Comparação entre os resultados da expressão gênica da desmina, alfa-actina e TGF-beta1 obtidos a partir dos métodos da reação em cadeia de polimerase via transcriptase reversa (RT-PCR) semiquantitativa e em tempo real (qRT-PCR) no modelo / Comparison between the results of gene expression of desmin, alpha-actin and TGF-beta1 obtained from the methods of reverse transcriptase polymerase chain reaction (RT-PCR) semiquantitative and real-time (qRT-PCR) in model of selective bile ducts ligation in growing animals

Os mecanismos responsáveis pela fibrose hepática na infância são pouco conhecidos. Crianças com atresia das vias biliares, quando submetidas a portoenterostomia a Kasai com sucesso, se tornam anictéricas mas mesmo assim desenvolvem cirrose a longo prazo. Da mesma forma, a ocorrência de estenoses biliares segmentares intra-hepáticas no pós-operatório de transplante hepático podem levar ao desenvolvimento de cirrose em todo o órgão. Tais fatos sugerem que mecanismos endócrinos ou parácrinos estejam envolvidos na fibrogênese hepática. Para elucidar este processo o modelo de ligadura seletiva das vias biliares em ratos jovens foi desenvolvido em nosso laboratório. Usando este modelo, identificamos mudanças na expressão do gene da alfa-actina de músculo liso, tanto no parênquima hepático obstruído como no parênquima hepático adjacente à obstrução. No entanto, o perfil de expressão gênica da desmina, uma proteína presente em níveis elevados durante a ativação das células estreladas hepáticas e o TGF-beta1, principal citocina pró-fibrogênica, não demostraram diferenças significantes quando analisados pelo método do RT-PCR semiquantitativo. Assim, os mecanismos moleculares envolvidos na modulação da fibrogênese hepática nesse modelo experimental não estão totalmente compreendidos. A metodologia do qRT-PCR (PCR em tempo real), têm sido previamente descrita como um método mais preciso e sensível, possibilitando a detecção do aumento do número de cópias do gene à medida que ocorre a amplificação, enquanto que o método do RT-PCR semiquantitativo a análise dos transcritos só é realizada após a etapa da amplificação. O objetivo deste estudo foi avaliar as alterações moleculares no modelo experimental de ligadura seletiva das vias biliares e comparar os resultados obtidos entre os métodos do RT-PCR semiquantitativo e do qRT-PCR em tempo real. Foi realizada a ligadura seletiva do ducto biliar em ratos Wistar com 21 dias de vida, os grupos foram separados de acordo com o momento da morte: 7 ou 60 dias após a cirurgia. A expressão da desmina, alfa actina de músculo liso e TGF-beta1 foi avaliada no tecido do parênquima hepático com obstrução biliar (ducto ligado - DL) e no parênquima hepático adjacente à obstrução biliar (ducto não ligado - DNL) usando o RT-PCR semiquantitativo e o qRT-PCR em tempo real. A metodologia do qRT-PCR em tempo real permitiu identificar mudanças no perfil de expressão gênica que não foram demonstrados pelo método semiquantitativo. O parênquima DL mostrou reação fibrogênica mais intensa, com aumento na expressão da alfa actina de músculo liso e TGF-beta1 após 7 dias. O parênquima DNL apresentou resposta fibrogênica tardia, com aumento da expressão da desmina 7 dias e 60 dias após a cirurgia, além de aumento da alfa-actina de músculo liso 60 dias após a cirurgia. O qRT-PCR em tempo real apresentou maior sensibilidade para identificar mudanças no perfil de expressão gênica em relação ao método convencional do RT-PCR semiquantitativo. Nossos resultados ajudam a esclarecer a dinâmica das alterações moleculares envolvidas na modulação da fibrogênese hepática no modelo experimental de ligadura seletiva do ducto biliar e pode ser diretamente aplicado para o estudo de estenoses biliares intra-hepáticas e atresia das vias biliares / The mechanisms responsible for liver fibrosis in childhood are poorly understood. Children suffering from biliary atresia, when submitted to the successful Kasai portoentostomy, become anicteric, but nonetheless develop cirrhosis in the long term. Similarly, the occurrence of intrahepatic biliary stenosis in the postoperative period of liver transplantation may lead to cirrhosis of the whole organ. Such facts suggest that endocrine or paracrine mechanisms are involved in hepatic fibrogenesis. To elucidate this process, the selective bile duct ligation model in young rats was developed in our laboratory. Using this model, we identified changes in the expression of smooth muscle alpha-actin both in the obstructed parenchyma and the hepatic parenchyma adjacent to the obstruction. However, the expression profiles of desmin, a protein present at high levels during activation of hepatic stellate cells and TGF-beta1, the main pro-fibrogenic cytokine, were unchanged when analyzed with semiquantitative RT-PCR. Thus, the molecular mechanisms involved in the modulation of hepatic fibrogenesis in this experimental model are not fully understood. The methodology of qRT-PCR (real time PCR) has previously been described as a more precise and sensitive method, allowing the detection of increased copy number of the gene while amplification occurs, whereas by semiquantitative RT-PCR analysis transcripts is only perfomed after the amplification step. This study aimed to evaluate the molecular changes in experimental model of selective bile duct ligation and compare the results between semiquantitative RT-PCR and real-time qRT-PCR methods. Selective biliary duct ligation was performed on Wistar rats with 21 days of life, the groups were separated according to the moment of death: 7 or 60 days after surgery. The expression of desmin, alpha-actin smooth muscle and TGF-beta1 was examined in tissue from hepatic parenchyma with biliary obstruction (duct ligation - DL) and in the adjacent hepatic parenchyma (duct non-ligated - DNL) using semiquantitative RT-PCR and real-time qRT-PCR. The methodology of the real-time qRT-PCR allowed to identify changes in gene expression profile that were not shown by semiquantitative method. The DL parenchyma showed a more severe fibrogenic reaction, with increased alpha-actin smooth muscle and TGF-beta1 expression after 7 days. The DNL parenchyma presented a later fibrotic response, with increased desmin expression 7 and 60 days after surgery, besides of increased alpha-actin smooth muscle 60 days after surgery. Real-time qRT-PCR was more sensitive to identify changes in gene expression profile comparated to the semiquantitative method. Our results help to clarify the dynamic of molecular changes involved in the modulation of hepatic fibrogenesis in an experimental model of selective bile duct ligation and can be directly applied to the study of intrahepatic biliary stenosis and biliary atresia

Nuclear Factor (Erythroid 2-like) Factor 2 (Nrf2) as Cellular Protector in Bile Acid and Retinoid Toxicities

Exposure to toxic bile acids (BA) and retinoic acids (RA) is implicated in toxicities related to excessive oxidative stress. This thesis examined roles and mechanisms of the oxidative stress-responsive nuclear factor (erythroid 2-like) factor 2 (Nrf2) in adaptive cell defense against BA and RA toxicities. Using liver cells and mouse models, many antioxidant proteins known to be Nrf2 target genes, particularly the rate-limiting enzyme for glutathione (GSH) biosynthesis, i.e., glutamate-cysteine ligase subunits (GCLM/GCLC), were induced by BA [lithocholic acid (LCA)] or RA (all-trans, 9-cis and 13-cis) treatment. Evidence for increased Nrf2 transactivation by LCA and all-trans-RA was exemplified in HepG2 by: (1) reduced constitutive and inducible expression of GCLM/GCLC upon Nrf2 silencing via small-interfering RNA; (2) increased inducible expression of GCLM/GCLC genes by Nrf2 overexpression, but overexpression of dominant-negative Nrf2 decreased it; (3) increased nuclear accumulation of Nrf2 as signature event of receptor activation; (4) enhanced Nrf2-dependent antioxidant-response-element (ARE) reporter activity as indicative of increased Nrf2 transactivation; and (5) increased Nrf2 occupancy to AREs of GCLM and GCLC. Additionally, in BA-treated HepG2 cells, we observed concomitant increases of many ATP-binding cassette (ABC) transporters (MRPs 1-5, MDR1 and BCRP) in parallel with increased cellular efflux. Nrf2 silencing in HepG2 cells decreased constitutive and inducible expression of MRP2, MRP3 and ABCG2. However, Nrf2-silenced mouse hepatoma cells, Hepa1c1c7, and Nrf2-/- mice had decreased constitutive and/or inducible expression of Mrps 1-4, suggesting species differences in Nrf2-dependent regulation of hepatic ABC transporters. Protection by Nrf2 against BA and RA toxicities was confirmed by observations that Nrf2 silencing increased cell susceptibility to BA- and RA-induced cell death. Moreover, Nrf2-/- mice suffered more severe liver injury than the wildtype. Increased GSH and efflux activity following increased GCLM/GCLC and ABC transporters, respectively, can mitigate LCA toxicity. Activation of MEK1-ERK1/2 MAPK was shown to primarily mediate Nrf2 transactivation and LCA-induced expression of antioxidant proteins and Nrf2-dependent and -independent ABC transporters. In conclusion, Nrf2 activation by BA and RA led to coordinated induction of antioxidant and ABC proteins, thereby counteracting resultant oxidative cytotoxicity. The potential of targeting Nrf2 in management of BA and RA toxicities merits further investigation.

Biological Roles of the Vitamin D Receptor in the Regulation of Transporters and Enzymes on Drug Disposition, Including Cytochrome P450 (CYP7A1) on Cholesterol Metabolism

Nuclear receptors play significant roles in the regulation of transporters and enzymes to balance the level of endogenous molecules and to protect the body from foreign molecules. The vitamin D receptor (VDR) and its natural ligand, 1alpha,25-dihydroxyvitamin D3 [1,25(OH)2D3], was shown to upregulate rat ileal apical sodium dependent bile acid transporter (Asbt) to increase the reclamation of bile acids, ligands of the farnesoid X receptor (FXR). FXR is considered to be an important, negative regulator of the cholesterol metabolizing enzyme, Cyp7a1, which metabolizes cholesterol to bile acids in the liver. In rats, decreased Cyp7a1 and increased P-glycoprotein/multidrug resistance protein 1 (P-gp/Mdr1) expressions pursuant to 1,25(OH)2D3 treatment was viewed as FXR effects in which hepatic VDR protein is poorly expressed. In contrast, changes in rat intestinal and renal transporters such as multidrug resistance associated proteins (Mrp2, Mrp3, and Mrp4), Asbt, and P-gp after administration of 1,25(OH)2D3 were attributed directly as VDR effects due to higher VDR levels expressed in these tissues. Higher VDR expressions were found among mouse hepatocytes compared to those in rats. Hence, fxr(-/-) and fxr(+/+) mouse models were used to discriminate between VDR vs. FXR effects in murine livers. Hepatic Cyp7a1 in mice was found to be upregulated with 1,25(OH)2D3 treatment, via the derepression of the short heterodimer partner (SHP). Putative VDREs, identified in mouse and human SHP promoters, were responsible for the inhibitory effect on SHP. The increase in hepatic Cyp7a1 expression and decreased plasma and liver cholesterol were observed in mice prefed with a Western diet. A strong correlation was found between tissue Cyp7a1 and P-gp changes and 1,25(OH)2D3 plasma and tissue concentrations, confirming that VDR plays an important role in the disposition of xenobiotics and cholesterol metabolism. Moreover, renal and brain Mdr1a/P-gp were found to be directly upregulated by the VDR in mice, and concomitantly, increased renal and brain secretion of digoxin, a P-gp substrate, in vivo. The important observations: the cholesterol lowering and increased brain P-gp efflux activity properties suggest that VDR is a therapeutic target for treatment of hypercholesterolemia and Alzheimer’s diseases, since beta amyloid, precursors of plague, are P-gp substrates.

Le rôle de la barrière hémato-encéphalique dans la pathogénèse de l'oedème chez des rats souffrant d'insuffisance hépatique chronique

L’œdème cérébral est une complication associée à l’encéphalopathie hépatique (EH) lors d’une insuffisance hépatique chronique (cirrhose du foie). Présentement, l’origine de sa pathogenèse, vasogénique (rupture de la barrière hémato-encéphalique (BHE)) ou cytotoxique (prise anormale d’ions), n’a pas encore été déterminée. Il a été démontré que le co-transporteur Na-K-Cl (NKCC1) du côté luminal des microvaisseaux sanguins cérébraux (CMV) joue un rôle dans le développement de l’œdème cérébral dans des modèles d’ischémie où la bumetanide, un inhibiteur de NKCC, atténue l’œdème cérébral. Deux modèles d’EH ont été utilisés pour cette étude i) la ligature de la voie biliaire (BDL) qui présente l’hyperammoniémie chronique, l’œdème cérébral et le stress oxydatif systémique ; ii) l’anastomose portocave (PCA) qui présente de l’hyperammoniémie chronique seulement. Les buts du projet étaient de: i) définir l’origine du développement de l’œdème chez les rats BDL en étudiant l’extravasation de macromolécules, les jonctions serrées et l’activation des métalloprotéinases matricielles de la BHE; ii) observer les effets de l’hyperammoniémie chronique indépendamment sur la BHE chez les rats PCA; iii) évaluer le rôle de l’hyperammoniémie et du stress oxydatif et iv) étudier le rôle du NKCC1 dans les CMV dans la pathogenèse de l’œdème cérébral. Les résultats du projet démontrent que l’œdème est d’origine cytotoxique chez les rats BDL et que l’intégrité de la BHE est conservée chez les rats PCA malgré l’hyperammoniémie. L’expression génique du NKCC1 est associée à l’œdème mais pas son expression protéique et sa phosphorylation. Enfin, l’étude démontre que l’hyperammoniémie et le stress oxydatif indépendant ne jouent pas un rôle dans la pathogenèse de l’œdème mais suggère qu’ils y aient un effet synergique. / Brain edema is a complication associated with hepatic encephalopathy (HE) due to chronic liver failure (cirrhosis). It is unclear whether brain edema is of vasogenic (blood brain barrier (BBB) breakdown) or cytotoxic (abnormal cellular uptake of ions) origin. It has been demonstrated that the Na-K-Cl cotransporter (NKCC1) located on the luminal side of the cerebral microvessels (CMV) is implicated in the pathogenesis of brain edema in animal models of ischemia and that the administration of bumetanide, an inhibitor of NKCC, attenuates brain water increase. Two distinct animal models of chronic liver failure and HE are used in the present study; 1) bile duct ligation (BDL) where brain edema, chronic hyperammonemia and systemic oxidative stress are observed; 2) portacaval anastomosis (PCA) where only chronic hyperammonemia is observed. The aims of the study were to: i) determine the origin of brain edema in BDL rats measuring brain extravasation, tight junctions expression and matrix metalloproteinase activation; ii) observe the effects of chronic hyperammonemia on the BBB in PCA rats; iii) study the role of oxidative stress and hyperammonemia; iv) evaluate the role of NKCC in CMV in the pathogenesis of brain edema. The results of the study determined that brain edema in BDL rats is of cytotoxic origin and chronic hyperammonemia independently has no effect on the BBB. An increase of NKCC1 mRNA is associated with brain edema but protein expression and phosphorylation are not. Furthermore, hyperammonemia and oxidative stress independently are not implicated in the development of brain edema however a synergistic effect between the two pathogenic factors in BDL rats remains a possibility.

In vitro-Versuche mit dem Polo like-Kinase 1 Hemmstoff BI 2536 an Zelllinien von Gallenwegskarzinomen

Karzinome der Gallenwege sind mit einer schlechten Prognose assoziiert. Eine potentiell kurative chirurgische Resektion ist bei der Mehrzahl der Patienten aufgrund des späten Zeitpunkts der Diagnosestellung nicht möglich, so dass derzeit vorrangig palliative Therapieansätze Anwendung finden. Das nur geringe Ansprechen auf konventionelle Radio- oder Zytostatikatherapie begründet die Notwendigkeit neuer Therapieansätze. Einen möglichen Angriffspunkt stellt hierbei die Polo-like-Kinase 1 (Plk1) dar, da ihre zentrale Rolle in der Regulation des Zellzyklus zunehmend erkannt und eine vermehrte Expression in malignem Tumorgewebe verglichen mit gesundem Gewebe nachgewiesen wurde. Das Dihydropteridinon BI 2536 ist ein poten-ter, niedermolekularer und selektiver Hemmstoff der Plk1 und sollte daher auf seine Wirksamkeit an Gallenwegskarzinomen untersucht werden. In der vorliegenden Arbeit konnte gezeigt werden, dass BI 2536 die untersuchten 14 Zelllinien von Gallenblasen- und Gallengangskarzinomen wirkungsvoll hemmt. Das Ansprechen unterschied sich zwischen den Zelllinien und ordnet sich vergleichbar zu Veröffentlichungen an anderen malignen Tumoren ein. Die Expression von Plk1 und dessen assoziierten Transkriptionsfaktor FoxM1 konnte bei Westernblot-Versuchen bei allen Zelllinien nachgewiesen werden, was eine Bedeutung in der Onkogenese vermuten lässt. Die Behandlung mit BI 2536 beeinflusste jedoch die Proteinmenge beider nicht. An für die folgenden Versuche ausgewählten drei Zelllinien zeigten sich in der reversen Transkription mit anschließender Echtzeit-Polymerase-Kettenreaktion (qRT PCR) ähnliche Ergebnisse in Bezug auf die exprimierte mRNA von Plk1. Westernblot-Analysen ermittelten keine signifikanten Veränderungen der an wichtigen intrazellulären Kaskaden beteiligten Proteine p42/44 und Akt sowie deren phosphorylierten Formen. Obwohl die Proteinmenge des Mitosemarkers Phospho-Histon H3 ebenso unverändert blieb, führte die Behandlung mit BI 2536 – dies zeigen Ergebnisse der Durchflusszytometrie – zu einer signifikanten, dosisabhängigen Zunahme der G2/M Fraktion des Zellzyklus und Zunahme der Apoptose-rate. Der maximale Hemmeffekt in der Behandlung von BI 2536 lag bei einer Inkubations-dauer von vier Tagen. Die Empfehlungen aus den klinischen Studien der Phase II von BI 2536 sowie dem Ziel der Vermeidung von Resistenzen ergibt sich die Notwendigkeit von Kombinationsversuchen mit Zytostatika, die in einer anderen Phase des Zellzyklus angreifen. Die in der Behandlung von Gallenwegskarzinomen etablierten Antimetaboliten 5-Fluorouracil und Gemcitabin wurden hierzu ausgewählt und es zeigten sich für 5 Fluorouracil synergistische, für Gemcitabin hingegen additive Kombinationseffekte. Zusätzlich wurde die Wechselwirkung mit dem IGF 1-Rezeptor-Inhibitor NVP-AEW541 untersucht, der ebenfalls einen neuen Behand-lungsansatz in der Krebstherapie darstellt und bei Gallenwegskarzinomen in vitro wirksam ist. Auch hier zeigen sich synergistische Effekte, die jedoch erst in höheren Behandlungs-dosen auftraten. Die Ergebnisse dieser Arbeit zeigen, dass die Hemmung der Plk1 bei Gallenblasen- und Gallengangskarzinomen einen wirksamen Behandlungsansatz darstellt. Auf der Grundlage der in dieser Arbeit beschriebenen Ergebnisse wird eine weitere präklinische und klinische Testung von selektiven Plk1-Hemmstoffen wie BI 2536 an Gallenwegskarzinomen empfohlen.

Tratamento endoscópico das estenoses biliares pós-transplante  hepático: revisão sistemática da literatura e metanálise / Endoscopic treatment of post-liver transplantation biliary strictures: systematic literature review and meta-analysis

As complicações biliares mais comuns pós-transplante hepático são as estenoses da anastomose, as estenoses não-anastomóticas e as fístulas biliares e podem ocorrer de diferentes modos, de forma isolada ou associada. A origem do enxerto (doador cadáver ou doador vivo) tem influência na incidência de estenose biliar, bem como na resposta ao tratamento endoscópico. A terapêutica endoscópica utilizando-se esfincterotomia, dilatação balonada da estenose e inserção de próteses biliares através da CPRE é utilizada como método inicial de tratamento dessas complicações. Objetivos: Comparar as diferentes técnicas de tratamento endoscópico das estenoses biliares pós-transplante hepático. Método: Foi realizada uma revisão sistemática da literatura e metanálise sendo a busca conduzida nas bases MEDLINE, EMBASE, Scielo - LILACS e Biblioteca Cochrane até junho de 2015. A metanálise foi executada utilizando-se os softwares Review Manager, 2012 (RevMan) versão 5.2 e OpenMetaAnalyst e os cálculos dos desfechos foram feitos comparando-se os resultados dos estudos incluídos utilizando-se a diferença de risco absoluto e adotando-se um intervalo de confiança (IC) de 95%. Os estudos foram agrupados comparando-se transplantes hepáticos com doador cadáver versus doador vivo; dilatação biliar endoscópica com balão exclusiva versus dilatação biliar endoscópica com balão associada à inserção de próteses plásticas e próteses biliares plásticas comparadas à prótese biliar metálica por endoscopia. Os desfechos clínicos analisados foram incidência da estenose biliar, falha do tratamento endoscópico, resolução da estenose, recorrência da estenose e complicações. Resultados: Foram recuperados 1.110 artigos, sendo motivo de análise dez ensaios clínicos, com apenas um Ensaio Clínico Randomizado e nove Ensaios Clínicos não randomizados, dos quais sete foram incluídos na metanálise. Comparando-se doador cadáver e doador vivo observou-se redução da incidência de estenose biliar (p=0,0001), bem como da falha técnica do tratamento endoscópico (p=0,0009) e da recorrência da estenose biliar (p=0,03) nos transplantes realizados com enxertos provenientes de doador cadáver. Dois estudos compararam o tratamento da estenose da anastomose biliar pós-transplante hepático utilizando dilatação com balão exclusiva versus dilatação com balão associada à inserção próteses plásticas e não foram observadas diferenças estatisticamente significantes em relação aos desfechos falha de tratamento, recorrência da estenose ou complicações. Somente o desfecho clínico complicações teve resultado estatisticamente significante na comparação entre prótese metálica autoexpansível versus prótese plástica no tratamento da estenose da anastomose biliar pós-transplante hepático (p= 0.03). Conclusões: O tratamento da estenose biliar anastomótica pós-transplante hepático com prótese metálica foi igualmente efetivo quando comparado ao uso de prótese plástica, mas associou-se a um menor risco de complicações. A comparação entre dilatação com balão exclusiva e dilatação com balão associada à prótese plástica apresentou resultados semelhantes em relação à falha do tratamento endoscópico, complicações e recorrência da estenose. A utilização de enxerto proveniente de doador cadáver reduziu o risco de estenose biliar pós-transplante hepático e o tratamento endoscópico nesse grupo de pacientes, foi mais efetivo quando comparado com as estenoses biliares após transplante com doador vivo / The most common biliary complications after liver transplantation are anastomotic strictures, non-anastomotic strictures and biliary fistulas and they can occur in different fashions, isolated or in combination. Graft source (cadaveric liver donor or living liver donor) has an influence on the incidence of biliary strictures as well as on the response to endoscopic treatment. The endoscopic treatment using sphincterotomy, balloon dilation and insertion of biliary stents by ERCP (Endoscopic Retrograde Cholangiopancreatography) is used as an initial endoscopic approach to treat these complications. Objectives: To compare different endoscopic techniques to treat post-liver transplantation biliary strictures. Method: It was performed a systematic review of the literature and meta-analysis and the search was carried out on MEDLINE, EMBASE, Scielo-LILACS and Cochrane Library databases until June, 2015. The meta-analysis was made using Review Manager, 2012 (RevMan) version 5.2 and OpenMetaAnalyst software and the calculations of the outcomes were made comparing the results from the included papers by using the difference in absolute risks, adopting a confidence interval of 95%. The studies were grouped comparing cadaveric liver donor versus living liver donor grafts; exclusive balloon dilation versus balloon dilation associated with plastic stents insertion; and plastic stents versus totally covered selfexpandable metal stents. The clinical outcomes were biliary stricture incidence, endoscopic treatment failure, stricture resolution, stricture recurrence and complications. Results: There were retrieved 1,100 articles. Ten clinical trials were analyzed, with just one Randomized Clinical Trial and nine Non-Randomized Clinical Trials, out of which seven were included in the meta-analysis. When comparing cadaveric liver donor transplantation to living liver donor transplantation, it was observed a decrease in the incidence of biliary strictures (p=0.0001), as well as in the technical failure rate of the endoscopic treatment (p=0.0009) and in the biliary stricture recurrence (p=0.03) in the cadaveric liver donor graft group. Two studies have compared the treatment of anastomotic biliary strictures after liver transplantation using balloon dilation exclusive to balloon dilation associated with the insertion of plastic stents, and no statistically significant differences in relation to endoscopic treatment failure, stricture recurrence or complications rates were observed. Only the clinical outcome complications had statistically significant result in a comparison between self-expandable metal stents versus plastic stents in the treatment of post-liver transplantation anastomotic biliary strictures (p=0.03). Conclusions: The treatment of post-liver transplantation anastomotic biliary strictures was equally effective when compared the use of self-expandable metal stents to plastic stents, but the use metallic stents was associated with a lower complication risk. The comparison between exclusive balloon dilation to balloon dilation associated with plastic stents presented similar results in relation to endoscopic treatment failure, complications and stenosis recurrence. The use of graft from cadaveric donor reduced the risk of biliary stenosis after liver transplantation and endoscopic treatment of biliary strictures in these patients were more effective when compared to biliary strictures after living liver donor transplantation

Drenagem biliar na paliação dos tumores malignos da confluência biliopancreática: estudo comparativo das abordagens cirúrgica e endoscópica ecoguiada / Biliary drainage in the palliative management of malignant tumors in the biliopancreatic junction: a comparative study of surgical and endosonography-guided approaches

Introdução: A maioria dos pacientes acometidos pela neoplasia que envolve a confluência biliopancreática é diagnosticada em fase avançada. A Colangiopancreatografia Retrógrada Endoscópica (CPRE) é o método de escolha para a drenagem da via biliar obstruída. Todavia, existe um índice de insucesso em torno de 10%. Nesses casos, técnicas alternativas serão aplicadas, como drenagem percutânea trans-hepática e drenagens cirúrgicas. Objetivo: Avaliar o sucesso técnico, clínico, qualidade de vida e sobrevida da drenagem biliar pela cirurgia convencional e técnica endoscópica ecoguiada em pacientes portadores de neoplasia maligna da confluência biliopancreática. Método: No período de abril de 2010 a setembro de 2013, foram estudados 32 pacientes portadores de neoplasia maligna da confluência biliopancreática. Todos os que foram incluídos nesse estudo apresentaram falha na drenagem biliar por CPRE. Três deles foram excluídos por insucesso técnico (falha na confecção da anastomose hepaticojejunal e da formação da fístula coledocoduodenal ecoguiada). O Grupo I foi formado por 15 pacientes submetidos à Hepaticojejunostomia (HJT) em \"Y\" de Roux e derivação gastrojejunal. O Grupo II foi formado por 14 pacientes submetidos à coledocoduodenostomia ecoguiada (CDT). O sucesso clínico foi avaliado pela queda da bilirrubina sérica total em mais de 50% nos sete primeiros dias após o procedimento. A qualidade de vida foi avaliada pelo questionário SF-36 e a sobrevida pela curva de Kaplan-Meier. Resultados: O sucesso técnico foi de 93,75% (15/16) no Grupo I e de 87,5% (14/16) no Grupo II (p = 0,598). O sucesso clínico ocorreu em 14 (93,33%) pacientes pertencentes ao Grupo I e em 10 (71,43%) do Grupo II. Não houve diferença estatisticamente significativa (p = 0,169). O comportamento médio dos escores de qualidade de vida foi estatisticamente igual entre as técnicas ao longo do seguimento (p > 0,05 Técnica * Momento). Houve alteração média estatisticamente significativa ao longo do seguimento nos escores de capacidade funcional, saúde física, dor, aspectos sociais, aspectos emocionais e saúde mental em ambas as técnicas (p < 0,05). O escore de saúde mental foi, em média, estatisticamente maior nos do Grupo II (CDT) em todos os momentos (p = 0,035). O tempo médio de sobrevida daqueles pertencentes ao Grupo I foi de 82,27 dias e os do Grupo II, de 82,36 dias. Sessenta por cento dos pertencentes ao Grupo I faleceram até 90 dias após o procedimento cirúrgico. Por outro lado, 42,9% dos submetidos à CDT faleceram no mesmo período. Não houve diferença estatisticamente significativa no tempo de sobrevida entre os Grupos (p = 0,389). Conclusão: Os dados relacionados aos sucessos técnico, clínico, qualidade de vida e sobrevida foram semelhantes em ambos os grupos, não se verificando diferença estatisticamente significativa / Introduction: Most patients with neoplasm in the biliopancreatic junction are diagnosed at an advanced stage. Endoscopic retrograde cholangiopancreatography (ERCP) is the method of choice for drainage of obstructed biliary tract. However, there is a failure rate of about 10%. In such cases, alternative techniques, such as, percutaneous transhepatic drainage and surgical drainage are applied. Aim: To evaluate the technical and clinical success, quality of life and patient survival of biliary drainage by conventional surgery and endosonography-guided technique in patients with malignant neoplasm of the biliopancreatic junction. Methodology: From April 2010 to September 2013, 32 patients with malignant neoplasm of the biliopancreatic junction were studied. All patients included in this study had failed biliary drainage by ERCP. Three patients were excluded due to technical failure (failure in the construction of hepatico-jejuno anastomosis and formation of endosonography-guided choledochoduodenal fistula). Group I comprised of 15 patients who underwent Roux-en-Y hepaticojejunostomy (HJT) and gastrojejunal bypass. Group II consisted of 14 patients who underwent endosonography-guided choledochoduodenostomy (CDT). Clinical success was assessed by the decrease of more than 50% in total serum bilirubin in the first seven days after the procedure. Quality of life was assessed by SF-36 questionnaire and survival by Kaplan-Meier curve. Results: Technical success rate was 93.75% (15/16) in group I and 87.5% (14/16) in group II (p = 0.598). Clinical success occurred in 14 (93.33%) patients in group I and 10 (71.43%) patients in group II. There was no significant statistically difference (p = 0.169). The average quality of life score were statistically equal between the techniques during follow-up (p > 0.05 * Technical Moment). There were statistically significant mean changes during follow-up of functional capacity score, physical health, pain, social functioning, emotional and mental health aspects in both techniques (p < 0.05). The mental health score was, on average, statistically higher in group II (CDT) at all times (p = 0.035). The median survival time of patients in group I was 82.27 days and Group II patients was 82.36 days. Sixty percent of patients in group I died within 90 days after the surgical procedure. On the other hand, 42.9% of the patients who underwent CDT died in the same period. There was no statistically significant difference in survival time between the groups (p = 0.389). Conclusion: Data relating to technical and clinical success, quality of life and survival were similar in both groups and there were no statistically significant differences
