Trastorno por estrés postraumático en menores que han sufrido maltrato familiar: Directo y exposición a violencia de género

Esta investigación tiene como objetivo estimar la presencia del Trastorno por Estrés Postraumático (TEPT) en menores que han sufrido maltrato intrafamiliar crónico y presentan alteraciones psicológicas graves. La muestra está formada por 102 menores entre 8 y 17 años. Se divide en dos grupos: I (64 menores que viven con su familia biológica y han estado expuestos/as a violencia de género) y II (38 menores tutelados/as por la Administración). El instrumento utilizado es el Child PTSD Symptom Scale (CPSS) de Foa et al. (2001). Las conclusiones son: los criterios DSM son muy exigentes y no sensibles para detectar TEPT en infancia; es necesario valorar la afectación subsindrómica; el criterio de Evitación es demasiado restrictivo. Si se baja el umbral, tal como propone el DSM-V, se mejora la detección de casos; se encuentra mayor prevalencia de TEPT en chicas; no se hallan diferencias significativas entre grupos de edad ni entre submuestras.

Emotion Perception in Asperger's Syndrome and High-functioning Autism: The Importance of Diagnostic Criteria and Cue Intensity

Asperger's syndrome (AS) is a pervasive developmental disorder that is associated with marked social dysfunction. Deficits in the perception of nonverbal cues of emotion may be related to this social impairment. Research has indicated that children with autism are limited in their emotion perception abilities, but studies that have addressed this issue with individuals with AS or high-functioning autism (HFA) have yielded inconsistent findings. These inconsistencies may be related to methodological differences across studies including diagnostic criteria and failure to consider the intensity of the emotion cues. It was hypothesized that children with AS and HFA would both have deficits in emotion perception compared to typically-developing children. However, children with HFA were expected to have an even greater emotion perception deficit than children with AS and this difference was hypothesized to be most pronounced for low intensity cues of emotion. It is important to clarify whether individuals with AS and HFA differ in emotion perception because most studies of this skill combine them into one group or use poorly defined diagnostic criteria. This study examined the ability of 30 8- to 15-year-old children with either AS or HFA to perceive emotion from high and low intensity cues. In order to address limitations with the differential validity of the DSM-IV criteria for AS, diagnoses were based on diagnostic criteria proposed by Klin et al. (in press). A researcher who was blind to diagnosis administered a test that presented low and high intensity cues of emotion in photographs of facial expression and audiotapes of tone of voice. Comparison of the emotion perception accuracy of children with AS to the normative means of this instrument for typically-developing children did not reveal any significant differences. In contrast, the children with HFA were significantly less accurate in their perception of facial expressions and tone of voice than the normative sample and the participants with AS. Contrary to expectations, IQ was significantly related to emotion perception accuracy. After controlling for IQ, the difference in perception of facial expressions between children with AS and HFA was not significant. On the other hand, cue intensity moderated the relation between diagnosis and emotion perception accuracy for tone of voice even after IQ was taken into account. Children with AS perceived high and low intensity tone of voice cues with similar accuracy, but children with HFA had significantly poorer performance on the low intensity tone of voice cues. Although emotion perception accuracy was related to better adjustment, it was not correlated with the most sensitive measure of current social functioning. This suggests that even when children with AS or HFA perceive cues correctly, they may not know how or be able to properly integrate them for adaptive responses in social interaction. The findings have important implications for understanding inconsistencies in past research and identifying future directions.

There has been an increase of attention placed on the diagnosis of Attention Deficit Hyperactivity Disorder (ADHD), within South Africa. This has led to a number of controversies surrounding the legitimacy of ADHD diagnoses. And how effective the systems of categorising and diagnosing disorders are in aiding a number of practitioners in formulating a disorder. There is a substantial agreement within the literature that the understanding of ADHD is limited, the focus is mainly on the symptoms of disorders. This study explores the perceptions practitioners in the field, in identifying the effectiveness of the Diagnostic and Statistical Manual for Mental disorders (DSM) is for diagnosing ADHD. It became evident throughout this study that there is no consensus around the efficacy of the DSM. Furthermore, ADHD is not completely understood and therefore creates serious implications for the treatment and diagnosis of the disorder.

Personality Factors, Obsessive-Compulsive Behavior, and Sexual Fantasy as Predictors of Paraphilic Disorder Intensity

Researchers vary on their definitions of paraphilia. A difference exists between an individual possessing a paraphilia versus an individual possessing a paraphilic disorder. Hanson (2010) proposed a dimensional model of sexual deviance that includes a measure of intensity. However, research on sexual intensity has been lacking. A majority of existing research focuses on the potential risk factors of possessing a paraphilia or paraphilic disorder (e.g., criminality). There is less focus on whom in the population has the potential to develop a paraphilia; or which factors predict paraphilic behavior. The Big Five personality factors (openness to experience, conscientiousness, extraversion, agreeableness, and neuroticism), obsessive-compulsive behavior, and sexual fantasy (exploratory, intimacy, impersonal, and sadomasochism) were used to predict paraphilic intensity using the Edwards Paraphilic Inventory (EPI). Surveys were placed on Amazon Mechanical Turk (n = 100), the Celebrity Feet in the Pose website (via https://celebrityfeetinthepose.com), and its social media (n = 163) to reach a total of 263 participants. Results indicated that obsessive-compulsive behavior, sadomasochism, and agreeableness significantly predicted the level of paraphilic intensity. Such findings support that paraphilic disorders are likely obsessive-compulsive in nature. Furthermore, agreeableness and paraphilic intensity were negatively correlated. This suggests that the lower the individual is in agreeableness, the higher the likelihood he or she falls on the paraphilic spectrum. Lastly, those who practice sadomasochistic roleplay in the bedroom are likely to report higher levels of paraphilic intensity. According to the United States sample, 1 out of every 10 participants reported some type of paraphilic activity. Individuals who participated in the survey from the website self-reported higher levels of paraphilic behavior than those who completed the survey from Amazon Mechanical Turk. In addition, these individuals are represented in more than one paraphilic category. It remains unclear how large of a role pleasure plays in an individual seeking therapeutic or pharmacological help with paraphilic disorders. Pedophilic disorder was not examined due to ethical concerns with the United States and other various countries. Future research should examine education level and sexual orientation as predictors of paraphilic intensity.

Eidolon: adapting distributed applications to their environment.

Grids, multi-clusters, NUMA systems, and ad-hoc collections of distributed computing devices all present diverse environments in which distributed computing applications can be run. Due to the diversity of features provided by these environments a distributed application that is to perform well must be specifically designed and optimised for the environment in which it is deployed. Such optimisations generally affect the application's communication structure, its consistency protocols, and its communication protocols. This thesis explores approaches to improving the ability of distributed applications to share consistent data efficiently and with improved functionality over wide-area and diverse environments. We identify a fundamental separation of concerns for distributed applications. This is used to propose a new model, called the view model, which is a hybrid, cost-conscious approach to remote data sharing. It provides the necessary mechanisms and interconnects to improve the flexibility and functionality of data sharing without defining new programming models or protocols. We employ the view model to adapt distributed applications to their run-time environment without modifying the application or inventing new consistency or communication protocols. We explore the use of view model properties on several programming models and their consistency protocols. In particular, we focus on programming models used in distributed-shared-memory middleware and applications, as these can benefit significantly from the properties of the view model. Our evaluation demonstrates the benefits, side effects and potential short-comings of the view model by comparing our model with traditional models when running distributed applications across several multi-clusters scenarios. In particular, we show that the view model improves the performance of distributed applications while reducing resource usage and communication overheads.

Syftet med detta arbete har varit att undersöka om värdeflödesanalys inom produktutveckling kan hjälpa företag att effektivisera utvecklingen av produkter. Metoden för värdeflödesanalys som vi valt att prova har tagits fram av MIT. Arbetets syfte var att pröva denna metod, detta gjordes genom att utföra två studier. Modellen föreskriver att en kartläggning görs i tre steg: först genom ett ganttschema, därefter flödesschema och till sist en DSM. Datainsamlingen är gjord med hjälp av intervjuer och en enkät, enligt Millards rekommendation. Med hjälp av metoden och stöd av litteratur kartlades och analyserades flödet. Bland annat framkom följande åtgärdsförslag: Tydligare ansvarsområden och klara rutiner Utför FEM-beräkning mer parallellt med konstruktionsarbetet Tydligarepull event för att klargöra varje aktivitets syfte. Vi kan konstatera att resultaten av värdeflödesanalysen har tagits väl emot av företagen, både när det gäller kartläggning och åtgärdsförslag. Detta leder oss fram till vår slutsats att modellen har varit till hjälp för företagen och därmed får anses fungera.</p>

This book is an attempt to formulate a sociological theory of the phenomena which is otherwise known as substance abuse and substance dependency in the medical field. The first step toward such a theory is a critique of how DSM-IV presents diagnostic criteria for substance abuse and substance dependency as if these criteria are valid and reliable ways of measuring mental illness and bodily dysfunction. I point to the fact that these criteria neither are valid or reliable variables for measurement nor theoretically grounded defi nitions of the phenomena. My point is that diagnosis is an elaborate way of disguising moral judgments as medical assessments of illness or dysfunction. The second step towards the formulation of my theory goes via my empirical study of nine men and women who call themselves sex and love addicts. By conducting deep interviews with them I conclude that it is impossible to talk about sex and love addiction without constantly referring to stereotypes and widely held assumptions of alcoholics and drug addicts. Moreover it is apparent that these men and women are making moral judgments about themselves rather than pointing to some form of genuine disruption which they cannot control. From my critique and my empirical study I am able to identify a number of areas that a theory of addiction, as opposed to the medical view where this phenomena are being broken down in two sub categories (abuse and dependency), must be able to handle to explain addiction generally and specifically in relation to alcohol, drugs, sex and other forms of social deviance. A number of philosophical hallmarks of medicine is also identifi ed and seen as part of the problem, therefore I mean that a successful theory of addiction must transcend the mind body dualism of Descartes as it is a cornerstone in the medical view upon and understanding of humans. By reconstructing the habitus and field concepts in Pierre Bourdieus theory of practice I mean that it is possible to understand addiction from an action theory point of view. This discussion is also broadened by a pragmatist discussion of the mind and a neo meadian theory of the emotional self. My conclusion is that addiction can be understood and explained within an action theory which focus upon individual as well as collective action and understand this as habitual practice that is partly embodied and therefore not discursive – habits of which we cannot easily speak is seen as one of the compelling components in addiction rather than a mythical loss of control which is nowhere to be found but as a rhetoric grip stemming from the AA view upon addiction as a disease.

The relevance of age at first alcohol and nicotine use for initiation of cannabis use and progression to cannabis use disorders

Background: A younger age at onset of use of a specific substance is a well-documented risk-factor for a substance use disorder (SUD) related to that specific substance. However, the cross-substance relationship between a younger age at onset of alcohol use (AU) and nicotine use (NU) and the risk of cannabis use disorders (CUD) in adolescence and early adulthood remains unclear. Aims: To identify the sequence of and latency between initial AU/NU and initial cannabis use (CU). To investigate whether younger age at AU- and NU-onset is associated with any and earlier CU-onset and a higher risk of transition from first CU to CUD, taking into account externalizing disorders (ED) and parental substance use disorders as putative influential factors. Methods: Prospective-longitudinal community study with N = 3021 subjects (baseline age 14–24) and up to four assessment waves over up to ten years with additional direct parental and family history information. Substance use and CUD were assessed with the DSM-IV/M-CIDI. Results: Most subjects with CU reported AU (99%) and NU (94%). Among users of both substances, 93% reported AU prior to CU (87% for NU). After adjustment for ED and parental substance use disorders younger age at AU-onset was associated with any CU. Younger age at NU-onset was associated with earlier CU initiation. Younger age at AU- and NU-onset was not associated with a higher risk of CUD. Conclusions: The cross-substance relevance of younger age at first AU and NU for the risk of CUD is limited to early CU involvement.
