Padrões alimentares, nutrientes do metabolismo do folato e homocisteína e três desfechos em saúde / Dietary patterns, nutrients involved in one-carbon metabolism and three health outcomes

Introdução - Os hábitos alimentares e os nutrientes da via metabólica do folato e homocisteína possuem grande importância na manutenção da saúde. Objetivo - Investigar a relação entre padrões alimentares (PAs) e os nutrientes envolvidos nessa via metabólica, com medidas antropométricas do recém-nascido, duração da infecção por HPV em homens e concentrações de homocisteína (Hcy) em adultos. Métodos - Foram utilizados dados dos estudos de coorte ProcriAr (Influência dos fatores nutricionais e poluentes atmosféricos urbanos na saúde pulmonar de crianças: um estudo de coorte com gestantes da zona oeste do município de São Paulo, n=299); e HIM (História natural da infecção por HPV em homens, n=1.194); e do estudo transversal ISA-Capital 2008 (Inquérito de saúde do estado de São Paulo, n=281). Os padrões alimentares foram derivados por análise fatorial por componentes principais nos estudos ProcriAr e ISA-Capital 2008 e utilizando reduced rank regression (RRR) no estudo HIM. Modelos multivariados de regressão de Poisson e lineares foram utilizados nos estudos ProcriAr e HIM para identificar a relação entre PAs e medidas antropométricas do recém-nascido e duração da infecção por HPV em homens, respectivamente. Utilizando modelo de equação estrutural, investigou-se a relação entre PAs, concentrações bioquímicas de folato, vitamina B12 e ácido docosahexaenoico (DHA) e concentrações de homocisteína em adultos do estudo ISA-Capital, considerando polimorfismo da enzima metilenotetrahidrofolato redutase (MTHFR 677C>T). Os três estudos utilizaram questionário de frequência alimentar para avaliação do consumo alimentar. Resultados - No estudo ProcriAr, a maior adesão materna ao PA \"Snacks, sanduíches, doces e refrigerantes\", rico em energia, gordura, e folato sintético, esteve diretamente associada a ter um filho pequeno ao nascer (peso e/ou comprimento ao nascer, ajustado pela idade gestacional, abaixo do percentil 10 - INTERGOWTH-21st) (RR: 2,01; IC 95%: 1.13-3.57). No estudo HIM, homens com maior adesão ao \"PA3\" tiveram, em média, um aumento de 1,15 (IC95% 0,09-2,21) à 1,18 (IC95% 0,11-2,24) meses na duração da infecção por HPV. O \"PA3\" esteve positivamente correlacionado com vitamina B6 (r = 0,59), vitamina B12 (0,27) e DFE (0,07) e negativamente correlacionado com DHA (-0,37). No estudo ISA-Capital o PA \"Prudente\" esteve inversamente associado à concentração de Hcy (β = -0,12). O DHA esteve diretamente associado ao PA \"Prudente\"; composto por verduras e legumes, peixe, frutas, frango, suco natural e batata/mandioca/polenta (cozida ou assada). Conclusões - Os PAs estão associados às medidas antropométricas do recém-nascido, à duração da infecção por HPV em homens e às concentrações de homocisteína em adultos. Estes resultados reforçam a importância de estudos sobre alimentação e nutrição que considerem não somente nutrientes, mas principalmente o consumo de alimentos e suas combinações, servindo como base para a elaboração de estratégias e políticas públicas de promoção à saúde. / Introduction - The dietary habits and nutrients involved in one-carbon metabolism are of great importance in health. Objective - To investigate the relationship between dietary patterns (DP) and the nutrients involved in this metabolism, with newborn\'s anthropometric measurements, duration of HPV infection in men, and homocysteine (Hcy) levels in adults. Methods - Data from the cohort studies ProcriAr (Influence of nutritional factors and urban air pollutants on the pulmonary health of children: a cohort study with pregnant women from the western region of the city of São Paulo, n=299); and HIM (Natural history of HPV infection in men, n=1,194); and the cross-sectional study ISA-Capital 2008 (São Paulo State Health Survey, n=281) were used. The DP were estimated using factor analysis with principal component\'s estimation in ProcriAr and ISA-Capital 2008 studies and using reduced rank regression (RRR) in HIM study. Multivariate Poisson and linear regression models were used in the ProcriAr and HIM studies to identify the relationship between DP and newborn\'s anthropometric measurements and duration of HPV infection in men, respectively. Using a structural equation model, the relationship between DP, biochemical levels of folate, vitamin B12 and docosahexaenoic acid (DHA) and homocysteine levels was investigated in adults from the ISA-Capital 2008 study, considering the polymorphism of the enzyme methylenetetrahydrofolate reductase (MTHFR 677C>T). The three studies used a food frequency questionnaire to evaluate dietary intake. Results - In the ProcriAr study, the higher maternal adherence to the \"Snacks, sandwiches, sweets and soft drinks\" DP, which is a DP rich in energy, fat, and synthetic folate, was directly associated with having a child small at birth (weight and/or birth length by gestational age and sex below the 10th percentile - INTERGOWTH-21st) (RR: 2.01, 95% CI: 1.13-3.57). In the HIM study, men with higher adherence to \"DP3\" had, on average, an increase from 1.15 (95% CI 0.09-2.21) to 1.18 (95% CI 0.11-2.24) months in the duration of HPV infection. \"DP3\" was positively correlated with vitamin B6 (r = 0.59), vitamin B12 (0.27) and DFE (0.07) and negatively correlated with DHA (-0.37). In the ISA-Capital study, the \"Prudent\" DP was inversely associated with Hcy levels (β = -0.12). DHA was directly associated with \"Prudent\" DP; composed of vegetables, fish, fruits, chicken, natural juice and potato/cassava/polenta (cooked or roasted). Conclusions - Dietary patterns are associated with newborn\'s anthropometric measurements, duration of HPV infection in men, and Hcy levels in adults. These results reinforce the importance of studies on food and nutrition that consider not only nutrients, but mainly the consumption of foods and their combinations, serving as a basis for the elaboration of public health promotion strategies and policies.

The first chapter describes valuation and other important factors relating to the Act valuation. The second chapter deals with the necessary documentation for property valuation and basic terminology. The third section deals with defects that could occur in the residential building as well as their possible correction. Another part of this work is focused on the actual valuation of the object to a defect with property damage and the last chapter deals with the actual real estate market and the subsequent sale of such property with the defect. The attachments of the case study is then focused on the valuation of the house and without defects and possible solutions and proposed procedures for sale.

These phases are divided into agricultural land, other land-use plan that is designed for building and land intended to be built according to zoning. Final evaluation determines how different land prices in the three phases of spatial planning and price comparison of the two plots each other in different locations.

The practical part focuses on the description and awards selected six houses using a cost comparison method according to the decree and comparative non-regulation method. It also includes the valuation of land in a single functional unit. Acquired prices are converted to unit prices and evaluated.

The diploma thesis deals with the analysis of a selected segment of the real estate market in a specific border locality (CZ-DE-PL). Furthermore, the influences and development that affects the real estate market with a focus on housing construction in the context of regional development. Based on the collected data about available properties and locations, databases are created where properties are divided by type, layout and size. From these databases, the average market prices of real estate are then calculated, that are then compared within the border regions. The prices reflect all the positive and negative properties of buildings and localities that may have an impact on the price of real estates in the surveyed localities. The last part compares the regional development of housing units in border regions.

EXAMINING DIETARY INTAKE, FOOD SECURITY AND HEALTH AMONG THE POPULATION WITH LOW INCOMES

<p>  </p> <p>Food insecurity describes the lack of access to foods and affects 10.2% of general U.S. households and 27% of low-income households in 2021. Food insecurity is a pervasive public health concern in the United States and has been linked to poor dietary intake and diet quality, overweight and obesity (especially among women), and risk of other chronic diseases, such as diabetes, hypertension and dyslipidemia. </p> <p>To better understand food security status and address its associated health and dietary outcomes among low-income populations, a conceptualized model was built and served as research framework for the dissertation, including 1) internal factors and motivations, such as traits related to self-efficacy and sufficiency that may influence diet and health; 2) external factors of temporary support, such as financial benefits from assistance programs that low-income populations are eligible for that may influence diet and health; and 3) external factors of potentially long-term support, such as nutrition education programs targeting low-income groups that may foster internalized knowledge that could sustain impact and improvement of diet and health in the long-term. Each chapter of this dissertation addresses a component of the model.</p> <p>Cross-sectional analysis of a sample of rural veterans using food pantries quantified psychological traits related to self-motivation and efficacy including grit and help seeking, at the individual and internal factors level of the conceptualized model, and their links to food security and resource use, and revealed an inverse association between grit score and risk of food insecurity. The findings provided evidence for future interventions targeting food insecurity improvement to include education and resources that address traits related to self-efficacy, such as grit, among low-income populations to improve health outcomes directly or through improving food security or use of resources. </p> <p>Using nationally representative data, the second study investigated relationships between food assistance through the Supplemental Nutrition Assistance Program (SNAP) participation, a type of societal level external support, and dietary outcomes among low-income older U.S. adults. There were no differences in dietary quality, usual nutrient intake or risk of inadequacy between SNAP participants and eligible nonparticipants. Furthermore, results revealed a high prevalence of not meeting the Estimated Average Requirement from dietary sources for several nutrients (vitamins A, C, D, E, calcium, and magnesium) but the prevalence was lower when nutrients from dietary supplements were included. The results highlight a need for continued effort to improve nutrient and dietary intake among low-income older adults.</p> <p>External factors of potentially long-term support (e.g. nutrition education and food assistance) were evaluated for relationships with body mass index. A longitudinal sample of low-income women interested in participating in nutrition education through SNAP-Education (SNAP-Ed) was examined to determine the relationship between nutrition education (SNAP-Ed) and food assistance program participation through (SNAP, WIC), separately and in combination, with long-term changes in body mass index. No differences in changes of weight status over time were observed by nutrition education, food assistance, or combination participation. The prevalence of obesity was high among this sample, calling for targeted obesity prevention interventions and further support of healthy lifestyle promotion among low-income populations. </p> <p>The findings shown in this dissertation further reveal a high health burden among low-income groups. The studies filled several research gaps described in the conceptualized model. The results may be used to inform future tailored interventions to address food insecurity, dietary and health outcomes at individual and societal levels, incorporating internal motivation and external support to mediate health and dietary risks among low-income population.</p>

Strafzumessungsrelevanz der Vorsatzformen

Fragen: Es gibt nach h.M. der deutschen Strafrechtswissenschaft drei Vorsatzformen: Absicht, sicheres Wissen und dolus eventualis. Soll unterschiedliche Vorsatzform, ceteris paribus, auch bei der Strafzumessung unterschiedlich bewertet werden? Warum? Methode: Diese Fragen werden in einem interdisziplinären Dialog zwischen Norm und Empirie untersucht. Neben der strafrechtlichen und verfassungsrechtlichen Ausführung bezieht sich diese Arbeit großenteils auf die Erkenntnisse aus den Bereichen Kriminalpolitik, Kriminologie, Psychologie, Verhaltensökonomie und Neurowissenschaft. Zur Auswertung der Erkenntnisse wird großer Wert auch auf Forschungsmethode gelegt. Ergebnisse: Zur ersten Frage: Die Schuldangemessenheit gebietet, unterschiedliche Vorsatzformen bei der Strafzumessung unterschiedlich zu bewerten, wenn sie quantitative oder qualitative beachtliche Unterschiede aufweisen. Zur zweiten Frage: Der in Kapitel 6 erarbeitete Maßstab, Vorsatzformen nach dem Risiko für die Tatbestandsverwirklichung abzustufen, erweist sich als am aussichtsvollsten, sich auf solide Grundlagen der Norm und Empirie zu stützen. Möglichkeiten für Vorsatzabstufung nach diesem Maßstab werden erörtert. Eine klare Antwort bedarf aber einer Anpassung der Vorsatzformen an empirisches Wissen, weil die Studien aus anderen Bereichen den Nuancen von rechtlichen Begriffen nicht genug Rechnung tragen. Ferner wäre eine Vorsatzabstufung nach dem Bedarf einer Straftäterbehandlung, wie in Kapitel 4 gezeigt, erfolgversprechend, sollte die Beziehung zwischen den Vorsatzformen und dem Rückfallrisiko festgestellt werden. Ausblick: Der Verfasser plädiert für eine normativ sowie empirisch fundierte Strafrechtsdogmatik. Das heißt, Strafrechtler sollen empirische Beweise vorbringen, nötigenfalls durch eigene Studien, soweit ihre Argumente empirische Thesen enthalten. Zum Schluss verweist der Verfasser auf die Erforderlichkeit, das Potenzial und die Grenzen dieses Ansatzes und die Herausforderungen auf diesem Weg. / Question: According to the prevailing view of German legal doctrines, there are three severe types of mens rea, collectively referred to as "Vorsatz," i.e., intent, knowledge, and dolus eventualis. This work examined two questions: Should criminal acts committed with different "Vorsatz" types, all other things being equal, be punished differently? Why? Method: This work answered these questions in an interdisciplinary discourse between jurisprudence and empirical research, based chiefly on knowledge from criminal and constitutional law jurisprudence, penal policy, criminology, psychology, behavioral economics, and empirical research methods. Conclusion: To the first question: The proportionality between the penalty and criminal act demands that criminal acts committed with different "Vorsatz" types shall be differently punished if they exhibit considerable qualitative or quantitative differences. To the second question: The state of research, as discussed in Chapter 6, indicated that a gradation of "Vorsatz" types according to the risk they pose for crime completion is the most promising guideline that could enjoy both normative and empirical justifications. Possibilities of "Vorsatz" gradation are suggested. However, revisions of the definition of "Vorsatz" types are prerequisite for all these possibilities, because the studies in other disciplines did not take sufficiently into consideration the nuances of legal notions. On the other hand, a gradation of "Vorsatz" types according to the requirements for successful offender rehabilitation, as shown in Chapter 4, would also be promising if the relation between "Vorsatz" types and recidivism risk could be proven. Prospects: The author called for a criminal legal dogmatics on both normative and empirical bases. It means that criminal law scholars should provide evidence for arguments comprising empirical hypotheses. The necessity, potential, and limitations of this approach and challenge toward it are discussed.

Health for community dwelling older people : trends, inequalities, needs and care in rural Vietnam

Background InVietnam, the proportion of people aged 60 and above has increased rapidly in recent decades. The majority live in rural areas where socioeconomic status is more disadvantaged than in urban areas.Vietnam’s economic status is improving but disparities in income and living conditions are widening between groups and regions. A consistent and emerging danger of communicable diseases and an increase of non-communicable diseases exist concurrently. The emigration of young people and the impact of other socioeconomic changes leave more elderly on their own and with less family support. Introduction of user fees and development of a private sector improve the coverage and quality of health care but increase household health expenditures and inequalities in health care. Life expectancy at birth has increased, but not much is known about changes during old age. There is a lack of evidence, particularly in rural settings, about health-related quality of life (HRQoL) among older people within the context of socioeconomic changes and health-sector reform. Knowledge of long-term elderly care needs in the community and the relevant models are still limited. To provide evidence for developing new policies and models of care, this thesis aimed to assess general health status, health care needs, and perspectives on future health care options for community-dwelling older people. Methods An abridged life table was used to estimate cohort life expectancies at old age from longitudinal data collected by FilaBavi DSS during 1999-2006. This covered 7,668 people aged 60 and above with 43,272 person-years. A 2007 cross-sectional survey was conducted among people aged 60 and over living in 2,240 households that were randomly selected from the FilaBavi DSS. Interviews used a structured questionnaire to assess HRQoL, daily care needs, and willingness to use and to pay for models of care. Participant and household socioeconomic characteristics were extracted from the 2007 DSS re-census. Differences in life expectancy are examined by socioeconomic factors. The EQ-5D index is calculated based on the time trade-off tariff. Distributions of study subjects by study variables are described with 95% confidence intervals. Multivariate analyses are performed to identify socioeconomic determinants of HRQoL, need of support, ADL index, and willingness to use and pay for models of care. In addition, four focus group discussions with the elderly, their household members, and community association representatives were conducted to explore perspectives on the use of services by applying content analysis. Results Life expectancy at age 60 increased by approximately one year from 1999-2002 to 2003-2006, but tended to decrease in the most vulnerable groups. There is a wide gap in life expectancy by poverty status and living arrangement. The sex gap in life expectancy is consistent across all socioeconomic groups and is wider among the more disadvantaged populations.  The EQ-5D index at old age is 0.876. Younger age groups, position as household head, working, literacy, and belonging to better wealth quintiles are determinants of higher HRQoL. Ageing has a primary influence on HRQoL that is mainly due to reduction in physical (rather than mental) functions. Being a household head and working at old age are advantageous for attaining better HRQoL in physical rather than psychological terms. Economic conditions affect HRQoL through sensory rather than physical functions. Long-term living conditions are more likely to affect HRQoL than short-term economic conditions. Dependence in instrumental or intellectual activities of daily living (ADLs) is more common than in basic ADLs. People who need complete help are fewer than those who need some help in almost all ADLs. Over two-fifths of people who needed help received enough support in all ADL dimensions. Children and grand-children are confirmed to be the main caregivers. Presence of chronic illness, age groups, sex, educational level, marital status, household membership, working status, household size, living arrangement, residential area, household wealth, and poverty status are determinants of the need for care. Use of mobile teams is the most requested service; the fewest respondents intend to use a nursing centre. Households expect to use services for their elderly to a greater extent than did the elderly themselves. Willingness to use services decreases when potential fees increase. The proportion of respondents who require free services is 2 to 3 times higher than those willing to pay full cost. Households are willing to pay more for day care and nursing centres than are the elderly. The elderly are more willing to pay for mobile teams than are their households. ADL index, age group, sex, literacy, marital status, living arrangement, head of household status, living area, working status, poverty and household wealth are factors related to willingness to use services.   Conclusions                                                                                         There is a trend of increasing life expectancy at older ages in ruralVietnam. Inequalities in life expectancy exist between socioeconomic groups. HRQoL at old age is at a high level, but varies substantially according to socioeconomic factors. An unmet need of daily care for older people remains. Family is the main source of support for care. Need for care is in more demand among disadvantaged groups.  Development of a social network for community-based long-term elderly care is needed. The network should focus on instrumental and intellectual ADLs rather than basic ADLs. Home-based care is more essential than institutionalized care. Community-based elderly care will be used and partly paid for if it is provided by the government or associations. The determinants of elderly health and care needs should be addressed by appropriate social and health policies with greater targeting of the poorest and most disadvantaged groups.
