Aspekter i vårdmiljö som påverkar välmående : Allmän litteraturstudie / Aspects of health care that affect well-being : General literature study

BACKGROUND: When a person becomes unwell and develop an illness, they become vulnerable and more sensitive to the surrounding of the environment. The importance of health care has long been seen as a factor for well-being. The Nursing role as a caregiver is central and paramount and must have a holistic approach to care. AIM: The purpose was to describe the aspects of the healthcare environment that affect well-being. METHOD: A general literature study based on 21 articles with quantitative data was researched. RESULTS: The analysis process of data resulted in the emergence of two main themes with additional SUB themes. The senses- light, sound, smell. Physical environment that includes spaces, technical equipment, garden and design. CONCLUSION: It appears in our literature review, that different aspects of the healthcare environment can have both positive and negative effects in patients, close relatives and nurses' well-being. It is very important to quickly identify which aspects promote well-being and implement it in the healthcare environment to prevent that which leads to adverse effects. IMPLICATIONS: Nurses should have knowledge of the health care environment's effects in order to implement it in their work. By introducing these aspects, it is possible to create a better healthcare environment for patients, close relatives and nurses. More research into the roles of nurses in hospital is required. / SAMMANFATTNING BAKGRUND: När en människa utsätts för sjukdom blir den sårbar och mer känslig för stimuli från omgivningens. Vårdmiljöns betydelse har länge sett som en faktor för välmående. Sjuksköterskans roll som vårdare är centralt och bör ha holistiskt förhållningssätt vid vårdandet. SYFTE: Syftet var att beskriva de aspekter i vårdmiljön som påverkar välmående. METOD: En allmänlitteraturstudie baserad på 21 artiklar med kvantitativa data. RESULTAT: Analysprocessen av data resulterade till uppkomst av två huvudteman med tillbehörande SUB teman. Sinnen- ljus, ljud, lukt. fysisk miljö som innefattar utrymmen, teknisk utrustning, trädgård och design. SLUTSATS: Det framkommer i vår litteraturöversikt att olika aspekter i vårdmiljön kan ha både positiva och negativa effekter i patienter, närstående och sjuksköterskors välmående. Det är av stor vikt att snabbt identifiera vilka aspekter som verkar främjande för att implementera det i vårdmiljön och förhindra det som leder till negativa effekter. IMPLIKATIONER: Sjuksköterskor bör ha kunskap om vårdmiljöns effekter för att kunna implementera det i sitt arbete. Genom att införa dessa aspekter kan det skapas en främjande vårdmiljö med bättre välmående för patienter, närstående och sjuksköterskor. Det krävs mer forskning gällande sjuksköterskans roll vid planlösningar av sjukhus.

Demographic Factors Associated with Consistent Hand Hygiene Adherence Among ICU Nurses

Healthcare associated infections cause 75,000 to 80,000 deaths a year. Many are preventable with proper hand hygiene adherence (HHA). Worldwide, HHA range is between 40-60%, far below the 100% recommended. The purpose of this quantitative, cross-sectional, prospective study was to investigate any association between 15 demographic variables and HHA of ICU nurses. A convenience sample of 613 hand hygiene opportunities was collected by direct observation at each of 5ICUs (4 hospitals) in Texas for 8 consecutive hours each day for 3-5 days. The theoretical foundation guiding this study was the healthcare environment theory. The Statistical Package for Social Sciences software was used for descriptive and inferential analysis of data. An aggregated overall HHA rate of 64.09% was identified among all nurses, 66.88% among male nurses and 62.27% among female nurses. Number of children, age of the nurse, number of years of living in the U.S., and the number of years of active nursing practice were significantly associated with HHA (p = .000) using paired sample t-test. The potential social change impact of this study is identifying variables associated with HHA, identification and measurement of 4 barriers to HHA, measuring the Hawthorne Effect, identification of Low Gelers, High Gelers, and Super Gelers, average rate may not be indicative of what is happening in hospital, and call for standardization of surveillance methodology. Findings may lead to specific interventions to increase HHA among nurses with certain demographic characteristics.

Light Image Therapy in the Health Care Environment.

Use of positive distraction in the built healthcare environment to assist in alleviating stress in a patient was investigated. A backlit light image was mounted in the ceiling of an examination room to create a positive distraction for patients in the ETSU Pediatric Clinic in Johnson City, TN. Survey instruments were used to collect sample data from patients and physicians in a randomized, balanced controlled study designed to determine if patients experienced less stress in the room with the backlit image as compared to other rooms (treatments). Although a statistical difference was not determined between the room with the backlit image and positive and negative control rooms, patients in rooms containing nature art tended to exhibit less anxiety. Researched based knowledge for creating positive distractions in the built healthcare environment helps designers create environments that benefit the patients, their families and medical staff of healthcare facilities.

To improve their services and maintain patients’ satisfaction, healthcare organisations have adopted and applied different quality tools and models in recent times, with some even developing their own quality-based initiatives. For example, the approach of Lean Six Sigma (L6σ) has recently been gradually and slowly implemented in healthcare institutions. However, the nature and complexity of healthcare environment which directly impact on humans require leaders to carefully apply appropriate Quality Management (QM) systems suitable for this critical environment. The aim of this research project is to develop a Knowledge Based System (KBS) to assist healthcare managers and practitioners during decision-making process in the context of achieving excellent benchmark and action plans prioritisation. The system will be built based on a conceptual framework for Quality Management in Healthcare Environment (QMHE) which will be modified into a model. The KBS will be developed from this model with the integration of Gauging Absence of Pre-requisite (GAP) method for benchmarking and Analytical Hierarchy Process (AHP) method for prioritisation. The contribution of this research is the use of KBS with GAP and AHP to develop an integrated Knowledge-Based Lean Six Sigma (KB-L6σ) in QMHE. This will accomplish the necessities of investigating quality problems and recommend suitable solutions according to international best practices. It will use a systematic approach that can be applied multiple times, follow defined steps to secure consistency in the approach and integrate different healthcare management levels to maintain strategic decision-making alignment. It consists of 964 KB rules that have been produced via a knowledge acquisition process from the literature and interviewing experts in the field of QM and L6σ in healthcare environment. Feedback from conferences and system testing were used for the verification of the model, whilst validation was carried out through three case studies implementation at three tertiary hospitals in Oman. The analysis of using the KB system in these hospitals has shown clearly that the developed system is a consistent and reliable methodology for assisting decision-makers in designing, planning, and implementing L6σ for QMHE.

Gynovation : Redesigning Gynaecologist Clinics for Enhanced Well-being

The objective of this project is to challenge the traditional interior design of gynaecologist clinics and develop a fresh and innovative concept that enhances the overall experience for patients. By utilising colour, material, and form, this interior concept aims to transform the perception of healthcare facilities, ultimately improving the well-being and comfort of patients during gynaecological visits. To exemplify a different, more welcoming and safe environment, my experiments have involved using colour, material and form, as well as purpose designed chairs for the waiting room as well as the examination rooms, respectively. Besides this I have explored distractive elements during the patients’ examination. The design process for this project involved conducting qualitative research and studying existing gynaecologist clinic designs, patient experiences, and current trends in healthcare environments. Valuable insights has been gathered through interviews with healthcare professionals and patients, which helped identify areas for potential improvement. Additionally, the project explored the use of innovative design techniques, materials, and spatial configurations that foster a sense of privacy, and well- being. The design process also involves the development of design sketches and digital visualisations to aid in the visualisation and refinement of the interior concept. By utilising all design elements, the concept aims to create a health-promoting experience that ensures a safe, pleasant, and welcoming environment for both patients and staff.

Patienters upplevelse av vårdmiljö på operations- och intensivvårdsavdelning : Systematisk litteraturstudie

Abstrakt Bakgrund: Vårdmiljön på operations- och intensivvårdsavdelningen är avancerad och högteknologisk. Att vara patient och behöva vårdas på sjukhus innebär att komma till en främmande miljö med risk att förlora kontroll över sig själv och behöva överlämnas till andra. Förhållandet mellan patient, vårdpersonal, närstående, teknik, omgivning samt omvärld anses vara avgörande för hur patienten kommer att uppleva vårdmiljön.  Syfte: Att beskriva patienters upplevelser av vårdmiljön på operations- och intensivvårdsavdelning. Metod: Systematisk litteraturstudie med induktiv och kvalitativ ansats. Analysen utfördes genom att syntetisera och summera innehållet i 17 kvalitativa primärstudier.  Resultat: Resultatet består av 3 huvudteman och 8 underteman. Tema nr 1: Att vara patient i en främmande och teknisk vårdmiljö - patienternas förändrade upplevelser av sig själva, att befinna sig mellan fantasi och verklighet, att ha och inte kontroll över sin situation samt familjens betydelse för patienternas upplevelser. Tema nr 2: Patienternas upplevelser av rädsa - att vara rädd samt rädsla mot existensen. Tema nr 3: Personalens betydelse för patienternas upplevelser - att vara väl eller icke väl omhändertagen samt att behandlas som subjekt eller objekt.  Slutsats: Patienternas upplevelser av vårdmiljön på operations- och intensivvårdsavdelningen beskrevs som både positiva och negativa, betryggande och skrämmande men var framförallt individuella. Relationen med vårdpersonalen och att som patient känna sig väl omhändertagen samt att bli bemött och behandlad med respekt, där patientens integritet och individualitet låg till grund för vårdandet, var den viktigaste aspekten i patienternas möte med vården.

Patienters upplevelser av omvårdnad på akutmottagningar / Patient´s experiences of nursing care in emergency departments

Bakgrund: Akutmottagningar är enheter avsedda för patienter i akuta skeden. Med hjälp av triagering kan prioritering av de mest akuta fallen ske. Långa väntetider förekommer ofta på akutmottagningar. Sjuksköterskan har en ledande roll i att säkerställa patientsäkerheten och arbetar utifrån olika etiska koder. Viktiga komponenter för att utföra snabb, effektiv och god omvårdnad är gott bemötande, tydlig kommunikation samt information till både patienter och annan vårdpersonal. Syfte: Syftet var att belysa patienters upplevelser av omvårdnad på akutmottagningar. Resultat: Resultatet redovisades i tre olika teman: Upplevelsen av kommunikation och information, upplevelsen av bemötandet och upplevelsen av vårdmiljön.  Metod: Denna litteraturöversikt utfördes med tio vetenskapliga artiklar, både av kvalitativ och kvantitativ metod. Datainsamling genomfördes genom databaserna Cinahl Complete och PubMed. Genom analysering av artiklarnas innehåll kunde teman skapas.  Slutsats: En del patienter upplever vårdlidande på akutmottagningarna, speciellt i väntan på vård och väntan på information från vårdpersonalen. Patienter som blivit bemötta på ett respektfullt sätt har generellt bättre upplevelser. Patienter som blivit bemötta respektlöst, inte fått hjälp eller information har generellt sämre upplevelser. Upplevelserna blir mer negativa när akutmottagningar har en bristande vårdmiljö och mindre resurser eller bemanning. Patienterna har ett behov av mer information, kommunikation och stöd från sjuksköterskorna för att minska deras sjukdomslidande. God omvårdnad minskar sjukdomslidandet och risken för vårdlidande. / Background: Emergency departments are units intended for patients in acute conditions. With the help of triage, prioritization of the most urgent cases can take place. Long waiting times often occur in emergency departments. Nurses have a leading role in ensuring patient safety and ensuring that ethical codes are being followed. To ensure that the acute care is rapid and efficient it is necessary to execute good nursing care. Good nursing care is characterized by having good behavior towards patients, giving proper information to patients and communicating with other healthcare staff. Aim: The aim of this study was to describe patient ́s experiences of nursing care in emergency departments. Method: This literary review was completed with ten scientific articles. The articles were of qualitative and quantitative methods. Collection of data was done with the databases Cinahl Complete and PubMed. Themes were created through analysis of the articles. Results: The results were presented in three different themes: The experience of communication and information, the experience of approach and the experience of the healthcare environment. Conclusions: Some patients experience suffering in the emergency departments, especially while waiting for care and waiting for information from the healthcare staff. Patients who are treated respectfully generally have better experiences. Patients who have been treated disrespectfully, have not received help or information generally have worse experiences. The experience becomes more negative when emergency departments have a deficient care environment and less resources or staffing. The patients have a need for more information, communication and support from the nurses to reduce their suffering. Good care and approach reduce the suffering caused by illness and reduces the risk of increased suffering caused by the healthcare system.

Bakgrund: Vårdmiljön kan delas in i två kategorier, varav den ena är objektiv och den andra är subjektiv. Båda dessa miljöfaktorer påverkar livskvaliteten för personer med demens. Vårdmiljön inkluderar rum, byggnader, inom-/utomhus och trädgårdar. Förståelse för vårdmiljön och dess påverkan hos demenssjuka är därför avgörande för vårdpersonal, sjuksköterskor och anhöriga som bör engagera sig i att implementera rätt vårdmiljö som skapar en kvalitativ levnadsstandard hos individer med demenssjukdom. Syfte: Att undersöka forskning om vårdgivares erfarenheter av hur patienter med demenssjukdom påverkas av sin vårdmiljö. Metod: Litteraturundersökningen grundar sig på det systematiska flödesschemat enligt Polit & Beck, (2012). Litteraturstudien börjar med att formulera en frågeställning och finna därmed relevanta sökord. Granskningen gick utifrån SBU:s (2014) och studien styrkas med aktuell forskning. Resultat: Utifrån 12 granskade artiklar som genomförts i USA, England, och Australien gällande vårdmiljön och socialmiljön instämmer vårdpersonal, sjuksköterskor och anhöriga enhälligt att en trivsam hemmiljön stärker självvärdet för individer med demenssjukdom och måste individanpassas. I och med detta behöver vårdpersonal, sjuksköterskor och anhöriga vara delaktiga i tillämpande och implementeringen av rutiner och anpassningen av miljön för de demenssjuka. Konklusion: Vårdmiljön och socialmiljön och hur den inrättas och implementeras är direkt avgörande för levnadsstandarden hos individer med demenssjukdom. En trevlig och behaglig hemmiljö som individen med demenssjukdom kan känna igen skapar trygghet. En socialmiljö där individen med demenssjukdom kan vara delaktigt och kognitivt engagera sig skapar självvärde. / Background: The care environment can be divided into two categories, one of which is objective, and the other is subjective. Both environmental factors affect the quality of life of people with dementia. The care environment includes rooms, buildings, indoor / outdoor and gardens. Understanding the care environment and its impact on people with dementia is therefore crucial for care staff, nurses and relatives who should be involved in implementing the right care environment that creates a qualitative standard of living for individuals with dementia. Objective: To investigate research on caregivers' experiences of how patients with dementia are affected by their care environment. Method: The literature review is based on the systematic flow chart according to Polit & Beck, (2012). The literature study begins with formulating a question and thereby finding relevant keywords. The literature study is supported by current research. The review template used was from SBUs (2014). Result: Based on 12 reviewed articles conducted in the USA, England, and Australia regarding the care environment and the social environment, care staff, nurses and relatives unanimously agree that a pleasant home environment strengthens the self-esteem of individuals with dementia and must be individually adapted. As a result, healthcare professionals, nurses and relatives need to be involved in the application and implementation of routines and the adaptation of the environment for people with dementia. Conclusion: The care environment and the social environment and how it is set up and implemented are directly correlated with the standard of living for individuals with dementia. A pleasant and comfortable home environment that the individual with dementia can recognize creates security. A social environment where the individual with dementia can be involved and cognitively get involved creates self-worth.
