Aspects and stories of helpful therapy and outcomes : a narrative enquiry

This research is concerned with the question of what constitutes a good therapeutic outcome and which therapeutic practices are most likely to bring this about. It comprises, first, a narrative literature review which examines recent claims of 'positive psychologists' to have devised interventions that are more effective at directly increasing human 'happiness' and well-being than those that aim to reduce distress. This review concludes that such claims are not backed up by clear evidence, that they lack conceptual clarity and that it is therefore not clear that the field of positive psychology has added anything new to clinical practice. The qualitative analysis of the narratives of former therapy clients who have assessed their therapy as helpful forms the basis for the main research paper, which explores clients' stories of what constitutes helpful therapy. The paper constructs and discusses a single shared plot line that is common to all participants and concludes that helpful therapy is that which helps clients to construct their own sense of understanding and 'story' about their difficulties as well as to develop some strategies for dealing with their effects. The critical review takes a reflective stance to considering how far the research study has represented the range of clients' views on the therapeutic practices that they find useful. It considers one particular study participant whose narrative suggests that the shared story derived from the research paper may represent only one of a range of possible different conceptions of 'helpful therapy', that which occurs when clients use therapy as a discrete and focused episode in their life rather than an ongoing source of support and development. The paper concludes that future research could benefit from exploring with service users different conceptions of the role of therapy that might lead to different 'stories'.

Cultural factors in the provision of psychological support

Section one of this thesis considers the impact of culture on the therapeutic relationship. A metasynthesis of client experiences of cross cultural therapy is presented, which generated three themes. The first recognises the assumption of difference in a cross-cultural therapeutic relationship and the assumption that lived experience is qualitatively different from what can be learned by a therapist. The second elaborates the processes by which clients bridge this assumed divide in order to get benefit from therapy, including divorcing problems from the context of their culture. The third explores the emergence of the known variables of therapeutic relationship in this context. Clinical implications and the importance of recognising individual context in therapy is discussed. Section two of this thesis reports an interpretative phenomenological analysis of the experiences of six Islamic scholars in supporting members of the community. Four themes were derived from this analysis 'Building a personal relationship with the ultimate authority'; 'Treating the whole person'; 'God's Medicine'; and 'Duty bound to help'. The implications of these themes for the process of supporting people of Muslim faith are considered and the development of closer relationships with other support providers discussed. The third section of this thesis critically reviews and reflects upon the above research and explores the use of qualitative research as a cultural outsider and the process of developing and carrying out cross cultural research including reflections on the specific process of Interpretative Phenomenological Analysis in this research context.

Substance use and psychosis : an exploratory study

The needs of individuals who have a diagnosis of psychosis and use substances pose one of the greatest challenges faced by mental health services today (Department of Health, 2002). A large amount of research has been undertaken attempting to understand why apparently higher rates of substance use are observed in this population, yet the findings are equivocal at best. As such there is little understanding of the process by which substance use might be maintained in this population. This study aimed to address the research questions posed by investigating the experiences of the client group (individuals with a diagnosis of psychosis and substance use), a group whose experiences are notable by there absence from the research to date (Dixon et al, 1990). The objectives of the study were fulfilled, these being the exploration of individuals' experiences of use and the impact that this has upon their lives and the development of a model by which to understand the process by which substance use is maintained. Twelve qualitative interviews were analysed using Interpretative Phenomenological Analysis. Three super-ordinate themes were identified as emerging from the participants' transcripts; 'Increased control', 'Feeling out of control', and 'Ambivalence'. Ten composite themes contributed to the formation of these themes. The stages of change model and dialectical theory were used to understand the ambivalence that was seen to maintain participants' substance use. Further hypotheses were drawn from the literature regarding the process of change and a model of compiled. The implications for clinical practice and future research were discussed.

Elle constate un paradoxe : l’art dramatique qui fraye avec la folie aurait des vertus thérapeutiques.Ainsi peut se résumer l’intrigue de cette thèse en 3 actes dont les personnages principaux sont des patients, psychotiques pour la plupart, en mal de psychothérapie, et un artiste fou, grand homme de théâtre : Antonin Artaud.Par l’exposition de saynètes comme expression brute de l’inconscient, la présente thèse illustre le cheminement des réflexions de son auteure. Elle reprend d’abord la genèse d’une médiation singulière : l’improvisation dramatique. Ensuite, inspirée par le cas emblématique d’A. Artaud et à travers la psychanalyse, elle explore les mécanismes mis en jeu lors d’ateliers théâtre et leurs effets sur le psychisme des patients devenus comédiens. Enfin, en suivant le cheminement du noeud borroméen, elle conclut que l’improvisation dramatique, foncièrement inscrite dans le Réel, qui convoque l’Imaginaire et introduit le Symbolique, s’impose comme une activité sinthomatoire / During the 21st century, a clinical psychologist decides to offer to her patients a theatre stage as an exhibition space for their unconscious. She notes a paradox : drama, which has to do with madness, would have therapeutic properties.Thus can be synthesized the plot of this three-act PhD thesis, whose main characters are patients, mostly psychotic ones needing to undergo psychotherapy, and an insane artist, also great man of the Theatre: Antonin Artaud.Through presenting role-plays as raw expression of the unconscious, this thesis shows how its author’s thinking unfolds. It first draws on the genesis of a singular mediation: theatrical improvisation. Then, inspired by A. Artaud’s emblematic case and through psychoanalysis, it explores the mechanisms involved during theatrical workshops and their effects on the patients who became actors. Lastly, by following the path of the borromean node, it concludes that theatrical improvisation, which is deeply inscribed in the Real, which summons the Imaginary and introduces the Symbolic, emerges as a sinthomatic activity.

The Child-therapist / L'enfant-thérapeute

Le « child-therapist » évoque un enfant qui sacrifie son psychisme et ainsi son développement pour la survie du système familial. Cette thèse cherche à explorer son vécu et à mieux comprendre ce qu’il vit. A travers la méthode d’observation et une analyse utilisant la méthode d’IPA (Interpretational Phenomenological Analysis), je questionne son développement psychique, sa mentalisation et la colère latente qu’il peut ressentir envers ses parents. Cette recherche a révélé que certains child-therapists se développent à travers un conditionnement, c’est-à-dire à partir des tâtonnements vers des comportements désirés, ainsi que des comportements qui réduisent les tensions au sein de la famille. Aussi, en présence de ses parents, il démontre de fausses capacités de mentalisation. Enfin, sans exception, il porte une colère envers ses parents défaillants. Cette recherche a donc élucidé le fonctionnement mental du child-therapist. / The child-therapist speaks of a child who sacrifices his own psychical development for the survival of his or her family. This dissertation sets out to understand the mind of the child. Using the observational method and the IPA (Interpretational Phenomenological Analysis) to analyse the data,I set out to investigate his or her development, his or her mentalization capacities and a latent anger that he or she could harbour towards his or her parents. This research showed that the child develops through a form of conditioning. In addition, he or she exhibits false mentalization capacities, but only in the presence of his or her parents. Lastly, the child holds anger and resentment towards his parents that failed him or her. This study helped shed light on the mind of the child-therapist.

A partir de sa clinique au sein d'une unité d'accueil familial thérapeutique pour adultes psychotiques, l'auteure revient à la clinique et à la théorie des psychoses dissociatives non pas à partir d'une théorie des maladies mentales et d'une psychopathologie générale (comme peuvent l'être la psychanalyse, la phénoménologie ou l'organodynamisme) mais à partir du « travailler » et du « travail vivant » de ceux qui prennent soin de ces malades. Si on part de l'analyse pratique du soin (Accueil Familial Thérapeutique) avec les psychotiques et jusqu'à leur caractérisation clinique, on en arrive à une conception renouvelée de la psychose dissociative.Ces hypothèses théoriques permettent alors à l'auteure de proposer une nouvelle anthropologie de la famille et du maternel, à partir des apports théoriques de la psychodynamique du travail. / From her clinical experience in a therapeutic foster care unit for psychotic adults, the author returns to the clinic and the theory of dissociative psychosis, not from the perspective of the theory of mental illness and general psychopathology (as in psychoanalysis, phenomenology or organo-dynamism), but rather from the "work" and "living work" of those who care for these patients.If one starts from the practical analysis of treatment (Therapeutic Foster Care) with psychotic patients and their clinical characterization, one arrives at a renewed understanding of dissociative psychosis.These theoretical assumptions then allow the author to propose a new anthropology of the family and mother, rooted in theoretical contributions from the psychodynamics of work.

Η γνωσιακή ψυχοθεραπεία είναι ένα είδος θεραπείας που στηρίζεται στην παραδοχή ότι οι σκέψεις των ανθρώπων είναι εκείνες που επηρεάζουν τη συμπεριφορά και τα συναισθήματά τους. Πρόκειται για μια επικοινωνιακή περίσταση με βασικό της εργαλείο το λόγο. Στην παρούσα διπλωματική εργασία θελήσαμε να εξετάσουμε διεξοδικά τη χρήση της γλώσσας στο συγκεκριμένο ψυχοθεραπευτικό πλαίσιο. Πιο συγκεκριμένα, σκοπός της διπλωματικής εργασίας ήταν η εξέταση των γλωσσικών επιλογών που συντελούνται στο πλαίσιο της γνωσιακής ψυχοθεραπείας και ο βαθμός στον οποίο αυτές οι επιλογές επηρεάζουν τη θεραπευτική σχέση και τη χρήση των θεραπευτικών τεχνικών. Για την επίτευξη αυτού του σκοπού, υιοθετήθηκε η οπτική της συστημικής λειτουργικής γλωσσολογίας, σύμφωνα με την οποία η γλώσσα αποτελεί πόρο νοήματος σε συνάρτηση με το κοινωνικό- πολιτισμικό περιβάλλον. Δόθηκε έμφαση στη διαπροσωπική διάσταση της γλώσσας, επομένως η γνωσιακή ψυχοθεραπεία αντιμετωπίστηκε ως ένα είδος διάδρασης. Το διαπροσωπικό νόημα της γλώσσας είναι εκείνο που μας επιτρέπει να αντιληφθούμε τους ομιλιακούς ρόλους που επιλέγουν οι μετέχοντες στο θεραπευτικό πλαίσιο, τα χαρακτηριστικά τους και τη μεταξύ τους σχέση. Όσον αφορά το μεθοδολογικό πλαίσιο, έγινε ανάλυση λόγου σε αποσπάσματα από γνωσιακές συνεδρίες που αφορούσαν όλα τα στάδια της θεραπείας. Χρησιμοποιήθηκαν δύο μεθοδολογικά εργαλεία: το σύστημα Λειτουργίας του Θεραπευτικού Λόγου (system of Therapeutic Speech) και το σύστημα της Αποτίμησης. Το σύστημα Λειτουργίας του Θεραπευτικού Λόγου προέκυψε από τη σύνθεση των βασικών σημείων δανεισμένων από δύο διαπροσωπικά γλωσσολογικά συστήματα, το διαπροσωπικό σύστημα (Interpersonal System) των Halliday & Hasan (1989) και το σύστημα των Γλωσσικών Λειτουργιών (system of Speech Function) των Halliday and Matthiessen (2004) σε συνδυασμό με την τυπολογία του Labov (1997). Τα αποτελέσματα της ανάλυσης λόγου ανέδειξαν γλωσσικά μοτίβα που διέπουν το θεραπευτικό λόγο γενικά, καθώς και γλωσσικά μοτίβα που αφορούν ειδικά το γνωσιακό ψυχοθεραπευτικό λόγο. Επιπλέον, τα αποτελέσματα έδειξαν ότι οι γλωσσικές επιλογές των μετεχόντων φαίνεται να διαμορφώνουν τη μεταξύ τους σχέση και να συμβάλουν στη χρήση των θεραπευτικών τεχνικών. / Cognitive psychotherapy is a type of therapy based on the premise that people's thoughts are those that affect their behavior and feelings. It is a communicative circumstance with discourse as its main tool. In the present thesis we wanted to examine in detail the use of language in that particular psychotherapeutic context. More specifically, the thesis’ goal was the investigation of the language choices that occur in the context of cognitive psychotherapy and the extent to which those choices affect the therapeutic relationship and the use of therapeutic techniques. To achieve this goal, the perspective of Systemic Functional Linguistics was adopted, according to which language is a resource of meaning in relation to the socio-cultural environment. Emphasis was given at the interpersonal dimension of language therefore cognitive psychotherapy was treated as a kind of interaction. The interpersonal meaning of language is that which allows us to understand the speech roles chosen by participants in the therapeutic context, their characteristics and their interrelationship. Concerning the methodological framework, a discourse analysis of extracts from cognitive sessions, involving all stages of treatment, took place. Two methodological tools were used: the System of Therapeutic Speech and the Appraisal System. The System of Therapeutic Speech resulted from the synthesis of main elements adopted from two linguistic systems, the Interpersonal System of Halliday & Hasan (1989) and the system of Speech Function of Halliday and Matthiessen (2004) in combination with Labov’s typology (1977). The results of discourse analysis revealed linguistic patterns governing the therapeutic discourse in general as well as linguistic patterns specific to the discourse of cognitive psychotherapy. Furthermore, the results showed that the linguistic choices of the participants seem to shape their relationship and contribute to the use of therapeutic techniques.

Cette thèse mène une réflexion historique et épistémologique sur la façon dont les altérations de la parole ont été étudiées en médecine mentale. Retraçant les étapes de l’attention accordée aux paroles des patients dans l’aliénisme de la première moitié du XIXe siècle jusque dans la clinique psychiatrique du début du XXe siècle, elle tente de comprendre avec quelles motivations et dans quels buts les médecins ont observé ces phénomènes. L’analyse précise la manière dont certaines modifications de la parole sont devenues fondamentales pour le diagnostic des maladies mentales et indique la place que les spécialistes leur ont attribuée dans leur sémiologie de la folie. L’interrogation porte ainsi sur les ressources pratiques et théoriques avec lesquelles les médecins ont observé et analysé ces troubles du langage. Les méthodes et les procédures mises en place pour récolter les paroles des patients et les outils conceptuels permettant de les appréhender sont examinés. L’enquête fait en outre ressortir les partages disciplinaires qui s’opèrent à cette époque dans le champ des sciences médico-psychologiques, en tenant compte de la façon dont les auteurs nomment, définissent et classent ces désordres langagiers, tout en soulignant les liens explicites ou implicites qu’ils établissent avec des domaines ne relevant pas directement de la médecine, tels que la philosophie, l’anthropologie, la psychologie et la linguistique. / This doctoral thesis offers a historical and epistemological reflection on the way in which mental medicine has studied speech alterations.Tracing the various stages of the interest given to patients’ speech in alienist medicine and in psychiatry from the 19th century to the early 20th century, this work attempts to understand the goals and motivations of medical doctors observing these phenomena. The analysis helps clarify how speech modifications have become fundamental in diagnosing mental illness and how specialists have attributed a special place to them in their semiology of madness. It also questions the practical and theoretical resources with which medical doctors have observed and analysed language disorders. It examines the methods and processes used to gather patients’ speech and the conceptual tools that enable its apprehension. Moreover, this survey emphasizes the partition of the medico-psychological disciplines occurring at the time, by observing the ways in which the authors name, define and classify these speech and language disorders, and considering the links implicitly or explicitly made with other non-medical fields such as philosophy, anthropology, psychology and linguistics.

Formes et figures du psychodrame : l'exemple de l'adolescence / Forms and figures of psychodrama : the example of adolescence

Prenant le parti de la valeur mutative du psychodrame psychanalytique, cette recherche vise une description métapsychologique de ce qui en fonde l'efficacité, comme de ses spécificités dans la mise en oeuvre des desseins psychothérapiques de la psychanalyse. L'exemple du renforcement pulsionnel propre à l'adolescence questionne les capacités de symbolisation et de montage pulsionnel permis tant par l'appareil psychique que par les dispositifs (de prothèse ou de détour) que sont les dispositifs psychothérapiques. Trois études longitudinales constituées par des récits de cures psychodramatiques d'adolescents donnent un appui à la construction d'une « figurabilité» spécifiquement psychodramatique grâce à trois opérateurs, nommés le « narratif », le « scénique » et le « dramatique ». Le but de cette recherche est la fabrique métapsychologique de ces opérateurs à l'intersection de la pratique en séance et de la littérature psychanalytique. La notion freudienne de « figurabilité » sert de cadre à la construction de ces opérateurs et à ce qu'ils articulent des capacités de symbolisation du psychisme et de celles d'un dispositif de psychothérapie. La continuité d'un éprouvé de subjectivation permis par l'opérateur du « narratif » se complète des spécificités fictionnelles de la narrativité psychodramatique. De salutaires contournements des mécanismes de censure et une plus ample fréquentation de la réalité psychique soutiennent ainsi la mise en forme des quantités pulsionnelles. La circulation de la notion de « scène » dans la métapsychologie décrit la dimension processuelle que soutient l'opérateur du « scénique ». Si la mise en scène est au service du saisissement conscient, elle opère aussi par le recours à l'hallucinatoire. Ce que la scène du psychodrame permet d'une distinction des objets et contraint d'un commerce avec eux est un facteur subjectivant d'« objectalisation » qui fait contrepoids au repli narcissique. L'objet fabriqué par le psychodrame naît des capacités d'accordage de la scène psychodramatique, celles-ci étant décrites à travers leur usage singulier de la transitionnalité. Enfin, l'opérateur du « dramatique » déploie la fécondité des modes de présence de l'acte dans la représentation psychodramatique. L'excitation véhiculée par le jeu en groupe appelle un travail constant de mise en forme qui utilise divers médiums expressifs. L'objet est à la fois celui qui provoque les quantités d'excitation et celui qui en soutiendra la liaison. L'activité figurative s'articule à une activité pare-excitante qui peut relancer l'activité régulatrice du refoulement. Entre impression et expression, l'activité esthétique du jeu de l'acteur consiste en la modulation des valeurs attribuées aux représentations. Le « dramatique » peut alors se définir comme un art des variations en quête de sens. Les objets de sens issus de ces variations d'affectation sont ensuite détaillés comme visée topographique et subjectivante des entrelacs entre réalité matérielle et réalité psychique. Par sa mise en jeu et en scène, une place de représentation est plus aisément donnée ou re-donnée au transfert afin d'en défaire la potentialité agissante. Pour finir, le dernier récit clinique permet de montrer l'utilité et le caractère structurant de ces opérateurs dans la mise en scène adolescente du second acte de la dramaturgie oedipienne. / Granting that psychoanalytic psychodrama can effect change, this project seeks to produce both a metapsychological description of the basis for its effectiveness and an account of the specific ways in which it pursues the therapeutic aims of psychoanalysis. The example of adolescent drive reinforcement raises questions about the capacities for symbolization and for the organization of drives that are made possible as often by the psychic apparatus as by the mechanisms (whether of prostheses or of detours) of psychotherapy. Three longitudinal studies, narratives of psychodramatic treatments of adolescents, anchor the construction of a specifically psychodramatic "figurability" with three modalities, the "narrative", the "scenic", and the "dramatic". This research project aims to produce a metapsychological account of these modalities at the intersection between clinical practice and the psychoanalytic literature. The Freudian notion of "figurability" serves as the framework for constructing these modalities and for representing their capacity to symbolize the psyche and to articulate a psychotherapeutic instrument. The continuity of an experience of subjectivization permitted by the "narrative" modality is completed by the fictional specificities of psychodramatic narrativity. Salutary evasions of the mechanisms of censorship and increased commerce with psychic reality thus support the shaping of drive quantities. The circulation of the notion of "scene" in metapsychology describes the dimension of process that is supported by the "scenic" modality. If the staging is at the service of a conscious awareness, it also operates through recourse to the hallucinatory. What the scene of psychodrama permits regarding a distinction of objects and constraints regarding interactions with them, is a subjectivizing factor of "objectalization" that provides a counterweight to narcissistic retreat. The object created by the psychodrama grows out of the capacities for harmonization of the psychodramatic scene, the capacities being described through their singular use of transitionality. Finally, the "dramatic" modality unfolds the fecundity of the modes of presence of the act in psychodramatic representation. The excitement conveyed by group play-acting calls for a constant work of shaping which uses various expressive media. The object is at once the one that provokes quantities of excitement and the one that will support their linking. The figurative activity is articulated with a protective shield that can relaunch the regulating activity of repression. Between impression and expression, the aesthetic activity of the actors performance consists in the modulation of the values attributed to the representations. The "dramatic" can then be defined as an art of variations in search of meaning. The meaningful objects that emerge from these variations of affectation are then scrutinized as the topographic and subjectivizing aim of the interlacings between material reality and psychic reality. By being set into play and put on stage, a place of representation is more easily given or restored to the transference so as to undo its "agieren" effective potentiality. In conclusion, the last clinical narrative makes it possible to show the utility and the structuring character of these modalities in the adolescent staging of the second act of the Oedipal drama.
