Psychosocial predictors of quality of life post Myocardial Infarction : a prospective cohort study in Pakistan

Introduction: The current study examined the psychosocial predictors (Type D personality, anxiety, depression & social support) in patients following MI. It further assessed the influence of these psychosocial predictors on quality of life of MI patients. Methods: In this prospective cohort study, 300 patients with definite myocardial infarction were recruited and assessed at baseline (2 -8 weeks) post MI (time 1). Out of 300 participants 191 completed assessments at 9 months follow up (time 2). Type D personality was evaluated with Distress scale 14(DS-14). Depression and anxiety were assessed with Hospital anxiety and depression scale (HADS). Social support was measured with Social support scale (SSS), while quality of Life was assessed with WHO quality of life scale (WHOQOL-BREF) at time 1 and time 2. Results: Analysis revealed that a significantly high percentage of MI patients had Type D personality characteristics, high levels of anxiety, depression and low level of social support. Type D personality and depression emerged as most significant predictors of quality of life after controlling for sociodemographic and clinical variables at time 1 and time 2 assessments. Discussion & Conclusion: This research emphasized the importance of Type D personality, anxiety, depression, and lack of social support in risk stratification for adverse outcomes such as impaired quality of life. This research highlighted the need for a more personalized approach to therapeutic interventions along with medical treatment for the management and rehabilitation of MI patients.

Eles possuem, ainda, espinhos curtos e um periprocto disposto posteriormente (MOOI, 1990a). Esta ordem chegou a ser representada por aproximadamente 800 espécies em 67 gêneros. Durante o Eoceno (50 milhões de anos atrás), 60% de todas as espécies de equinóides pertenciam a ordem Cassiduloida (KIER, 1962). Cassidulus mitis Krau é uma espécie endêmica do Rio de Janeiro. Para verificar seu ciclo reprodutivo foram feitas coletas mensais na Praia Vermelha de outubro de 1998 a abril de 2000. A razão sexual encontrada foi de 1:1 (x2= 1.48; a0.05=3.84; DF= l). A reprodução é contínua e não há sincronia entre os indivíduos. O índice gonadal dos machos foi superior ao das fêmeas (x2=18.33, a0.05=3.84). Os machos possuem cinco estágios de desenvolvimento gametogênico: Crescimento, Pré-maduro, Maduro, de Liberação e Estágio de liberação e crescimento concomitante. Os machos não apresentam os estágios de Recuperação e vazio que as fêmeas possuem. Já as fêmeas possuem seis estágios: Recuperação, Crescimento, Pré-maduro, Maduro, de Liberação e Vazio. Todavia, não apresentam o estágio de liberação e crescimento concomitante que ocorre nos machos. O diâmetro médio dos ovócitos foi de 382 μm (DP=49 μm). O número médio de juvenis por fêmea foi de 99.8 indivíduos (DP= l08.25; SE= 16.32). As gônadas anteriores são comumente menores que as posteriores e diferenças nos estágios gametogênicos foram encontradas entre elas. A primeira maturação ocorre em indivíduos a partir de 17mm de diâmetro. O peso úmido do intestino é aproximadamente duas ou três vezes maior que o peso úmido da carapaça. Isto significa que C. mitis utiliza seu intestino como um cinto de lastro para evitar seu deslocamento. / Cassidulus mitis Krau is an endemic species from Rio de Janeiro, Brazil. To verify its reproductive cycle, samples were taken from October 1998 to April 2000 at Vermelha Beach. Sex ratio of 1 (x2= 1.48; a0.05=3.84; DF= l) was found for C. mitis. Reproduction occurs overall the year. Gonadal index (GI) of males was greater than GI of females (x2= 18.33, a0.05=3.84). Reproduction is asynchronous. Males have five gametogenic stages: Early growth, Premature, Mature, Partially spawned and Early growth with partially spawned stage. There is no Spent or Recovery stage as in females. Females have six gametogenic stages: Recovery, Early growth, Premature, Mature, Partially spawned and Spent stage but there is no Early growth with partially spawned stage as in males. Mean diameter of oocytes was 382 μm ( DP=49 μm). Mean number of juveniles per female was 99.8 individuals (DP= 108.25; SE= l6.32). Anterior gonads are commonly smaller than posterior ones. Differences in gametogenic stages were found between anterior and posterior gonads. First sexual maturity occurs in individuals larger than 17 mm. Intestine wet weight is about two or three fold higher than test wet weight. It means that C. mitis uses its intestine as a weight belt to avoid displacement.

Resonant and high resolution photoemission of rare-earth cobalt oxides

LnBaCo2O5+δ (Ln-112) where Ln = lanthanide element; 0 ≤ δ ≤1 and LnBaCo4O7+δ (Ln-114) are highly correlated cobalt oxides. Synchrotron photoemission spectroscopy of LnBaCo2O5+δ (Ln = Gd, Dy, Dy1-xTbx) and LnBaCo4O7 (Ln = Yb) has been undertaken at the UK Synchrotron Radiation Source (Daresbury Laboratory).During the photoemission experiments, the samples were observed to be contaminated due to residual gases inside the main vacuum chamber. The surface degradation of the samples is studied using the difference spectra generated from the valence band spectra of freshly scraped and contaminated samples and the nature of contaminated species on these samples is identified in the light of the reviewed literature. High-resolution photoemission is carried out to study the metal-insulator (MI) transition in double perovskites LnBaCo2O5+δ (Ln = Gd, Dy, Dy1-xTbx - Ln-112; 0 ≤ δ ≤ 1) as a function of temperature. The high-resolution photoemission results of single crystal samples of GdBaCo2O5.5, DyBaCo2O5.5 and Dy1-xTbxBaCo2O5+δ show that the temperature-based MI transitions in these compounds occur in the 300-400 K temperature range. A post-growth oxygen annealing treatment for as-grown single crystals of Ln-112 is necessary, achieving oxygen contents close to 5.50, to observe a marked nonmetal-to-metal transition. Resonant photoemission is used to identify the atomic parentage of the valence band states. A comparison of the electronic structure of LnBaCo2O5+δ (Ln = Gd, Dy, Dy1-xTbx - Ln-112; 0 ≤ δ ≤ 1) and LnBaCo4O7 (Ln = Yb - Ln-114) single-crystal surfaces is made using synchrotron photoemission spectroscopy. In both cases, the states close to the Fermi energy are found to be of mixed Co 3d/O 2p character, and the comparison allows identification of states due to low spin Co³⁺ in octahedral environments. The contributions from Ln elements to the valence band are found at higher binding energies.

Motivational interviewing (MI) is a counselling approach developed in part by clinicalpsychologists William R. Miller and Stephen Rollnick. It is a directive, client-centredcounselling style for eliciting behaviour change by helping clients to explore andresolve ambivalence. The method has grown in popularity during the last years and has spreadoutside the therapy room and into new fields where it is being applied such as healthcare, socialservice and the police. The usage of MI in therapeutic and healthcare settings is well researchedbut MI has recently emerged in a new field where the research is scarce: leadership. In thisstudy we are exploring the experiences of Swedish social workers that are trained in MI whichhas a manager that is also trained in the method and applies MI in leadership. Our results showthat the social workers are generally positive to their managers using the method; MI is seen asa factor that influences the manager to listen and be more empathetic. Our study also showsthat MI has become a general tool for interactions; the social workers are using the methodoutside client meetings and in interactions with colleagues and managers to facilitate a goodrelationship. However, there are also concerns among some of the respondents that MI couldbe used by their manager to manipulate and create behaviour changes that only benefits theorganisation. This explorative study shows that MI has the potential to impact management andleadership approaches and warrants further investigation to develop this growing field ofinterest.

Study of Diamond Folds in Mass-Impregnated(MI) Cables

High Voltage (HV) cables are designed to transmit electrical power from the source to the placewhere it is consumed. The global trend towards renewable energy sources e.g. wind farms, hy-droelectric and solar panels have led to an increase in the use of HV cables, as these sources aresituated in remote locations far from the areas where they are consumed. Therefore. there is aneed for developing efficient power transmission cables.Mass-impregnated (MI) cable is a type of HV cable which contains an electrical insulation layerof oil or mass impregnated cellulose paper. Due to its excellent dielectric properties, MI cableshas been established as a solution for high voltage electrical transmission system, which has beenused for several decades.The main area of interest in this thesis is the paper insulation in the MI cable. It is importantto ensure that the paper insulation structure is robust enough to avoid any formation of damageand/or defects due to excessive deformations during cable production, handling and installation,which might affect the overall electrical performance of the cable. During handling, the drylapped cable needs to be bent and unbent several times and there is a risk that the insulationwill develop voids due to structural defects, thereby reducing the dielectric capacity of the cable.An example of typical structural defects in MI cables is, so-called, diamond folds, i.e. a collec-tive buckling of several paper strips due to axial compressive load resembling a diamond shapedpattern.The goal of this thesis work is to contribute to the knowledge of the mechanism of diamond foldsin MI cable; the onset of diamond fold and post-buckling responses. The contributions from thecable design or production parameters are also studied. The analyses will be conducted usingnumerical simulation techniques and will be compared to results from characterization tests al-ready conducted in mechanics lab at NKT Technology consulting.The results of this thesis has given an insight about the effect of insulation design parameterssuch as registration, butt-gaps, coefficient of friction and number of layers, on the diamond foldbuckling behavior in an MI cable insulation. It is recommended that these parameters have tobe carefully controlled so as to avoid formation of diamond fold buckling in the insulation.

Motiverande samtal (MI) : Patientens erfarenheter av MI i ett vårdande sammanhang

I bakgrunden beskrivs hur motiverande samtal (MI) kan förebygga och behandla livsstilssjukdomar. MI har visat sig vara effektiv men har även fått kritik eftersom samtalsmetoden kan vara bristande i vårdandet av patienten. Vidare beskrivs sjuksköterskans ansvar, vad hälsa innebär och hur ett vårdande samtal kan ge patienten förutsättningar för att uppnå hälsa. Frågan är vad patienterna har för erfarenheter av MI. Syftet med denna litteraturöversikt är att beskriva patientens erfarenheter av MI i ett vårdande sammanhang. Artikelsökningar genomfördes i databaserna PsycInfo, Medline och Cinahl. Totalt 13 vetenskapliga artiklar med en kvalitativ ansats användes för att besvara syftet. Dataanalysen genomfördes med en metod för litteraturöversikt beskriven av Friberg (2017, ss. 141-152). I resultatet beskrivs patienternas erfarenheter av MI i fyra olika teman: ‘Tillåtande miljö’, ‘Utmanar till förändring’, ‘Utvecklar och utmanar autonomin’ och ‘Sjuksköterskans betydelse’. Metoddiskussionen lyfter styrkor och svagheter i val av metod och de artiklar som inkluderades i resultatet. I den avslutande resultatdiskussion diskuteras huruvida MI kan ses som ett vårdande och lärande samtal, om det är en gynnsam metod för livsstilsförändringar och autonomistöd samt sjuksköterskans betydelse under MI. Även resultatet i förhållande till hållbar utveckling diskuteras. Avslutningsvis beskrivs studiens slutsats och vad resultatet kan användas till.

Die perkutane Behandlung der sekundären Mitralklappeninsuffizienz (MI) ist eine Therapieoption für Patienten mit hohem Operationsrisiko. Der Einfluss der perkutanen Edge-to-edge-Mitralklappenrekonstruktion (PMKR) mittels MitraClip® auf die Geometrie der Mitralklappe ist noch nicht gänzlich verstanden. In der vorliegenden Arbeit wurde ein neuartiges 4-dimensionales Mitralklappenmodell, das auf Real Time 3-dimensionaler transösophagealer Echokardiographie (RT3DTEE) basiert, dazu genutzt, die morphologischen Veränderungen der Mitralklappe während des gesamten Herzzyklus vor und nach PMKR zu bestimmen. Dazu wurden retrospektiv prä- und postprozedurale RT3DTEE-Datensätze von 28 Patienten mit sekundärer MI analysiert, an denen eine elektive PMKR vorgenommen wurde. Mit Hilfe einer halb-automatischen Mitralklappen-Modellierungssoftware wurde die Mitralklappenmorphologie über den gesamten Herzzyklus in 10%-Schritten quantifiziert. Es wurden signifikante Veränderungen in der Mitralklappengeometrie der Segel und des Anulus nachgewiesen: Nach PMKR zeigten die Patienten eine sofortige signifikante Reduktion der Mitralklappenöffnungsfläche und des anteroposterioren Durchmessers über den gesamten Herzzyklus, sowie eine signifikante Vergrößerung des anterolateral-posteromedialen Durchmessers und des kommissuralen Durchmessers ausschließlich in der Diastole. Nach bestem Wissen wird in dieser Arbeit die erste 4D-Analyse der Mitralklappe über den gesamten Herzzyklus vor und nach PMKR vorgelegt. Die Ergebnisse ermöglichen ein besseres Verständnis der funktionellen und anatomischen Mechanismen der Mitralklappe.

Motiverande samtal som verktyg : Sjuksköterskans och patientens upplevelse

Folkhälsosjukdomar som bland annat diabetes typ 2 och hjärt- och kärlsjukdom blir vanligare för varje år och är en av orsakerna till den globala dödssiffran. Folkhälsosjukdomar går att förebygga genom kost, motion och att undvika faktorer som bidrar till ökad risk för ohälsa som rökning och alkoholkonsumtion. För att hjälpa patienten som riskerar att drabbas av en folkhälsosjukdom eller som redan är drabbad kan sjuksköterskan tillämpa motiverande samtal (MI) i kommunikationen med patienten. Författarna har valt att undersöka hur MI som verktyg fungerar för sjuksköterskan och patienten. Syftet är att belysa sjuksköterskans och patientens upplevelse av motiverande samtal som verktyg. Det har gjorts en litteraturöversikt efter Fribergs modell. Översikten har gjorts på 10 artiklar, både kvalitativa och kvantitativa. Resultaten delades in i sex olika teman: samspelet mellan sjuksköterska och patient, arbetsbelastning och värdet av tid, kunskap och erfarenhet, organisatoriskt stöd, behov av att ändra arbetssätt och inställning till MI som verktyg. Studien visar att samspelet mellan patienten och sjuksköterskan är mycket viktigt under arbetet med MI. Det krävs ett intresse och en vilja från sjuksköterskans sida för att kunna använda verktyget tillsammans med patienten. Sjuksköterskan behöver en bred kunskap för att kunna möta patienten. För att sjuksköterskan inte ska falla tillbaka till tidigare rutiner så är det egna intresset och stöd från organisationen essentiellt för sjuksköterskan.

Studien syftar till att undersöka olika målgruppers mottaglighet och uppfattning av Motiverande samtal (MI) utifrån ett universalistiskt och kulturrelativistiskt perspektiv. Bakgrunden är att MI är en frekvent använd samtalsmetod som används i det lösningsfokuserade sociala arbetet samt att en forskningslucka identifierats kring hur MI faktiskt tas emot och uppfattas av olika målgrupper. Studien avser analysera MI utfört i olika kontexter och utifrån både kulturrelativism och universalism. För att undersöka detta har en scoping review gjorts där 35 akademiska artiklar inom forskningsområdet valdes ut och sedan analyserades tematiskt. De fem teman som identifierats är mottaglighet och uppfattning av MI bland olika målgrupper, betydelsen av handledning och utbildning för olika målgruppers inställning till MI, MI:s påverkan för relationsbyggande och dess betydelse för mottagligheten och uppfattningen hos olika målgrupper, potentialen MI har i kombination med andra insatser för olika målgruppers mottaglighet och uppfattning, samt ökad mottaglighet och förbättrad uppfattning genom målgruppsanpassning av MI. Det huvudsakliga resultatet har varit att ett antal aspekter utifrån dessa teman bör beaktas och kan förstås bättre utifrån de två nämnda teoretiska perspektiven. Övergripande förefaller MI mottas och uppfattas väl av olika målgrupper, men flera studier tyder ändå på att resultatet av MI interventioner inte får en långvarig påverkan. / The study aims to investigate the receptivity and perception of different target populations of Motivational Interviewing (MI) from a universalist and cultural relativistic perspective. The background is that MI is a frequently used conversational method used in the solution-focused social work and that a research gap has been identified around how MI is received by different target groups. The study intends to analyse MI performed in different contexts and based on both cultural relativism and universalism. To investigate this, a scoping review has been made where 35 academic articles in the research area were selected and then analysed thematically. The five themes identified are receptivity and perception of MI among different target populations, the importance of guidance and education for different target populations attitude to MI, MI's impact on relationship building and its significance for receptivity and perception in different target populations, the potential MI has in combination with other initiatives for the receptivity and perception of different target populations, as well as increased receptivity and improved perception through target populations adaptation of MI. The main result has been that several aspects based on these themes should be considered and can be better understood from the two mentioned theoretical perspectives. Overall, MI seems to be received and perceived well by different target populations, but several studies still indicate that the results of MI interventions do not have a long-term impact.
