Development, validation and application of an effective convectivity model for simulation of melt pool heat transfer in a light water reactor lower head

Severe accidents in a Light Water Reactor (LWR) have been a subject of the research for the last three decades. The research in this area aims to further understanding of the inherent physical phenomena and reduce the uncertainties surrounding their quantification, with the ultimate goal of developing models that can be applied to safety analysis of nuclear reactors. The research is also focusing on evaluation of the proposed accident management schemes for mitigating the consequences of such accidents. During a hypothetical severe accident, whatever the scenario, there is likelihood that the core material will be relocated and accumulated in the lower plenum in the form of a debris bed or a melt pool. Physical phenomena involved in a severe accident progression are complex. The interactions of core debris or melt with the reactor structures depend very much on the debris bed or melt pool thermal hydraulics. That is why predictions of heat transfer during melt pool formation in the reactor lower head are important for the safety assessment. The main purpose of the present study is to advance a method for describing turbulent natural convection heat transfer of a melt pool, and to develop a computational platform for cost-effective, sufficiently-accurate numerical simulations and analyses of Core Melt-Structure-Water Interactions in the LWR lower head during a postulated severe core-melting accident. Given the insights gained from Computational Fluid Dynamics (CFD) simulations, a physics-based model and computationally-efficient tools are developed for multi-dimensional simulations of transient thermal-hydraulic phenomena in the lower plenum of a Boiling Water Reactor (BWR) during the late phase of an in-vessel core melt progression. A model is developed for the core debris bed heat up and formation of a melt pool in the lower head of the reactor vessel, and implemented in a commercial CFD code. To describe the natural convection heat transfer inside the volumetrically decay-heated melt pool, we advanced the Effective Convectivity Conductivity Model (ECCM), which was previously developed and implemented in the MVITA code. In the present study, natural convection heat transfer is accounted for by only the Effective Convectivity Model (ECM). The heat transport and interactions are represented through an energy-conservation formulation. The ECM then enables simulations of heat transfer of a high Rayleigh melt pool in 3D large dimension geometry. In order to describe the phase-change heat transfer associated with core debris, a temperature-based enthalpy formulation is employed in the ECM (the phase-change ECM or so called the PECM). The PECM is capable to represent possible convection heat transfer in a mushy zone. The simple approach of the PECM method allows implementing different models of the fluid velocity in a mushy zone for a non-eutectic mixture. The developed models are validated by a dual approach, i.e., against the existing experimental data and the CFD simulation results. The ECM and PECM methods are applied to predict thermal loads to the vessel wall and Control Rod Guide Tubes (CRGTs) during core debris heat up and melting in the BWR lower plenum. Applying the ECM and PECM to simulations of reactor-scale melt pool heat transfer, the results of the ECM and PECM calculations show an apparent effectiveness of the developed methods that enables simulations of long term accident transients. It is also found that during severe accident progression, the cooling by water flowing inside the CRGTs plays a very important role in reducing the thermal load on the reactor vessel wall. The results of the CFD, ECM and PECM simulations suggest a potential of the CRGT cooling as an effective mitigative measure during a severe accident progression. / QC 20101119

The ARMC5-Cullin3-RBX1 forms an RPB1-specific ubiquitin ligase essential for RNA polymerase II homeostasis

ARMC5 est une protéine qui contient sept motifs Armadillo répétitifs organisés en tandem et un domaine BTB. Nous avons observé que cette protéine était fortement exprimée dans les organes lymphoïdes, les glandes surrénales et le cerveau. Les souris avec une délétion d’Armc5 (souris KO) étaient de petite taille, et présentaient une diminution de la prolifération et la différenciation des lymphocytes T. L’absence d’ARMC5 entraînait une déficience de la réponse immunitaire médiée par les lymphocytes CD4+ et CD8+ dans les modèles expérimentaux d’encéphalomyélite auto-immune et d’infection au virus de la chorioméningite lymphocytaire, respectivement. Par la suite, plusieurs études ont révélé que la mutation ARMC5 était associée à l’hyperplasie macronodulaire bilatérale primitive des surrénales (HMBPS), qui représente une cause rare du syndrome de Cushing. Nous avons ensuite confirmé que l’hyperplasie des glandes surrénales s’était développée chez les souris KO âgées, et qu’elle s’accompagnait d’une légère augmentation des taux sériques de glucocorticoïdes. Comme ARMC5 ne présentait pas d’activité enzymatique, il était probable qu’elle faisait appel à d’autres protéines pour exercer sa fonction. Nous avons identifié plusieurs protéines qui se liaient à ARMC5, et plus particulièrement le complexe ARMC5/Cullin3 qui formait une ubiquitine ligase (E3) spécifique de la sous-unité RPB1 de l’ARN polymérase II. ARMC5 contrôlait le processus d’ubiquitination de RPB1 qui, par conséquent, s’accumulait dans plusieurs organes majeurs : les glandes surrénales, les ganglions lymphatiques, le cerveau, les poumons, le foie, etc. chez la souris KO. Ces résultats démontrent un rôle clé de l’ubiquitine ligase dans la dégradation de la protéine RPB1. Une accumulation similaire a également été observée dans les tissus hyperplasiques des surrénales provenant de patients atteints d’HMBPS et porteurs de la mutation ARMC5, ce qui souligne la pertinence clinique de nos résultats de recherche fondamentale dans les maladies humaines. Un défaut de dégradation de RPB1 augmentait le pool d’ARN polymérase II. Par ailleurs, nous avons identifié un groupe de gènes fortement surexprimés dans les glandes surrénales déficientes en ARMC5, parmi lesquels figurent les gènes effecteurs qui seraient impliqués dans l’hyperplasie des surrénales chez les souris KO et l’HMBPS chez les patients porteurs de la mutation ARMC5. Finalement, nous avons montré que la délétion ou la mutation d’Armc5 augmentait considérablement le risque des anomalies du tube neural chez les souris et les humains. Chez les patients souffrant de myéloméningocèle, nous avons constaté neuf différentes mutations faux-sens délétères, dont une diminuait l’interaction entre ARMC5 et RPB1. L’augmentation du pool d’ARN polymérase II dans les cellules précurseurs neurales (CPN), causée par la délétion ARMC5, influençait un groupe particulier de gènes, dont certains (p. ex. Folh1) seraient susceptibles de participer au développement du tube neural. En résumé, l’association ARMC5 et Cullin3 forme un complexe E3 qui cible RPB1 provoquant son ubiquitination et sa dégradation. En absence d’un tel mécanisme, on observe une perturbation de l’homéostasie de l’ARN polymérase II, qui mène à une diminution de la réponse immunitaire médiée par lymphocytes T, le développement d’HMBPS et un risque accru d’anomalies du tube neural. / ARMC5 protein contains seven tandem Armadillo repeats and one BTB domain. We observed that Armc5 was highly expressed in the lymphatic organs, adrenal glands, and brain. Armc5 knockout (KO) mice were small in size and exhibited compromised T cell proliferation and differentiation. The absence of ARMC5 resulted in an impairment of the CD4 + cell- and CD8 + cell-mediated immune response in the experimental autoimmune encephalomyelitis model and lymphocytic choriomeningitis virus infection model, respectively. Subsequently, several studies revealed that ARMC5 mutations were related to primary bilateral macronodular adrenal hyperplasia (PBMAH), which is a rare cause of Cushing’s syndrome. We then confirmed that adrenal gland hyperplasia was indeed developed in aged Armc5 KO mice with mildly increased serum glucocorticoid levels. Since ARMC5 did not exhibit enzymatic activity, its function likely depends on the interaction with other proteins. We identified several proteins that binds to ARMC5, most notably ARMC5 binding to Cullin3, forming a ubiquitin ligase (E3) specific for RNA polymerase II subunit I (RPB1). ARMC5 regulated the ubiquitination of RPB1, and its deletion resulted in RPB1 accumulation in major organs (e.g., adrenal glands, lymph nodes, brain, lung, and liver), indicating the critical role of this E3 in RPB1 degradation. A similar accumulation was also found in hyperplasia tissues from adrenal glands of PBMAH patients carrying ARMC5 mutations, underscoring the clinical relevance of our basic research findings in human disease. Defective degradation of RPB1 led to an enlarged RNA polymerase II (Pol II) pool. In addition, we have identified a group of genes strongly upregulated in KO adrenal glands, including the effector genes which would be involved in adrenal gland hyperplasia in Armc5 KO mice and PBMAH patients carrying ARMC5 mutation. Finally, we have shown that deleting or mutating Armc5 significantly augments the risk of neural tube defects in mice and humans. In patients with myelomeningocele, we found nine deleterious missense mutations in ARMC5, one of which weakened the interaction between ARMC5 and RPB1. The enlarged Pol II pool in Armc5 KO neural precursor cells (NPCs) influenced a particular group of genes, some of which (e.g., Folh1) are thought to be involved in the development of the neural tube. In summary, ARMC5 and CUL3 form an E3 complex, which targets RPB1 causing its ubiquitination and degradation. In the absence of such a mechanism, there is a disturbance of RNA polymerase II homeostasis, which leads to a decrease in the T cell-mediated immune response, the development of PBMAH and an increased risk of neural tube defects.

Testing and Refining a Unique Approach for Setting Environmental Flow and Water Level Targets for a Southern Ontario Subwatershed

In this study Bradford’s (2008) approach for setting ecological flow and water level targets is tested and refined through application within the Lake Simcoe Region Conservation Authority’s (LSRCA) subwatershed of Lover’s Creek. A method for defining subwatershed objectives and identifying habitat specialists through expert input is proposed and tested. The natural regime of each streamflow and wetland site is characterized along with the hydrological alteration at each site. Potential ecological responses to the hydrologic alterations are then hypothesized for the different types of changes calculated at each site. Methods for setting overall ecosystem health and specific ecological objective flow targets are proposed and tested. These targets are integrated into a flow regime for each site and a process for using this information for decision making is suggested. Flow magnitude quantification is attempted using hydraulic modelling and sediment transport equations, however the data used were found to be inadequate for this application. The accuracy of the targets developed using the method presented in this paper is mainly limited by the accuracy of the hydrological model and quantified flow magnitudes. Recommendations for improving these components of the assessment are made. The unique approach and recommendations presented in this paper provide explicit steps for developing flow targets for subwatersheds within the LSRCA. This research contributes toward the advancement of EFA within the LSRCA, which provides opportunity for enhanced protection and restoration of ecosystem health across the watershed. / Lake Simcoe Region Conservation Authority

「被忽視」的熱帶疾病(NTDs)中的寄生蟲疾病(Parasitic Diseases; PDs)除對非洲人群健康之危害甚鉅外,並進而對非洲地區社經體系造成嚴重衝擊與造成巨大的「失能調整人年」損失。由於不易取得治療PDs傳統基本藥物的問題,許多非洲民眾便以其部落社區的傳統治療師所採用的傳統草藥來進行PDs的治療,雖然這些傳統草藥容易取得,但是成分的內容和藥效品質甚或產生嚴重的致命副作用。雖然TRIPS協定第31條和杜哈宣言的第五和第六段對於製藥能力不足或大部分皆無製藥能力的貧窮國家,可以基於「國家緊急危難或其它緊急狀況」的事由,以強制授權方式取得專利藥或較便宜的學名藥以解決造成國家危難的特定公共健康事件,但是國際大藥廠認為無利可圖,不願意花費資金投注於預防或治療此類疾病藥物的相關研發外,高收入的國家為保護其大藥廠的藥物專利,也往往使用一些經濟制裁手段逼迫上述國家就制定國內專利法以保護其藥物專利。雖然經杜哈宣言修正TRIPS協定第31(f)條有關強制授權對外出口的障礙,但是出口國對於強制授權程序與是否能取得政治上和藥廠業者的支持,仍充滿困難。應用「專利池」可以減少交易成本或法律爭議而可調和「強制授權」與解決「權利耗盡」的爭議,有助於解決非洲開發中國家取得專利藥物的困境。PDs造成非洲開發中國家民眾失能等長期痛苦與健康生活損失,類推適用SARS模式,可依TRIPS協定第31條(b)與杜哈宣言第五段(c)將之視為「造成國家緊急危難或其它緊急狀況」的重大公共健康的事由而可行使強制授權。雖然「生醫專利池BVGH」的「非獨家個別授權與免授權費」的操作模式與傳統電信技術專利池不盡相同,但因藥廠不願投入治療PDs的傳統基本藥物存在的副作用與抗藥性的新藥研發,為鼓勵對治療PDs的藥物進行創新研發, BVGH彈性的授權方式與免繳交授權費,將有利於解決開發中國家未來取得新專利藥的困境。除BVGH外,建議結合全球獎勵基金以「激勵拉拔」的方式獎助願意投入治療PDs新藥研發並將專利自願授權給BVGH的藥廠。鑒於全球暖化與最近中東難民潮大量湧進歐洲,罕見的PDs可預期會大量傳播開來,將嚴重衝擊歐洲等先進國家良好的公共衛生體系,而使得「NTDs尤其是PDs不再只是專屬於貧窮國家的疾病,亦將常現於富有的先進國家」。這些NTDs疾病將提供藥廠進行新藥研發的利基,然而在未來可能產生專利池的反競爭問題,導致支配市場獨占性的隱憂值得關注。 / Parasitic diseases (PDs) not only cause the huge health hazards to African populations, but also they further severely impact on African socio-economic system as resulting in huge economic and health losses as assessed by disability adjusted life-years. Since it is not easy for Africans to access the essential medicine to treat PDs, many of them will seek for the help of local healers in tribal communities to treat PDs. Although these traditional herbs are readily available, the content and quality of drug ingredients may even cause serious fatal side effects. Poor countries with insufficiencies or lacks of the pharmaceutical capacities may still access the patented medicines or cheaper generics to solve the national crisis caused by the specific public health events through compulsory licensing (CL) based on "national emergency or the other emergency situations" according to TRIPS Article 31 and Doha Declaration on the fifth and sixth paragraph due to that the large international pharmaceutical companies consider unprofitable, unwilling to spend money to invest on the research and development (R&D) of new drugs for prevention or treatment purpose. Moreover, the high-income countries also tend to exert some of the economic sanctions to force those poor countries to enact national patent law in order to protect drug patents. Furthermore, the mandatory obstacle of exportation authorized by CL from the amended TRIPS Agreement Article 31 (f) by the Doha Declaration has been improved; nevertheless, it is still fraught with difficulties in utilization of CL for the exporting countries because this should be dependent on whether they may actually get the supports from political and the pharmaceutical industry. Application of patent pools model may benefit to reduce transaction costs or legal dispute thus reconciling and resolving issues related to CL as well as doctrine of patent exhaustion and that it is beneficial to help solve dilemma for African countries to access patented drugs. Because Africans severely suffer from disabled caused by PDs thus leading to long-term pain and health life losses, African countries can grant CL as PDs may be regarded as national crisis like SARS causing "national emergency or the other emergency situations" as authorized from TRIPS Agreement Article 31 (f) and Doha Declaration paragraph 5 (c). Although the practice of individual licensing with royalty-free for BVGH is somewhat different from that of traditional patent pools, this licensing practice mode is beneficial to innovation in new drugs R&D to improve the side effects and drug-resistance of traditional essential medicines and help African countries to access patented new drugs in the future. Finally, it is recommended to cooperate with Award Foundation to encourage incentive for pharmaceutical companies which contribute most to new drugs R&D and voluntary licensing to BVGH. Owing to global warming and recent emergence of huge refugees into Europe rare PDs will be obviously spread out thus causing severe impacts on well-established public health system as leading to emergence of PDs in developed countries like Europe. Altogether, such situations definitely provide a good incentive in new drugs R&D for pharmaceutical companies; however, it guarantees concerns on anti-competitive and monopoly issues derived by biomedical pools in the future.

Human Behaviour in Social-Ecological Systems : Insights from economic experiments and agent-based modelling

Progress towards sustainability requires changes in our individual and collective behaviour. Yet, our fundamental understanding of behaviour in relation to environmental change remains severely limited. In particular, little attention has been given to how individual and collective behaviours respond to, and are shaped by, non-linear environmental change (such as ‘regime shifts’) and its inherent uncertainties. The thesis makes two main contributions to the literature: 1) it provides one of the first accounts of human behaviour and collective action in relation to ecological regime shifts and associated uncertainties; and 2) extends the incipient behavioural common-pool resource literature that acknowledges social-ecological dynamics and ecological complexity. The overarching aim of this thesis is to further advance an empirically grounded understanding of human behaviour in social-ecological systems. In particular, the thesis attempts to unravel critical social-ecological factors and mechanisms for the sustainability of common-pool resources. This is especially relevant for contexts in which livelihoods can be more directly threatened by regime shifts. The following methods are applied: behavioural economic experiments in the lab (with students; Papers I and II) and in the field (with small-scale fishers from four different communities in the Colombian Caribbean; Paper III), and agent-based modelling empirically informed by a subset of the lab experiments (Paper IV). Paper I tests the effect of an endogenously driven regime shift on the emergence of cooperation and sustainable resource use. Paper II tests the effect of different risk levels of such a regime shift. The regime shift in both papers has negative consequences for the productivity of the shared resource. Paper III assesses the effect of different degrees of uncertainty about a climate-induced threshold in stock dynamics on the exploitation patterns; as well as the role of social and ecological local context. Paper IV explores critical individual-level factors and processes affecting the simultaneous emergence of collective action and sustainable resource use. Results cumulatively suggest that existing scientific knowledge indicating the potential for ecological regime shifts should be communicated to affected local communities, including the remaining uncertainties, as this information can encourage collective action for sustainable resource use. Results also highlight the critical role of ecological knowledge, knowledge-sharing, perceived ecological uncertainties, and the role local contexts play for sustainable outcomes. This thesis enriches the literature on social-ecological systems by demonstrating how a behavioural experimental approach can contribute new insights relevant for sustainability. Overall, these insights indicate that, given the opportunity and the willingness of people to come together, share knowledge, exchange ideas, and build trust, potential ecological crises can encourage collective action, and uncertainties can be turned into opportunities for dealing with change in constructive ways. Approche multidisciplinaire pour l’amélioration de l’estimation de l’exposition aux sous-produits de désinfection de l’eau en milieu domestique et en piscine

La désinfection de l’eau de consommation et des piscines induit la formation de sous-produits (SPD) potentiellement nocifs pour la santé, parmi lesquels les trihalométhanes (THM), les acides haloacétiques (HAA) et les chloramines (CAM). La difficulté d’estimer l’exposition humaine à ces SPD empêche de cerner précisément les risques sanitaires possiblement associés (i.e., cancérigènes, reprotoxiques, irritatifs). Nos travaux s’articulent autour d’une méthodologie consistant à intégrer des données d’occurrence environnementales à des modèles toxicocinétiques à base physiologique (TCBP) pour améliorer les mesures de l’exposition aux SPD. Cette approche multidisciplinaire veut prendre en compte de manière aussi appropriée que possible les deux composantes majeures des variations de cette exposition : les variations spatio-temporelles des niveaux de contamination environnementale et l’impact des différences inter- et intra-individuelles sur les niveaux biologiques. Cette thèse, organisée en deux volets qui explorent chacun successivement des aspects environnemental et biologique de la problématique, vise à contribuer au développement de cette stratégie innovante d’estimation de l’exposition et, plus généralement, à des meilleures pratiques en la matière. Le premier volet de la thèse s’intéresse à l’exposition en milieu domestique (i.e., résultant de l’utilisation de l’eau potable au domicile) et est consacré au cas complexe des THM, les plus abondants et volatils des SPD, absorbables par ingestion mais aussi par inhalation et voie percutanée. Les articles I et II, constitutifs de ce volet, documentent spécifiquement la question des variations inter- et intra- journalières de présence des SPD en réseau et de leurs impacts sur les estimateurs de l’exposition biologique. Ils décrivent l’amplitude et la diversité des variations à court terme des niveaux environnementaux, présentent les difficultés à proposer une façon systématique et « épidémiologiquement » pratique de les modéliser et proposent, de manière originale, une évaluation des mésestimations, somme toute modestes, des mesures biologiques de l’exposition résultant de leurs non-prise en compte. Le deuxième volet de la thèse se penche sur l’exposition aux SPD en piscine, d’un intérêt grandissant au niveau international, et se restreint au cas jugé prioritaire des piscines publiques intérieures. Ce volet envisage, pour quantifier l’exposition dans ce contexte particulier, l’extension de l’approche méthodologique préconisée, élaborée originellement pour application dans un contexte domestique : d’abord, à travers une analyse approfondie des variations des niveaux de contamination (eau, air) des SPD en piscine en vue de les modéliser (article III); puis en examinant, dans le cas particulier du chloroforme, le THM le plus abondant, la possibilité d’utiliser la modélisation TCBP pour simuler des expositions en piscine (article IV). Les résultats mettent notamment en évidence la difficulté d’appréhender précisément la contamination environnementale autrement que par un échantillonnage in situ tandis que la modélisation TCBP apparait, sur le plan toxicologique, comme l’outil le plus pertinent à ce jour, notamment au regard des autres approches existantes, mais qu’il convient d’améliorer pour mieux prédire les niveaux d’exposition biologique. Finalement, ces travaux illustrent la pertinence et la nécessité d’une approche multidisciplinaire et intégratrice et suggère, sur cette base, les pistes à explorer en priorité pour mieux évaluer l’exposition aux SPD et, in fine, cerner véritablement les risques sanitaires qui en résultent. / Disinfection of drinking and swimming pool waters disinfection is unavoidable but induces the formation of by-products (DBPs), such as trihalomethanes (THMs), haloacetic acids (HAAs) and chloramines (CAMs), that could be harmful to human health. The still challenging DBP exposure assessment prevent their suspected adverse effects (i.e., cancers, adverse pregnancy outcomes, irritations) to be clearly established. A methodology has been conceptualized which consists of integrating environmental occurrence data with physiologically based toxicokinetic (PBTK) modeling to improve DBP exposure assessment. It was designed to allow both spatial and temporal variations of the environmental contamination and the biological impacts of between- and within- individual differences to be accounted for. This thesis comprised of two parts. Each one investigates successively both environmental and biological aspects. The objective is to contribute to the development of an innovative integrated strategy and to the definition of best practices for DBP exposure assessment. The first part of the thesis, comprising papers I and II, focuses on household exposure (i.e., resulting from drinking water use at home) and on THMs, the most abundant and volatile DBPs that can be absorbed not only by ingestion but also by inhalation and dermal absorption. These two papers investigate particularly the short-term (day-to-day and within-day) variations of THM levels in the drinking water and then their impact on the internal exposure indicators. They described the amplitudes and the diversity of the environmental variations, failed to model them in a systematic and practical way for epidemiological purposes but assessed, for the first time, their impacts on the predicted biological levels which appeared quite low. The second part concerns the exposure to DBPs in swimming pool which is of a growing international interest. Only the allegedly worrying case of public indoor swimming pool was regarded. This section focuses on the feasibility of using the previously mentioned approach, which was first designed for dealing with household exposure, for DBP exposure assessment in swimming pools. First, Paper III investigated the occurrence and spatial and temporal variations of DBPs in both water and air of swimming pools to model them. Focusing on chloroform, the most abundant THM, Paper IV examined the ability and reliability of PBTK modeling to simulate various swimming pool exposure events and predict the resulting biological levels in individuals. The results show, among other things, the difficulty of explaining precisely the environmental contamination and point out the necessity to carry out a minimal in situ sampling to monitor the environmental levels of DBPs. Compared to other approaches, PBTK modeling is a powerful but still to be improved tool for predicting swimming pool exposure. Eventually, these works underline the relevance and the necessity of a multidisciplinary and integrating approach for better estimating exposure to DBPs and therefore health risks. Further issues that should be addressed are recommended.

Quantification of ventricular mechanical dyssynchrony under stress

L'évaluation de l'asynchronisme mécanique ventriculaire sous stress a soulevé une attention importante en tant que facteur prédictif de la réponse au traitement de resynchronisation cardiaque (CRT). De plus, il semble exister une relation significative entre le devenir du patient et la présence d’asynchronisme au repos. Plusieurs méthodes échocardiographiques peuvent être utilisées pour évaluer l’asynchronisme. Cependant, parmi toutes les différentes méthodologies ou index existant dans ce domaine, aucun critère ne fait l’unanimité. Cette thèse étudie l'importance des techniques d'imagerie nucléaire dans le cadre de l’évaluation de l’asynchronisme cardiaque induit par le stress en utilisant trois différents modèles canins expérimentaux. Le premier chapitre vise à examiner les effets du stress sur le synchronisme de la contraction du ventricule gauche (VG) en utilisant l'imagerie synchronisée de perfusion myocardique dans une cohorte canine normale. Le stress a été induit par différents niveaux d’infusion de dobutamine sur six sujets sains. Les paramètres hémodynamiques et l’asynchronisme ont été évalués par des mesures de pressions ventriculaires. L'analyse de phase sur l’imagerie s’est effectuée en utilisant un logiciel commercialement disponible (QGS) et un logiciel interne (MHI4MPI), basée sur le déplacement et l’épaississement des parois ventriculaires. L’augmentation de la concentration de dobutamine a démontré une amélioration de la capacité fonctionnelle et une réduction de l’asynchronisme ventriculaire. L’analyse de l’asynchronisme calculée à partir de l’épaississement de la paroi semble plus robuste et plus sensible que l’utilisation du déplacement des parois. (Salimian et. al., J Nucl Cardiol., 2014) Le second chapitre étudie les différents paramètres d’asynchronisme au repos et à différents niveaux de stress dans un modèle de cardiomyopathie dilatée et à QRS étroit. Ce modèle a été créé sur dix chiens par tachycardie via stimulation de l'apex du ventricule droit pendant 3-4 semaines, permettant d’atteindre une fraction d'éjection cible de 35% ou moins. Le stress a ensuite été induit par une perfusion de dobutamine jusqu'à un maximum de 20 μg/kg/min. Les données hémodynamiques et l’asynchronisme ont été analysés par des mesures de pression ventriculaire et l’analyse de l’imagerie dynamique du compartiment sanguin. L’importante variabilité individuelle des sujets inclus dans notre cohorte empêche toute conclusion définitive sur la mesure de l’asynchronisme interventriculaire. Cependant, les différents niveaux de stress, même dans des intervalles rapprochés, ont démontré un effet significatif sur les paramètres hémodynamiques et l’asynchronisme. (Salimian et. al., J Nucl Cardiol., 2015) La troisième section vise à déterminer si l’estimation du mode de stimulation optimal effectuée au repos demeure le choix optimal lorsque le niveau d’activité cardiaque s’intensifie pour des sujets avec bloc auriculo-ventriculaire (AV) et fonction ventriculaire normale. Cinq chiens ont été soumis à une ablation du nœud AV et des sondes de stimulation ont été insérées dans l'oreillette droite pour la détection, l’apex du ventricule droit (VD) et une veine postérolatérale du VG pour la stimulation. Cinq modes de stimulation ont été utilisés : LV pur, biventriculaire (BiV) avec pré-activation de 20 ms du LV (LVRV20), BiV pur, BiV avec pré-activation de 20 ms du VD (RVLV20), VD pur. Des niveaux jusqu’à 20 μg/kg/min de dobutamine ont été atteints. Le stress a modifié l’étendue de l’asynchronisme de base et ce, pour tous les modes de stimulation. De plus, les effets physiologiques intrinsèques du stress permettent une évaluation plus précise de l’asynchronisme ventriculaire, diminuant la variabilité inter-sujet. Le mode de stimulation LVRV20 semble le mode optimal dans ce modèle, supportant l’utilisation de la stimulation bi-ventriculaire. / Assessment of ventricular mechanical dyssynchrony (MD) under stress has attracted a large amount of attention as a stronger predictor of response to cardiac resynchronization therapy (CRT) and as a parameter whose variation bears a greater relationship to clinical outcomes than resting-MD either in CRT candidates or another subset of patients. Several echocardiographic methods can be used to assess stress-MD. However, no standardized approach is currently used to explore stress-induced variations in inter- and intraventricular MD. This dissertation studies the importance of nuclear imaging techniques in assessing stress-induced MD variations by providing three different experimental canine models. The first chapter sought to examine the impacts of stress on the left ventricular (LV) synchrony with phase analysis of gated SPECT myocardial perfusion imaging (GMPS) within a normal canine cohort. Stress was induced by different levels of dobutamine infusion in six healthy subjects. Hemodynamic and LV MD parameters were assessed by LV pressure measurements and phase analysis of GMPS using commercially available QGS software and in-house MHI4MPI software with thickening- and displacement-based methodology. The increase of dobutamine level was shown to be in accordance with the improvement of LV functional capacity and reduction of MD parameters. MD analysis based on wall thickening was more robust and sensitive than the global wall displacement. (Salimian et. al., J Nucl Cardiol., 2014) The second chapter investigated the range of difference in inter- and intraventricular MD parameters from rest to various levels of stress in a dilated cardiomyopathy (DCM) and narrow QRS complex model. Ten large dogs were submitted to tachycardia-induced DCM by pacing the right ventricular apex for 3-4 weeks to reach a target ejection fraction of 35% or less. Stress was then induced by infusion of dobutamine up to a maximum of 20 μg/kg/min. Hemodynamic and MD data were analyzed by LV pressure measurements and gated-blood pool SPECT (GBPS) imaging. Individual differences in the magnitude and pattern of change in the various levels of stress precluded any definitive conclusion about interventricular MD. However, different levels of stress, even in close intervals, showed a significant positive impact on hemodynamic and intraventricular MD parameters. (Salimian et. al., J Nucl Cardiol., 2015) The third chapter sought to examine if the optimal pacing mode at rest could be the best one during the maximum stress level in terms of MD parameters in subjects with an atrioventricular (AV) block and normal function. Five dogs were submitted to AV node ablation and pacing leads were placed in the right atrium for sensing, in right ventricular (RV) apex, and in posterolateral LV vein for pacing in five modes of LV, biventricular (BiV) with 20 ms of LV pre-activation (LVRV20), BiV, BiV with 20 ms of RV pre-activation (RVLV20) and RV pacing. Stress was induced by dobutamine infusion up to a maximum of 20 μg/kg/min. Data analyses were the same as chapter one. Dobutamine stress changed the extent of resting-LV MD at all pacing modes. Intrinsic physiologic effects of stress resulted in more accurate MD assessment with lesser variability in subjects who underwent pacing. LVRV20 was the preferred site of stimulation in this model rather than single-site pacing.
