Perfil funcional do auto-enxerto de tecido paratireóideo em pacientes submetidos à paratireoidectomia total por hiperparatireoidismo secundário à doença renal crônica / Function of the autotransplanted parathyroid tissue in renal hyperparathyroidism after total parathyroidectomy

INTRODUÇÃO: A paratireoidectomia total com auto-enxerto imediato heterotópico (PTH+AE) é uma das técnicas cirúrgicas hoje usadas no tratamento do hiperparatireoidismo secundário à doença renal crônica e do hiperparatireoidismo persistente após o transplante renal. Os níveis adequados de paratormônio sistêmico (PTHs) no pós-operatório ainda são controversos e o perfil funcional do auto-enxerto de tecido paratireóideo, pouco esclarecido. No presente estudo, nós analisamos a função do tecido paratireóideo implantado pacientes com hiperparatireoidismo secundário e terciário. MÉTODO E CASUÍSTICA: Em um estudo prospectivo observacional, 19 pacientes portadores de doença renal crônica (PDRC) e quatro pacientes transplantados renais (PTR) foram submetidos à PTX+AE com e seguidos por um ano. Todos os pacientes apresentaram PTHs indetectável no pós-operatório imediato (POi). Os níveis séricos de PTH em ambos os membros superiores, cálcio, fósforo, fosfatase alcalina e reposição de cálcio elementar e calcitriol foram verificados com um mês, dois, três, quatro, seis, nove e 12 meses após a cirurgia. A 25-Hidroxivitamina D (25OHD) foi medida no POi, seis e 12 meses após a cirurgia. A função do auto-enxerto foi classificada em estados funcionais (EF) de acordo com os níveis de PTHs. RESULTADOS: A maioria dos PDRC e PTR mostraram níveis detectáveis de PTHs já no primeiro mês. No segundo mês pósoperatório, todos os pacientes apresentaram níveis detectáveis de PTHs quando também houve estabilização dos níveis séricos em ambos os grupos. Os gradientes de concentração de PTH não mostraram correlação com o PTHs. No pós-operatório, a hipercalcemia foi observada em 73,7% dos PDRC em pelo menos um episódio, reduzindo ou inibindo a secreção de paratormônio em oito pacientes. Não houve melhora dos níveis de 25OHD em ambos os grupos, permanecendo com níveis insuficientes. Oito PDRC regrediram de EF ao final do estudo. CONCLUSÃO: O perfil funcional do AE é semelhante entre os PDRC e PTR. A função do AE, inicia-se no primeiro mês pós-operatório, atingindo um maior EF até o quarto mês pós-operatório na maioria dos pacientes, porém pode haver declínio da função ao longo do tempo / BACKGROUND: Total parathyroidectomy with immediate forearm transplantation (PTX+AT) is employed in renal hyperparathyroidism. Appropriate postoperative systemic parathyroid hormone (sPTH) levels are still controversial and autograft functional behavior is unclear. In the present study, we analyzed the function of autotransplanted parathyroid tissue in secondary and tertiary hyperparathyroidism. METHODS: In a prospective observational study, 19 dialysis patients (DPs) and 4 kidney transplant patients (KTPs) who underwent PTX+AT were followed. All patients had undetectable PTH in the early postoperative period (ePO). Autograft function and the following biochemical variables were assessed at one, two, three, four, six, nine and 12 months following the operation: serum calcium, phosphorus, alkaline phosphatase, and intact PTH before and after the operation. 25-Hidroxyvitamin D levels (25OHD) were measured in the ePO, six and twelve months. Oral doses of calcium and calcitriol were recorded. Autograft function was stratified into four functional status (FS) according to sPTH. RESULTS: All functioning grafts presented sPTH until the second month. On the second postoperative month, all patients had sPTH detectable levels also when serum levels were steady in both groups. PTH gradients showed no significant correlation with sPTH levels. In at least one occasion in the postoperative period, hypercalcemia was observed in 73.7% of DPs, and it reduced or inhibited PTH secretion in 8 patients. There was no improvement in levels of 25OHD in both groups, resulting in insufficient levels. Eight DPs changed to a lower FS at the end of the study. CONCLUSION: The function of the autotransplanted parathyroid tissue usually initiates during the first month following operation and is similar in patients. The most of the patients reachs FS2 during the fourth month but autograft function can decrease over time

Validação cultural e confiabilidade das versões em português das escalas de mobilidade na UTI: Perme Intensive Care Unit Mobility Score e Intensive Care Unit Mobility Scale (IMS) / Perme Intensive Care Unit Mobility Score and Intensive Care Unit Mobility Scale (IMS): Cross-Cultural validation and Reliability of the Brazilian Portuguese version

Objetivo: Realizar a tradução, validação cultural para a língua portuguesa do Brasil e análise de concordância e confiabilidade entre avaliadores das escalas de mobilidade em unidade de terapia intensiva (UTI) Perme Intensive Care Unit Mobility Score - Perme Score e a Intensive Care Unit Mobility Scale - IMS. Métodos: O processo de tradução e adaptação seguiu as seguintes etapas: Preparação, tradução, reconciliação e síntese, tradução reversa, revisão, aprovação e pré-teste. Após estes processos, a versão em português das duas escalas foi utilizada por dois pesquisadores na avaliação de 103 pacientes críticos internados em UTI. O índice de Kappa e a análise de Bland-Altman foram utilizados para verificar a concordância entre as escalas. O coeficiente ? de Cronbach foi utilizado para verificar a confiabilidade entre os avaliadores no uso das escalas. A correlação entre as escalas foi verificada pelo teste de Spearman. Resultados: Ambas as escalas, Perme Escore e Escala de Mobilidade na UTI - EMU, foram devidamente traduzidas para o Português falado no Brasil. As características dos pacientes críticos avaliados neste estudo demonstrou uma predominância masculina 56 (54%) com idade média de 52±18 anos, apresentando SAPS 3 = 66 (24%). O principal motivo de internação nas UTIS foi descompensação clínica de origem respiratória (44%). Ambas as escalas apresentaram excelente concordância (k > 0,90) e confiabilidade (alfa > 0,90) para todos os domínios. Na análise de Bland-Altman, constatou-se um baixo viés entre os avaliadores tanto para o EMU (-0,048 ± 0,35) quanto para o Escore de Perme (-0,06 ± 0,73). Os limites superiores e inferiores de 95% de concordância foram de 0,64 a -0,73 para o EMU e de 1,36 a -1,5 para o Perme Escore. Além disso, verificou-se forte correlação positiva entre as duas escalas utilizadas para avaliar os pacientes ( =0,941). Conclusão: A versão em português do Escore Perme de mobilidade em UTI (Perme Escore) e da escala de mobilidade em unidade de terapia intensiva (EMU) apresentaram alta concordância e confiabilidade entre os avaliadores / Objectives: Translate, cross cultural validate to Brazilian Portuguese language and analyze the inter-rater reliability with both instruments Perme Score and IMS. Methods: The translation process and the cross cultural validation followed the following steps: preparation, translation, synthesis, back translation, review, approval and pre-test. After this process the Brazilian Portuguese version of the both scales were used by two researchers to evaluated 103 critical care patients. The weighted kappa and Bland Altman analysis were used to verify inter rater agreement. Cronbach-? test was used to evaluate inter rate reliability. The correlation between the scales was verified by the Spearman correlation test. Results: Both scales, Perme escore e EMU, were translated to the Brazilian Portuguese. Most of the patients were male 56 (54%), mean age 52±18, SAPS 3 = 66 (24%). Respiratory failure was the most prevalent reason for admission (44%). Both scales showed an excellent inter rater agreement (k > 0,90) and reliability (alpha > 0,90) for all domains. Bland-Altman analysis showed a low bias between raters either for EMU (-0,048 ± 0,35) and Perme Escore (-0,06 ± 0,73). Upper and lower 95% limits of agreement were 0.64 to -0.73 for the EMU and 1.36 - 1.5 for the Perme Escore. Moreover, it also presented a strong positive correlation between the two instruments ( = 0,941). Transcranial direct current stimulation (tDCS) for improving capacity in activities and arm function after stroke: a network meta-analysis of randomised controlled trials

Background: Transcranial Direct Current Stimulation (tDCS) is an emerging approach for improving capacity in activities of daily living (ADL) and upper limb function after stroke. However, it remains unclear what type of tDCS stimulation is most effective. Our aim was to give an overview of the evidence network regarding the efficacy and safety of tDCS and to estimate the effectiveness of the different stimulation types. Methods: We performed a systematic review of randomised trials using network meta-analysis (NMA), searching the following databases until 5 July 2016: Cochrane Central Register of Controlled Trials (CENTRAL), MEDLINE, EMBASE, CINAHL, AMED, Web of Science, and four other databases. We included studies with adult people with stroke. We compared any kind of active tDCS (anodal, cathodal, or dual, that is applying anodal and cathodal tDCS concurrently) regarding improvement of our primary outcome of ADL capacity, versus control, after stroke. PROSPERO ID: CRD42016042055. Results: We included 26 studies with 754 participants. Our NMA showed evidence of an effect of cathodal tDCS in improving our primary outcome, that of ADL capacity (standardized mean difference, SMD = 0.42; 95% CI 0.14 to 0.70). tDCS did not improve our secondary outcome, that of arm function, measured by the Fugl-Meyer upperextremity assessment (FM-UE). There was no difference in safety between tDCS and its control interventions, measured by the number of dropouts and adverse events. Conclusion: Comparing different forms of tDCS shows that cathodal tDCS is the most promising treatment option to improve ADL capacity in people with stroke.

É aplicada através de um tratamento intensivo durante duas semanas consecutivas, com seis horas diárias de exercícios e uso de uma tipóia, durante 90% do dia, no membro superior não-afetado. O ganho motor da TCI é validado através das escalas funcionais de avaliação do membro superior (EFAMS) Wolf Motor Function Test (WMFT) e Action Research Arm Test (ARAT). Objetivo: Realizar uma análise comparativa entre ambas as EFAMS. Métodos: As EFAMS foram aplicadas em quatro períodos distintos em 17 pacientes com o diagnóstico de acidente vascular cerebral isquêmico, que realizaram individualmente a TCI durante duas semanas com seis horas diárias de exercícios e após foi feito uma análise comparativa entre as EFAMS em relação ao tempo de aplicação, tempo de preparação da mesa, reprodutibilidade e análise fatorial. Resultados: Diminuição no escore da EFAMS WMFT e aumento no escore da EFAMS ARAT pós TCI, não houve variação significativa dos escores entre os períodos pré e pós TCI das EFAMS, diminuição do tempo de preparação da mesa e aplicação de ambas as EFAMS pós TCI e a análise fatorial detectou dois e três componentes qualitativos na EFAMS ARAT e WMFT, respectivamente. Conclusões: Ambas as EFAMS conseguem mensurar o ganho motor da TCI, com alta reprodutibilidade, sendo que o tempo de aplicação e preparação da mesa é menor na EFAMS ARAT e somente a EFAMS WMFT apresenta o componente qualitativo “lateralidade”. / The Constraint-induced Movement therapy (CIMT) is an intervention which main goal is the recuperation of affected upper extremity in hemiparetic patients with acquired encephalic lesion. Its protocol consists by an intensive treatment of two consecutive weeks with six hours of exercises and the wear of arm sling in non affected upper arm during 90% of daily activities. The motor improvement of CIMT is validity by the functional evaluation scales of upper extremity (FESUE) Wolf Motor Function Test (WMFT) and Action Research Arm Test (ARAT). Objective: Realize a comparative analysis between both scales. Methodology: The FESUE had been applied during four different times in 17 patients with diagnosis of ischemic stroke, who made individually the CIMT during two weeks and six hours of exercises and after made a comparative analysis between the FESUE by time for application, time for preparation of the table, reproducibility and factorial analysis. Results: Decreased in score of FESUE WMFT and increased in score of FESUE ARAT after CIMT, no significant variation in the score between before and after CIMT of the FESUE, decreased in time to preparation the table and application of FESUE in both scales and the factorial analysis showed two and three qualitative components in FESUE ARAT and WMFT, respectively. Conclusions: Both FESUE can measure the therapeutics gains of CIMT, with high reproducibility, but the time for application and preparation of the table are minor in FESUE ARAT and only the FESUE WMFT shows the qualitative component “laterality”. / TEDE / BV UNIFESP: Teses e dissertações

Multidisciplinary rehabilitation after primary total knee arthroplasty:a study of its effects on health- related quality of life, functional capacity and cost-effectiveness

Abstract Knee osteoarthritis (OA) is one of the main causes of chronic disability in elderly people. In end-stage knee OA, patients experience both short- and long-term benefits from total knee arthroplasty (TKA). All the significant deficiencies in functional capacity and health-related quality of life (HRQOL) are not spontaneously resolved after surgery. Therefore, it is essential that patients receive effective rehabilitation. So far, there are no gold standards in terms of outcome measurements of TKA and rehabilitation in connection with surgery. The present series of studies was designed for the purpose of examining the effectiveness and cost-effectiveness of a multidisciplinary rehabilitation program compared with conventional orthopedic care, the attributes of disability in elderly knee OA patients waiting for primary TKA, and the impact of patient-relevant factors on the outcome of TKA one year after surgery. Special emphasis was placed on self-reported functional capacity, HRQOL, and objectively measured functional capacity. Objective and subjective physical function and HRQOL were investigated with a battery of physical function tests and questionnaires (WOMAC, 15D, and RAND-36) during a one-year follow-up. Pain, body mass index (BMI), and antero-posterior laxity of the affected knee were the main attributes of self-reported disability in the patients waiting for primary TKA. The multidisciplinary rehabilitation program did not yield faster attainment of recovery than did conventional orthopedic care. Equal, significant improvement in HRQOL and functional capacity was found in both groups, but conventional care was clearly cost-saving. A preoperative score below the general population level on the 15D, the presence of pain, higher age, and the presence of pulmonary disease preoperatively decreased the possibility of reaching the HRQOL level of the general population. More severe pain preoperatively was associated with a higher degree of functional recovery. The presence of osteoporosis, male gender, impaired function of the opposite knee, and higher age were associated with a lower degree of improvement in self-reported function. The results highlight the multifactorial nature of health status in TKA. Further intervention studies are needed to identify patients who would benefit most from intensive rehabilitation interventions after TKA and to create standards for outcome tools after joint replacement surgery and rehabilitation interventions. / Tiivistelmä Polvinivelrikko on merkittävimpiä niistä sairauksista, jotka rajoittavat ikääntyneiden toimintakykyä. Vaikka tekonivelleikkauksella voidaan sekä välittömästi että pidemmän ajan kuluessa kohentaa merkittävästi polvinivelrikosta kärsivien elämänlaatua ja toimintakykyä, osa potilaista ei leikkauksen jälkeenkään selviydy hyvin arjestaan. Siksi on tärkeää, että potilasta kuntoutetaan tekonivelleikkauksen jälkeen asianmukaisesti ja tehokkaasti. Leikkauksen ja kuntoutuksen tuloksellisuutta arvioivien mittareiden käytöstä ei ole olemassa standardoituja suosituksia. Tämän väitöskirjan päätavoitteena oli selvittää 2–4 kuukautta leikkauksen jälkeen toteutetun, moniammatillisen, polikliinisen kuntoutuksen kustannustehokkuutta sekä sen vaikuttavuutta elämänlaatuun ja toimintakykyyn verrattuna perinteiseen ortopediseen hoitokäytäntöön. Lisäksi selvitettiin tekijöitä, jotka heikentävät potilaan toimintakykyä pitkälle edenneessä polvinivelrikossa, sekä potilaslähtöisiä tekijöitä, jotka vaikuttavat leikkaustulokseen vuosi leikkauksen jälkeen. Itsearvioitua toimintakykyä sekä elämänlaatua arvioitiin kyselykaavakkeiden (WOMAC, 15D ja RAND-36) avulla ja fyysistä toimintakykyä mitattiin testipatteristolla. Tutkimuksen seuranta-aika oli yksi vuosi. Painoindeksi, kipu ja leikattavan polven etu-takasuuntainen väljyys selittivät eniten tekonivelleikkausta odottavien potilaiden toimintakyvyn rajoitteita. Potilaiden elämänlaatu ja toimintakyky kohenivat merkittävästi tekonivelleikkauksen jälkeen. Moniammatillisesti toteutettu kuntoutus ei tehostanut leikkauksesta toipumista verrattaessa sitä perinteiseen ortopediseen hoitokäytäntöön, joka vertailussa myös säästi selkeästi kustannuksia. Väestökeskiarvoa huonommaksi koettu elämänlaatu jo ennen leikkausta, kivuliaisuus, korkea ikä ja keuhkosairaudet heikensivät mahdollisuutta saavuttaa väestökeskiarvon tasoinen koettu elämänlaatu vuosi leikkauksen jälkeen. Leikkausta edeltävä vaikea-asteinen kivuliaisuus assosioitui merkittävään toimintakyvyn parantumiseen. Osteoporoosi, miessukupuoli, vastakkaisen polvinivelen epänormaali toiminta ja korkea ikä taas liittyivät vaatimattomampana koettuun toimintakyvyn parantumiseen. Tekonivelleikattujen potilaiden terveydentilan monitekijäisyys heijastuu tutkimustuloksiin. Jatkossa tarvitaan interventiotutkimuksia, jotta voitaisiin paremmin tunnistaa ne potilaat, jotka hyötyisivät eniten tehostetusta leikkauksen jälkeisestä kuntoutuksesta. Lisäksi olisi tärkeää tutkia, miten kuntoutuksen tuloksellisuutta arvioivia mittareita voitaisiin yhtenäistää.

Our NMA showed evidence of an effect of cathodal tDCS in improving our primary outcome, that of ADL capacity (standardized mean difference, SMD = 0.42; 95% CI 0.14 to 0.70). tDCS did not improve our secondary outcome, that of arm function, measured by the Fugl-Meyer upperextremity assessment (FM-UE). There was no difference in safety between tDCS and its control interventions, measured by the number of dropouts and adverse events. Conclusion: Comparing different forms of tDCS shows that cathodal tDCS is the most promising treatment option to improve ADL capacity in people with stroke.

Prognostički faktori za povratak na posao kod bolesnika operisanih zbog lumbalne diskus hernije / Prognostic factors for return to work after lumbar discectomy

<p>Povratak na posao nakon operacije lumbalne diskus hernije determinisan je funkcionalnim stanjem, prisustvom i stepenom tegoba od strane lumbosakralne kičme, zahtevima na radnom mestu bolesnika ali i psihosocijalnim faktorima, koji pri oceni radne sposobnosti zaposlnih zahtevaju individualni pristup. Grupa pacijenata koja se neće vratiti na posao može biti identifikovana putem prognostičkog modela. Cilj ove studije je definisanje prognostičkog modela za povratak na posao bolesnika operisanih zbog lumbalne diskus hernije kao i identifikacija najznačajnijih faktora rizika odgovornih za lo&scaron; ishod operativnog lečenja, posmatrano kroz prizmu povratka na posao. Istraživanje je prospektivna studija koja je obuhvatila ukupno 200 ispitanika, koji su operisani zbog lumbalne diskus hernije na jednom nivou i praćeni su u vremenskom period do 12 meseci nakon operativnog lečenja. U statističku ananlizu je u&scaron;lo 153 bolesnika, koji su ispunili kriterijume selekcije ispitanika studije. Nakon određivanja značaja posmatranih biolo&scaron;kih, profesionalnih i psihosocijalnih faktora rizika za povratak na posao, kreirani su i evaluirani prognostički modeli bazirani na svim i na odabranim atributima desetostrukom kros-validacijom: stablo odlučivanja (DT), model vi&scaron;eslojnih perceptrona (MLP) i model potpornih vektora (SVM). Za predviđanje povratka na posao najveću tačnost, specifičnost i senzitivnost za odabrane atribute postiže model potpornih - podržavajućih vektora (SVM). Najbolju intuitivnu i praktičnu vrednost za predviđanje povratka na posao pruža model stabla odluka (DT). Identifikacijom najznačajnijih faktora rizika za nepovoljan ishod povratka na posao omogućeno je preventivno delovanje na iste, u cilju smanjenja broja pacijenata sa umanjenjem radne sposobnosti i invaliditeta.</p> / <p>Return to work after lumbar discectomy is determinated by functional status, presence and degree of discomfort in the lumbosacral spine, the requirements in the workplace of patients and psychosocial factors that in the assessment of working capabilities require an individual approach. Groups of patients which don&rsquo;t return to work after surgery could be identified by predictive model. The aim of this study is to define prognostic model to return to work patients after lumbar discectomy, as well as the identification major risk factors responsible for the poor outcome of operative treatment viewed through the prism of returning to work. This prospective study included a total of 200 patients, who underwent surgery for lumbar disc herniation on one level and were followed up in period of 12 months following surgery. The statistical analysis included 153 patients who fulfilled all selection criteria of the study subjects. After determining significance of the observed biological, professional and psychosocial risk factors for return to work, prognostic models were designed and evaluated based on all and selected attributes by tenfold cross-validation: decision tree (DT) model of multilayer perception (MLP) model and support vector (SVM). For the prediction of return to work best accuracy, specificity and sensitivity for selected attributes, is achieved by supporting vector model (SVM). The decision tree model (DT) provides the best intuitive and practical value for predicting return to work. By identifying the most important risk factors for adverse outcome for return to work it is made possible for preventive actions, to reduce the number of patients with reduced work ability and disability.</p>
