Akutmottagningar idag får ofta in barn som behöver opereras akut. Personalen på akutmottagningen ställs då inför en situation där de inte hinner informera och förbereda barnet på vad som ska ske. Detta kan skapa en otillräcklighetskänsla hos anestesisjuksköterskorna på operation som därmed får ta hand om ett barn som inte är förberett på vad som kommer att hända. Att skapa förtroende hos barnet är en process som inte kan skyndas på och kräver mycket tid ända från första mötet med barnet. Denna studie vill belysa vad anestesisjuksköterskorna baserar sina val och strategier på i detta möte. Syftet är därför att beskriva vad anestesisjuksköterskorna baserar sitt bemötande på i mötet med barn och deras familjer i akuta situationer på en operationsavdelning. Syftet är också att beskriva om det finns ett behov av riktlinjer för det preoperativa mötet med akut sjuka barn. Studien har en kvantitativ, deskriptiv design där ett frågeformulär utarbetat av författarna till den här studien används. Resultatet analyserades med beskrivande statistik. Resultatet visar på att behovet av riktlinjer och utbildning hos anestesisjuksköterskorna är stort och att anestesisjuksköterskorna baserar sitt bemötande på erfarenhet. Resultatet visar dock att det inte går att dra några paralleller mellan lång erfarenhet och att känna sig trygg i sin yrkesroll i mötet med akut sjuka barn. Resultatet i denna studie visade även att anestesisjuksköterskorna är medvetna om vikten av den preoperativa förberedelsen och vikten att skapa en dialog. / Program: Specialistsjuksköterskeutbildning med inriktning mot anestesisjukvård

Foram preparadas canaletas medindo 2,0 mm nas faces vestibular e lingual de molares decíduos que foram divididas aleatoriamente em 3 grupos de acordo com a técnica de preparo cavitário: Grupo I - alta rotação associado à ponta diamantada; Grupo II - sistema de abrasão a ar; e Grupo III - laser Er:YAG. Após o condicionamento com ácido fosfórico por 15 segundos, foi utilizado o adesivo Single Bond (3M) e a resina Filtek Flow (3M) para restaurar as canaletas. Os espécimes foram então mantidos em estufa a 37ºC por 6 dias, termociclados (500 ciclos com temperatura variando entre 5 e 55ºC), isolados e colocados em solução de fucsina básica à 0,5% durante 24 horas. Após serem seccionados a microinfiltração foi avaliada em microscópio ótico ligado a uma câmera de vídeo com 10 X de aumento. Para se avaliar a forma das cavidades medimos os ângulos oclusal, cervical e de abertura das cavidades, bem como a largura e a profundidade das mesmas por meio de fotomicrografias obtidas em MEV. Os resultados demonstraram semelhança estatística entre as larguras dos preparos cavitários, porém os ângulos oclusal, cervical e de abertura das cavidades apresentaram-se diferentes estatisticamente e que não houve microinfiltração marginal em nenhum dos grupos avaliados e que a diferença na forma de preparo das cavidades não influenciou na microinfiltração marginal dos espécimes estudados. / The aim of this research was the evaluation of the cavity prepare technique on the marginal microleakage in deciduous teeth. Cavities measuring 2.0 mm were prepared on the buccal and lingual faces and were divided into three groups according to the cavity prepare technique: Group I - High speed associated to diamond bur; Group II - air abrasion system; and Group III - Er:YAG laser. After the phosphoric acid etching for 15 seconds, the Single Bond (3M) adhesive and te Filtek Flow (3M) were used for the restoration of the cavities. Then the sample was stored at 37ºC for 6 days, termocycled (500 cycles with temperature varying from 5 to 55º C), isolated and immersed in a 0.5% funcsin solution for 24 hours. After being sectioned the microleakage was examined with a magnification optical microscope connected to a color video camera. In order to evaluate the shape of the cavities the occlusal, cervical and cavity opening angles were measured, as well as the width and depth of then, using SEM photographs. The results showed that there was no marginal microleakage in any of the evaluated groups and that the difference in the cavity shape did not influence the marginal infiltration on the studied samples.

A técnica analítica mais utilizada para o monitoramento de exposição a metais tóxicos ou para avaliação de deficiência a elementos essenciais é a espectrometria de absorção atômica chama (FAAS) ou com forno de grafite (GF AAS). Entretanto, cada vez mais os laboratórios de pesquisa na área clínica estão mudando seus métodos de análise, baseados nestas técnicas, para a utilização da espectrometria de massas com plasma acoplado (ICP-MS). Isso vem acontecendo porque o ICP-MS permite a determinação de elementos químicos, em diversas matrizes, numa ampla faixa de concentração (ng L-1 a mg L-1), além de possibilitar alta rapidez de análise, capacidade multielementar e limites de detecção menores que outras técnicas analíticas. Uma das principais características do método a ser utilizado para a análise de elementos químicos em rotina por ICP-MS é que ele seja rápido, com o mínimo de manipulação da amostra. Neste sentido, métodos que propõem a análise direta de amostra são mais atrativos. Todavia, ainda é limitado o número de métodos que propõem análise direta de fluidos biológicos por ICP-MS. Neste sentido, o presente trabalho teve como objetivo o desenvolvimento de um método para análise direta de sangue por ICP-MS para determinação de Ag, As, Cd, Co, Mn, Ni, Pb e Se. Para isso, amostras de sangue (200 µL) foram misturadas com 500 µL de hidróxido de tetrametilamônio (TMAH) (10% v/v) e incubadas por 10 minutos. Posteriormente, a solução resultante foi diluída para 10 mL com uma solução contendo 0,05% m/v EDTA; 0,005% v/v Triton® X-100. Em seguida, as amostras foram analisadas diretamente por ICP-MS (ELAN DRC II). Ródio (Rh) foi usado como padrão interno e a calibração das amostras foi feita por meio de ajuste de matriz com sangue base ovino. Os limites de detecção (LD) do método foram de 0,008; 0,02; 0,004; 0,009; 0,003; 0,09; 0,04; 0,1 µg L-1 para Ag, As, Cd, Co, Mn, Ni, Pb and Se, respectivamente. A validação dos resultados foi realizada com análise de um material de referência de sangue do Programa de Proficiência do Institut National de Santé Publique du Quebec, no Canadá. Validação adicional foi obtida pela comparação dos resultados obtidos pela análise de 20 amostras de sangue pelo método proposto e pela técnica de GF AAS. O método foi também comparado a outros dois existentes na literatura e comumente utilizados em laboratórios dos Estados Unidos e Suécia, apresentando limites de detecção comparáveis ou melhores e melhores exatidão e precisão. / The most used analytical technique for monitoring the exposure to toxic metals or for the assessment of the deficiency of essentials elements is the atomic absorption spectrometry with flame (FAAS) or graphite furnace (GF AAS). Especially for Small and Medium Sized Enterprises (SME) an implementation of a Performance Measurement System is mostly critical in terms of the fact that SMEs often is restricted of limited resources, lack of strategic thinking, lack of formal planning, as well as a reactive mentality. A model to prepare and secure an implementation of a performance measurement system at SMEs, has been developed, tested and validated through a single case study at a small enterprise, that is a well-renowned refiner of castings. The model is constituted by phases and steps supporting the advantages as well as over bridges the most common disadvantages for SMEs. The significant advantages on behalf of SMEs; loose structure, informal operating practices and lack of bureaucracy are reinforced by integrating the project and especially the output such as specification of requirements, new routines, improvement groups and forums within the organisation and in turn over bridge the disadvantages of limited resources and lack of formal planning. Identification and prioritisation of strategic objectives is the first step and creates a foundation of a continuous strategic thinking, which often SMEs are lacking, and in turn navigate the company in the right direction in order to maximise the utilisation of resources and consequently increase the profitability. / Vad som inte mäts kan inte kontrolleras och vad som inte kontrolleras kan inte förbättras, indikerar att mätning är en förutsättning för att åstadkomma förbättringar. Implementation av ett driftsuppföljningssystem är särskilt kritiskt på ett små och medelstora företag (SMEs) då dessa ofta är hindrade av begränsade resurser, bristfälligt strategiskt tänkande, otillräcklig formell planering, men även av en reaktiv mentalitet. En modell för att förbereda och säkra en implementation av ett driftsuppföljningssystem har utvecklats, testats och validerats genom en enskild fallstudie på ett mindre företag som bearbetar gjutgods. Preparedness of the mainstream primary school teachers in implementing inclusive education policy in Nongoma Circuit, KwaZulu-NatalZulu, Phindile Doreen 06 1900 (has links)
Inclusive Education has gained significant currency nationally and internationally. It demands that the teacher be able to meet the needs of learners with impairments in ordinary classrooms. The success of inclusive education rests on quality teacher preparation geared towards inclusive education. The focus of this investigation is on preparedness of mainstream primary school teachers in implementing inclusive education policy in the Nongoma circuit. The lack of teachers prepared to provide quality inclusive teaching to learners and the limitations of existing support structures both impact on inclusion. Through a questionnaire an effort was made to ascertain teachers’ preparedness for inclusive education in the Nongoma circuit. This measure was utilized to determine the extent to which teachers are prepared for the implementation of inclusive education in the classroom. It was also used to describe the nature of training provided by District Based Support Team in an advancement of inclusive education practices in the classroom. The researcher also identified enabling strategies required for teachers in an inclusive classroom environment. Finally, recommendations on strategies for the successful implementation of inclusive education in the classroom were made. The data was collected through a questionnaire. Denna undersökning besvarar följande problemformulering med stöd av en kvalitativ textanalys i form av en dokumentundersökning. Utifrån ett säkerhetsstrategiskt perspektiv; Hur kan problamtiken med hybrida hot i gråzonen beskrivas och hur omhändertas dessa i den nationella säkerhetsstrategin? Genom att läsa i empiri och teori om gråzon, hybrida hot och säkerhetsstrategier skapar sig författarna en modell för analys. Denna analysmodell används sedan för att analysera den (Svenska) nationella säkerhetstrategin som publicerades 2017. Analysmodellen grundar sig på de av Treverton identifierade hybrida hoten: Desinformation, cyberattacker, ekonomiska påtryckningar, irreguljära beväpnade styrkor, reguljära beväpnade styrkor.Undersökningen visar att den nationella säkerhetsstrategin, trots en bristande tydlighet inom vissa områden och en bristande balans mellan mål, metoder och medel, identifierar och ger styrningar för omhändertagandet av de identifierade hybrida hoten. Vidare visar säkerhetsstrategin på en bred förståelse och tolkning av begreppen hybrida hot och gråzonen, utan att försöka definiera dessa komplexa begrepp. Då analysmodellen appliceras, belyses en möjlig hotbild; miljöhot. Resultaten av analysen visar också att det kan vara klokt att beskriva just de samtida hoten och utnyttjande av dessa, istället för att argumentera avseende definitioner. Författarna menar att det är av större vikt att skapa en större förståelse för fenomenen, i syfte att åstadkomma en allomfattande ansats för att motverka de verkliga effekterna av hoten. / In 2014 Russia, in front of the rest of the world, claimed and performed an annexing of Crimea. The larger parts of the western world were left in shock. Without any acknowledgment Russia moved forward with “little green men” and implied a different tactic. It was not officially war and nor peace, the fighting and the action took place in what was popularly named as the gray zone. This thesis is answering the following questions by using a qualitative document examination. From a strategic security perspective: How can the problem regarding hybrid threats in the gray zone be described and how does the national Swedish strategy respond to these threats? By reading and concluding existing theory about hybrid threats within the gray zone the authors have created their own model of analysis to utilize on the Swedish National Security Strategy. The model of analysis consists of the five main hybrid threats chosen from the theory presented by Treverton: Disinformation, Cyber-attacks, Economic pressure, Irregular armed forces and Regular forces.The thesis concludes that although unclear and found lacking in coherence regarding ends, ways and means; the National Security Strategy addresses and gives guidance on how to counter the effects of the above-mentioned threats. The Security Strategy also shows a consciousness regarding the complexity of the hybrid threats and the gray zone, without trying to define the actual area or methods utilized. When looked at through the model of analysis, it also highlights a possible new threat to be considered; environmental threats. The results of the analysis also indicate that it might be wise to use the term contemporary threats and the utilization of these, instead of arguing about definitions. The authors argue that it is of greater importance to create a larger understanding of the phenomena, in order to counter the actual effects by employing a truly comprehensive approach.
