Um dos contextos propícios para evocar e modelar o comportamento verbal e a qualidade na interação em crianças com TEA é o ambiente de contação de histórias. A leitura dialógica permite uma postura ativa da criança diante da contação, que é estimulada a partir de estratégias de formulação de perguntas e consequenciação. Estudos sobre o efeito da leitura dialógica e suas possíveis adaptações para crianças com TEA ainda são escassos. Este estudo investigou os efeitos de uma adaptação da leitura dialógica no comportamento verbal sob controle da narrativa em crianças com TEA. Buscou, ainda, testar o uso de hierarquia de dicas para auxiliar nas respostas vocais das crianças, e verificar se houve aumento nas suas iniciações verbais e no engajamento na tarefa. Duas crianças de sete anos com diagnóstico de TEA participaram do estudo. Foi realizado um procedimento com delineamento de sujeito único, a partir da utilização de linha de base múltipla por sujeito. Os resultados mostraram aumento nas respostas independentes (sem dica) diante de perguntas tipo “Q” e “O que está acontecendo aqui?”, à medida que houve diminuição gradual no uso da hierarquia de dicas. Ademais, o estudo mostrou aumento nas iniciações verbais para uma das crianças e tendência ascendente no engajamento na tarefa ao longo das sessões para ambas. Esse estudo corrobora com estudos anteriores e agrega dados sobre leitura dialógica com crianças com TEA. / Autistic Spectrum Disorder (ASD) has as its main characteristics difficulties in communication and social interaction, as well as restricted and repetitive patterns of behavior. One context for evoking and modeling verbal behavior and improving the quality of interaction in children with ASD is shared reading. Dialogic reading favors dialogue by prompting and differentially reinforcing verbal interaction about the storybook. Studies on the effect of dialogic reading and possible adaptations of this strategy for use with children with ASD are still scarce. The present study investigated the effects of an adaptation of dialogic reading, using verbal prompts to help children answer questions about the story. We verified the proportion of correct answers as well as verbal initiations and on-task behaviors, using an across-subject multiple baseline design. A baseline condition with no prompts was compared to two conditions in which a least-to-most scaffolding strategy was used to prompt the answers as needed. Participants were two boys (7 y) with formal diagnosis of ASD. Results showed a steady and gradual increase in correct answers to “wh” type questions during Intervention 1 and a rapid improvement in answers to open-ended questions (“What is going on here?”) during Intervention 2, both accompanied by growing independence of prompts. There was an increase in verbal initiations for one of the children. On-task behavior was high from the beginning and showed a slight increasing trend throughout sessions. This study corroborates and expands previous studies on adapting dialogic reading as a naturalistic approach for teaching verbal repertoires to children with ASD.

Investigar essa questão, em campo psicanalítico, tem aqui o sentido de aprender com a clínica e se deixar levar pelos solavancos, pelas delícias do contato, pelas mudanças abruptas e sofridas de estados emocionais, com vistas a despertar a sensibilidade da criança e do analista, por vias inauditas. Se essa conduta é capaz de deslocar o analista das palavras, convidando-o para um tal aproximar-se de corpos, também pode levá-lo a cogitar, mais tarde, em suas especulações teóricas, sobre o psíquico e suas marcas arcaicas, sobre a conformação auto-engendrada do corpo sensível e sobre as formas estéticas de existência, subordinadas pela atividade de prazer/desprazer e mantidas em regulação, ritmos, contatos e desenhos singulares. Estará aí, esse intrigante sujeito, em sua vivência autística? Essas e outras questões são tratadas ao longo deste trabalho, que considera essa dimensão sensível, que quase nunca admite tradução, indispensável à restituição do elemento trágico do qual nos afastamos, cujo corpo é o teatro privilegiado, tornando claudicante nossa experiência de sofrimento humano. / The clinic with the autistic child, involved in so many enigmas, reflects the place where the reach of our imagination presumes the first plot of the existence of each one of us. Where does the psychic life of these children who had came inside themselves beat? Investigating this question, in a psychoanalytic field, means learning with the clinic and being led by hard jolts, delights of the contact, and abrupt and suffered changes of emotional states, with the purpose of arousing the child and the analyst’s sensibility by unheard-of ways. If this behavior is capable of hushing up the interpretation of the analyst, inviting him to such an approximation of bodies, it also can take him to cogitate, later, in his theoretical speculations, on the psychic and its archaic marks, on the self-produced conformation of the sensitive body and on the aesthetic shapes of existence, subordinated by the activity of pleasure/displeasure and kept in regulation, rhythms, contacts and singular drawings. Will be this intriguing person in her autistic living here? These and other questions are treated throughout this work that considers this sensible dimension, that almost never admits translation, indispensable to the restitution of the tragic element from which we keep ourselves away, whose body is the privileged theater, making wavering our experience of human suffering.

Os pressupostos teórico-metodológicos que embasaram este estudo advém das articulações estabelecidas entre fundamentos da fonoaudiologia, da psicanálise lacaniana e winnicottiana e da linguística. Os objetivos do trabalho são: 1) refletir a clínica fonoaudiológica de crianças autistas a partir do olhar psicanalítico; 2) investigar e discutir as formas de abordagem e de intervenção clínica fonoaudiológica de crianças autistas; e 3) explorar a importância dos conceitos winnicottianos de placement e holding nas intervenções fonoaudiológicas, contemplando a clínica interdisciplinar. A dissertação abrange a área da linguística e evidencia o encontro da psicanálise com a fonoaudiologia, traçando o caminho realizado para o estudo do surgimento da linguagem da criança. Ao final da dissertação apresentamos vinhetas clínicas do atendimento fonoaudiológico de duas crianças com sintomas autísticos. A partir da análise dessas vinhetas, procuramos demonstrar a importância do profissional estar sintonizado com a criança, com a família e também com a equipe clínica. Nesta perspectiva, é possível acolher a criança e humanizar o ambiente, favorecendo as aquisições da comunicação. Por fim, buscamos demonstrar a importância da relação terapêutica fonoaudiológica para a criança autista, auxiliando em seu desenvolvimento da comunicação em seu meio social. ______________________________________________________________________________ ABSTRACT / This dissertation is focused on the theoretical and clinical analysis of speech therapy treatment for autistic children. The theoretical and methodological assumptions on which this study is based come from the connection between principles of Speech Therapy, Lacanian and Winnicottian Psychoanalysis and Linguistics. This study aims at: 1) reflecting upon the speech therapy practice for autistic children from a psychoanalytic point of view; 2) examining and discussing the approaches and clinical intervention on autistic children in the field of Speech Therapy; and 3) considering the importance of Winnicott’s concepts of placement and holding to therapeutic interventions, including the interdisciplinary clinical practice. This dissertation embraces the linguistic field and points out the connections between Psychoanalysis and Speech Therapy, describing how language development in children has been studied. Finally, the clinical vignettes of the speech treatment of two children with autistic symptoms are presented. Based on the analysis of these vignettes, the study intends to show how important it is that the speech therapist be closely bound to the child, the family and the whole clinical staff. On this perspective, it is possible to protect the child and humanize the environment, fostering language acquisition. To conclude, it shows how important the speech treatment for autistic children is, helping them establish communication in their social environment.

Nesta tese, refleti e demonstrei que existe uma diferença na formulação da noção de transferência quando realizamos um estudo comparativo desse conceito nas obras de Freud, de Klein e de Winnicott, e, ainda, que o que difere, nessas conceituações, auxilia a compreensão e o manejo da transferência nessa clínica. Verifiquei que a noção de transferência era insuficiente para compreender os fenômenos clínicos abarcados neste estudo e passei a utilizar o conceito de relação analítica, relacionando-o às noções de transferência, de contratransferência, de experiência e de espaço potencial. Posteriormente, pensei a relação analítica como espaço potencial a partir do qual é possível acolher as experiências dos pacientes em suas dimensões constitutivas, compreendendo o encontro entre analista e analisando como uma possibilidade de constituição psíquica e de retomada do desenvolvimento emocional. Assim, entendi a relação analítica como algo que transcende à reedição de vivências, proporcionando a instauração de experiências novas que favoreçam a constituição do sujeito e da realidade externa. Nesta tese, em vez de pensar na interpretação da relação analítica, coloquei a ênfase na própria relação. A partir da constatação de que a relação analítica com essas crianças não é algo dado, mas uma relação a ser construída, a tese do trabalho é a de que a relação analítica com essas crianças pode ser estabelecida por outros meios que não a interpretação, como o holding, o manejo, os vínculos sensoriais não verbais, o brincar e a imitação. O trabalho foi organizado em cinco capítulos, além da introdução e das considerações finais. O último capítulo contém reflexões clínicas e técnicas, além do relato e da discussão de quatro casos clínicos. Nesse capítulo, trago a imitação enquanto recurso técnico e outros aspectos técnicos como: o uso da contratransferência, a abertura e a disponibilidade emocional do analista, a encenação, a personificação, a reclamação, a linguagem diferenciada, a sustentação do enquadre e o uso da interpretação, dentre outros. ______________________________________________________________________________ ABSTRACT / The objective of this study is to perform a theoretical and clinical study, taking the transference concept, the work of Winnicott and the psychoanalysis with children in autistic and psychotic states, in order to understand some peculiarities of the transference and some guiding principles for this clinic. In this thesis, I reflected and demonstrated that there is a difference between the transference notion when we perform a comparative study of this concept in the works of Freud, Klein and Winnicott, and also that the distinctive traits of these conceptualizations help to understand and to manage the transference in this clinic. I realized that the transference notion was insufficient to understand the clinical phenomena covered in this study, so I started to use the concept of analytic relationship related to the notions of transference, counter transference, experience and potential space. Later, I thought the analytic relationship as a potential space, where the patient’s experiences are welcome in their constitutive dimensions, understanding the bond between analyst and patient as a possibility of psychic constitution and of resuming emotional development. So I understand the analytic relationship as something that transcends the reissue of experiences, providing the establishment of new experiences, encouraging the constitution of the self and of the external reality. In this thesis, instead of thinking in the interpretation of the analytic relationship, I put the emphasis on the relationship itself. From the fact that the analytic relationship with these children is not something given, but a relationship to be built, this paper's thesis is that the analytic relationship with these children may be established by other means besides the interpretation such as holding, handling, nonverbal sensory links, play and imitation. The work was organized into five chapters, plus the introduction and closing remarks. The final chapter contains clinical and technical reflections, and also the discussion of four clinical cases. In this chapter, I bring the imitation as a technical resource and other technical aspects such as: the use of counter transference, the analyst openness and emotional availability, the enactment, the personification, the reclamation, the distinctive language, the setting maintenance and the use of interpretation, among others.

A proposta central é pesquisar a função do ritmo na constituição psíquica a partir do paradigma do autismo. Há o relato de três casos clínicos. Para isso, procura-se realizar o trabalho de teorização flutuante. Os fundamentos teóricos são as concepções psicanalíticas sobre percepção, corpo, constituição psíquica e linguagem. Freud e Winnicott serão privilegiados. Ao discutir a função do ritmo nos fenômenos clínicos observados, apresentam-se situações que implicam principalmente o corpo e a relação transferencial. Esse sentir corporal e estético tornou-se um ponto central de interesse e inquietação e configurou-se a partir do trabalho psicanalítico com bebês, crianças, psicoses e autismos.O encontro terapêutico e as sensações contratrasnferenciais representam a motivação principal para a elaboração teórica. O caso clínico é tomado na forma de recortes das “presenças vivas” do sujeito e da terapeuta que se transformaram num conhecimento compartilhado. O relato de cada situação transferencial é singular e fornece um aspecto enriquecedor sobre a temática do ritmo. Por fim, é mostrado como a prática clínica e a análise mais detalhada do tratamento com a criança autista levam à elaboração do ritmo sensorial e do ritmo relacional. O questionamento sobre o autismo como paradigma do psíquico introduziu uma compreensão metapsicológica freudiana sobre o ritmo na constituição psíquica. É visto também que o ritmo tem papel importante no desenvolvimento da linguagem por estar presente desde as primeiríssimas formas de comunicação na criança. __________________________________________________________________________________________ ABSTRACT / This study draws on psychoanalytic theory and clinical practice to examine the concept of rhythm in the body, as evidenced via sensorial manifestations, body movement and language. The key objective is to study the function of rhythm in the makeup of the psyche according to autism’s paradigm. Three clinical cases are described and an attempt is made to work with free-floating theorization, based on theoretical underpinnings rooted in psychoanalytical concepts regarding perception, body, psychic make-up, and language. Freud and Winnicott inform the thinking found throughout the paper. The discussion on the function of rhythm in the clinical phenomena observed during the study is punctuated by situations that primarily involve the body and relational transference. This bodily and esthetic manner of feeling became the key focus of interest and concern, and arose from psychoanalytic work with babies, children and cases of psychosis and autism. The therapeutic encounter and counter-transferential sensations were the primary motivation for the study’s theoretical development. Clinical cases were approached via instances of the “living presence” of the subject and the therapist which then became shared knowledge. The description of each situation involving transfer is unique and thus enriches some aspect of our understanding of rhythm. Finally, the study shows how clinical practice and more detailed analyses of autistic children in treatment lead to the development of sensorial rhythm and relational rhythm. Questions regarding autism as a paradigm for the psyche introduce a meta-psychological Freudian understanding of rhythm in the makeup of the psyche. It was also seen that rhythm has an important role to play in the development of language because of its presence in even the very earliest forms of communication by children.

O tema dessa pesquisa é a função do corpo do analista no tratamento psicanalítico com a criança autista. A proposta central é pesquisar e discutir a função do corpo do analista, um corpo que favorece a criação de um ambiente holding e continente das angústias do paciente, pensando a relação do analista com a criança para além dos limites da interpretação. Para isso, adota-se a postura de “analista nãointérprete” (Tafuri, 2003), uma postura que implica “estar com” a criança à sua maneira, oferecendo uma escuta diferenciada. Nessa pesquisa, relata-se a evolução do tratamento de uma criança autista, dando ênfase à função de continência do analista a partir da relação sensorial corporal estabelecida entre ele e a criança. A relevância do corpo do analista no tratamento da criança autista remete às primeiras experiências sensoriais mãe-bebê, onde a mãe se adapta às necessidades do seu bebê, fornecendo o ambiente para que ele possa se constituir como sujeito. A relação entre o analista e a criança autista se baseia no paradigma da relação mãe-bebê, que será desenvolvida ao longo desse estudo. _________________________________________________________________________________________ ABSTRACT / The psychoanalytic clinic with self-engrossed children woke the interest and the necessity of investigating the sensory importance of the body of the analyst in the treatment with the autistic child. The subject of this inquiry is the function of the body of the analyst in the psychoanalytic treatment with the autistic child. The central proposal is to investigate and to discuss the function of the body of the analyst, a body that favors the creation of a holding environment and that can contain the anguishes of the patient, considering the relation between the analyst and the child beyond the limits of interpretation. For that, it is advisable to adopt the posture of a “non-interpreter analyst” (Tafuri, 2003), a posture that implies “being with” the child his/her way, offering him/her differentiated listening. In this inquiry, the evolution of the treatment of an autistic child is reported, giving emphasis to the containing function of the analyst from the sensory physical relation established between him/her and the child. The relevance of the body of the analyst in the treatment of the autistic child sends to the first mother-baby sensory experiences, where the mother adapts herself to the necessities of her baby, providing him/her with the environment so that he/she can become a person. The relation between the analyst and the autistic child is based on the paradigm of the relation mother-baby, which will be developed along this study.

A clínica psicanalítica com essas crianças reacende a fenomenologia do movimento, enigma a ser desvelado por meio da semântica do sensível. O corpo do analista, nessa circunstância clínica, torna-se ambiente favorável à reconstrução estética do psíquico e dos laços de humanização. Recuar a esse estado de coisas sugere uma certa anomia. A existência autística provoca o analista, a ponto de levá-lo a revisitar porções e possibilidades de seu corpo desconhecidas em sua geografia psíquica. Ao tempo em que o analista se volta para essas insígnias de seu corpo, sem o saber, ele se reposiciona, em condições de desfraldar gestos de semântica e terapêutica equiparáveis ao ambiente psíquico da criança. Com a apresentação de cenas clínicas, busca-se demonstrar esse modo estético de existência, no encontro analista criança. _______________________________________________________________________________________ ABSTRACT / The paper investigates the role of the analyst, taken into his body, awake in the encounter with the self-absorbed and speechless child, meeting considered as a sensitive trial of shapes in movement (Gestaltung). The psychoanalytical clinic with these children rekindles the phenomenology of the movement, puzzle to be unveiled by the semantics of the sensible. The body of the analyst, in this clinical circumstance, becomes favorable environment for the aesthetic reconstruction of the psychic and of the bonds of humanization. Receding this state of affairs suggests a certain anomie. The autistic existence provokes the analyst, taking him to revisit portions and possibilities of his own body, which are unknown in his psychic geography. By the time the analyst turns himself to those badges of his body, without knowing it, he repositions himself in conditions to unfurl gestures of semantic and therapeutic equivalent to the psychic environment of the child. With the presentation of clinical scenes, it is sought to demonstrate this aesthetic mode of existence, in the analyst-child encounter.

O presente estudo analisou os prontuários de 165 pacientes de um serviço público de saúde mental da infância e adolescência em Brasília e encontrou 15 pacientes com autismo. Durante o período analisado, pelo menos 53,3% destes pacientes foram medicados. As drogas mais utilizadas foram os antipsicóticos, seguidas por drogas serotoninérgicas, como os IRSs, indo ao encontro dos dados encontrados na literatura. Foi realizada também a avaliação psicológica de 4 crianças com autismo em outro serviço público, por meio dos testes psicométricos tradicionalmente utilizados. Os resultados são discutidos quanto à relevância do levantamento realizado, implicações da análise dos dados investigados e, ainda, quanto a aspectos da avaliação psicológica de crianças com autismo. _______________________________________________________________________________ ABSTRACT / The pharmacological intervention and the psychological evaluation in autism have always been a challenging task for the clinical practice and for those conducting research in the area. This study has analyzed 165 patient’s medical charters from a public service of child and adolescent mental health in Brasilia and found 15 patients with autism. In the analyzed period, at least 53.3% of these patients were on medication. The drugs used the most were antipsychotics, followed by serotoninergic drugs, like the SRIs, meeting the results found in the literature. A psychological assessment of 4 children with autism at another public service was also conducted with the traditionally used psychometric tests. The results were discussed in terms of the relevance of the survey, the implications of the analyzed data as well as in terms of some aspects of the psychological assessment of the children with autism.

Em primeiro lugar, realiza-se breve resgate do contexto histórico de surgimento do AT. Em seguida, delineia-se o caminho percorrido pelo AT desde que surgiu na América Latina até sua chegada ao Brasil. Discute-se a respeito das expressões utilizadas para nomear a prática e são traçados os pressupostos nos quais se baseiam o presente trabalho, refletindo-se sobre a função do AT a partir de conceitos criados por Winnicott. Posteriormente, introduz-se o tema do AT com crianças e são apresentadas três instituições uma estrangeira e duas brasileiras importantes por trabalharem com princípios adotados na prática que motivou este estudo: École Expérimentale de Bonneuil-sur-Marne, fundada na França, Pré-Escola Terapêutica Lugar de Vida, em São Paulo e Centro de Pesquisa em Psicanálise e Linguagem (CPPL), em Recife. Introduz-se a questão da Inclusão Escolar e apresenta-se o tema do AT com crianças nesse contexto. Há o relato de duas experiências de AT com crianças autistas. Por fim, reflete-se sobre o AT e a Inclusão Escolar a partir da articulação entre seus pressupostos e as experiências relatadas. Discute-se a importância de o at acompanhar a criança autista na escola com uma postura de estar-junto, de oferecer uma presença afetiva. Esse acompanhar acaba por se estender a toda a rede que envolve a criança, com a qual o at estabelece laços que serão fundamentais para desfazer alguns nós que atam o processo de inclusão escolar. __________________________________________________________________________________________ ABSTRACT / Experience with autistic children, mainly in Therapeutic Accompaniment (TA) activity, inspired this study, which has as purpose reflect on the TA function in the scholar inclusion of those children. Firstly, the TAs historical context of origin is carried out. After, the pathway through which TA has taken course from Latin America to Brasil is outlined. The dissertation discusses about the expressions used to designate this practice and the tenets on which this work is based are delineated, reflecting on the TA function built on Winnicott concepts. Later, the subject of TA with children is introduced and three institutions are presented one foreigner and two brazilians , importants for working with principles adopted in the practice that motivated this study: École Expérimentale de Bonneuil-sur-Marne, established in France, Pré-Escola Terapêutica Lugar de Vida, in São Paulo and Centro de Pesquisa em Psicanálise e Linguagem (CPPL), in Recife. The question of scholar inclusion is introduced and the subject of TA with children in this context is presented. Two TA experiences with autistic children are described. Finally, this study reflects on TA and scholar inclusion from the joint between its tenets and the told experiences. The importance of TA to accompany the autistic child at school with a position of being-together, offering an affective presence, is argued. This accompany ends being extended to all the net that involves the child, with which ta establishes bondings that will be crucial to undo some knots that tie the scholar inclusion process.

Este estudo foi desenvolvido em uma escola pública de Educação infantil do Distrito Federal (Brasil), em uma Classe Especial com seis alunos com transtorno do espectro autista (TEA) na faixa etária dos 4 aos 6 anos. Os dados foram construídos a partir de uma análise microgenética; se pautaram em observações registradas em diário de campo e videogravações, com transcrições das situações de faz de conta da criança com autismo. Destarte, problematizamos: o que caracteriza o brincar da criança com autismo? Num desdobramento, quais são os recursos simbólicos que ela utiliza para significar as ações lúdicas no uso dos brinquedos e assunção de papéis? Na análise e discussão dos dados, evidenciamos processos complexos de simbolização durante o faz de conta da criança com autismo, contrariando a literatura tradicional. Concluímos que a criança com autismo, de uma maneira qualitativamente distinta, brinca, cria, imagina e assume papéis. / This study analyzes play in children with autism from a cultural-historical perspective, whose main exponent is Vigotski. It focuses on the emerging symbolic resources of the make-believe play: pivot object use and configuration of roleplay. The research was developed in a public preschool in Distrito Federal, Brazil; more specifically, in a special needs classroom with six 4-6-year-old students with ASD (autism spectrum disorder). The data collected were analyzed with the microgenetic method, and consisted of field observations and video recordings with thorough transcripts of the autistic child make-believe situations. The questions I asked in this study were: what characterizes play in children with autism? What are the resources they use to symbolize the playful actions in the use of toys and role-taking? In the analysis and discussion section, we provide evidence of complex processes of symbolization during make-believe play in children with autism, which contradicts traditional literature. I conclude that the child with autism plays, creates, imagines and takes roles in a qualitatively different manner.
