Autism Assessment from Home: Evaluating the Remote Childhood Autism Rating Scale, Second Edition (rCARS2) Observation for Tele-Assessment of Autism

Since the start of the COVID-19 pandemic, tele-based methods of autism assessment have been relied upon to a previously unparalleled degree; however, the need for such advancements is not new. Observation-based measures are a crucial component of face-to-face autism diagnostic evaluations, but few validated observation tools exist for remotely assessing autism across childhood, particularly for older children and adolescents, providing minimal guidance in this arena. Sanchez and Constantino (2020) previously validated a brief, face-to-face, clinician-facilitated observation coded according to the Childhood Autism Rating Scale, Second Edition (CARS-2). During the pandemic, this measure was adapted as a remote observation (rCARS2 Observation), but has yet to be validated in this format. The current study validated the rCARS2 Observation against the "gold-standard," in-person Autism Diagnostic Observation Schedule, Second Edition (ADOS-2). In this sample of 30 children ages 1.97 to 16.66 years (M = 7.35, SD = 4.00), ADOS-2 and rCARS2 Observation total scores were highly correlated (ρ = .644, p < .001). Receiver operating characteristic (ROC) curves optimized rCARS2 Observation cutoff scores to maximize sensitivity and specificity in predicting possible diagnostic classification (88.9% sensitivity, 80% specificity) and ADOS-2 classification (83.3% sensitivity, 70.6%, specificity). At optimal cutoffs, the rCARS2 Observation predicted the correct diagnosis in 82.8% of cases and the same classification as the ADOS-2 in 75.9% of cases. Validation of this instrument provides support for an accessible and efficient remote autism observation. This is crucial in ensuring uniform training and clinical procedures for tele-assessment of autism, to help mitigate long-standing barriers to service access (e.g., geography, cost, availability). / Doctor of Philosophy / During the COVID-19 pandemic, many in-person services, including assessments for an autism diagnosis, were forced to shift to telehealth. When an autism assessment is completed in-person, it requires an "observation-based" measure, meaning that a clinician directly observes a child's behavior, in addition to an interview with a parent or caregiver. Families have faced barriers to accessing in-person autism assessments even prior to the pandemic, including geography, transportation difficulties, shortage of autism-specific providers, and high cost of services. As such, tele-assessment of autism is an important tool to consider in helping families to access these services. However, observation-based autism assessment measures that can be completed through telehealth from families' homes are few and not well-understood. The current study explores a brief telehealth observation tool (the rCARS2 Observation) and compares it to the "gold-standard" autism observation-based tool (the ADOS-2) that is typically used during in-person assessments of autism. This study found that scores on this remote measure and the in-person measure were strongly related and the remote observation measure showed great promise for predicting a diagnosis of autism in this study. This is the first study to explore the rCARS2 Observation to understand how well it can evaluate children's autism characteristics from home as compared with an in-person assessment tool. The success of this remote observation measure in the current study is promising and suggests that this may be one remote tool that clinicians can use in combination with a parent- or caregiver-interview to evaluate a child for an autism diagnosis as a shorter and more accessible tool to overcome many barriers to accessing services.

An approach to the notions of body, by the Psychoanalysis and the Phenomenology, to the notion of quasi-body, with a breakthrough about the body representation in Samuel Beckett’s lyric, prose and dramatic works, added with a detailed overview of the body representation in Samuel Becket’s works written for the stage. There is also an aesthetic and historical study about Samuel Becket’s five tele-plays, with a special look on the analysis of the tele-play …but the clouds…, and details about the translation process, production and the accomplishment of the video that is part of this dissertation. At the end, there is an appendix with some of the Portuguese translations of plays, poems and tele-plays quoted in this dissertation, made by the author of the thesis.

The object of study is the spectral, signic corporality, the signic-imagerial arrangement, which gives the user an apparent organicity that renders him present in communication environments. The delimitation of the corpus involves the observation of profiles, avatars and other interfaces of subjective projection in social networks, metaverses and social games to understand the imperative process of continuous spectralization of existence resulting from the natural desire for self-exposure. Having stated the above, the main question guiding this research is: If the projection of existence in cyberspace is aimed at providing a thumbnail sketch of the individual in the midst of information flows, giving him cybermediatic visibility, why is its effect usually that of contributing to his dissolution or invisibility? To answer this question, we propose the hypothesis that to tele-exist, albeit perceived as a pleasant pastime, constitutes an agonistic dispute for the mediatic center stage, which leads to the excessive production of information as a strategy to keep the user continually in the limelight. Everyone wants to appear because they fear insignificance, which is associated with the fear of nonexistence. Because existence from now on seems to make sense only when it passes through cybermediatic hyper-spectacularization, the essence of the individual&#8223;s composition in cyberspace has shifted from Who I am to What I&#8223;m thinking/doing. The effect of the information saturation and excessive visibility resulting from this shift is that every act of projection simultaneously becomes one of dissolution (in)visibility. The methodology for this research consisted of a literature survey and a qualitative field study, based on netnography (translated into its main elements: noninvasive entry into the aforementioned platforms, exploratory survey of profiles and avatars, participant observation, and unstructured interviews with users). The theoretical foundation of this interdisciplinary research (philosophical, sociological, anthropological and psychoanalytical), which provided the necessary epistemological frameworks for the analysis of the object of study, for checking hypotheses, and for solving the research problem, includes philosophical postmodernism, French post-structuralism, cultural semiotics and phenomenological sociodromology / Esta pesquisa tem por objetivo compreender o fenômeno da tele-existência cibermediática existência em tempo real facultada por plataformas ciberculturais de comunicação e relacionamento, em dispositivos tecnológicos fixos ou móveis para dimensionar sua repercussão socioantropológica e, a partir da análise da relação entre subjetividade, tele-existência e visibilidade cibercultural, pensar o estatuto do sujeito na dimensão hiperespetacular. Seu objeto de estudo é o corpo sígnico, espectral, arranjamento sígnico-imagético que confere organicidade aparente ao usuário para presentificá-lo nos ambientes comunicacionais. Na delimitação do corpus, trata-se de observar perfis, avatares e outras interfaces de projeção subjetiva em redes sociais, metaversos e social games para compreender o processo imperativo de espectralização live da existência, decorrente da naturalização do desejo de autoexposição. Isto posto, a principal questão norteadora da pesquisa é: se a projeção da existência no cyberspace tem por objetivo delinear o sujeito em meio a fluxos informacionais, conferindo-lhe visibilidade cibermediática, por que o efeito, via de regra, concorre para a sua dissolução ou invisibilidade? Para tal problemática, acenam-se as hipóteses de que tele-existir, embora percebido como agradável passatempo, constitui disputa agonística pelo centro da cena mediática, o que leva à produção desmesurada de informações como estratégia para manter o usuário continuamente em destaque. Todos querem &#8213;&#8213;apareSer&#8214;&#8214; porque temem a insignificância, correlata ao medo de inexistir. Como doravante a existência só parece adquirir sentido quando passa pela instância (ciber)mediática, o eixo de constituição do sujeito no cyberspace deslocou-se de &#8213;quem sou eu&#8214; para &#8213;em que estou pensando/fazendo&#8214;. Entretanto, por efeito de saturação informacional e excesso de visibilidade, todo ato de projeção é, simultaneamente, dissolução (in)visibilidade. In this sense, I search for demonstrating the importance of the scenic presence in the space formed by the encounter between the tele/spectator (the viewer) and the actor/actress. In order that, the research was developed fundamentally by the dialogue among the philosophical reflections of phenomenology by Martin Heidegger (the dasein concept) and the experience of the actors: Yoshi Oída and Ryszard Cieslak. In the second moment will be focusing the problematic about the relationship manner between the actor/actress with the camera, as well as, the following categories: tele/presence, co/presence, through the analyzing of the Somatic spectacle. To conclude, I submit and analyze an audiovisual experience developed together with the Cia Pã Theater. This study is divided in three parts (chapters) by the intention that each one has been looked as a photogram, an independent fragment, in order to find on the other parts, the reason of his own presence / Esse estudo aborda algumas possibilidades da relação do ator com a câmera e as categorias de presença que identifico na cena contemporânea. Neste sentido, busco demonstrar a importância da presença cênica no espaço constituído a partir do encontro entre o tele/espectador e o ator/atriz. Para tanto, a pesquisa foi desenvolvida tendo como fundamento o diálogo entre as reflexões filosóficas da fenomenologia de Martin Heidegger (conceito de Dasein) e as experiências dos atores Yoshi Oida e Ryszard Cieslak. Em um segundo momento será problematizado a forma como o ator/atriz se relaciona com a câmera e as categorias: tele/presença e co/presença, através da análise do espetáculo Somático. Investigation of the adoption of telemedicine technology in the Kuwaiti health system : strategy and policy of implementation for overseas referral patients

The cost of health care services is rising and the number of patients requesting overseas treatment from the Kuwait Government continues to increase; this is becoming an unsustainable financial burden. A telemedicine system has the potential to support the Kuwaiti health system to provide medical consultations from global medical centres, and thereby reduce the number of Kuwaiti patients being sent abroad for treatment, and so reduce costs. This research investigates the readiness of the key stakeholders in regard to adoption of telemedicine systems in the Kuwaiti healthcare system. This is achieved by assessing the readiness of individuals (physicians and patients), the organisation (policy makers) and the technical infrastructure (IT managers). Moreover, the lack of research on this topic, particularly in the Arabian Gulf Region, was the reason behind carrying out this study. An in-depth study was conducted by using a quantitative and qualitative approach. A questionnaire was used to explore the attitudes of specialised physicians and patients by obtaining their opinions based on specific criteria. Next, semi-structured interviews were conducted: (1) with IT managers to assess the technical infrastructure of the health system; and (2) with policy makers to assess the organisation in regard to its readiness towards telemedicine adoption. The study shows that the overseas referral patients are ready to use telemedicine systems, as a consequence of their need to receive medical consultations from global experts. However, they expressed some concerns, such as privacy and confidentiality of their data. The specialised physicians also showed readiness towards adoption of telemedicine in the Kuwaiti health system, and their responses indicated that they are willing to use such systems. However, the findings of the interviews reveal that the current technical infrastructure for ICT in the Kuwaiti healthcare system is not ready for telemedicine adoption and there are needs to be change in the way management is organized in order to improve and enhance trust among departments. The qualitative approach identified factors that would inhibit and hinder the adoption of telemedicine in Kuwait, as well as the supportive factors in the organisation. Using the results, a strategy was developed to demonstrate how the Kuwaiti health system could be made ready to receive a telemedicine system, together with policies for the use of telemedicine systems by overseas referral patients. A comparison with Jordan and Syria shows that the Kuwaiti healthcare system is more ready to adopt a telemedicine system with respect to the individuals, technical infrastructure and governmental support. Finally, an economic analysis was performed in order to investigate the cost effectiveness of the proposed system.

Development of a computerized assessment tool for hand-arm function after stroke- test-retest reliability and convergent validity

Many performance-based assessment tools are available to measure upper extremity function. Most define hand function by the time taken to complete a task. A new computer game-based hand-arm function evaluation (GHA) tool has been developed to quantify fine and gross object manipulations skills. Objective of this study was to evaluate the test-retest reliability and convergent validity of the GHA assessment tool. Thirty stroke clients with the mean age of 68.41 years were recruited. Test-retest reliability was assessed by Intraclass correlation coefficient (ICCs) and t-test. Convergent validity between GHA and the Wolf Motor Function Test (WMFT) was determined using the Pearson’s correlation coefficient. Most of the GHA outcome measures had moderate to high ICC (0.5-0.9). With scant exceptions, low correlations were observed between GHA measures and WMFT score. The ICC values reflected the complexity of the tasks, more complex task showed lower ICCs values. / October 2016

How should we understand the relations between human beings and technology? Where does technology end and where does the human being begin? How should we understand the relation between thinking and technology? These and related questions have been increasingly salient during the last decades. The aim of this study is to provide an outline of a philosophy of technology in the work of a major contemporary philosopher: Jacques Derrida.    The study is divided into seven chapters, each dealing thematically with a part of Derrida’s philosophy of technology. The opening chapter begins with an exposé of the way the question of technology has evolved historically. It argues that Derrida’s deconstructive thinking avoids many of the problems associated with earlier as well as contemporary approaches. The next chapter sketches the basic features of a coherent philosophy of technology, a deconstructive phenomenology of technology that enlarges the concept of technology to include the entire phenomenological field: technology is here not opposed to the psychical, it is rather a close relation between the psychical and the non-psychical, between life and death. Life is always already contaminated with technology. Chapter three explores in some detail Derrida’s thesis that contemporary life is contaminated by what he calls a “gigantic tele-technological machine” including the media, the Internet, mobile systems, digital archives, etc. As is discussed in the following chapter, this contamination also has consequences for our experience of time. More than ever before, time is today produced artificially by the tele-technological machine that transforms our temporal experience. Chapter five suggests that also the ethical dimension in Derrida’s thinking is closely linked to technology. The main part of the chapter is devoted to three technological events of historic proportions: the electronic revolution, the hypothesis of a total nuclear war, and global terrorism, all of which require an urgent ethical response beyond current politics. This ethico-political response is further elaborated in the subsequent chapter, dealing especially with Derrida’s radical thesis that the tele-technological machine will transform the concept of the political as we know it. According to this thesis, the ability of the new technologies to facilitate extremely rapid circulation of ideas, voices and images around the world will eventually erase the borders between nation states, a fact that will force us to think the political beyond politics. The final chapter on religion argues that Derrida’s provocative thoughts about the close link between religion and science, religion and technology, and religion and the media provide keys to a more comprehensive understanding of his thinking about the technical, if not of his thinking as a whole.

Analýza užitkovosti u masného skotu plemene Hereford

Theme of this work is breeding of cattle meat breed Hereford breed and their crossbreds with Charolais and Czech spotted cattle. We monitored the influence of sex, breed of father and mother, parity and month of calving on the average weight of calves at 120, 210 and 365 days of animal age. Growth of calves in individual cross combinations were evaluated too. The study was performed on the selected herd of 215 pieces of cattle: 85 of Hereford breed, 68 of Charolais breed and 62 of Czech spotted cattle breed. The herd is bred on the private farm with a total area 90ha of pastures located in the South Bohemian village Lhota near Mladosovice. We used information and performance of the herd of cattle between the years: 2017 and 2018. This diploma thesis confirms that higher average weights can be attain as crossbreeding of meat breeds and also by crossbreeds with dual purpose cattle breeds. Calves born in the late months of the year come up to higher average weights too. From the statistical analysis it is possible to prove the influence of the parent combination on the average weights of calves and their weight gains. The lowest weight gains were observed in calves of crossbred combination of Charolais vs. Czech spotted cattle (0.83 kg per day) and the highest values were detected in calves of Charolais vs. Hereford breeds (1 kg per day).

In cattle breeding, it is necessary to pay great attention to calves, especially to the quality of their housing. Calf rearing is one of the riskiest parts of the whole breed. This requires a thorough development of biological, nutritional, ethological and technological factors of breeding. The aim of this thesis was to compare two different ways of housing calves (individually vs. pairs) in roofed outdoor individual boxes with feedeng twice a day and evaluate their influence on the course of growth and health from birth to weaning. The experiment was carried out at the Číčov farm, which i sone of the centers of Alimex a.s. Nezvěstice, farming in the district of Plzeň - South. The actual monitoring of the calves took place from 1. 1. 2018 to 31. 7. 2018. Both groups of calves included in the experiment were fed the same amount of feed (native milk, milk replacer, TMR). Each group had the same number of pieces - 12 calves (heifers) of the Holstein cattle breed. At the end of the reporting period, calves that were raised together in pairs at average weight of 227.17 kg during the dairy period, while calves reared individually during the same period reached a final weight of 215 kg. The average daily increase in calves with a common rearing was 0.89 kg, in calves of the second group this value was lower, namely 0.83 kg. However, tthe difference between the mean final weights of both groups was not statistically significant (p > 0.05). All calves included tn the exxperiment were fed with appropriate quality colostrum (immunoglobulin content of 100 g/l or more) within 2 hours of delivery. Immunity of all experimental calves was sufficient, so the level of immunoglobulins did not fall below 50 g/l, with more than 85 % of calves having an immunoglobulin content above 55 g/l. In calves reared individually during the experiment, diarrhea was noted in three animals and respiratory disease in four animals. In rearing together, there was diarrhea in five calves. Respiratory disease occured in four calves. In both experimental groups of calves, it was observed that immediately after drinking, the animals were mutually sucking mules, but the calves housed together with a lower frequency also licked the polypropylene plates separating the VIB and nibbled empty buckets.

Tele-educação: Saúde auditiva em trabalhadores expostos ao ruído / Tele-education: Hearing health in workers exposed to noise

O ruído ocupacional é considerado o problema que mais atinge o sistema auditivo do trabalhador brasileiro, perturba o trabalho, o descanso, o sono e a comunicação nos seres humanos; prejudica a audição e pode causar ou provocar reações psicológicas, fisiológicas e talvez até patológicas, afetando diretamente a qualidade de vida. A perda auditiva induzida por níveis de pressão sonora elevados (PAINPSE) é considerada uma entre as dez principais etiologias de perdas auditivas populacionais, sendo que, de todas as causas de lesão auditiva, é a que apresenta maior possibilidade de prevenção. Os efeitos auditivos encontrados em trabalhadores portadores da PAINPSE imitam a funcionalidade auditiva, provocando alteração de sensibilidade auditiva, alterações na seletividade de frequências, na resolução temporal e espacial, recrutamento e zumbido. Tais alterações influenciam diretamente a discriminação auditiva, dificultando a percepção, principalmente, dos sons da fala, podendo, também, alterar o padrão de fala de acordo com o grau da perda. O investimento em implantação de programas de conservação auditiva possibilita minimizar os problemas de saúde geral e auditiva, assim como a ocorrência da PAINPSE, promovendo melhor qualidade de vida ao trabalhador. Um trabalho intensivo de promoção da saúde auditiva ou prevenção de perdas auditivas deve ser enfatizado, principalmente para trabalhadores expostos a níveis elevados de ruído ocupacional, além da utilização, de forma adequada, de equipamento de proteção auditiva individual. Sendo assim este estudo teve por objetivo elaborar um material multimídia em tele-educação sobre prevenção de perdas auditivas de origem ocupacional para ser disponibilizado pelas equipes de profissionais do Centro de referência em saúde do trabalhador (CEREST), Rede Nacional de Atenção Integral à Saúde do Trabalhador (RENAST) e agentes comunitários, em consonância com os objetivos do CEREST. A elaboração do CD-ROM contribuirá para os programas de educação em saúde auditiva fornecendo informações a respeito dos riscos causados por níveis de pressão sonora elevada (NPS). / Occupational noise is considered the issue that most affects the hearing system of the Brazilian worker, disrupts work, rest, sleep and communication in humans, impairs hearing and may cause or provoke psychological reactions, physiological and maybe even pathological, affecting directly to quality of life. These changes directly affect hearing discrimination, making the perception, especially speech sounds, it can also change the default speech according to the level of loss. Investment in implementation of hearing conservation programs enables minimizing the general health problems and hearing, as well as the occurrence of PAINPSE, promoting better quality of life to the worker. A intensive work health promotion hearing or hearing loss prevention should be emphasized, especially for workers exposed to high levels of occupational noise, and the use, as appropriate, of individual hearing protection equipment. Therefore, this study had the objective elaborate a multimedia teleducation materials on prevention of occupational hearing loss to be provided by teams of professionals from health reference center worker (CEREST), National Network of Integrated Healthcare Workers (RENAST) and community, in line with the objectives of CEREST. The development of the CD-ROM will help education programs in hearing health providing information about the risks caused by high sound pressure levels (SPL).
