Treatment is time consuming and costly, they can cause physical, social, economic and emotional problems. They are considered complex and difficult to heal, have a high number of relapses, and most of them are critically colonized or infected. Service to people with vascular ulcers is challenging to both health professionals and the public administration by virtue of its complexity, including the absence of public policies for this population, structuring services, access to care, professional qualification, and appropriate resources. GENERAL OBJECTIVE: To analyze the outcomes of healthcare for people with vascular ulcers treated in the examination rooms of the primary care system from the perspective of the evolution of healing over a period of six months. METHODOLOGY: Analytic longitudinal study with nested cross-sectional studies, in the examination rooms of the public health service of the city of Aparecida de Goiania, Goias, Brazil, from August 2012 to October 2013. Population consists of users receiving treatment by nurses, and professionals who provided nursing care in the study setting. Data collection occurred through structured nonparticipant observation, interview, physical examination, and use of Pressure Ulcer Scale for Healing (PUSH), planigraphy of ulcers, and photographic record. Absolute frequencies and percentages were used for data collection. The study complies with guidelines for research ethics, being approved under protocol 085/2012. RESULTS: The three rooms designed to care for people with vascular ulcers in the studied scenario showed non-washable walls, windows not screened, without adequate hand hygiene resources, without proper procedures for cleaning and disinfection of the environment, and lack of essential resources and inconsistent supply thereof. As for users, there was gender parity, prevalence among 60 years or older, and social and economic conditions in the lower middle class and below. The majority had lesions for more than one year, which were usually painful. Microbiological examination showed predominantly Gram-negative rods, including Pseudomonas aeruginosa. Among professionals, most were females, aged between 18 and 36 years and "reasonable experience" in treating people with vascular ulcers. CONCLUSION: Conditions were inadequate for serving people with vascular ulcers and contribute to care not compliant with current recommendations, which creates a discontinuity of care and diminished capacity to resolve these issues. Therefore, it is necessary to structure the service with respect to people with chronic ulcers, and maintain supplies and appropriate resources for care. / INTRODUÇÃO: As úlceras vasculares constituem um importante problema de saúde pública, afetam em torno de 1% da população em geral, sendo mais presentes entre os idosos. Seu tratamento é demorado e oneroso. Podem acarretar problemas físicos, sociais, econômicos e emocionais. Consideradas feridas complexas e de difícil cicatrização, apresentam elevado número de recidivas, e, a maioria delas é colonizada criticamente ou infectada. A assistência a pessoas com úlceras vasculares constitui um desafio aos profissionais de saúde e para o poder público, em virtude de sua complexidade, que abrange ausência de políticas públicas voltadas para essa população, estruturação dos serviços, acesso ao atendimento, qualificação profissional, insumos apropriados. OBJETIVO GERAL: Analisar a resolutividade da assistência à saúde a pessoas com úlceras vasculares atendidas em salas de curativo da rede de atenção primária, na perspectiva de evolução da cicatrização no período de seis meses. METODOLOGIA: Estudo analítico longitudinal, com estudos transversais nele aninhados, nas salas de curativo da rede municipal de saúde de Aparecida de Goiânia, Goiás, Brasil, no período de agosto de 2012 a outubro de 2013. A população consiste de usuários que receberam atendimento de enfermagem e profissionais que prestaram o atendimento de enfermagem no cenário de estudo. A coleta de dados se deu por meio da observação estruturada não participante, entrevista, exame físico, utilização da Pressure Ulcer Scale for Healing (PUSH), planigrafia das úlceras e registro fotográfico. Para análise de dados utilizaram-se frequências absolutas e percentuais. O estudo atende às diretrizes de ética em pesquisa, sendo aprovado sob protocolo 085/2012. RESULTADOS: As três salas destinadas ao atendimento de pessoas com úlceras vasculares no cenário estudado apresentaram paredes não laváveis, janelas não teladas, sem recursos adequados à higienização das mãos, sem padronização adequada de higienização e desinfecção do ambiente e desabastecidas de insumos indispensáveis e com suprimento descontinuado. Quanto aos usuários, houve paridade de gênero, predominância entre os de 60 anos ou mais e condições sociais e econômicas compatíveis com as classes C e D. A maioria das lesões possuía duração superior a um ano e geralmente dolorosas. Ao exame microbiológico predominaram os bastonetes Gram-negativos, entre os quais Pseudomonas aeruginosa. Quanto aos profissionais, a maioria era do gênero feminino, com faixa etária entre 18 e 36 anos e “razoável experiência” no atendido a pessoas com úlceras vasculares. CONCLUSÃO: O cenário estudado não apresenta condições adequadas para atendimento a pessoas com úlceras vasculares e corroboram uma prática assistencial discordante com as recomendações atuais, o que implica na descontinuidade do cuidado e diminuição da resolutividade da assistência à saúde. Diante disso, é necessário estruturar o serviço voltado para pessoas com úlceras crônicas, qualificação profissional e manter insumos e recursos apropriados para atendimento.

Quarenta e nove amostras EPEC típica (tEPEC) e atípica (aEPEC) pertencentes a diferentes sorotipos, isoladas de humanos e animais (cães, gatos, bovinos, ovinos, coelhos e sagüis) foram investigadas quanto ao perfil de virulência pela PCR e similaridade clonal por Multilocus Sequence Typing (MLST) e Pulsed-Field Gel Electrophoresis (PFGE). O objetivo deste estudo foi verificar se animais atuam como reservatório e fonte infecção de aEPEC para humanos. Os marcadores de virulência analisados revelaram que cepas aEPEC isoladas de animais possuem potencial para causar diarréia em humanos. As técnicas MLST e PFGE revelaram que amostras isoladas de animais e humanos compartilham relações clonais próximas ou idênticas. Estes resultados indicam que os animais estudados atuam como reservatório de aEPEC e representam fonte de infecção para humanos. Pelo fato de humanos, também atuarem como reservatório de aEPEC, ciclos de infecção cruzada animal-humano não podem ser descartados, pois a dinâmica de transmissão entre reservatórios de aEPEC não é muito bem compreendida. / Forty-nine typical and atypical EPEC strains belonging to different serotypes, isolated from humans, pets (cats and dogs), farm (bovines, sheep and rabbits) and wild animals (monkeys) were investigated for virulence markers and clonal similarity by pulsed-field gel electrophoresis (PFGE) and multilocus sequence typing (MLST). The virulence markers analyzed revealed that atypical EPEC strains isolated from animals have the potential to cause diarrhea in humans. Close clonal relationship between human and animal isolates was found with MLST and PFGE. These results indicate that these animals act as atypical EPEC reservoirs and may represent sources of infection for humans. Since humans also act as a reservoir of atypical EPEC strains, the cycle of mutual infection of atypical EPEC between animals and humans, mainly pets and their owners, cannot be ruled out, since the transmission dynamics between the reservoirs are not yet clearly understood.

A deficiência da proteína C5 do sistema complemento humano é rara com 38 casos relatados na literatura e freqüentemente associada a severas infecções provocadas por bactérias Neisseria. O objetivo do trabalho é caracterizar imunológica e geneticamente esta deficiência encontrada pela primeira vez em brasileiros. Por imunodifusão dupla obtivemos níveis expressivos de C3, C4, C6, C7, C8, C9, Fator B, Fator H e Fator I em todos os membros desta família, a proteína C5 não foi detectada no soro de três irmãos: II:9, II:4 e II:5. Por ELISA a concentração de C5 nestes indivíduos foi (0,9; 1,0; 1,3 µg/ml, 45- 190 µg/ml). Soros destes probandos não apresentam atividade hemolítica mediada pelo sistema complemento. O cDNA de C5 dos indivíduos I:1, I:2, II:4 e II:9 apresenta a deleção do éxon 30. Causada pela substituição de GAG4028 por GAA4028 no último nucleotídeo deste éxon que leva a um erro no splicing. Este defeito provavelmente produz uma proteína incompleta e destinada à degradação. / The deficiency of the C5 component of the complement system is rare with 38 described cases in the literature. This deficiency is frequently associated with severe infections, especially caused by Neisseria. Our objective is to characterize immunologically and genetically this deficiency, the first of its type described in the Brazilian population.We noted that C3, C4, C6, C7, C8, C9, Factor B, Factor H and Factor I have expressive levels in all the individuals sera of this family. C5 was absent in individuals II:4, II:5 and II:9. By ELISA a C5 concentration in this individuals were 0,9; 1,0; 1,3 µg/ml (normal: 45 - 190 µg/ml). Their serum doesn´t present hemolytic activity mediated by complement system. The C5 cDNA from individuals I:1, I:2, II:4 and II:9 has éxon 30 deleted. Caused by the substitution of GAG4028 for a GAA4028 in the last codon of exon 30. This defect was responsible for the deficiency of C5 in this family and this deletion would probably produce an unstable protein destined for degradation.

Os PAMs são mediadores multifuncionais da resposta imune inata, com atividade antibacteriana direta. O uso de PAMs como agentes terapêuticos tem algumas limitações, como a estabilidade, a toxicidade e alta massa molecular. Apesar dessas limitações, eles apresentam propriedades compensatórias, como imunomodulatória e antitumoral bem como a capacidade de inibir β-lactamases. O desenho racional de PAMs tem sido usado para gerar análogos com atividade melhorada. No presente trabalho avaliamos a atividade antibacteriana in vitro e in vivo da clavanina A e através da modificação incremental criamos dois análogos dos peptídeos mostoparano-L e clavanina A (clavanina-MO e mastoparanoMO),além disso, utilizamos o desenho racional de peptídeos para a criação de dois inibidores de β-lactamase (dBLIPs 1 e 2). A clavanina A mostrou se eficiente na eliminação de S. aureus em um modelo de infecção de ferida e impediu o início da sepse e, assim, reduziu a mortalidade de camundongos infectados em um modelo de infecção bacteriana sistêmica. A clavanina-MO e mastoparano-MO impediram o crescimento de bactérias planctônicas e levaram à erradicação de biofilmes bacterianos maduros. Os peptídeos modificados mostram-se promissores como agentes terapêuticos contra infecções bacterianas sistêmicas e biofilmes causadas por uma variedade de bactérias. dBLIP-1 e dBLIP-2 em combinação com antibióticos convencionais foram eficazes na eliminação de Escherichia coli e Staphylococcus aureus que expressam β-lactamases em um modelo murino de infecção sistêmica. dBLIPs 1 e 2 fornecem pistas para superar a resistência à base de β-lactamase. / With the increased incidence of multiple antibiotic resistant infections, there is huge interesting in antimicrobial peptides (AMPs) as templates to produce novel antibiotics. The AMPs are multifunctional mediators of innate immune response with direct antibacterial activities. Nevertheless, the use of AMPs as therapeutic agents has certain limitations such as stability, toxicity and high molecular mass. Despite such limitations, they show additional properties such as antitumor and immunomodulatory as well as the ability to inhibit β-lactamases. Furthermore, the rational AMPs design has been used to produce analogues with improved activity. In the present study, we utilized the rational AMPs design for generation of two β-lactamase inhibitors (dBLIPs 1 and 2) and through two incremental modification created analogues of clavanin A and mostoparan-L (clavanin-MO and mastoparan-MO respectively) peptides. Both inhibitors in combination with conventional antibiotics were effective for control of Staphylococcus aureus and Escherichia coli expressing β-lactamase in a murine model of systemic infection. dBLIPs 1 and 2 provide clues to overcome resistance to β-lactamase base. The clavanin-MO and mastoparan-MO prevented the growth of planktonic bacteria, leading to the mature biofilm eradication of pathogenic Gram-negative and -positive. The clavanina-MO and mastoparan-MO are promising therapeutic agents against systemic infections and bacterial biofilms caused by a wide bacterial variety.

DISCOVERING A NOVEL ANTIFUNGAL TARGET IN DOWNSTREAM STEROL BIOSYNTHESIS USING A SQUALENE SYNTHASE FUNCTIONAL MOTIF

The sterol biosynthetic pathway is essential for growth of all eukaryotic cells and the main target of antifungal agents. The emergence of resistance to these antifungals in an already ill patient population indicates a need to develop drugs that have a broad spectrum of activity among pathogenic fungi and have minimal patient toxicity. Squalene synthase is the first committed step in the sterol pathway and has been studied intensively for development of antifungal agents. While the overall architecture of this enzyme is identical throughout eukaryotes, it was shown that plant and animal genes cannot complement a squalene synthase knockout mutation in yeast unless the carboxy-terminal domain is swapped for one of fungal origin. This implies that there is a component of the fungal carboxy-terminal domain that is responsible for the complementation phenotype and that is unique to the fungal kingdom of life. To determine the role of the carboxy-terminal domain of squalene synthase in the sterol pathway, we used the yeast Saccharomyces cerevisiae with a squalene synthase knockout mutation and expressed squalene synthases originating from fungi, plants, and animals. In contrast to previous observations, all enzymes tested could partially complement the knockout mutation when the genes were weakly expressed. When induced, non-fungal squalene synthases could not complement the knockout mutation and instead led to the accumulation of carboxysterol intermediates, suggesting an interaction between squalene synthase and the downstream sterol C4-decarboxylase. Overexpression of a sterol C4-decarboxylase from any kingdom of life both decreased the accumulation of carboxysterol intermediates and allowed non-fungal squalene synthases to complement the squalene synthase knockout mutation. Using chimeric squalene synthases from each kingdom of life, the motif in the C-terminal domain responsible for preventing this toxicity was mapped to a kingdom-specific 26-amino acid hinge motif adjacent to the catalytic domain. Furthermore, over-expression of the carboxy-terminal domain alone containing a hinge motif from fungi, not from animals or plants, led to growth inhibition of wild-type yeast. Since this hinge region is unique to and highly conserved within each kingdom of life, this data provides evidence for the development of an antifungal therapeutic as well as for tools to develop an understanding of triterpene catalytic activity and identify similar motifs in other biosynthetic pathways.

Prevalence and risk factors for Helicobacter pylori transmission in the Eastern Cape Province application of immunological molecular and demographic methods

Helicobacter pylori (H. pylori) is a microaerophilic, Gram-negative motile curved rod that inhabits the gastric mucosa of the human stomach. The organism chronically infects billions of people worldwide and is one of the most genetically diverse of bacterial species. Infection with the organism potentially induces chronic gastritis and peptic ulcer disease. In addition, H. pylori plays a role in the etiology of gastric cancer and gastric MALT lymphoma. The risk of infection is increased in those living in the developing world, which has been ascribed to precarious hygiene standards, crowded households, and deficient sanitation common in this part of the world. Thus, the aim of this study was to identify the risk factors in the transmission of H. pylori in our environment, i.e. in Nkonkobe Municipality in the Eastern Cape Province, South Africa. Faecal samples were collected from 356 apparently healthy subjects, consisting of 168 males and 188 females aged from 3 months to 60 years (Mean = 31 years). A standardized questionnaire was applied, it described demographic characteristics including age, sex, household hygiene, socioeconomic status, area of residence, duration of stay in the area, sharing bath water, sharing tooth brush, habit of sucking thumb, medication currently being taken or medication taken within the past three months, source of water, type of toilet used, education and occupation. A sandwich-type enzyme immunoassay amplification technology (Amplified IDEIA TM Hp StAR TM , Oxoid, UK) was used to analyze the faecal samples for the detection of H. pylori antigens using monoclonal antibodies specific for H. pylori antigens. To assess the possibility of faecal oral route with tap water as an intermediary link, PCR targeting the ureC (glmM), a highly conserved gene in H. pylori ii was carried out to detect H. pylori DNA in faecal samples of already positive samples by HpSA test as well as in direct tap water used by the H. pylori positive subjects. QIAamp DNA stool mini kit was used to extract DNA from faecal samples. Tap water samples were then obtained using sterile bottles from areas inhabited by H. pylori positive subjects as determined by HpSA test and PCR. DNA extraction from water samples was done using UltraCleanTM Water DNA Isolation Kit (0.22μm) according to the manufacturer’s instructions. PCR with primers specific for H. pylori glmM gene was carried out with both positive and negative controls incorporated. Fisher’s exact test was used to assess the univariate association between H. pylori infection and the possible risk factors. Odds ratio (OR) and the corresponding 95 percent confidence interval (CI) were calculated to measure the strength of association using EPI INFO 3.41 package. P values of < .05 were required for significance. The precision rate of the diagnostic tests used was also determined. H. pylori antigen was detected in 316 of the 356 subjects giving an overall prevalence of 88.8 percent. Prevalence increased with age from 75.9 percent in children < 12 years age to 100 percent in the age group from 13 years to 24 years, also 100 percent prevalence of H. pylori was recorded in young adults aged 25-47 years and subjects aged 60 years (P < .05). H. pylori prevalence was higher in females than in males. Of 188 females who participated in the study, H. pylori antigen was detected in 172 (91.5 percent) versus 144 (85.7 percent) of 168 males (P > .05). Interestingly, H pylori antigen was detected more often (100 percent) in the high socioeconomic group than in those of low socioeconomic group (85.9 percent). Sixteen (66.7 percent) of twenty four faecal samples that had previously tested positive for the organism by HpSA test were confirmed positive by PCR. However none of the treated tap water samples tested positive for the organism by PCR. The present iii study revealed a high prevalence of H. pylori in faecal samples of asymptomatic individuals in the Nkonkobe Municipality, an indication of active infection. The obtained results also revealed that direct treated tap water might not be playing a crucial role in the oral transmission of H. pylori in the studied population.

As espécies Klebsiella pneumoniae, Escherichia coli e Pseudomonas aeruginosa são responsáveis por infecções neonatais hospitalares. Lipopolissacarídeo (LPS) é o principal indutor de respostas inflamatórias. Os objetivos foram avaliar a transferência placentária de IgG reativa ao LPS de K. pneumoniae, E. coli O111, O26 e O6 e P. aeruginosa empregando ELISA para dosar IgG em soro materno e de cordão de 29 neonatos pré-termos e 32 a termo; analisar IgM total e específica no soro materno; e investigar a influência das patologias apresentadas pelas mães na transferência placentária. Concentrações de IgG total foram reduzidas em pré-termos como esperado, porem índices de transferência placentária de IgG total e IgG anti-LPS foram sistematicamente reduzidos quando comparados aos neonatos a termo. Níveis de IgM total e anti-LPS foram equivalentes em mães de ambos os grupos. As patologias das mães influenciaram os níveis de IgM no grupo de mães de pré-termos. Estes resultados indicam uma imunidade adquirida deficiente pelo grupo pré-termo aumentando os riscos de infecção. / Klebsiella pneumoniae, Escherichia coli and Pseudomonas aeruginosa species are responsible for neonatal nosocomial infections. Bacterial lipopolysaccharide (LPS) is the major inducer of the inflammatory responses. The aims were to evaluate the placental transfer of IgG reactive to LPS present in K. pneumoniae, in E. coli O111, O26 and O6 and in P. aeruginosa employing ELISA to detect IgG in maternal and cord sera from 29 preterm and 32 term neonates; to analyze total and specific IgM on the mothers sera; and to investigate the influence of the pathologies presented by some mothers in the placental transfer. Total IgG concentrations were reduced in preterm neonates as expected, but placental transfer indexes of total and anti-LPS IgG were systematically reduced when compared with term neonates. Total and anti-LPS IgM levels were equivalent on mothers of both groups. The mothers pathologies influenced only the IgM levels in the preterm mothers group. These results indicate a deficient acquired immunity by the preterm group increasing the risk of infection.

A leptospirose é uma zoonose causada por bactérias do gênero Leptospira. A patogênese da doença em humanos é observada principalmente no pulmão, fígado e rins. Neste trabalho, foi avaliado o papel da resposta imune inata na proteção contra a leptospirose usando camundongos como modelo experimental. Os animais foram infectados com L. interrogans e o desenvolvimento da doença foi acompanhado, observando-se a morte de animais C3H/HeJ, enquanto C3H/HePas apresentou icterícia e BALB/c não apresentou sintomas. O perfil de mRNA foi medido por qPCR nas amostras de rim, fígado e pulmão e as concentrações de proteinas TNF-<font face=\"Symbol\">&#945;, TGF-<font face=\"Symbol\">b, MCP-1, MIP-1<font face=\"Symbol\">&#945;, MIP-2 e IL-8 foram analisadas por ELISA em extratos dos tecidos e no soro. Os resultados demonstraram que L. interrogans estimula a expressão prematura de TNF-<font face=\"Symbol\">&#945;, TGF-<font face=\"Symbol\">b, MCP-1, MIP-1<font face=\"Symbol\">&#945;, MIP-2 e IL-8 na linhagem BALB/c resistente à infecção. A análise histológica indica que estes mediadores podem estar relacionados com o influxo de diferentes células do sistema imune desempenhando importantes funções na proteção contra leptospirose. / Leptospirosis is a worldwide zoonosis caused by Leptospira. The pathogenesis in humans is mainly observed in lungs, livers and kidneys. In this work the role of innate immune response in protection against leptospirosis is being studied using different mice models. The animals were infected intraperitoneally with virulent cells of L. interrogans serovar Copenhageni and the development of the disease was followed, being observed mortality of C3H/HeJ mice, whereas C3H/HePas presented jaundice and BALB/c mice remained asymptomatic. Samples of liver, kidney, lungs and sera were analyzed following the profiles of mRNA and protein of the cytokines TNF-<font face=\"Symbol\">&#945; and TGF-<font face=\"Symbol\">b and chemokine MCP-1, MIP-1<font face=\"Symbol\">&#945;, MIP-2 and CXCL1/IL-8. We showed that Leptospira infection stimulates early expression of cytokine TNF-<font face=\"Symbol\">&#945; and TGF-<font face=\"Symbol\">b and chemokine MCP-1, MIP-1<font face=\"Symbol\">&#945;, MIP-2 and IL-8 in the resistant mice strain BALB/c. Histological analysis indicates that the expression of those molecules can be related to the influx of distinct immune cells, which play a role in the naturally acquired protective immunity.

Escherichia coli enteropatogênica atípica (EPECa) é causadora de diarréia infantil e apresenta alta heterogeneidade quanto aos fatores de virulência. O objetivo deste trabalho foi estudar o comportamento de EPECa na interação com fagócitos profissionais. Duas amostras de EPECa sorotipo O55:H7 mostraram-se capazes de reduzir a fagocitose. Os sobrenadantes dos cultivos foram submetidos a SPE e HPLC e as frações com efeito antifagocítico foram submetidas a espectrometria de massas. A fração capaz de reduzir a fagocitose de bactérias reduziu também a fagocitose de Saccharomyces cerevisiae. Além de mostrar que EPECa é capaz de induzir a antifagocitose, mostrou-se também que o fator antifagocitico é secretado, solúvel em meio aquoso, termoestável, apresenta baixo peso molecular, não é microbicida ou citotóxico e, por último, há indicativos de que possa apresentar uma região glicosídica. Estes achados sugerem que o fator antifagocítico pode, embora não sozinho, exercer um papel importante na adaptabilidade e patogenicidade das EPECa. / Atypical enteropathogenic Escherichia coli (aEPEC) causes diarrhea mainly in children and presents a high heterogeneity of virulence factors. The objective of this work was to study the behavior of aEPEC regarding its interaction with professional phagocytes. Two samples of aEPEC serotype O55:H7 were able to reduce phagocytosis, The culture supernatants were submitted to SPE and HPLC and the active fractions were tested and analyzed by mass spectrometry. The results show that the fraction with bacterial antiphagocytic activity also reduces phagocytosis of Saccharomyces cerevisiae. In addition to demonstrating that aEPEC can induce antiphagocytosis, this work shows that it is due to a secreted antiphagocytic factor that is soluble in aqueous medium, is thermo-stable, has a low molecular weight, is not bactericide or cytotoxic and, finally, possibly presents a glycosidic region. These findings suggest that the antiphagocytic factor may, though maybe not alone, play an important role in the adaptability and pathogenicity of aEPEC.

Using the polymerase chain reaction to determine the prevalence of Lyme Disease bacteria, Borrelia burgdorferi, in ixodes pacificus ticks from San Bernardino County in Southern California

The purpose of this study was to determine the prevalence of Lyme Disease (LD) bacteria in adult Ixodes pacificus ticks collected from the mountains of San Bernardino County in Southern California. Seven hundred fifty four I. pacificus adults were collected from the Pacific Crest Trail and adjacent areas. The Polymerase Chain Reaction (PCR) was used to screen ticks for Borrelia burgdorferi infection by targeting two different DNA loci. Oligonucleotide primers targeting both the ospA and fla genes were used in the assay. Ticks were processed in pools of three, and genomic DNA from the ticks was extracted with a commercial mini-kit utilizing silica matrix spin-columns. All ticks tested negative for B. burgdorferi infection regardless of primer pair used. In addition, ticks were negative following examination by dark-field microscopy. This study confirms previous reports that the prevalence of LD in Southern California is quite low.
