En kvantitativ studie genomfördes som en mindre kartläggning på några orter i Skaraborg. Det framkom i studien att kontinuiteten av vårdpersonal vid behandling av bensår varierar. Kartläggningen genomfördes under en månads tid inom både primärvård och kommuners hemsjukvård. En grupp patienter hade få omläggare medan en annan grupp hade många som lade om bensåret. Nästan alla patienter med bensår i kommunens hemsjukvård hade en patientansvarig sjuksköterska. Av dessa hade en tredjedel träffat sin sjuksköterska under den kartlagda månaden. I primärvården hade drygt hälften av patienterna med bensår en patientansvarig sjuksköterska och majoriteten av dem hade träffat sin patientansvariga sjuksköterska under innevarande månad. Kommunens patienter blev huvudsakligen omlagda av undersköterskor medan primärvårdens patienter blev omlagda av distriktssköterskor och sjuksköterskor. / A quantitative study has been done, carrying out through a small survey in some places in thecounty of Skaraborg. During a month the survey was done in both the primary care and also inthe community care. It was found that the continuity of staff was varying. One sample ofpatients had few nursing staff taking care of the patient’s leg ulcer while another sample ofpatients had many care givers. Almost all patients in the community care had a nurse who wasresponsible for the patient’s leg ulcer. A third of the patients in the community had met theresponsible nurse during the month. At the primary care more than a half of the patients had anurse who was responsible for the patient’s leg ulcer and a majority of them had seen the nurseduring the month. The patients in the community for the most had been taken care of by theassistant nurse who had changed the bandage of the leg ulcer while in primary care, the patientshad been taken care of by a nurse or a district nurse.

Att leva med venösa bensår : en kvalitativ intervjustudie om patienters upplevelser / Living with venous leg ulcer : a qualitative interview study about patients´experiences

<p>I Sverige lider ca 50 000 personer av bensår vilket definieras som ”sår på ben och/eller fot nedom knät som ej läkt på 6 veckor”, ungefär hälften av dessa är venösa bensår. De flesta som drabbas av bensår är 65 år eller äldre. Syftet med denna studie var att beskriva patienters upplevelse av att leva med venösa bensår. Studien är baserad på en kvalitativ metod med en induktiv ansats. Data samlades in genom intervjuer från sex patienter och analyserades sedan med hjälp av innehållsanalys. Resultatet baseras på två kategorier; begränsat och bundet liv samt önskan att bli sedd. Dessa kategorier bildade temat; att vara hel men ändå inte. Utifrån denna studie kan personal som vårdar och behandlar bensårspatienter få ökad kunskap om hur patienter upplever att det är att leva med venösa bensår. Med hänsyn till detta vore det önskvärt att effektiva och välstrukturerade vårdrutiner utarbetas för att uppnå helhetssyn inom vården och behandlingen av bensårspatienter.</p> / <p>Approximately 50 000 people in Sweden are suffering from leg ulcers which is defined as "wounds on the legs and/or foot below the knee, which is not healed in 6 weeks", about half of them are venous leg ulcers. Most people who suffer from leg ulcers are 65 years or older. The purpose of this study was to describe patients' experiences of living with venous leg ulcers. The study is based on a qualitative method with an inductive approach. Data were collected through interviews from six patients and analyzed with help of content analyzing. The results are based on two different categories: limited and restricted life, and desire to be seen. These categories formed the theme; to be whole but still not. Based on this study, staff who care for and treat leg ulcer patients can increase knowledge about how patients feel it is to live with venous leg ulcers. Given this, it would be desirable that there are effective and well-structured care practices designed to achieve holistic healthcare and treatment of patients with leg ulcers.</p>

Yes / BACKGROUND: Venous leg ulcers (VLUs) are the commonest cause of leg ulceration, affecting 1 in 100 adults. There is a significant health burden associated with VLUs - it is estimated that the cost of treatment for 1 ulcer is up to pound1300 per year in the NHS. The mainstay of treatment is with graduated compression bandaging; however, treatment is often prolonged and up to one quarter of venous leg ulcers do not heal despite standard care. Two previous trials have suggested that low-dose aspirin, as an adjunct to standard care, may hasten healing, but these trials were small and of poor quality. Aspirin is an inexpensive, widely used medication but its safety and efficacy in the treatment of VLUs remains to be established. METHODS/DESIGN: AVURT is a phase II randomised double blind, parallel-group, placebo-controlled efficacy trial. The primary objective is to examine whether aspirin, in addition to standard care, is effective in patients with chronic VLUs (i.e. over 6 weeks in duration or a history of VLU). Secondary objectives include feasibility and safety of aspirin in this population. A target of 100 participants, identified from community leg ulcer clinics and hospital clinics, will be randomised to receive either 300 mg of aspirin once daily or placebo. All participants will receive standard care with compression therapy. The primary outcome will be time to healing of the reference ulcer. Follow-up will occur for a maximum of 27 weeks. The primary analysis will use a Cox proportional hazards model to compare time to healing using the principles of intention-to-treat. Secondary outcomes will include ulcer size, pain evaluation, compliance and adverse events. DISCUSSION: The AVURT trial will investigate the efficacy and safety of aspirin as a treatment for VLU and will inform on the feasibility of proceeding to a larger phase III study. This study will address the paucity of information currently available regarding aspirin therapy to treat VLU. TRIAL REGISTRATION: The study is registered on a public database with clinicaltrials.gov ( NCT02333123 ; registered on 5 November 2014).

I Sverige lider ca 50 000 människor av svårläkta bensår och av dessa är ca 50 procent or-sakade av venös insufficiens. Patienter med bensår upplever ofta att de påverkas negativt fysiskt, psykiskt och socialt. Syftet med studien var att beskriva distriktssköterskors upple-velser av att kunna främja hälsa hos patienter med venösa bensår. Metoden som valdes var kvalitativ innehållsanalys med induktiv ansats. Data samlades in genom intervjuer med nio distriktssköterskor. Ur analysen framkom fyra teman; Samtala och informera för att skapa ökad förståelse, Arbeta med livsstilsrådgivning, Kunna underlätta vardagen samt Kunna se hela människan i omvårdnaden. Slutsatsen är att distriktssköterskorna blir ett stöd för pati-enterna, både under den tid de har sina sår och även genom att ge patienterna kunskap för att förebygga uppkomst av nya sår. Distriktssköterskorna upplever att de kan främja hälsa genom att flytta fokus från bensåret och istället se till hela människan. Fortsatt forskning inom området behövs ur ett patientperspektiv; hur patienter med venösa bensår upplever att distriktssköterskor kan ge stöd och främja hälsa. / Approximately 50 000 people in Sweden are suffering from slow healing leg ulcers, and about 50 per cent of these ulcers are caused by venous insufficiency. Patients with leg ul-cers often experience being adversely affected physically, psychologically and socially. The purpose of the study was to describe district nurses experiences of being able to pro-mote health in patients with venous leg ulcers. The chosen method was qualitative content analysis with inductive approach. Data was collected through interviews with nine district nurses. The analysis resulted in four themes; Communicate and inform to increase com-prehension, Work with lifestyle counselling, Being able to facilitate everyday life and Be-ing able to have an holistic approach in nursing care. The conclusion is that district nurses become a support to the patients during the time they are affected by ulcers, and also by providing the patients with knowledge of how to prevent the emergence of new ulcers. District nurses experience they are able to promote health by not only focus on the ulcer but also see the person behind the ulcer. Further research is needed from a patient perspec-tive; how patients with venous leg ulcers experience that district nurses can provide sup-port and promote health.

Patienters upplevelser av att leva med svårläkta bensår : en litteraturstudie

Introduction: Hard-to-heal leg ulcers are common, especially among the elderly. A leg ulcer is not a disease in itself but a symptom of an underlying disease. Venous insufficiency is the most common cause. The quality of life deteriorates for the patient and the treatment time is often long and resource-intensive for healthcare and society. The district nurse has an important role in the meeting with the patient. With knowledge of the patient's experience of living with hard-to-heal leg ulcers, good care can be provided.   Aim: The aim of this literature study was to describe patients' experiences of living with hard-to-heal leg ulcers.   Method: A descriptive design with a general literature study as a method. The study was based on 17 articles with a qualitative method   Results: The results showed 3 main categories and 15 subcategories. The main categories were living with one's body, a changed everyday life and experience of the care situation. Patients described that it was difficult to identify oneself with a body with wounds. The pain was the most prominent symptom that affected sleep, mental state, and led to physical limitation and activities were limited. Smell, exudate and bandage contributed to feelings of shame. Relationships were affected and many isolated themselves. Patients developed different coping strategies and never stopped hoping for healing. The treatment time was long and the care relationship was of great importance.   Conclusion: Living with hard-to-heal leg ulcers was a subjective experience. The results showed that the wound had a negative effect on the quality of life. The care relationship was of great importance and that it was favorable for wound healing and quality of life when the patient felt seen as a person and was involved in the care. Applying person-centered and holistic care was essential for good nursing.

Syftet med studien var att undersöka kommunsjuksköterskan omvårdnadsdokumentation om bensår i hemsjukvård. Som metod valdes journalgranskning. Resultatet visade att omvårdnadsdokumentationen brister på många sätt. / Background: In Sweden, approximately 50 000 people suffers from slow-healing leg ulcers. The number of municipal registered nurses ́ has decreased in recent years. The municipal registered nurses ́ often hold function as an advisory consultant for the nursing staff and without participating in nursing care. This documentation is based on second-hand information from the nursing staff. Purpose: The aim was to investigate how municipal registered nurse care documents about leg ulcers in patients in home care practice. Method: An audit of patient journals was chosen to investigate how municipal registered nurse ́ care documents about leg ulcers in patients in home care practice. Results: The results are presented in five themes: contact cause, health and medical history, skin functions alternatively skin / tissue, skin action alternatively action / prescription skin / tissue and Ulcer case record. Beneath these themes the assessment of documentation was into incomplete, partially complete and complete. The result shows that the municipal registered nurse often documents in chronological order and not applying to the nursing process. The results also showed that the old documentation was not completed in the journal, which meant that it was difficult to follow a course of treatment. Discussion: The result shows that documentation in home care failures in many ways. Municipal registered nurse works as a consultant and is not involved in daily care. The documentation is based on second-hand information from the nursing staff and resulting in the documentation of leg ulcers is poor. Patient journals documentation was divided into different keyword, which means that it was difficult to follow the nursing process.

Bakgrund: Många personer lever idag med någon typ av bensår, varav venösa bensår är de allra vanligaste. Bensåret uppkommer ofta till följd av ett yttre hudtrauma hos de med venös insufficiens. Sjuksköterskan har en viktig roll när det kommer till omhändertagande och behandling av patienter i denna grupp. Syfte: Syftet med litteraturstudien var att undersöka patienters upplevelser av att leva med svårläkta venösa bensår. Metod: Litteraturstudie med kvalitativ ansats där tio studier analyserats. Resultat: Litteraturstudien utmynnade i fyra kategorier samt tio underkategorier. Svårläkta venösa bensår påverkar patienters förmåga att utföra vardagliga aktiviteter, dels på grund av de fysiska symtomen och dels på grund av den negativa psykiska inverkan som känslor av skam och nedstämdhet har. Såret innebär att ingå i ett förhållande med sjukvården, där kontinuitet och delaktighet är viktigt för patienterna. Olika metoder för att hantera situationen framkom. Konklusion: Resultaten visar på vikten av att se på patienten och omvårdnaden ur ett helhetsperspektiv, där inga aspekter får glömmas bort. Reflektioner över litteraturstudiens resultat leder till en bredare kunskap om hur vården kan förbättras för dessa patienter. / Background: Many persons are affected by hard-to-heal wounds with venous leg ulcers being the most common. The ulcer is usually caused by trauma to the leg in patients suffering from venous insufficiency. Nurses carry an important role in caring for this group of patients. Aim: The aim of this literature study was to explore patients’ experiences of living with hard-to-heal venous leg ulcers. Method: Literature study with a qualitative approach, where ten studies have been analyzed. Results: Four categories and ten subcategories emerged. Hard-to-heal venous ulcers greatly affect patients’ everyday life, partly due to the physical symptoms and partly to the psychological impact of feelings of shame and depressive thoughts. Developing an ulcer means entering into a relationship with health care professionals, such as nurses, and the study found that patients place a high value on the continuity of care and involvement. Different strategies to cope with the situation emerged. Conclusion: The results point to the importance of holistic care and provide a broad understanding of the experiences of patients’ living with hard-to-heal venous ulcers; the results can be used to improve care for these patients.

Effekten av antiseptiska och mikrobreducerande förband på svårläkta bensår : en systematisk litteraturstudie

Introduktion En stor del av distriktsköterskans arbetstid utgörs av sårvård. Cirka två procent av befolkningen drabbas någon gång av svårläkta bensår vilket orsakar stort fysiskt och psykiskt lidande för dessa personer. Studier på andra sårtyper visar att användning av antiseptiska och mikrobreducerande förband har en positiv effekt på sårläkning. Denna studie sammanställer studier om effekten av förband med honung, silver, jod, PHMB och DACC på svårläkta bensår. Syfte Att sammanställa studier som undersöker effekten hos antiseptiska och mikrobreducerande förband som används i dagens sjukvård för att behandla svårläkta bensår. Metod Systematisk litteraturstudie. Sökningar gjordes i databaserna PubMed, Cinahl och Cochrane, samt i referenslistor till review studier som framkom vid sökningarna. Efter en urvalsprocess hittades 15 studier som besvarade syftet.   Resultat De inkluderade studierna gav inget entydigt resultat. Honungsförband förbättrade läkningen eller var likvärdigt med standardbehandling i de inkluderade studierna Silverförband hade en positiv effekt på läkningen i övervägande del av de inkluderade studierna. I en studie var ett annat antiseptiskt ämne bättre än silver och i en studie visade sig silverförband vara likvärdigt med standardbehandling. Jodförband visade sig vara likvärdigt med silverförband i fråga om läkning och PHMB-förband var likvärdigt med standardbehandling Slutsats Majoriteten av de inkluderade studierna tyder på att antiseptiska förband har en positiv effekt på sårläkningen hos svårläkta bensår. Då ingen av studierna visade att de undersökta förbanden var sämre än standardbehandling, eller hade negativa effekter på sårläkningen, så är det förenligt med evidensbaserad omvårdnad att använda förbanden på patienter med svårläkta bensår utan risk för att fördröja eller försämra läkningen. / Introduction A considerable part of the district nurse's working time consists of wound care. Approximately two percent of the population will sometime be affected by hard to heal leg ulcers which is the cause of physical as well as psychic suffering for these people. Studies of other wound types shows that the usage of antiseptic and microbe reducing wound dressings have a positive effect on wound healing. This study review studies about the effect of wound dressings with honey, silver, iodine, PHMB and DACC on hard to heal leg ulcers.   Aim To review studies that examines the effect of antiseptic and microbe reducing wound dressings that are being used in today’s health care to treat hard to heal leg ulcers. Method Systematic review. Searches were made in databases Pubmed, Cinahl and Cochrane as well as manually among references in review articles that were found through the searches. After a selection process 15 studies were found which proved to be relevant to the study’s aim.   Results The included studies gave no coherent result. Honey dressings improved healing or were equivalent to standard care in the included studies. Silver dressings had a positive effect on wound healing in the predominant proportion of the included studies. In one study, another antiseptic substance had better results than silver, in another study silver dressings turned out to be equivalent to standard care. Iodine dressings turned out to be equivalent to silver dressings in the case of healing and PHMB dressings were equivalent to standard care. Conclusion The majority of the included studies indicates that antiseptic wound dressings have a positive effect on healing of hard to heal leg ulcers. None of the included studies showed that the examined wound dressings were less effective than standard care or had any negative effects on wound healing. Therefore, it can be considered safe and compatible with evidence-based nursing to use the antiseptic dressings on hard to heal leg ulcers.
