Dependent Personality Characteristics and Clinical Symptomatology in Three Clinical Syndromes in Inpatient vs Outpatient Settings

The purpose of this exploratory study was to investigate the differences between dependent personality characteristics and clinical symptomology as measured by the MCMI-II, in three major psychiatric syndromes in inpatient and outpatient treatment settings. Results show differences in profile in all groups. Treatment setting differences show higher scores on alcohol and drug dependence and major depression for inpatients. The affective group exhibited higher scores on dependent personality, dysthymic and major depression, confirming previous research on depressive disorders and dependent personality.

In each of these summaries, the meanings of the content are reseached. Ten articulated the areas were chosen:sickness, pulsion, life/death, pain/suffering, loss/mourning, symptom, trauma, narcisism, compulsion to repetition, body, being emphasized the following areas: sickness, pulsion, symptom and body.The reseach points out the relation between the psychological sickness and the opression of the lust,reflecting the existance of the mecanism of sickness and the formation of the symptom.The process of sickness involves a lack of sexual desire, and also a misfunction in the living of the pulsions of life and death, and this fragmentation leads to partial pulsions. The destinies of the pulsions suggest a connection with the psychological sickness,causing pathological manifestations like traumatic neurosis,compulsion to repetition,force of resistences,neurotic contents and their state of morbid suffering. / Ao tomar referência à abordagem psicanalítica, esta pesquisa se propõe a uma revisão bibliográfica sobre Os Sentidos do Adoecer: refazendo o percurso freudiano, escolhendo como objetivos investigar os sentidos, os mecanismos e as manifestações envolvidas no âmbito do adoecer psíquico. A metodologia baseia-se numa pesquisa qualitativa feita através de uma análise de conteúdo de um total de 75 resumos selecionados. Buscam-se, em cada resumo, os significados de conteúdo; escolhem-se dez áreas temáticas articuladas: Adoecer, Pulsão, Vida/Morte, Dor/Sofrimento, Perdas/Luto, Sintoma, Trauma, Narcisismo, Compulsão à repetição, Corpo, priorizando-se quatro dessas áreas que apresentam uma ligação maior com o estudo, quais sejam: Adoecer, Pulsão, Sintoma e Corpo. A pesquisa aponta a relação do adoecer psíquico com o “represamento da libido”, refletindo a existência do mecanismo de adoecimento e da formação do sintoma. O processo de adoecimento envolve uma dessexualização das funções egóicas (as de autopreservação e as pulsões libidinais), além de ocorrer também uma vivência desfusional das pulsões de vida e de morte, tendo as pulsões parciais como conseqüências dessa fragmentação. Os destinos das pulsões sugerem uma ligação com o adoecimento psíquico, provocando manifestações patológicas como as neuroses traumáticas, os fenômenos da compulsão à repetição, a força das resistências, a presença dos conteúdos neuróticos e seus estados mórbidos de sofrimento.

Combat Posttraumatic Stress Disorder: Effect of Intelligence on Symptomatology

The objective of this study was to examine the relations between Posttraumatic Stress Disorder symptomatology and intelligence. Thirty American combat veterans of the Vietnam War, diagnosed with chronic PTSD, were given a psychodiagnostic structured interview. Participants were assessed for Intelligence Quotient as well as the veracity of their self report. The study found that there were significant differences in how participants experienced their PTSD symptoms that were correlated with intelligence. The higher IQ participants reported more frequent and intense guilt related symptoms as well as more intense intrusive recollections. The lower IQ participants experienced more frequent startle responses, more intense problems related to falling or remaining asleep and more frequent affective symptoms related to emotional numbing. Psychologists could use these differences in how PTSD is experienced in treatment planning. It may be useful for therapy to address sleep disturbances and affective numbing in lower IQ individuals. Therapy for higher IQ individuals may be more useful if it addresses feelings of guilt and intrusive recollections.

Firstly, I concentrate this study on Freud s concept of neurosis, clarifying his clinical and meta-psychoanalytical data throughout his work, to show afterwards, a new form of conceiving the neuroses inside the human maturational process. Explaining the human development without the support of the drive theory, Winnicott points to the good-enough care as something that guarantees a complete development of infantile levels, which is the basis for thinking about the possibility of a neurosis manifestation. There is a corporal way of the instinctive excitements, that will be only integrated at the moment this instinctuality makes any personal sense in the concerning stage. In this sense, the pre genital, phallic and genital organizations will be reconsidered according to the new frame. In this study, I expose that the etiology of neuroses, according to Winnicott, is located in a determined moment of the maturational process, i.e. the moment in which the triangular relationships are already possible with whole people. I make use of fragments from clinical cases attended by Winnicott to illustrate such considerations. Nevertheless, the neurotic disturbance can happen associated with other disturbances, or even to an anti social tendency that brings the notion of a clinical frame in which the neurosis doesn t appear in its pure form. Using Winnicott s frame of castration defenses, I point out to the differences between the regression of neurosis and the regressions related to the concern stage. Finally, I retake the differences between the concept of neurosis in Freud and Winnicott that happen as a consequence of a paradigmatic change in the patterns proposed by Thomas S. Kuhn. / O presente estudo tem por finalidade esclarecer alguns aspectos do conceito de neurose, segundo o médico e psicanalista inglês Donald Winnicott (1896-1971). O estudo deste conceito, para o autor, está submetido à investigação dos processos de amadurecimento humano que não são definidos, como proposto por Freud, por uma teoria do desenvolvimento sexual. Inicialmente, detive-me no conceito de neurose em Freud esclarecendo sua organização de dados clínicos e metapsicológicos, ao longo de sua obra, para depois mostrar, em Winnicott, uma nova forma de conceber as neuroses no interior do processo de amadurecimento humano. Sem uma teoria das pulsões como base para explicar o desenvolvimento humano, Winnicott aponta para o cuidado suficientemente bom como aquilo que garante um desenvolvimento pleno dos níveis infantis precoces. Esse desenvolvimento é a base para pensar na possibilidade da manifestação de uma neurose. Há um caminho corporal das excitações instintuais, que somente será integrado no momento em que essa instintualidade fizer algum sentido pessoal, no estágio do concernimento. Neste sentido, as organizações pré-genital, fálica e genital serão reconsideradas a partir desse novo quadro. Exponho que a etiologia das neuroses, para Winnicott, localiza-se em um determinado momento do amadurecimento, a saber, o momento em que as relações triangulares, com pessoas inteiras, já são possíveis. Utilizo alguns fragmentos clínicos de casos atendidos por Winnicott para ilustrar tais considerações. O distúrbio neurótico pode, contudo, aparecer associado a outros distúrbios ou, até mesmo, à tendência anti-social trazendo a noção de um quadro clínico em que a neurose não aparece em sua forma pura. Com o quadro winnicotiano das defesas de castração, faço uma diferenciação das regressões na neurose com as regressões relativas ao concernimento. Retomo, então, as diferenças do conceito de neurose em Freud e Winnicott, que ocorrem em conseqüência de uma mudança paradigmática nos moldes propostos por Thomas S. Kuhn.

From the works of Viktor Frankl, the Viennese psychiatrist and psychologist, this paper describes what are these neuroses, called noogenics, neuroses that have their origin in the dimension of the human existence. The concept of noogenic neurosis goes beyond a psychophysical condition, therefore, to the author, the human being is essentially spiritual. This spiritual core, around which are grouped the psychic and physical, is responsible for the moral conscience, love and art. As the modern man deviates himself away from his religion, he has increased the existential emptiness, because he can not find a reason for his actions, which can lead to a lack of meaning, characteristic of noogenic neurosis. Thus, the man would get ill in his spirituality. For this type of illness, Frankl has created the Logotherapy. A specific type of such spiritual disease is the mass neurosis, which is the highlight of this research. For the author, God, who is the ultimate meaning of human existence, dialogues with the man when he talks to his own moral conscience, which is transcendent. Using the concepts of God and man given by the author, this research addresses the relationship between human being and the transcendence and how it would, through this relationship, and a subsequent meeting with the meaning, the cure of the mentioned neuroses / Este trabalho aborda a relação entre Deus e cura das neuroses da falta de sentido. A partir das obras de Viktor Frankl, psiquiatra e psicólogo vienense, descreve quais seriam essas neuroses, denominadas noogênicas, neuroses que têm sua origem na dimensão da existência humana. O conceito de neurose noogênica vai além de uma patologia psicofísica, pois, para o autor, o homem é essencialmente espiritual. Esse núcleo espiritual, em torno do qual se agrupam o psíquico e o físico, é o responsável pela consciência moral, pelo amor e pela arte. O homem moderno, ao se afastar de sua religiosidade aumentou o vazio existencial, pois não encontra o porquê de suas ações, o que pode gerar uma ausência de sentido, característica da neurose noogênica. Assim, o homem adoeceria em sua espiritualidade. Para esse tipo de adoecimento, Frankl criou a Logoterapia. Um tipo específico de tal doença espiritual é a neurose de massa, que tem um destaque nesta pesquisa. Para o autor, Deus, que é o sentido último da existência humana, dialoga com o homem quando este conversa com sua própria consciência moral, que é transcendente. Utilizando os conceitos de Deus e de homem dados pelo autor, esta pesquisa aborda a relação entre o ser humano e a transcendência e como se daria, através dessa relação, e de um conseqüente encontro de sentido, a cura das neuroses citadas

This study has presented a comprehensive overview of the origins of modern British and American military psychiatry, chiefly in response to World War I shell shock. The study examined the state of British psychiatry during the nineteenth century, as the new railroads, mines, and factories produced accident victims with post-traumatic stress disorders. As World War I began, psychoanalysis was in its infancy, and most British psychiatrists faced with a victim of shell shock fell back on an eclectic mix of treatments, including electro-shock therapy, hot baths, massages, moral persuasion, lectures, exhortation, etc. While a few British and American psychiatrists practiced either psychotherapy or disciplinary methods exclusively, the majority of practitioners used a variety of methods, depending on the doctor's point of view and the circumstances of the case at, hand. Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Esta dissertação propõe investigar o sintoma anoréxico na estrutura neurótica como um sintoma contemporâneo em consequência da baixa operatividade da função paterna e da incidência avassaladora do Desejo Materno. A pesquisa foi iniciada por um levantamento histórico-bibliográfico do jejum e da recusa alimentar ao longo da história, até o estabelecimento da anorexia como uma psicopatologia. Para dar sustentação ao eixo central desta Dissertação, a pesquisa explora o conceito de pai na Obra de Freud e no ensino de Lacan, investigando a baixa operatividade desta função e seus efeitos na formação do sintoma. O conceito de pulsão é explorado com o objetivo de compreender a anorexia como uma invasão pulsional no corpo decorrente da precária sustentação da armadura simbólica que a lei paterna insere. Deste modo, esta Dissertação expõe duas hipóteses centrais: a anorexia como uma manobra de separação, operação que a função paterna não inscreve devidamente e desta forma um recurso diante da hegemonia materna; e a anorexia como estragos que apontam a invasão da mortífera pulsão no corpo, decorrentes da presença avassaladora do Outro materno e da inconsistência da função paterna / The purpose of this thesis is to investigate the anorexic symptom in the neurotic structure as a contemporary symptom resulting from the low operatibility of the paternal role and the overwhelming incidence of the Maternal Desire. The research was initiated by a historical-bibliographical survey of fasting and food refusing throughout history, until the establishment of anorexia as a psychopathology. Giving support to the control axis of this thesis, the research explores the father concept in the work of Freud and Lacans teaching, investigating the low operability of this function and its effects on symptom formation. The drive concept is explored in order to understand anorexia as an encroachment drive in the body due to the precarious support of the symbolic armor that the paternal law inserts. Thus, this thesis exposes two central hypotheses: anorexia as a separation ploy, an operation that the paternal function does not fulfil properly and, therefore, is not an appeal in the face of maternal hegemony, and anorexia as damages that point the invasion of deadly drive in the body, arising from the overwhelming presence of the Maternal Other and of the inconsistency of the paternal function
