In a first moment, the clinical material collected from the treatment of three psychotic patients attended by the author is built in the form of three narratives. Secondly, many pieces of work that approach the essential subject of this research, in a very meaningful way and under quite singular perspectives, are examined. More specifically, those pieces of work focus on setting in three contexts: in the psychoanalytical clinic, exactly as it would be practiced in the offices; in the institutional sphere; in the Therapeutic Attendance. Lastly, the author of the study got to the conclusion that the clinical setting, adapted to the peculiar needs of the psychotic patient, would present, among many other possibilities, the following characteristics: flexibility or elasticity; consistency; steadiness; materiality; a non-intrusive quality / O presente estudo tem como o seu objetivo maior distinguir alguns aspectos do setting clínico que, no tratamento de pacientes psicóticos, possivelmente desempenhariam um importante papel. Num primeiro momento, o material clínico extraído de três tratamentos de pacientes psicóticos que foram conduzidos pelo autor é elaborado na forma de três narrativas. Em seguida, são examinados vários trabalhos que abordam, de um modo significativo e desde perspectivas bastante singulares, o assunto fundamental desta pesquisa. Mais precisamente, tais escritos versam sobre o setting em três contextos: na clínica psicanalítica, tal como ela seria praticada nos consultórios; no âmbito institucional; no Acompanhamento Terapêutico. The dissertation was defended in 1998 in the context of the laboratory of Fundamental Psychopathology of the Center for Psychoanalysis in the Graduate Study Program in Clinical Psychology at the Catholic University of São Paulo. This thesis does not have the objective of defending any classificatory structural position related to the need for a differential diagnosis of anorexia and bulimia as clinical entities. It will defend the perspective that disorders of orality are symptomatic manifestations of melancholia, a narcissistic neurosis. This perspective does not rule out the possibility of there also being disorders of orality in other defensive structures, such as in hysteria or perversion. In other words, anorexia, bulimia, obesity and addictions in general are symptomatic expressions that can occur in melancholic dimensions of the transference neuroses and of perversion. The superego being a structural dimension of the psychic apparatus, the shadow of the object can fall on the ego, regardless of the subject's psychopathological structure. This means that narcissistic neurosis has dynamics that are relatively independent of the transference neurosis where, contrary to narcissistic neurosis, the conflict takes place between id and ego / Esta tese não terá como objetivo defender uma posição estruturalclassificatória que responde à necessidade de um diagnóstico diferencial da anorexia e da bulimia como entidades clínicas. Defenderá o ponto de vista de que os distúrbios da oralidade são manifestações sintomáticas da melancolia, uma neurose narcísica. Este ponto de vista não exclui a possibilidade de haver distúrbios da oralidade em outras estruturas defensivas, como por exemplo, na histeria ou na perversão. It is broadly agreed, that migration has clear implications for the individual, her family, her community, and the nation as a whole. Listening to patients in clinical settings, showed that suffering associated with migration had as undercurrents stories of how these individuals were psychologically built-up. We raise the hypothesis that migration in some cases could be an exit attempt from certain psychological impasses. Our objective therefore became to identify and investigate the origins and vicissitudes of this kind of psychological movement that we found in the geographical dislocation of certain subjects. This psychoanalytical study, within the field of fundamental psychopathology, allowed us to identify a movement we called an attempt of parental (re)construction. From the clinical case, we were able to formulate three streams of parental (re)construction: Migration and Foreignness / paternal (re)construction, Migration as the space in between and Migration and the second mirror / maternal (re)construction / Os problemas em torno da migração, como surtos psicóticos, depressão, alcoolismo, desenraizamento, dificuldade de adaptação e estresse são apontados em muitos estudos, revelando a importância do tema no campo da saúde. É ainda consenso que o fenômeno migratório tem implicações para o sujeito, seu grupo familiar, comunidade e até mesmo nação. A escuta de pacientes na clínica revelou que o sofrimento primeiramente atribuído à experiência migratória trazia de forma subjacente estórias que pareciam articular-se ao modo como ocorreu o processo de subjetivação desses sujeitos. Passamos a levantar a hipótese de que em alguns casos a migração poderia ser uma tentativa de saída diante de certos impasses psíquicos. Tornou-se portanto, nosso objetivo, identificar e interrogar as origens e vicissitudes deste tipo de movimento psíquico que encontramos presente no deslocamento geográfico de alguns sujeitos. The clinical method is the one adopted by the Fundamental Psychopathology, so psychoanalysis is more suitable for such discipline. Within this theoretical-clinical, this work refers to the involvement of obsessive neurosis in a woman. Such manifestation finds little support in the current theory, enabling the importance of this production. The construction of the narrative of the cited case reveals theoretical, clinical and transferential barriers, which the author conducts a possible detailed study the most. The desire, its cause and its impossible of accomplishment were brought to the fore, as well as the constitution of the feminine in rescuing the eternal question: anatomy is destiny or not? / Este trabalho se inicia com um caso clínico, portanto seu método é eminentemente clínico e se insere na tradição da Psicopatologia Fundamental, disciplina que visa um resgate da subjetividade humana, pois entende que desde a criação da Psicopatologia Geral proposta por Jasper (1913) a subjetividade humana vem sendo preterida em detrimento de uma psicopatologia objetivista que visa à observação do sofrimento humano através de mecanismos tecnológicos de classificação. O método clínico é o método adotado pela Psicopatologia Fundamental, portanto a psicanálise é a casa mais confortável para esta disciplina. Dentro deste referencial clínico-teórico, este trabalho versa sobre o acometimento da neurose obsessiva em uma mulher. Manifestação que encontra pouco respaldo na teoria vigente, o que viabiliza a importância desta produção. A construção da narrativa do caso exposto revela entraves teóricos, clínicos e transferenciais, os quais o autor realiza um estudo o mais detalhado possível. According to the position of Fundamental Psychopathology, pathos constitutes the human, in the measure that psychism is a reply to the incidence of pathos. Research was conducted whereof the clipping of a clinical case, considering the possibility of the violence to constitute symptomatic expression of pathos, either for the repetition of the pathogenic scene or through the way of the symptom. Facing the unbearable character of pathos excessive by definition - the capacity of complex symbolic creation, to suture the mark of the trauma, could remain disengaged. The present research is destined to investigate the possibility of considering the repetition of the violence suffered for the subject as being a symptom of this subject, as well as investigating ahead on the nature of this symptom towards the given nature of pathos, excessively terrifying. The clinical case that motivates the present research offers subsidies to think the question. We cogitate if the patient, in the observed case, offers himself to violence as in a species of sacrifice, writing repeatedly in his body a pledge, order of recognition, petition and repetition that informs and certifies the impossibility to deal with the uncommon violence of pathos that reached him. His acts, that victimize himself in repeated situations of violence practiced by the other (Other), would configure the impossibility towards symptom? / A presente Dissertação de Mestrado diz respeito a uma possibilidade de constituição do sujeito diante da violência, pathos excessivo. Segundo a posição da Psicopatologia Fundamental, pathos constitui o sujeito, na medida em que o psiquismo é uma resposta à incidência do pathos. Pretendemos investigar, a partir do recorte de um caso clínico, a possibilidade da violência constituir expressão sintomática de pathos, seja pela repetição da cena patogênica ou pela via do sintoma. Diante do caráter inefável de pathos excessivo - a capacidade de simbolização complexa, a suturar a marca do trauma, poderia restar comprometida. A presente pesquisa destina-se a investigar a possibilidade de se considerar a repetição da violência sofrida pelo sujeito como sendo um sintoma deste sujeito, assim como investigar sobre a natureza deste sintoma diante da obviedade de pathos, excessivamente aterrador. O caso clínico que motiva a presente pesquisa oferece subsídios para pensarmos a questão. Cogitamos se o paciente, no caso observado, se oferece à violência como em uma espécie de sacrifício, escrevendo repetidamente em seu corpo um manifesto, pedido de reconhecimento, petição e repetição que informa e atesta sua impossibilidade de lidar com a violência descomunal do pathos que o atingiu. The clinical care in the Brasilian Unified Health System (SUS) was the source of the production of fertile clinical narratives for this research. These fragments have allowed me to discuss cases that constitute both the clinical method as the research method. The clinical method guides the practices of this research to the construction of narratives about the clinical cases, besides allow a clinical reflection of this material and the phenomenon under analysis. This work is focused on the subjective position of clinical experience in the obscure that underlies the analytic encounter. Such research requires put the question about the preservation of margins, borders and boundaries on the territories, continuously marked and remarked. These lines show up in the field and in the work of mental health nowadays and they are very similar to the demarcations and categories in force in the past. Users of mental health system seen as marginal from point of view of the system of rights, are still massively kept within the walls of official confinement of the insanity, banished from society, but now outside of the walls. When we find In the SUS so striking separation of bodies, spaces of distinction and demarcation of places, still in force, it is evident the urgency to inquire about the function of this clinical position, its foundations and the basic assumptions which maintains frequently separate the suffering of pathos from the conditions to establish otherness / Este estudo registra o curso de vinte e cinco anos de trabalho na saúde pública da cidade de São Paulo. O atendimento clínico no Sistema Único de Saúde (SUS) foi se constituindo como um material para a produção de narrativas clínicas férteis para esta pesquisa. Estes fragmentos me permitiram discutir casos que constituem tanto o método clínico, como método de pesquisa. O método clínico orienta as práticas de pesquisa para a construção de narrativas sobre casos atendidos, permite a reflexão clínica deste material e do fenômeno em análise. Este trabalho está focado na posição subjetiva da experiência clínica no obscuro que subjaz ao encontro analítico. Essa investigação requer se indagar sobre a preservação das margens, bordas e fronteiras nos territórios sucessivamente marcados e remarcados. Hoje em dia, essas linhas aparecem no campo e no trabalho de saúde mental muito semelhantes ao das demarcações e categorias vigentes em outros períodos do passado. Os usuários de saúde mental, vistos como marginais do ponto de vista do sistema de direitos, ainda são maciçamente mantidos dentro das paredes de confinamento oficial da insanidade, banidos da sociedade, mas agora do lado de fora das paredes. On one hand it is demanded that we build a competent clinical strategy regarding the prevention and treatment, capable of handling the community issues, on the other hand, mental health is still haunted by the figure of the professional focused on individual assistance, in a closed room unit, at the mercy of referrals. Trying to escape from reference s scheme and cross-reference and under or over recovery of mental-health clinic, we sought in this work, based on the construction of clinical narratives, given from the development of the clinical research s method in Fundamental Psychopathology, to understand "how?" to make a consistent clinical strategy, but flexible and open at the same time, before the problems that arise in the mental health field, in this case, bringing as context a unit of the Family Health Program and a Judicial Asylum, both located in cities in the state of Sao Paulo. We worked on the trajectory of these patients and the path followed by the clinician in these cases, focusing primarily a networking and co-responsibility / São muitas as questões que envolvem a clínica em saúde mental, desde a incorporação dos serviços até a maneira como estes devem ser oferecidos. Se por um lado exige-se que se construa uma estratégia clínica competente no que se refere à prevenção e ao tratamento e que saiba manejar as questões comunitárias, por outro, a saúde mental é assombrada pela figura do profissional focado nos atendimentos individuais encerrados numa sala da unidade, à mercê dos encaminhamentos. Our main goal was to analyze the dynamics of the psychic vulnerability in the clinical position and in the patient. Inserted in the field of Fundamental Psychopathology, this research made use of the clinical method to offer a careful thought about the human pathos, highlighting what was peculiar to each case. To achieve such result, hospitality, the capacity to receive those who are in dislocation, was dealt with from the clinical perspective, which considers hospitality itself one of the cornerstone of the clinical position. On the other hand, it was revealed that in this availability to receive the páthico content that comes from the patient lies the vulnerability that accompanies the clinical position. In view of how important it is to support this belief and establish limits to the notion of hospitality, the concept of dietetics, developed in Ancient Greek, was elected, which suggests the fair measure to the use of all pleasures. The art of living proposed by the dietetics can be of great help to think about the regime that is needed to set the laws of hospitality in the psychopathological clinic. When it comes to the patient vulnerability, the journey of young Sara was narrated. In the moment she was hospitalized, Sara received the HIV diagnosis and presented insufficient defensive resources to face the disease. Based on the concept of Psychic Immunological Insufficiency and its articulation with the clinical material, the present study identified some predisposition to this psychopathological manifestation and pointed out some creative strategies that enable defense against the virulent internal and external attack / A partir de atendimentos realizados aos portadores do vírus da aids em um hospital geral, algumas questões surgiram e fomentaram o interesse para a realização da presente dissertação, a qual teve o intuito de analisar a dinâmica da vulnerabilidade psíquica na posição clínica e no paciente. Inserido no campo da Psicopatologia Fundamental e utilizando do método clínico, o presente estudo ofereceu uma reflexão sobre o pathos constituinte do humano, ressaltando o que há de peculiar em cada caso. Para tal feito, primeiramente, a hospitalidade, ou seja, a capacidade de receber aquele que está em deslocamento, foi abordada sob o enfoque clínico, sendo esta, nesta perspectiva, configurada como um dos pilares que fundamenta a posição clínica. Todavia, relevou-se, também, que nesta disponibilidade em receber o conteúdo páthico vindo do paciente está presente a vulnerabilidade que acompanha a posição clínica. Diante da importância em sustentar tal posicionamento e do estabelecimento dos limites à hospitalidade, recorreu-se à noção de dietética desenvolvida na Grécia antiga, a qual propunha a justa medida ao uso dos prazeres. A arte de viver proposta pela dietética serve de reflexão para se pensar o regime necessário para se estabelecer as leis da hospitalidade na clínica psicopatológica. Sobre a vulnerabilidade do paciente, foi narrado o percurso de Sara, uma jovem que, no momento da hospitalização, recebeu o diagnóstico positivo para HIV e manifestou uma insuficiência de recursos defensivos diante do adoecimento. A partir da noção de Insuficiência Imunológica Psíquica articulada ao material clínico, o presente estudo identificou alguns fatores predisponentes desta manifestação psicopatológica e apontou algumas estratégias criativas, as quais possibilitam a defesa diante do ataque virulento interno e externo

Bringing together clinical report and metapsychology-making of the case, a panel is presented following the postulates of the Clincal Method and Fundamental Psychopathology. It delves into the psychic dimensions implied in cases of: acute appendicitis, multiple sclerosis, ghost limb syndrome, and amnesia and blindness due to cranioencephalic post-multitraumatism. We speculate that critical moments are enigmatic forms of reorganization of the body seeking a harmonic. Organic disorders due to illness or accident are possible conditions to reach new balances. We conjecture that reordering in the body is an answer to the paradigm of creative philo-ontogenetic solutions. Introducing into the transference the experience of feeling alive constitutes the element of therapeia, aiming the reestablishment of the vital rhythm of the body, its pulsional harmonics / O presente trabalho deriva da observação clínica com pacientes passando por um momento crítico sob tratamento em instituição hospitalar. Constelando relato clínico e metapsicologização do caso, apresenta-se um painel configurado, seguindo os postulados do Método Clínico e da Psicopatologia Fundamental. Aprofunda-se nas dimensões psíquicas implicadas em casos de: apendicite aguda, esclerose múltipla, dor do membro fantasma, e amnésia e cegueira pós-multitraumatismo cranioencefálico. Especula-se que momentos críticos são formas enigmáticas de reorganização do corpo visando uma harmônica. Clinical situations with manifestations of erectile dysfunction, which lead one to conceive of a psychopathology of impotence, allow an approach with several different perspectives. This research is being carried out from a psychoanalytic perspective and is being developed within the field of fundamental psychopathology. The text has five chapters. First we present a methodology based on the clinical method, where we describe an investigation which began with masters work and a masters degree, and moved on to the area of doctoral studies. This historical path was possible due to the enigmas that the clinical work produced, permitting articulation between aspects that originally consisted of inhibition in an individual s early childhood and later developed into impotence in adult life. The chapter on psychic and sexual impotence in men is comprised of discussions of clinical cases. The situations related to treatment led to formulations on the nature of inhibition in each case. We thus conceived that there is both a specific and a generalized inhibition, as developed by Freud when dealing with this notion. In terms of specific inhibition, desire is blocked, whereas, in generalized inhibition, the subject's capacity to desire is put into doubt. This distinction would seem to be essential for listening and conducting treatment in cases where the disappearance of sexual desire leads one to recognize the existence of psychic suffering / A proposta deste trabalho é apresentar os desdobramentos de uma pesquisa sobre impotência sexual masculina. Situações clínicas com manifestação de uma disfunção sexual nos homens, as quais nos sugerem pensar numa psicopatologia da impotência, permitem uma abordagem sob diferentes perspectivas. Esta pesquisa se desenvolve desde uma perspectiva psicanalítica e está circunscrita pelo campo da psicopatologia fundamental. O texto está composto por cinco capítulos. No primeiro deles nos dedicamos à apresentação da metodologia baseada no método clínico, onde percorremos os caminhos de uma investigação que teve origem no mestrado, chegando à situação problemática do doutorado. Tal percurso foi possível realizar pelos enigmas que a clínica produziu permitindo uma articulação entre o que, num primeiro momento, a inibição na infância interrogava com o que, num segundo momento, entendemos como impotência no adulto. Os casos clínicos compõem o capítulo sobre impotência psíquica e sexualidade masculina, sendo que tais situações de tratamento conduziram às formulações sobre a natureza da inibição em cada uma delas. Foi assim que pensamos na inibição específica e na inibição generalizada, influenciada pelas formulações freudianas sobre tal noção.
