Recently, several records of pressure pain threshold become useful tools as topographical pressure pain sensitivity maps to assess the state of awareness on various musculoskeletal conditions. However, there are no studies that have proposed topographic maps to assess shoulder sensitivity. Objective: To develop topographical pressure pain sensitivity maps of the shoulder in subjects with SI as compared with healthy subjects, and to analyze the intra-rater reliability of the topographic maps of the shoulder in healthy and asymptomatic subjects. Methods: Initially, 29 predetermined points and 4 points in fixed anatomical locations have been identified on both shoulders of 25 patients with SI and 25 healthy and asymptomatic subjects. The map was determined from anatomical landmarks and anthropometric measurements of each individual. Of all 32 points assessed, 11 are located in bony structures, 11 in muscle bellies, 9 in tendons, 1 on the coracoacromial ligament and 1 over the acromioclavicular joint. The pressure pain threshold was evaluated at all points in the dominant and nondominant side of healthy subjects and in the symptomatic and asymptomatic sides in subjects with SI. The sides and points were randomized prior to the evaluation. The pressure pain threshold was assessed 3 times on each point (20 s of rest), and the mean of each measure was considered for analysis. Results: The methodology used allowed us to characterize a topographic map for assessing shoulder pain sensitivity in subjects with SI. There was no difference between the SI and healthy groups (p> 0,05). However, the symptomatic side of subjects with SI showed higher sensitization (p <0.05), in some locations (points 6 and 7, located on the spine of the scapula and point 10, located on the infraspinatus muscle). The tendons were the most sensitive structures, followed by the bones for both groups. There was excellent intra-rater reliability between the trials of pressure pain threshold for each point (non-dominant side of healthy subjects, ICC: 0.86-0.98; dominant side of healthy subjects, ICC: 0.89-0.96). The standard error of measurement and minimal detectable change presented range, respectively, 28.4-55.9kPa and 66.7-131.4kPa (non-dominant side), 29.4-60.8kPa and 69.6-142.2kPa (dominant side). Conclusion: The proposed topographical pressure pain sensitivity maps of the shoulder was useful for detecting the state of peripheral hyperalgesia in different anatomical structures (bones, muscles and tendons) in subjects with SI, and was reliable for assessing pressure pain sensitivity on the shoulder in healthy subjects. / Contextualização: A dor no ombro é uma das queixas mais comuns e incapacitantes, e a síndrome do impacto do ombro (SI) é uma das causas mais comuns desta sintomatologia. Recentemente, mapas topográficos com vários registros de limiar de dor à pressão tornaram-se ferramentas úteis para avaliar o estado de sensibilização em diversas condições musculoesqueléticas. No entanto, não há ainda estudos que tenham proposto mapas topográficos para avaliar a sensibilidade dolorosa no ombro. Objetivo: Propor e caracterizar um mapa topográfico de sensibilidade dolorosa à pressão no ombro em indivíduos com SI e comparar a indivíduos saudáveis, e analisar a confiabilidade intra-examinador do mapa topográfico de sensibilidade dolorosa à pressão no ombro em indivíduos saudáveis e assintomáticos. Métodos: Inicialmente, 29 pontos pré-determinados e 4 pontos em localizações anatômicas fixas, foram identificados em ambos os ombros de 25 portadores da SI e 25 indivíduos saudáveis e assintomáticos no ombro. O mapa foi determinado a partir de pontos anatômicos e medidas antropométricas de cada indivíduo. De 32 pontos avaliados, 11 se localizam em estruturas ósseas, 11 em ventres musculares, 9 sobre tendões ou junções miotendíneas, 1 sobre o ligamento coracoacromial e 1 sobre a articulação acrômioclavicular. O limiar de dor à pressão foi avaliado em todos os pontos, nos lados dominante e não dominante dos indivíduos saudáveis e, nos lados sintomático e assintomático dos indivíduos com SI do ombro. Os lados e os pontos a serem avaliados foram randomizados antes da coleta de dados. O limiar de dor à pressão foi examinado 3 vezes sobre cada ponto (intervalo 20 s), e a média entre cada medida foi considerada para a análise. Resultados: A metodologia utilizada permitiu caracterizar um mapa topográfico para a avaliação da sensibilidade dolorosa do ombro em indivíduos com SI. Não houve diferença entre os grupos SI e saudáveis (p>0.05). No entanto, o lado sintomático dos indivíduos com SI apresentou maior sensibilização (p<0.05) em algumas localizações (pontos 6 e 7, localizados sobre a espinha da escápula e ponto 10, localizado sobre o músculo infraespinal). Os tendões foram as estruturas mais sensíveis, seguido dos ossos, para ambos os grupos SI e indivíduos saudáveis. Houve excelente confiabilidade intra-examinador, entre as repetições de limiar de dor á pressão para cada ponto (lado não dominante dos indivíduos saudáveis, ICC: 0.86-0.98; lado dominante dos indivíduos saudáveis, ICC: 0.89-0.96). O erro padrão da medida e a mínima diferença detectável apresentaram amplitude, respectivamente, 28.4-55.9kPa e 66.7- 131.4kPa (lado não dominante); 29.4-60.8kPa e 69.6-142.2kPa(lado dominante). Conclusão: O mapa topográfico proposto foi útil para detectar o estado de hiperalgesia periférica em diferentes estruturas anatômicas (ossos, músculos e tendões) em indivíduos com SI, e mostrou-se confiável para avaliar a sensibilidade dolorosa à pressão no ombro em sujeitos saudáveis.

INTRODUÇÃO: A doença renal crônica (DRC) favorece o desenvolvimento de inúmeras complicações entre elas a amiloidose. Nos ombros os depósitos de amilóide desencadeiam reações inflamatórias e dor intensa o que compromete a qualidade de vida dos pacientes acometidos. As propostas terapêuticas para o tratamento dessa complicação são escassas. OBJETIVO: O objetivo do presente estudo foi avaliar o efeito da estimulação elétrica nervosa transcutânea (do inglês, TENS) no tratamento da dor e capacidade funcional do ombro e dos fatores relacionados à qualidade de vida em pacientes com DRC. MÉTODOS: Trinta pacientes foram avaliados 5 vezes num período de 6 semanas. Na 1ª semana, o aparelho foi programado para não transmitir a corrente elétrica (placebo) e nas 5 semanas subseqüentes, utilizamos corrente de baixa frequência (10Hz, 150 µs, 2xdia, 40 minutos). A dor (escala visual analógica), a funcionalidade do ombro (Escore de Constant e Escala da UCLA) e os fatores relacionados à qualidade de vida (SF-36) foram avaliados. RESULTADOS: Nossos resultados mostram que o tratamento com a TENS reduziu significativamente a dor (p<0,05), melhorou a funcionalidade do ombro (tanto pelo Escore de Constant quanto Escala da UCLA; p<0,05) já na primeira semana e estes benefícios permaneceram até o final do tratamento (5 semanas) quando comparado com o período placebo. Os domínios capacidade funcional, dor, vitalidade e aspectos sociais apresentam uma melhora após 5 semanas de tratamento quando comparado com o período placebo (p<0,05). Foi verificado ainda que este efeito foi observado mesmo com a redução da quantidade de medicação analgésica. CONCLUSÃO: Nossos resultados mostram que a TENS de baixa frequência reduz a dor e melhora a funcionalidade do ombro e os fatores relacionados à qualidade de vida de pacientes com DRC. / INTRODUCTION: Chronic kidney disease (CKD) favors the development of numerous complications such as amyloidosis. In the shoulders, amyloid deposits promote inflammatory reactions and severe pain, which compromises the quality of life of patients with this disease. There are few therapeutic options for the treatment of this complication. OBJECTIVE: The objective of the present study was to evaluate the effect of transcutaneous electrical nerve stimulation (TENS) in the treatment of pain and in shoulder functional capacity, as well as factors related to the quality of life in patients with CKD. METHODS: Thirty patients were evaluated 5 times over a period of 6 weeks. At week 1, the equipment was programmed to not transmit the electrical current (placebo) and in the 5 subsequent weeks we used low frequency current (10Hz, 150 µs, twice a day, 40 minutes). Pain (visual analog scale), shoulder functional capacity (Constant score and UCLA Shoulder Rating Scale) and factors related to the quality of life (SF-36) were evaluated. RESULTS: Our results show that the treatment with TENS significantly reduced pain (p<0.05), improved shoulder functional capacity (using both Constant score and UCLA Shoulder Rating Scale; p<0.05) at week 1 and these benefits remained until the end of the treatment (5 weeks) when compared with the placebo period. The domains of functional capacity, pain, vitality and social functioning present improvement after 5 weeks of treatment when compared with the placebo period (p<0.05). It was also found that this effect was observed even after the administration of analgesics was reduced. CONCLUSION: Our results show that low frequency TENS reduces pain and improves shoulder functional capacity and factors related to the quality of life of patients with CKD.

Background: Microscopy at work involves a combination of external exposures that pose an increased risk for eye-neck/shoulder symptoms such as; visually demanding near work, eye-hand coordination, static work postures and repetitive work. Objectives: To explore the pattern of fluctuations in eye-neck/shoulder symptoms and the reproducibility of these fluctuations during separate workdays and workweeks, for individuals who regularly perform microscopic duties at work. Method: Data was collected at 8 different times for all participants (n=16); Monday morning and afternoon and Friday morning and afternoon, during 2 different weeks at work. Results: Musculoskeletal complaints in the neck/shoulders and symptoms of eye strain increased significantly (∆Musculoskeletal complaints; p&lt;0,01, ∆Symptoms of eye stain; p&lt;0,05)  during the workweek.  The highest levels of self-reported symptoms did not correlate with the self-reported time in microscopy during the workweek (p&gt;0.05).  The same tendency for the increments in symptoms was observed during both work weeks. Only for musculoskeletal complaints did the increments of symptoms correlated with one another (p&lt;0.05).  Conclusion: The results indicate that microscopy at work is a risk factor, primarily for increased musculoskeletal complaints in the neck/shoulders. More studies are needed to assess the relationship between microscopy at work and the onset and development of eye-neck/shoulder complaints. / Bakgrund: Mikroskopering som arbetsuppgift medför en kombination flera aspekter av extern exponering som innebär ökad risk för muskuloskeletala besvär och besvär från ögonen; synkrävande närarbete med krav på öga-hand koordination, låsta arbetsställningar som medför långvarigt lågintensivt muskelarbete i nacke och axel/skuldra, samt repetitiva arbetsmönster. Trots det har mycket få studier publicerats inom området sedan början av 1980-talet. Syfte: Att undersöka variationer av muskuloskeletala besvär i nacke-axel/skuldra och besvär från ögonen under arbetsdagen och arbetsveckan, samt att undersöka upprepbarheten av variationer under arbetsdagen och arbetsveckan för personer som mikroskoperar i arbetet. Metod: Studien genomfördes med en webbaserad enkät. Datainsamling skedde vid 8 tillfällen; måndag förmiddag och eftermiddag samt fredag förmiddag och eftermiddag, under två separata arbetsveckor. Studiedeltagarna kom från en elektronisk industri (n=11) och en laboratoriemedicinsk enhet (n=6). Studien hade ett bortfall (n=1). Resultat: Muskuloskeletala besvär i nacke-axel/skuldra och värk/överansträngning i ögon ökade signifikant (∆Muskuloskeletala besvär; p&lt;0,01, ∆Värk/överansträngning i ögon p&lt;0,05) under arbetsveckan. De högsta nivåerna av besvär kunde inte relateras till deltagarnas skattade tid i mikroskopering under arbetsveckan. Samma trend av ökning i besvär kunde ses för både arbetsvecka 1 och 2. Enbart för muskuloskeletala besvär i nacke-axel/skuldra så korrelerade ökningar under arbetsvecka 1 signifikant med ökningar under arbetsvecka 2 (p&lt;0,05). Slutsats: Resultaten indikerar att mikroskoparbete är en riskfaktor, primärt för ökade muskuloskeletala besvär i nacke-axel/skuldra. Fler studier behövs inom området, bl.a. för att undersöka förekomst av kausalitet mellan mikroskopering som arbetsuppgift och utvecklandet av muskuloskeletala besvär i nacke-axel/skuldra samt besvär i ögon.

Training excessively results in the overuse of muscles. The overuse of muscles commonly causes the production of myofascial trigger points (MFTPs) within the overworked muscles. The presence of MFTPs is a condition known as myofascial pain syndrome (MPS). Myofascial trigger points may be active or latent. Either way, they produce a wide range of effects. This study focuses on the effect of reduced muscle strength. Muscle strength is essential to athletes as it determines performance. Swimmers with MFTPs will not perform at their full ability. Dry needling is an effective form of treatment for MFTPs as it produces immediate relief from the effects of MFTPs. There is not enough information on the immediate effects of dry needling on athlete performance. Since dry needling brings about the immediate relief of MFTPs, this study aims to restore a swimmer’s muscle power and hence improvement of their swimming performance post-intervention. Aim The aim of this study was to determine the immediate effect of dry needling common myofascial trigger points (MFTP) found in four muscles of the shoulder girdle on competitive swimmers’ 100m freestyle lap-times. Methods The design was a pre-test post-test quasi-experimental study. Thirty five competitive swimmers between the ages of 16 and 30 years old participated in this study. Each participant underwent one assessment. Participants’ lap-times were taken using a Sportline Econosport Stopwatch. The pre- and post-intervention lap-times were compared to each other using statistical analysis. The intervention for the purpose of the study was trigger point dry needling. Myofascial trigger points were assessed using manual palpation and the Myofascial Diagnostic Scale (MDS). Results The median lap time was slightly longer post intervention (0:01:16.10) than pre-intervention (0:01:16.03), and was highly statistically significant (p=0.001). The results of the study were inconclusive, however, as there were too many confounding variables (for example, fatigue due to repeatedly swimming laps, swimmers of a lower caliber and hence quicker fatigue rate being included in the study)which negated the effect of dry needling and so the poorer performance of the participants post-intervention could not be attributed entirely to the intervention. A small number of participant’s lap-times decreased post-intervention i.e. they performed better post-intervention. These individualswere predominantly sprint-swimmers. Conclusion Dry needling negatively affects immediate lap-time performance. Future studies should reduce the number of variables affecting the study, for example, having a sprinter versus long-distance swimmer group, testing the outcome of dry needling after the swimmer has had sufficient time (for example, a day) to rest post-intervention. / M

Introduction: Shoulder pain cases represents about 7% to 50% in the population and its prevalence in athletes between 15 and 19 years old is about 43.5%. Additionally, another pathology in the upper limb is scapular dyskinesia, caused by the scapular´s movements and position alteration. Objective: To determine association between scapula-vertebral distance and shoulder pain in athletes from a private university in Lima. Materials and Methods: Cross-sectional analytical study - preliminary. The study population were athletes from a private university in Lima. The shoulder pain variables were measured by four orthopedic tests Jobe, Patte, Gerber, and Hawkins Kennedy, where shoulder pain (YES) is considered when at least one of the tests is positive and no shoulder pain (NO) when all tests are negative. And the scapular-vertebral distance variables were evaluated in three positions, by the lateral scapular sliding test (LSST); measured in (cm). Results: 51 athletes from climbing, rugby, volleyball, and basketball sports were evaluated in this study. The age ranged was 19 to 21 years. The scapula-vertebral distance of the right side was mayor than the left. 17.65% had shoulder pain. The frequency of training and sex associated with shoulder pain (p<0.05). The scapula-vertebral distance from athletes who had shoulder pain was higher than those without it (p<0.05). Conclusion: Was found association between scapula-vertebral and shoulder pain in athletes from a private university in Lima. Its recommended doing more researches with a considerable sample to reach an adequate statistical power. / Introducción: El dolor de hombro representa casos de 7% al 50% en la población, su prevalencia en deportistas entre 15 y 19 años es de 43,5%. Adicionalmente, otra patología en la extremidad superior es la disquinesia escapular, generada por la alteración de la posición y el movimiento de la escápula. Objetivo: Determinar si existe asociación entre la distancia escápulo-vertebral y el dolor de hombro en deportistas de una universidad privada de Lima. Materiales y métodos: Estudio transversal analítico - preliminar. La población fueron deportistas de una universidad privada de Lima. Las variables fueron, dolor de hombro medido por cuatro test ortopédicos Jobe, Patte, Gerber y Hawkins Kennedy, donde se considera dolor de hombro (SI) cuando al menos de unos de los test da positivo y no dolor de hombro (NO) cuando todos los tests dan negativo, respecto a la variable distancia escápulo-vertebral evaluada en tres posiciones, por la prueba de deslizamiento escapular lateral (LSST) medido en (cm). Resultado: Se evaluaron 51 deportistas de escalada, rugby, vóley y básquetbol. La edad tuvo un rango de 19 a 21 años. La distancia escápulo-vertebral del lado derecho fue mayor que el izquierdo. El 17.65% de la población si tuvo dolor de hombro. La frecuencia de entrenamiento y el sexo estuvieron asociados con el dolor de hombro (p<0.05). La distancia escápulo-vertebral de los que tenían dolor de hombro era mayor que en los que no tenían dolor (p<0.05). Conclusiones: Si se encontró asociación entre la distancia escápulo vertebral y el dolor de hombro en los deportistas de una universidad privada de Lima. Se recomienda realizar estudios con mayor tamaño de muestra para alcanzar un adecuado poder estadístico. / Tesis

Ambulance Work : Relationships between occupational demands, individual characteristics and health-related outcomes

Although musculoskeletal disorders (MSDs) and other health complaints are an occupational problem for ambulance personnel, there is a lack of knowledge regarding work-related factors associated with MSDs and other health complaints. The overall aim of this thesis was to investigate the relationships between occupational demands, individual characteristics and health-related outcomes among ambulance personnel. A random sample of 234 female and 953 male ambulance personnel participated in a national questionnaire survey on work-related factors, and musculoskeletal and other health complaints. Physical demands was associated with activity limitation due to neck-shoulder and low-back complaints among the female personnel. Among the male personnel, physical demands was associated with low-back complaints and activity limitation due to low-back complaints. Psychological demands was significantly associated with neck-shoulder complaints, sleeping problems, headache and stomach symptoms among both female and male ambulance personnel. Worry about work conditions was associated with musculoskeletal disorders and sleeping problems, headache and stomach symptoms. A local sample of 26 ambulance personnel was followed during a 24-hour work shift and for the next two work-free days. Subjective stress- and energy levels, and cortisol levels were measured at regular intervals, and heart rate was registered continuously by electrocardiogram (ECG). Autonomic reactivity to standardized tests before (pre-work) and at the end of the work shift (post-work) was also investigated. For the whole group, baseline values of heart rate were higher pre-work than post-work, but autonomic reactivity did not differ. Increased reactivity to the mental test, modest deviation in heart rate variability (HRV) pattern during the late night hours at work and higher morning cortisol values during work than during leisure time were observed in personnel with many health complaints, but not among their co-workers without or with few complaints. Ambulance personnel with many health complaints also reported higher psychological demands and tended to be more worried about work conditions. Heart rate (HR), lactate level (LL) and perceived exertion (RPE) were investigated in 17 female and 48 male ambulance personnel during a simulated standardized work task “carry a loaded stretcher”. The ambulance personnel had to carry the loaded stretcher (920 N) up and down three flights of stairs twice. The high physiological strain (HR, LL, RPE) for the male, and near or at maximal strain for the female ambulance personnel, implied the importance to identify what kind of physical capacity is most important for ambulance personnel. Therefore, the explained variance of developed fatigue by tests of cardiorespiratory capacity, muscular strength and endurance, and coordination was investigated. The results showed that VO2max and isometric back endurance were important predictors for development of fatigue when carrying a loaded stretcher. The influence of body size on the relationships between maximal strength and functional performance was investigated in a methodological study. The results confirm that the assessment of physical performance could be confounded by the body weight. Therefore, the models for explaining development of fatigue when carrying the loaded stretcher were adjusted for height and weight. Including height in the models significantly increased the explained variance of accumulated lactate among female, but not among male personnel. Lactate levels were higher among short compared to tall female personnel. Weight had no effect on any of the models. In conclusion, the national survey showed that self-reported physical demands was a risk factor of having MSDs, and that self-reported psychological demands and worry about work were important risk factors of having MSDs and other health complaints. Stress monitoring of ambulance personnel during work and leisure time showed that physiological and subjective stress markers did not show any differences between the 24-hour ambulance work shift and leisure time afterwards. However, ambulance personnel with many health complaints had certain physiological changes during the work shift in comparison with the next two work-free days. The physiological and subjective responses during carrying a loaded stretcher, especially among the female ambulance personnel, showed that female and male ambulance personnel could be exposed to internal exposures at different levels when performing the same work task. A better understanding of the relationships between occupational demands and health-related outcomes require further studies on age- and gender matched groups in long-term perspective studies.

This cross-sectional postal survey explores the relationship between musculoskeletal painand level of education in Ullensaker municipality. We used a Standard Nordic Questionnaire(SNQ) to register self-reported musculoskeletal pain in 3325 persons in 2004 (participationrate 55 %). We registered educational level and a series of other demographic and life-stylevariables as well as functional ability and subjective health complaints. We found in thisstudy that educational level and musculoskeletal pain is associated. People with loweducational levels are prone to have more musculoskeletal pain than persons with highereducation. Vi registrerte utdanningsnivå og en rekke andre demografiske- og livsstilsvariablesamt funksjonsevne og subjektive helseplager. Vi fant i denne studien ensammenheng mellom utdanningsnivå og muskelsmerter. Personer med lavtutdanningsnivå har mer muskelsmerter enn personer med høyere utdanning. Realizamos estudo prospectivo randomizado para comparar cinco métodos específicos de avaliação funcional do ombro e uma avaliação genérica da saúde em pacientes operados de capsuloplastia e descompressão subacromial associado à reparação do manguito rotador. Foram estudados 30 pacientes de cada grupo, avaliados no período entre cinco a seis meses após o tratamento cirúrgico com os questionários dos Cirurgiões Americanos de Ombro e Cotovelo(ASES), Sistema de Pontuação do Ombro de Constant & Murley (CONSTANT), Folha de Classificação da Reparação de Bankart (ROWE), Teste Simples de Ombro (SST), Escala de Avaliação do Ombro da Universidade de Califórnia - Los Angeles (UCLA) e o Questionário Genérico de Avaliação de Qualidade de Vida (SF-36). A idade no grupo de \"Descompressão\" variou de 44 a 77 anos (média 59,2) e no grupo de \"Capsuloplastia\" foi de 17 a 65 anos (média de 31,4). Em relação ao sexo, o grupo da \"Descompressão\" foi predominantemente feminino e no grupo da \"Capsuloplastia\" foi predominante o sexo masculino. As médias da pontuação no grupo de \"Capsuloplastia\" são maiores que no \"Descompressão\" (93,6 e 71,7 respectivamente). A variabilidade dos escores observado no grupo \"Capsuloplastia\" (89,1 a 100) é inferior ao do grupo \"Descompressão\" (65,1 a 95,9). Não existe uma forte concordância entre os resultados obtidos pelos diferentes questionários (variação de zero a 0,51). A média nos oito domínios do SF-36 no grupo \"Descompressão\" foi de 70,8 variando de 60,4 (Vitalidade) a 89,2 (Aspectos Sociais). Em ambos os grupos a maior correlação encontrada foi entre os questionários UCLA e ASES (0,900 na \"Descompressão\" e 0,893 na \"Capsuloplastia\"). Concluiu-se que nas cirurgias de descompressão subacromial o questionário UCLA foi o mais completo mostrando maior confiabilidade e reprodutibilidade e para as capsuloplastias o método ROWE apresentou maior confiabilidade, reprodutibilidade e praticidade. / A prospective randomized study was realized to compare methods of both disease specific and generic health status of shoulder\'s functional assessment in patients after operative treatment of subacromial decompression associated a repair of a tear of the rotator cuff and capsuloplasty. The thirty patients in each group were studied and at follow-up five to six months later, with the following questionnaires: American Shoulder and Elbow Surgeons Shoulder Evaluation Form (ASES), Constant-Murley Shoulder Scoring System (CONSTANT), Rating Sheet for Bankart Repair (ROWE), Simple Shoulder Test (SST), University of California at Los Angeles Shoulder Rating Scale (UCLA) and the Short Form-36 (SF-36). The age in Decompression group was greater than Capsuloplasty group and the gender of first group was predominantly consisted of women and in the second group was men. The scores averages of Capsuloplasty group were higher than Decompression group (93,6 and 71,7 respectively). The variability of the scores observed in the Capsuloplasty group, 89,1 to 100,0 was less than in the Decompression group, 65,1 to 95,9. A strong interrater reliability between the questionnaires does not exist - variation 0 to 0,51. The average in the eight domains of SF-36 in Decompression group was 70,8 [range 60,4 (Vitality) to 89,2 (Social Aspect)]. In Capsuloplasty group the average was 91,2 [range 83,4 (Mental Health) to 96,6 (Emotional Aspect) ]. Pearson\'s coefficient analysis, shows that the correlation between CONSTANT and SF-36 was not detect, and in Capsuloplasty was not detect in ROWE and SF-36. In both groups Capsuloplasty (0,893) and Decompression (0,900), a strong correlation between UCLA and ASES were observed. It follows that to subacromial decompression surgery, UCLA shows more reliability and in Capsuloplasty, the Rowe method shows more applicability.

Avaliação do uso do plasma rico em plaquetas no reparo da rotura do manguito rotador / Evaluation of platelet-rich plasma use in the repair of rotator cuff tear

O plasma rico em plaquetas (PRP) tem sido utilizado na ortopedia como método para melhorar a cicatrização tecidual. Existem poucos estudos com alto nível de evidência sobre o seu efeito no reparo do manguito rotador e os resultados são conflitantes, não havendo consenso sobre sua eficácia. O objetivo primário deste estudo foi avaliar o efeito do uso do PRP em pacientes submetidos ao reparo do manguito rotador por via artroscópica através da escala da University of California at Los Angeles (UCLA). A avaliação clínica através da escala de Constant-Murley, de dor através da escala visual analógica (EVA), a presença de rerroturas na análise da ressonância magnética (RM) e a ocorrência de complicações foram considerados desfechos secundários. Estudo prospectivo, randomizado, duplo-cego. Dois grupos com 27 pacientes (Grupo PRP e Grupo Controle) foram submetidos ao reparo artroscópico em fileira simples de âncoras, sendo que, no Grupo PRP, foi aplicado o concentrado de plaquetas ao final do procedimento. O PRP foi obtido por aférese, aplicado na consistência líquida, com adição de trombina autóloga. Foram incluídas apenas roturas de espessura completa do supraespinal com retração inferior a 30 mm. Os procedimentos foram realizados pelo mesmo cirurgião, entre setembro de 2008 e abril de 2012. Os desfechos foram avaliados através das escalas da UCLA, de Constant-Murley, EVA e pela RM, pré-operatoriamente e aos 3, 6 e 12 meses. A EVA foi aplicada adicionalmente no primeiro e sétimo dia. O nível de significância empregado foi de 5%. Os pacientes apresentaram melhora clínica significativa com o procedimento nos dois grupos (p < 0,001). Evoluíram de 13,63 ± 3,639 para 30,04 ± 4,528 no Grupo Controle e de 13,93 ± 4,649 para 32,30 ± 3,506 no Grupo PRP aos 12 meses (p = 0,046) de acordo com a escala da UCLA, com um poder de 84% e tamanho do efeito de 0,56. De acordo com a escala de Constant-Murley, os pacientes evoluíram de 47,37 ± 11,088 para 76,89 ± 13,198 no Grupo Controle e de 46,96 ± 11,937 para 83,26 ± 11,141 no Grupo PRP aos 12 meses (p=0,061). Ocorreu um caso de rigidez articular em cada grupo (p = 1). O PRP obtido por aférese, aplicado na consistência líquida e com adição de trombina propiciou melhores resultados pela escala da UCLA aos 12 meses de pós-operatório / Platelet-rich plasma (PRP) has been used in orthopedics as a method to enhance tissue healing. There are few studies with a high level of evidence about its effect on rotator cuff repair, and the results are conflicting, with no consensus about its effectiveness. Clinical assessment as measured by the UCLA (University of California at Los Angeles) shoulder rating scale was established as the primary outcome. The secondary outcomes included clinical assessment on the Constant-Murley scale, pain as measured by a visual analog scale (VAS), the retear rate assessed by magnetic resonance imaging (MRI) and the complication rate. A prospective, randomized, doubleblind trial was conducted. Two groups of 27 patients (PRP Group and Control Group) were subjected to arthroscopic single-row repair. Liquid PRP prepared by apheresis was applied to the PRP Group at the end of the surgical procedure, with autologous thrombin. Complete supraspinatus tears with retraction less than 30 mm were included. The procedures were performed by the same surgeon between September 2008 and April 2012. Outcomes were assessed using UCLA and Constant-Murley scales, VAS and magnetic resonance imaging preoperatively and at 3, 6 and 12 months. The VAS was also applied on days one and seven. The significance level was 5%. The two groups of patients exhibited significant clinical improvement (p < 0.001). The score on the UCLA scale increased from 13.63 ± 3.639 to 30.04 ± 4.528 and from 13.93 ± 4.649 to 32.30 ± 3.506 in the Control and PRP groups, respectively, between the preoperative assessment and after 12 months (p=0.046), with a 84% power and a 0.56 effect size. The score on Constant-Murley scale increased from 47.37 ± 11.088 to 76.89 ± 13.198 in the Control Group and from 46.96 ± 11.937 to 83.26 ± 11.141 in the PRP Group (p = 0.061). Assessment at 3 and 6 months did not identify significant differences. The VAS did not statistically differ at any investigated timepoint and varied from 7.00 ± 1.939 and 6.67 ± 1.617 before surgery to 1.70 ± 2.127 and 1.04 ± 1.808 at the 12-month assessment in the Control and PRP groups, respectively (p = 0.220). The Control Group exhibited one case of complete and four of partial retears, and the Group PRP exhibited two cases of partial retears (p = 0.42).
