Sjuksköterskans erfarenheter av akut konfusion hos äldre i akutsjukvård – En kvalitativ intervju studie / Nurses´ experience of acute confusion in elderly patients in emergency care – A qualitative interview study

Titel: Sjuksköterskans erfarenhet av akut konfusion hos äldre i akutsjukvård – En kvalitativ intervjustudie. Bakgrund: Äldre som vårdas i akutsjukvård har en incidens mellan 11–51% att utveckla akut konfusion under vårdtiden. Tillståndet är förenat med en ökad sjuklighet och dödlighet hos äldre. Multifaktoriella orsaker ligger bakom att äldre drabbas. Akut konfusion är en klinisk diagnos, idag finns inga biomarkörer eller undersökningar som kan identifiera tillståndet. Akut konfusion kräver en tidig upptäckt, snabb utredning och ett medicinskt omhändertagande, god omvårdnad och en anpassad vårdmiljö för att patienten skall återhämta sig. Syfte: Att beskriva sjuksköterskans erfarenhet av att upptäcka, förebygga och vårda patienter med akut konfusion i akutsjukvård. Design: En kvalitativ intervjustudie med fem sjuksköterskor verksamma i geriatrisk akutsjukvård på två sjukhus i södra Sverige. Resultat: I resultatet framkom tre kategorier; strategier för att identifiera och bedöma tecken på akut konfusion, strategier och hinder för förebyggande insatser och god omvårdnad vid akut konfusion, förslag på förbättringar i vården av äldre med akut konfusion. Sjuksköterskan använde sig av observation, kommunikation och inhämtning av information från journal och anhöriga för att identifiera akut konfusion. En helhetssyn i omvårdnaden, kontinuitet i vårdkontakten, anpassning av vårdmiljön och anhörigas delaktighet var viktiga faktorer för att förebygga akut konfusion och ge god omvårdnad. Hindrande faktorer orsakades av brister i vårdmiljön, för lite resurser och tid samt bristande kunskap hos övrig vårdpersonal kring lämplig bemötande och förhållningssätt. Slutsats: Ett evidensbaserat screeninginstrument som identifierar olika konfusionstillstånd skulle underlätta bedömning och dokumentation för sjuksköterskor. En personcentrerad vård som bygger på att omvårdnaden planeras utifrån en helhetssyn och en kontinuitet i vårdkontakten samt att anhöriga görs mera delaktiga i vården förespråkas av sjuksköterskorna. Vårdmiljön skall anpassas, vara lugn och stödjande till sin utformning. Personal som arbetar med konfusoriska patienter behöver ha mer kunskap om lämpligt bemötandet och förhållningssätt för att underlätta patientens återhämtning. / Title: Nurses experience of acute confusion in elderly patients in acute emergency care- A qualitative interview study. Purpose: To describe nurses experiences in detecting, preventing and caring for patients suffering from acute confusion in emergency care. Design: A qualitative interview with five nurses working within geriatric emergency care at two hospitals in the south of Sweden. Result: Three categories emerged in the result; strategies to identify and assess signs of acute confusion, strategies and hinders for preventative measures and good nursing upon acute confusion, and improvement proposals for the care of elderly suffering from acute confusion. Nurses used observation, communication, and gathering of information from medical records and relatives in order to identify acute confusion. A comprehensive view in care, continuity in contacts, adaption of health care environment and relatives engagement were important factors to prevent acute confusion and provide good nursing. Hindering factors were a result of shortcomings in the health care environment, insufficient resources and time, and lack of knowledge among other requisite staff regarding appropriate reception and approach. Conclusion: An evidence-based screening instrument which identifies various states of confusion would render assessment and documentation easier for nurses. Personal centered care built upon that such is planned based on a comprehensive view and continuity in contacts, and that relatives are more engaged in care, is advocated by the nurses. Health care environment shall be adapted, be calm and supportive in form. Staff that work with confused patients need more sufficient knowledge regarding appropriate reception and approach to render patient recovery easier.

The planetary crisis is a factor with undoubted effects on society. Education in Sweden concerns itself with preparing students for societal life through passing on values and emphasising the need for critical thinking. Many students today engage in environmental activism with the agenda to demand change from governing bodies to mitigate climate change. To understand the process of change, additional information is needed regarding what it means to be human. This text concerns itself with environmental teaching for the EFL-setting. Through focusing on literature, and contributing to an in-depth understanding of the characters, increased awareness about what it means to be human can be achieved. By selecting the short story “Bears Discover Fire,” it is illustrated how such an understanding can be found through fictional works. This paper covers an ecocritical reading focusing on how representation, relationship, and structures detrimental for the climate figure in Terry Bisson’s story. In addition, it argues that a deeper understanding of the characters results from applying the biophilia hypothesis. The findings from the analysis of characters and the representation of nature and relationships are applied to the EFL-setting, providing examples for how teaching can be conducted to promote further activism and strengthen the relationship between students and nature.

Denna kvalitativa studie syftar till att, genom semistrukturerade intervjuer,undersöka professionellas erfarenheter i arbetet med barn som utsätts för våld. Intervjuerna inkluderade fyra socionomer som jobbar med olika uppgifter, skolkurator, socialsekreterare på mottagningsenheten för inkomna anmälan, socialsekreterare på utredningsenheten och socialsekreterare på kriscentrum som jobbar med frivilliga insatser för personer utsatta för våld. Valet av intervjupersonerna landade också på att de jobbar inom olika kommuner för att påvisa om det finns någon skillnad i arbetet, beroende på kommunens storlek. I syfte att besvara frågeställningarna framställdes en intervjuguide, som med sin tematiska uppbyggnad skulle följa hela processen, från att upptäcka våldsutsattheten till att bevilja insats eller avsluta ett ärende. De teoretiska grunderna i studien, organisationsteorin, anknytningsteorin, handlingsutrymme,samt maktteori är också en grund till besvarande av frågeställningarna. Förutom intervjuguiden och de teoretiska utgångspunkterna lägger bakgrunden och tidigare forskning en viktig grund att förstå vad våld är, vilka konsekvenser det kan innebära för barnet, samt vilka svårigheter de professionella kan stöta på i sitt arbete. Resultatet påvisade att det kan vara väldigt svårt att upptäcka våldsutsatthet hos ett barn, dels för att barn inte vågar berätta om det, dels att symtomen kan vara antingen uteblivna eller vaga för att kunna avgöra att det rör sig om misshandel. Intervjupersonerna uppger att det är viktigt att vara säker att det rör sig om misshandel eftersom en anmälan kan utgöra en stor förändring för barnet och familjen, men samtidigt ska tecken på våld aldrig ignoreras. Relationsskapande med barnet uppges vara den viktigaste förutsättningen för att både upptäcka våld och hjälpa barnet. Men, för att hinna skapa en relation kräver tid och det kan vara svårt eftersom lagen säger att det finns en begränsad tid för alla moment i handläggningsprocessen. / This qualitative study aims to investigate, through semi-structured interviews, the experiences of professionals in working with children who are exposed to violence. The interviews included four social workers who work with different tasks, school counselor, social worker at the reception unit for reports, social worker at the investigation unit and social worker at the crisis center, who work with voluntary efforts for people exposed to violence. The choice of the interviewees also landed on the fact that they work in different municipalities to show if there is any difference in the work depending on the size of the municipality. In order to answer the questions, an interview guide was produced that, with its thematic structure, would follow the entire process, from detecting the exposure to violence to granting intervention or closing a case. The theoretical foundations of the study, organizational theory, attachment theory, room for manoeuvre and power theory are also a basis for answering the questions. In addition to the interview guide and theoretical starting points, the background and previous research lay an important foundation for understanding what violence is, what consequences it can mean for the child and what difficulties the professionals may encounter in their work. The results showed that it can be very difficult to detect exposure to violence, partly because children do not dare to tell about it, and partly because the symptoms can be either absent or vague in being able to determine that it is abuse. The interviewees state that it is important to be sure that it is an abuse because a report can be a big change for the child and the family, but at the same time signs of violence should never be ignored.Relationship building with the child is stated to be the most important prerequisite for both detecting violence and helping the child. But, to have time to create a relationship requires time and it can be difficult because the law says that there is a limited time for all steps in the processing process.

A community college evaluation of DISCOVER and Virginia VIEW

The purpose of this study was to evaluate a computer-assisted career information delivery system (CIDS), Virginia VIEW, with a computer-assisted guidance system (CAGS), DISCOVER, at a Virginia community college using parts of Shealy' s 1982 evaluation model patterned on the National Occupational Information Coordinating Committee's specifications. User impact, user satisfaction, and economic efficiency components were studied. Screened volunteers in this five-week unstructured study were randomly assigned to four treatment groups of 25 students each: (A) Virginia VIEW; (B) Virginia VIEW and DISCOVER; (C) Control, wait for five weeks before using DISCOVER or VIEW; and (D) DISCOVER. Program completers (A=l7), (B=l8), (C=l9), and (D=20) took the Career Development Inventory (CDI) developed by Super and the Career Decision Scale (CDS) developed by Osipow as pre- and post-test instruments. Counseling assistance was available at all times upon request and records were kept on CIDS/CAGS User Logs. Student Planning and Counselor's Observation Logs were also kept. Based on the pre- and post-test results of the CDI and CDS, students did not differ in the affective, behavioral, and cognitive areas of career development and decision making at the end of five weeks (user impact). Students did not differ in their opinions (user satisfaction) of the information development, information delivery, or user service components of the two systems. Results on economic efficiency revealed that Virginia VIEW was less expensive per student user, while DISCOVER was less expensive per student hour of use. There were no clear differences in the three areas evaluated by this study between Virginia VIEW or DISCOVER. It was recommended that Shealy' s model be used again to evaluate the Interactive Virginia VIEW against DISCOVER and SIGI. Further research was also recommended in the use of the weekly Quantitative and Qualitative Student Career Planning Log. / Ed. D.

This essay will address the topic of child abuse through healthcare interventions, ormore precisely medical child abuse - which is also called Münchausen syndrome byproxy. A social problem that is quite unusual, and therefore creates complexity. Witha quantitative method, the essay is based on a web-based survey, which social workerson the Swedish social services at the children- and family units in Småland haveresponded to. The questions in the survey processes the social workers' knowledgeand preparedness which will be analyzed by their discretionary space and reasoning.These themes are also linked to Lipsky’s theory of street-level bureaucracy which isused to analyze the empirical data of the study. The results from the essay are analyzedand tested through the statistics program SPSS. The results and analysis show forinstance that the longer the social workers have been working at their units and insocial work in general, the more knowledge they considered themselves to have. Theyalso possessed a higher readiness the longer they worked at the unit. They are morerecon with the term “Münchausen by proxy” than “Medical child abuse”. Regarding guidelines and legislation, they feel uncertain, which we connect to the knowledge-gaps that comes with the social problem. Medical child abuse in the Swedish social services is quite unexplored and with this essay we want to contribute to a betterresearch, to help the children who are victims of medical child abuse.

The Young Adult novel Aristotle and Dante Discover the Secrets of the Universe portrays the coming out stories of two queer Mexican American boys who struggle with their ethnicity and sexuality, which leads to internal conflicts. The focus of the analysis in the essay is to investigate how the novel can be taught in Upper Secondary classrooms through queer pedagogy and anti-oppressive approaches focusing on silenced discourse. When teachers allow their students to work with the coming out story in the novel within this framework, teachers can engage their students in exploring and finding meaning in the silences that the protagonists Aristotle and Dante speak through. The anti-oppressive practice of reading for silences allows students to look beyond what is being represented in the novel in regard to Aristotle’s and Dante’s sexuality and ethnicity and thereby find meaning in what is unsaid.

Active Approaches provide chances for individual, comprehension-oriented learning and can facilitate the acquirement of general mathematical competencies. Using the example of pinboards, which were developed for different areas of the secondary level, workshop participants experience, discuss and further develop learning tasks, which can be used for free activities, for material based concept formation, for coping with heterogeneity, for intelligent exercises, as tool for the presentation of students’ work and as basis for games. The material also allows some continuous movements and can thus prepare an insightful usage of dynamic geometry programs. Central Part of the workshop is a work-sharing group work with learning tasks for grades 5 to 8. The workshop will close with a discussion of general aspects of material-based learning.
