Resultados do tratamento cirúrgico do adenocarcinoma de reto médio: estudo comparativo entre pacientes submetidos à quimioterapia adjuvante, com e sem quimio e radioterapia neo-adjuvantes / Results of surgical treatment of adenocarcinoma of the middle rectum: a comparative study between patients submitted to adjuvant chemotherapy, with and without neoadjuvant chemo and radiotherapy

A cirurgia ainda é o principal método de tratamento do câncer do reto. Recentemente a quimio e radioterapia neo-adjuvantes têm sido preconizadas, com freqüência cada vez maior, com o intuito de reduzir os índices de recidiva e mortalidade. O objetivo desse estudo foi avaliar, retrospectivamente a sobrevida e a recidiva tumoral de pacientes submetidos a quimioterapia adjuvante, com e sem quimio e radioterapia neoadjuvantes. Foram avaliados retrospectivamente 36 pacientes submetidos a ressecção anterior baixa por adenocarcinoma do reto. Subdivididos em três grupos: grupo I (N=11), submetidos exclusivamente a tratamento cirúrgico; grupo II (N=8), submetidos a tratamento cirúrgico, seguido de quimioterapia adjuvante; grupo III (N=17), submetidos a tratamento cirúrgico com quimio e radioterapia neo-adjuvantes. O período de seguimento foi de 36 meses. Seis pacientes (16,6%) apresentaram recidiva, sendo 1 paciente do grupo I, 3 pacientes do grupo II e 2 pacientes do grupo III. A sobrevida global foi de 88,9%, assim distribuída: grupo I, 80,0%; grupo II, 100,0% e grupo III, 87,5%. Não houve diferença significante nos índices de recidiva nem na sobrevida entre os grupos. Concluiu-se que na amostra estudada os métodos terapêuticos tiveram resposta equivalente, não tendo sido possível demonstrar a interferência da quimio e ou radioterapia nos índices de sobrevida ou recidiva / Surgery continues to be the principal method for treating cancer of the rectum. Recently, chemo and neo-adjuvant radiotherapy have been considered, with increasing frequency, with the intention of reducing the rates of recurrence and mortality. The objective of this study was to evaluate, retrospectively, survival and tumor recurrence in patients submitted to adjuvant chemotherapy, with and without neo-adjuvant chemo and radiotherapy. A retrospective evaluation of 36 patients submitted to lower anterior resection for adenocarcinoma of the rectum was conducted, subdivided into three groups: Group I (n=11), submitted exclusively to surgical treatment; Group II (n=8), submitted to surgical treatment followed by adjuvant chemotherapy; Group III (n=17), submitted to surgical treatment with chemo and neo-adjuvant radiotherapy. The time period was 36 months. Six patients (16.6%) presented recurrence: 1 patient from Group I, 3 patients from Group II and 2 patients from Group III. Overall survival was 88.9%, distributed in the following way: Group I, 80.0%; Group II, 100.0% and Group III, 87.5%. No significant differences in the rates of recurrence and survival were observed between the groups. It is therefore concluded that within the sample the therapeutic methods had similar response, not having been possible to demonstrate the interference of chemo and radiotherapy in the rates of survival or recurrence

Dans cette thèse, nous étudions les propriétés quantitatives de récurrence de certains systèmes dynamiques préservant une mesure infinie. Nous nous intéressons au premier temps de retour des orbites d'un système dynamique dans un petit voisinage de leurs points de départ. Tout d'abord, nous commençons par considérer un modèle jouet probabilistique pour éclairer la stratégie de nos preuves. On s'intéresse particulièrement au cas où la mesure est infinie, plus précisément, nous considérons les Z -extensions des sous-shift de type fini. Nous étudions le comportement asymptotique du premier temps de retour au voisinage de l'origine, et nous établissons des résultats de type de convergence presque partout, et aussi de convergence en loi par rapport à toute mesure de probabilité absolument continue par rapport à la mesure infinie. Dans ce travail, nous nous également intéressons à d'autres systèmes dynamiques. Nous considérons un flot Axiome A(gt)t sur une variété riemannienne M munie d'une mesure σ -finie μ. Nous supposerons que la mesure μ est une mesure d'équilibre pour (gt)t. Afin d'établir nos résultats, nous introduisons des notions de dynamique hyperbolique. En particulier, nous considérons la section de Markov qui a été introduite par Bowen et Ratner. / In this thesis, we study the quantitative recurrence properties of some dynamical systems preserving an infinite measure. We are interested in the first return time of the orbits of a dynamical system into a small neighborhood of their starting points. First, we start by considering a toy probabilistic model to clarify the strategy of our proofs. Our interest is when the measure is indeed infinite, more precisely we consider the Z-extensions of subshifts of finite type. We study the asymptotic behavior of the first return time near the origin, and we establish results of an almost everywhere convergence kind, and a convergence in distribution with respect to any probability measure absolutely continuous with respect to the infinite measure. In this work, we are also interested in another dynamicals systems. We consider an Axiom A flow (gt)t on a Riemannian manifold M endowed with a σ-finite measure μ. We will assume that the measure μ is an equilibrium measure for (gt)t. In order to establish our results, we introduce notions from hyperbolic dynamics. In particular, we consider the Markov section which was constructed by Bowen and Ratner.

"Análise dos fatores associados à recorrência de gravidez na adolescência" / Analysis of factors associated to recurrence in adolescent pregnancy

O objetivo desta pesquisa foi estudar um grupo de 106 gestantes adolescentes multigestas, comparando-as com 510 adolescentes primigestas quanto a variáveis demográficas, obstétricas e psicossociais. As pacientes foram recrutadas no ambulatório de obstetrícia no setor de gravidez na adolescência da clínica obstétrica do HC-FMUSP entre janeiro de 2000 a janeiro de 2006. As pacientes tinham até 18 anos e tiveram acompanhamento multiprofissinal. Os dados foram coletados de prontuário eletrônico (Acess-microsoft 98), apresentados de forma descritiva e analisados por teste do χ2, exato de Fisher, t de Student, Mann-Whytney e índice de Kappa, de acordo com a indicação do uso de cada um. Concluiu-se que não ocorrreram diferenças estatisticamente significativas entre multigestas e primigestas quanto a cor, naturalidade, renda familiar, exercício de alguma atividade remunerada, a idade das mães das adolescentes grávidas, a idade gestacional ao iniciar as consultas de Pré-Natal, número de consultas, estatura, peso no início e término do Pré-Natal, índice de massa corpórea, ganho ponderal durante a gravidez, classificação do ganho de peso pelo gráfico de Rosso, presença de doença hipertensiva específica da gravidez, presença de oligoâmnio, complicação por infecção urinária e amniorrexe prematura. O mesmo foi observado em relação à instituição onde o parto ocorreu, condição dos recém-nascidos, peso dos recémnascidos, adequação peso para idade gestacional dos recém-nascidos, índice de Apgar de primeiro e de quinto minutos não foram diferentes. Também não houve diferença significativa quanto ao desejo e aceitação da atual gravidez, intenção e tentativas de interrompê-la, tempo de uso de métodos anticoncepcionais, serem filhas de mulheres que foram mães adolescentes, tipo de relacionamento com as mães, com os pais e com os companheiros, reação do companheiro à notícia da gravidez, manutenção de grupo de amigos e de atividade esportiva. Diferenças com significância estatística foram observadas entre as multigestas e primigestas no que se refere à idade que foi maior no primeiro grupo. Em relação à escolaridade algumas multigestas atingem níveis mais altos, porém com menos probabilidade de continuarem os estudos durante a gestação sendo que mais da metade não concluí o ensino fundamental. As multigestas vivem em união estável mais freqüentemente e dependem financeiramente mais de seus companheiros e formam núcleo familiar independente da família de origem. Também seus companheiros são mais velhos que os das primigestas. Em relação às variáveis obstétricas a média da idade gestacional na última consulta foi menor e o trabalho de parto prematuro mais freqüente. Também a idade gestacional ao parto foi menor assim como a freqüência de recémnascidos de pré-termo. No parto a aplicação de fórcipes foi menos freqüente nas multigestas. Das variáveis psicossociais detectou-se que as multigestas planejaram mais suas gestações, iniciaram mais precocemente suas atividades sexuais, no entanto conheciam e se utilizavam com maior freqüência de métodos contraceptivos. Os pais e mães das adolescentes multigestas apresentaram melhor reação frente a noticia da gestação que os das primigestas. / This research studied a group of 106 adolescents patients with more than pregnancies and compared them to 510 with first pregnancy considering the demographics, obstetrics and psychosocial variables. The patients were recruited in the ambulatory service of obstetrics in the clinic of adolescent pregnancy of the HCFMUSP from January 2000 to January 2006. The oldest patients were 18 and assisted in comprehensive prenatal care. The data were collected from their electronic records (Access-Microsoft 98) presented in a descriptive form and analyzed through the χ2 test, exact of Fisher, t of Student, Mann-Whytney and Kappa index, according to the indication of each one. It was concluded that no statistical differences occurred between both groups regarding color, place of birth, family income, practice of any paid activity, maternal age of the teenagers, gestational age at the beginning of the pre-natal care, number of consultations, stature, initial and final weight in the pre-natal care, index of corporal mass, weight gain during pregnancy, classification of weight gain through the Rosso graphic, presence of pregnancy specific hypertension, presence of oligohydramnios, urinary infection complication, and preterm ruptured membranes. It was also the same in relation to the institution, condition and weight of the babies, balance of weight and gestational age, Apgar index at the first and fifth minutes were not different. Also, it was included the wish and acceptance of the present pregnancy, intention and tentative of abortion, length of use of contraceptive methods, daughter of adolescent mothers, type of relationship with mothers, with parents and with partner, reaction of the partner when the pregnancy was notified, maintenance of friends and athletic activities were not different. Significant statistical differences were observed between the patients with more pregnancies that are older than the primigravidas. Yet, considering their education, the patients with more pregnancies presented lower possibilities of continuing their studies during pregnancy. These patients presented a more stable union and financial dependence on their partners and formed a familiar nucleus independent of the original family. Their partners were also older than the ones of the patients with first pregnancy. The obstetric variables showed a lower gestational visit age in the last doctor visit with a more frequent premature labor. It was noticed that the smaller gestational age in labor had a higher frequency of preterm births. During labor the use of forceps was lesser in adolescents patients with more pregnancies. The psychosocial variables stated that this patients planned better their gestations, began their sexual activities earlier, knew and used contraceptive methods. The parents of the adolescents with more pregnancies had better reaction when compared with the parents of the adolescents in their first pregnancy.

Recidiva local de carcinomas epidermóides da boca e orofaringe: estudo de variáveis anatomopatológicas e de marcadores biológicos associados ao prognóstico em pacientes submetidos à cirurgia de resgate / Local recurrence of squamous cell carcinomas of the mouth and oropharynx: a study of anatomic pathology variables and biological markers associated with prognosis in patients submitted to salvage surgery

INTRODUÇÃO: Recidivas locais e loco-regionais são as principais causas de falha do tratamento em pacientes portadores de carcinomas epidermóides de boca e orofaringe. A cirurgia de resgate é geralmente a melhor opção terapêutica para esses pacientes. Esse estudo tem por objetivo avaliar a importância prognóstica da expressão das proteínas EGFR, MMP-2, MMP-9 e VEGF em pacientes com recidiva local submetidos à cirurgia de resgate. CASUÍSTICA E MÉTODOS: Os prontuários de 111 pacientes portadores de recorrência local de carcinomas epidermóides de boca e orofaringe foram analisados de forma retrospectiva. A localização do tumor primário foi o lábio em 10 casos (9%), a cavidade oral em 68 (61%) e a orofaringe em 33 (30%). O tratamento prévio foi cirurgia em 33 casos (30%), radioterapia associada ou não à quimioterapia baseada em cisplatina em 46 (41%) e cirurgia com radioterapia adjuvante em 32 (29%). A expressão das proteínas EGFR, MMP-2, MMP-9 and VEGF foi avaliada com a técnica do Tissue Microarray. RESULTADOS: O intervalo livre de doença variou de 0,89 a 140,9 meses, com uma mediana de 6,87 meses. As recidivas foram diagnosticadas em intervalo de tempo inferior a 1 ano em 69 pacientes (62,2%) e após 1 ano em 42 (37,8%). Os pacientes com intervalo livre de doença inferior a 1 ano apresentaram pior resultado de sobrevida (p=0,01). O estádio clínico da recidiva (rEC) foi I ou II em 31 casos (27,9%) e III ou IV em 80 (72,1%). Pacientes com doença em estádio clínico mais avançado (rEC III ou IV) apresentaram piores taxas de sobrevida específica por câncer (p=0,04). Hiper-expressão do EGFR foi associada a pior resultado do tratamento. Os casos com EGFR positivo obtiveram sobrevida específica por câncer em 3 anos de 27,2%, enquanto pacientes com EGFR negativo alcançaram 64,3% de sobrevida em 3 anos (p=0,001). A expressão das proteínas MMP-2, MMP-9 e VEGF não se mostrou significativa para o prognóstico (p=0,83, p=0,15 e p=0,86, respectivamente). Na análise multivariada, apenas o intervalo livre de doença e a expressão do EGFR foram associadas à maior risco de morte. CONCLUSÕES: Recidivas locais de carcinomas epidermóides de boca e orofaringe são associadas a mau prognóstico. Intervalo livre de doença superior a 1 ano e ausência de expressão do EGFR foram os principais fatores associados a melhores resultados de sobrevida específica por câncer em pacientes submetidos à cirurgia de resgate. / INTRODUCTION: Local and regional relapses are the main sites of treatment failure in patients with oral and oropharyngeal squamous cell carcinoma. In these instances, salvage surgery is the most widely used treatment approach. The aim of this study is to analyze the prognostic effect of the expression of EGFR, MMP-2, MMP-9 and VEGF in patients with recurrent cancer sumitted to salvage surgery. METHODS: The charts of 111 patients with local recurrence of oral or oropharyngeal squamous cell carcinomas were retrospectively analyzed. The tumor sites were: the lip in 11 cases (9%), the oral cavity in 68 (61%) and the oropharynx in 33 (30%). The previous treatment was: Surgery in 33 patients (30%), radiotherapy with or without cisplatin based chemotherapy in 46 (41%) and surgery with adjuvant radiotherapy in 32 (29%). EGFR, MMP-2, MMP-9 and VEGF expressions were analyzed with tissue microarray immunohistochemical technique. RESULTS: The disease-free interval ranged from 0.89 to 140.9 months with a median of 6.87 months. The patients were categorized into two groups: Those with recurrence in less than 1 year (69 patients - 62.2%) and those with recurrence after 1 year (42 - 37.8%). The group with the shorter disease-free interval presented a worse prognosis (p=0.01). The clinical stage of recurrence (rCS) was I/II in 31 cases (27.9%) and III/IV in 80 cases (72.1%). Patients with more advanced diseases (rCS III/IV) had worse rates of cancer specific survival (CSS) than patients with rCS I/II (p=0.04). An over-expression of EGFR was associated with worse treatment results. Positive EGFR cases had a 3 year CSS of 27.2%, while EGFR negative patients had 64.3% (p=0.001). The MMP-2 and MMP-9 over-expression were also associated with a worse prognosis but without statistical significance (p=0.83 and p=0.15). VEGF expression did not show prognostic significance in this group of patients. In a multivariate analysis only the disease-free interval and over-expression of EGFR were associated with a higher risk of death. CONCLUSION: Local recurrence in oral and oropharyngeal squamous cell carcinomas usually indicates an unfavorable prognosis. A disease-free interval greater than 1 year and a negative EGFR expression are the main prognostic factors which indicate a better cancer specific survival rate in patients submitted to salvage surgery.

Estudo prospectivo da mielite transversa longitudinalmente extensa: análise dos fatores clínicos, laboratoriais e imagenológicos / Prospective study of longitudinally extensive transverse myelitis: analysis of clinical, diagnosis and prognosis factors

ntrodução: A mielite transversa longitudinal extensa (MTLE), uma forma grave de síndrome medular (SM) inflamatória associada à lesão da medula espinhal (LME) propagada por três ou mais segmentos vertebrais (SV) à ressonância magnética (RM), é considerada um espectro da neuromielite óptica (ENMO). A neuromielite óptica (NMO), caracterizada por episódios graves e recorrentes de neurite óptica e mielite, é associada a um anticorpo específico, direcionado contra a aquaporina-4 (AQP4-IgG), presente em cerca de 80% dos casos da NMO e 40% dos casos de MTLE isolada. O uso de terapia imunossupressora de manutenção, indicada em pacientes com NMO, é indefinido naqueles com primeiro episódio de MTLE (PEMTLE), cujos casos soronegativos podem representar o evento inicial da NMO ou uma forma isolada da MTLE (MTLEI). A distinção entre estas entidades, por critérios clínicos, paraclínicos e evolutivos, ainda não foi estudada e constitui o objetivo deste estudo. Métodos: Entre 2005 e 2011, 182 pacientes consecutivos admitidos na Clinica Neurológica da Faculdade de Medicina da Universidade de São Paulo com MTLE foram avaliados. Critérios de exclusão: doença vascular, desmielinizante, infecciosa ou sistêmica que justificasse o quadro de MTLE. Critérios de inclusão: 41 pacientes com PEMTLE, sem etiologia definida. A casuística consistiu de 27 (65,9%) mulheres, e 33 (80,5%) afrodescendentes, com idade média de 39,5 anos. Avaliados quanto à sorologia para o AQP4-IgG (imunofluorescência indireta) e quanto à recorrência, os pacientes foram classificados como ENMO ou MTLEI ao final do acompanhamento prospectivo. Os dados clinicos, laboratoriais, imagenológicos e evolutivos foram comparados, e o modelo de análise discriminante (AD) foi utilizado para diferenciar as duas condições clínicas. Resultados: Após o seguimento prospectivo médio de 3,7 anos (DP±2,2), 22 pacientes (53,%) apresentaram MTLE isolada monofásica e soronegativa e 19 (46,3%) receberam diagnóstico de ENMO, em razão da soropositividade para AQP4-IgG em 11 (26,8%) e/ou recorrência em 12 (29,3%) pacientes. O perfil demográfico não distinguiu os grupos (p>0,5), e as seguintes variáveis clínicas sugeriram o diagnóstico de MTLEI (p<0,05): SM estrita de instalação aguda, com média de 7 dias, com déficit motor moderado, menor freqüência de sinal de Lhermitte e espasmos tônicos, ausência de sintomas sistêmicos e sugestivos de comprometimento do tronco, tais como mialgia, astenia, anorexia, soluços, náuseas ou vômitos. As seguintes variáveis paraclínicas suportaram o diagnóstico de MTLEI (p<0,05): discreta pleocitose no líquido cefalorraquidiano, sobretudo com menos de 50 células/mm3, e LME à RM com extensão menor que 6 SV, associado à ausência de lesões bulbares e alterações encefálicas inespecíficas. A distinção entre a MTLE isolada e o ENMO foi validada pelo modelo de AD (p=0,005). Na fase aguda, a redução na incapacidade funcional ocorreu em todos os pacientes tratados (p<0,05). Na fase crônica, o uso de tratamento imunossupressor associou-se à menor chance de recorrência quando comparada à historicamente estimada, em pacientes soropositivos, e não implicou em menor chance de recorrência no grupo de soronegativos (p=1,0). Conclusão: O primeiro episódio de MTLEI apresenta características clinicas, laboratoriais e radiológicas distintas do ENMO. O perfil clínico, a soronegatividade ao AQP4-IgG e a evolução monofásica no período de 3 anos, permitem delinear uma entidade clínica restrita à medula espinhal / Background: Longitudinally extensive transverse myelitis (LETM), refers to severe inflammatory spinal cord syndrome associated with propagate lesion spanning over three or more vertebral segments (VS) on spinal cord Magnetic Resonance Image (MRI), and is regarded a spectrum of NMO (NMOSD). NMO, characterized by recurrent course of severe optic neuritis and myelitis, is associated with serum antibody that target aquaporin-4 (AQP4-IgG) in 80% of NMO and around 40% of LETM. Chronic immunosupression is suggested in NMO, while is uncertain in first-ever LETM (FELETM). In this context, seronegative cases may represent a limited form of NMO or an isolated LETM (ILETM). Discriminate between this clinical conditions, through significant therapeutic implications, are not clear. The aim of present study is distinguish ILETM from NMOSD by clinical, paraclincal and evolution parameters. Methods: We evaluated all 182 consecutive patients admitted with LETM at Neurologic Clinic of Sao Paulo University School of Medicine between 2005 until 2011. Exclusion criteria were vascular, demyelinating, infectious or systemic disease associated with LETM. The sample consists of 41 FELETM patients, 27 (65.9%) female, e 33 (80.5%) afro-brazilian, with mean age of onset of 39.5 (±15.5 of standard deviation (SD). Patients, evaluated to AQP4-IgG serologic status and recurrence, were classified as ILETM or NMOSD at end of follow-up. Clinical, serological, neuroimaging and evolutive data were compared and discriminant analysis model were done to distinguish between both conditions. Results: Over the mean time of follow-up of 3.7 years (±2.2SD), 22 patients presents as monophasic seronegative ILETM, while 19 (46.3%) were diagnosed as NMOSD due AQP4-IgG seropositivity in 11 (26.8%) or recurrent evolution in 12 (29.3%) patients. Demographic data did not discern groups (p>0.5), while following clinical features suggested ILETM (p<0.05): acute installation, over seven days mean, of strict spinal cord syndrome, characterized by moderate motor impairment, and lower frequency of Lhermitte sign and tonic spasms, associated without systemic and brainstem symptoms such as myalgia, anorexia, asthenia, nausea, hiccups or vomiting. Subsequent paraclinical findings discern both conditions and endured ILETM (p<0.05): mild pleocitosis, typically under 30 cels/mm3, and SCL propagate over lesser then six VS on MRI beside absence of bulbar and nonspecific brains abnormalities. AD model validate distinction between ILETM and NMOSD (p=0.005). Reduction in disability occurred in all patients treated with immunesuppressors at acute stage (p<0.001). Seropositive LETM patients developed lower recurrence rate when compared with hystoric controls. In seronegative LETM patients, relapse rate did not correlate with chronic imunossupression (p=1.0). Conclusion: First-ever LETM presents clinical and paraclinical features distinctive from NMOSD. Thus, clinical pattern, AQP4-IgG negative serologic status and monophasic evolution over three years follow-up, allowed us delineate a spinal cord-restrict clinical entity

conditions. Although in very low concentrations, they can show some toxic effects. While trying to bind electrons, in the chemical reaction of oxidation, they rapidly and unpredictably bind to adjacent molecules- proteins, lipids, carbohydrates and nucleic acids from which the structural elements of the cell are made, triggering the internal pathway of apoptosis. Antioxidants are substances that prevent or significantly reduce the oxidation of biomolecules. Oxidative stress is a condition that occurs when the production of free radicals exceeds the capacity of antioxidant enzymes to neutralize them. The antioxidant enzymes include: superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase (GPx), glutathione reductase (GR), and glutathione-S-transferase (GST). Lipid Peroxidation (LP) is the process of oxidation of polyunsaturated fatty acids by free radicals. Malondialdehyde is a biochemical marker by which it is possible to measure the degree of oxidative damage of cell membranes. The oxidative modification of DNA leads to a change in DNA structure that results in genetic damage. The most commonly used marker of oxidative stress is urinary 8-hydroxy-2-deoxiguanosine (8-OHdG). The damage to proteins, lipids and DNA is an important basis for many diseases such as atherosclerosis, neurodegenerative diseases, diabetes, obesity, aging, retinopathy, chronic inflammatory disease and cancer. Starting from the hypothesis that these biomolecules are different at different stages of the disease, they could represent a prognostic marker of the progression of the disease. The aim of the study was to examine whether there were differences between the control group (healthy women), the patients with precancerous lesions on the cervix (HSIL), the patients with early stage cervical cancer (FIGO Ia-Ib) and the patient with locally advanced cervical cancer (IIa - IV) in the indicators of DNA damage (determining the value of 8-OHdG), indicators of oxidative stress (by determining the lipid peroxidation intensity (TBARS)), indicators of antioxidant defense (by determining the activity of antioxidative enzymes of superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase GPx), glutathione reductase (GR), and glutathione-S-transferase (GST)). In addition, the aim of the study was to compare the values of 8- OHdG, lipid peroxidation products (TBARS) and the activity of antioxidant enzymes (SOD, CAT, GST, GPx, GR) within the group of patients with early stage cervical cancer divided into two subgroups- with low and high risk in relation to the relapse of the disease. The research was performed at the Clinic for operative oncology, Department of Gynecology at the Institute of Oncology of Vojvodina, Medical Faculty in Novi Sad, Department of Pharmacy and the Institute for Health Care of Novi Sad in the period from 2013 to 2017. Samples of blood and urine of the patients were collected, prepared adequately and stored at -80 &deg; until the analysis. The activity of the antioxidant enzymes as well as the lipid peroxidation were determined by spectrophotometric methods, and the concentration of 8-OHdG was determined by gas chromatography with mass detection. The approval of the Ethical Committee of the Institute for Oncology of Vojvodina was obtained before conducting the research. It has been shown that there are statistically significant differences between the control group (healthy women), patient with precancerous cervical lesions (HSIL), the patients with early stage cervical cancer (FIGO Ia-Ib) compared to a group of patients with locally advanced cervical cancer (IIa-IV) in indicators of damage to DNA (concentration of 8-OHdG), indicators of oxidative stress (lipid peroxidation (TBARS)), indicators of antioxidant defense (activities of antioxidant enzymes SOD, CAT and GST). There was no difference between the groups in activity of glutathione peroxidase enzyme (GPx) and glutathione reductase (GR). There were no differences in the concentration of 8-OHdG, lipid peroxidation products (TBARS) and the activity of antioxidant enzymes (SOD, CAT, GST, GPx and GR) within the group of patients with locally restricted cervical cancer divided into two subgroups with low and high risk in relation on relapses of the disease. CAT and GST activities were the best predictors of disease recurrence among defined groups. Based on the activities of these two oxidative enzymes, the separation of the group of patients who did not experience disease recurrence after a follow-up period from the other two groups in which recurrence of the disease occurred was possible. Based on the obtained results it is concluded that it is possible to use the studied biomarkers as diagnostic markers in patients with cervical cancer. These biomolecules can help in the patient&#39;s classification into certain groups according to the stage of the disease, and consequently the more efficient choice of appropriate treatment. In addition, CAT and GST enzyme activity have been shown to be predictors of disease recurrence in defined patient groups.</p>

Diferències entre dones i homes en el càncer de bufeta urinària: etiologia, clínica i pronòstic

La present tesi avalua les diferències entre homes i dones quant a les característiques sociodemogràfiques i clínicopatològiques, procés diagnòstic, tractament i pronòstic en una sèrie de casos diagnosticats de novo de càncer de bufeta en 18 hospitals de 5 regions espanyoles (estudi EPICURO). També s'estudia l'associació entre tabac i risc de càncer de bufeta segons el sexe en un estudi agregat d'estudis cas-control europeus i nord-americans de càncer de bufeta.Es trobaren diferències entre sexes quant a la incidència de la malaltia, en algunes característiques anatomopatològiques dels tumors i quant a tractament. No es varen observar diferències entre sexes davant d'un mateix nivell d'exposició al tabac. També s'observaren diferències entre homes i dones quant al risc de recidivar i de progressar dels tumors vesicals superficials, però no en el risc de morir dels pacients amb tumors invasius. / The thesis evaluates differences related to sociodemographic and clinic-pathological characteristics, diagnostic tests, treatment and prognosis of bladder cancer patients newly diagnosed in 18 hospitals from 5 Spanish areas according to sex. The work also assess the association between tobacco and bladder cancer risk according to sex in a pooled analysis of case-control studies of bladder cancer from Europe and North America.Differences between sex concerning disease incidence, pathological characteristics and treatment were observed. The relative risk of bladder cancer associated with tobacco was similar in both sex. Differences between men and women were observed regarding risk of recurrence and progression of their superficial tumors but not regarding risk of death because of an invasive tumor.

Long-Term Outcome after Lithium Augmentation in Unipolar Depression: Focus on HPA System Activity

Background: Lithium augmentation is a first-line strategy for depressed patients resistant to antidepressive therapy, but little is known about patients’ subsequent long-term course or outcome predictors. We investigated long-term outcomes of unipolar depressed patients who had participated in a study on the effects of lithium augmentation on the hypothalamic-pituitary-adrenocortical system using the combined dexamethasone/corticotrophin-releasing hormone (DEX/CRH) test. Methods: Twelve to 28 months (mean 18.6 ± 4.6 months) after lithium augmentation, 23 patients were assessed with a standardized interview, of which 18 patients had complete DEX/CRH test results. Relapse was diagnosed by DSM-IV criteria (Structured Clinical Interview for DSM-IV; SCID I). Results: Only 11 patients (48%) had a favorable follow-up, defined as absence of major depressive episodes during the observation period. Patients with a favorable and an unfavorable course did not differ in clinical or sociodemographic parameters, endocrinological results or continuation of lithium. However, fewer previous depressive episodes tended to correlate (p = 0.09) with a favorable course. Conclusion: Results from studies using the DEX/CRH test to predict relapse in depressed patients treated with antidepressants were not replicated for lithium augmentation. Our finding could reflect the elevation of DEX/CRH results by lithium, independent of clinical course. Limitations of the study are its small sample size, the heterogeneous clinical baseline conditions and the lack of lithium serum levels.
