An interaction between statins and clopidogrel : a pharmacoepidemiology cohort study with survival time analysis

Clopidogrel is an antiplatelet drug prescribed to prevent stent thrombosis after a percutaneous coronary intervention (PCI). Previous evidence suggests that some widely prescribed statins may inhibit the antiplatelet effects of clopidogrel via competitive metabolism of its activating enzyme cytochrome P450 3A4 (CYP3A4). / The objective was to investigate the possibility of an interaction post-PCI between statins and clopidogrel. / We carried out a population-based cohort study identifying 10,491 patients using clopidogrel post-PCI (2001-2004). The outcome was a composite of death of any cause, myocardial infarction, unstable angina, repeat revascularization, and cerebrovascular events. We found that co-prescription of CYP3A4-metabolized statins (hazard ratio (HR) 0.95, 95% confidence interval (CI) 0.79-1.15), or non-CYP3A4-metabolized statins (HR 0.82, 95% CI 0.63-1.07) with clopidogrel was not associated with increase in adverse outcomes. / We observed no evidence of interaction between clopidogrel and statins in a large population cohort of PCI patients, suggesting unlikelihood of an important interaction.

Treatment of Locally Advanced Prostate Cancer – The Case for Radical Prostatectomy

The treatment of clinically locally advanced prostate carcinoma (stage cT3) remains controversial. One of the main reasons for this controversy results from the substantial staging error attached to the clinical diagnosis cT3 with overstaged T2 tumors and understaged node-positive cases. Treatment options in this situation include radical prostatectomy, external beam radiotherapy, immediate or delayed androgen deprivation treatment and the so-called ‘watchful waiting’. Acceptable and often surprisingly good tumor-specific survival rates have been reported for radical prostatectomy in pT3 series – based on good clinical case selection – approaching those of pT2 series. In lymph node-positive pT3 cases, adjuvant hormone deprivation seems to prolong survival which it does not in lymph node-negative pT3 disease. A benefit of adjuvant external beam radiotherapy after radical prostatectomy for pT3 cases in prolonging overall survival has not been shown, despite the fact that it can prevent or delay biochemical and local recurrence. External beam radiotherapy as the only treatment for cT3 disease results in unfavorable tumor-specific survival rates, which can be significantly improved with adjuvant hormonal treatment with LHRH agonists. Conselho Nacional de Desenvolvimento Científico e Tecnológico / Objectives: To evaluate the survival analysis and the fracture resistance of weakened (W) and not weakened (NW) roots restored with different intracanal systems posts. Methods: Eighty teeth were sectioned in 16mm and prepared in 12mm with a custom drill. Fifty roots were weakened with a tapered diamond drill and 30 kept without weakening. The specimens were embedded with acrylic-resin in PVC cylinders until 3mm coronal (bone limit) and the periodontal ligament was simulated with polyether material. The experimental design was: G1 and G2: cast and post core (Golden Alloy) with (CPC-GL W) or without (CPC GL NW) weakened roots, respectively; G3 and G4: cast and post core (Ni-Cr alloy) with (CPC Ni W) or without (CPC Ni NW) weakened roots, respectively; G5 and G6: fiber-posts with (FP NW) or without (FP W) weakened roots; G7: fiber-posts especially designed for weakened roots (FP-E W); G8: fiber-posts relined with composite-resin for weakened roots (FP-CR). All posts were adhesively cemented. All of the restored specimens were submitted to the mechanical cycling (37ºC, 45º, 130N, 2.2Hz and 1.5 million pulses) and follow up intervals between cycles 5x104 evaluating the presence of the event "crack". The specimens that survived the mechanical cycling were subjected to the fracture-resistance test under load application on palatal zone at 45º-inclination (cross-head speed = 1 mm/min) until failure. Failure mode was classified in favorable (above the simulated bone limit) and catastrophic (below the simulated bone limit). Survival rates were estimated using the technique of Kaplan-Meier method and survival rates according to the study groups were analyzed using the log-rank test (p≤0.05). Fracture resistance data were analyzed by the one-way ANOVA and Tukey test (p=0.05). Results: The post-hoc comparisons showed differences between specimens in groups FP-E and CPC (p=0.004). For fracture resistance statistical differences (p<0.0001) were depicted among the weakened groups: CPC-GL 541.4227.4A, CPC-Ni 642.6219.5A, FP- 282.264.7B, FP-E- 274.151.3B, FP-RC- 216.663.6B. No differences were observed for groups without weakening: CPC-GL- 459.3111.1A, CPC-Ni- 422.0151.9A and FP- 347.991.8A, with majority of favorable failures. Conclusions: weakened roots restored with CPC promoted high values of fracture toughness and failure rates unfavorable. There was higher survival rate for FP-E in relation to CPC (GL / Ni). / Objetivos: Avaliar a sobrevida e a resistência à fratura de raízes fragilizadas e não fragilizadas restauradas com diferentes pinos intracanais. Métodos: oitenta incisivos bovinos foram seccionados em 16 mm e preparados em 12 mm com brocas padronizadas. 50 raízes foram fragilizadas com ponta diamantada mantendo 0,5 mm de espessura e 6 mm intra canal e 30 foram mantidas sem fragilização. Os espécimes foram embutidos em cilindros de PVC até 3 mm coronários (limite ósseo) e o ligamento periodontal foi simulado com poliéter. A divisão dos grupos foi: G1 e G2, núcleo metálico fundido (liga nobre) com (NMF-LN FRAG) e sem (NMF-LN NFRAG) fragilização, respectivamente; G3 e G4: núcleo metálico fundido (liga não nobre) com (NMF-LNN FRAG) e sem (NMF-LNN NFRAG) fragilização, respectivamente; G5 e G6: pino de fibra de vidro com (PFV FRAG) e sem (PFV NFRAG) fragilização, respectivamente; G7: pinos de fibra especiais para canais alargados (PFV-E FRAG) e G8: pinos de fibra re-embasados com resina composta (PA FRAG). Todos os pinos foram cimentados adesivamente. Os espécimes foram submetidos à ciclagem mecânica (37ºC, 45º, 130N, 2.2Hz e 1.5x106 ciclos) e acompanhados entre intervalos de 5x104 ciclos quanto à presença do evento trinca . Os espécimes que sobreviveram foram submetidos ao teste de resistência à fratura sob aplicação de carga na face palatina da coroa (45º e 1 mm/min) até a falha. O modo de falha foi classificado em favorável (acima do limite ósseo simulado) e desfavorável (abaixo do limite ósseo simulado). As taxas de sobrevida foram estimadas pela curva de Kaplan-Meier e as taxas de sobrevivência de acordo com os grupos de estudo foram analisadas através do teste log-rank (p≤0,05). Os dados de resistência à fratura foram analisados pelo teste de ANOVA 1 fator e Tukey (p=0,05). Resultados: para análise de sobrevida, 7 espécimes fraturaram durante a ciclagem mecânica (grupos FRAG: 2 fraturas para PFV, 1 para NMF-LN e 2 para PA, grupos NFRAG: 2 para PFV). As comparações post-hoc demonstraram diferença apenas entre os espécimes dos grupos PFV-E e NMF (p=0,004). Para resistência à fratura houve diferença estatística (p<0.0001) entre os grupos fragilizados: G1- 541,4227,4A, G3- 642,6219,5A, G5- 282,264,7B, G7- 274,151,3B, G8- 216,663,6B com predominância de falhas desfavoráveis. Conclusão: Observou-se maior taxa de sobrevida para PFV-E em relação aos NMF (LN/LNN). Quanto ao teste de resistência à fratura, as raízes fragilizadas restauradas com núcleos metálicos fundidos apresentaram pior comportamento em relação às raízes restauradas com pinos de fibra de vidro (PFV, PFV-E e PA).

This study analyzed ten companies microbusiness retail sector in the city of São Bernardo do Campo who are active and able to overcome and win the mortality business and marketing competition. The investigation was initiated by the ABC region, carrying out a survey in SEBRAE (Brazilian Service of Support for Micro and Small Enterprises) and JUCESP (Board of Trade of the State of São Paulo) to find the amount of micro and small enterprises that were active and that opened between 2006 and 2007. Later the focus was for the city of São Bernardo do Campo where they inaugurated 744 MSEs (Micro and Small Enterprises), and 49 of them were categorized in retail clothing and finally set up ten companies that met the pre-methodological requirements of the study. The discussion on the topic of business longevity, in other words, to understand what were the capabilities that some companies had to keep working for so many years was an essential factor for the study. The aim of this work was to check how to manage and treat their business where entrepreneurs who have made had survival on the market. This study drew on qualitative method of transversal cut with non-probability sample and quota in order to make the methodological process and build the final analysis of the research. / A quantidade de micro e pequenas empresas que encerram suas atividades no Brasil, antes de dois, três ou cinco anos, chega a taxas de mortalidade que se aproximam dos 75%. O presente estudo buscou analisar dez empresas de micro porte do setor de varejo na cidade de São Bernardo do Campo que estão em atividade e que conseguiram superar e vencer as taxas de mortalidade empresarial e de concorrência mercadológica. A investigação iniciou-se pelo ABC paulista, efetuando um levantamento no SEBRAE (Serviço Brasileiro de Apoio às Micro e Pequenas Empresas) e na JUCESP (Junta Comercial do Estado de São Paulo) para descobrir a quantidade de micro e pequenas empresas que estavam em atividade e que inauguraram entre 2006 e 2007. Posteriormente o foco foi para a cidade de São Bernardo do Campo onde foram inauguradas 744 MPE´s (Micro e Pequenas Empresas), sendo que 49 delas estavam categorizadas no setor de varejo do vestuário e por fim definiram-se dez empresas que atenderam aos pré-requisitos metodológicos do estudo. A discussão sobre o tema da longevidade empresarial, ou seja, entender quais foram às capacidades que algumas empresas tiveram de manter-se operante por tantos anos, foi fator essencial para o estudo. O objetivo central deste trabalho foi de verificar a forma de administrar e de tratar de seus negócios destes dez empresários que fizeram com que tivessem sobrevida no mercado. Este estudo valeu-se dos métodos qualitativos, de recorte transversal com amostra não probabilística e por cota para poder efetuar todo processo metodológico e construir a análise final da pesquisa.

O presente trabalho teve como objetivo comparar clinicamente através da taxa de sobrevida e análise de freqüência de ressonância, dois implantes instalados na região anterior da mandíbula, submetidos à carga funcional imediatamente a instalação dos implantes, através de anéis de retenção isolados presos a uma sobredentadura, com um implante instalado na mesma região, livre de carga funcional. Com este objetivo, onze pacientes foram selecionados para instalação de três implantes osteointegráveis em cada, sendo o implante livre de cargas funcionais utilizado como grupo controle. Através dos resultados obtidos, dos vinte e dois implantes submetidos à carga funcional imediata, dois foram perdidos, apresentando uma taxa de sobrevida proporcional a amostra de 93.9%. Quanto ao grupo controle, nenhum implante foi perdido. Após 4 anos de controle clínico, todos os implantes apresentam-se osteointegrados. / The present study aimed to clinically compare through resonance frequency analysis and survival rate, one implant installed in the mandibular anterior region, free from functional load, with two implants installed in the same region, submitted to immediate functional load through isolated retention rings attached to an overdenture. Impacto do número de implantes empregados em protocolos mandibulares - Revisão sistemática

Objetivo: Avaliar pacientes reabilitados com prótese tipo protocolo mandibular e analisar o impacto do diferente número de implantes utilizados na taxa de sobrevivência dos implantes, perda óssea marginal e sobrevivência das próteses. Material e métodos: Esta revisão sistemática foi conduzida segundo orientações do PRISMA e registrada sob número de registo CRD42016048523 (PROSPERO). A base de dados electrônica PubMed/MEDLINE foi pesquisada para artigos publicados até 17 de julho de 2016 sem restrições quanto ao ano de publicação e teve como objetivo responder a seguinte pergunta em formato PICO: "Em pacientes desdentados, protocolos mandibulares suportados por três implantes, comparados com diferente número de implantes, mostram taxa de sobrevivência de implantes, perda óssea marginal e sobrevivência da prótese satisfatórios?" Os estudos foram avaliados segundo os níveis de evidência OCEBM e a qualidade metodológica foi avaliada de acordo com a escala MINORS e ferramenta de risco de viés Cochrane. Foi realizada a estatística descritiva quando aplicável. Curvas de sobrevivência para os implantes foram construídas com o metódo de Kaplan Meyer e a perda óssea marginal foi analisada pelos testes de kruskal-Wallis, Dunn’s e Mann Whitney. Resultados: 21 estudos foram incluídos na síntese quantitativa. 4712 implantes e 1245 protocolos mandibulares foram examinados em 1245 pacientes. Os resultados foram agrupados em categorias com base no número de implantes instalados em cada paciente: grupo 1 (três implantes) mostrou uma taxa de sobrevivência do implante de 90%, grupo 2 (4 implantes) apresentou 95% e o grupo 3 (cinco implantes) atingiu a menor taxa de sobrevivência do implante (74%). Os grupos 1 e 3 apresentaram os menores valores de perda óssea no primeiro ano (0,73 e 0,70 mm respectivamente), com diferença estatisticamente significante do grupo 2 que registrou mediana = 1,31 mm (p = < 0,001). Conclusão: Apesar das limitações quanto à falta de estudos de elevado nível de evidência e da própria metodologia da pesquisa por termos MeSH, pôde-se concluir que protocolos mandibulares suportados por três implantes demonstraram sobrevivência de implantes e perda óssea marginal no primeiro ano satisfatórios. A sobrevivência de próteses, no entanto, foi inferior aos demais grupos e isto sugere um maior acompanhamento de tais reabilitações a fim de esclarecer dados e buscar soluções. / Purpose: To assess edentulous patients rehabilitated by implant-supported mandibular full-arch bridges and to analyze the impact of the different number of implants used on implant survival rate, bone loss around the fixtures and prosthesis survival rate. Material and methods: This systematic review followed PRISMA guidelines and was registered on PROSPERO (registration number CRD42016048523). PubMed/MEDLINE electronic database was searched for articles published up until July 17th, 2016 without restrictions about publication year and aimed to answer the following question in PICO format: “In edentulous patients, full arch-bridges supported by three implants, compared to those with different number of implants, show satisfactory implant survival rate, bone loss and prosthesis survival rate?” Evidence level of studies were evaluated according to OCEBM e the methodological quality by MINORS scale and Cochrane Risk of Bias Tool. Descriptive statistics was performed when applicable. Implant survival curves were constructed with Kaplan-meyer method and marginal bone loss was analyzed with kruskal-Wallis, Dunn’s and Mann Whitney tests. Results: 21 studies were enrolled. Overall, 4712 implants and 1245 implant-supported fullarch bridges were examined in 1245 patients. Results were grouped on categories based on the number of fixtures on each patient: Group 1 (three implants) showed an implant survival rate of 90%, Group 2 (four implants) presented 95% and the Group 3 (five implants) reached the lowest implant survival rate (74%). Group 1 and Group 3 showed the lowest values of first year bone loss (0.73 and 0.70 mm respectively), statistically significant different of Group 2 that registered median = 1,31mm (p = <0.001). Conclusion: Despite of the limitations regarding the lack of high level of evidence studies and the methodology of MeSH terms research itself, it was concluded that full archbridges supported by three implants show satisfactory implant survival rate and first year bone loss. The prosthesis survival rate, however, was inferior to the other groups and this suggests a bigger follow-up of these rehabilitations with the aim of clarifying data and search for solutions. / Dissertação (Mestrado)

The feeding experiment was performed with fry of vimba bream (Vimba vimba) during the first 24 days from the beginning of exogenous nutrition. Six diets were used: algae (Chlorella vulgaris), starter fodder BioOptimal 0,8 mm by Biomar company ? for cyprinids, live nauplii of Artemia salina, combination of alga and starter fodder BioOptimal, combination of alga and nauplii of Artemia salina, combination of starter fodder BioOptimal and nauplii of Artemia salina. Individual weight, total lenght, survival rate of larvae, specific growth rate (SGR) and conditions of the fry (condition coefficient according to Fulton ? FWC) were observed during the experiment. The results were compared statistically using the method of one ? way analysis of variance (ANOVA ? = 0,05; Microsoft Excel).
