Bakgrund: Transition kan beskrivas som övergången från en fas, tillstånd eller status i livet till ett annat. Transition mellan barn- och ungdomspsykiatrisk och vuxenpsykiatrisk vård är en viktig hälsofråga, där patienters upplevelser är sparsamt belysta. Tidigare studier har framhållit behovet av att utveckla övergångsprocessen och att då utgå från ett personcentrerat perspektiv med förståelse för ungdomarnas förutsättningar. Syfte: Syftet med studien var att utforska unga vuxnas upplevelser av transitionsprocessen från barn- och ungdomspsykiatrisk vård till specialiserad psykiatrisk vård för vuxna. Metod: Åtta unga vuxna som gjort övergången till vuxenpsykiatrisk vård för mellan ett och fem år sedan intervjuades genom en semistrukturerad intervju. Insamlad data analyserades med kvalitativ innehållsanalys. Resultat: Analysen genererade tre huvudkategorier: Att bli sedd och bekräftad är grund för delaktighet och att känna sig värdefull, att övergången kan visa sig vara en ny möjlighet och att sammanhållet stöd behövs för tillgång till god och jämlik vård. Studien bekräftar tidigare forskning, som beskriver transitionen som en period där behov av stöd från vården och närstående kan vara avgörande. Samtidigt framkommer upplevelsen av övergången som en möjlighet att växa som människa. Kritiska punkter som kan associeras med positiva och negativa utfall i transitionsprocessen går att urskilja, vilket ligger i linje med studiens utgångspunkt i transition som omvårdnadsteori. Slutsats: Transitionen från barnpsykiatrisk vård till specialiserad psykiatrisk vård för vuxna är ett område där samskapande perspektiv och personcentrerad vård har potential att minska hälsorisker och främja personlig utveckling och återhämtning. Transition between child and adolescent psychiatric care and adult psychiatric care is an important health issue, where patients' experiences are sparsely highlighted. Previous studies have emphasized the needs of developing the transition process, starting from a person-centered perspective with an understanding of young people's conditions. Aim: The aim was to explore young adults' experiences of the transition process from child and youth psychiatric care to specialist psychiatric care for adults. Method: Eight young adults who made the transition to adult psychiatric care one to five years ago were interviewed using a semi-structured interview. Collected data was analyzed using qualitative content analysis. Results: The analysis generated three categories: that being seen and validated is the basis for participation and feeling valuable, that the transition may turn out to be a new opportunity and that cohesive support is needed to gain access to good and equitable care. The study confirms previous research that describes the transition as a period where support from care providers and relatives can be crucial. At the same time, the experience of the transition emerges as an opportunity for personal growth. Critical points that can be associated with positive and negative outcomes in the transition process can be distinguished, which is in line with the study's starting point in transition as a nursing theory. Conclusion: The transition to specialized psychiatric care for adults is an area where co-creative perspectives and person-centered care have the potential to reduce health risks and promote personal development and recovery. By exploring the experience of young adults who has undergone this transition, the study can contribute to increased knowledge of opportunities and obstacles during the process.

The purpose: This study is meant to explore the perceptions of social workers from the municipalsocial services within the cooperation with the regional healthcare, focusing on the psychiatric careunits, and if there are any power relations within this cooperation. Method: A qualitative study with a deductive approach based on semi structured interviews with fivesocial workers in the greater Stockholm area compiled by a thematic analysis of the interviews. Theory: The studies theoretical approach originates from Contingency theory and a power perspectivefrom Theory of Collaborative Advantages Results: The results are presented in four themes; Faults in cooperation, Fragmented welfare, Powerrelations within cooperation and When cooperation is successful. These are summarized by how mostcooperation is successful but certain problems occur such as a lack of communication between theorganizations, vague laws and rules regarding cooperation, a lack of mutual trust and understandingfor each other and an uneven power structure between doctors and social workers. Conclusion: The lack of cooperation is emblematic of the Swedish welfare system's highspecialization and fragmentation meant to cut cost and increase efficiency. It instead leaves clientswith dual diagnosis with no clear direction of who to turn to for help.

Bakgrund: Tidigare forskning visar på att relationen mellan sjuksköterska och patient i allra högsta grad påverkar patientens mående. Genom vårdrelationen kan sjuksköterskan stärka patientens förmåga till egenvård, hälsa och välbefinnande. Specialistsjuksköterskan inom psykiatrisk vård har ett ansvar för att personcentrerad vård bedrivs, där en bra vårdrelation är av vikt för att personcentrerad vård ska kunna utföras. Syfte: Att beskriva sjuksköterskors erfarenheter av vårdrelationen inom vuxen psykiatrisk öppenvård. Metod: En strukturerad litteraturstudie med inslag av metodologi för systematisk litteraturöversikt. Integrerad analys enligt Whittemore och Knafl valdes som analytisk strategi. Resultat: Fyra huvudkategorier formulerades i resultatet: kompetens, återhämtningsprocessen, terapeutisk relation och dubbla relationer. Huvudkategorin kompetens hade underkategorierna färdigheter och förmågor, personliga egenskaper och attityder samt kompetensutveckling. Återhämtningsprocessen hade underkategorierna återhämtning samt personcentrerad vård. Terapeutisk vårdrelation hade underkategorierna allians samt partnerskap. Huvudkategorin dubbla relationer hade underkategorierna gränser i relationen samt privata relationer. Slutsats: Sjuksköterskor behöver ett flertal olika färdigheter för kunna skapa en god vårdrelation. Målet med vårdrelationen är att främja återhämtning. En bra vårdrelation skapar möjligheter en personcentrerad vård. / Background: Previous research shows that the relationship between nurse and patient affects the patient's well-being to the greatest extent. Through the caring relationship, the nurse can strengthen the patient's ability to self-care, health and well-being. The specialist nurse in psychiatric care has a responsibility to ensure that person-centred care, where a good caring relationship is important for person-centred care to be carried out. Aim: To describe nurses' experiences of the caring relationship within adult psychiatric outpatient care. Method: A structured literature study with elements of methodology for systematic literature review. Integrated analysis according to Whittemore and Knafl was chosen as the analytical strategy. Result: Four main categories were formulated in the result: competence, recoveryprocess, therapeutic relationship and dual relationships. The main category competence had the subcategories skills and abilities, personal characteristics and attitudes, competence development. Recoveryprocess had the subcategories recovery and person-centred care. Therapeutic relationship had the subcategories alliance and partnership. The main category dual relationships had the subcategories boundaries in the relationship and private relationships. Conclusion: Nurses need several different skills to be able to create a good caring relationship. The goal of the caring relationship is to promote recovery. A good caring relationship creates opportunities for person centered care

Bakgrund: Asylsökande patienter drabbas ofta av psykisk ohälsa relaterat till stress i samband med flykt och svåra levnadsförhållanden. Som omvårdnadsansvarig ansvarar sjuksköterskan bland annat för att hjälpa människor med det som de själva hade utfört om de hade kraften. På grund av språkförbistringar och kulturbetingade förväntningar kan ibland vårdandet försvåras. För att kunna arbeta effektivt med asylsökande patienter behövs fördjupade kunskaper i hur sjuksköterskor beskriver sitt arbete med asylsökande patienter. Syfte: Syftet med uppsatsen är att beskriva sjuksköterskors erfarenheter av att vårda asylsökande patienter i psykiatrisk vård. Metod: Studien är en kvalitativ studie med induktiv ansats där semistrukturerade intervjuer genomfördes med fem sjuksköterskor verksamma inom psykiatrisk vård. Det insamlade datamaterialet analyserades enligt kvalitativ innehållsanalys. Resultat: Dataanalysen resulterade i två kategorier med fem respektive två subkategorier vardera. Den första kategorin beskriver sjuksköterskors erfarenheter av hinder och möjligheter i relation till vårdandet av asylsökande patienter med underkategorier: kulturella skillnader, egenpåverkan och behovet av handledning, behov av information, förhållningssätt samt patientens nätverk. Den andra kategorin beskriver sjuksköterskors erfarenheter av hinder och möjligheter i relation till externa instanser, med underkategorier; tolk och asylsökningsprocessen. Diskussion: Resultatet diskuteras utifrån Leiningers Soluppgångsmodell samt annan relevant forskning. Soluppgångsmodellen ger stöd för sjuksköterskor i mötet med patienter med en annan kulturell bakgrund, vilket kan vara viktigt för att arbeta holistiskt. / Background: Asylum seeking patients often suffer from mental health problems related to stress during flight and harsh living conditions. Being responsible for the care, nurses are responsible to help people with what they themselves have difficulties performing or are unable to do themselves. Due to language difficulties and cultural conditional expectations the caring of patients within this group can sometimes be suffering. In order to work effectively with asylum seeking patients in psychiatric care, it is required to possess a thorough knowledge of how nurses describe their work with asylum seeking patients. Aim: The purpose of this paper is to describe nurses' experiences of caring for asylum seeking patients in psychiatric care. Method: The study is a qualitative study with the inductive approach in which semi-structured interviews were conducted with five nurses working in psychiatric care. The collected data were analyzed according to qualitative content analysis. Results: Data analysis resulted in two categories with five and two subcategories each. The first category describes the nurses' descriptions of the obstacles and opportunities in relation to asylum seekers patients with subcategories: cultural differences, personal impact and the need for guidance, the need of information, approaches, and patient social networks. The second category describes nurses' descriptions of the obstacles and opportunities in relation to external instances, with subcategories; interpreter and asylum process. Discussions: The results are discussed from Leininger’s Sunrise Model and other relevant research. Sunrise model provides support for nurses in the meeting with patients with a different cultural background, which can be important to work holistically.

O presente trabalho foca a questão da implementação da Reforma Psiquiátrica nos Hospitais de Custódia e Tratamento Psiquiátrico, objetivando delinear os limites, desafios e possibilidades da referida Reforma no Estado de São Paulo. O trabalho, do ponto de vista metodológico, partiu da técnica de análise do discurso de entrevistas realizadas com pessoas chave no processo de discussão e implementação das diretrizes da Reforma Psiquiátrica nos Hospitais de Custódia e Tratamento Psiquiátrico Paulistas, além da revisão de literatura, subsídio a dados estatísticos e pesquisa jurisprudencial. Os principais limites e desafios para a implementação da Reforma Psiquiátrica nos Hospitais de Custódia e Tratamento Psiquiátrico no Estado de São Paulo são comuns a outros Estados da Federação. Em seu funcionamento, os HCTPs apresentam a predominância do aspecto custodial sobre o terapêutico, sendo que a contenção química e a física continuam a representar o agir institucional. As principais questões legais envolvidas e questionadas pelos entrevistados referem-se à não aplicação da Lei 10.216/01 na seara das medidas de segurança e a questão da presunção da periculosidade legal, reservada ao considerado louco infrator. A desinstitucionalização, vista como processo complexo, ainda apresenta grandes desafios nas dimensões técnico-assistencial, consistente na ampliação da rede de serviços substitutivos, bem como o acesso da população confinada nos HCTPs a essa rede e na dimensão sócio-cultural, uma vez que as pessoas com transtorno mental em conflito com a lei enfrentam uma tripla exclusão e estigmatização: vistos como loucos e criminosos e apresentando, antes mesmo da internação, um quadro de vulnerabilidade social. O Estado de São Paulo possui a maior população de internos, considerados números absolutos, e em seus três HCTPs criou-se uma espécie de regime de cumprimento de medida de segurança mais ou menos rigorosa, analogicamente ao existente em relação ao cumprimento de pena. Dentro dessa dinâmica, a existência do Programa de Desinternação Progressiva realizado em Franco da Rocha é indicado como uma possibilidade de aplicação das diretrizes da Reforma Psiquiátrica. O SAIPEMS Sistema de Atenção Integral às Pessoas em Medida de Segurança visto como uma forma de implantar, no Estado de São Paulo, um programa semelhante ao PAILI e PAI-PJ, desde 2009, aguarda um encaminhamento por parte dos entes governamentais. A questão do transtorno de personalidade e a possibilidade de responsabilização dos chamados sociopatas merecem estudos mais aprofundados, assim como a questão da mudança do perfil tradicional dos internos dos HCTPs, que são cada vez mais jovens e com questões relacionadas ao uso de drogas. Upplevelse av stöd från den psykiatriska vården till barn som har föräldrar med psykisk sjukdom : sett ur barns och vårdpersonals perspektiv

<p>Barn påverkas av föräldrars psykiska sjukdom i form av oro, rädsla och skuld samt har en förhöjd risk att själva drabbas av psykisk sjukdom. En utmaning för den psykiatriska vården är det ökande antalet av vuxna med psykiatrisk sjukdom som även är föräldrar till minderåriga barn. Sjuksköterskor inom psykiatrin har en viktig roll när det gäller att uppmärksamma dessa barn samt se till att de får adekvat hjälp och stöd. Syftet med studien var att beskriva hur barn till föräldrar med psykisk sjukdom upplever det stöd de får ifrån den psykiatriska vården samt beskriva vårdpersonals upplevelser av stöd till dessa barn. Metoden som användes var kvalitativa forskningsintervjuer som<em> </em>genomfördes med sex barn som har föräldrar med psykisk sjukdom och fjorton vårdpersonal som arbetar inom den psykiatriska vården. Intervjuerna analyserades med kvalitativ innehållsanalys och resultatet utmynnade i fyra kategorier. 1. Barn till föräldrar med psykisk sjukdom upplever sig bli hjälpta av stödet från den psykiatriska vården även om det bör förbättras. 2. Vårdpersonal upplever att den psykiatriska vården erbjuder olika verksamheter som ger ett gott stöd till barn även om det bör förbättras. 3. Vårdpersonal upplever att barn behöver uppmärksammas, informeras och stödjas i kontakten med sina föräldrar. 4. Vårdpersonal upplever svårigheter i form av osäkerhet kring sin egen kunskap samt föräldrars ovilja att blanda in barn i den psykiatriska vården. En slutsats av studien är att barn till föräldrar med psykisk sjukdom får ett relativt gott stöd även om det behöver förbättras. För att göra detta krävs utbildning av vårdpersonal och att verksamheten har framtagna rutiner och användbara metoder för att kunna stödja barn.</p> / <p>Children get affected by parents mentally illness in form of anxiety, fear and guilt and also have an enhanced risk to suffer from mentally illness themselves. A challenge to the psychiatric care is the increasing number of adults with a mental illness who also are parents to under aged children. Nurses in the psychiatric care have an important role when it comes to notice these children and make sure that they receive adequate help and support. The purpose of the study was to describe how children of mentally ill parents experience the support they receive from the psychiatric care and describe psychiatric healthcare workers experiences of these children’s support. The method that was used in this study was qualitative interviews with six children of mentally ill parents and fourteen psychiatric healthcare workers. The interviews were analysed based on qualitative content analysis and gave four categories as result. 1. Children of mentally ill parents experience help due to the support from the psychiatric care even though it should be improved. 2. Psychiatric healthcare workers experience that the psychiatric care offers a variable of activities that give a good support to children even though it should be improved. 3. Psychiatric healthcare workers experience that children need attention, be informed and supported in their relationship to their parents. 4. Psychiatric healthcare workers experience difficulties like insecurity about their own knowledge and parents’ unwillingness to involve the children in the psychiatric care. A conclusion from this study is that children of mentally ill parents receive a relatively good support even though it needs to be improved. To do that it takes education of the psychiatric healthcare workers and that the psychiatric care is given routines and useful methods to support children.</p>

Den missbrukande/beroende patientens upplevelse av omvårdnadspersonalens bemötande inom vården

<p>Syftet med denna intervjustudie var att undersöka hur patienter med missbruks- och beroendeproblem upplever attityder och bemötande från omvårdnadspersonal.<strong> </strong>Begreppet missbruk/beroende innefattar alla former av missbruk/beroende av både narkotika, alkohol och läkemedel. Uppsatsen är en deskriptiv empirisk intervjustudie med kvalitativ ansats som bygger på 10 intervjuer med drogmissbrukare/drogberoende. Urvalskriterierna har varit 5 män och 5 kvinnor i åldrarna 25 - 60 år som haft minst 5 vårdtillfällen på en sluten missbruksenhet. Intervjuerna har analyserats utifrån metoden kvalitativ innehållsanalys. Resultatet visade att de intervjuade haft både negativa och positiva upplevelser av attityder och bemötande av omvårdnadspersonal. Slutsatsen är dock att det negativa, fördomsfulla bemötandet övervägde. De intervjuade upplevde också att det råder en stor brist på kunskap om missbruks- och beroendeproblematik inom sjukvården samt att yngre, nyutbildad omvårdnadspersonal uppvisar fler fördomsfulla attityder och bemöter på ett sämre sätt än äldre, erfaren omvårdnadspersonal gör.</p> / <p>The objective of this study was to gain insight into how patients with addiction or dependency problems feel that nursing staff behave towards them. The concepts of addiction and dependency cover all forms of addiction to and dependency on narcotics, alcohol and pharmaceuticals. The essay is a descriptive empirical study based on 10 interviews with people addicted to or dependent on drugs. The selection criteria has been 5 men and 5 women aged between 25 and 60 who have been treated at least 5 times at substance abuse inpatient units. The interviews have been analyzed based on the method qualitative content analysis and resulted in two main categories: Kind treatment and Not so kind treatment. The results showed that those people interviewed had both positive and negative experience of how nursing staff behaved towards them. The conclusion, however, is that negative, prejudiced behaviour was predominating. Those people interviewed also felt that there was a large lack of knowledge about the problems of substance abuse and dependency within healthcare and that the younger, recently qualified nursing staff behaved more badly towards them and were more prejudiced than the older and more experienced nursing staff.</p>
