Previous issue date: 2017-04-03 / Among the parts of the body, the hand is one of the most important for the humans because of their versatility and ability to manipulate objects. Many people do not have this member, either congenital malformation or necessity of amputation which make their day by day hardy. The existing comercial prosthesis have high costs making it inusual for a signifcative part of patients, on the other hand, there are at Internet communities to develop more accessible hand prostheses, for manufacturing by 3D printing but, It generally use unefficient activation methods. The target of this study is to develop the mechanical structure of a prosthesis hand with individual drive for each fingers, which can be make by using 3D printing, making it the most accessible price and the most efficient prosthesis when compared with current ones. For this, a mathematical model that describes the motion of the robotic finger will be determined. This finger has as actuator, DC motors, coupled to gears which in by their self are coupled in arms. The results will be checked by comparing the movements performed bettewen a prototype of the prosthesis and the determined mathematical model. It is expected that the prosthesis proposed by this research is the structural base for future stages of the development of a full prosthesis activated by voice command. / Dentre os membros motores, a mão tem grande importância para os seres humanos devido à sua versatilidade e capacidade para manipular objetos. Muitas pessoas não têm este membro, seja por má formação ou por amputação, o que dificulta na execução das atividades cotidianas. As próteses comerciais existentes têm custo elevado inviabilizando a utilização para a maioria das pessoas que dela necessita, por outro lado, existem comunidades na internet que desenvolvem próteses de mão acessíveis, via processo de fabricação por manufatura aditiva, porém, geralmente com métodos de acionamento pouco eficientes e pouco ergonômicas. Este estudo apresenta o desenvolvimento da estrutura mecânica de uma prótese de mão com acionamento individual dos dedos. Para isso, é desenvolvido o modelo matemático que descreve o movimento do dedo robótico que tem como acionadores, motores de corrente contínua, acoplados a engrenagens que por sua vez estão conectadas a braços de movimento. É realizado um estudo cinemático do movimento dos dedos, além de simulação computacional para a estrutura mecânica proposta. Também foi construído um protótipo virtual parametrizado para checar a viabilidade de execução da mecânica proposta. A prótese proposta por esta pesquisa será a base estrutural para etapas do desenvolvimento de uma prótese acionada por comando de voz.

L’acteur est-t-il l’objet d’une nouvelle mise en cause radicale, un siècle après Maeterlinck, Jarry et Craig ? La discussion prend aujourd’hui une tournure inédite. Il ne s’agit plus d’écarter l’homme de la scène pour le remplacer par une marionnette idéale. Les acteurs artificiels, avatars, robots et autres doubles, semblent désormais pouvoir devenir autonomes. La question n’est pas seulement : qu’est-ce qui manque, lorsque l’acteur manque ? Mais aussi, qu’est-ce qui ne manque pas ? Pour étudier ce sujet, cette thèse s’appuie sur trois œuvres qui ont fait événement à l’orée du XXIe siècle : Les Aveugles de Denis Marleau, Stifters Dinge de Heiner Goebbels, et Re : Walden de Jean-François Peyret. Elle introduit la conjecture anthropologique d’une « disparition » de l’homme. Expression problématique, qui est à entendre de prime abord au sens d’un retrait ou d’une instrumentation de l’acteur, mais qui sous-tend en arrière-plan d’autres enjeux : brouillage des frontières entre vivant et artificiel, perte du lien au monde, déclin de l’humanisme, effondrement écologique. Si l’homme devait disparaître, c’est peut-être au théâtre qu’il résisterait le plus longtemps à sa disparition. Mais la proposition peut s’inverser : n’est-ce pas là qu’il ne cesse de s’effacer avec le plus d’ostentation, jouant depuis toujours avec les oppositions apparence / réalité, apparition / disparition ? La thèse explore ces problématiques, en s’intéressant aussi bien à la scénographie, à la dramaturgie, à la relation acteur spectateur, et aux nouvelles machines qui transfigurent le dispositif scénique en une anamorphose visuelle, sonore, éminemment troublante, de l’être humain en train de disparaître. / Is the actor thrown back into doubt again, one century after Maeterlinck, Craig and Jarry ? The topic happens to be brought up to date. But the purpose is no longer to get rid of the actor, so that he could be replaced by puppets. Artificial actors, like avatars, robots and other doubles, are likely to get their autonomy soon, on stage. The question is not only : what is missing, when the actor is missing ? But also, what is not missing ? This thesis is based upon three performances that stroke the minds on the first decade of the 21st century : Les Aveugles by Denis Marleau, Stifters Dinge by Heiner Goebbels, and Re :Walden by Jean-François Peyret. Our conjecture is about human disappearing on stage, with a special focus on anthropological aspects. The notion of « disappearing » means, at first sight, the withdrawing of actors, but we show that other deep challenges stand in the background : body instrumentation, interferences between life and artificial, disconnection to the phenomenal world, mankind decline and ecological disasters. If the human being is supposed to disappear soon, it’s probably on theatre stage that he will stand for a while. At the same time, isn’t it on stage that he has been playing for ever with appearing and disappearing, reality and illusion ? This thesis explores these problems, dealing with all the aspects : scenography, dramaturgy, actors and spectators, technology and new machines that are completely transforming the stage into a visual, sound, and highly confusing anamorphosis, by which the human being is involved in a vanishing process.

Patients with Lower Limb Amputation in Vietnam : A quantitative study on Patients’ Satisfaction with their given Prosthetic Device and Service / Patienter med nedre extremitetsamputationer i Vietnam : En kvantitativ studie på patienters nöjdhet med deras utgivna protes och serviceRexhaj, Behar, Danielsson, Theresia January 2017 (has links)
Aim: The aim of this thesis is to evaluate patients’ satisfaction with prosthetic device and service on lower limb amputations in Danang, Vietnam and to implement comparisons between the subgroups gender, living area, amputation cause and level. Method: A cross-sectional study design was conducted using the Quebec User Evaluation of Satisfaction with assistive Technology (QUEST) 2.0 questionnaire to measure patients’ satisfaction. Fifty patients were recruited through the International Committee of the Red Cross (ICRC) and the data collection was performed in the patients own homes with an interpreter. The collected data was statistically analyzed with suitable tests using Statistical Package for the Social Sciences (SPSS). Result: The patients were quite satisfied with their prosthetic devices (mean 4.16 SD ± 0.561) and more or less satisfied with their given services (mean 2.83 SD ± 1.213). Significant differences were found between genders regarding the patients’ satisfaction with prosthetic device. There were no significant differences between the remaining subgroups; living area, amputation cause and amputation level. Conclusion: This thesis demonstrates that the patients in Danang appear to be quite satisfied with their prosthetic device, even though they reported problems with the durability of the device. Furthermore, the patients were more or less satisfied with their prosthetic services. The findings in this thesis also reveals that women were less satisfied compared to men regarding the prosthetic device. These conclusions should however be taken with caution, since the reliability and validity was lower than desired and it’s difficult to generalize the results to the population. / Syfte: Syftet med denna studie är att evaluera patienters nöjdhet med deras utgivna protes och service på nedre extremitetsamputerade i Danang, Vietnam och att implementera jämförelser mellan subgrupperna kön, bostadsområde, amputations- orsak och nivå. Metod: En cross-sectional studiedesign blev utförd genom användning av Quebec User Evaluation of Satisfaction with assistive Technology (QUEST) 2.0 enkäten för att mäta patienters nöjdhet. Femtio patienter blev rekryterade genom International Committee of the Red Cross (ICRC) och datainsamlingen blev utförd i patienternas egna hushåll med en tolk. Den insamlade datan blev statistiskt analyserad med lämpliga tester genom användning av Statistical Package for the Social Sciences (SPSS). Resultat: Patienterna var ganska nöjda med deras utgivna proteser (medelvärde 4,16 SD ± 0,561) och mer eller mindre nöjda med deras utgivna service (medelvärde 2,83 SD ± 1,213). Signifikanta skillnader hittades mellan kön gällande patienters nöjdhet med deras utgivna protes. Slutsats: Den här studien demonstrerar att patienterna i Danang verkar vara ganska nöjda med deras utgivna proteser, fastän de rapporterade problem med protesens hållbarhet. Dessutom var patienterna mer eller mindre nöjda med deras utgivna service. Fynden i denna studie påvisar också att kvinnorna var mindre nöjda gällande protesen jämfört med männen. Dessa slutsatser skall dock tas med aktsamhet, eftersom reliabiliteten och validiteten blev lägre än önskat och det var svårt att generalisera resultaten till populationen.

Résumé : L’imagerie TEP est une modalité puissante qui permet de suivre d’infimes concentrations de traceurs marqués pour la détection de cancers et d’autres pathologies. Il y a actuellement un intérêt croissant pour le développement de peptides comme outils diagnostiques et de traitement en oncologie. Cet intérêt se justifie entre autres par le fait que les peptides sont tolérants à la présence de chélateurs bifonctionnels ou de groupements prosthétiques pour le marquage avec divers radiométaux (64Cu, T1/2 = 12,7 h, 68Ga, T1/2 = 68 min, etc.) ou le 18F (T1/2 = 109,8 min) sans perte de leur activité biologique. L’objectif des travaux rapportés dans ce document était de développer des outils moléculaires innovateurs et efficaces qui facilitent le marquage de peptides pour l’imagerie TEP. Il s’agit spécifiquement d’un chélateur bifonctionnel et d’une méthode de conjugaison rapide et sélective de groupe prosthétique. Sur un volet, un chélateur bifonctionnel analogue de la lysine avec des ligands méthylhydroxamates a été synthétisé en solution par double bisalkylation. Les résultats préliminaires indiquent une faible chélation avec le Cu(II), mais sont à poursuivre avec les 68Ga et 89Zr. Pour le second volet de radiomarquage au 18F, les procédures synthétiques ont été optimisées en deux étapes, soient le marquage du groupe prothétique et sa conjugaison au peptide. Tout d’abord, des conditions de marquage par une réaction de SNAr en présence de 18F- ont été développées pour donner le groupe prosthétique 18F-thioester nécessaire à la conjugaison. Par la suite, sa conjugaison au peptide par la réaction de ligation chémosélective, ce qui implique trois étapes 1) une transthioestérification favorisée entre les groupements thioester et thiol des segments de peptides; 2) un réarrangement irréversible de l’intermédiaire thioester en N-(oxyalkyl)amide, suivi; 3) du clivage de l’auxiliaire. Par les présents travaux, il a été prouvé que la nouvelle méthodologie en un seul pot réactionnel accélère la réaction et permet le marquage au 18F de peptides non protégés, limitant ainsi les réactions secondaires et le nombre d’étapes après le marquage des peptides. La conjugaison du groupe prothétique à un composé et un peptide modèle se produit en 26-55 min comparativement aux 48 h des conditions originales rapportées. La méthode proposée permet également le marquage de peptides non protégés. Dans le futur, le chélateur bifonctionnel et le groupe prothétique seront conjugués à différents dérivés peptidiques ciblant des récepteurs impliqués dans le cancer et des tests de compétition, de saturation, de biodistribution et d’imagerie µTEP seront effectués. / Abstract : PET is a powerful imaging modality that follows tiny labeled tracer concentrations for the detection of cancer and other pathologies. There currently is a growing interest for the development of peptides as diagnostic and treatment tools in oncology. This interest is justified by the fact that peptides are tolerant to the presence of bifunctionnal chelators or prosthetic groups for labeling with many radiometals (64Cu, T1/2 = 12,7 h, 68Ga, T1/2 = 68 min, etc.) or 18F (T1/2 = 109,8 min), without losing their biological activity. The objective of the work reported in this document was to develop innovative and efficient molecular tools that facilitate peptide labeling for PET imaging. Specifically, they are a bifunctionnal chelator and a fast and selective prosthetic group conjugation method. On the first aspect, a lysine analogue bifunctionnal chelator bearing methylhydroxamate ligands was synthesized in solution through a double bisalkylation. The preliminary results show a weak Cu(II) chelation, but they are to be pushed forward with 68Ga and 89Zr. On the second aspect of 18F radiolabeling, synthetic procedures were optimised for two steps, which are the prosthetic group’s labeling and its conjugation to the peptide. First, the labeling conditions for a SNAr reaction with 18F- were developed, to yield the necessary 18F-thioester prosthetic group. Then, its conjugation to the peptide by the chemical ligation reaction through its three steps 1) a favored transthioesterification between the thioester and thiol peptide segments; 2) an irreversible rearrangement of the thioester intermediate to a N-(oxyalkyl)amide, followed; 3) the auxiliary cleavage. By this work, it has been proven that the new one pot methodology accelerates the reaction and allows the 18F labeling of unprotected peptides, which limits secondary reactions and the number of post peptide labeling steps. The prosthetic group conjugation to both model compound and peptide takes place in 26-55 min, compared to the 48 h initially reported. The proposed method also allows the labeling of unprotected peptides. In the future, the bifunctionnal chelator and the prosthetic group will be conjugated to different peptide derivatives that target cancer implied receptors and competition, saturation, biodistribution and µPET imaging assays will be performed.

Ce travail de thèse cherche à s’interroger sur la pratique du pianiste soliste, interprète de la musique dite « pure » ou absolue. Nous nous appuyons pour cela sur plusieurs réflexions psychanalytiques à propos de la musique. En psychanalyse, la musique vocale ou instrumentale est abordée à partir de l’objet de la « pulsion invocante » qu’est la voix. Nous concevons la « voix instrumentale » produite par le pianiste et son instrument comme étant une prothèse de la voix chantée humaine. Nous nous appuyons également sur la notion d’« humain augmenté », apparue dans les années 1990, pour faire l’hypothèse que la musique pour piano est une voix augmentée. L’objectif du pianiste est de pouvoir s’exprimer à travers ce que nous avons nommé la « voix prothétique augmentée ». Pour s’exprimer à travers cette « voix prothétique augmentée », le pianiste doit d’abord faire transcender son propre corps, car le support de cette voix est ses gestes corporels. / This thesis seeks to question the practice of a piano soloist – the interpreter of so-called “pure” or absolute music – drawing on several psychoanalytical reflections about music. In psychoanalysis, vocal or instrumental music is approached from the object of the “invocative impulse” – the voice. We perceive the “instrumental voice”, produced by the pianist and his or her instrument, as being a prosthesis of the singing human voice. We also draw on the notion of the “augmented human”, which first appeared in the 1990s, to form the hypothesis that music for piano is itself an augmented voice. The objective of the pianist is to be able to express him or herself through what we have termed the “prosthetic augmented voice”. To express him or herself through this “prosthetic augmented voice”, the pianist has to, first of all, transcend his or her own body. This is because the support of this voice is in fact his or her own body gestures.

Background: Meeting the prosthetists needs, and barriers is important to successful implement evidence into practice. Thus, several implementation methods have been initiated, the authors saw a need to create a Communication Tool based on evidence from transfemoral socket designs, to ease the process of assisting prosthetists in facilitating evidential reasoning. Aim: To investigate if a Communication Tool can facilitate the usage of evidence into practice by assisting the clinical decision making. Method: The study is divided into two parts. First the development of the Communication Tool, which is based on the Knowledge to Action model plus an update of studies from a systematic review by Brodie et al.2021. The second part consists of a qualitative pilot-study using semi-structured synchronously interviews to collect data from two Danish working prosthetists. For the analysis a conventional content analysis with an inductive approach was used. Findings: Findings from the analysis of the interviews compiled eight categories and twenty-five sub-categories covering three key themes identified during the creation of the Communication Tool: Barriers, internal usability, and external usability of the Communication Tool. Conclusion: The Communication Tool showed to be an effective method to assist and facilitate evidence-based reasonings. The participants found the tool to be useful and appraised positively regarding communicating relevant content an easy to navigate in. Minor improvements are needed, which are related to the quality assessment system and the system of reference as well as some linguistic formulations.

Energy Harvesting, also known as power harvesting or ambient power, is the process of obtaining small amounts of power from secondary sources, such as vibrations, light, temperature variations and even radio-frequency emissions. These systems have been uncommon in personal and wearable electronics in the past, however they are slowly gaining traction. With the increasing sophistication of prosthetic limbs and implants, devices that in some cases require a consistent and reliable power source, the potential field of application for energy harvesting grows wider. This thesis project evaluates whether energy harvesting methods could be implemented in future prosthetic limb designs without significantly affecting weight, user comfort, complexity of design etc., and whether the gains of such an implementation would be worth the effort and cost put into it. For reference the project used the RHEO KNEE® by Össur Hf., a microcontroller controlled prosthetic knee, as a device that such a system could be integrated with. Energy harvesting is still an emerging field and is a long time away from being a viable primary power source for most electronic devices. However, it still might have potential as a supplementary source for extending charge cycles or making smaller (and therefore more lightweight) power cells viable. This master’s thesis project was broad in scope and included 3D-design; mechanical, electrical and embedded software design; and setting up a miniature kinetic power generator as well as a photovoltaic harvesting system. No amputees were available for testing the designs so the system was tested with a 3D-printed model that was moved by hand to simulate the generation process. Due to some incorrect inital assumptions, the final electronic design was not optimal for this kind of system. However, a kinetic generator that harvested power from a modeled heel striking the ground 50 times a minute produced about 23mW of power. 53cm2 of photovoltaic panels produced 42μW of power in an ambient light setting. For comparison, a low-power microcontroller needed about 119μW of power on average to do some simple processing and send Bluetooth transmissions once every two seconds. / Energiinsamling (e. Energy Harvesting), är processen för att erhålla små mängder kraft från sekundära källor, såsom vibrationer, ljus, temperaturvariationer och utstrålning i radiofrekvens. Dessa system har varit ovanliga i hemelektronik och bärbar teknik, men de vinner sakta dragkraft. Med den ökande förfining av proteser och implantat, som i vissa fall kräver en jämn och pålitlig strömkälla, växer det potentiella användningsområdet för energiinsamling. Detta examensarbete utvärderar huruvida energiinsamlingsmetoder skulle kunna implementeras i framtida proteskonstruktioner utan att nämnvärt påverka vikt, användarkomfort, komplexitet i design etc., och om vinsterna med en sådan implementering skulle vara värd ansträngningen och kostnaden. Som exempel använde detta projekt en datoriserad knäprotes av Össur HF, RHEO KNEE®, som exempel på ett system som energiinsamling skulle kunna integreras med. Energiinsamling är fortfarande ett växande forskningsområde och är långt ifrån att en strömkälla för det mesta elektronik.. Det kan ändå ha potential som en kompletterande strömkälla som kan förlänga laddningscykler eller göra mindre (och därför lättare) batterier möjliga. Detta examensarbete var brett i omfattning och inkluderade 3D-design; mekanisk-, elektrisk- och mjukvara-design; och inrättning av en kinetisk kraftgenerator i miniatyr samt ett ljusdrivet energiinsamlingssystem. Inga amputerade var tillgängliga för att testa designen, därför så testades systemet med en 3D-printad modell som rördes för hand för att simulera strömförsörjelseprocessen. På grund av några felaktiga initiala antaganden var den slutliga elektroniska designen inte optimal för denna typ av system. Ändå lyckades en kinetisk generator som använde energiinsamlingsprinciper producera cirka 23mW ström genom en simulerad häl som träffade marken cirka 50 gånger i minuten. 53cm2 solcellspaneler producerade 42μW energi i en ljussatt miljö. Þetta meistaraverkefni var viðamikið að því leiti að það fól í sér þrívíddarhönnun; vél-, raf- og hugbúnaðarhönnun; og uppsetningu á hreyfirafal ásamt ljósorkuöflunarkerfi. Engir einstaklingar sem misst hafa fót voru til staðar til að prófa hannanir þessa verkefnis.
