To achieve our goal we present, at first, the process of construction of the hospital as a place of healing, and then visualize the paths that allowed discussions on the inclusion of play and toy libraries in the hospital. We start from the understanding that tensions and conflicts are part of social relations as a whole, being pronounced in spaces permeated by power relations. Introducing the hospital as an environment marked by tensions, conflicts and power relations and the toy library as a possibility – through free play – subvert the powers that be in these spaces, while they are also marked by disputes, conflicts and tensions. We adopt the theoretical category daily by their importance for understanding the processes of change and disruption that occur in society. From Foucault (1987) and Certeau (1994), we understand the daily as the space in which the micro powers are at the same time, exercised and circumvented by the subjects. The research is qualitative and the methodology adopted is the Case Study understood by Gil (2009) as a research design, and relevant to recent phenomena. We perform the triangulation of techniques and sources, being they the field observations, interviews and document analysis. In this work we adopt an interdisciplinary perspective – as this concept is presented by Muñoz and Oliveira Junior (2009) – understood as a dialogue between different disciplines in order to build both theoretical and methodological paths, and the drafting of new problems. We observed that the toy library of the institution was built on the principles of humanization of health care for the child, and their services are grounded in free play. In this sense such spaces collaborate in developing bonds of friendship, the bond between children and mothers through play, besides being one of the few places within the hospital in which the child has the power to choose, and to say no to any activity that not like. In contrast, we found that the main problems for the maintenance of services is the lack of professionals in sufficient numbers to work in these areas, limiting the opening time of the toy library (one hour per day), acquiring and retaining the playful collection (acquired solely through donations), and cleaning the space that often does not occur due to lack of professionals in the institution. / O presente trabalho tem por objetivo analisar a função da brinquedoteca hospitalar e o cotidiano desse espaço num hospital especializado em atendimento pediátrico administrado pela Prefeitura Municipal de Ponta Grossa –PR. Para alcançarmos nosso objetivo apresentamos, num primeiro momento, o processo de construção do hospital enquanto espaço de cura, para então visualizarmos os caminhos que possibilitaram as discussões sobre a inserção do brincar e das brinquedotecas no ambiente hospitalar. Partimos da compreensão que as tensões e os conflitos fazem parte das relações sociais como um todo, sendo acentuadas em espaços permeados por relações de poder. Apresentamos o hospital como um ambiente marcado por tensões, conflitos e relações de poder e a brinquedoteca como uma possibilidade de – por meio do livre brincar – subverter os poderes existentes nesses espaços, ao mesmo tempo em que também são marcados por disputas, conflitos e tensões. Adotamos a categoria teórica cotidiano por sua importância para a compreensão dos processos de mudança e ruptura que ocorrem na sociedade. A partir de Foucault (1987) e Certeau (1994) compreendemos o cotidiano como o espaço em que os micropoderes são, ao mesmo tempo, exercidos e burlados pelos sujeitos. A pesquisa é de natureza qualitativa e a metodologia adotada é o Estudo de Caso entendida por Gil (2009) como um delineamento de pesquisa, sendo pertinente para fenômenos recentes. Observamos que a brinquedoteca da instituição foi construída a partir dos princípios da humanização do atendimento à saúde da criança, sendo que seus serviços estão embasados no livre brincar. Neste sentido tais espaços colaboram na elaboração de laços de amizades, no vinculo entre crianças e acompanhantes através da brincadeira, além de ser um dos poucos ambientes dentro do hospital em que a criança tem o poder de escolher, e até dizer não a alguma atividade que não lhe agrada. Em contrapartida, vimos que os principais problemas para a manutenção dos serviços são: a falta de profissionais, em número suficiente, para trabalhar nesses espaços, a limitação do tempo de abertura da brinquedoteca (uma hora por dia), aquisição e manutenção do acervo lúdico (adquirido unicamente através de doações), e a limpeza do espaço que muitas vezes não ocorre devido a falta de profissionais na instituição.

A Farmácia Hospitalar tem por finalidade promover o uso racional do medicamento, pesquisando e gerenciando produtos com qualidade, desenvolvendo profissionais e prestando assistência integrada ao paciente e à equipe da saúde, com responsabilidade socioambiental. Dessa forma, o desenvolvimento de um modelo de construção e aplicação de um conjunto de indicadores de desempenho na Farmácia Hospitalar com foco na comparabilidade é imprescindível para o monitoramento dos resultados e o estabelecimento da melhoria contínua dos serviços prestados aos pacientes e à equipe da saúde, de forma alinhada às diretrizes institucionais. Objetivo: Desenvolver um modelo de construção e aplicação de um conjunto de indicadores de desempenho na Farmácia Hospitalar com foco na comparabilidade. Método: Utilizou-se do método qualitativo Pesquisa-ação, sendo que na fase exploratória se fez o diagnóstico da amostra do estudo; na realização dos seminários se fez a sensibilização dos sujeitos da pesquisa, análise, seleção, construção, aplicação em campo e validação da ficha técnica e especificações para construção, e do conjunto de indicadores de desempenho. Realizou-se 22 seminários presenciais de forma estruturada. A amostra foi intencional de seis hospitais (3 públicos e 3 privados) e os sujeitos da pesquisa os farmacêuticos especialistas tomadores de decisão na Farmácia Hospitalar. A coleta de dados foi realizada no período de setembro de 2006 a abril de 2008, foram aplicados dois questionários e sete formulários (manual ou eletrônico). Os dados coletados foram tratados em forma de quadros, tabelas, figuras, gráficos, cálculos de somatório, porcentagem, freqüência, média aritmética e mediana, como também, compilação por semelhança e agrupamento por categorias-chave, e apresentados de forma consolidada, para obtenção de consenso quando necessário. Resultados: Apresentou-se a caracterização do Hospital e da Farmácia Hospitalar; obteve-se o aprimoramento da ficha técnica e especificações para construção de indicadores; foi selecionado, construído, aplicado em campo e validado um conjunto com 12 indicadores de desempenho passíveis de comparação. Verificouse o grau de dificuldade e vantagens obtidas da aplicação de indicadores. Apresentou-se a estrutura para o desenvolvimento do modelo de construção e aplicação de um conjunto de indicadores de desempenho na Farmácia Hospitalar com foco na comparabilidade. Conclusão: Os resultados obtidos na pesquisa evidenciam a hipótese formulada de que, o desenvolvimento de um modelo de construção e aplicação de um conjunto de indicadores de desempenho sistematizado e passível de comparação, é factível na Farmácia Hospitalar, assim como, busca contribuir com a disseminação da cultura de utilização de indicadores de desempenho, possibilitando a Farmácia Hospitalar se comparar com referenciais internos e externos, promovendo as melhores práticas embasadas em evidências objetivas / The Hospital Pharmacy has as objective to promote the rational drug use, by researching and managing products with quality, creating professionals and providing integrated assistance to the patient and the health team, with socialenvironmental responsibility. For this, the development of a construction model and application of a set of performance indicators in the Hospital Pharmacy with focus on the comparability is absolutely necessary for monitoring results and setting up the continuous improvement of the services provided to patients and to the health team, aligned with the institutional guidelines. Objective: To develop a construction model and application of a set of performance indicators in the Hospital Pharmacy with focus on the comparability. Method: The action-research qualitative method was used and in the exploratory stage was made the diagnosis of the study sample; the awareness of the research people arose from the seminars, the analysis, selection, construction, field application and validation of the data sheet and specifications for construction, and of the set of performance indicators as well. Twenty-two seminars in situ took place in a structured way. The sample was intentional and designed by six hospitals (three public and three private hospitals) and the people of the research were the decision-makers specialist pharmacists in the Hospital Pharmacy. The collection of data was carried out from September, 2006, to April, 2008; two questionnaires and seven forms were applied (manually or electronically). The collected data were arranged in the form of pictures, tables, figures, graphs, total sum calculations, percentage, frequency, arithmetic and medium average, as well as, compilation by similarity and grouping by key-categories, and presented in a consolidated way for consensus building if necessary. Results: The improvement of the data sheet and specifications for construction of indicators was achieved; a set with twelve performance indicators subject to comparison was selected, constructed, applied in field and validated. It was verified the degree of difficulty and advantages obtained in the application of the indicators. The structure for the development of the construction model and the application of a set of performance indicators in the Hospital Pharmacy with focus on the comparability was presented. Conclusion: The results achieved in the research show clearly the formulated hypothesis, which the development of a construction model and the application of a systemized set of performance indicators and subject to comparison, is tangible in the Hospital Pharmacy, as well as, seeks to cooperate with the dissemination of the culture of the performance indicators use, enabling the Hospital Pharmacy to be compared with internal and external references, by fomenting the best practices based on objective evidences

Busca compreender o que leem esses sujeitos, qual concepção de leitura, linguagem, texto e sujeito fundamenta essas práticas e, ainda, quais são os suportes e gêneros textuais mais utilizados pelos professores da classe hospitalar. A pesquisa fundamentou-se na concepção bakhtiniana de linguagem como interação verbal, e a metodologia caracterizou-se como um estudo de caso de caráter qualitativo. Utilizou o recurso de observação participante e de entrevista individual com as professoras, as crianças e os adolescentes para caracterizá-los, bem como registros do diário de campo durante observação, fotografia e filmagem dos eventos de leitura para a produção das análises. Foi possível saber que as crianças e adolescentes no hospital leem uma variedade de gêneros discursivos, tais como: conto, fábula, crônica, informativo, explicativo, opinião, verbetes, poema, poesia, música, Histórias em Quadrinhos, curiosidades, mito, lenda, aventura, rima, jogral e outros. As professoras possibilitaram que a leitura desses gêneros fosse realizada de diferentes maneiras: leitura individual em voz alta e silenciosa, leitura coletiva e leitura ajudada pela professora para aqueles em fase de alfabetização. Para o trabalho de leitura, as professoras utilizaram também vários suportes, como jornal, quadro-branco, revistas, livros de literatura e folha xerocopiada. A pesquisa analisou as leituras de contos, lendas e verbetes e verificou que as concepções de leitura, linguagem e texto que fundamentaram as práticas das professoras não eram homogêneas, pelo contrário, elas variavam. A leitura era trabalhada, ora restrita à simples decifração de signos, ou seja, como decodificação da escrita, ora de forma mais ampla, quando o diálogo era muito explorado, levando a leitura a uma atividade complexa de produção de sentidos / This thesis aims to analyze the reading practices that are performed in the hospital with children and adolescents in health care. The work tries to understand what they read, what is the conception of reading, language, text, subject, and their practices. In addition, it also tries to understand what are the brackets and text genres commonly used by teachers in a hospital class. The research was based on the Bakhtinian conception of language as verbal interaction, and the methodology was characterized as a case study of a qualitative nature. The work consists in the use of participant observation and individual interviews with teachers, children and adolescents. The methods were worked out to characterize them, as well as with daily field records of observation, photography and film events. It was possible to find out that children and adolescents in the hospital read a variety of genres such as fairy tales, fables, chronics, informative newspapers, explanatory opinions, entries, poems, poetry, music, comics, trivia, myth, legend, adventure, rhyme, and others. The teachers allowed the reading of these genres in different ways: aloud individual reading, silent reading, and collective reading helped by the teacher for those beginning in literacy. To read the work, the teachers have also used various media such as paper, whiteboard, magazines, books and xerox copies of known literature. The survey examined the reading of stories, legends, and entries and found out that the conceptions of reading, language, and text justifying the practices of the teachers were not homogeneous. The reading was crafted, sometimes restricted to the mere deciphering of signs, ie, decoding and writing. On the other hand, the reading sometimes was allowed to vary broadly and the dialogue was very exploited, leading to a complex reading activity and production of meaning and senses

Limitações e potencialidades da passagem de plantão de enfermagem na atenção hospitalar / Limitations and potentialities of the nursing shift in hospital care

A passagem de plantão é um recurso estratégico para a organização dos cuidados de enfermagem, que permite a continuidade da assistência efetiva. Trata-se de uma atribuição do enfermeiro, que precisa desenvolver a competência em comunicação para coordenar, organizar e realizar a passagem de plantão. A reconfiguração do papel do hospital, importância da integralidade e continuidade do cuidado, relevância da temática para o trabalho do enfermeiro e da equipe de enfermagem e a carência da produção científica sobre a passagem de plantão na atenção hospitalar justificaram a realização desta pesquisa. O objetivo foi identificar potencialidades e limitações da passagem de plantão de enfermagem na atenção hospitalar. Trata-se de estudo descritivo, abordagem qualitativa, utilizando Técnica do Incidente Crítico. Desenvolvido nas Unidades Funcionais da Neurologia e Clínica Cirúrgica, de hospital público universitário, referência para atenção às urgências e emergências, em município da região nordeste do Estado de São Paulo. Os participantes foram enfermeiros que atenderam aos critérios de inclusão: ser profissional nos serviços selecionados, não estar direta ou indiretamente envolvido com o estudo e estar presente na unidade à época da coleta de dados, e foram critérios de exclusão: estar ausente da unidade em decorrência de afastamentos de qualquer natureza. Para coleta de dados, foram realizadas entrevistas semiestruturadas, gravadas, transcritas integralmente. Para análise, foi utilizada a estatística descritiva e para os relatos utilizou-se a análise de conteúdo. Foram encontradas 76 situações, 103 comportamentos e 126 consequências. As situações tiveram predomínio de referências negativas (73,7%), agrupadas em quatro categorias: comunicação, interrupções na passagem de plantão, aspectos ambientais e aspectos organizacionais. Os comportamentos tiveram maioria de referências negativas (63,1%), agrupados em quatro categorias: comunicar, interromper a passagem de plantão, questionar a estrutura de trabalho e utilizar recursos tecnológicos. As consequências concentraram referências negativas (65,8%), agrupadas em quatro categorias: comunicação, tempo, organização do trabalho e relações interpessoais. As referências positivas foram entendidas como potencialidades e as referências negativas como limitações para a passagem de plantão. Nesse sentido, pode-se afirmar que os resultados referentes a situações, comportamentos e consequências evidenciam predomínio de limitações para a passagem de plantão incluindo as interrupções, falhas na comunicação, ausência de um local adequado para a realização da passagem de plantão e extensão na carga horária de trabalho, que podem repercutir em fatos inadequados para o atendimento ao usuário, tais como possibilidade de erros, duplicação/repetição de cuidados ou supressão destes inadvertidamente. Cabe destacar que, embora menos frequentes, as referências potencializadoras da passagem de plantão dizem respeito à comunicação em sua forma, foco, objetividade e conteúdo, as informações, utilização de recursos que facilitam a transmissão de informações e ambiente adequado. Os resultados permitiram um diagnóstico situacional sem, contudo, avançar para proposição de sugestões e intervenções, uma vez que a construção conjunta com os implicados das respectivas unidades pode estimular o processo participativo, criativo e de corresponsabilização. É preciso resignificar a passagem de plantão como parte das atividades de enfermagem, inserida em contexto institucional, que produz impacto para a equipe de enfermagem e multiprofissional, mas acima de tudo para os usuários / The shift is a strategic resource for the organization of nursing care, which allows continuity of effective care. This is task of the nurse, who must develop communication skills to coordinate, organize and perform the shift. The reconfiguration of the role of hospital, the importance of integrality and continuity of care, the relevance of the theme to nurses\' work and nursing team, and the lack of scientific production about shift in hospital care justified this research. The aim was to identify the potentialities and limitations of the nursing shift in hospital care. This is a descriptive study with qualitative approach, using the Critical Incident Technique. It was developed at the Functional Units of Neurology and Surgical Clinic of a public university hospital, which is reference for urgency and emergency care, in a municipality in the northeastern region of the São Paulo state. Participants were nurses who met the inclusion criteria, which were the following: to be professional in the selected services, not being directly or indirectly involved with the study and to be present at the Unit at the time of data collection; and the exclusion criteria: absent from the Unit due to work leaves. For data collection, semi-structured, recorded and fully transcribed interviews were used. Descriptive statistics was used for the analysis and content analysis was used for the reports. There were 76 situations, 103 behaviors and 126 consequences. The situations had a predominance of negative references (73.7%), grouped into four categories: communication, interruptions in shift, environmental aspects and organizational aspects. The behaviors had a majority of negative references (63.1%), grouped into four categories: to communicate, to interrupt the shift, to question the work structure and to use technological resources. The consequences concentrated negative references (65.8%), grouped into four categories: communication, time, work organization and interpersonal relations. Positive references were understood as potentialities and negative references as limitations to shift. In this sense, the results regarding situations, behaviors and consequences showed a predominance of limitations for the shift, including interruptions, communication failures, lack of a suitable place for performing the shift and extension of working hours, which can have repercussions on inappropriate facts for user\'s assistance, such as the possibility of mistakes, duplication/repetition of care or interruption of it inadvertently. It is highlighted that, although less frequent, the potential references of the shift are related to communication in its form, focus, objectivity and content, to the information, use of resources that facilitate the transmission of information and adequate environment. The results allowed a situational diagnosis without, however, moving forward to propose suggestions and interventions, since the joint construction with those involved in the respective units can stimulate the participatory, creative and co- responsible process. It is necessary to re-signify the shift as part of the nursing activities, inserted in an institutional context that produces impact for the nursing and multiprofessional team, but above all for the users

From their perspectives: Children and young people's experience of a paediatric hospital environment and its relationship to their feeling of well-being

Doctor of Philosophy (PhD) / This study was conducted to increase our understanding of children and young people’s experience of a hospital environment and to identify the salient attributes of the physical environment in their experience. There were three specific aims: to describe children and young people’s experience of a hospital environment and identify what constitutes a supportive paediatric environment; to examine the role of the physical environment in patients’ feeling of well-being; and to highlight the capacity of participatory research with children and young people to inform evidence-based paediatric design. At this stage, there has been very little healthcare design research carried out with populations of children and young people. Well-being research with children and young people in paediatric environments that identifies the potential supportive attributes in this environment is also very limited. Historically research on children’s health and well-being has been dominated by a focus on the prevalence of disorders, problems and disabilities. More recently, in response to the change to health promotion, positive attributes have been included in well-being and satisfaction measures. At this stage, there are still many fewer positive measures. Within the body of literature that exists in healthcare, healthcare design research, and well-being research, there are only a small number of participatory studies that focus on children and young people’s experience of hospitalisation, and an even smaller number that include children and young people’s experience of hospital environments. The picture that is created by the research that exists is patchy. There is a need for a more holistic understanding of children and young people’s experience of hospitalisation and of hospital environments from their own perspectives. Based on these gaps in current knowledge, two research questions were developed. The first was concerned with describing children and young people’s experience of the sociophysical environment of a paediatric hospital. The second question was concerned with understanding the role of the physical environment in children and young people’s feeling of well-being in a hospital environment. In addressing these questions, the intention was to identify attributes within the hospital setting which collectively comprise a supportive environment for children and young people and which contribute to children and young people’s feeling of well-being in a paediatric setting. The current study was conducted as an exploratory qualitative case study and carried out at the Children’s Hospital at Westmead, in Sydney, Australia. Using participatory research techniques, the sequence of the study included two pilot studies and the main study. The focus was on understanding the experiences of longer-term patients of a paediatric hospital environment. In the main study 25 children and young people, aged between 9-18 years, who had been in hospital for at least a week completed semi-structured interviews in which they talked about their response to the environment of the hospital and their experience of hospitalisation. Data analysis was completed using a combination of concept mapping and thematic analysis techniques. Preliminary findings were used as the basis of a further member-checking task carried out with a further six children and young people before conclusions were reached. The findings reveal that children and young people’s experience of a paediatric setting involves a number of major areas of influence including their personal situation, their social experience, their interaction with the physical environment, opportunities and characteristics of the organisation, and the effect of time. The findings also reveal that children’s feeling of well-being within this experience is linked to their ability to feel comfortable in the environment, to maintain a positive state of mind, and to remain positively engaged with the experience and the environment. This research reveals a dynamic relationship between children and young people and a paediatric environment that children and young people actively manage and shape. It reveals some of the key considerations in children and young people’s experience of hospitalisation. It also reveals why these considerations are important and what role they play in patients’ experience and feeling of well-being. These findings provide the basis for further research and they have implications for future design and research practice in paediatric healthcare settings.

<p>Much evidence exists that nurses are leaving the public health sector for the private sector, or leaving the country to seek better working conditions and higher salaries. Studies conducted on the job satisfaction of nurses are proof that there is a need to know more about the factors that influence their sense of job satisfaction. Most of these studies focus on the general nursing context. Due to its unique circumstances, many studies abroad have identified the field of psychiatric mental health nursing to investigate job satisfaction of nurses. The minithesis is an attempt to fill the gap that exists in job satisfaction studies in South Africa of nurses in a provincial psychiatric hospital. The study was a cross sectional, correlational, survey design study. The instrument was a self-administered questionnaire, combining a quantitative questionnaire with one qualitative open-ended question. The study was conducted on nurses of all categories in a provincial psychiatric hospital in the Western Cape. Sixty- eight nurses participated in the study. The data was analyzed statistically using the SAS v9 statistical software and Statistical Package for Social Sciences (SPSS). The open-ended question was analyzed qualitatively. The results revealed that the participating nurses were dissatisfied with remuneration, recognition and appreciation, training and development, as well as benefits and incentives. Nurses were most satisfied with supervision and support, interpersonal relationships, and rendering patient care. The study identified the factors influencing job satisfaction and job dissatisfaction of nurses in a provincial psychiatric hospital. Recommendations were made based on the results of the research.</p>

The entrance space of a hospital has formed an effective period of hospital life since it has reflected the scope and the image of the entire facility. Therefore it has directly influenced by the new formation of healthcare facilities on preserving and growing role of the community health instead of threatening the illnesses. Since entrance space is apart from the other spaces in the facility that has shaped by the restrictive nature of the medical technology, the space most efficiently implement these new objectives more than any space of the entire facility. However, entrance spaces of hospitals in Turkey are still bothered with many insufficiencies and displayed a problematic panorama. Therefore, this thesis has obtained the problems of main entrance space, and has analyzed it with a consistent form of criteria to gather new solution proposals. In order to serve for this purpose, the present Turkish health care environment has explored and entrance space has been analyzed around new emerging concepts that reflect the changing ideals of the community. To present the problems and solution proposals about this specific place sufficiently, they are also evaluated through design and performance criteria. The essence of coping with the stress created by the environment with healing potential is emphasized. As a result, this thesis is expected to influence further researches, new hospital main entrance space designs as well as the renovation of older ones.
