A situação de supervisão psicanalítica de tratamento como escuta infinita

Esta pesquisa problematiza a situação psicanalítica de supervisão de tratamentos com base na experiência da pesquisadora em sua prática numa clínica-escola ligada a uma universidade pública. Para a consecução da proposta, foi delimitada a noção de ‘situação psicanalítica’ considerando existir uma pontualidade relativa a esta noção no âmbito da pesquisa. Consoante esta pontualidade, foi realizada uma reflexão e uma apresentação do método da pesquisa psicanalítica. Nessa perspectiva, a autora apresenta três eixos considerados os nortes da pesquisa: a questão da transferência na situação psicanalítica de supervisão; os efeitos da situação psicanalítica de supervisão na direção de tratamentos e a situação psicanalítica de supervisão numa experiência ‘extra-muros’. Os três eixos são discutidos através de ensaios metapsicológicos, forma de reflexão que se insere na pontualidade inerente ao método de pesquisa psicanalítica. A investigação contribui para as pesquisas psicanalíticas sobre a situação de supervisão de tratamentos, ocorra essa em uma clínica-escola ou em uma instituição de transmissão da psicanálise. A contribuição principal da pesquisa é a abordagem psicanalítica do processo de supervisão, isto é, a leitura da transferência e das formações do inconsciente na situação de supervisão. / This research paper examines the psychoanalytical treatment supervision situation based on the researcher’s practice background at a Public University’s Clinics - School of Psychological Treatment. In order to achieve the proposal’s purpose, the “psychoanalytical situation” notion was delimited taking into consideration the existence of a unique aspect regarding such concept in the scope of the research. In accordance to this specific facet, some thoughts on the subject and the psychoanalytical research method are presented. From this perspective, the author presents three research guiding coordinates: the transference phenomena in the psychoanalytical situation of supervision; the effect of the psychoanalytical situation of supervision on treatment direction and the psychoanalytical situation of supervision in an `extra-walls' experience. The three coordinates are discussed in metapsychological essays, considered to be the inherent reasoning form to such particular aspect of the psychoanalytical research method. This investigation makes contributions to psychoanalytical research on treatment supervision situation, considering its setting at a Clinics-school facility or at a Psychoanalysis transmission institution. The research’s main contribution is the psychoanalytical approach of the supervision process, namely the analysis of the transference phenomena and the unconscious formations in the supervision situation.

Avaliação do ensino na clínica integrada da Faculdade de Odontologia de Araraquara - UNESP em função do planejamento global de tratamento e sua execução /

Orientador: Maria Salete Machado Cândido / Banca: Renato Luiz Terence / Banca: Luiz Antônio Borelli Barros / Banca: Alberto Magno Gonçalves / Banca: Denise Pedrini / Resumo: Foram analisados 176 prontuários gerais da Clínica Integrada da Faculdade de Odontologia de Araraquara - UNESP, referentes aos pacientes atendidos no período de 1999 e 2000, assim como, os planejamentos realizados pelos alunos, através do odontograma para plano de tratamento e sequência dos trabalhos a realizar e confrontados os dados com o tratamento realizado no odontograma de trabalho executado, a fim de determinar a eficácia da disciplina quanto ao cumprimento das necessidades dos seus pacientes. Foram realizadas tabelas e gráficos utilizando o software EPIINFO 6.0 e aplicada a estatística Kappa para conhecer a concordância das atividades realizadas e planejadas. Conclue-se neste periódo de estudo que o grupo etário 25-34 anos foi o mais significativo e o sexo feminino (62,5%) procurou mais atendimento; com relação as necessidades odontológicas encontrou-se que 97,6% de pacientes que necessitaram exodontia; 78% raspagem e polimento dental, 67% restaurações estéticas, 59,5% próteses parciais removíveis, 53,4% próteses unitárias, 46,1% endodontias; 44% próteses fixas; 43% cirurgias periodontais; 22% restaurações de amálgama e 5,7% próteses totais. Para os tratamentos realizados foram encontrados respectivamente: 98,7%; 68,7%; 62,6%; 24,5%; 31%; 44,3%; 8,9%; 38%; 20% e 4,2%. Ressaltando que a disciplina de clínica integrada foi eficaz quanto ao cumprimento do seu objetivo de ensino, voltado para a formação do clínico geral em odontologia apesar de possuir deficiências quanto a satisfação integral das necessidades dos seus pacientes através dos tratamentos evidenciados nos planejamentos globais. / Abstract: The 176 patients who received treatment from 1999 to 2000 in the Integrated Clinics of the Araraquara Dental School - UNESP, wher choused for this study. The treatment register for the treatment plan and clinical sequence of treatment was confronted the data with the treatment accomplished in the plan of the executed treatments, in order determine the effectiveness of the discipline with relationship to the execution of your patient's needs. Table and graphics were accomplished using EPIINFO 6.0 software and to applied the statistics type Kappa (k). It was concluded that during this period of study the groups aged from 25-34 years was the most significant and the women looked more for attendance; in relation to the dental needs it was observed that 97,6% of the patient's need extraction; 78% scaling and dental polishment; 67% esthetic restorations; 59,5% partial removable dentures; 53,4% unitary fixed prosthesis; 46,1% endodontics; 43% periodontal surgery; 22% amalgam restorations and 5,7% total dentures. For the accomplished treatments they were observed respectively: 98,7%; 68,7%; 62,6%, 24,5%; 31%; 44,3%; 8,9%, 38%; 20% and 4,2%. The integrated clinics discipline was effective with the relationship to the execution of your teaching objective, general clinicians formation in dentistry, in spite of deficiencies as the total satisfaction of your patient's needs through the treatments evidenced in the global planning. / Doutor

Atividade, experiência e narrativa : produzindo dispositivos crítico-clínicos / Activity, experience and narrative: producing critic-clinic apparatuses on work

Esta dissertação de mestrado trata de uma investigação teórica por entre as conceituações da Clínica da Atividade e da Ergologia, na direção da produção de uma clínica-crítica do trabalho tomado enquanto experiência. Partimos de leituras a respeito de como o trabalho tem se configurado no contemporâneo, procurando compreender seus desafios e pensar uma clínica do trabalho afeita a fazer frente aos mesmos. Empreendemos um mergulho conceitual pelas abordagens da Ergologia e da Clínica da Atividade, a fim de rastrear os limiares entre os conceitos de atividade e experiência, buscando extrair a potência de tal discussão para o desenvolvimento das clínicas do trabalho. Contamos, ainda, com a contribuição das formulações de Walter Benjamin a respeito da íntima relação entre a experiência e a narrativa, como forma de produção e transmissão de uma história coletiva aberta, criadora de novos sentidos e transformadora dos sujeitos e do mundo. Concluímos que o conceito de atividade, quando inflexionado pelo conceito de experiência, nos permite expandir as possibilidades de análise e intervenção no campo do trabalho. Ao explorarmos a intimidade entre os conceitos de narrativa e experiência, tal como proposta por Walter Benjamin, esta inflexão nos permite, ainda, produzir dispositivos de intervenção junto aos trabalhadores na direção de uma clínica do trabalho que se vale da produção da própria história dos ofícios para produzir expansão das potências de ação coletiva e, por consequência, da produção de saúde no e pelo trabalho. Neste ponto da discussão, encontramos em Michel Foucault e suas formulações a respeito de experiência e de crítica, potentes operadores conceituais a dialogarem com as formulações clínicas do trabalho, de maneira a nos dirigirmos a uma clínica-crítica neste âmbito, aquela capaz de fazer frente aos poderes subjetivantes que marcam o trabalho no contemporâneo. / This Masters thesis consists of a theoretical research which passes through the conceptualizations of the Clinic of Activity and Ergology approaches, aiming at the critic-clinic production of work as experience. Our starting point are analysis concerning how work has been set on contemporaneity, trying to comprehend its challenges, in order to think a clinic of work which is interested in facing them. We employ a conceptual immersion in the Ergology and the Clinic of Activity approaches, tracking the boundaries of the concepts of activity and experience, in an effort to bring out the potency this discussion can impart in the development of the clinics of work. We also lay hold of Walter Benjamin's formulations regarding the intimate relationship between experience and narrative, as a mean of production and transmission of an open and collective story; a story which create new senses/meanings and transform the subjects and the world. We conclude that the concept of activity, when inflected by the concept of experience, allows us to expand the possibilities of analysis and intervention in work. By exploring the intimacy between the concepts of narrative and experience, as Walter Benjamin proposed, this inflection allows us the production of intervention apparatuses together with the workers in the direction of a clinic of work which relies on the production of the craft's own history to expand the powers of collective action and, therefore, of the production of health at and by work. At this point in our discussion, we find powerful conceptual operators in Michel Foucault and his formulations regarding experience and critic, conceptual operators which we put in dialogue with the clinic formulations of work, aiming at a critic-clinic of work, a clinic capable of facing the subjectifying powers that mark the work on contemporaneity.

FundaÃÃo Cearense de Apoio ao Desenvolvimento Cientifico e TecnolÃgico / From the first decade of this century, there is a relevant phenomenon in Fortaleza Center: the expansion of popular medical clinics, private establishments that perform low-complexity procedures such as consultations, exams and health treatments aimed at low-income population. These new forms of medical care are associated with strong demand of a population seeking health services in public hospitals and is not answered, forming a pent-up demand. The insertion of the popular health services revealed a new scene to the Centro neighborhood through soil recovery and rescue areas before depreciated. Therefore, this study aimed to understand the relationship between the insertion of popular medical clinics and deficiencies present in the Unified Health System (SUS), based on authors who deal with the socio-spatial dynamics in urban geography and the distribution of health services network in the country. The research discusses the position of state health on the national scene, the regional influence of health services in Fortaleza, the importance of popular clinics on the precariousness of the public health system and the recent pattern of changes in Brazilian household consumption, in addition to addressing the refunctionalization Center neighborhood driven by the installation of the popular medical clinics. / A partir da primeira dÃcada do sÃculo XXI, observa-se um relevante fenÃmeno no Centro de Fortaleza: a expansÃo das clÃnicas mÃdicas populares, estabelecimentos privados que realizam procedimentos de baixa complexidade como consultas, exames e tratamentos de saÃde voltados à populaÃÃo de baixa renda. Essas novas formas de atendimento mÃdico estÃo associadas à forte demanda de uma populaÃÃo que busca por serviÃos de saÃde em hospitais pÃblicos e nÃo à atendida, formando uma demanda reprimida. A inserÃÃo dos serviÃos populares de saÃde revelou um cenÃrio novo ao bairro Centro, por meio da valorizaÃÃo do solo e do resgate de Ãreas antes depreciadas. Portanto, esta pesquisa buscou compreender a relaÃÃo existente entre a inserÃÃo das clÃnicas mÃdicas populares e as deficiÃncias presentes no Sistema Ãnico de SaÃde (SUS), com base em autores que tratam da dinÃmica socioespacial na geografia urbana e da rede de distribuiÃÃo dos serviÃos de saÃde no paÃs. Essas interrogações incidem sobre a própria nomeação que tais casos recebem nessa área; sobre a prática fonoaudiológica e suas articulações com a Psicanálise; sobre as improvisações em que o tempo da constituição de um sujeito falante é composto. Os (a)casos com os quais a Fonoaudiologia se depara e se surpreende no exercício de sua prática clínica impuseram esta escrita: não há criança sem fala; há crianças que exigem um tempo singular na sua constituição como falante. A clínica fonoaudiológica atua sobre os retardoS de fala, reconhecendo a pluralidade e a particularidade que está em cena no atendimento dessas crianças. Escutar no silêncio da criança uma fala que se improvisa e sustentar uma prática em um tempo outro da constituição dessa criança como falante é possível, quando se toma a interdisciplinaridade como fundamental na atuação clínica. São essas parcerias que permitem compassos no silêncio / Abstract: This thesis was written in view of some issues raised in the treatment of children presenting the so-called ¿delayed speech acquisition¿. These issues focus on the very names used in this area, on the Speech Therapy practice and its articulations with Psychoanalysis; and on the improvisations that also compose the time of constitution of a speaking subject. The cases (as well as chances) ¿ and the surprises ¿ faced by Speech Therapy imposed this work: there is no child without speech; some children demand a singular time for their constitution as a speaker. The clinic treats delayS in speech acquisition, taking into account the plurality and the particularity at work while these children are being treated. It is possible to hear in the child's silence a speech improvisation and sustain a certain practice in a different time of the constitution of this child as a speaker, as long as one takes interdisciplinarity as a fundamental approach to the clinical practice. Factors influencing the uptake of long acting reversible contraceptives among women at primary health clinics in eThekwini District

Unintended pregnancy is a major global challenge among sexually active women of childbearing age. Non-use of modern contraception and inconsistent use of short term contraceptive methods are the main reasons associated with unintended pregnancy. Long acting reversible contraceptives (LARCs) have proven to be highly effective with good continuation rates, and are cost-effective compared to other methods, when used more than one year. However, there is low uptake of LARC methods globally and in South Africa. Aim of the study. The aim of the study was to determine factors influencing the uptake of LARC methods among women at the primary health care (PHC) clinics in eThekwini District. Methodology. A quantitative, descriptive survey was used in this study. Purposive sampling of six fixed PHC clinics from the three sub-districts was done. Convenience sampling resulted in 371 participants. A survey questionnaire in English and isiZulu was used to collect data. Data was analysed using SPSS version 23.0. Inferential statistics were used to determine the relationship between the variables. Results. The results of this study revealed that the contraceptive injection was most common LARC used, and the least used method was the intrauterine contraceptive device. The side effects were the main reasons for discontinuation with LARCs and all other contraceptive methods. Irregular vaginal bleeding was the main side effect cited by respondents associated with contraceptive implant usage. The respondents had positive attitudes and perceptions towards LARCs; however, the majority of respondents were not interested in using LARC methods. Common myths and misconceptions were not negatively associated with LARCs, since respondents disagreed with them all. / M

M.Cur. / In a changing South-Africa new challenges regarding multiculturality emerge. In psychiatric nursing the psychiatric nurse also faces new challenges. During a cultural discussion, the personnel of a private psychiatric clinic realised that culture has numerous different meanings. (that numerous different meanings may be attached to culture). Culture has an interesting nature and certain expectations exist around culture. I have described this discussion in order to present a qualitative, descriptive, singular case study, within the paradigm of the Theory for Health promotion in Nursing. The data was processed according to the recommendations of Tesch (in Creswell, 1994:155). The discussion is presented according to certain themes. No guidelines have been set, but I have made certain recommendations that may be utilised in training of psychiatric nurses as well as in practise and research in the field of psychiatric nursing. The discussion was important because the participants were able to develop a sensitivity toward culture. This lead to the promotion of co-operation, productivity, and mutual understanding and respect within the context. The importance of the description of the discussion lies in the information that has been generated. This information will be available and will be incorporated as part of the body of knowledge in psychiatric nursing.

The nurse in the national family planning programme of South Africa

(Nursing Administration) / In this study the activities carried out by registered nurses working in family planning are assessed as well as the training nurses receive to equip them to function satisfactorily. Registered nurses are the main providers of family planning services in South Africa and function in an extended role to a far greater degree than nurses in most developed and developing countries. The study shows that registered nurses form a stable workforce. They perform a large number of family planning tasks but also spend a great deal of time on clerical and housekeeping tasks which limit the time available for quality client care. The need for more regular updating of clinical knowledge is evident as is the need for nurses to be kept informed on changes taking place in the Family Planning Programme. The study reveals that registered nurses are interested in and have a positive attitude towards family planning. If registered nurses are relieved of non-nursing tasks they will be able to meet the growing need for family planning services.

Professional health care workers' experiences of care at two Community Day Clinics on the Cape Flats

Magister Artium (Social Work) - MA(SW) / Primary Health Care (PHC) is the cornerstone of health care globally, nationally and locally and, therefore, should be regarded as the foundation of health care provision. In South Africa, Community Day Clinics (CDCs) are part of the bouquet of services that is being offered at a PHC level. There are various factors that generate inconsistency in the provision of care to people accessing these CDCs. The purpose of this study was to identify and explore how these factors impact on the care practices that health care professional’s provide. Research suggests that the majority of health care workers are women, who play a double role as carers in their professional and private lives. Therefore, the political ethics of care, a feminist theoretical approach, was utilized to understand care practices in these health settings. The aim of the study was to develop an in-depth understanding of the care practices of health care workers at two CDC facilities on the Cape Flats. A qualitative research methodology was used to explore and identify the phenomenon under study. The research project followed an explorative and descriptive research design, as the researcher sought to understand the care practices of health care workers and how their values and ethics further influenced care practices at these two CDC settings. The data was gathered using semi-structured one-on-one interviews, and later analysed using qualitative thematic analysis. The research findings were grouped in terms of the values entrenched in the political ethics ofcare, which are attentiveness, responsibility, competence, responsiveness and trust. The research findings identified various aspects that, either negatively or positively, impact on these values. Finally, recommendations were made to management, as well as care workers. These recommendations were in terms of implementing care services that are attentive to service-users and care-workers; providing a service that takes into consideration the value of responsibility; the provision of competent services; and finally creating trusting relationships within the CDC.
