short term, structured individual therapy. The importance of this model is linked to the underlying theoretical basis that it provides as well as the guidelines suggested for therapy. Intervention research was used as research methodology. This methodology implies a combined quantitative and qualitative approach. The first part (Chapters 2, 3&4) addresses the problem analysis and information gathering linked to the development of a theoretical basis for the intervention model. The latency years as developmental stage is discussed and suggestions are made as to which this information can be used during the planning and conducting of intervention. The reasons for the child’s vulnerability to become a sexual abuse victim are explained. The different forms of sexual abuse are indicated and it is argued that one child can be abused by another. The social environments where sexual abuse might occur are described as well as the possible consequences that sexual abuse holds for the victim. The second part of this research (Chapters 5&6) is related to the design of the intervention model, the conducting of a pilot test and the refining of the intervention. The key elements of abused focused therapy, cognitive behavioural therapy and movement therapy is outlined. These approaches serve as theoretical background for the formulation of the principles of the intervention model. The areas of focus during intervention are described. In Chapter 7 an outline of the intervention program is described. This programme can be viewed as a practical application of the intervention model. A detailed description of every therapy session is provided. The intervention program was tested on three children from the Jakaranda Children’s Home in Pretoria. Chapter 8 indicates the results of the empirical research as well as the interpretation thereof. The focus of this chapter is on the results as referring to the changes in the respondent’s experience of compliance, relationships, security and anxiety. The research results clearly shows that the three respondents were helped by the intervention and that they were no longer overwhelmed by their experience of the sexual abuse. A psychosocial educational programme for caregivers of people diagnosed with bipolar disorder

Bipolêre versteuring is ‘n kroniese, herhalende en ernstige siekte met ‘n voorkoms van 1 % wêreldwyd. Pasiënte kan psigoties word, selfmoordideasie hê en ook soms aggressief raak. Dikwels moet hulle gehospitaliseer word - ook teen hulle wil, want hulle mag soms geen insig toon nie. Pasiënte mag presenteer met ‘n ko-morbiede toestand, byvoorbeeld substansmisbruik of ‘n persoonlikheidsversteuring. Dit gebeur dikwels dat hulle nie hulle toegewysde rolle, byvoorbeeld dié van gade, broodwinner, ouer of volwasse kind, kan vervul nie. Versorgers van pasiënte gediagnoseer met bipolêre versteuring, word eweneens beïnvloed deur die siekte en die gepaardgaande stigma daarvan. Dit kan vir hulle baie moeilik wees wanneer hulle die verantwoordelikhede en rolle van die pasiënt moet oorneem. Beskou vanuit die oogpunt van ‘n ekologiese sisteemperspektief is die aard van hierdie impak op die maatskaplike funksionering van die versorgers relevant vir maatskaplike werk in die geestesgesondheidsveld. Die verkenning van hierdie impak en die ontwikkeling van ‘n intervensie om die impak aan te spreek het dus die rasionaal van hierdie studie gevorm. Die geïntegreerde biopsigososiale model, binne ‘n ekologiese sisteem perspektief, word aangewend by die psigiatriese instelling waar die navorser werk. Hierdie benadering het dus ook gedien as die vertrekpunt vir die betrokke studie. Die gemengde-metode navorsingsbenadering is tydens hierdie studie benut ten einde ‘n uitgebreide analise van die navorsingsprobleem te verskaf. Die voorgenome studie het binne die kategorie van toegepaste navorsing geval, want die doel daarvan was om ‘n program te ontwikkel om hulp te verleen aan versorgers van individue gediagnoseer met bipolêre versteuring. Die navorser het haar navorsing gerig aan die hand van Rothman en Thomas (1994) se intervensie ontwerp en ontwikkelingsmodel (D&D) deur gebruik te maak van kwalitatiewe en kwantitatiewe metodes. Na afloop van die analise van die kwalitatiewe data, is ‘n unieke psigo-maatskaplike opvoedkundige program, die SEE-SAW program, ontwikkel en toe geïmplementeer. Die konsep van balans versus wanbalans binne die sisteem is essensieel; die navorser se program het ten doel gehad om beter balans binne die sisteem van die pasiënt en versorger te bewerkstellig. Die gevolgtrekkings het duidelik getoon dat die versorger van die bipolêre pasiënt blootgestel is aan ‘n wye spectrum van behoeftes en uitdagings, maar dat intervensies soos die SEE-SAW program van groot waarde mag wees. / Bipolar disorder (BD) is a chronic, recurrent, serious illness that occurs in 1 % of people globally. Patients might become psychotic, suicidal and sometimes violent. They often need to be hospitalised, even against their will because they may have no insight. Patients may have a comorbid condition such as substance abuse or a personality disorder. They are frequently unable to fulfil their assigned roles of spouse, breadwinner, parent or adult child. Caregivers of patients diagnosed with BD are also affected by the illness and the associated stigma. It becomes very difficult for them when they have to take over the responsibilities and roles of the patient. The nature of this impact on the social functioning of the caregivers, understood from an ecological systems perspective, is relevant for social work in the mental health care field. Discovering more about this impact and developing an intervention to curb it therefore formed the rationale for this study. The integrated biopsychosocial model, within an ecological systems perspective, is being used at the psychiatric institution where the researcher works. This approach therefore also served as the point of departure from which this research was conducted. A mixed methods research approach was utilised to provide a comprehensive analysis of the research problem. The proposed study fell into the category of applied research, due to its aim of designing a programme to assist caregivers of people diagnosed with BD. The researcher directed the study according to Rothman and Thomas’s (1994) intervention design and development model (D&D), using qualitative and quantitative methods. After the analysis of the qualitative data a unique psychosocial educational programme, the SEE-SAW programme, was developed and then implemented. The concept of equilibrium versus disequilibrium in the system is central; the researcher’s programme therefore strove towards better equilibrium within the system of patient and caregiver. Le financement basé sur la performance au Cameroun : analyse de son émergence, sa mise en œuvre et ses effets sur la disponibilité des médicaments essentiels

L'accès aux médicaments essentiels (ME) est un élément clé de la qualité des soins dans un système de santé. Par ailleurs, le financement basé sur la performance (FBP) attire de plus en plus l'attention des décideurs comme une intervention pour améliorer la prestation des services de santé, y compris l’accès aux ME, dans les pays à faible et moyen revenus (PFMR). Malgré l’intérêt croissant de la recherche sur le FBP, très peu d’étude ont porté sur la mise à l’agenda d’une telle réforme ou son maintien à l’ordre du jour au fil du temps, encore moins sur l’influence de celle-ci sur l’accès aux ME dans les PFMR. A travers une analyse du programme de FBP au Cameroun, la présente thèse vise à faire avancer les connaissances en examinant les questions suivantes : qu’est-ce qui explique l’apparition du FBP au niveau de la politique nationale de la santé et quel est l’impact de ce programme sur l’accès aux ME? Le devis de recherche est celui d’une étude de cas et la démarche analytique s’appuie sur la combinaison des données qualitatives, à travers des entrevues réalisées auprès des acteurs clés du programme FBP au Cameroun, et quantitatives, issues de l’évaluation d’impact de ce programme. La perspective conceptuelle est celle des cycles de politique, du cadre de transfert des politiques et de la recherche interventionnelle. Les résultats sont structurés en quatre articles scientifiques. La mise du FBP à l’agenda au Cameroun s’est construite à partir des rapports et événements identifiant l'absence d'une politique de financement de la santé adaptée comme une question importante à laquelle il fallait s'attaquer (article 1). L'évolution du discours politique vers une plus grande responsabilisation a permis de tester de nouveaux mécanismes. Un groupe d'entrepreneurs politiques de la Banque mondiale, par le biais de nombreuses formes d'influence (financière, conceptuelle, fondée sur la connaissance et les réseaux) et en s'appuyant sur plusieurs réformes en cours, a collaboré avec de hauts fonctionnaires du gouvernement pour mettre le programme FBP à l'ordre du jour. Des organisations non gouvernementales internationales ont été recrutées au début du programme pour assurer sa mise en œuvre rapide. Toutefois, il a fallu transférer ce rôle aux organisations nationales pour assurer la pérennité, l'appropriation et l'intégration de l'intervention du FBP dans le système de santé (article 2). L'expérience de ce transfert montre que les éléments favorisant la réussite d’un tel processus incluent des directives structurées, une appropriation et planification conjointe de la transition par toutes les parties, et un soutien post-transition aux nouveaux acteurs. Les données qualitatives suggèrent que la mise en œuvre du programme FBP influence l’accès aux médicaments essentiels par l’entremise de plusieurs facteurs, notamment une plus grande autonomie des formations sanitaires, une régulation appliquée des équipes cadre de santé, une plus grande responsabilisation des acteurs du médicament et la libéralisation du système d’approvisionnement (article 3). Cependant, le programme a eu un impact très limité sur la disponibilité des ME (article 4). L'intervention n’a été associée à aucune réduction des ruptures de stock de ME, sauf pour la planification familiale (PF), avec une hétérogénéité des effets entre les régions et les zones urbaines et rurales. Ces résultats sont la conséquence d'un échec partiel de la mise en œuvre de ce programme, allant de la perturbation et de l'interruption des services à une autonomie limitée des formations sanitaires dans la gestion des décisions et à un retard considérable dans le paiement des prestations. / Access to essential medicines (EM) is a key element of quality of care in a health system. Accordingly, performance-based financing (PBF) is increasingly attracting the attention of policy makers as a promising intervention to improve health service delivery, including access to essential medicines, in low and middle-income countries (LMICs). Despite the growing interest in PBF research, very few studies have focused on how such a reform has been put on the agenda or how it has been maintained over time, much less how it has influenced access to EMs in low- and middle-income countries. Through an analysis of the PBF program in Cameroon, this thesis aims to advance knowledge by examining the following questions: What explains the emergence of PBF at the level of national health policy and what is the impact of this program on access to EMs? The research design is a case study and the analytical approach is based on a combination of qualitative data, through interviews conducted with key actors of the PBF program in Cameroon, and quantitative data from the impact evaluation of this program. The conceptual perspective is that of policy cycles, the policy transfer framework and intervention research. The results are structured into four scientific articles. Putting the PBF on the agenda in Cameroon was built from reports and events identifying the lack of an appropriate health financing policy as a critical issue that needed to be addressed (article 1). The evolution of political discourse towards greater accountability made it possible to test new mechanisms. A group of political entrepreneurs from the World Bank, through many forms of influence (financial, conceptual, knowledge-based and networked) and building on several ongoing reforms, worked with senior government officials to put the PBF reform on the agenda. International non-governmental organizations were recruited at the beginning of the programme to ensure its rapid implementation. However, this role had to be transferred to national organizations to ensure sustainability, ownership and integration of the PBF intervention into the health system (Article 2). The experience of this transfer shows that the elements for the success of such a process include structured guidelines, joint ownership and planning of the transition by all parties, and post-transition support to new actors. The implementation of the PBF programme influences access to essential medicines through several factors, including greater autonomy of health v facilities, enforced regulation of district medical teams, greater accountability of drug stakeholders and liberalization of the supply system (Article 3). However, the programme had a very limited impact on the availability of EMs (Article 4). The intervention was not associated with any reduction in EM stock-outs, except for family planning (FP), where the reduction was 34% (P = 0.028), with a heterogeneity of effects between regions and urban and rural areas. These poor results were likely the consequence of partial implementation failure, ranging from disruption and discontinuation of services to limited facility autonomy in managing decision‐making and considerable delay in performance payment.

A life coaching programme for the support of social work students within an open and distance learning context

Compared to other South African universities, the Department of Social Work at the UNISA has the highest intake of social work students but also the lowest throughput. Through post-graduate research, the Department of Social Work became aware of the often impeding influence of the personal, social and learning contexts of social work students on their performance, and identified a need for social work-specific student support. The following central research question was formulated: What would a life coaching programme to support social work students within an ODL context comprise of? To explore and describe the specific support needs of social work students, the qualitative research approach was used and data was gathered from focus groups of social work students and individual interviews with recently graduated and employed social workers who studied at UNISA. Tesch’s steps (in Creswell, 2009:186) were used to analyse the data systematically and data was verified by integrating Guba’s model (in Krefting, 1991:214-222) with Yin’s (2011:19-20) three objectives for building trustworthiness and credibility. The Intervention Design and Development (IDD) model of Rothman and Thomas (1994) was adapted and selectively employed, concentrating on Phase 1, 2, 3 (only Step 2) and Phase 4 in order to develop a support programme for this specific context. The goals of the support programme were to enhance student success and throughput, facilitate the personal, academic and professional development of students and to empower students to take ownership of their learning process. An online self-coaching support programme was developed and structured around seven actions towards growth, namely, clarifying my strengths, connecting to my context, clarifying my vision, completing my plan, committing to action and growth, confirming my direction and celebrating completion. The programme is divided into eight coaching conversations, two per level, contains many activities, stories and references to resources. It is designed to be compulsory and integrated into the practical work modules. Although activities are to be completed independently by students, support will be provided by e-tutors, workshop facilitators and supervisors.
