The development and evaluation of a programme to promote sensitive pscyhotherapeutic practice with gay men and lesbians

Clinical psychology’s relevance and future viability depend on its ability to render services that are relevant and sensitive to multicultural and minority issues. Lesbian, gay, bisexual and transgendered people are one such group that professional psychology – both in South Africa and abroad - has identified as having unique treatment needs for which psychologists require specialised knowledge and skills in order to render appropriate treatment. Competence to treat non-heterosexual patients has been framed in terms of a gay affirmative paradigm which has as its basic tenet the recognition that same-gender orientation is not pathological but rather a healthy alternative to heterosexuality. From this perspective being “gay friendly” or “gay accepting” is not enough. To implement a gay affirmative approach in practice, practitioners must have resolved their possible prejudice and heterosexist bias and have the requisite knowledge of concerns unique to lesbian, gay, and bisexual (LGB) individuals to be able to apply their skills in a culturally sensitive manner. Although more American post-graduate psychology programmes are addressing sexual diversity, their failure to produce psychologists who feel competent to treat lesbian/gay or bisexual individuals has highlighted the need to develop effective training strategies based on empirical nvestigation. The dearth of comparable data about local South African psychology training prompted this inquiry which had four broad aims namely, (i) to establish to what extent trainees’ prior training had equipped them with the knowledge, awareness, and skills to approach their work with non-heterosexual patients in a gay affirmative manner, and (ii) to implement and (iii) to evaluate to what extent a brief structured training programme is effective in engaging the trainees; in increasing knowledge, in raising awareness, and in changing specific attitudes and imparting specific skills required for treating lesbian and gay patients.; and (iv) what, if any, recommendations should be made for the future with respect to training of sychologists in this area? The field of sexual orientation research has been expanded to include issues pertaining to bisexual, transgendered and intersexed people, but serious time constraints meant that issues pertaining to these groups could not be addressed in depth. Although the exclusion of these groups is problematic and may be seen as reinforcing their invisibility, it was decided to focus primarily on gay and lesbian issues s an introduction to same-sex orientation. It is envisaged that bisexual and transgender issues would be dealt with in depth in more advanced training. Nine trainee psychologists employed at hospitals in the greater Cape Town area volunteered to participate in the programme which comprised a series of two-hour experiential workshops offered once a week over six weeks. The study employed both quantitative and qualitative data analysis methods. The first stage entailed gathering information to better understand trainees’ existing level of competence. Individual interviews were conducted prior to the course to obtain data about their attitudes and perceptions regarding the need for such specialised training, and how qualified they considered themselves to be to treat LGB patients, and their experience in this regard. For the purpose of the over-all analysis information was also gathered about pertinent personal and social characteristics of the trainees, as well as their contact with lesbian/gay persons. In addition, an attitude survey and the Lesbian, Gay, and Bisexual Counselling Self-efficacy Scale (LGB-CSI) were administered to obtain benchmarks against which change could be measured. The second stage involved the implementation of the educational programme and gathering information about trainees’ responses to its various components. This stage concentrated on discovering how individual trainees reacted to material on lesbian, gay, and bisexual issues and how they used the programme to improve their self-awareness and skills. The results indicate that local psychology training might not address same-gender orientation adequately, thus reinforcing trainees’ belief that sexual orientation is irrelevant, and that their generalist training equips them to work with gay/lesbian/bisexual patients. While the training strengthened existing positive attitudes, it was less effective in changing blatant antigay prejudice. However, both quantitative and qualitative data suggest that the programme increased individuals’ awareness and insight into their previously unrecognised heterosexist biases and created greater understanding of the effects of stigmatisation on sexual minority individuals. In addition, the training increased trainee’s sense of competence to provide affirmative treatment as evidenced by the significant differences between the pre- and post-training mean scores on the Relationship, Knowledge, and Advocacy Scales and between the mid- and post-training means scores on the Assessment and Awareness Scales of the LGB-CSI. Despite the limited generalisability of these findings on account of possible sampling bias, the need and value of such training was confirmed by trainees’ recommendation that this programme should be a mandatory offering in the first year of clinical psychology training.

The possibility of psychotherapeutic privilege in South Africa

Privilege is an evidential principle which, on the grounds of public policy, excludes evidence relevant and otherwise admissible. This thesis aims to discover whether privilege should be applied to the psychotherapeutic profession in South Africa. At present, the only profession in South Africa afforded privilege is the legal profession. There are two main theoretical justifications for privilege: the utilitarian and the individual/human rights approach. This thesis considers whether the psychotherapeutic profession wan-ants privilege under either theory, and recommends that the law of privilege integrate both theories rather than adopt one or the other. The impact of the Constitution and the right to privacy receive particular attention. Very little literature or case law on the question of psychotherapeutic privilege was found in South Africa. Consequently, extensive comparative research into the common-law systems of England, Canada and United States of America was done. This research yielded some interesting findings. The first is that case-by-case development of the law of privilege is uncel1ain and fragmented. The next is that psychotherapeutic privilege exists in almost absolute form in the United States of America, but is littered with lacunae causing as much uncertainty as the case-by-case approach to privilege law. The most helpful method of dealing with the problem was found in Canada, where a two-stage approach to protecting personal information, including psychotherapeutic records, has been developed. In light of this research, this thesis reassesses the original viability of psychotherapeutic privilege in South Africa. Privilege, it was found, is not the best solution to protecting psychotherapeutic communications. This thesis recommends legislative adoption of an amended two-stage approach based on the Canadian model for sexual offence trials as the best method of protecting psychotherapeutic communications in both civil and criminal proceedings. The thesis ends by suggesting draft legislative provisions.

The management of psychosocial hazards at work throufg the psychological lens

Due to the rise of psychosocial hazards and its increasing healthcare associated costs in Europe but also in the whole World, the poor management of those risks becomes an urgent issue to overcome for the states and companies but above all, for the people who suffer from it. The problem is that there is not enough awareness among the society about psychosocial factors and its associated risks. Our master thesis aims at examining and determining the roots of psychosocial hazards by starting from an already existing case study to set and classify the main variables. Thus, through the lens of psychologists and psychiatrists we interviewed with a semi-structured questionnaire, we compare and analyze these variables and their answers in order to identify what are the most important factors to deal with in priority and how to manage them properly. Again, through the lens of the specialists in the field and ours, we question the risk management paradigm presented as a solution to tackle psychosocial hazards in the European survey we found. The main goals of our thesis are to raise awareness among people about psychosocial factors and its associated risks but also to provide a solution to identify and manage them properly in a lucid and realistic leadership but also more human, thanks to the enlightening vision of our dear interviewees.

Uma abordagem para indicar o estado emocional de usuários em tempo de interação / An approach to indicate the users\' emotional state at interaction time

O estado emocional dos usuários influencia na tomada de decisão e é essencial para o conhecimento e explicação do comportamento dos usuários com aplicações computacionais. Sistemas computacionais fazem parte da vida cotidiana, influenciando o comportamento humano e estimulando mudanças nos estados emocionais. A avaliação das emoções dos usuários, durante a interação com aplicações computacionais, pode ajudar a fornecer interfaces flexíveis e melhores sistemas de recomendação. No entanto, as emoções são complexas e difíceis de identificar ou avaliar. Pesquisas anteriores demonstraram que o uso de sensores individuais, em um cenário do mundo real, não fornece uma avaliação emocional precisa. Acredita-se que somente uma visão holística pode permitir tirar conclusões significativas sobre o estado emocional dos usuários. Assim, nesta Tese, propõe-se uma abordagem chamada UserSense, que considera vários sensores para estimar os estados emocionais dos usuários em tempo de interação. A abordagem proposta, a partir de múltiplos sensores, considera várias entradas de usuários (como a fala, movimentos faciais, frequência cardíaca e atividades) e utiliza métodos de inteligência artificial para mapear essas diferentes respostas em um ou mais estados emocionais. Para a validação, os dados coletados pelos sensores durante a interação do usuário foram confrontados com os dos especialistas (psicólogos). A Teoria Componencial das Emoções e o Espaço Emocional Semântico de Scherer são utilizados na fundamentação teórica da abordagem UserSense que, baseada no Middleware Adaptativo OpenCom, incorpora funcionalidades para carregar e descarregar novos recursos, necessários a infraestrutura proposta. Os resultados experimentais mostram que a combinação de resultados gerados por vários sensores, fornece uma avaliação mais precisa dos estados emocionais do que considerar sensores individualmente. / Users emotional state influence decision making and is essential for the knowledge and explanation of users behavior with computer applications. Computer systems are part of everyday life, influencing human behavior and stimulating changes in users emotional states. The assessment of users emotions during interaction with computer applications can help to provide tailorable interfaces and better recommendations systems. However, emotions are complex and difficult to identify or assess. Previous studies have shown that the use of single sensors, in a real-world scenario, does not provide accurate emotional assessment. We believe that only this holistic view can allow us to draw significant conclusions about the users emotional states. Hence, in this Thesis we propose a approach called UserSense, that takes into account multiple sensors to estimate the users emotional states at interaction time. The proposed multi-sensing approach considers several inputs from users (such as speech, facial movements, heart rate and activities), and uses artificial intelligent methods to map these different responses into one or more emotional states; for validation, the data collected by the sensors during the user interaction were confronted with specialists (psychologists). Componential Emotion Theory and Scherers Emotional Semantic Space are used in the theoretical foundation of the UserSense approach, that takes into account Adaptive Middleware OpenCom to embed functionalities to load and unload new required features to proposed infrastructure. The experimental results show that the combination of outputs generated by multiple sensors provides a more accurate assessment of emotional states than considering sensors individually.

In this context, they interfere the actions in mental health, based on the presuppositions of the Brazilian Psychiatric Reform. Like this being and before the lack of studies concerning this reality, the accomplishment of this study was looked for with the objective of understanding as it is had given the these professionals' praxis, with emphasis in the subject of the worker's health, as manifestation of an attitude of the caretaker's care. The scenery that permeates the day by day of the workers of health aims us subjects concerning the relationship suffering/care for besides a dynamics strictly intra-psychic, crossed, in whole moment, for a series of subjects of stamp social-psychic and institutional. In this sense and starting from an approach with the logic and guidelines that the operation of the services and models of the attendance in the ambit of the Attention to the Mental Health in the context of SUS, an amplification of the understanding of the stress was looked for in the work and, more specifically, of the Syndrome of Burnout, as possible indicators of the suffering panorama in which interfere these subjects. The approach of this problematic one allowed the ransom of the implication of subjects related to the modern vision of technique, to the processes of construction of the caretaker's social image, to the professional formation and, finally, to the relationship among anguish and care in its existential aspects. Starting from a positioning of the existential phenomenological, as attitude of to be and to research, they were picked narratives, while reports of lived experience, of professionals inserted in this net of services of attendance. The depositions pointed, among other subjects, aspects related to the transitory character and, per times, paradoxical, in that she meets the formation of the substitution net in Mental Health, with emphasis in the painful existences, fruit of the discrepancy between the professional's expectation and institutional conditions. Before this, it discusses him that, starting from the complexity implied in the situation of the caretakers' negligence, she makes himself necessary the maintenance of the tension among the several involved aspects, as well as the constant rising of subjects of the paper of each element of the system, in a care attitude that implies them collectively ambit of the care while ethical and political attitude, crossed by the care of itself, the institutional care and the care of the another / Diante das recentes transformações no cenário da assistência em saúde pública em nosso país, fruto da Reforma Sanitária e implantação do Sistema Único de Saúde (SUS), no final da década de 80, percebe-se a necessidade de ampliação de estudos voltados a compreender a dinâmica dos profissionais envolvidos neste processo. Neste contexto, inserem-se as ações em saúde mental, baseadas nos pressupostos da Reforma Psiquiátrica Brasileira. Assim sendo e diante da carência de estudos acerca desta realidade, buscou-se a realização deste estudo com o intuito de compreender como tem se dado a práxis destes profissionais, com ênfase na questão da saúde do trabalhador, como manifestação de uma atitude de cuidado do cuidador. O cenário que permeia a cotidianidade dos trabalhadores de saúde nos aponta questões acerca da relação sofrimento/cuidado para além de uma dinâmica estritamente intra-psíquica, perpassada, em todo momento, por uma série de questões de cunho psicossocial e institucional. Neste sentido e a partir de uma aproximação com a lógica e diretrizes que balizam o funcionamento dos serviços e modelos assistenciais no âmbito da Atenção à Saúde Mental no contexto do SUS, buscou-se uma ampliação da compreensão do estresse no trabalho e, mais especificamente, da Síndrome de Burnout, como sinalizações possíveis do panorama de sofrimento no qual se inserem estes sujeitos. A aproximação desta problemática permitiu o resgate da implicação de questões relacionadas à visão moderna de técnica, aos processos de construção da imagem social do cuidador, à formação profissional e, finalmente, à relação entre angústia e cuidado em seus aspectos existenciais. A partir de um posicionamento fenomenológico existencial, como atitude de ser e pesquisar, foram colhidas narrativas, enquanto relatos de experiência vivida, de profissionais inseridos nesta rede de serviços assistenciais. Os depoimentos apontaram, dentre outras questões, aspectos relacionados ao caráter transitório e, por vezes, paradoxal, em que se encontra a implantação da rede substitutiva em Saúde Mental, com ênfase nas vivências dolorosas, fruto da discrepância entre expectativa do profissional e condições institucionais. Diante disto, discute-se que, a partir da complexidade implicada na situação de descuido dos cuidadores, faz-se necessária a manutenção da tensão entre os diversos aspectos envolvidos, bem como a constante problematização do papel de cada elemento do sistema, numa atitude de cuidado que implique coletivamente os âmbito do cuidado enquanto atitude ética e política, atravessados pelo cuidado de si, o cuidado institucional e o cuidado do outro

This severe health situation leads the patient into a deep depression and consequently makes him/her feel that his/her life may come to an end some time in the near future. Therefore the severely-ill patients feel themselves depressed and helpless. Because they underwent very serious burns and due to their high level of morbidity they feel emotinally impaired. They need strength so that they are able to stand and face treatment. Moreover, they need to be conscious of their own deep suffering in order to face it with courage and in a positive way. The methodology chosen has a phenomenal and existential basis. Hence interviews were made with the patients and the medical staff as well so that one can understand how this tough experience happens and also what it means to the patient. Based on the patients report it was found that they feel anxious and anguished both for not knowing what is ahead for them, e. g. if they will get better and also because they are aware of death: they know they may die! The author decides to name vividação that very situation which Frankl mentions when the patients make use of their own courage, hope and sympathy to overcome anxiety and suffering. Heidegger thinks that anguish and anxiety both force the patients to care for themselves, and turn them strong in their deep inner loneliness. This fact shows that vividação is in fact ação vivida , e. g. the patients themselves fighting to defeat all that anguish, anxiety and suffering / O objetivo deste estudo foi refletir sobre o paciente grande queimado na situação-limite em que se encontra, e quais os recursos psicológicos presentes para que ele possa enfrentar, junto com as ações da equipe de saúde, o tratamento necessário. O adoecimento agudo põe este ser/paciente numa situação de (des)continuidade existencial. Desestabiliza-se emocionalmente, pela experiência vivida e pelo seu alto grau de morbidade. Precisa de força para suportar e enfrentar o tratamento e se (re)conhecer em seu sofrimento. Para compreender como essa experiência acontece e que sentido o paciente dá à situação, escolhi uma metodologia de cunho fenomenológico existencial e, através de entrevistas com os pacientes e com os profissionais da equipe, fui percorrendo o caminho que cunhei vividação, articulando-o ao que os estudiosos do cuidado podiam me dizer. Ouvindo as narrativas, pude concluir que, nessa experiência, a angústia revela-se, por um lado, pela incerteza e, por outro lado, pela certeza da finitude. Tanto no limite do paciente, quanto no cuidado da equipe hospitalar, ela é uma só. Ouvindo Frankl encontrei o sentido da vividação na coragem, na esperança e na solidariedade, enquanto que à luz de Heidegger, a angústia mobiliza o cuidar de si e o encontrar-se na solicitude, indicando que vividação é ação vívida e percorre a serenidade para manter-se corajosamente sendo

From this view point when one researched about the saber fazer , that is, the psychological and clinical know-how, there were a great number of possibilities as to the fazer saber , e.g. how to reach the suffering child in his/her innerself and let him/her know that there is someone who in fact can help. It was found out that there were a number of attitudes, actions, interventation and interpretation which the psychologist could make use of in his relationship and care for the subject, the burned child. Therefore, the psychologist and/or researcher should be conscious about how broad the psychological clinical field is, and further he/she needs to understand clearly what may help as well as what may prevent a certain contextual action . One should start from understanding the subject s individual experience as a unique human being. This study aims to take a rather different look at the burned child specific experience, and tries to provide a new meaning to the psychological clinical action, inside a hospital, enlightened by both the existential phenomenon and attitudes for the psychological clinical action. The qualitative methodology was chosen, and it was inspired by the narrator himself. It was also supported by the phenomenal method for interpretation. The material analysed is made up of narratives based on the psychologist/researcher experiences, as well as from the burned child and the mother, who were in the Burned Treatment Center (BTC). In addition, reports from professionals belonging to the same health staff were considered. Finally , special attention was given to both the experienced/affectionate knowledge and the theoretical one, all deriving from talks to area field experts. It was discovered that the psychological action done at the BTC is in the pratical-theoretical field from the Psychological Advice, and it is deemed as an educational and psychological-clinical practice, in an interdisciplinary dimension of a clinical social action. M oreover, some special peculiarities were found from the communication among the professional psychologist, the burned child and the mother. Those peculiarities were the tools which facilitated the psychologist / Considerando a experiência de aflição da criança queimada hospitalizada, buscou-se pensar, como maior propriedade, sobre os modelos assistenciais clínicos instituídos, disponibilizados no atendimento a sua demanda psicológica. Nessa perspectiva, ao pesquisar sobre o saber/fazer clínico psicológico, revelaram-se múltiplas atitudes, ações, intervenções, orientações, interpretações, possíveis e cabíveis, ao psicológico na relação/cuidado com o sujeito. Lançando como profissional e/ou pesquisador no vasto campo da clínica psicológica , é importante compreender o que facilita ou dificulta uma ação situada. Parte-se de uma compreensão de ação implicada como constituinte no modo de ser humano: sujeito que age. Assim, este trabalho visa, a partir de uma experiência específica, trazer um outro olhar, ressignificando o fazer clínico psicológico no contexto do hospital, à luz da fenomenologia existencial e de dispositivos para a ação clínica psicológica. Recorreu- se à metodologia qualitativa, inspirada na figura do narrador e amparada no método fenomenológico para interpretação. A matéria-prima desta obra consiste em narrativas da experiência do psicólogo/pesquisador e dos interlocutores nos atendimentos, criança e sua mãe, no Centro de Tratamento de Queimados, além de relatos de profissionais dessa mesma equipe de saúde. Ao final, procurou-se uma articulação entre o conhecimento vivido/afetivo e o conhecimento teórico, a partir do diálogo com autores. Vislumbrou-se que a ação psicológica no C.T.Q. situa-se no campo teórico-prático do Aconselhamento Psicológico, como prática clínico- psicológica e educacional, numa dimensão interdisciplinar de ação social clínica. Foram, ainda, evidenciadas peculiaridades da comunicação entre a psicóloga, a criança e sua mãe, compreendidas como utensílios/utilitários facilitadores da intervenção psicológica. Espera-se, com este trabalho, apontar aberturas para ousadias responsáveis na atuação clínica, com implicações para o estudo e prática interdisciplinar

Attitudes Toward Psychodiagnostic Testing and Doctoral Clinical Psychology Students' Professional Expectations and Training

Responses of 111 doctoral clinical psychology students to Garfield and Kurtz' (1973) Testing Attitude Scale were subjected to a 2 x 2 factorial analysis. Attitudes toward psychodiagnostic testing were found to be related both to academic versus nonacademic professional expectations (academics scoring more negatively, M = 32.69, than nonacademics, M = 37.19), F (1, 107) = 5.994, p < 0.016, and to internship training exposure (non-interns scoring more negatively, M = 34.64, than interns, M = 38.80), F (1, 107) = 10.321, p< 0.002. Results paralleled previous research on academic and nonacademic working psychologists' attitudes. Similarities in students' and role models' attitudes were discussed in terms of Kelman's (1953; 1958), Festinger's (1957), and Bem's (1970) attitude theories. Results seemed to imply continued controversy over both the desirability of producing psychodiagnostically oriented clinicians, and also traditional paradigms of psychodiagnostic training.

Discourses on Racism

Philosophiae Doctor - PhD / A central aim of this study is to examine the meanings which (i) a group of South African psychologists and (ii) a group of Black parents give to racism in their discourses and how these meanings are linked to existing relations of domination. To this end the discourses on racism produced by the former in various journal articles and the latter in various group discussions are submitted to analysis. The study basically utilizes the following working hypotheses as its point of departure: (i) that the discourses produced by the group of psychologists - in so far as they could broadly be seen as being representative of prevailing dominant discourses - would, to varying degrees, reflect attempts to legitimate and reinforce the relations of domination which the ideology of racism entails; and (ii) that despite certain similarities between . dominant group discourses and dominant group discourses on racism, the latter's discourses would, to varying degrees, be the site of resistance against dominant group discourses as well as against their domination. The findings of the study seem to support the basic postulates contained in the two working hypotheses presented above. More specifically, the analysis of the discourses collected reveals (i) that, though ostensibly very disparate, the discourses produced by the group of psychologists, by and large, appear to justify and dissimulate the asymmetric relations of power which the ideology of racism maintains; and (ii) that despite the similarities between dominant group and dominated group discourses the latter, in a variety of ways, undermine the ideology of racism as well as dominant group discourses on the ideology. Syftet var att översätta och påbörja utvärdering av självskattningsformuläret The Lesbian, Gay, Bisexual, and Transgender Development of Clinical Skills Scale (LGBT-DOCSS) i ett urval av psykologer inom hälso- och sjukvården; detta för att kunna undersöka deras kliniska färdigheter att arbeta med hbtq-klienter/patienter. Översättning av LGBT-DOCSS genomfördes, och påståenden relaterade till queer lades till för att inkludera denna grupp. Bakgrundsfaktorer som undersöktes inkluderade utbildning, relationer till hbtq-personer och professionell erfarenhet av arbete med hbtq-personer, social önskvärdhet och förekomst av normkritiskt förhållningssätt. 218 psykologer verksamma i svensk sjukvård besvarade en webbaserad enkätundersökning. Resultatet visade att faktorstrukturen för LGBT-DOCSS reviderades från originalversionen. Deltagarnas kliniska färdigheter gällande hbtq var måttligt hög, med mycket låga nivåer av negativa attityder. Utbildning, privata relationer till hbtq-personer, erfarenhet av att arbeta med patienter ur hbtq-gruppen samt normkritiskt förhållningssätt hade positiva samband med utfallsmåttet, inget samband fanns med social önskvärdhet. Förekomsten av hbtq-relevant undervisning på psykologprogrammen var låg. Studien bidrar med kunskap om psykologers kliniska färdigheter att arbeta med personer som identifierar sig som hbtq. / The purpose was to translate and begin the evaluation of The Lesbian, Gay, Bisexual, and Transgender Development of Clinical Skills Scale (LGBT-DOCSS) self-assessment form in a sample of psychologists in health care; this in order to be able to examine their clinical skills to work with LGBT clients/patients. Translation of LGBT-DOCSS was performed, and items related to queer were added to include this group. Background factors that were examined included education, relationships with LGBTQ people and professional experience of working with LGBTQ people, social desirability and the existence of norm-critical approaches. 218 psychologists working in Swedish health care answered a web-based survey. The result showed that the factor structure of the LGBT-DOCSS was revised from the original version. Participants' clinical skills in LGBTQ were moderately high, with very low levels of negative attitudes. Education, private relationships with LGBTQ people, experience working with patients from the LGBTQ group and norm-critical approach had positive correlations with the outcome measure, no relationship was found with social desirability. The presence of LGBTQ-relevant teaching on the psychology programs was low. The study contributes with knowledge of psychologists' clinical skills to work with people who identify themselves as LGBTQ.
