Do psychosocial interventions for psychotic disorders improve quality of life in adults with psychotic disorders in forensic settings? : a systematic review and narrative synthesis ; and, Modified metacognitive training for negative symptoms in psychosis : a feasibility study

This thesis focuses on psychosocial interventions for psychosis. It consists of two parts: a systematic review on quality of life in forensic settings and an empirical study on negative symptoms. The systematic review follows the publication guidelines of the journal International Journal of Forensic Mental Health whilst the empirical study follows the publication guidelines of the journal Clinical Psychology and Psychotherapy. Reasonable adjustments have been made to the formatting of this thesis to enhance readability. Purpose: The systematic literature review aimed to summarise and critically appraise studies that have evaluated the effects of psychosocial interventions for psychotic disorders in forensic settings on quality of life. The empirical study aimed to evaluate the feasibility of Metacognitive Training (MCT) for negative symptoms and to identify mechanisms of change. Methods: The literature was systematically searched (using four databases) for research that included any quantitative measure of quality of life (i.e. self-esteem, quality of life, life satisfaction, and/or self-efficacy in relation to life-goals). In the empirical study, a new intervention was developed by modifying MCT for negative symptoms and four aspects of feasibility were evaluated: acceptability, practicality, demand and limited efficacy. The quantitative approach was supplemented with qualitative interviews on participants' views of the intervention. In addition, potential mechanisms of change were evaluated using a promising new method for analysing data from case-series: multilevel modeling. Results: In total, 10 papers met the inclusion criteria in the systematic review. Significant improvements in quality of life were found in five studies. The modified version of MCT showed good feasibility as demonstrated by the attendance rate, the positive oral feedback from participants and the multidisciplinary team, and the improvements on negative symptoms that were found following the intervention. Multilevel modeling proved useful in explaining the variance attributable to three different predictors: depression, internalised stigma, and reflective functioning. Conclusions: It was found that quality of life can be improved in forensic settings using psychosocial interventions. The pilot study indicated that MCT for negative symptoms has high feasibility and that changes in negative symptoms can partially be explained by depression, stigma, and reflective functioning.

A formulação da agenda e o ativismo em torno do marco civil da Internet

Esta dissertação se debruça sobre a formulação da agenda, o processo de tramitação, de aprovação, e de constituição de uma rede de apoio ao Marco Civil da Internet (MCI), sob a perspectiva da literatura de políticas públicas e ativismo. O MCI foi proposto em 2009, pelo Executivo, a partir de uma consulta pública realizada via Internet. Em 2013 o projeto foi colocado em regime de votação urgente no Congresso Nacional como resposta política às denúncias de espionagem eletrônica por parte dos EUA, sendo finalmente aprovado no início de 2014. A lei foi considerada pioneira por estabelecer direitos e responsabilidades entre os diversos entes que compõem o ecossistema da rede. Para analisar a formulação da agenda é utilizado o conceito de “janela de oportunidades” de Kingdon (2013), que designa o momento em que há a convergência de fluxos independentes da política pública. Para analisar o ativismo e a mobilização dos atores em torno das políticas de Internet é utilizado o conceito de insurgent experts, proposto por Shaw (2011). Para a realização do trabalho foram entrevistados 37 atores-chave ao longo de 2013 e 2014. A conclusão é que houve a constituição de uma policy community que mobilizou Estado e estabeleceu laços de cooperação entre atores individuais e coletivos em torno de ideais sobre as políticas relacionadas a neutralidade da rede, a defesa da liberdade de expressão e a proteção da privacidade na Internet. / This master’s thesis is focused on the process of agenda-setting and approval of the bill “Marco Civil da Internet” (MCI), drawing from the sociological literature on public policy and civic engagement. The MCI was proposed in 2009 by the executive power through a public consultation online. In 2013, it was fast-tracked by president Dilma Rousseff after classified documents were leaked by the NSA whistleblower Edward Snowden, revealing the fact that the US intelligence agency had been monitoring the president's communication. The bill was finally approved in early 2014 and pioneered a new framework for a civil Internet with rights and duties. To analyze the process of agenda-setting, I use Kingdon's (2013) concept of "policy window" which addresses the convergence of independent streams of public policy. In order to analyze the civic engagement and the network formation for mobilization in support of the project, I use the concept of "insurgent experts" proposed by Shaw (2011). The corpus of analysis for this research was composed by 37 interviews with key-actors conducted from 2013 to 2014. In conclusion, I demonstrate how the constitution of a policy community around the issue of net neutrality, the defense of freedom of expression and protection of privacy on the Internet, that created the conditions for mobilization of State, creating cooperation ties among activists and policy makers for the defense of policy proposals for the Internet.

The four homes that will be more closely analyzed are: the "Cottage" ( Casinha ), from 1942; the Second House of the Architect, from 1949; Olga Beata s House, from 1957, and Elza Berquó s House, from 1967. Other homes will also be studied, such as Rubens de Mendonça s House, or Triangles s house, from 1959; the Telmo Porto s house, from 1968, and Martirani s house, from 1974. The analyzis will be performed from the comparison of photographic sources from different times. Each set of users, at different times, helped set up the arrangement of the rooms in different ways, which allows to indirectly check their interference in the internal arrangements of the houses. / Este trabalho pretende tratar o tema da arquitetura paulista moderna, com foco prioritário nos interiores arquitetônicos, e o estudo de caso de quatro residências selecionadas de João Batista Vilanova Artigas, analisando sua arquitetura e configuração interna, ou seja, o conjunto de elementos que criam a atmosfera dos ambientes. As quatro casas que serão mais atentamente analisadas são: a Casinha , de 1942; a Segunda Casa do Arquiteto, de 1949; a Casa Olga Baeta, de 1957 e a Residência Elza Berquó, de 1967. Outras residências do arquiteto serão mais brevemente estudadas, tais como a Casa Rubens de Mendonça, ou Casa dos Triângulos, de 1959; a Casa Telmo Porto, de 1968 e a Casa Martirani, de 1974. As análises serão realizadas a partir da comparação de fontes fotográficas de diferentes épocas e atuais. Cada conjunto de usuários, em diferentes épocas, ajudou a configurar as ambientações dos cômodos de diferentes maneiras, o que possibilita verificar, indiretamente, suas interferências nos arranjos dos interiores das casas.

Den här undersökningen är en kvalitativ innehållsanalys för vilka typer av gestaltningstekniker Socialdemokraterna, Moderaterna och Sverigedemokraterna använder sig av i sina valfilmer under valkampanjerna 2010, 2014 och 2018. Ett kodschema inspirerat av Fairhurst och Sarr i boken “ The art of Framing: Managing the Language of Leadership” har legat till grund för att kunna besvara frågorna “vilka budskap som förs fram i filmerna och hur de gestaltas, hur denna gestaltning förändrats över tid genom valrörelserna och om det i så fall finns skillnader mellan valrörelser och partier.” Sammanlagt har nio filmer analyserats, ett urval baserat på vilka filmer med flest visningar på YouTube som släppts närmast inpå respektive val. I denna kvalitativa innehållsanalys har frågor ställts till materialet utifrån Fairhurst och Sarrs framingverktyg; metaforer, kontraster, jargong och slagord, spin och stories. H uvudresultatet som framkommit är att även det svenska valkampanjandet via filmmediet gått mot ett ökat skuldbeläggande på meningsmotståndare, under kategorin spin (en strategi som funnits med Sverigedemokraterna hela vägen, men nu även blivit tydligt hos Socialdemokraterna och Moderaterna), likväl som ett ökat fokus på partiledaren, där samtliga partiledare tar rollen som den “allvetande berättaren”.

Parabolic boundary value problems with rough coefficients

This thesis is motivated by some of the recent results of the solvability of elliptic PDE in Lipschitz domains and the relationships between the solvability of different boundary value problems. The parabolic setting has received less attention, in part due to the time irreversibility of the equation and difficulties in defining the appropriate analogous time-varying domain. Here we study the solvability of boundary value problems for second order linear parabolic PDE in time-varying domains, prove two main results and clarify the literature on time-varying domains. The first result shows a relationship between the regularity and Dirichlet boundary value problems for parabolic equations of the form Lu = div(A∇u)−ut = 0 in Lip(1, 1/2) time-varying cylinders, where the coefficient matrix A = [aij(X, t)] is uniformly elliptic and bounded. We show that if the Regularity problem (R)p for the equation Lu = 0 is solvable for some 1 < p < then the Dirichlet problem (D*) 1 p, for the adjoint equation L*v = 0 is also solvable, where p' = p/(p − 1). This result is analogous to the one established in the elliptic case. In the second result we prove the solvability of the parabolic Lp Dirichlet boundary value problem for 1 < p ≤ ∞ for a PDE of the form ut = div(A∇u)+B ·∇u on time-varying domains where the coefficients A = [aij(X, t)] and B = [bi(X, t)] satisfy a small Carleson condition. This result brings the state of affairs in the parabolic setting up to the current elliptic standard. Furthermore, we establish that if the coefficients of the operator A and B satisfy a vanishing Carleson condition, and the time-varying domain is of VMO-type then the parabolic Lp Dirichlet boundary value problem is solvable for all 1 < p ≤ ∞. This is related to elliptic results where the normal of the boundary of the domain is in VMO or near VMO implies the invertibility of certain boundary operators in Lp for all 1 < p < ∞. This then (using the method of layer potentials) implies solvability of the Lp boundary value problem in the same range for certain elliptic PDE. We do not use the method of layer potentials, since the coefficients we consider are too rough to use this technique but remarkably we recover Lp solvability in the full range of p's as the elliptic case. Moreover, to achieve this result we give new equivalent and localisable definitions of the appropriate time-varying domains.

Percepção de competência e aprendizagem motora / Perceived competence and motor learning

O objetivo desse estudo foi verificar a mútua relação entre percepção de competência e aprendizagem motora. Para isso, 60 indivíduos entre 18 e 35 anos de idade, de ambos os sexos, inexperientes na habilidade forehand open space do tênis de campo foram divididos em três grupos de acordo com o preenchimento do questionário PSPP de percepção de competência: (1) alta percepção, (2) média percepção e (3) baixa percepção. Ainda, cada grupo foi subdividido de acordo com a possibilidade de controlar ou não uma determinada variável que afeta a aprendizagem motora, o estabelecimento de meta. Desta forma, os três grupos foram subdivididos em grupo escolha (auto-estabelecimento de meta) e grupo espelho (meta estabelecida pelo instrutor). A tarefa consistiu no forehand do tênis de campo com o objetivo de acertar um alvo redondo posicionado na quadra adversária. Foram três blocos de 20 tentativas durante três dias de aquisição e um teste de transferência, alterando a velocidade da bolinha, aplicado 10 minutos após o último bloco de tentativas da fase de aquisição. Os indivíduos preencheram o PSPP, todos os dias antes da prática e, no último dia antes e após a prática. Os resultados mostraram que a percepção de competência influenciou a aprendizagem motora de maneira que, indivíduos com alta percepção de competência aprenderam melhor que indivíduos de baixa percepção de competência. E, que o inverso não ocorreu, isto é, a aprendizagem motora não afetou a percepção de competência. Além disso, verificou-se que os grupos espelho obtiveram melhores desempenhos e superiores percepções de competência quando comparados aos grupos escolha / The aim of this study was to verify the mutual relationship between perceived competence and motor learning. For this purpose, 60 individuals between 18 and 35 years of age, of both sexes, inexperienced in the forehand open space skill of the tennis were divided into three groups according to the questionnaire PSPP of perceived competence: (1) high perception, (2) average perception and (3) low perception. Still, each group was subdivided by considering the possibility to control or not a particular variable that affects motor learning, the goal setting. Thus, they were divided into choice group (target of self-establishment) and yoked group (target set by the instructor). The task consisted in the forehand of the tennis field in order to hit a target positioned round the opponent\'s court. There were three blocks of 20 trials over three days of acquisition and a transfer test by changing the speed of the ball. This test was applied 10 minutes after the last block of trials of the acquisition phase. The participants answered the PSPP every day before the practice, and on the last day before and after practice. Results showed that the perception of competence influenced the motor learning so that individuals with high level of perceived competence learned better than those individuals with low levels of perceived competence. And, the inverse did not occur, i.e., motor learning did not affect the perception of competence. Furthermore, it was found that the yoked groups had better performances and higher levels of perceptions when compared to choice groups

Da violência à ética: considerações sobre o enquadre de entrevistas com crianças no âmbito do judiciário / From violence to ethics: considerations on the framing of childrens interviews at a juridical setting

A presente pesquisa visa refletir sobre o lugar e o papel da psicologia no contexto do judiciário, propondo-se a realizar uma aproximação entre as áreas da psicologia e do direito, por meio do campo da ética. Objetiva, de forma mais específica, discutir o enquadre de entrevista com crianças que sofreram experiências de violência, dispondo-se a fazê-lo a partir de uma perspectiva assentada no ethos humano. Metodologicamente o trabalho constitui-se como pesquisa qualitativa, fundamentada no método clínico. Configura-se ao redor de situações de atendimento referentes a crianças que sofreram experiências de violência, tiveram os pais destituídos do poder familiar e encontravam-se sob a responsabilidade legal do poder judiciário. Como procedimento são utilizadas narrativas transferenciais de entrevistas, as quais foram estruturadas por meio de desenhos e histórias. A análise tem como principais referências as concepções de autores como Winnicott e Safra. Prioriza aspectos associados a transicionalidade e ao que se entende como necessidades éticas da condição humana - dentre elas, a necessidade de ser reconhecido e compreendido em sua singularidade, encontrar um lugar na subjetividade do outro e uma morada no mundo dos homens. Dentro dessa perspectiva, o termo ethos é tomado prioritariamente na pesquisa como lugar, campo relacional e posicionamento diante do outro. Considera-se, em contrapartida, violência, toda ação que reduz o ser humano à condição de coisa/objeto ou repercute no sentido do esfacelamento ou fratura do seu ethos. Entende-se, por sua vez, como intervenção ética o posicionamento diante do outro que preserva a sua condição de pessoa e favorece o vir a ser de si mesmo. A partir desse posicionamento epistemológico/ético e político, a pesquisa busca pensar um modelo de enquadre que considere a criança não só em sua condição biográfica e ôntica, mas em sua dimensão ontológica. Procura estruturar uma proposta de entrevista que possa, por um lado, atender à demanda e exigência institucionais, fornecendo subsídios para tomada de decisão judicial sobre o caso, mas por outro, constitua-se como uma intervenção ética, sob a forma de um olhar que não objetifique/coisifique a criança e se faça testemunho frente a fraturas éticas. Um olhar que reconheça a criança em singularidade, favoreça uma comunicação mais espontânea e profunda, 12 dialogue a partir do seu idioma pessoal e sustente o seu vir a ser. Em última instância, a pesquisa visa pensar em um enquadre de entrevista no contexto jurídico, que seja mais justo à condição humana / This research has the general objective of approximating the areas of psychology and law through the ethical field. It aims, more specifically, to discuss the framing interviews with children who have suffered violence, proposing to do so from a perspective based on the human ethos. Methodologically speaking, the task is configured as qualitative research, with fundaments based on the clinical method. It centers around reports of situations related by children in attendance, who have suffered violence, had their parents destituted of family authority and are under legal custody of the judiciary. Procedures used were transference narratives of the interviews, structured by means of drawings and stories. The main references for the analysis are drawn from the concepts of authors such as Winnicott and Safra. Aspects associated to transitionality are prioritized and those understood to be ethical necessities of the human condition - amongst which: the need to be singularly recognized and understood, to find a place in the subjectivity of the other and a dwelling in the world of men. Within this perspective, the term ethos in this research is taken to mean primarily the place, relational field or positioning vis-a-vis the other. Conversely, violence is considered as all action which reduces the human being to the condition of a thing/object or has repercussions in the sense of destroying or fracturing his ethos. The positioning in front of the other that preserves the persons condition and encourages her to become herself, is in turn understood to be an ethical intervention. From this political, epistemological/ethical stand, the research strives to think a framework (setting) model which considers the child not only in her biographical and ontical condition, but also in her ontological dimension. The study seeks to structure a proposal for interviewing which can on the one side attend institutional demands, furnishing subsidies for judicial decision-making, but on the other constitutes an ethical intervention, from an optic which neither objectifies the child nor turns her into a thing. Such an optic is a testimony with regard to ethical fractures, which recognizes the child in her singularity, favoring a deeper and more spontaneous communication, dialoguing in her personal language and sustaining her future development. In the last instance, the research seeks to contemplate the setting interview into the juridical framework in a manner most fitting for the human condition

Um estudo da temporalidade nos romances: O amanuense Belmiro e Para sempre / A study of temporality in the novels: O Amauense Belmiro and Para Sempre

O objetivo deste trabalho é, à luz da Literatura Comparada, tecer uma rede de vínculos entre os romances O Amanuense Belmiro, de Cyro dos Anjos, e Para Sempre, de Vergílio Ferreira, para cotejá-los, realçando suas semelhanças e dessemelhanças, e, conseqüentemente, compreendê-los melhor. Para isso, após breves considerações sobre determinados conceitos relativos à composição do romance, como enunciado e enunciação, narrador e narratário, o foco narrativo, o espaço e a personagem, centralizamos a discussão no conceito de tempo distinguindo três noções: tempo físico, tempo interior e tempo lingüístico, com destaque para as duas últimas e o utilizamos como eixo no estudo das obras, na intenção de verificar como os narradores-personagens vêem seu passado e vivem seu presente. No que tange ao tempo lingüístico, privilegiamos o estudo do uso dos verbos, dentre outras palavras temporais, a fim de verificarmos os efeitos de sentido nas escolhas dos dois autores; quanto ao tempo interior, que não é marcado pelo relógio, mas pela intensidade com que as personagens vivem diferentes momentos, enfatizamos o estudo da relação das personagens com o passado. Também, em virtude da clara diferença entre os tipos de romance analisados, destinamos considerável parte do trabalho ao estudo do gênero narrativo. / The aim of this paper is to weave a set of relations, at the light of the Comparative Literature, between the novels O Amanuense Belmiro (Cyro dos Anjos, 19XX) and Para Sempre (Vergílio Ferreira, 19XX), highlighting similarities and differences in a way such as to provide a deeper understanding of them. To cope with this task, after briefly considering some concepts related to the novel composition, like the enunciation and the enunciated, the narrator and the narrated, the point of view, the setting, and the character, we centered the discussion over the concept of time making a tripartite distinction among time notions: the physical, the internal, and the linguistic, with emphasis on the latter two using it as an axis in the study of the novels, seeking to verify how the character-narrators see through their past and live their present. With regard to the linguistic time, we gave particular attention to the study of verbs, detached from other temporal words, in order that we could note the meaning effects resulting from the authors verb choices; as for the internal time, which is not measured by the clock, but by the intensity with which the characters go through different moments, we focused our study on the relation of the characters with the past. Also, owing to the clear difference between the types of novel analyzed, we dedicated a considerable effort to the study of the narrative genre.

Psicoterapia psicanalítica da fobia: o uso de imagens em um estudo de caso / Psychoanalytic psychotherapy of phobias: the use of images in a case study

Este trabalho tem o objetivo de descrever o atendimento de uma paciente utilizando imagens relacionadas à sua fobia no contexto clínico. É uma proposta de um enquadre diferenciado em psicoterapia psicanalítica com o fundamento na teoria de objetos e fenômenos transicionais de Winnicott, a qual se refere a uma área para a qual contribuem tanto realidade interna, subjetiva, quanto externa, objetiva, constituindo a área do brincar. A teoria freudiana sobre a angústia é revisada desde o início, em que a fobia era considerada um sintoma o qual podia aparecer em diversas afecções psíquicas. A denominação histeria de angústia foi proposta por Freud em 1909, para descrever a entidade nosológica em que a fobia é a questão central e cujo mecanismo é semelhante ao da histeria. Este trabalho foi desenvolvido a partir do método clínico com a apresentação do estudo aprofundado de um caso. A paciente, de 37 anos, buscou atendimento para aracnofobia. No psicodiagnóstico, foi aplicado o Procedimento de Desenhos-Estórias de Walter Trinca. Para a psicoterapia, realizada em 19 atendimentos, foram compostas, junto com a paciente, duas pastas, uma com fotos que lembram aranha e outra com fotos de aranhas e de teias. O caso foi compreendido como histeria de angústia, tendo ocorrido um rápido e intenso processo psicoterapêutico. Discute-se que o imagético instaurou uma linguagem onírica nas sessões, e o onírico, assim como o brincar, é campo de simbolização. As associações a partir das imagens foram eminentemente em torno da temática da aranha, sendo tais associações comparadas ao papel que as fantasias presentes no trabalho do sonho realizado pela consciência vígil têm com relação ao conteúdo latente do sonho. Discute-se que a transicionalidade propiciada pelo imagético permitiu um trabalho de figurabilidade e, ao mesmo tempo, teria permitido a constituição de um pensar por imagens, sendo as imagens associadas a palavras ao longo do processo. A paciente conseguiu uma transformação interna, uma recriação de si em que a pulsão sexual de morte (na concepção de Laplanche) foi transformada, a partir da função objetalizante (Green), em pulsão sexual de vida, com seus processos de integração, síntese e permitindo a constituição de unidades e vínculos. Pode-se considerar que houve cura, no sentido proposto por Herrmann. Conclui-se que o presente estudo de caso evidenciou a potencialidade do uso de imagens em favorecer a simbolização e as elaborações na psicoterapia psicanalítica da fobia. Novas investigações nessa área poderão responder quanto à questão das possibilidades de generalização desta experiência / This paper aims to describe the attendance of a patient by using images related to her phobia in the clinical context. It is a proposal for a differentiated framing in psychoanalytic psychotherapy based on the theory of objects and transitional phenomena by Winnicott, which refers to an area where realities, both internal subjective and external objective contribute and constitute the area of play. The Freudian theory about anguish is reviewed from the very beginning when the phobia was considered a symptom which could appear in diverse mental disorders. The term anguish hysteria was proposed by Freud in 1909 to describe the nosological entity in which the phobia is the central question and whose mechanism is similar to that of hysteria. This work was developed by using the clinical method and a presentation of a detailed case study. A 37-year-old female patient sought treatment for arachnophobia. In her psychodiagnosis, the Drawing-and-Story Procedure by Walter Trinca was applied. Her psychotherapy was conducted in 19 attendances, for which two folders were arranged with the help of the patient, one with photographs that reminded of spiders and another with pictures of real spiders and webs. The case was understood as anguish hysteria and treated in a rapid and intense psychotherapeutic process. It is discussed that the imagery established an oniric language in the sessions and the oniric context is a field of symbolization, just like the area of play. The associations made with the use of images were predominantly around the spider theme; such associations were compared to the role played by fantasies in the dreaming activity, which is carried out by the waking consciousness, in relation to the latent content of dreams. It is discussed that the transitionality provided by the imagery allowed an activity of figurability and at the same time it would have allowed a process of thinkingby images, having images associated with words all along the way. The patient achieved her inner transformation, a re-creation of herself in which her sexual pulsion of death (in Laplanche\'s conception) was transformed from an objectalizing function (Green) into a sexual pulsion of life, with its processes of integration and synthesis, enabling the constitution of units and links. It can be considered that healing did take place, as proposed by Herrmann. It is concluded that the presentcase study demonstrated the potentiality of using images to favor symbolizations and elaborations within the psychoanalytic psychotherapy of phobias. Further investigations in this area may respond to the question of possibilities towards generalizing the experience reported herein

Vertical Examination of Reading Environment and Student Engagement in 1st-3rd Grade Classrooms

The purpose of this study was to examine the relationship between instructional environment and student engagement during reading instruction. Environment is composed of three key elements: teacher attributes, instructional methods, and the physical classroom setting (Blair, Rupley, & Nichols, 2007; De Naeghel, Van Keer, Vansteenkiste, & Rosseel, 2012; Guthrie, Hoa, Wigfield, Tonks, & Perencevich, 2006; Housand & Reis, 2008). This study examined a first, second, and third grade classroom in one East Tennessee school. Qualitative data was collected using a combination of instructional observation and teacher interviews in order to examine existing practices for successfully engaging young readers. Teachers for each of the classrooms were interviewed; following the interview, each teacher’s classroom was observed three times to examine the teacher’s attributes and most frequently used instructional methods, the physical classroom setting, and the expressed level of engagement of the student body in the classroom. The findings indicate that environment in terms of teacher attributes, instructional methods, and physical classroom setting affects student reading engagement; classrooms with high levels of organization, novel reading areas, and opportunity for students to select reading material were found particularly effective for reading engagement.
