Postmilitary Life Satisfaction and Social Support, Educational Attainment, and Length of Service

The U.S. military has downsized since the early 2000s and has plans to continue to decrease their forces. There are negative implications many veterans experience after discharge. Using the transition theory as the framework for this study, the purpose of this between-groups study was to explore the differences in life satisfaction scores between voluntarily- and involuntarily-discharged U.S. service members and the associations between educational attainment, social support, and length of service. The participants were honorable discharged U.S. service members. A total of 182 participants were included in this study; 141 were voluntarily discharged and 41 were involuntarily discharged. The participants were recruited through Walden's participant pool, flyers, and Facebook. The Satisfaction with Life Scale, the Multidimensional Scale of Perceived Social Support, and a demographic questionnaire were used to identify participants' life satisfaction scores, amount of perceived social support, and personal information. Based on the results of t tests and hierarchical linear regressions, there were no differences in life satisfaction scores between voluntarily and involuntarily discharged service members. Social support and educational attainment were statistically significant predictors of life satisfaction for discharged service members. The positive social change implications of this study include increasing mental health workers' awareness of U.S. service members and the factors that affect life satisfaction after discharge.

Patienterna fråntas självbestämmandet och blir tvungna att delta i vård och behandling mot sin vilja. När självbestämmandet fråntas patienterna övertar vårdpersonalen ansvaret vilket kan leda till ett lidande för patienterna (1991:1128).</p><p><strong>Syfte</strong>: Syftet var att beskriva patienters upplevelser av tvångsvård inom psykiatrisk slutenvård. </p><p><strong>Metod</strong>: Systematisk litteraturstudie, sex artiklar användes för uppnått resultat.</p><p><strong>Resultat: </strong>Både positiva och negativa upplevelser av tvångsvård påvisades. Patienterna beskrev ett möte med vården kantat av upplevelser kring människovärde, delaktighet i sin egen vård, kommunikation, relationer, synen på våld, hot och straff samt upplevt lidande.</p><p><strong>Slutsats</strong>: Om patienterna bemöttes med respekt, människovärde samt fick tillräcklig information kring vård och behandling blev upplevelserna av tvångsvården mer positiv. Brist på delaktighet i fråga om vård och behandling och kommunikationsbrister var ett problem då patienterna hade lite sjukdomsinskt samt inte hade förståelse för behovet av tvångsvård.</p><p>Vidare forskning med kvalitativa intervjuer om patienters upplevelser av tvångsvård föreslås för att ge utökad förståelse och kunskap.</p>

Aims: To examine how patients with experience of being involuntarily discharged from medication-assisted treatment with methadone or buprenorphine have coped with the consequences following the discharge. The study also aim to examine how the patients cope with the stigma related to their abuse of heroin and how they manage to cope with the rules regarding themedication-assisted treatment. Method: Three patients with experience of involuntary discharge from medication-assisted treatment have been interviewed. All were heroin abusing men living in Stockholm. The interviewees were recruited through the Swedish Drug Users Union and through a shelter for homeless people with drug problems. Results: All the discharged patients experienced a hard time after the discharge. One went back to abuse heroin and the other two began to buy methadone or burprenorphine on the black market. All of them also experienced it difficult to cope with the stigma and considered that there were many obstacles in their life caused by the treatment rules. Conclusion: The discharge led to harsh consequences for all the patients. Heroin addicts need to deal with a lot of stigma related both to their drug abuse and the medical assisted treatment and find it difficult to manage that.

Hur par drabbade av ofrivillig barnlöshet och vårdpersonal upplever vårdsituationen : En litteraturstudie

Syftet med denna litteraturstudie var att undersöka upplevelser av vårdsituationer hos par diagnostiserade med ofrivillig barnlöshet, och hur sjukvårdspersonal upplevde att bedriva denna vård. Studien sammanställdes under 10 veckor under vårterminen 2012 och har en kvalitativ-deskriptiv design. Upprepade sökningar i databaserna CINAHL och Pubmed kombinerades med sökning av annan litteratur såsom böcker. Artiklarna som påträffades i databaserna granskades utifrån kriterier som publikationsdatum och språk. Huvudresultatet i studien visade att sjukvårdspersonal innehar en unik position för att skapa en tillfredsställande kontakt mellan dem själva och patienterna, och att stödja patienterna under behandlingen. Patienterna berättade om vikten av att sjukvårdpersonal visar respekt, empati och kompetens och medkänsla ansågs vara den viktigaste aspekten vid vårdsituationer. Sjuksköterskor på infertilitetskliniker talade om att de anser sig ha ett stort ansvar vilket kräver stor kunskap och erfarenhet. Slutsatsen visade att det är vanligt med negativa konsekvenser (både fysiska och psykiska) för patienterna, och att det saknas riktlinjer för hur vården av dessa patienter skall bedrivas samt att det finns en brist på forskning inom detta område. / This literature review aimed to investigate the experiences of couples’ treatment for their diagnosed involuntary childlessness, and how health care providers experienced the act of providing the treatment. The review was completed at University of Gävle during 10 weeks under the first semester of 2012. The study had a qualitative-descriptive design. The literature that was used to compile the results was collected through repeated searches in the databases CINAHL and PubMed in combination with searches for other literature (such as books) in the library catalogue at University of Gävle. The writers of the study reviewed the articles from criteria such as date of publication, and the articles should have been written in English or Swedish. The main results of this study showed that nurses had a unique position to create a satisfying contact between themselves and patients, and also provide infertility patients with support. The result further showed that the health-care professionals is in a exemplary position to uphold a good first groundcontact, and thereby give infertility patients support and advise during this procedure. The nurses also stressed the importance that they had a great responsibility which called for a great knowledge and experience. The patients spoke about the importance of health care providers showing respect, empathy and competence. The most important aspect in caretaking was that the nurses show compassion, which according to the patients first of all came when the nurses showed that they understood what every couple individually, went through during treatment. The conclusion then showed that it was common with negative consequences (both fysical and psychological) for the patients and health care providers found lack of birectyons and that there was not enough research within this area.

Dam Construction and Transforming State-Society Relations in China

Before 1978, Chinese dam policies were tallied with the political movement ¡§Big Leap¡¨, people all obeyed the order no matter they were under the situation of lacked of instruments. Sanmenxia can be a special sample. ZhouEnlai¡¦s chrisma made people did their best on dam projects. This could be said that dam construction policies were methods to show the authority of the leader and the state. And further, under the system of people¡¦s commune and SOEs, the state can totally control the society. After executing the opening reform policy in 1978, social organizations were in bud in China. Since 1994, environmental non-government organizations (ENGO) dramatically grew up and played an important role in anti-dam movements, in addition the international non-government organizations joined the movement, and then the two kinds of organizations became transnational advocacy networks. However, the transnational advocacy networks successfully stopped the projects of Nu River and Hutiaoxia. In this study, the documentary analysis is the key research methodology and the dam construction is the case to illustrate the purpose of constructing dam in China. Furthermore, this thesis would make a description of the reasons of anti-dam movements, the emergence of NGOs and ENGOs, and the cooperation with INGOs to confer the transforming State-Society relations in China.

Understanding Involuntary Job Loss Among Former Newspaper Staff Photographers

This study examines former newspaper photographers' experience with being laid-off from their staff positions. The purpose was to identify emerging themes within the context of involuntary job loss, job satisfaction, and occupational identity via interviews with 8 photojournalists who experienced the phenomenon of being laid-off. The newspaper industry has long been considered both the starting point for young and aspiring photojournalism careers and the most consistent and stable venue for an income. Yet recent changes in the media landscape, particularly economic stress on traditional business models and rapid adoption of digital technology sway the occupational future of photojournalism within newsrooms. The research method employed for this study includes in-depth interviews with a hermeneutical phenomenology approach focused on involuntary job loss, job satisfaction, and occupational identity.

Involuntary hospitalisation : the discrepancy between actual practice and legal requirements in the Lentegeur Hospital (Cape Town) catchment area.

The aim of this study was to document the safeguards inherent in the Mental Health Act (MHA) of 1973, and to examine the extent " to which these are observed in practice. The research was conducted at Lentegeur Hospital in Mitchells Plain, Cape Town. The population consisted of 726 certified patients who were admitted involuntarily (i.e. under sections 9 and 12 of the MHA) from 01 January 1990 to 31 December 1990. Data for each of these patients was collected from the admission register, clinical files, administrative files, and the certified post book. In addition, the official hospital statistics were examined. Measurements obtained included demographic data, the validity of the document contents, the validity of the certification process, and an overall measure of the validity of each of t he certifications taking into account both document contents and observance of the time strictures set out in the MHA. Twenty nine patients (4,0%) were admitted by Urgency (Section 12), and 697 (96,0%) on Reception Order (Section 9). The study focused mainly on the Section 9 patients, because of the small sample size for Urgency admissions. It was found that 609 (87,4%) of the 697 admissions were legally flawed in terms of document contents criteria and the time limits in the certification process. Document content criteria were not fulfilled in: 3,0% of the Applications for Reception Order; 32,1% of Medical Certificates; 20,1% of Reception Orders; and 3,6% of Reports to the Attorney-General. In 40,0% of certifications the Report to the Attorney-General (G2/28) could not be traced. Examination of temporal safeguards revealed that the least satisfactory aspect was the delay in the completion of the post-admission Report to the Attorney-General. It was found that 32,3% of these Reports were not submitted on time. Reasons for the discrepancy ("gap") between legal standards and actual practice are discussed. Recommendations are made which could help minimise or eradicate this "gap". These include suggestions for changes in the document format, for the use of a certification booklet, for stricter control of late and inadequate documentation, and for inservice training of all those involved in the certification process. / Thesis (M.Med.)-University of Natal, Durban, 1993.

Involuntary breath stacking in children with neuromuscular disorders

ABSTRACT Rationale: Respiratory insufficiency is one of the most common causes of death in patients with neuromuscular disorders (NMD). Due to weakness and cognitive level, children with NMD often cannot perform required maneuvers to recruit lung volume. Data from cooperative adults suggest that breath stacking with a mask and one-way valve can obtain significantly higher lung volumes. Methods: To study the effectiveness of a breath stacking mask in patients with NMD, we studied 23 children (17 male, 6 female) over 3 years, mean age 11 y (range 3-19 y) and body mass 43.8 kg (range 12-80 kg). Fifteen were cognitively aware and able to communicate verbally. For involuntary breath stacking a one-way valve and pneumotach were attached to a cushioned mask that was held to the face, covering around nose and mouth with a tight seal. Flow signals were acquired to computer (AcqKnowledge BIOPAC Inc.). Tidal volumes (Vt) and minute ventilation (VE) were calculated from the recording for 30 s before and 30 s after 15 s of valve closure during which expiration was prevented. Oxygen saturation (SaO2) was measured. Results: The mean Vt before valve closure was 277 ml (range 29-598 ml). The mean increase in volume by stacking was 599 ± 558 ml (range -140 to 2,916 ml). When normalized to body mass, mean increase above normal end inspiratory level was 14.7 ± 14.7 ml/kg (range -2.7 to 52.2 ml/kg). The mean number of stacked breaths was 4.5 ± 3.6 (range 0-17). VE increased on average by 18% after stacking (p<0.05, paired t-test). There was no change in SaO2 after stacking. Four of the 23 children did not stack. Conclusions: Our findings show that breath stacking with a mask and a one-way valve can achieve breath volumes approximately 3x Vt. The mask was tolerated well, and cooperation of the child was not required.
