Stinson1, PhD, and Megan Quinn2, DrPH, MSc, East Tennessee State University, Johnson City, TN 1Dept. of Psychology, College of Arts and Sciences 2Dept. of Biostatistics and Epidemiology, College of Public Health Existing literature demonstrates a dose-responsive relationship between adverse childhood experiences (i.e., abuse, neglect, and household dysfunction) and the risk of problematic substance use later in life. While controlling for the effects of early adversity, a recent study found that childhood household dysfunction independently increased the risk of excessive alcohol use in adulthood. However, there are few studies focusing specifically on differences in substance use behaviors in individuals who had a caregiver with a mental health diagnosis. This study aims to fill this gap by examining whether having a caregiver with mental illness during one’s childhood increases the risk of abusing alcohol and illegal or prescription drugs in adulthood in a college sample. Our sample includes University students in the Southeastern U.S. (N = 995) who are mostly Caucasian (84%) and female (63%). Participants completed an online, anonymous survey for course credit. Approximately 23% (n = 227) reported having a caregiver with mental illness or who attempted or died by suicide. Regarding substance abuse, nearly 11% of the sample (n = 106) reported abuse of alcohol, 13% (n = 126) indicated abuse of illegal drugs, and almost 5% (n = 47) reported abuse of prescription medication. Pearson Chi-square tests were conducted to analyze the relationship between caregiver mental illness, including a caregiver who attempted or died by suicide, and abuse of alcohol, illegal, and prescription drugs in adulthood. Interestingly, individuals with a mentally ill caregiver were less likely to abuse alcohol, c2 (1, N = 983) = 12.56, p = .000, and illegal drugs, c2 (1, N = 984) = 22.68, p = .000, as an adult, compared to those without a mentally ill caregiver. In contrast, students with a mentally ill caregiver were more likely to abuse prescription drugs as adults, c2 (1, N = 979) = 32.54, p = .000, compared to those without a caregiver suffering from a mental illness. Thus, findings suggest that having a mentally ill caregiver differentially impacted the risk of abusing varying types of substances. Additional analyses examining the influence of mental illness in biological versus non-biological caregivers (e.g., stepparent, foster parent) and further discussion of implications will be included.

The Effectiveness of Educating Medical Residents on ACE Scores

Adverse Childhood Experience Scores, more commonly known as ACE scores, have provided new insight into the effects of adverse events on longstanding health and chronic illnesses in adults. Evidence shows that individuals with history of ACEs have increased risk of developing multiple chronic illnesses with or without increased exposure to activities that are detrimental to health, i.e. tobacco, alcohol, or other drug use. Additionally, a history of ACEs significantly increases an individual’s likelihood of participating in behaviors that are detrimental to long-term health, including risky behaviors and resulting addiction issues. However, in most individuals with ACEs, their childhood experiences, or the extent of the experience, is not likely to be mentioned in casual conversation. By increasing knowledge on ACE scores and implementing scoring of all individuals possible, it would improve a physician’s ability to assess risk and health screening needs for a patient on a more individualized basis. The purpose of this study is to educate Bristol Family Medicine residents on the daily uses and clinical correlations provided by ACE scoring in an attempt to increase score documentation of future patients.We will begin by providing a short knowledge assessment about ACE scores to residents in the Bristol Family Medicine residency program. We will then give a detailed presentation on the usefulness and effectiveness of using ACE scores in clinical judgement and screening. Following the presentation, we will provide a similar knowledge assessment as well as a questionnaire to assess likelihood of residents to implement ACE scores in their current practice.

Dental health effects adverse childhood experiences on U.S. adults: BRFSS 2010-2012

OBJECTIVE: To examine association between Adverse Childhood Experiences (ACE) and teeth extracted due to dental caries or periodontitis among young and middle-aged US adults. METHODS: Study sample included 15,474 and 17,430 respondents aged 18-64 completing 2010 and 2012 Behavioral Risk Factor Surveillance System (BRFSS) survey respectively. Outcome was any self-reported tooth extraction due to caries or periodontitis. Primary predictor were self-reported 11 ACE answers (categorized into none, 1, 2, and 3+) before 18 years of age. Covariates included smoking, heavy alcohol consumption, diabetes, health coverage and access to dental care. Odds ratios were computed from unadjusted and adjusted logistic regression models. RESULTS: Overall ACE prevalence was 66.5% and 64.9% in BRFSS 2010 and 2012 study sample respectively. In unadjusted models we observed a graded association wherein adults reporting experienced 2 and 3+ ACE were more likely to experience tooth extractions compared to those reporting no ACE exposure. (BRFSS 2010 OR = 1.47,1.48; BRFSS 2012 OR = 1.33, 1.54 respectively; p<0.05). We found similar results in multivariate models after adjusting for covariates (BRFSS 2010 OR = 1.56,1.34; BRFSS 2012 OR = 1.36, 1.42 respectively; p<0.05). CONCLUSIONS: This study suggests potential long lasting impact of early adverse life events on oral health in young and middle adulthood. Young and middle-aged adults experiencing multiple ACE had significantly more extractions compared to adults who reported not experiencing any ACE. Multidisciplinary efforts between dentists, physicians and social therapists are needed to raise awareness about ACE as means of identifying and reducing dental health inequities. / 2020-06-13T00:00:00Z

According to the organisation Maskrosbarn around 500.000 children in Sweden live with a parent or guardian that suffers from mental illness, addiction or subject them to violence. An even number of children live in lower socio-economic brackets of society. Most children in Sweden come in contact with sports in one way or another throughout their childhood. With that in mind the aim of this study was to research the sport movements attitudes and resources are when it comes to children from the form mentioned disadvantaged backgrounds. The method used was a qualitive method where semi-structured interviews were conducted with six different individuals. These individuals all work with in the Swedish sports movement in some way and have relevant insights to this paper.    The result was analyzed through a thematic analysis which resulted in three different main themes; The effects of sports which was then split into two subthemes organized sports and physical activity, the next main theme was the child’s perspective which was split into: barriers and participation. The final main theme was the sport movements current state which was spilt into two subthemes: Barriers and knowledge. The results show that the respondents considered that sports and physical activity had a profound role in children’s lives and that it has specific and relevant impact for children from disadvantaged backgrounds.

Na terceira etapa são estudadas as inter-relações entre as práticas prescritas dos dois sistemas em uma das unidades de manufatura da UTC. A identificação e análise das inter-relações realizadas neste estudo apresentam uma estrutura organizada que permite o planejamento de um projeto de unificação dos sistemas. Outra contribuição relevante deste estudo é a possibilidade de utilização do método para analisar inter-relações entre outros sistemas. / This research investigates the Operating System of the United Technologies Corporation - UTC, called ACE, identifying and analyzing their interrelations with the Quality Management System ISO9001. The methodology of this research is characterized by a qualitative approach and case study incorporated, which is conducted in three stages. The first step is a broad description of the ACE Operating System to introduce this system to the academy. The second step is the study of the interrelationship between the two theoretical systems. In the third stage are studied the relationship between the practices prescribed for the two systems in one UTC manufacturing unit. The identification and analysis of the interrelations in this study have made an organized structure that allows the planning of a project of unification of the systems. Another important contribution of this study is the use of the method for analyzing the interrelations between others systems.

Η στεφανιαία νόσος (ΣΝ) αποτελεί τη συχνότερη πάθηση του καρδιαγγειακού συστήματος. Κύρια αιτία της νόσου είναι η αθηροσκλήρυνση, που σήμερα θεωρείται πλέον μια χρόνια φλεγμονώδης αντίδραση του αγγειακού τοιχώματος. Η λειτουργικότητα του ενδοθηλίου του αγγειακού τοιχώματος που αποτελεί προγνωστικό δείκτη καρδιαγγειακών συμβαμάτων και μέθοδο ελέγχου της ανταπόκρισης σε φάρμακα μπορεί να προσδιοριστεί με μέτρηση της αγγειοδιαστολής που διαμεσολαβείται από τη ροή (Flow-Mediated Dilation, FMD) στη βραχιόνιο αρτηρία. Σημαντικό ρόλο στην παθοφυσιολογία της αθηροσκλήρυνσης ασκεί το σύστημα Ρενίνης-Αγγειοτενσίνης-Αλδοστερόνης (Renin-Angiotensin-Aldosterone System – RAAS) που διακρίνεται σε ενδοκρινές και ιστικό. Κεντρικά σημεία του συστήματος RAAS που αποτελούν και φαρμακευτικούς στόχους είναι η μετατροπή της Αγγειοτενσίνης Ι (ΑΤ Ι) σε Αγγειοτενσίνη ΙΙ ( ΑΤ ΙΙ ) με τη δράση του μετατρεπτικού ενζύμου της ΑT Ι (Angiotensin Converting Enzyme-ACE) και η επίδραση της ΑΤΙΙ στους υποδοχείς της. Στο ιντρόνιο 16 του γονιδίου του ενζύμου ACE(17q23) έχει βρεθεί ο πολυμορφισμός I/D που προκύπτει από την παρουσία ( Insertion– I) ή την απουσία (Deletion–D) μιας Αlu αλληλουχίας μήκους 287 bp, δημιουργώντας τρείς διακριτούς γονότυπους: II, ID και DD. Οι ομοζυγώτες DD παρουσιάζουν αύξηση κατά 50% των επιπέδων του ACE στον ορό σε σχέση με τους ομοζυγώτες II. Οι ετεροζυγώτες ID εμφανίζουν ενδιάμεσα επίπεδα. Με δεδομένο το ρόλο του συστήματος RAAS στη ΣΝ, ο ρόλος του πολυμορφισμού ACE I/D έχει καταστεί αντικείμενο μελέτης ως προς την εκδήλωση στεφανιαίας νόσου και την ανταπόκριση στεφανιαίων ασθενών στα διάφορα θεραπευτικά σχήματα αποκλεισμού του συστήματος RAAS. Στην παρούσα μελέτη προσδιορίστηκε ο πολυμορφισμός ACE I/D σε 100 φυσιολογικά άτομα και σε 100 στεφανιαίους ασθενείς. Το συμπέρασμα μετά τη στατιστική ανάλυση ήταν ότι ο γονότυπος DD δεν συνδέεται στατιστικώς σημαντικά με στεφανιαία νόσο στον ελληνικό πληθυσμό που εξετάστηκε (p>0,05). Επιπλέον προσδιορίστηκε ο πολυμορφισμός ACE I/D σε 30 στεφανιαίους ασθενείς που έλαβαν διάφορα θεραπευτικά σχήματα αποκλεισμού του συστήματος RAAS. Η ανταπόκριση των ασθενών στην αγωγή εκτιμήθηκε μέσω της μεταβολής της FMD. Ο μικρός αριθμός των ασθενών και η μεγάλη τυπική απόκλιση στις μεταβολές της FMD δεν επέτρεψε την εξαγωγή ασφαλών συμπερασμάτων για το ρόλο του πολυμορφισμού στην ανταπόκριση των ασθενών στην φαρμακευτική αγωγή. / The Renin-Angiotensin-Aldosterone System (RAAS) is an important factor for the pathogenesis of Coronary Artery Disease (CAD). The key component of RAAS is the Angiotensin Converting Enzyme (ACE). An Insertion/Deletion polymorphism (I/D) has been identified in ACE gene which accounts for half the variance of serum ACE levels (1). Subsequent reports investigated the relationship between the D allele and cardiovascular diseases, icluding CAD, with conflicting results (2). We determine ACE I/D polymorphism genotype in 100 normal individuals and 100 patients with CAD of greek origin. No association was found between D allele and CAD. Moreover, we determine ACE I/D polymorphism in 30 patients with CAD who were in treatment inhibiting RAAS. No association was found between ACE I/D polymorphism and respond.

Alternatively, the use of whey for the production of dairy beverages has been a very attractive option, especially due to its nutritional value. Probiotic cultures are composed of microorganisms that confer health benefits for the host, can be used in fermented beverages. Several food proteins contain physiologically active peptides, known as bioactive peptides. Milk proteins are considered a major source of potentially inhibitory peptides on the activity of the angiotensin-converting enzyme (ACE). This study aimed to prepare fermented dairy beverages produced with goat cheese whey powder considering the addition of a probiotic microorganism and its viability in the product, as well as, to evaluate bromatological, microbiological and physicochemical properties during the storage, and also to investigate the inhibitory activity on the ACE. The fermentation process was performed for the beverages of the control trial (BLC), added with Streptococcus thermophilus TA 40, and the probiotic trial (BLP), added with Streptococcus thermophilus TA 40 and Lactobacillus casei BGP 93. The chemical composition of the whey powder and the final milk beverages was evaluated, as well as the pH, titratable acidity and viability of the microorganisms in the reconstituted whey after the addition of the cultures, before fermentation (T0), after fermentation (TF), and after 1, 7, 14 and 21 days of storage at 4 °C. The instrumental texture for both beverages was analyzed during the storage period. The sensory analysis was performed on D7, D14 and D21 sampling periods. The protein content was analyzed for the reconstituted whey (SR) and for beverages on D1, D7, D14 and D21 sampling periods. The degree of proteolysis by the o-ftaldialdeíde (OPA) method and sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) analysis were carried out to evaluate the protein degradation in samples during the processing and storage. The inhibitory activity on the angiotensin converting enzyme (ACE) was measured in order to obtain the bioactive potential of the product. The pH during fermentation (T0 and TF) and storage decreased from 5.71 to 4.92 and from 5.72 to 4.97 for BLC and BLP, respectively. The acidity for both trials increased significantly (p<0.05) between T0 and TF and showed significant differences neither between storage periods nor between trials (p>0,05). The viability of S. thermophilus was in accordance with the levels required by the Brazilian legislation for fermented dairy beverages, showing populations higher than 10 CFU/ml in the final product, during the whole storage period. Viable L. casei cells in the probiotic trial were consistent with the recommended levels by the literature (above 10 – 10 CFU/ml) for products with probiotic benefits. The final products were in accordance with the Brazilian legislation for microbial contaminants, which present less than 3 most probable number (MPN)/ml to 100 MPN/ml for coliforms at 30/35 °C and at 45 °C. The texture parameters did not differ significantly (p>0.05) between the trials and the storage period. The presence of the probiotic microorganism did not result in significant differences between trials in the sensory evaluation. The protein content of the SR was significantly higher (p<0.05) than that verified in the dairy beverages during the storage periods for the two trials, which showed values of 2.2 g/100 g for BLC and of 2.1-2,17 g/100 g for BLP. Both beverages showed, during the whole storage, protein levels higher than the minimum of 1.0 g/100 g required by the Brazilian legislation for dairy beverages with added ingredients. Although the degree of proteolysis did not differ significantly between trials and storage time (p>0.05), there was a tendency for the increasing of this parameter during the storage period, as well as a trend of higher proteolysis in the probiotic trial, suggesting that the addition of L. casei promoted the release of peptides, since it was also found electrophoretic bands with larger size ranges for BLP in relation to those verified for BLC. For the ACE assay, both BLP and BLC were able to inhibit the enzyme under study, with increased values for BLP on D1, close to that found with captopril (reference medicine to treat hypertension). / O soro caprino é considerado um subproduto da produção queijeira e muitas vezes descartado sem nenhum tratamento como efluente, tornando-se uma fonte de poluição. Como alternativa, a utilização de soro de queijo para a produção de bebidas lácteas tem sido uma opção muito atraente, notadamente devido ao seu valor nutricional. Culturas probióticas são constituídas de microrganismos que conferem benefícios à saúde do hospedeiro, podendo ser utilizadas em bebidas fermentadas. Muitas proteínas dos alimentos contêm peptídeos fisiologicamente ativos, chamados de peptídeos bioativos As proteínas do leite são consideradas uma das principais fontes de peptídeos com potencial de inibição da atividade da enzima conversora de angiotensina (ACE). Este trabalho teve por objetivo elaborar bebidas lácteas fermentadas produzidas a partir do soro de queijo de cabra em pó considerando a adição de um microrganismo probiótico e sua viabilidade no produto e avaliar as propriedades bromatológicas, microbiológicas e físico-químicas ao longo do armazenamento, e a capacidade inibitória sobre a ACE. O processo de fermentação foi realizado para as bebidas lácteas controle (BLC), adicionada de Streptococcus thermophilus TA 40 e probiótica (BLP), adicionada de Streptococcus thermophilus TA 40 e Lactobacillus casei BGP 93. Foram avaliados os valores de composição do soro em pó e das bebidas lácteas finais, pH, acidez titulável, viabilidade dos microrganismos no soro reconstituído após a adição das culturas, antes da fermentação (T0), depois da fermentação (TF), após 1, 7, 14 e 21 dias de armazenamento a 4 °C (D1, D7, D14 e D21, respectivamente). A textura instrumental para ambas as bebidas foi analisada durante o período de armazenamento. A análise sensorial realizada nos tempos D7, D14 e D21. A quantificação de proteínas foi efetuada no soro reconstituído (SR) e nas bebidas nos tempos D1, D7, D14 e D21. As análises de grau de proteólise pelo método o-ftaldialdeído (OPA) e eletroforese em gel de poliacrilamida-dodecil sulfato de sódio (SDS-PAGE) foram realizadas para avaliar a degradação proteica nas amostras ao longo do processamento e armazenamento. A atividade inibidora sobre a ACE foi avaliada a fim de se obter a potencial atividade bioativa do produto. Os valores de pH durante a fermentação (T0 e TF) e o armazenamento diminuíram de 5,71 para 4,92 e de 5,72 para 4,97, para BLC e BLP, respectivamente. A acidez para os dois tratamentos aumentou significativamente (p<0,05) entre T0 e TF, mas não diferiu significativamente nem entre os períodos de armazenamento (D1 a D21), nem e os tratamentos (p>0,05). A viabilidade de S. thermophillus esteve em conformidade com o exigido pela legislação para bactérias láticas viáveis em bebidas lácteas fermentadas, apresentando populações superiores a 10 UFC/ml, no produto final, durante todo o período de armazenamento. A viabilidade de L. casei, no tratamento probiótico, foi condizente com o valor recomendado pela literatura (acima de 10 – 10 UFC/ml) para produtos com benefícios probióticos. Os produtos finais estiveram em conformidade com a legislação para contaminantes, apresentando resultados menores que 3 número mais provável (NMP) /ml até 100 NMP/ml, para coliformes a 30/35 °C e a 45 °C. Os parâmetros de textura não diferiram significativamente (p>0,05) quanto aos tratamentos e ao período de armazenamento. A presença do microrganismo probiótico não resultou em diferença significativa entre os tratamentos na avaliação sensorial. O teor proteico do SR foi significativamente maior (p<0,05) que o verificado nas bebidas lácteas dos dois tratamentos, em todos os períodos de amostragem, as quais apresentam valores de 2,2 g/100 g na BLP e de 2,1-2,17 g/100 g na BLP. Ambas as bebidas apresentaram durante todo o armazenamento teores proteicos maiores que o mínimo de 1,0 g/100 g exigido pela legislação para bebidas lácteas com adições. O grau de proteólise, embora não tenha diferido entre os tratamentos e o tempo de amostragem (p>0,05), apresentou uma tendência de aumento ao longo do período de armazenamento, assim como tendência de maior proteólise no tratamento probiótico, sugerindo que a adição de L. casei promoveu maior liberação de peptídeos, pois também foram encontradas bandas eletroforéticas inferiores a 6 kDa com faixas de tamanhos maiores para BLP em relação à BLC. Para o ensaio com a ACE, tanto BLP quanto BLC foram capazes de inibir a enzima em estudo, com valores superiores para BLP, sendo que para esta bebida no D1 os valores foram próximos ao encontrado para o captopril (medicamento de referência para o tratamento da hipertensão).
