Physical health outcomes for young people commenced on clozapine

Abstract BACKGROUND: Psychotic disorders are associated with symptoms such as delusions and hallucinations as well as impaired functioning. Second generation antipsychotic medications are the first line treatment for psychotic disorders, however a proportion of individuals will not respond to the medication. Clozapine is the most effective second generation anti-psychotics, however it is only used as a third line treatment because of serious side-effects. It is associated with metabolic side-effects, which increase the risk of obesity and diabetes mellitus. AIM: Determine the changes in the weight, waist circumference and blood pressure as well as triglycerides, fasting glucose and cholesterol in young people commenced on clozapine after 18 weeks and 6 months of commencement. METHOD: This is a cohort study, including participants aged 15-24 commenced on clozapine between the time period 1st of April 2016 and 30th of September 2017. Data concerning measured outcomes was gathered from patient journals. RESULTS: A total of 36 young people were commenced on clozapine during the study period and the mean age was 19.8 (SD±3.1). At the time of commencing clozapine, the mean weight of the cohort was 86.30kg (±17.08) and after 18 weeks it was 88.90kg (±16.71), and this difference was statistically significant (n=27,df=26,p=0.02) and this weight gain was present for males (n=17,df=16,p=0.038) but not females (n=10,df=9,p=0.214). CONCLUSION: Clozapine may be associated with weight gain in the early stages of commencement and it appears that males are more susceptible to this side-effect. More interventions aimed at attenuating this weight gain is needed.

Perspectives of young sexual abuse survivors, whose abuse is unknown to child protection services, about confidentiality : a participatory study involving young abuse survivors as co-researchers

A systematic narrative literature review explored views of young child sexual abuse (CSA) survivors whose abuse was unknown to authorities, about confidentiality. Due to paucity of research, the review included literature about sexual health service users. Seventeen databases and eleven journals were searched and thirty-three papers were identified. Analysis involved exploratory interpretist approach focusing on emergent themes and methodologies. Findings indicated limited participation and that young people want; (i) confidentiality (ii) control of their lives and (iii) they fear child protection agencies. Empirical research then explored views about confidentiality with young CSA survivors whose abuse was unknown to child protection agencies. Eight survivors engaged as co-researchers in participatory action research utilising a mixed methods approach; using surveys, focus groups, online chats, graffiti walls and interviews. One hundred and forty survivors participated. Age range was 11- 30, with 25 males, 114 females and 1 gender neutral. Researchers designed tools, gathered and analysed qualitative and quantitative data using a social construction thematic approach. Results found (i) all participants wanted higher confidentiality, with males and under 16's demanding highest confidentiality (ii) males were reluctant to disclose CSA compared to females (iii) abuse retractions were related to loss of confidentiality (iv) involvement as co-researchers was positive and empowering. With limited confidentiality the identified themes for young people were: fear of consequences, lost control, abuse retractions and protecting abusers. With high confidentiality themes were: talking openly, control, empowerment, exploring options, respect and being believed. Young researchers reported improvement in self-esteem and positive life changes. Conclusions: Young CSA survivors whose abuse was unknown to child protection services want higher confidentiality to build trust and be able to talk about abuse; and involvement of young people in participatory research is positive and empowering. Recommendations: There appears to be a need for increased confidentiality if young people whose abuse is unknown to child protection services are to disclose their abuse. Further participatory research is needed to explore whether other groups of young people unknown to services report similar and/or different needs.

Design, implementation, and evaluation of school-based sexual health education interventions in sub-Saharan Africa

School-based sexual health education is commonly used to promote the sexual health of young people and guide them in their relationships. This thesis reports on research that aimed to provide evidence-based recommendations to optimise the effectiveness of school-based sexual health education in sub-Saharan Africa (sSA). There are six chapters in the thesis. Chapter 1 introduces the thesis, Chapters 2 to 5 consist of four empirical studies, and Chapter 6 provides an overall discussion and looks at the strengths, limitations, and implications of the findings. Chapter 2 is a systematic review and meta-analysis of school-based sexual health education in sSA. It provides some evidence of the interventions in promoting self-reported condom use. However, it shows there are no harmful or beneficial effects with respect to sexually transmitted infections (STI) as evidenced by biomarkers. It highlights the paucity of evaluated interventions using biomedical markers, and reports on the process of evaluation, which limits our understanding of why interventions work or do not work. Features associated with effective interventions are noted. Chapter 3 is a case study involving MEMA Kwa Vijana, an adolescent sexual and reproductive health intervention implemented in Tanzania. This study highlights the influence of structural factors in schools and wider environmental factors on the effectiveness of school-based sexual health interventions. Furthermore, it identifies the social and cultural factors that influence young people’s sexual behaviours and that must be addressed beyond the education and health sectors. Chapter 4 is a multiple case study of seven school-based sexual health interventions implemented in five sub-Saharan African countries. It 4 identifies the design, implementation, and evaluation features that differentiate between effective and ineffective interventions. Chapter 5 is a qualitative study of researchers’ experiences of school-based sexual health education in sSA. This study extends previous work by generating a set of valuable recommendations based on researchers’ experiences of interventions that could improve future interventions in sSA. Overall, this research project demonstrates the potential of school-based sexual health education in promoting sexual health and preventing STIs in sSA. It provides a series of recommendations for the design, implementation, and evaluation of school-based sexual health interventions.

This study had as general objective the knowledge whether Campus USP Leste of University of São Paulo had provided access to college education to young people in the region. Our specific goal was to understand the relation among family cultural capital, school trajectories and experience of these young people in the frames of that University. To achieve our goals, we used quantitative methodology based on questionnaires and qualitative methodology based on in-depth interview. 183 questionnaires were answered and 20 in-depth interviews were executed. For our research, we relied on cultural reproduction theory, represented by the work of Pierre Bourdieu and Jean-Claude Passeron, specifically on the concept of "cultural capital" and on empirical researches conducted in the brazilian society context about the problematization of lower classes schooling (VIANA, 1998, PIOTTO, 2007 and LACERDA, 2014). The choice of University of São Paulo was due to it being one of the renowned public institution, internationally recognized, that, on the last decade, went through a expansion process that culminated in the implementation of Campus USP Leste, whose region is one of the most densely populated in the capital. We chose to study this topic in a specific region of the city due to organization and range of proposals of the Education Movement from East Zone, that, since 1970, struggle for access of population to education at all levels throught social movements / Esta dissertação se constitui de um estudo sobre a trajetória escolar e a experiência universitária de jovens oriundos da Zona Leste da capital paulista na Universidade de São Paulo, Campus USP Leste. Esse estudo teve como objetivo geral saber se o Campus USP Leste da Universidade de São Paulo propiciava o acesso à educação superior aos jovens da região. O nosso objetivo específico foi compreender a relação entre capital cultural da família e a trajetória escolar e experiência desses jovens nos quadros daquela Universidade. Para realização de nossos objetivos foi utilizada metodologia quantitativa baseada na aplicação de questionário e metodologia qualitativa baseada em entrevista em profundidade. Foram respondidos 183 questionários e realizadas 20 entrevistas em profundidade. Para construção de nosso objeto de pesquisa, nos apoiamos na teoria da reprodução cultural, representada pela obra de Pierre Bourdieu e Jean Claude Passeron, mais especificamente no conceito de “capital cultural” e em pesquisas empíricas realizadas no contexto da sociedade brasileira sobre a problematização da escolarização de camadas populares (VIANA, 1998; PIOTTO, 2007; LACERDA, 2014). A escolha da Universidade de São Paulo se fez por ser essa uma das instituições públicas de renome, reconhecida internacionalmente, que na última década passou por um processo de expansão que culminou na implantação do Campus USP Leste, cuja região é uma das mais populosas da capital. Situação de estudo e trabalho e escolaridade da população juvenil, na região metropolitana de Porto Alegre, no período 1993-2014

Esta dissertação tem como objetivo analisar a evolução de indicadores de escolaridade pré-selecionados (frequência escolar, média de anos de estudo, níveis de escolaridade, concluída ou frequentada, e a variável situação de estudo e trabalho) dos jovens na faixa etária de 15 a 29 anos, na Região Metropolitana de Porto Alegre, entre 1993 e 2014, através das políticas de ampliação de acesso, seja na universalização do Ensino Fundamental, seja na ampliação significativa do acesso ao Ensino Médio, seja na inserção no Ensino Superior, por meio de programas como o Programa Universidade Para Todos, o Fundo de Financiamento Estudantil e o Reestruturação e Expansão das Universidades Federais e a Lei de Cotas (Lei nº 12.711/2012). A fonte de informações utilizada é a Base de Microdados da Pesquisa de Emprego e Desemprego na Região Metropolitana de Porto Alegre (PED-RMPA). Os resultados obtidos apontam significativa ampliação do acesso à escola, elevação da escolaridade e redução das desigualdades educacionais para os jovens com idade entre 15 e 29 anos. Entretanto o alcance das políticas de acesso ainda está muito aquém de acabar com os diferenciais de acesso e escolaridade de jovens mulheres e jovens homens e pouco contribuiu com a igualdade entre jovens negros e jovens não negros. Duas variáveis foram de fundamental importância para a realização desta pesquisa: situação de estudo e trabalho e grupos de renda familiar per capita. A variável situação de estudo e trabalho compreende as categorias jovens que só estudam, jovens que estudam e trabalham, jovens que estudam e procuram trabalho, jovens que só trabalham, jovens que só procuram trabalho e jovens que não estudam e/ou não trabalham e/ou não procuram trabalho. A variável grupos de renda familiar per capita foi dividida em quatro categorias: Grupo 1 - 25% das pessoas com menor renda familiar per capita; Grupo 2 - 25% das pessoas com renda familiar per capita imediatamente superior à do Grupo 1; Grupo 3 - 25% das pessoas com renda familiar per capita imediatamente superior à do Grupo 2; Grupo 4 - 25% das pessoas com maior renda familiar per capita. No aspecto renda familiar per capita, o impacto das políticas em todos os estratos de renda foi importante, em especial para as Categorias I (jovens que só estudam) e II (jovens que estudam e trabalham e/ou procuram trabalho). Deve--se considerar que ainda há um enorme desafio para um acesso pleno à educação, com a permanência dos jovens na escola, bem como com a conclusão dos estudos, especialmente para os jovens homens, os jovens negros e para os jovens pertencentes ao grupo de menor 7 rendimento familiar per capita (Grupo 1), onde são verificados avanços, mas persistem as desigualdades de acesso e permanência escolar. / The aim of this thesis is to analyze the evolution of preselected school indicators (school attendance, average years of schooling, completed or attended school levels, and the variable study and work context) of young people in the 15- 29 age group, in the Porto Alegre metropolitan area, between 1993 and 2014, through the expansion policies access, either in the universalization of Primary Education, or in the significant expansion of access to Secondary Education, or in the insertion in higher education, by means of programs such as the University for All Program, the Student Financing Fund and the Restructuring and Expansion of Federal Universities, and the Quotas Law (Law no. 12.711 / 2012). The source of information used is the micro database of Employment and Unemployment Research of Porto Alegre Metropolitan Area (PED-RMPA). The results obtained indicate a significant increase in access to schools, higher schooling and reduction of educational inequalities among youths aged 15 -29. However, the extent of access policies still fall short of ending up with the differentials in access and schooling among young men and women and has contributed little to the equality of young black and non-black youths. Two variables were of fundamental importance for this research: study and work context and per capita family income groups. The variable study and work context includes the categories of young people who only study, young people who study and work, young people who study and seek work, young people who only work, young people who are looking for work and young people who do not study and / or do not seek work. The variable per capita family income groups was divided into four categories: Group 1 - 25% of people with lower per capita family income; Group 2 - 25% of people with per capita family income immediately higher than Group 1; Group 3 - 25% of people with per capita family income immediately higher than Group 2; Group 4 - 25% of people with higher per capita family income. In the per capita family income aspect, the impact of policies on all income strata was important, especially for Categories I (young people who only study) and II (young people who study and work and / or seek work).One must consider that there is still a huge challenge for full access to education, the permanence of young people in school and the completion of studies, especially for young men, young blacks and young people belonging to the group with a lower per capita family income (Group 1), where progress is made, but access inequalities and school permanence persist.

A pesquisa realizada englobou a avaliação de características dos jovens Nativos Digitais, nascidos após o ano 2000, desenvolvendo-se sobre os aspectos pessoais, comportamentais, valorização e sobre como observam as instituições financeiras. Utilizou-se de diferentes fontes de pesquisas com jovens do Brasil e de diferentes partes do mundo. Da mesma forma, a pesquisa observou uma instituição financeira cooperativa, o Sicredi, buscando avaliar como os jovens nascidos a partir do ano 2000 se relacionam atualmente com essa empresa. Além disso, a pesquisa procurou identificar, através de entrevistas com diferentes executivos da empresa, como o Sicredi está se preparando para se relacionar com essa nova geração que começa a adentrar no mercado financeiro, quais os desafios que terá o trabalho aborda a constituição de uma estratégia para as instituições financeiras cooperativas de aproximação e relacionamento com a geração de nativos digitais, jovens nascidos a partir do ano 2000, com ênfase no Sicredi – Sistema de Crédito Cooperativo para se aproximar dessa geração e como a tecnologia pode ser uma aliada nesse processo. Os resultados da pesquisa identificaram a conexão do modelo cooperativista com os anseios dos jovens e ao mesmo tempo a ausência de uma estratégia do cooperativismo, e do Sicredi, para atingir esses jovens. Esse é um trabalho que busca apresentar uma visão de futuro de médio e longo prazo, uma vez que a geração de Nativos Digitais ainda é uma geração em ascensão no mercado financeiro. / This work deals with a construction of a strategy for cooperative financial institutions to approach relationship with the generation of digital natives, young people born after 2000, with the emphasis on Sicredi (Cooperative Credit System). In this applied research, the evaluation of characteristics of young people covered the following topics: personal development with behavioral aspects, valuation and theirs vision about the financial institutions. To make this survey, several sources of research with young people from Brazil and other countries were used. Likewise, this research observed the Sicredi, seeking to evaluate how young people born from the 2000 year are currently related themselves to this company. In addition, the research sought to identify, through interviews with different Sicredi executives, how the company is preparing to relate to this new generation that begins to enter the financial market, what challenges Sicredi will have to overcome to get closer them and how technology can be an ally in this process. The results of this research identified the connection between the cooperative model and the young people's wishes and at the same time the absence of a cooperative strategy of Sicredi to reach these young people. This is a work that seeks to present a medium-term and long-term vision of the future, since the generation of Digital Natives is still a generation that rises in the financial market.

Représentations sociales de l'espace public chez des jeunes issus de milieux d'exclusion à Santiago du Chili / Social representations of public space in young people from poor neighbourhoods in relation to their participation (or lack of participation) in associative groups

Cette recherche, consacrée aux quartiers difficiles de la ville de Santiago du Chili, a pour objet d’étudier la vision que des jeunes habitants élaborent de leur espace public. L’étude a nécessité une adaptation psychosociale du concept d’espace public et l’élaboration d’un dispositif de recherche permettant de l’appréhender empiriquement. La recherche avait pour but d’étudier le rôle de la participation, ou de la non participation, aux associations de quartier (culturelles, éducatives, sportives, musicales, religieuses) sur la manière d’envisager l’espace public et se représenter autrui et les relations entre les habitants. Cinq quartiers déshérités de la ville de Santiago du Chili, distincts quant à leur histoire, ont constitué notre terrain d’étude et 45 jeunes habitants de ces quartiers ont été interviewés. La méthodologie combine l’approche qualitative et quantitative. Le corpus des entretiens a été analysé avec le logiciel Alceste. La participation aux associations s’avère être une variable importante dans la manière d’envisager la vie des quartiers et le rôle de différentes institutions qui y interviennent. Les représentations se distinguent selon le type des associations auxquelles ces jeunes prennent part. Cette recherche fournit des éléments pour réfléchir sur la pertinence des interventions en matière de sécurité publique, dont ces quartiers font l’objet de la part des autorités et les potentialités de ces jeunes habitants à modeler les relations quotidiennes et envisager leur avenir. Loin de se limiter à la réalité chilienne, ces questions concernent les phénomènes urbains dans une perspective internationale et constituent des enjeux dans les sociétés contemporaines. / The main purpose of this research, dedicated to the poor areas of the city of Santiago, Chile, is to understand the representation that young people develop of their public space. A psychosocial adaptation of the concept of public space and the construction of an empirical research device were needed. The approach is based on the study of concrete aspects of living in neighbourhoods with a special focus on identity and relational dimensions of the subject.Five poor neighbourhoods of Santiago city, distinct in their history, were chosen as fields of study and 45 young people living in these districts were interviewed. The main independent variable in this analysis was to distinguish whether young people were members of a neighbourhood association (and the kind of association: cultural, educational, sporting, musical, or religious) or whether they did not take part in any association.The methodology combines both qualitative and quantitative approaches. Semi-structured interviews have two parts: an oral description of the neighbourhoods, activities and social relationships with the illustration of remarks through the drawing of a socio-spatial map. In the second part, three scenarios evoking the difficulties of daily life were presented and the young people were asked to think of solutions to these difficulties. The corpus of interviews was analysed using the software Alceste. We were able to study their representations of others, social exchanges and various institutions involved in the life of the districts.The results have shown significant differences in the representations that these young people develop of neighbourhood life, their ability to have an impact on it and their perception of the future, according to the association activities they participate in.This research provides elements to reflect on the relevance of interventions in public safety, the potential of these young people to shape their everyday relationships and the representation of their living places oriented towards the future. Far from being limited to the Chilean reality, these issues concern all the major cities of the world and are issues specific to contemporary society.

Cette thèse porte sur un phénomène actuel d'intérêt social et sociologique, influencé par la crise économique et financière mondiale, qui a débuté en 2008. La recherche vise à identifier les stratégies de réalisation du projet migratoire temporaire construit sur la base de critères de carrière professionnelle (passée, présente et future) de la population espagnole récemment installée à Toulouse (France). En résumé, les stratégies d'organisation du processus de mobilité sont le produit d'une succession de situations d'interaction, qui contribuent à construire et à activer une série d'informations utiles qui permettent d'accéder à des ressources concrètes pour trouver un emploi et résider dans un nouveau contexte.Le processus de mobilité explique la diversité des formes d'insertion sociale, l'accès à l'âge adulte et la stabilisation relationnelle de la population. Ces stratégies relationnelles transnationales sont en constante construction et transformation, en tant que processus impliqué dans les interactions et les milieux sociaux auxquels les personnes participent, sans oublier le contexte structurel rencontré par ces personnes. Ainsi, les liens interpersonnels transnationaux nous permettent de discerner l'émergence d'espaces collectifs de partage et de groupes sociaux qui expliquent les structures de sociabilité et de soutien social. La présence d'individus dans des groupes transnationaux forme leurs réseaux personnels et affecte la stabilisation relationnelle de ces personnes dans leurs lieux de résidence.Cet ensemble d'influences réciproques entre réseaux personnels plus ou moins stabilisés, les relations identifiées dans le stade initial de la migration et l'interaction entre les relations personnelles qui génèrent des collectifs de sociabilité transnationale, contribuent à l'explication des phénomènes d'expatriation et de migration de travail abordés dans cette thèse. / This thesis deals with a current phenomenon of social and sociological interest, influenced by the global economic and financial crisis, which began in 2008. The research aims to identify strategies for the implementation of the temporary migration project based on career criteria (past, present and future) of the Spanish population recently settled in Toulouse (France). In summary, strategies to organize the mobility process are the product of a succession of transnational interaction situations which help to construct and activate useful information that provides access to concrete resources to find employment and reside in a new context.The mobility process explains the diversity of forms of social integration, access to adulthood and the relational stabilization of the population. These transnational relational strategies are constantly being constructed and transformed, as a process involved in the interactions and social environments in which people participate, not to mention the structural context encountered by these people. Thus, transnational interpersonal links allow us to discern the emergence of collective spaces for sharing and social groups that explain the structures of sociability and social support. The presence of individuals in transnational groups leads to the formation of personal networks and affects the relational stabilization in their places of residence.This set of reciprocal influences between more or less stabilized personal networks, the relationships identified in the initial stages of migration and the interaction between personal relationships that generate transnational sociability collectives contribute to the explanation of expatriation phenomena and work migration covered in this thesis. / Esta tesis se centra en un fenómeno actual de interés social y sociológico, influenciado por la crisis económica y financiera, que comenzó en 2008. La investigación tiene como objetivo identificar las estrategias de realización de proyectos de migración temporal construidos sobre la base de criterios de carrera profesional (pasada, presente y futura) de la población española instalada recientemente en Toulouse (Francia). En resumen, las estrategias de organización del proceso de movilidad son el producto de una sucesión de situaciones de interacción que contribuyen a construir y activar una serie de informaciones útiles que permitan el acceso a recursos específicos para conseguir un empleo y establecerse en un nuevo contexto de residencia.El proceso de movilidad explica la diversidad de formas de inclusión social, el acceso a la edad adulta y la estabilización relacional de la población. Estas estrategias relacionales transnacionales están en constante construcción y transformación como un proceso involucrado en las interacciones sociales y los entornos en los que participan las personas, así como el contexto estructural al que se enfrentan estas personas. Por lo tanto, las relaciones interpersonales transnacionales nos permiten discernir la aparición de espacios colectivos de intercambio y de grupos sociales que explican las estructuras de sociabilidad y de apoyo social. La presencia de individuos en grupos transnacionales da forma a sus redes personales y afecta a la estabilización relacional de estas personas en sus lugares de residencia.Este conjunto de influencias recíprocas entre redes personales más o menos estabilizadas, relaciones identificadas en la etapa inicial de la migración y la interacción entre las relaciones personales que generan colectivos de sociabilidad transnacional, contribuyen a la explicación de los fenómenos de expatriación y a la migración laboral discutidos en esta tesis.

Evasão escolar nos cursos técnicos do PROEJA : um estudo de caso no IFSULDEMINAS - Campus Passos /

Orientador: Maria Madalena Gracioli / Resumo: Esta pesquisa focaliza o Programa Nacional de Integração da Educação Profissional com a Educação Básica na Modalidade de Educação de Jovens e Adultos (PROEJA). Instituído pelo governo federal no ano de 2005, o programa objetiva elevar a escolaridade e, ao mesmo tempo, oferecer profissionalização a jovens e adultos, preferencialmente trabalhadores, que não tiveram a oportunidade ou não puderam concluir alguma etapa da escolarização na idade recomendada por lei. Em sua materialização, um dos desafios observados são os elevados índices de evasão escolar. No Instituto Federal de Educação, Ciência e Tecnologia do Sul de Minas Gerais (IFSULDEMINAS), até o presente momento, cinco campi ofertam ou já ofertaram cursos do PROEJA, e todos apresentam índices de evasão próximos ou superiores a 50%. O IFSULDEMINAS – Campus Passos, em específico, ofereceu o primeiro curso técnico do PROEJA (Técnico em Orientação Comunitária) no ano de 2015. Ingressaram na turma 26 estudantes dos quais apenas três o concluíram; nesse caso, a taxa de evasão chegou a 88,46%. Assim, o objetivo desta pesquisa, que se configura como um estudo de caso, foi identificar os motivos que levaram jovens e adultos a evadirem do curso técnico do PROEJA no IFSULDEMINAS – Campus Passos. Para atingir esse intento, realizou-se uma revisão bibliográfica sobre as políticas de educação profissional, educação de jovens e adultos, evasão escolar e o PROEJA. Established by the federal government in 2005, the program aims to increase schooling and, at the same time, offer professionalism to young people and adults, mostly workers, who did not have the opportunity or could not complete some schooling stage at the age recommended by law. In its materialization, the school's high rates dropout is one of the challenges observed. At the Education Federal Institute, Science and Technology from the South of Minas Gerais (IFSULDEMINAS) until the present moment, five campuses offer or have already offered the "PROEJA" technical courses, and all have dropout rates close to or greater than 50%.
