The thesis solves the processing of a project documentation of a veterinary clinic in Nachod. The project and attachments are processed in accordance with current laws, regulations and standards. The Veterinary Clinic is located on parcels č.122 / 3 and 122/4, which are parts of the cadastral municipality Nachod. Near the land under the adjoining local roads, there are utilities routed. In the building of veterinary clinic, there will be 2 surgeries, 2 operating rooms, laboratory, pharmacy, grocery breeders and editorial office. Veterinary Clinic is a two-storey building with a rectangular ground plan with dimensions of approximately 40 x 19 meters. The building has 3 main entrances. Admission to the clinic and store are situated on the northwestern side. Entry into the pharmacy and the editorial office, which is located on the 2nd floor, are on the southeast side. The supporting wall system is made of aerated concrete, ceiling is made of reinforced concrete. On the building's flat roof with reinforced concrete supporting structure.

Vocabulary tests have long been used for estimating premorbid intelligence level in the neuropsychological assessment of dementia. However, doubts exist about the validity of such intelligence tests. The present study examines whether the Multiple-Choice Vocabulary Test – Version A (Mehrfachwahl-Wortschatz-Test – Version A, MWT-A) is valid for assessing premorbid intelligence level. Data from a total of 821 patients in a specialized outpatient clinic for dementia (memory clinic), covering the whole spectrum of cognitive impairment, were evaluated using analysis of variance with the dependent variable premorbid intelligence level (MWT-A) and the independent variable extent of global cognitive impairment (Mini-Mental-State Examination, MMSE: mean = 25.2, SD = 3.9). The latter was divided into six MMSE ranges or groups, respectively (29–30, 28–28, 27–27, 25–26, 22–24, 05–21). In the case of a pathologically relevant global cognitive impairment (24–26 MMSE points), the MWT-A underestimates the premorbid intelligence level. This effect is moderated neither by age nor education. Results indicate that the MWT-A is unsuitable for estimating premorbid intelligence level in neuropsychological assessments of cognitively impaired patients or demented patients.

Entlassungsmanagement als Aufgabe des Sozialdienstes: Eine prospektive Katamnesestudie und daraus resultierende Kooperationsempfehlungen in der Tagesklinik für Psychiatrie und Psychotherapie GörlitzJanke, Maik 02 August 2016 (has links)
Ziel der im Oktober 2015 in der Tagesklinik für Psychiatrie und Psychotherapie Görlitz durchgeführten prospektive Katamnesestudie war es, den Übergang der Patienten/innen von der Therapie in den Alltag, evtl. Unterstützungsbedarfe sowie mögliche Schnittstellenprobleme zu evaluieren. Dies geschah zum einem vor dem Hintergrund des Entlassungsmanagements als Aufgabe des Sozialdienstes und zum anderen mit Blick auf die sich verändernde Versorgungs- und Finanzierungsstruktur durch den Paradigmenwechsel im deutschen Gesundheitswesen sowie in Psychiatrie und Psychosomatik. Hierfür konnten 37 Studienteilneh-mer/innen, bis zu 21 Monate nach ihrer Behandlung, für die Erhebung gewonnen werden. Die Ergebnisse der Studie liefern Hinweise über die Zufriedenheit mit der Behandlung, zur Bewältigung des Übergangs und filterten darüber hinaus weitere Bedarfe der Patienten/innen.
Ein überwiegender Teil der Studienpopulation (71%) war ‚sehr zufrieden’ oder ‚zufrieden’ mit der Behandlung und der Mehrheit der Patienten/innen gelang darüber hinaus der Übergang aus der Therapie zurück in den Alltag ‚gut’ (62,16 %). Obwohl die Mehrzahl der Patienten/innen den Übergang von der Therapie zurück in den Alltag ‚gut’ bewältigt haben, konnten spezifische Prädiktoren des Übergangs aus den Ergebnissen gefiltert werden, welche diesen positiv und negativ beeinflussen. Positive Prädiktoren sind die allgemeine Zufriedenheit mit der Behandlung und im speziellen die Zufriedenheit mit dem Sozialdienst und den Psychologen/innen und Psychiater/innen. Darüber hinaus gelang den Patienten/innen der Übergang gut, welche den Entlassungszeitpunkt als ‚genau richtig’ empfanden. Des Weiteren konnte ein positiver Zusammenhang zwischen der weiteren Einnahme von den in der Tagesklinik verschriebenen Medikamenten und der aktuellen Lebensqualität festgestellt werden.
Obwohl viele Patienten/innen (83,78 %) weiter Unterstützungsangebote nach der Entlassung wahrnahmen, wünscht sich ein Großteil (67,57) bereits während der Therapie eine Vermittlung und Begleitung zu anderen, externen Hilfsangeboten oder weitere Beratungsgespräche durch den Sozialdienst (45,95 %). Aus diesem Grund wurden über die Forschung hinaus Optionen einer effektiven und an den Bedürfnissen der Patienten/innen ausgerichteten Entlassungsplanung durch den Sozialdienst diskutiert und spezifische Kooperationsempfehlungen ausgesprochen.

This thesis traces the inception and evolution of a combined collaborative action research project and living theory action research project. Six academic staff members attempted to improve our practice of generating locally relevant research in a university psychology clinic. This process impacted not only on the lives of the participants, but facilitated the enactment of the three tasks of universities and so influenced the lives of the student and residential communities to whom we had a responsibility. This thesis explores two research questions that formed the first part of the study, namely: “How can we improve the functioning of Itsoseng Psychology Clinic?” and “How can we increase our research output?” The second part of the study was a self-study action research project in which I examined my attempts to improve my academic practice by inquiring into my practice of facilitating the collaborative action research project as a peer support initiative. In the form of my living theory, this thesis therefore also explored my answers to the questions: “How can I facilitate a peer support research initiative?” and “How can I improve my academic practice through facilitating such an initiative?” I take a macro-level view of the relationship between a university and surrounding communities and discuss within the South African context three discernable mandates or tasks that universities fulfil: teaching, research and community engagement. I discuss the relevance of this study to psychology and specifically university psychology clinics as potential interface between the university and the surrounding community when enacting community engagement as the third academic task. I also discuss the implications of this study to action research methodology and the concept of transformation in emancipatory research. Objetivo:
Describir y hacer un análisis crítico de los casos clínicos que formaron parte significativa del aprendizaje obtenido como interno de medicina durante los meses de rotación en el Centro de Salud Gustavo Lanatta Luján y el Hospital Nacional Dos de Mayo, durante el periodo de junio 2021 a febrero 2022, los cuales contribuyeron a mi formación como futuro médico cirujano.
Metodología:
Se realizó un estudio observacional, de tipo descriptivo y analítico de los casos que consolidaron la experiencia clínica obtenida durante el internado médico
Resultados principales o hallazgos:
Recopilación de 30 casos clínicos, que fueron presentados de manera descriptiva. Los cuales evidencian de forma global los principales aprendizajes obtenidos en la práctica clínica, tales como: comunicación oral, pensamiento crítico y analítico, manejo de información y profesionalismo.
Conclusiones:
El internado médico es una etapa fundamental en la formación de todo profesional del campo de la salud, en el cual se sintetizan, integran y aplican los conocimientos adquiridos durante los años previos. Actualmente, el contexto de pandemia nos ha permitido ser parte del desarrollo de la ciencia, experimentando de primera mano la evolución de una enfermedad, la cual generó cambios a los que se debió adaptar, no solo el personal de salud, sino también la población en general; asimismo atravesamos una etapa en la cual día a día se emitían nuevos avances científicos, los cuales debíamos analizar y aplicar en los diferentes campos con la finalidad de lograr el bienestar de nuestros pacientes. Finalmente, se obtuvo un aprendizaje íntegro, enfocado en el paciente, respetando y considerando sus creencias y culturas; fortaleciendo la relación médico paciente y la comunicación activa, puesto que en nuestra profesión se trata personas, no solo patologías. / Objective: To describe and make a critical analysis of the clinical cases that formed a significant part of the learning obtained as a medical intern during the months of rotation at Gustavo Lanatta Luján Health Center and Dos de Mayo National Hospital, during the period of June 2021 to February 2022, which contributed to the training as a future physician.
Methodology: An observational, descriptive and analytical study of the cases that consolidated the clinical experience obtained during the medical internship.
Main results or findings: Compilation of 30 clinical cases, which were presented using descriptive instruments. Which evidence the main learning obtained in clinical practice, such as: oral communication, critical and analytical thinking, information management and professionalism.
Conclusions: The medical internship is a fundamental stage in the training of all professionals in the health field, in which the knowledge acquired during the previous years is synthesized, integrated and applied. Currently, the pandemic context has allowed us to be part of the development of science, experiencing first-hand the evolution of a disease, which generated changes to which not only health personnel had to adapt, but also the population in general; Likewise, we went through a stage in which new scientific advances were issued day by day, which we had to analyze and apply in the different fields in order to achieve the well-being of our patients. / Trabajo de Suficiencia Profesional

ANDRUM “Creating a shelter, a mental health-care clinic, for body and soul.” Fredrika Stelander How can architectural elements be altered to be better suited to the mental healthcare patients? Furthermore, how can architecture work as an active vessel to let hospitals, in this case a mental health care clinic, favor from the forest and natures capacity to calm a worried mind and tend a broken soul? The architecture in mental healthcare are repeatedly based upon the same qualities set for the rest of the hospital. Through my research I came to the realization that architecture within the psychiatry, was more customized to physical healthcare and not mental healthcare. By decomposing these impersonal white institutions, we can start bringing in the human scale and let the structure reconnect to nature which, throughout my research, turned out to be reoccurring requests by this specific patient group. My answer became to develop a more humane and adapted mental health care clinic, by extracting the long-term patients from the psychiatric emergency wards to buildings in much smaller scale, where the patients wouldn’t feel intimidated, but could begin her/his journey to recovery. Another important strategy was to insert nature in the healing process. The aim was to give the patients a feeling of safety yet freedom. Thus, the premises should be enclosed and secure but at the same time, shouldn’t give the patients a sense of being trapped or institutionalized. My architecture would rather create a sense of playfulness, where the spaces are to be divided in smaller islands linked with passageways which invite nature into the building. A close attention to the topography and the identification of cultural values through site visits, became the most efficient methods to develop a project that would relate to function and the nature of the site. The corridors which collectively were called “the promenade through seasons”, became important elements to be able to interlace the forest with the buildings and a way for the patients’ to reconnect and enjoy nature, regardless season.

L'histoire du Rythme, l'histoire de l'Art et l'histoire de la Psychanalyse, c'est aussi l'histoire de la mémoire du Corps, surface d'inscription incarnée et surface incarnante immatérielle. Le corps comme matière informante. Du Chaos à la Forme, mouvant. Nous proposons une considération du rythme dans une perspective psychanalytique à l'épreuve de la clinique de la création artistique. La théorie du rythme, arrimée à différents champs, nous a conduite à l'analyse d'une revue restreinte de la littérature théorique élargie permettant d'en confronter les apports afin d'enrichir la réflexion psychanalytique. Ainsi, le rythme trouve un centre à l'endroit d'une quête de l'origine, vers l'originaire. Quête d'une mise en sens de la cosmogonie et du corps pulsionnel. Le rythme se voit conférer une dimension civilisatrice et historisante au sein du socius, au travers de la langue et de l'histoire des corps. La création est étudiée comme paradigme privilégié du rythme qui se compose via une traversée mouvante et simultanée des trois coffrets du maternels. Nous postulons d'un rythme subjectif, interface corps-psyché. Le rythme informe et forme la matière-sensible corporelle autant qu'immatérielle, au travers de la langue. Inscription sensible des corps en présence autant qu'inscription sur la matière support. Ainsi le lieu de l'art, celui de la forme artistique, se fait surface d'inscription externe d'un engramme corporel effractant. La quête d'une forme est quête de lieu pour le non-lieu de la mémoire. Cette thèse repose sur une double méthodologie de recherche : celle de la rencontre clinique et celle de l'analyse pathographique de l’œuvre d'art, éprouvée au travers d'une étude qualitative et casuistique, au recours d'une lecture clinique du sensible de l'immatérialité de la parole et du discours latent. L'étude de cas met en évidence un vécu d'effraction du fantasme et le recours à l'éprouvé sensoriel informant le langage et l'expression artistique. La confrontation à la sexualité génitale, dans l'après-coup, révèle sa potentialité traumatique, conduisant à une réactualisation de l'archaïque à l'aune de la relation objectale, source d'une aliénation spatio-temporelle interne. Par le travail du rythme, l'œuvre artistique se fait lieu d'inscription déterritorialisé de l'informe, sans pour autant endiguer la trace primitive, à l'inverse ; mais cependant lui confère une reconfiguration spatio-temporelle. Du point de vue intrapsychique, le rythme est envisagé comme élément non-langagier concourant au système représentationnel. D'un point de vue dynamique, le rythme dispose une forme mouvante au service de la dialectique liaison-déliaison, visant un départage du temps et de l'espace interne non-advenu, vecteur de lien et d'historicisation. The theory of rhythm connected to different fields, has led us to analyse a limited inspection of broad theoretical literature enabling us to compare the contributions in order to enhance and refine psychoanalytical contemplation. Thus, rhythm finds itself in the middle of the quest for the origin, towards the origin-point. A quest for making sense of the cosmogony and the instinct of the body. Rhythm is given a dimension of enlightenment and history within socius and through the history of bodies. Creation is studied as a the privileged paradigm of rhythm. We postulate a subjective rhythm, an interface between the body and the psyche. Rhythm informs and forms the sensitive matter both bodily and also immaterial, through language. Sensitive inscription of bodies present as well as inscription on the support matter. Thus the place of art, that of artistic form, becomes to the surface of the external inscription of an invasive bodily engram. The quest for form is the quest for place for a nonsuit of the memory. This thesis rests on a dual methodology of research : that of the clinical meeting and that of the pathographic analysis of the work of art, tested with a qualitative and casuistic study, calling upon a clinical reading of the sensitivity of the immateriality of speech and latent discourse. The case study highlights experience of invasion of fantasy and the necessity to resort to sensorial testing informing language and artisitic expression. Confrontation with genital sexuality, after the event, reveals its traumatic potentiality, leading to a renewal of the archaic in terms of object relationship, a source of internal spatiotemporal alienation. By working with rhythm, the artistic work becomes the place of the deterritorialized inscription of formlessness, without however curbing the primitive trace, on the contrary ; but confers a spatiotemporal reconfiguration. From an intrapsychic point of view, rhythm is seen as a non-linguistic element which competes however with the representational system. From a dynamic point of view, rhythm has a changing form serving the liaison-dissociation dialectic, aiming to separate time from internal non-existant space, vector of bonds and historicisation. The study of rhythm opens the perspective for a clinic of sensitive speech of the affected body.

Health Portal Functionality and the Use of Patient-Centered TechnologySimmons, Anita Joyce 01 January 2017 (has links)
Health portals are dedicated web pages for medical practices to provide patients access to their electronic health records. The problem identified in this quality improvement project was that the health portal in the urgent care setting had not been available to staff nor patients. To provide leadership with information related to opening the portal, the first purpose of the project was to assess staff and patients' perceived use, ease of use, attitude toward using, and intention to use the portal. The second purpose was to evaluate the portal education materials for the top 5 urgent care diagnoses: diabetes, hypertension, asthma, otitis media, and bronchitis for understandability and actionability using the Patient Education Material Assessment Tool, Simple Measures of Goobledygook, and the Up to Date application. The first purpose was framed within the technology acceptance model which used a 26-item Likert scale ranging from -3 (total disagreement) to +3 (total agreement). The staff (n = 8) and patients (n = 75) perceived the portal as useful (62%; 60%), easy to use (72%; 70%), expressed a positive attitude toward using (71%; 73%), and would use the technology (54%; 70%). All materials were deemed understandable (74%-95%) with 70% being the acceptable percentage. Diabetes, otitis media, and bronchitis were deemed actionable (71-100%), but hypertension (57%) and asthma (40%) had lower actionability percentages. Hypertension, asthma, and otitis media had appropriate reading levels (6-8th grade). However, diabetes (10th grade) and bronchitis (12th grade) were higher with the target being less than 8th grade level. All handouts were found to be evidence-based. Recommendations were to revise the diabetes and bronchitis educational handouts to improve readability. Social change can be promoted by this project by facilitating positive patient outcomes at urgent care clinics.

Understanding the Differences Between Novice and Experienced Reading TeachersAnderson, Katie 01 January 2018 (has links)
Education researchers have documented that first-year teachers are often less effective at reading instruction than their more experienced peers. Accordingly, this qualitative, comparative case study was designed to assess the instructional skills and strategies utilized by first-year and experienced teachers using Danielson's Framework for Teaching as the conceptual framework. The research questions were used to examine two groups of teachers using the framework and the Teacher's College Reading and Writing Project's defined levels of performance for effective reading instruction. The goal was to identify the instructional differences between the two groups of teachers. Purposeful sampling was used to select 3 first-year and 3 experienced teachers at the 4th or 5th grade levels from 3 different schools across 3 districts in a midwestern state. Data from lesson plans, observations, and interviews were analyzed using an open coding process, followed by axial coding using the Danielson framework to determine the themes of the study. The results indicated that the novice teachers had not developed automaticity in any of the domains of the Danielson Framework. The most challenging domain for novice teachers was instruction, especially communicating with students and using assessment during instruction to meet students' needs. A curriculum plan project consisting of a reading methods course and clinical component was constructed for a local college using the identified underdeveloped skills of novice teachers as actionable data that shaped the development of the plan. Positive social change might be realized as the goal of the plan is to improve teacher quality upon program completion, develop automaticity in reading instruction, and increase K-12 literacy achievement.
