Staff Education Intervention to Enhance Care Planning for Older Adults

The increased prevalence of cognitive deterioration has increased the challenges of caring for older adults. This study's project site offers a psychiatric program for older adults with customized care for complex geriatric mental health patients. Clinical assessment and care management are often overlooked in geriatric mental health patients diagnosed with behavioral and psychological symptoms of dementia (BPSD). The purpose of this project was to deliver an education program developed from the Staff Training in Assisted Living Residences-Veterans Administration, P.I.E.C.E.S model and the Castle framework to 42 nursing and allied health staff of the project site. The project question explored whether an education program on care of patients with BPSD increased staff members' perceived knowledge and competence in providing care to these patients. This education program focused on comprehensive assessment, individualized care planning, and individualized nonpharmacological interventions to manage older adults with dementia. Descriptive statistics were used to analyze performance outcomes data before and after staff education. In addition, care plans of patients were reviewed. Results included a 100% increase in recognition of core concepts of the education program; a 48% to 86% increase in staff willingness to use interdisciplinary care plans; and a 6.6% to 95% increase in documentation of key interventions in care plans for 6 behaviors of dementia. The results of this project might bring about social change by improving the skills and competence of nursing staff in managing the patients with dementia, thus positively impacting the quality of life of patients with BPSD by benefiting from nonpharmacological interventions.

Computer Anxiety and Computer Self-Efficacy of Older Adults

Many older adults (aged 55 and older) need training to acquire computer knowledge and skills. Using computers and the Internet could provide access to vital resources for improving older adults' health and maintaining their connections with family and society. This study examined 2 psychological constructs--computer anxiety and computer self-efficacy--that have been shown to impact a person's successful use of computers and related technology. Guided by Bandura's self-efficacy theory, which emphasizes the importance of adult learners being motivated and taking charge of their learning, this study examined the impact of a computer knowledge and skills workshop on older adults' computer anxiety and computer self-efficacy. A concurrent, mixed-methods design was used to collect and analyze survey data and interview transcripts from a convenience sample of African American older adults (N = 11). Mobile technology (i.e., tablet PCs and portable hotspots) was used to access the Internet and e-mail. Data analyses included thematic coding of the interview notes and descriptive statistics to present the survey results. The themes that emerged from the interview data were learning opportunities, positive attitudes, and user-friendly tools and equipment for reducing computer anxiety and constructive attitude changes and learning environments for improving computer self-efficacy. The descriptive statistics indicated favorable changes for computer anxiety with scores averaging a decrease of -26.5% and computer self-efficacy with scores averaging an increase of 62.1%. This study illustrated the feasibility of a low-cost approach for establishing a mobile computer laboratory to help older persons become proficient in their use of computers, the Internet, and related technology.

Effects of Generic Group-Based Versus Personalized Individual-Based Exercise Programs on Balance, Gait, and Functional Performance of Older Adults with Mild Balance Dysfunction and Living in Residential Care Facilities - A Randomized Controlled Trial

Background and Purpose: To investigate the effect of an individualized exercise program versus a generic group-based exercise program on balance, gait, and functional performance of older adults categorized as having mild balance dysfunction and living in residential care facilities. Methods: Single blind randomized control design. One hundred-twenty residents fulfilled screening criteria for mild balance dysfunction based on the BioSwayTM balance and the Multi-Directional Reach Test (MDRT) primary outcome measures. Secondary assessment was completed using the Modified Physical Performance Test (PPT), hand-held dynamometer (lower-limb muscle strength testing), and gait speed analysis. Sixty subjects received individualized treatment from physical therapists (8 weeks). Another sixty subjects received generic group-based exercises (8 weeks). All outcome measures were collected at baseline and post-intervention (ninth week); and BioSwayTM and PPT measures at follow-up (thirteenth week) for the individualized group. Results: Individualized group (n=60) showed significant improvement compared to the group-based group (n=60) on the two BioSwayTM scores (limits of stability, p < .001; and postural stability, p = .016), the MDRT scores (forward reach, p < .001; backward reach, p = .007; right lateral reach, p < .001; and left lateral reach p < .001), the strength scores (hip flexors, p = .010; knee extensors, p = .002; hip abductors, p = .009; and ankle dorsiflexors, p = .025), the PPT outcomes (p < .001), and the gait scores (p = .012). Effect sizes ranged from small to large, with the largest sizes for limits of stability and MDRT. There were no significant differences between groups for the mCTSIB (p = .538). However, 96.7% of subjects in the individualized group scored within one SD of the reference mean, relative to 75% in the group-based group. At follow-up, the individualized group showed significant differences over time with medium to large effect sizes on the PPT (p < .001), limits of stability (p < .001), postural stability (p < .001), and mCTSIB (p = .005) measures. Post-hoc analysis revealed retention of gains for all measures at follow-up, except the mCTSIB. Conclusion: The individualized group showed significant improvements in the areas of balance, strength, mobility, and functional outcomes.

Hur medier omskriver ett fenomen påverkar allmänhetens inställning till det berörda fenomenet. I denna uppsats presenteras en studie av om, och i så fall hur, stigma framkommer då svenska medier skildrar äldre med depression. För att få en uppfattning om huruvida skildringarna är under förändring studeras och jämförs två tidsperioder, år 1995–1999 samt 2015–2019.  Studien som presenteras är en kvalitativ dokumentstudie av massmediedokument. Tidningsartiklar från de fyra största svenska dagstidningarna samlats in utifrån ett kriteriestyrt urval. Dessa tidningsartiklar har sedan analyserats och tematiserats genom en textanalys. Av resultatet framkommer att stigma är vanligt förekommande i skildringar av äldre med depression. Två teman framkommer; Missgynnande med anledning av ålder och Skildringar med negativa attribut. Mellan de båda tidsperioderna som studerats finns det såväl likheter som skillnader. Stigma framkommer under båda perioderna men skillnader har skett i hur skildringar görs i enskilda frågor utifrån rådande synsätt. Dessutom förekommer skillnader i hur olika negativa attribut präglar medias rapportering. Den skillnad som tydligast föreligger är att äldre med depression i hög grad skildras i relation till ensamhet under den senare tidsperioden. Det har också skett en utveckling i hur olika behandlingsmetoder skildras. Uppsatsen är relevant i förhållande till socialt arbete då äldre personer med depression i stor utsträckning är de kommunala socialtjänsternas ansvar. Det är därför viktigt att känna till hur äldre med depression skildras i medier då det har inverkan på allmänhetens kännedom om, och inställning till, åldrande och depression. / How the media writes about a phenomenon has an impact on the public´s perception of the concerned phenomenon. In this thesis a study is presented. The aim of the study is to investigate if, and in that case, how, stigma occurs when Swedish media portray elderly people with depression. To get an idea of whether the descriptions are changing, two time periods are studied and compared, 1995–1999 and 2015–2019. The study presented is a qualitative document study of mass media documents. Newspaper articles from the four largest Swedish daily newspapers were collected based on a criteria-driven selection. These newspaper articles have then been analyzed and thematized through a text analysis. The results show that stigma is common in descriptions of the elderly with depression. Two themes occur; Disadvantages due to age and Descriptions with negative attributes. Between the two time periods studied, there are both similarities and differences. Stigma occurs during both periods, but differences have occurred in how descriptions are made in specific issues based on prevailing views. In addition, there are differences in how different negative attributes characterize media reporting. The most obvious difference is that older people with depression are largely portrayed in relation to loneliness during the later time period. Also, there has been an evolvement in how different kinds of treatment are portrayed. The thesis is relevant in relation to social work as older people with depression are to a large extent the responsibility of the municipal social services. It is therefore important to know how the elderly with depression are portrayed in the media as it has an impact on the public's awareness of, and attitude towards, aging and depression.

El documento abarca la investigación y desarrollo de un centro intergeneracional, espacio que promueve la revaloración del adulto mayor y su reintegración en la sociedad, a través de la conexión con otras generaciones, en este caso con niños, para crear lazos amicales y una familia extendida. El proyecto se planea desarrollar en el distrito de Cajamarca, lugar que junto a otras provincias del departamento del mismo nombre, atraviesan momentos críticos en el aspecto económico-social, dejando en estado de vulnerabilidad tanto a adultos mayores como niños. Asimismo, cabe añadir que la situación de las personas de la tercera edad es cada vez más preocupante por su acelerado crecimiento poblacional y por el déficit de espacios propicios para su resguardo. Ante estas circunstancias, surge la idea de que el centro intergeneracional comprenda de una residencia y centro de día para el adulto mayor, además de un espacio para el cuidado infantil, de manera que se puedan brindar ambientes de protección, recreación y aprendizaje de las personas más vulnerables. Conviene señalar que, para lograr el objetivo de un centro intergeneracional, la propuesta será de una arquitectura con énfasis en espacios intermedios, la cual otorgará ambientes idóneos que ayudarán a conectar a personas de distintos grupos etarios, logrando la noción principal del proyecto. / The document covers the research and development of an intergenerational center, a space that promotes the revaluation of the elderly and their reintegration in society, through the connection with other generations, in this case with children, to create friendships and an extended family. The project is planned to be developed in Cajamarca district, a place that, together with other provinces of the department of the same name, are going through critical moments in the economic-social aspect, leaving both the elderly and children in a state of vulnerability. Likewise, it should be added that the situation of the elderly is increasingly worrying due to their accelerated population growth and the lack of suitable spaces for their protection. Given these circumstances, the idea arises that the intergenerational center comprises a residence and day center for the elderly, as well as a space for child care, so that protection, recreation and learning environments can be provided for people most vulnerable. El presente estudio es de carácter cualitativo y de diseño fenomenológico, tuvo como objetivo describir las representaciones sociales de la vejez en un grupo de mujeres adulto mayores en Lima metropolitana. Se utilizó como instrumento de recolección de datos la entrevista semiestructurada a profundidad. El escenario de la investigación se ubicó en una asociación de hipertensos, con seis mujeres adultas mayores. Como resultado de la investigación se obtuvo que las adultas mayores adoptan diferentes creencias de su entorno cercano como familiares e hijos. Asimismo, la vejez es vista como una etapa de nuevas oportunidades, tranquilidad, abandono y cambios fisiológicos. / This is a qualitative study with a phenomenological design, the objective of which was to describe the social representations of old age in a group of elderly women in metropolitan Lima. A semi-structured in-depth interview was used as a data collection instrument. The research scenario was located in an association of hypertensive patients, with six elderly women. As a result of the research, it was found that older women adopt different beliefs from their close environment such as family members and children.
