L'imagerie par tomographie en cohérence optique (OCT) est prometteuse comme support de la prise de décision au cours des procédures d'interventions coronariennes percutanées (PCI), pou évaluer les lésions athéromateuses, juger de la bonne implantation du stent, et dépister les lésions vasculaires dues au stent. L'OCT représente donc bien une aide potentielle pour le cardiologue interventionnel tout au long de la procédure de stenting, avec un impact certain sur la stratégie interventionnelle initialement programmée. De plus, l'OCT se révèle comme un nouvel outil pour prédire la dissection des bords de stent chez les patient avec ACS sans élévation du segment ST, rendant possible une stratification des patients quant à ce risque. Des essais cliniques randomisés sont maintenant nécessaires pour savoir si l'assistance par l'OCT pendant la procédure améliore le pronostic à long terme des patients après PCI / Optical Coherence Tomography (OCT) imaging is promising in decision making during Percutaneus Coronary Interventions {PCI) procedures, including evaluating controversial plaque lesions, assessing stent implantation, and surveying stent-related vascular injury. Thus, OCT has potential to guide interventional cardiologists throughout the stent implantation procedure, impacting on planned interventional strategy. In addition, OCT is the most novel image technology to predict stent edge dissection for patients with non-ST-segment elevation ACS, enabling risk stratification of patients who are at a higher risk of this complication. Large-scale randomized trials are now warranted to assess whether OCT results and guidance during de procedure improve long-term clinical outcomes of PCis.

Alterações histopatológicas de stents metálicos no endotélio coronariano \"in vivo\" / Histopathological abnormality in coronary artery bare stent metal \"in vivo\"Othon Amaral Neto 02 March 2012 (has links)
Duas técnicas invasivas para o tratamento da doença aterosclerótica coronariana oclusiva firmaram-se ao longo dos anos: revascularização cirúrgica do miocárdio e angioplastia transcutânea com stents metálicos. O estudo visa comparar as alterações histopatológicas causadas por stents metálicos coronarianos fabricados com a superliga de composição química em porcentagem em peso cromo 20%, tungstênio 15%, níquel 10% e cobalto restante, designada ASTM F.90, revestidos, ou não, com carbeto de silício pelo processo de asperção térmica originando uma superfície hidrofílica. Stents com espessura das hastes entre 80 a 90 nm, área das células entre 1,4 a 2,1 mm² e relação metal-artéria de 13 a 19%, em pacientes reestenosantes que sofreram posteriormente revascularização cirúrgica do miocárdio, com aqueles não submetidos à angioplastia prévia. Foram determinados dois grupos: grupo I ou grupo controle, pacientes que sofreram revascularização cirúrgica do miocárdio sem angioplastia prévia de qualquer natureza; grupo II pacientes submetidos à revascularização cirúrgica do miocárdio, após reestenose intra-stent coronariana. Pacientes de ambos os grupos foram avaliados rotineiramente quanto à indicação e risco cirúrgico e durante o procedimento convencional da revascularização cirúrgica do miocárdio utilizando circulação extracorpórea, antes de realizar a anastomose do enxerto vascular na coronária, amostras contendo pequenos fragmentos de endotélio foram retiradas juntamente com fragmentos dos stents, enviadas para análise histopatológica e produção de laminas coradas com H-E. Observou-se a presença de arterite crônica caracterizada por infiltrado mononuclear em conjunto com fenômeno de proliferação fibroblástica e de musculatura lisa naqueles pacientes que apresentavam reestenose intra-stent, sendo mais intenso no local do stent. A visualização das superfícies dos stents em escala nanometrica (MFA) é de suma importância para análise estrutural das próteses, avaliando irregularidades nas superfícies recobertas das hastes dos stents. A persistência de arterite crônica coronariana avaliada por infiltrado linfomononuclear e proliferação de fibrocolágeno foi constatada em pacientes reestenosantes. / Two invasive techniques for the treatment of occlusive coronary atherosclerosis disease were signed over the years: coronary artery bypass surgery and transcutaneous angioplasty with bare metal stent. The study attempts to compare the histopathological abnormality caused in patients with implantation of bare metal stent in coronary made with: Chromium 20%, Tungsten 15%, Nickel 10% and Cobalt remainder, ASTM F.90 alloy for surgical implant applications, and covered with a thin layer of amorphous silicon carbide, or not, and its total or partial obstruction, after undergoing coronary artery bypass grafting, with those who had coronary artery bypass surgery with no previous angioplasty. Two groups were studied: group I, or control group, patients who underwent coronary artery bypass grafting without previous angioplasty of any kind; group II of patients undergoing coronary artery bypass surgery after coronary-stent restenosis. Patients in both groups were evaluated for the indication and surgical risk; was done routinely during the procedure of conventional coronary artery bypass grafting with cardiopulmonary bypass. Before performing the anastomosis in coronary vascular graft, a small fragment of the endothelium was removed along whit a fragment of the stent, and sent for analysis with hematoxilin-eosin. The presence of chronic inflammatory coronary reaction was detected, mediated by mononuclear cells with phenomenon of fibroblast and smooth muscle proliferation in patients presenting in-stent restenosis. It w coronary reaction as also observed that the inflammatory and proliferative process is more intense at the site of stent implantation. The analysis of surface of the stents used atomic force microscopy proved to be an important method for the surface analysis for stents, and showed on nanometric scale an irregular coverage of silicon carbide. In conclusion, in the patients with restenosis in-stent occurs persistence of chronic inflammation with mononuclear cells and process of fibroblast proliferation.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Recent studies indicate a strong association among physical inactivity, low level of cardio-respiratory fitness and the presence of cardiovascular risk factors such as high blood pressure, diabetes and obesity. Medicine and Physical Education students are involved in the field of health rehabilitation, promotion and conservation, being physical activity a priority intervention to achieve this goal. However, little is known about the adoption of physical activity as part of the life style of Medicine and Physical Education students. The main goal of this paper is to compare the level of physical activity, the level of cardio-respiratory fitness and the cardiovascular risk in students of Medicine and Physical Education. The data collection took place at the Physical Education Department at the Federal University of Sergipe. In a first stage, the IPAQ questionnaire was applied to quantify the amount of physical activity in 126 students from the 7th and 8th periods of Medicine and Physical Education courses. In a second stage, 40 students were randomly selected, 20 from each course, to evaluate cardiovascular risk factors and cardio-respiratory fitness. We measured (a) blood pressure; (b) body mass index (BMI); (c) fat percentage (electrical bioimpedance); (d) waist circumference (WC); (e) biochemical lab tests and (f) cardio-respiratory fitness (Kline test). Comparing Medicine students versus Physical Education students, we noticed a higher frequency of subjects presenting: low level of physical activity (55 vs 15%; p=0,008); pre-high blood pressure by SBP (80 vs 25%; p=0,000) and by DBP (45 vs 5%; p=0,003); overweight (50 vs 10%; p=0,006); waist circumference (25 vs 0%; p=0,017); elevated total cholesterol (165±28 vs 142±28mg/dL; p=0,015); high LDLc (99±27 vs 81±23 mg/dL; p=0,026); high glycemia (81±8 vs 75±7 mg/dL; p=0,013); lower cardio-respiratory fitness (48±8 vs 56±7 L.min.-1; p=0,001). Even though the knowledge that practicing physical activities is an important preventive intervention for cardiovascular diseases, students of Medicine display a smaller amount of physical activities practice, lower level of cardio-respiratory fitness and, most importantly, higher frequency of cardiovascular risk, compared to those of Physical Education. / Estudos recentes indicam forte associação entre inatividade física, baixo nível de condicionamento cardio-respiratório e presença de fatores de risco cardiovascular como hipertensão arterial, diabetes e obesidade. O ensino da Medicina e da Educação Física está fundamentado com o campo da conservação, promoção e reabilitação da saúde, sendo a atividade física uma intervenção prioritária para este fim. Entretanto, pouco se sabe sobre a adoção da atividade física como parte do estilo de vida dos estudantes de Medicina e Educação Física. O objetivo principal do presente trabalho foi de comparar o nível de atividade física, o nível de condicionamento cardio-respiratório e o risco cardiovascular entre estudantes de Medicina e de Educação Física. A coleta de dados foi realizada no departamento de Educação Física da Universidade Federal de Sergipe. Em uma primeira etapa aplicou-se questionário IPAQ para quantificar atividade física em 126 alunos dos 7os e 8os períodos dos cursos de Educação Física e Medicina. Em uma segunda etapa, selecionou-se por intermédio de randomização, 40 alunos, 20 de cada curso, para avaliação de fatores de risco cardiovascular e avaliação do condicionamento cardio-respiratório. Comparando-se estudantes de Medicina vs Educação Física foi observado maior freqüência de indivíduos apresentando: baixo nível de atividade física (55 vs 15%; p=0,008); pré-hipertensão pela PAS (80 vs 25%; p=0,000) e pela PAD (45 vs 5%; p=0,003); sobrepeso (50 vs 10%; p=0,006); circunferência de cintura (25 vs 0%; p=0,017); colesterol total elevado (165±28 vs 142±28 mg/dL; p=0,015); LDLc elevado (99±27 vs 81±23 mg/dL; p=0,026); glicemia elevada (81±8 vs 75±7 mg/dL; p=0,013); menor condicionamento cardio-respiratório (48±8 vs 56±7 ml/Kg/min; p=0,001). Apesar do conhecimento de que a prática de atividade física é uma importante intervenção preventiva para as doenças cardiovasculares, estudantes de Medicina apresentam menor quantitativo de prática de atividade física, menor nível de condicionamento cardio-respiratório e, mais importante, maior freqüência dos fatores de risco cardiovascular, comparados aos de Educação Física.

Previous issue date: 2009-02-27 / The obesity is a morbid condition commonnly incident in veterinary practice,
generally associate to the companion animals reduction of time and quality of life.
This factor, added to the regularity that it is observed, makes from obesity,
nowadays, the most important metabolism disease in dogs, being still able to
promote serious health problems, including cardiovascular disorders.
For this study, 14 medium-sized neutered mongrel adult female dogs were divided
in two groups and submitted to a program of weight gain. Group I, presenting
highest percentage of weight gain (average 21.4% above the initial weight, in
average) and Group II, with shortest weight gain percentage (15% above the initial
weight, in average). Afterwards, both groups were submitted to a obesity
correction food program, to reach the initial weight. During the food programs, in
the initial phase (T1), obesity phase (T2) and final phase of weight reduction (T3),
examinations carried out were: electrocardiographic, radiographic and
mensurations of the blood pressure. During the phases T2 and T3,
ecodopplercardiographic examinations of all dogs were also performed. At the end
of the experimental phase, every variables obtained were statistically evaluated,
comparing the groups and the different phases within the same group. The results
revealed no significant eletrocardiographic and ecodopplercardiographic
alterations, considering up to 21% of increment on body weight; increase of
cardiac silhouette on radiographic evaluation during the obesity phase and
decrease after the weight loss, and increase of arterial systolic pressure after the
weight increment. / A obesidade é uma condição mórbida de ocorrência comum na prática
veterinária, estando geralmente associada à redução do tempo e da qualidade de
vida dos animais de companhia. Esse fator, unido à regularidade com que é
observada, faz da obesidade, atualmente, a forma mais importante de doença do
metabolismo em cães, podendo provocar sérios problemas de saúde, dentre eles
os distúrbios cardiovasculares. Neste trabalho, 14 cadelas de porte médio, sem
raça definida, adultas e castradas foram divididas em dois grupos e submetidas a
um programa de ganho de peso. O Grupo I com maior porcentagem de ganho de
peso (em média 21,4% acima do peso inicial) e o Grupo II com menor
porcentagem de ganho de peso (em média 15% acima do peso inicial).
Posteriormente foram submetidas a um programa alimentar de correção da
obesidade até atingirem o peso inicial. Durante os programas alimentares, na fase
inicial (T1), fase de obesidade (T2) e fase final de redução de peso (T3) foram
realizados exames eletrocardiográficos, radiográficos e mensurações da pressão
arterial e nas fases T2 e T3 exames ecodopplercardiográficos em todas as
cadelas. Ao final da fase experimental, todas as variáveis obtidas foram avaliadas
estatisticamente, comparando os grupos e as diferentes fases dentro do mesmo
grupo. Os resultados revelaram que não ocorreram alterações
eletrocardiográficas e nem ecodopplercardiograficas significativas, considerando
incremento de peso corporal de até 21%; aumento significativo da silhueta
cardíaca à avaliação radiográfica, na fase de obesidade, e diminuição da mesma
após perda de peso e a pressão arterial sistólica dos cães aumenta com o
incremento do peso.

La coagulopathie induite par la circulation extracorporelle (CEC) chez les enfants porteurs d’une cardiopathie congénitale et bénéficiant d’une chirurgie cardiaque est complexe et multifactorielle. La prise en charge de ces enfants est délicate et doit être spécifique aux caractéristiques de cette population. De plus, cette prise en charge doit être multimodale, basée sur la prévention et le traitement précoce et ciblé. <p>Dans la première partie de ce mémoire, nous avons étudié la place de l’acide tranexamique, agent antifibrinolytique, dans la prévention de la fibrinolyse chez les enfants bénéficiant d’une chirurgie cardiaque avec CEC. Nous avons évalué les propriétés pharmacocinétiques et pharmacodynamiques de la molécule dans cette population particulière. Notre hypothèse étant que l’utilisation de schémas adaptés pourrait permettre d’optimaliser la balance bénéfice/risque de l’utilisation prophylactique de l’acide tranexamique. <p>Nous avons également développé un modèle expérimental susceptible d’améliorer la sensibilité des tests viscoélastiques pour la détection de la fibrinolyse et qui pourrait être utilisé pour estimer la concentration minimale d’acide tranexamique nécessaire pour inhiber la fibrinolyse. <p>Dans la seconde partie de ce mémoire, nous avons étudié l’importance de l’implémentation d’une prise en charge adaptée de la coagulopathie, en utilisant un algorithme défini en tenant compte des caractéristiques de la population cible. Si le ROTEM® s’avère être un outil important, son utilisation doit être limitée aux enfants qui présentent un saignement anormal afin de guider l’administration de produits hémostatiques. The presentation of self-concept and emotional profile in a cardiological populationLouw, Charl 09 February 2005 (has links)
This research study examines the manner in which a cardiological sample presents in terms of their psychological make-up by making specific reference to the constructs self-concept and emotions. A literature overview of different aspects of self-concept, emotion and cardiology places the results and discussions of the study within a theoretical background. The study supplies descriptive information relating to the demographic profile of the sample, followed by a description of various aspects of self-concept and emotions, as well as a correlational exploration of the manner in which the sample group presents. The sample consisted of 29 individuals, all who had been diagnosed with coronary heart disease (CHD) and been subjected to surgery as a result thereof. The participants completed a questionnaire, containing two measurement instruments, namely the Adolescent Self-Concept Scale (ASCS) and the Emotions Profile Index (EPI). The scores, obtained by the sample, were subjected to statistical analysis to provide a self-conceptual and emotional profile of the sample. The Spearman Rank-Order Correlation Coefficient was then used to indicate the extent to which the sample tended towards portraying themselves in a positive light, more than they might be experiencing. The study further refers to the views of the sample group, relating to these findings, as obtained from an information and discussion session held with them. The study indicate a significant statistical trend amongst the sample group to portray themselves in a positive light in relation to emotions, even though they were not in reality experiencing such positive emotions. Considerações referentes à influência do posicionamento dos membros superiores sobre o resultado da cintilografia de perfusão do miocárdio / Considerations about the influence of upper arms positioning on myocardial perfusion scintigraphy resultsMarisa Izaki 01 August 2008 (has links)
I NTRODUÇÃO: Na cintilografia de perfusão do miocárdio, contrariamente ao desenvolvimento tecnológico ao longo do tempo, manteve-se desde o princípio a mesma forma, por vezes desconfortável ou mesmo inviável, de posicionamento na aquisição das imagens: decúbito dorsal com os braços elevados. OBJETIVOS: O objetivo da pesquisa foi o de avaliar se na aquisição da cintilografia de perfusão do miocárdio o posicionamento com os membros superiores abaixados(T) apresenta os mesmos resultados que os obtidos com os membros superiores elevados(C), tanto em termos de alterações perfusionais como em relação aos parâmetros funcionais. . MÉTODOS: Foram estudados 120 pacientes, 83 (69%) do sexo masculino, com idade 59,4 ± 11,4 anos e peso 72,8 ± 14 kg. Foi realizada primeiramente a aquisição C e na seqüência a aquisição T, utilizando protocolo de um dia de repouso-estresse (dose 370 MBq e 1110 MBq de sestamibi-99mTc), com estresse sincronizado ao eletrocardiograma (gated- SPECT). Três modelos distintos de equipamentos de dois detectores foram utilizados no estudo. Os estudos foram processados usando reconstrução iterativa (OSEM). Na interpretação foi utilizado o modelo de segmentação miocárdica de 17 segmentos, pontuados de 0 a 4 (normal a ausente) segundo o grau de captação. Baseada na análise de um observador dentre um grupo de sete, foi realizada a comparação da totalidade dos segmentos e também segmento a segmento das aquisições C e T nas etapas de estresse e de repouso. Foi comparada também a somatória das pontuações das etapas de estresse(SSS) e repouso(SRS). Os pacientes foram divididos segundo dois critérios: primeiramente, normais (SSS=0) e anormais (SSS>1) e posteriormente, em de baixo risco (SSS3) e de maior risco (SSS>3) A comparação dos parâmetros funcionais de fração de ejeção do ventrículo esquerdo (FEVE), volume diastólico final (VDF), volume sistólico final(VSF), somatória dos escores de motilidade (SMS) e somatória dos escores de espessamento (STS) foi baseada nos valores obtidos de modo automático utilizando o programa computacional Quantitative Gated SPECT. RESULTADOS: No modo C, 23,3% dos pts referiram algum grau de dor no membro superior ou no ombro e no modo T 5 %. Houve diferença significativa (p<0,05) na comparação dos 2040 segmentos miocárdicos entre C e T, ao estresse e ao repouso. Nos 63 pts normais não houve diferença significativa dos escores entre C e T ao estresse e ao repouso. O mesmo foi observado nos 80 pts de baixo risco. Nos 57 pts anormais houve boa correlação entre os valores de SSS (Rho=0,93, p=0,0001) e SRS (Rho=0,93, p=0,0001), mas com valores não equivalentes (p<0,05), sendo a média dos valores de SSS (9,28±8,10) e SRS (7,89±7,34) da aquisição T inferiores à média dos valores de SSS (10,07±7,71) e SRS (8,46±7,35) encontrada na aquisição C. Nos 40 pts de risco, houve boa correlação dos valores de SSS(Rho=0,95, p=0,0001) e SRS(Rho=0,96 p=0,0001), porém a média dos valores de SSS (12,53±7,54) e SRS (10,60±7,08) da aquisição T foi significantemente (p<0,05) inferior à média dos valores de SSS (13,43±6,81) e SRS(11,33±6,97) encontrados na aquisição C. Os parâmetros funcionais apresentaram boa correlação entre C e T, exceto o VDF(p=0,0001). CONCLUSÕES: Os modos C e T apresentam boa correlação dos parâmetros funcionais, exceto o VDF. Embora a aquisição T tenha sido um pouco mais confortável e tenha demonstrado boa correlação nos achados perfusionais, em pts anormais e de maior risco há tendência à subestimação dos defeitos perfusionais. Considerando a importância da medida precisa da intensidade e extensão das alterações perfusionais em termos de avaliação prognostica e decisão terapêutica, a aquisição com membro superior abaixado deve ser evitada. Em situações onde a aquisição com membros superiores abaixados é a única alternativa, os resultados da cintilografia devem ser interpretados com cuidado, especialmente em pacientes anormais e de maior risco, onde as alterações podem estar subestimadas / Contrary to the advances in imaging technology for nuclear cardiology applications, we keep using the same often uncomfortable and sometimes impracticable patient position- supine with arms raised above the head (C). We tested another position modality: supine with arms down at the sides of the trunk (T). The purpose of this study was to verify if the functional and perfusion results of the acquisition T are equivalent to those of the acquisition mode C. We studied 120 patients (pts), 83 (69%) male, aged 59.4±11.4 years and weighting 72.8±14 kg. We performed a one-day protocol (rest gated/stress), using 99mTc-sestamibi (370 MBq and 1110 MBq). In both times (rest and stress), we first performed acquisition in C and in sequence the acquisition in T. The studies were performed in three types of dual detector SPECT systems. T mode was executed successfully in all pts. Images were processed by the iterative reconstruction method (OSEM). Each study was independently interpreted by one nuclear medicine specialist from a group of seven physicians using the 17-segment model. The segments were scored using a 5-point model ranging from 0 (normal uptake) to 4 (uptake absent). The total score of the left ventricle at stress is referred to as the summed stress score (SSS) and at rest as the summed rest score (SRS). The patients were categorized in subgroups by two criteria: normal (SSS=0) or abnormal (SSS1) and low risk (SSS3) or risk (SSS>3). The values of the functional parameters of left ventricular ejection fraction (LVEF), end diastolic volume (EDV), end systolic volume (ESV), stress motion score (SMS) and stress thickening score (STS) for both C and T were automatically obtained by the quantitative gated SPECT (QGS) program and results were compared. Shoulder and/or back pain occurred in 23.3% of C patients and in 5% of T patients. There was no agreement between the 2040 segmental scores of both rest and stress in C and T modes (p<0.05). No significant differences between C and T were found for SSS and SRS in the 63 normal individuals nor in the 80 low risk patients. Good correlation between C and T was found for SSS (Rho=0.95, p=0.0001) and SRS (Rho=0.96 p=0.0001) in the 57 abnormal pts, but the mean SSS (9.28±8.10) and SRS (7.89±7.34) values of T were significantly lower (p<0.05) than the mean for SSS (10.07±7,71) and SRS(8.46±7,35) of C mode. Similar patterns were observed in the 40 risk pts; good correlation was found between C and T modes for SSS (Rho=0.95, p=0.0001) and SRS (Rho=0.96 p=0.0001), but the mean SSS (12.53±7.54) and SRS (10.60±7.08) values of T were significantly lower (p<0.05) than SSS (13.43±6.81) and SRS (11.33±6.97) of C mode. Good correlation between C and T was found for all functional parameters, except for EDV (p=0.0001). Although T mode appears to be more comfortable and presented a good correlation between SSS and SRS values, in abnormal and risk pts, the extent and severity of defects can be underestimated. Considering the important therapeutic and prognostic implications of an accurate perfusion measurement, the cardiac SPECT acquisition with the arms in down position should be avoided. When arms-down acquisition is the only alternative, scintigraphy results must be carefully interpreted, especially in abnormal scans whereas the amount of perfusion abnormalities can be underestimated.

Ventricular Arrhythmias Complicating Coronary Artery Disease: Recent Trends, Risk Associated with Serum Glucose Levels, and Psychological ImpactTran, Hoang V. 18 June 2018 (has links)
Introduction: Ventricular arrhythmias (VAs) are common after an acute coronary syndrome (ACS) and are associated with worse clinical outcomes. However, little is known about recent trends in their occurrence, their association with serum glucose levels, and their psychological impact in ACS setting.
Methods: We examined 25-year (1986-2011) trends in the incidence rates (IRs) and hospital case-fatality rates (CFRs) of VAs, and the association between serum glucose levels and VAs in patients with an acute myocardial infarction (AMI) in the Worcester Heart Attack Study. Lastly, we examined the relationship between in-hospital occurrence of VAs and 12-month progression of depression and anxiety among hospital survivors of an ACS in the longitudinal TRACE-CORE study.
Results: We found the IRs declined for several major VAs between 1986 and 2011while the hospital CFRs declined in both patients with and without VAs over this period. Elevated serum glucose levels at hospital admission were associated with a higher risk of developing in-hospital VAs. Occurrence of VAs, however, was not associated with worsening progression of symptoms of depression and/or anxiety over a 12-month follow-up period in patients discharged after an ACS.
Conclusions: The burden and impact of VAs in patients with an AMI has declined over time. Elevated serum glucose levels at hospital admission may serve as a predictor for in-hospital occurrence of serious cardiac arrhythmias. In-hospital occurrence of VAs may not be associated with worsening progression of symptoms of depression and anxiety in patients with an ACS.

The aim of this master´s thesis is to analyse and search unusual dependencies in database of patients from Internal Cardiology Clinic Faculty Hospital Brno. The part of the work is theoretical overview of common data mining methods used in medicine, especially decision trees, naive Bayesian classifier, artificial neural networks and association rules. Looking for unusual dependencies between atributes is realized by association rules and naive Bayesian classifier. The output of this work is a complex system for Knowledge discovery in databases process for any data set. This work was realized with collaboration of Internal Cardiology Clinic Faculty Hospital Brno. All programs were made in Matlab 7.0.1.
