Engaging men with penile cancer in qualitative research: reflections from an interview-based study

Yes / Aim To explore the challenges of engaging men with penile cancer in qualitative interview research. Background Qualitative interviewing offers an ideal tool for exploring men’s experiences of illness, complementing and providing context to gendered health inequalities identified in epidemiological research on men. But conducting interviews with men can be challenging and embarking on a qualitative interview study with males can feel like a daunting task, given the limited amount of practical, gender-sensitive guidance for researchers. Reflecting on a researcher’s experience of conducting qualitative research on men with penile cancer, this paper explores the potential challenges of interviewing this group, but also documents how engagement and data collection were achieved. Review methods This is a reflective paper, informed by the experiences of a male researcher (KW) with no nurse training, who conducted 28 interviews with men who had been treated for penile cancer. The researcher’s experiences are reported in chronological order, from the methodological challenges of recruitment to those of conducting the interview. Implications for practice/research The paper offers a resource for the novice researcher, highlighting some advantages and disadvantages of conducting qualitative interview research as a nurse researcher, as well as recommendations on how to overcome challenges. Conclusion Engaging men with penile cancer in qualitative interview raises practical, methodological, ethical and emotional challenges for the researcher. However, when these challenges are met, men will talk about their health. Methodological procedures must enable an open and ongoing dialogue with clinical gatekeepers and potential participants to promote engagement. Support from colleagues is essential for any interviewer, no matter how experienced the researcher is.

Blood Sugar and Brothers' Voices: An Exploratory Study Of The Self-Care Management Experiences of African-American Men Living With Type 2 Diabetes

Self-care is the key to living a long and healthy life for people with diabetes. Yet numerous studies show that self-care is far from optimal. This has resulted in attempts to understand the progress underlying self-care behavior in the efforts of mediating more effectively. While there are an abundance of studies focusing on African-American women and diabetes management, there is a considerable gap in health education literature regarding the self-care management experiences of African-American men living with type 2 diabetes. The management and impact of type 2 diabetes on the sense of self, lifestyle, and significant others of the African-American man are not clearly comprehended. Therefore, the purpose of the study was to explore the knowledge, beliefs, and self-care management practices of African American men living with type 2 diabetes. This was an exploratory study utilizing qualitative methodology to understand the knowledge, beliefs, and existing self-care management practices of up to 50 African-American men, ages 18-70, living with type 2 diabetes. Participants had one-on-one semi structured interviews with the primary investigator. The instruments that were used consisted of two parts: one was a self-administered paper/pencil questionnaire to collect socio-demographic information. The second was a semi-structured interview. The study significance was to identify and understand the barriers to type 2 diabetes management and how they had an adverse influence on self-care. A combination of high barriers, poor daily self-care, management, and knowledge possibly put those with type 2 diabetes at an elevated risk for subordinate self-care management. Barriers had a role in reality and diabetes researchers and educators are charged with recognizing the complications of what individuals with type 2 diabetes need to do improve and enhance their health. The study findings revealed that the participants labored with managing their diabetes and some counted on their own potential or performance, as well as the encouragement of their family, extended family, neighbors, and friends. Also, many of the participants had a unified theme in that regarding self-care management, having type 2 diabetes affected those in their surrounding environment, not just the person living with the illness. In conclusion, the study findings suggest that future diabetes research and education among African-American men should give attention to male masculinity and the powerful influence it has on utilizing preventive health services. The limited amount of African-American men included in empirical type 2 diabetes research could help explain the under addressed barriers and complexities to positive self-care management.

Trata-se de uma pesquisa exploratória e descritiva de abordagem qualitativa, realizada com 27 homens adolescentes/jovens de 17 a 19 anos, matriculados em cursos técnicos, vinculados ao Programa Jovem Aprendiz de uma Escola Técnica localizada em Recife-PE. Foram realizadas entrevistas individuais semiestruturadas, que foram gravadas, transcritas e submetidas a análise, a qual foi orientada pela técnica de análise de conteúdo na modalidade temática. Para organização dos dados utilizou-se o software Atlas.ti (versão 7.5.3). Foram identificadas três categorias temáticas: 1) “Ser homem” adolescente/jovem; b) Homens e saúde; e c) “Ser homem”, adolescente/jovem, saúde e trabalho. Os resultados revelaram que os significados de masculinidades se basearam em modelos hegemônicos, cuja relação com a saúde perpassa compreensões de invulnerabilidade e dificuldades no cuidado. Entretanto, identificaram-se variações nesse cenário, com a presença de homens jovens que afirmaram outros modelos de masculinidades e a importância da promoção/prevenção da saúde. O que permite concluir acerca da importância que os estudos de gênero e masculinidades têm no contexto da saúde, em especial em práticas de educação em saúde, tendo em vista que o modo como os homens jovens se posicionam nos discursos de masculinidades podem se relacionar a concepções e práticas de saúde, assim como práticas de saúde podem ser mecanismos utilizados para construções de modelos de gênero. Além de sua relevância como tema de debate no intuito de desconstruir estereótipos de gênero e masculinidades, fortalecendo a adoção de novos posicionamentos que possam repercutir a importância da prevenção e promoção da saúde. / Cultural models of gender and masculinities are highly influential elements in the men’s behavior patterns and about the meanings that they elaborate about the masculine. In everyday relationships they relate to several dimensions, including health. Then, depending on as the men understand and express the relationships of gender and masculinities, these can influence behavior and peculiar relationships with health. Furthermore, it is known that these constructions involve the entire population of men, among them, in a particular way, adolescents/young. Therefore, the present study aimed to understand the meanings of masculinities and health care for male adolescents/young in professional qualification process. It is a descriptive exploratory study of qualitative approach, accomplished with 27 adolescents/ young men with 17-19 years old, enrolled in technical courses, linked to the Jovem Aprendiz Program in a Technical School located in Recife-PE. Semi-structured individual interviews were realized, which were recorded, transcribed and subjected to analysis, using the content analysis technique in thematic modality. For organize the data we used the Atlas.ti software (version 7.5.3). Three thematic categories were identified: 1) “to be man” adolescent/young; b) Men and health; and c) “to be man”, adolescent/young, health and work. The results have revealed that the meanings of masculinities were based on hegemonic models, whose relationship to health pervades understandings of invulnerability and difficulties in the health care. However variations were identified in this scenario, with the presence of young men who claimed other models of masculinities and the importance of promotion and health prevention. What can be concluded about the importance of gender studies and masculinities have in the context of health, especially in health education practices, given that the way young men are positioned in speeches of masculinities can relate to conceptions and health practices, as well as health practices can be mechanisms used for constructions of gender models. In addition to its relevance as a theme of debate in order to deconstruct stereotypes of gender and masculinities, strengthening the adoption of new positions that may reflect the importance of prevention and health promotion.

HIV-Specific Unsupportive Social Interactions, Health, and Ethnicity in Men Living With HIV

Individuals living with HIV experience higher rates of stigmatizing social interactions that may negatively impact psychological and physical health. We examined depressive symptoms as a mediator of the relationship between HIV-specific unsupportive social interactions (USIs) and health behaviors in 87 Black and White men living with HIV (MLWH). We also examined ethnicity as a moderator of this model. Depressive symptoms were an indirect mechanism through which HIV-specific USIs explained poorer health behaviors. The indirect effects between disconnecting USI, more depressive symptoms, and poorer health behaviors were significant for Black men but not for White men. Depressive symptoms may be one pathway through which USI are associated with physical health, and disconnecting USI may be particularly detrimental for Black MLWH.

Mortality and sexuality after diagnosis of penile cancer: a participative study

Yes / Survival for penile cancer is high but treatment can have a long-term detrimental effect on urological function and quality of life. Owing to its rarity, it is difficult to include men with penile cancer in research about their condition.The aim of this study was to identify aspects of their diagnosis and treatment that they would want explored in penile cancer research. The study employed a participative, mixed qualitative methods design; it utilized focus groups and patient-conducted interviews, combined into a 1 day ‘pilot workshop’. The data were analysed using framework analysis. ‘Early signs and seeking help’, ‘disclosure of a ‘personal’ cancer’ and ‘urological (dys)function’ emerged as three key themes. Men with penile cancer want research about their condition to explore early signs and helping seeking, disclosure of a ‘personal’ cancer and urological (dys)function. Research could use methodologies that include consideration of the chronological narrative of the experiences of men with penile cancer, which could be applied in clinical practice by integrating opportunities to explore specific aspects of their experiences at appropriate times along the care pathway. / National Institute for Health Research (NIHR); Research for Patient Benefit (RfPB) Programme. Exploring male disordered eating: a hermeneutic study of men’s relationships with food, body and self

Disordered eating in men is said to be uncommon with men forming less than ten per cent of reported cases. Yet it has been suggested that the number of males with eating disorders is beginning to increase, affecting more men than ever before. This presents problems for healthcare services that have created previous models of support around women. Current research offers pathological and epidemiological data, including information about testing men for eating disorders using male-centred instruments. However, understanding is aetiolated due to a focus on medical accounts of male disordered eating, rather than focusing on the stories that men themselves have to tell. This exploratory qualitative study addresses this problem by focusing on men’s stories of disordered eating. Four men share their accounts of living with an eating disorder and these are analysed using a hermeneutic approach, in order to glean insights into their experiences with food, body and self. These insights include an examination of the meaning of fat male bodies, analysis of the phenomenon of gender ambivalence that prevails in men with disordered eating and the fragmented and colonised nature of the male eating disordered self. These represent an original contribution to understanding as they have not been considered elsewhere in the literature to date. Additionally, an extensive analysis of the male eating disorder literature, use of a hermeneutic methodology and the positioning of male eating disorders within the context of masculinities studies further add to the novel value of this research.

Prostate Cancer and PSA Testing: Implications of Provider-Patient Communication and Shared- Decision Making on National Screening Recommendations

The national recommendations for use of the prostate specific antigen (PSA) test for prostate cancer screening have been modified over the years as scientific evidence emerged. Current screening recommendations discourage widespread PSA screening for men at low to average risk, but provide specific guidelines for shared-decision making between men and their health providers about the benefits and risks of PSA testing. This study was an examination of relationships between men’s assessment of the quality of their care and communication with their health providers, the extent to which providers engage men in recommended discussions about PSA testing, and factors associated with shared-decision making and PSA testing. Secondary data from the U.S. Health Information National Trends Survey 4, Cycle 2 that included men with no history of prostate cancer and in the recommended age ranges for prostate cancer screening were analyzed (N=777). Non-Hispanic white men rated their quality of care higher than men of other races (c2 (49, n=635) = 7.23, p = 0.0098), whereas Hispanic men gave the lowest ratings compared to other men (c2 (49, n=635) = 5.42, p = 0.024). Previous PSA testing was reported by 64% of the men, 56% of whom stated that they discussed screening with their provider and 80% reported that they were asked if they wanted to have the test done. However, only 21% - 39% reported having ever discussed the pros and cons of PSA testing. Discussing PSA testing with a provider was the strongest predictor of obtaining the test (OR=69.5, CI = 23.6 – 204.6) but the effect was significantly modified when providers and patients engaged in the shared-decision making process (OR = 47.42, CI = 14.91 – 150.74). Age, education level and perceived quality of care were consistent, positive predictors of PSA testing. These results indicate there is a gap in provider-patient discussions about PSA screening and suggest that health providers may not be following the recommended guidelines for the content of the discussions needed to facilitate shared-decision making. Effective provider-based interventions to increase shared-decision-making about PSA testing are needed if the national objectives for prostate cancer screening are to be met.

Os sujeitos da pesquisa foram os graduandos de enfermagem do 10º período da Universidade Estácio de Sá (UNESA/RJ). Após a apreensão dos significados e interpretação do “mundo - da - vida” dos graduandos de enfermagem desvelou que a saúde do homem é negligenciada em relação às demais disciplinas como saúde da mulher e da criança. Denotando como resultados deste estudo que é urgente e imediata a intervenção neste cenário visando à formação do enfermeiro e viabilizando o acesso do graduando de enfermagem em relação à saúde do homem em seus diversos ciclos da vida. Apesar de avanços no campo da saúde do homem, muitas questões ainda precisam ser consideradas no cuidado destes com a própria saúde. Uma questão importante seria como esta população avalia seu próprio estado de saúde, bem como os fatores que estão associados a esta avaliação pois permite identificar o perfil desta população ainda pouco estudada, o que pode, assim, auxiliar no planejamento de medidas de prevenção e promoção de saúde, bem como subsidiar estratégias necessárias para o maior alcance destes homens em termos de saúde. O objetivo do presente estudo foi avaliar condições de saúde auto-referidas, e os possíveis fatores relacionados, da população masculina na cidade de Belo Horizonte (MG), nos anos de 2006 e 2011. Trata-se de um estudo epidemiológico longitudinal, que se propõe fazer a análise desta população em 2006 e 2011, através de inquérito telefônico (VIGITEL 2006 e 2011). As amostras foram compostas por 782 indivíduos em 2006 e 836 em 2011, com idades superiores a 18 anos. Os dados foram processados em um banco de dados criado por meio do software Statistical Package forthe Social Sciences (SPSS), versão 20.0, que permitiu verificar a associação entre cada uma das variáveis independentes com a variável dependente, através da utilização do teste Qui-quadrado (χ²). Houve uma significativa mudança na idade dos indivíduos entrevistados, onde a média passou de 41,9 em 2006 para 44,2 anos em 2011; diminuição significativa, de 59,2% para 51,7% (2006 e 2011, respectivamente), do consumo alcoólico;melhora nas condições de saúde auto-referidas, aumentando de 21,1% (0,18 – 0,23) para 28,1% (0,24 – 0,31) a proporção de indivíduos que referiram a saúde como excelente. Esses dados chamam a atenção para a necessidade de políticas públicas e ações de saúde voltadas para essa população, com ênfase em ações educativas, preventivas, e de promoção à saúde. Apesar de o VIGITEL constituir um importante instrumento de coleta de dados que favoreçam o entendimento acerca das doenças crônicas não transmissíveis, a inclusão de questões referentes às condições econômicas dos participantes seria importante para se traçar com maior clareza os fatores associados às morbidades nas populações estudadas. / The specific focus in the relationship between man and health has been occurring, in the last years, both in academic terms and in scope of health services. For many reasons, it's a challenge to include man's participation in care health. Although there are advances in the field of human health, many questions have to be considered in the care of man with own health. An important point would be how this population evaluates its own state of health, as well as the factors that are associated to this evaluation because it allows us to identify this population profile, which is still not very studied, which may, thus, help in the planning of prevention methods and health promoting, as well as support strategies, which are necessary to a higher reach of these men in terms of health. The objective of this study was to evaluate conditions of self-reported health and the possible factors related of the male population in Belo Horizonte (MG) in 2006 and 2011. This is a longitudinal epidemiological study which proposes to do an analysis of this population in 2006 and 2011 through telephonic survey (VIGITEL 2006 and 2011). The samples were composed of 782 people in 2006 and 836 in 2011 with more than 18 years. The data were processed on a database created through the software Statistical Package for Social Sciences (SPSS), version 20.0, which allowed to verify the association between each one of the independent variables with the dependent variable, through the use of qui-square test. There was a meaningful change in the interviewed individuals age, in which the average changed from 41.9 in 2006 to 44.2 years old in 2011; meaningful decrease, from 59.2% to 51.7% (2006 and 2011, respectively), in the alcoholical consume; self reported health condition improvement, increasing from 21.1% (0.18-0.23) to 28.1% (0.24-0.31) the individuals proportion that referred to the health as excellent. These data call our attention to the need for policies and health public actions focused on this population, with emphasis on educational actions, preventive and promotion to the health. Although VIGITEL be an important tool for data collection to encourage the understanding of chronic diseases, including questions relating to economic conditions of the participants would be important to more clearly delineate the factors associated with morbidity in the populations studied.
