Suicidnära patienters upplevelse av omvårdnad : en icke-systematisk litteraturöversikt / Suicidal patients' experiences of nursing care : a non systematic literature review

Bakgrund  Suicid orsakar stort lidande och är ett folkhälsoproblem. Sjuksköterskan har en viktig funktion i mötet med den suicidnära patienten. Genom att studera patienternas egna upplevelser kan omvårdnaden utformas till deras fördel. Katie Erikssons teori om lidande har använts som teoretisk utgångspunkt för att besvara syftet.  Syfte  Syftet var att beskriva suicidnära patienters upplevelser av omvårdnad.  Metod  En icke-systematisk litteraturöversikt baserad på 15 vetenskapliga artiklar med både kvalitativ och kvantitativ ansats användes för att besvara syftet. Artikelsökningar utfördes i PubMed, CINAHL samt PsycInfo. Sophiahemmet Högskolas bedömningsunderlag användes i kvalitetsgranskning av artiklarna. Resultatet sammanställdes genom en integrerad dataanalys.  Resultat  Resultatet presenterades under tre huvudkategorier: Att få omvårdnad genom relationer, Att bli påmind om sitt värde och Att ge upp sin autonomi och bli skyddad från sig själv. I resultatet framkom att relationen mellan patient och vårdpersonal hade stor betydelse för patientens upplevelse av omvårdnaden. Ett värdigt bemötande som inbegrep att bli bemött av vänliga, lyhörda och stöttande sjuksköterskor hade en positiv inverkan på patienten. Att få hjälp av sjuksköterskan med att få sina behov tillgodosedda och få nytt hopp var viktiga komponenter av omvårdnaden. Inläggning på sjukhus kunde upplevas som en trygghet och ett skydd mot sig själv, samtidigt som det medförde en begränsad autonomi. Observation upplevdes som ett intrång och vikten av interaktion poängterades.  Slutsats  Resultatet i litteraturöversikten visade att sjuksköterskans omvårdnad kan ha stor inverkan och förhindra suicid. Sjuksköterskan behöver vara lyhörd och inkännande inför patientens behov, sträva efter att jämna ut maktobalansen, samt att interagera och bygga relationer med patienten. Om sjuksköterskan lyckas med det kan hon lindra patientens lidande och inge hopp. Med anledning av detta behöver sjuksköterskor kunskap om vad den suicidnära patienten behöver. / Background  Suicide causes significant suffering and is a public health issue. The nurse plays a crucial role in encounters with suicidal patients. By studying the patients' own experiences, care can be tailored to their advantage. Katie Eriksson's theory of suffering has been used as a theoretical framework to address the purpose.  Aim  The aim of the study was to describe suicidal patients’ experiences of nursing care.  Method  This non-systematic literature review was based on 15 original articles, including both qualitative and quantitative method. Searches were conducted in the databases PubMed, CINAHL and PsycInfo. Sophiahemmet University ́s assessment instrument was used to evaluate the quality of the included articles. An integrated analysis was used to compile the findings.  Results  Three main categories were identified: Receiving care through relationships, Being reminded of one's worth, and Surrendering autonomy and being protected from oneself. The results highlighted that the relationship between the patient and healthcare personnel held significant importance in the patient's experience of care. Being treated by friendly, empathetic, and supportive nurses had a positive impact on the patient. Receiving assistance from the nurse in meeting their needs and gaining new hope were essential components of care. Hospitalization could be perceived as a source of security and protection from oneself, while also resulting in limited autonomy. Observation was experienced as an intrusion, and the importance of interaction was emphasized.  Conclusions  The conclusion is that nursing care can have a significant impact and prevent suicide. The nurse needs to be attentive and empathetic to the patient's needs, strive to balance power dynamics, and actively engage in building a relationship with the patient. If the nurse succeeds in doing so, she can alleviate the patient's suffering and inspire hope. Therefore, nurses need knowledge of what the suicidal patient requires.

The role of anxiety in the development of suicidal thoughts in pregnant women with mood disorders

BACKGROUND: Both mood and anxiety disorders are more prevalent in women than men with the onset typically occurring during adolescence or early childbearing years. These disorders are particularly prevalent during pregnancy and the postpartum period. While depression during the perinatal period has received significant attention recently, anxiety has not received the same amount of attention. METHODS: The current study was a secondary analysis of a prospective cohort study that followed 91 women with mood disorders through pregnancy and the postpartum period. Our objective was to determine if a correlation existed between anxiety and suicidality. We hypothesized that pregnant women with a history of a mood disorder and comorbid anxiety are more likely to be suicidal than those without comorbid anxiety. The presence of anxiety was determined using the anxiety subscale of the Edinburgh Postnatal Depression Scale, EPDS; a cut off score of six or greater was used to indicate significant anxiety. Suicidality was determined using three separate measures: question ten on the EPDS, question eighteen on the Inventory of Depressive Symptomatology, and question ten on the Montgomery-Asberg Depression Rating Scale. ANALYSIS: Chi square tests were used to compare the demographics of the anxious and non-anxious women based on both diagnosis of anxiety disorders and symptoms of anxiety. Z proportion tests were then used to compare the proportion women with anxiety versus those without anxiety who were suicidal. Lastly, binary logistic regression was used to determine if patients with anxiety were more likely to be suicidal. RESULTS: Among the women in this study, 62 (68.1%) had a diagnosis of major depressive disorder and 29 (31.9%) had a diagnosis of bipolar disorder based on DSM-IV-TR diagnostic criteria. In addition, 45 (49.5%) had a lifetime history of an anxiety disorder. The prevalence of significant anxiety symptoms, as determined by the anxiety subscale on the EPDS, ranged from 9.1-37.5% depending on the time point. Suicidality prevalence also varied depending on both the time point and the scale used: 0-17.5% using the MADRS, 6.7-24.7% using the EPDS, and 2.4-14.7% using the IDS. Using a binary logistic regression, we determined that anxiety was a risk factor for suicidality at time T3 (OR 2.106; 95% CI 1.274-3.481) and M1 (OR 2.057; 95% CI 1.179-3.586) on the MADRS and at T3 (OR1.758; 95% CI 1.219-2.535) on the EPDS.

Suicid är ett betydande folkhälsoproblem. De flesta som utför ett suicidförsök lider av en psykisk sjukdom och har oftast haft kontakt med hälso- och sjukvården innan händelsen. Antalet utalarmeringar till personer som mår psykiskt dåligt har ökat, därmed utgör suicidnära patienter en betydande patientgrupp prehospitalt. Trots det råder det en osäkerhet i omhändertagandet då ambulanssjuksköterskorna känner sig oförberedda och okunniga i vårdmötet. Syftet med studien var att belysa ambulanssjuksköterskors upplevelser av att vårda suicidnära patienter prehospitalt för att öka kunskapen och eventuellt åskådliggöra förbättringsområden som i förlängningen kan stödja den prehospitala vården av suicidnära patienter. Studien har genomförts som en kvalitativ intervjustudie med induktiv ansats. Resultatet mynnade ut i en huvudkategori; Inre och yttre faktorer påverkar det vårdande mötet samt tre kategorier; emotionell påverkan, ambulanssjuksköterskans yrkesroll samt organisationens betydelse. Med hjälp av nio subkategorier har struktur skapats. I resultatet framkom att vården av den suicidnära patienten berörde ambulanssjuksköterskorna på flera emotionella plan, bland annat uppdagades känslosamma skildringar, tragedi och medlidande i vårdmötet. En upplevelse av dränage av emotionell energi framkom också. Bristande kompetens i vårdmötet med den suicidnära patienten erfors emellanåt och det vårdande samtalet ansågs utgöra en viktig aspekt för att uppnå god vård.  Ambulanssjuksköterskan ville finnas till för patienten och såg det som sin plikt. Slutligen upplevde ambulanssjuksköterskorna att prekära omständigheter kunde försvåra vården och göra den otillräcklig samt att psykiatrin stundom misslyckades med att ge adekvat vård. I slutet av examensarbetet diskuterades suicid i relation till hållbar utveckling och en hållbar ambulanssjukvård, både ur en ekonomisk- och social kontext. / Suicide is a major public health concern. Most individuals who attempt suicide suffer from a mental disorder and have usually been in contact with healthcare professionals prior to the event. The number of mental health callouts have increased, making suicidal patients a considerable patient group in prehospital care. Despite this, some ambulance nurses express feelings of unpreparedness and ignorance in meeting and caring for suicidal patients. The aim of this study was to investigate ambulance nurses’ experiences of providing prehospital care to suicidal patients, to increase the knowledge and potentially identify areas of improvement in the care of suicidal patients. The study was conducted as a qualitative interview study with an inductive approach. The result was composed of one main category:  Internal and external determinants influence the caring encounter and three categories: emotional stress, the role of the ambulance nurse and the importance of the organisation. Nine subcategories were created for the purpose of structure. The results suggested that caring for suicidal patients influenced the ambulance nurses emotionally, for example through emotional stories, tragedies and compassion for the patient. The work was further described as emotionally draining. The nurses occasionally experienced a lack of competence in caring encounter for suicidal patients and the communication with the patient was considered important to deliver high-quality care. A desire and duty to offer support to the patient was further expressed. Lastly, ambulance nurses described that difficult conditions could complicate the care, rendering it insufficient, and that the psychiatric care occasionally failed to provide adequate care. The thesis finally discussed suicide in relation to sustainable development and sustainable ambulance care, from an economical as well as a social perspective.

Bakgrund: Suicid är ett globalt folkhälsoproblem hela världen över. Varje år dör 700 000 människor genom fullbordat suicid. I Sverige dör dagligen ca 4 människor genom suicid.   I WHO’s handlingsplan är målet att fram till 2030 att minska suicidantalet med en tredjedel och förebygga psykisk ohälsa. Syfte: Syftet med studien var att beskriva vårdpersonalens erfarenheter av att vårda patienter med suicidalt beteende. Metod: En litteraturstudie baserad på tio kvalitativa artiklar. Databassökning genomfördes i Cinahl, PubMed, PsykInfo och Scopus. Studier analyserades med hjälp av Fribergs femstegmodell. Resultat: Analysprocessen resulterade i 3 kategorier och 7 underkategorier. De tre kategorier var: Bristande organisation, känslomässiga reaktioner och attityder. Konklusion: Psykisk ohälsa är ett stigmatiserat ämne. Att vårda suicidala patienter kan vara psykiskt påfrestande för vårdpersonalen. Vårdpersonalen behöver mer kunskap inom det här ämnet för att kunna bedriva en säker och gynnsam vård som främjar hälsa för patienterna samt bidrar till att vårdkvalitén höjs. / Background: Suicide is a global public health problem worldwide. In the WHO's action plan, the goal is to reduce the number of suicides by a third by 2030 and prevent mental illness. Aim: The aim of this study was to describe the nursing stuff’s experiences of caring for patients with suicidal behavior. Methods: A literature study based on ten qualitative articles. Database search were conducted in Cinahl, PubMed, PsykInfo and Scopus. The articles were analyzed using Friberg’s five-step model. Results: The analysis process resulted in 3 categories and 7 subcategories. The four categories were: Lack of organization, emotional reactions and attitudes. Conclusion: Mental illness is a stigmatized subject. Caring for suicidal patients can be psychologically stressful for the nursing staff. The healthcare staff need more knowledge in this subject to be able to provide safe and beneficial care that promotes health for the patients and contributes to raising the quality of care.
