Transtorno de ansiedade social e habilidades sociais: estudo psicométrico e empírico / Social anxiety disorder and social skills: a psychometric and empirical study

O Transtorno de Ansiedade Social (TAS) tem sido considerado um grave problema de saúde mental pela sua alta prevalência em pessoas jovens e pelas incapacidades decorrentes no desempenho e interações sociais. É fundamental que se disponha de instrumentos validados e abrangentes que avaliem tanto os recursos e déficits comportamentais quanto os prejuízos sociais e funcionais destas pessoas. Objetivou-se, neste trabalho, verificar as associações entre as manifestações comportamentais e clínicas do TAS por meio de dois estudos, um psicométrico e outro empírico, visando: (a) aferir as propriedades psicométricas do Inventário de Habilidades Sociais (IHS-Del-Prette), enquanto medida do repertório comportamental de habilidades sociais, em relação à avaliação das manifestações clínicas próprias do TAS, medidas pelo Inventário da Fobia Social (SPIN); e (b) comparar e caracterizar o repertório de habilidades sociais apresentado por universitários brasileiros portadores de TAS e não portadores frente a uma situação experimental estruturada, o Teste de Simulação de Falar em Público (TSFP). Do estudo psicométrico, participaram 1006 universitários, na faixa etária entre 17 e 35 anos, de ambos os gêneros, provindos de duas instituições de ensino superior. Posteriormente, 86 participantes foram randomicamente selecionados desta amostra inicial e agrupados como casos e não-casos de TAS a partir de avaliação clínica sistemática por meio da Entrevista Clínica Estruturada para o DSM-IV. Do delineamento empírico, participaram 26 universitários, sendo 13 com diagnóstico de TAS e 13 não portadores de transtornos psiquiátricos. Quanto aos resultados, o IHS apresentou boa consistência interna para o escore total, reforçando a sua validade de construto. Observou-se boa validade concorrente entre o IHS e o SPIN, com um coeficiente altamente significativo de correlação negativa entre eles, indicando que quanto mais elaborado for o repertório de habilidades sociais de um indivíduo, menor é a sua probabilidade de satisfazer os critérios de rastreamento de indicadores para o TAS. O IHS demonstrou distinguir significativamente indivíduos com e sem TAS, atestando sua validade discriminativa e preditiva para esse diagnóstico, evidenciando-se assim a sua validade clínica e a possibilidade do seu uso em estudos empíricos que testem a eficácia terapêutica de programas de intervenção. No TSFP, os grupos caso e não-caso de TAS não demonstraram diferenças significativas, em termos de freqüência, para a maioria dos marcadores comportamentais de ansiedade avaliados. Um número maior de sujeitos do grupo não-caso foi avaliado como apresentando um nível de habilidades sociais apropriadas para falar em público, que variou de moderado a alto, em comparação ao grupo caso. Ao longo do TSFP, a freqüência de emissão dos marcadores comportamentais de ansiedade pelos sujeitos de ambos os grupos manteve-se estável. Os grupos diferiram significativamente na maioria dos itens indicativos da habilidade de falar em público do IHS e quanto ao escore geral desta habilidade. A análise dos resultados do estudo empírico aponta para a necessidade de novos estudos com amostras clínicas de indivíduos com TAS dos subtipos generalizado e circunscrito, e não-clínica, com maior número de sujeitos, previamente avaliados quanto ao medo de falar em público, e também para a possibilidade de uso do TSFP em programas de Treinamento em Habilidades Sociais. / Social Anxiety Disorder (SAD) has been considered a serious mental health problem for its high prevalence in young people and for the resulting disabilities in the performance and social interactions. It stands out, thus, as being fundamental to have comprehensive and validated instruments which evaluate both the resources and the social and functional impairments of these people. In this work, we aimed at verifying the associations between the behavioral and clinical manifestations of SAD by means of two studies, a psychometric study and an empirical one, in order to: (a) check the psychometric properties of the Social Skills Inventory (HIS-Del-Prette), as a measure of the social skills behavioral repertoire in relation to the evaluation of the typical clinical manifestations of SAD, measured by the Social Phobia Inventory (SPIN); (b) compare and characterize the social skills repertoire shown by Brazilian undergraduates with and without SAD in a structured experimental situation, the Simulated Public Speaking Test (SPST). A total of 1006 undergraduates of both genders participated in the psychometric study, with ages between 17 and 35, from two universities. Subsequently, 86 participants were randomly selected from this initial sample and grouped as SAD case and non-case from the systematic clinical evaluation. In the empirical outline, 26 undergraduates participated, 13 with a SAD diagnosis and 13 without the disorder. According to the results, IHS showed good internal consistency for the total score, reinforcing its construct validity. Good concurrent validity was demonstrated between IHS and SPIN, with a highly significant negative correlation coefficient between them, indicating that the more elaborate the social skills repertoire of an individual is, the smaller the probability that he or she will meet the screening criteria for the indicators of SAD. IHS proved to significantly distinguish individuals with and without SAD, attesting thus, discriminative and predictive validity for this diagnosis, showing its clinical validity for the diagnosis of this disorder and yet the possibility of using it in empirical studies testing the therapeutical efficacy in programs of intervention. In the SPST, the case and non-case groups of SAD did not show significant differences in terms of frequency for most of the social anxiety markers, except in relation to facial movements of discomfort, and the class of non-verbal markers, in which the non-case group presented higher values. A higher number of individuals from this group were evaluated as showing a level of appropriate social skills for speaking in public which varied from moderate to high, in comparison with the other group. Throughout SPST, the frequency of emission of anxiety behavioral markers by the participants of both groups was stable. The groups differed significantly in most items of IHS indicative of abilities to speak in public, as well as in the general score of this ability. The analysis of the results of the empirical study points to the necessity of new studies with clinical samples of individuals with SAD of the generalized and circumscribed subtypes and non-clinical, with a larger number of participants, previously evaluated as to the fear of speaking in public, and also to the possibility of using SPST in Social Skills Training.

Investigating the Role of Parenting in the Link between Social Anxiety and Coping-Related Drinking Motives among Adolescents

Elevated social anxiety is a well-documented risk factor for developing problematic alcohol use behaviors. Adolescents with high social anxiety often report drinking for coping-related reasons, and drinking to cope has been linked to both acute and chronic alcohol use problems. Research further suggests that parenting is a primary socialization domain in terms of adolescent alcohol use onset and trajectory; however, no work has yet examined the role of parenting factors in the relation between social anxiety and coping motives for drinking. The current study investigated the role of two parenting dimensions, rejection/warmth and psychological control, on the link between social anxiety and problematic drinking motives. Drawing from an ongoing assessment of an inpatient program, the sample consisted of 94 girls and boys (ages 11-17 years). Regression analyses evaluated main effects of social anxiety, rejection, psychological control, the interactive effects of the parenting dimensions, and the three-way interaction of both parenting dimensions with social anxiety on coping motives for alcohol use. As expected, social anxiety was positively and statistically significantly related to coping motives when examined via raw scores, proportional values, and in the final model. Further, zero-order correlations indicated a positive, statistically significant relation between proportional coping motives and both rejection and psychological control; however, no other hypothesis was supported. Collectively, this body of work underscores the potential benefit of integrating social stress and anxiety management in problematic alcohol use intervention efforts, as well as drinking motive education in efforts targeting social anxiety.

Assessing the relationship between resting autonomic nervous system functioning, social anxiety, and emotional autobiographical memory retrieval

Thesis advisor: Elizabeth Kensinger / Individuals with social anxiety disorder (SAD) tend to have emotional memory biases in the encoding and retrieval of social memories. Research has shown reduced heart rate variability (HRV) in clinical populations suffering from anxiety, including social anxiety. Heightened sympathetic activation—as measured by the electrodermal activity (EDA)—has also been associated with anxiety disorders. The aim of the present study was to examine the relation between HRV, social anxiety, and re-experiencing of emotional autobiographical memories. 44 healthy young adults were recruited from the Boston College campus through SONA. Participants were given an online survey that instructed them to retrieve 40 specific events from the past in response to 40 socially relevant cues. For each event, participants were instructed to provide a brief narrative, make several ratings for the event (on a scale from 1-7), and indicate the specific emotions they experienced both at the time of retrieval and of the event. Approximately one month after the completion of the memory survey, participants engaged in a 2-hour memory retrieval session while undergoing psychophysiological monitoring (heart rate, skin conductance, and respiration). Following the retrieval task, participants completed self-report questionnaires of social anxiety symptom severity and trait emotion regulation strategy (i.e., tendency to reappraise or suppress emotions). The present study found that positive memories had higher re-experiencing ratings as compared to negative memories. Contrary to the original study hypothesis, however, there was no significant interaction between average re-experiencing (or arousal) ratings of positive or negative social autobiographical memories and SAD likelihood. A nonlinear, cubic relationship was found between one of three metrics of HRV and social anxiety symptom severity. A significant effect was found between skin conductance and SAD likelihood, which was likely driven by an almost significance difference in skin conductance between the SAD unlikely and the SAD very probable groups; these findings provide further insight into the relationship between autonomic nervous system (ANS) functioning and social anxiety. Further, the present results suggest the intriguing possibility that there may be a nonlinear relationship between HRV and severity of social anxiety. The purpose of the study was to investigate the relationship betweensocial anxiety disorder and problematic internet use. Individuals withsocial anxiety disorder might prefer online interaction compared toface-to-face interaction. This could develop into problematic internetuse (PIU). PIU is when an individual uses the internet to the degreethat it negatively affects the individual’s life, based on Davis’ (2001)model of problematic internet use. The data was collected using anonline survey. There was a total of 484 participants in the finalanalyses, with 26.1% males, 71.5% females (Mage=27.99, SD= 10.1).First, a correlation analysis was done between social anxiety and PIU.The results showed that there was a positive correlation between socialanxiety and problematic internet use (r=.53). We then used age as amoderator to investigate if age had a moderating effect on socialanxiety and problematic internet use, using a moderation analysis. Theanalysis revealed a significant moderation effect of age, but a low leveloverall of PIU in the sample. This meant that even at high levels ofsocial anxiety, the sample did not display PIU-behaviors. Furtherresearch is needed to investigate the relationship between socialanxiety, PIU and what it looks like for different age groups. / Syftet med studien var att undersöka förhållandet mellan social fobioch problematisk internetanvändning och hur detta modererar medålder. Individer med social fobi kan uppleva det mer bekvämt attinteragera genom internet i jämförelse med i verkligheten. Detta kanleda till att individen utvecklar problematiskt internetanvändande(PIU). PIU innebär att en individ använder internet på ett sätt somnegativt påverkar individens liv, utifrån Davis (2001) modell förproblematiskt internetanvändande. Datainsamlingen skedde genom enwebbenkät. Totalt var det 484 deltagare i studien (Målder=27.99,SD=10.1), med 26.1% män och 71.5% kvinnor. Först genomfördes enkorrelationsanalys mellan social fobi och PIU. Resultaten visar att detfinns en positiv korrelation mellan social fobi och problematisktinternetanvändande (r=.53). Vi använde ålder som en variabel i vårforskning för att undersöka om ålder har en moderationseffekt påsocial fobi och problematiskt internetanvändande. För att undersökadetta genomförde vi en moderationsanalys. Resultaten visade att ålderhar en signifikant effekt på social fobi och PIU, men att det inte fannsen särskilt hög nivå av PIU hos deltagarna. Detta innebar att även vidhöga nivåer av social fobi uppvisade inte deltagarna PIU-beteenden.Framtida forskning är nödvändigt för att etablera hur förhållandetmellan social fobi och problematiskt internetanvändande ser ut mellanolika åldrar.

Social Skills And Social Acceptance In Childhood Anxiety Disorders

The present study examined the social skills and social acceptance of children with SAD (n=20), children with GAD (n=18), and typically developing (TD) children (n=20). A multimodal assessment paradigm was employed to address three study objectives: (a) to determine whether social skills deficits are unique to children with SAD or extend to children with GAD, (b) to assess whether skills vary as a function of social context (in vivo peer interaction Wii Task versus hypothetical Social Vignette Task) and (c) to examine the relationship between anxiety diagnosis and social acceptance. Parent questionnaire data indicated that both youth with SAD and GAD experienced difficulties with assertiveness, whereas children with SAD experienced a broader range of social skills difficulties. Blinded observers’ ratings during the behavioral assessment social tasks indicated that compared to children with GAD and TD children, children with SAD have deficits in social behaviors and social knowledge across settings, including speech latency, a paucity of speech, few spontaneous comments, questions and exclamations, and ineffective social responses. In addition, vocal analysis revealed that children with SAD were characterized by anxious speech patterns. By comparison, children with GAD exhibited non-anxious speech patterns and did not differ significantly from TD youth on social behaviors, with the exception of fewer spontaneous comments and questions. Lastly, children with SAD were perceived as less likeable and less socially desirable by their peers than both children with GAD and TD children. Clinical implications of these findings are discussed

Vitiligo is an idiopathic, non-contagious and often familial depigmentation disorder affecting both sexes equally. The mostly progressive and patchy loss of the inherited skin colour is not only a cosmetical problem, it has a profound impact on the patient¿s well-being. Stigmatisation and rejection often causes depression, self-consciousness, sexual problems and an impaired quality of life. To further substantiate earlier investigations and to introduce new aspects, we utilised the Dermatology Life Quality Index (DLQI), the Beck Depression Inventory (BDI) and the Adjustment to Chronic Skin Disorders Questionnaire (ACS) with its sub-scales on Social anxiety / avoidance, Helplessness and Anxious-depressive Mood in 422 patients and 55 healthy controls. We also included 103 children, their parents and 18 controls by using the Children¿s Dermatology Life Quality Index (CDLQI) and an adapted version of the ACS. We found that patients with vitiligo experience high levels of stigmatisation. They have an impaired quality of life and are more socially anxious / avoidant, helpless and (anxious-) depressive compared to healthy controls. The results correlate with disease severity, avoidant behaviour / hiding of vitiligo and the belief that psychological stress influences the disease. Female patients are generally more affected. Treatment with pseudocatalase PC-KUS improves quality of life and reduces anxious-depressive mood. Children also suffer from stigmatisation and an impaired quality of life. Social Anxiety and Subtypes of Empathy: The Moderating Influence of Biological Sex

Only a few studies have examined relationships between social anxiety and subtypes of empathy. Findings are mixed. The present study examined social anxiety severity on a continuum and how it related to affective and cognitive empathy in 684 nonpsychiatric adults (77% female). Participants completed an online battery of measures that included: a self-report measure of social anxiety severity (Fear of Negative Evaluation), a self-report measure with subscales for affective and cognitive empathy (Interpersonal Reactivity Index), and a behavioral measure of cognitive empathy (Reading the Mind in the Eyes Task; MIE). After statistically covarying for general anxiety severity, biological sex moderated the relationship between social anxiety severity and performance on the MIE task. In women, a higher severity of social anxiety related to better performance on the MIE. This relationship was not statistically significant in men. IRI subscale scores did not show significant main effects or interactions with sex in relation to social anxiety. The findings suggest a possible difference in how each sex experiences and/or develops social anxiety. This has implications for assessment and treatment. Future research should examine these relationships in more diverse psychiatric samples.
